3. Clinical characteristics of patients with early-and late-onset optic neuromyelitis optica spectrum disease.

Abstract

Objective: To analyze the different clinical features of patients with early-onset (EO-NMOSDs) and late-onset neuromyelitis optica spectrum diseases (LO-NMOSDs). Methods: A total of 51patients with neuromyelitis optica spectrum disease who were diagnosed in our hospital for the first time from January 2015 to December 2022 were included in the First Affiliated Hospital of Hainan Medical College and divided into 22 cases in the EO-NMOSDs group and 29 cases in the LO-NMOSDs group according to whether the age of onset was ≥ 50 years old. The basic data, Extended Disability Status Scale (EDSS) score, blood and cerebrospinal fluid test indicators of the two groups were statistically analyzed. Results: There were no significant differences in demographic characteristics, clinical features and serum AQP-4 antibody positivity rate between the two groups (all P>0.05), and there were significant differences in triglycerides (TG), low-density lipoprotein (LDL), apolipoprotein A (APOA), apolipoprotein B (APOB) and lipoprotein a (P=0.010, P=0.048, P=0.014, P=0.061, P=0.001, respectively), and cerebrospinal fluid LDH, There were significant differences between microprotein quantification and EDSS score (P=0.018, P=0.034, P=0.025, respectively), and the level of microprotein quantification in cerebrospinal fluid of LO-NMOSDs had a certain correlation with the degree of disability (r=0.52, P<0.03). Conclusion: LO-NMOSDs and EONMOSDs group patients have similar demographic characteristics, serum AQP-4 antibody positive rate and clinical features, but compared with EO-NMOSDs, patients in LO-NMOSDs group are prone to abnormal lipid metabolism, higher trace proteins in cerebrospinal fluid and more likely to be disabled, and among LO-NMOSDs, the higher the trace protein in the cerebrospinal fluid, the more severe the disability status of patients. Abstract

Background: Adipose stem cell‐derived exosomes (ADSC‐EXO) and botulinum toxin type A (BTX‐A) individually showed a therapeutic effect on skin wound repair. Aims: This study investigated their synergistic effect on promoting skin wound healing in vitro and in vivo and the underlying molecular events. Methods: ADSCs were isolated from Sprague–Dawley (SD) rats to obtain ADSC‐EXO by ultrafiltration and ultracentrifugation and were confirmed using nanoparticle tracking analysis and transmission electron microscopy. Human skin fibroblasts (HSF) were cultured and treated with or without ADSC‐EXO, BTX‐A, or their combination. Changes in cell phenotypes and protein expression were analyzed using different in vitro assays, and a rat skin wound model was used to assess their in vivo effects. Results: The isolated ADSC‐EXO from primarily cultured ADSCs had a circular vesicle shape with a 30–180 nm diameter. Treatment of HSF with ADSC‐EXO and/or BTX‐A significantly accelerated HSF migration in vitro and skin wound healing in a rat model. Moreover, ADSC‐EXO plus BTX‐A treatment dramatically induced VEGFA expression but reduced COL III and COL I levels in vivo. ADSC‐EXO and/or BTX‐A treatment significantly upregulated TGF‐β3 expression on Day 16 after surgery but downregulated TGF‐β1 expression, suggesting that ADSC‐EXO plus BTX‐A promoted skin wound healing and reduced inflammatory cell infiltration. Conclusions: The ADSC‐EXO plus BTX‐A treatment demonstrated a synergistic effect on skin wound healing through upregulation of VEGF expression and the TGF‐β3/TGF‐β1 and COL III/COL I ratio. Abstract

The skin covers the entire outer part of the body as the largest organ. Because this organ is directly exposed to microbial, thermal, mechanical, and chemical damage, several factors may injure it, including acute trauma, chronic wounds, or even surgical procedures. Mesenchymal stem cells- (MSCs-) derived extracellular vesicles (EVs) can inhibit the inflammatory response in the early stage of skin wound healing, promote angiogenesis and the proliferation and migration of epithelial cells, and regulate collagen synthesis and inhibit scar proliferation in the later stage. While MSCs-EVs have broad prospects for clinical applications, it will still be a long way to seamless healing. In this brief review, we focus on the role of MSCs-EV in skin wound repair, therapeutic effects, and potential mechanisms of MSCs-EV in reducing scar formation. It is concluded that MSCs-EV can reduce scar formation in skin wound repair by interfering with multiple inflammatory factors, regulating fibroblast proliferation, and expressing type I and type III collagens at different phases.
