Diabetic retinopathy (DR) is a prevalent microvascular complication of diabetes and the leading cause of blindness and severe visual impairment in adults. The high levels of glucose trigger multiple intracellular oxidative stress pathways, such as POLDIP2, resulting in excessive reactive oxygen species (ROS) production and increased expression of vascular cell adhesion molecule-1 (VCAM-1), hypoxia-inducible factor 1α (HIF-1α), and vascular endothelial growth factor (VEGF), causing microvascular dysfunction. Dihydromyricetin (DMY) is a natural flavonoid small molecule antioxidant. However, it exhibits poor solubility in physiological environments, has a short half-life in vivo, and has low oral bioavailability. In this study, we present, for the first time, the synthesis of ultra-small Fe-DMY nano-coordinated polymer particles (Fe-DMY NCPs), formed by combining DMY with low-toxicity iron ions. In vitro and in vivo experiments confirm that Fe-DMY NCPs alleviate oxidative stress-induced damage to vascular endothelial cells by high glucose, scavenge excess ROS, and improve pathological features of DR, such as retinal vascular leakage and neovascularization. Mechanistic validation indicates that Fe-DMY NCPs can inhibit the activation of the Poldip2-Nox4-H2O2 signaling pathway and downregulate vital vascular function indicators such as VCAM-1, HIF-1α, and VEGF. These findings suggest that Fe-DMY NCPs could serve as a safe and effective antioxidant and microangio-protective agent, with the potential as a novel multimeric drug for DR therapy.

AIM: To evaluate the clinical efficacy of adalimumab(ADA)with dose-reduced glucocorticoid for the treatment of Vogt-Koyanagi-Harada disease(VKH).METHODS: A total of 21 patients(37 eyes)with VKH who received ADA therapy in the Department of Ophthalmology of our hospital from August 2020 to December 2021 were included. The interval of ADA administration was progressively extended after intraocular inflammation controlled and lasted for 3mo, and it returned to the initial treatment interval once the inflammation recurred. After follow-up for 12mo, anterior chamber cell(ACC)grade, vitreous haze(VH)grade, retinal/choroidal lesions, serous retinal detachment(SRD), best corrected visual acuity(BCVA), central macular thickness(CMT)and doses of glucocorticoid and immunosuppressant were compared before and after the first ADA injection. ObjectiveThis study aimed to elucidate the relationship between retinopathy status or severity and the all-cause and specific-cause mortality risk based on the updated National Health and Nutrition Examination Survey (NHANES) database and 2019 Public Access Link mortality file.MethodsIn this prospective cohort study, a total of 6,797 participants aged over 40 years based on NHANES 2005–2008 were analyzed. The severity of retinopathy was classified into 4 grades-no retinopathy, mild non-proliferative retinopathy (NPR), moderate to severe NPR, and proliferative retinopathy (PR). Multiple covariate-adjusted Cox proportional hazards regression models and Fine and Gray competing risk regression models were used to assess the all-cause and cause-specific mortality risks, respectively. The propensity score matching (PSM) approach was also applied additionally to adequately balance between-group covariates to validate our findings.ResultsA final total of 4,808 participants representing 18,282,772 United States (US) non-hospitalized participants were included for analysis, 50.27% were male (n = 2,417), 55.32% were non-hispanic white (n = 2,660), and mean [SE] age, 56.10 [0.40] years. After a median follow-up of 12.24 years (interquartile range, 11.16–13.49 years), 1,164 participants died of all-cause mortality, of which 941 (80.84%) died without retinopathy and 223 (19.16%) died with retinopathy at baseline. The presence of retinopathy was associated with increased all-cause mortality, cardiovascular disease (CVD), and diabetes mellitus (DM)-specific mortality, and the results remain consistent after PSM. Severity analysis showed that only mild NPR was associated with an increased all-cause mortality risk (hazard ratio (HR) = 2.01; 95% confidence interval (CI), 1.00–4.03), while increased CVD and DM-specific mortality risk were associated with all grades of retinopathy and were exponentially greater with increasing retinopathy severity, and the trend test was also significant (P for trend 0.004 and 0.04, respectively).DiscussionOur findings suggest that the diagnosis of retinopathy is an independent risk factor for all-cause mortality in people over 40 years old. Retinopathy grading is significantly associated with the survival risk of patients with CVD or DM, it can be a valuable predictor in the stratified management and risk warning of CVD or DM patients, as well as in the monitoring of systemic vasculopathy status.

AIM:To analyze the correlation between optical coherence tomography(OCT)parameters and central retinal vein occlusion of macular edema secondary(CRVO-ME), and compare the clinical efficacy of ranibizumab combined with laser photocoagulation and ranibizumab alone in the treatment of CRVO-ME.METHODS:There were 43 case with 43 eyes of patients in CRVO-ME diagnosed in our hospital from January 2020 to December 2020 included in the present study and divided into two groups, namely A and B. Patients in group A were treated with ranibizumab combined with laser photocoagulation, while patients in group B were treated with ranibizumab alone. The structure of outer retina and “SAVE” scores were observed and estimated using OCT and fluorescein angiography(FFA)examination before and after the treatment at 1, 3, 6, 12mo, and then analyzed their correlation with best corrected visual acuity(BCVA, LogMAR). The BCVA, central macular thickness(CMT), intraocular pressure and average number of drug injections were also compared between the two groups before and after treatment.RESULTS:At 12mo after treatment, the BCVA in the OCT baseline external limiting membrane(ELM)intact group and baseline ellipsoid zone(EZ)intact group before and after treatment were significantly improved than those of the fracture group(0.47±0.16 vs 0.21±0.15, P=0.013; 0.44±0.20 vs 0.25±0.17, P=0.008). There was no statistically significant difference in BCVA changes between baseline RPE fracture group and RPE intact group(P>0.05). The number of patients with “S” and “A” at 1 score decreased significantly at 12mo after treatment in both groups, the BCVA of patients with “V” and “E” at 0 score before treatment was significantly improved than those patients at 1 score(all P0.05). In addition, there were no severe complications such as secondary glaucoma and endophthalmitis in both groups.CONCLUSION: Baseline status of ELM and EZ, presence or absence of vitreoretinal abnormalities(V), and focal leakage(E)could suggest the treatment efficacy of CRVO-ME. Ranibizumab in the treatment of CRVO-ME demonstrates prominent efficacy and great safety, and there was no better effect was observed when combined with laser photocoagulation.

AIM:To investigate the dynamic expression characteristics of interleukin-10(IL-10)after implantation of glaucoma drainage material, and to reveal the role of IL-10 on scarring formation.METHODS:Totally 75 New Zealand white rabbits were randomly divided into three groups, which were implanted with different types of material-Polymethyl methacrylate coated Parylene C(PMMA group), silicone together with injection of Mitomycin C(MMC)(silicon-MMC group)and silicone(silicone group). Aqueous humor were collected at 1, 3d, 1, 2, 3, 4 and 8wk after operation and enzyme-linked immunosorbent assay(ELISA)were utilized to detect the expression of IL-10 in the aqueous humor. The connective tissue surrounding the material were collected at 1, 2, 3, 4 and 8wk postoperatively. Hematoxylin-eosin(HE)staining was applied to evaluate the proliferation of fibroblasts and the infiltration of inflammatory cells. The protein expression and mRNA of IL-10 in the connective tissue were detected by immunohistochemistry and real-time PCR.RESULTS:Compared with PMMA and silicon-MMC group, silicone group showed significantly increased proliferation of fibroblasts and infiltration of inflammatory cells according to the HE staining result. AIM:To compare the effect of comprehensive treatment in children with anisometropia and strabismus amblyopia.METHODS: Prospective study. Forty-six individuals(mean age 9±3 years old, 26 males, 20 females)with previously untreated monocular amblyopia were recruited in our hospital from July 2018 to January 2020. There were 23 patients with strabismus monocular amblyopia(mean age 9±3 years old, 12 males, 11 females)and 23 patients with anisometropia monocular amblyopia(mean age 9±3 years old, 14 males, 9 females). The best corrected visual acuity(Tumbling E Chart)and stereopsis(Titmus)were measured before and after a 6mo period of comprehensive treatment(refractive correction, patching and precision eyesight training). The differences of monocular visual benefits(visual acuity)and binocular visual benefits(stereopsis)between two groups before and after treatment were compared. RESULTS: Before treatment, there was no significant difference of best corrected visual acuity between anisometropia amblyopia and strabismus amblyopia(t= -0.475, P>0.05), but the stereopsis of strabismus amblyopia was significantly lower than that of anisometropia amblyopia(t= -3.919, P0.05). AIM: To compare the curative effect of retropupillary iris claw intraocular lens(IOL)implantation and posterior chamber ciliary scleral suture fixed IOL in the aphakic eyes without capsular support.METHODS: The present retrospective study included 70 aphakic eyes without capsular support. According to the different surgical methods, the patients were divided into two groups, including group A(35 eyes)with treatment of retropupillary iris claw IOL implantation, group B(35 eyes)with treatment of posterior chamber ciliary scleral suture fixed IOL. The uncorrected visual acuity(UCVA), best corrected visual acuity(BCVA), intraocular pressure(IOP)and corneal endothelial cell density(CECD)of the two groups were documented and compared before operation and 3d, 1, 3, 6mo and 1a after operation. Furthermore, the stability of IOL in both groups, as well as the intraoperative and postoperative complications were recorded.RESULTS: The patients in the present study were followed up for 12 to 14mo. 3d after surgery, UCVA in group A was significantly improved compared with that before surgery(P0.05).There was no significant difference between whole eye astigmatism and corneal astigmatism in A group and B gruop at 1 and 6mo after operation(P>0.05). Additionally, There was 1 case of IOL dislocation in each group after operation, which was further recovered by operation. Significant tilt and deviation of IOL were not observed in the rest of the patients during the follow up period. The other postoperative complications were mild with no difference in complications between the two groups(P>0.05).CONCLUSION: For aphakic eyes without capsular support, both retropupillary iris claw IOL implantation and posterior chamber ciliary scleral suture fixed IOL are safe and effective surgical methods. Retropupillary iris claw IOL implantation has relatively simple operation, lower damage to the eyeball tissue, the shorter operation time, and the quicker recovery of postoperative visual acuity, which is one of the effective clinical treatments.

Inflammatory diseases present with dysregulated inflammatory reactions and oxidative stress, resulting in heavy health-associated economic burden to society. Current mainstream therapies, including traditional therapies such as steroids, non-steroidal anti-inflammatory drugs (NSAIDs) and immunosuppressants, as well as novel biological strategies such as cytokine and intracellular signal inhibitors, cannot cure inflammatory diseases and always cause a series of unexpected adverse effects. Natural products (NPs), which are complex chemical molecules with abundant biological activities derived from microorganisms and plants, have demonstrated great potential for treating inflammatory diseases with several unresolved drawbacks. Increasing studies have indicated that NP-based nanomedicine resulting from the combination of NPs and nanotechnology may open a brand-new vista for the therapy of inflammatory diseases. In the present review, we summarize the classification of NP-based nanomedicine and its potential therapeutic effects in various inflammatory diseases, as well as further elucidate the challenges in the wide clinical application of NP-based nanomedicine to promote a comprehensive understanding and development of inflammatory disease therapy by NP-based nanomedicine.

AIM: To compare the rotational stability of Toric intraocular lens (IOLs) implantation combined with four-eyelet or two-eyelet capsular tension rings (CTRs) in eyes with high myopia and cataract. METHODS: This prospective randomized controlled interventional study included 33 eyes which had preoperative corneal astigmatism ≥1.5 D and ocular axial length ≥25.5 mm. These eyes were randomly divided into two groups to undergo phacoemulsification and toric IOL implantation with either four-eyelet CTR implantation (group A, n=16) or two-eyelet CTR implantation (group B, n=17). Uncorrected visual acuity (UCVA), best-corrected visual acuity (BCVA), phoropter examination results, and toric IOL rotation degrees were tested 6mo after the surgery. RESULTS: In both groups, the toric IOL was in the capsular sac 6mo after surgery. The difference between the two groups in terms of visual outcome was not found to be statistically significant (P>0.05) at a follow-up of 6mo. AIM:To investigate the relationship between crystalline lens rise(CLR)measured by anterior segment-optical coherence tomography(AS-OCT)and parameters of the anterior segment. And also measure the effects of CLR on the vault after phakic posterior chamber implantable collamer lens(PPC-ICL)implantation.METHODS:The study included 80 eyes of 40 patients who underwent PPC-ICL implantation at the Department of Ophthalmology, the Second Affiliated Hospital of Anhui Medical University, from July 2017 to July 2018, to measure the sulcus-to-sulcus(hSTS)distance of the eyes using a Compact Touch STS ultrasound biomicroscope and lens curvature(LC), Pentacam was used to measure horizontal-white-to-white(hWTW), anterior chamber depth(ACD)and the vault one year after the operation. AIM: To evaluate the efficacy and safety of high-dose ultrasound cyclo-plasty (UCP) for the treatment of refractory glaucoma in Chinese patients. METHODS: In this 6-month retrospective study, 37 eyes of 37 patients suffering from severe glaucoma with uncontrolled intraocular pressure (IOP) of ≥21 mm Hg underwent 8-s ultrasonic cyclocoagulation with ten active piezoelectric elements. A complete ophthalmic examination was performed before and at 1d, 1, 3, 6mo after UCP. Therapeutic success was defined as IOP reduction from baseline ≥20% and IOP ≥5 mm Hg without adding new glaucoma medication compare to baseline at the 6-month follow-up visit. AIM: To analyze changes in objective visual quality before and after femtosecond laser in situ keratomileusis(FS-LASIK)and smart pulse technology-assisted transepithelial photorefractive keratectomy(SMART).METHODS: Prospective study. We collected 50 cases(100 eyes)treated with FS-LASIK(FS-LASIK group)and another 50 cases(100 eyes)treated with SMART(SMART group)from the Ophthalmology Department of our hospital between October 2018 and December 2018 using Optical Quality Analysis System(OQAS)to measure objective scatter index(OSI), modulation transfer function cut off frequency(MTF cut off), strehl ratio(SR)before and after surgery.RESULTS: In the FS-LASIK and SMART groups, the OSI values were higher in the 1 and 3mo after surgery, whereas the MTF cut off and SR were lower in the 1 and 3mo after surgery(PP>0.05). However, after 3mo, the OSI value of the FS-LASIK group was higher than the SMART group(0.88±0.28 vs 0.70±0.27, Pvs 0.24±0.05, P=0.002).CONCLUSION: Both FS-LASIK and SMART caused an increase in the intraocular scattering index and a decrease in objective visual quality. However, the visual quality of the SMART group was generally better than that of the FS-LASIK group, and long-term visual quality was more dominant.

AIM: To observe the clinical efficacy and safety in the treatment for refractory glaucoma by two kinds of cyclodestructive procedures.METHODS: A retrospective study of 50 patients(50 eyes)with refractory glaucoma diagnosed in our hospital between January 2018 and October 2018. Ultrasound cyclo plasty(UCP)were performed on 30 eyes and cyclocryotherapy were performed on 20 eyes, introcular pressure(IOP), pain grade scores and complications after operation were followed up.RESULTS: The average IOP of the UCP group on 1d, 1wk, 1mo and 3mo were 29.27±10.40, 23.87±8.61, 25.27±9.95, 23.70±10.06mmHg compared with 43.97±10.39mmHg before operation. The average IOP of the cyclocryotherapy group on 1d, 1wk, 1mo and 3mo were 22.15±7.78, 20.80±8.44, 22.50±7.12, 24.20±8.43mmHg compared with 47.30±8.53mmHg before operation. The IOP in both group was significantly reduced(P<0.01). The pain grade scores of patients in the UCP group at 1d, 1wk, 1mo and 3mo after surgery were significantly lower than those before operation(PP>0.05). There were no serious complications in the UCP group, four eyes appeared conjunctival hyperemia and one eye appeared superficial punctuate keratitis; but there were 16 eyes(80.00%)in the cyclocryotherapy group: ten eyes appeared conjunctival hyperemia and six eyes appeared anterior chamber hemorrhage. CONCLUSION: UCP is safe and convenient surgery for refractory glaucoma, and has significant effect on reducing intraocular pressure, alleviating ocular pain and decreasing complications.

AIM: To evaluate the early postoperative efficacy of SMART for myopia and compare it with TransPRK.METHODS: Retrospective non-randomized controlled study. Totally 120 eyes of 60 patients with myopia who underwent SMART and TransPRK in our hospital from January to August 2018 were selected respectively. The uncorrected visual acuity(UCVA), visual quality, pain score, corneal epithelial healing and subepithelial haze were compared between two groups.RESULTS: There was no statistical differences between two groups regarding the proportion of UCVA reaching or exceeding 1.0 at 5d, 1mo and 3mo after operation(P>0.05), but the visual quality of SMART group was better than that of TransPRK group at 5d after operation(Pvs 3.07±1.07; 1.22±0.61 vs 0.84±0.59)on the 1st day and 3rd day after operation(Pvs 83.3%, Pst and 3 mo after operation, there was no difference in haze between the two groups(5.0% vs 5.0% and 8.3% vs 10.0%; P>0.05).CONCLUSION: There is no significant difference between SMART and TransPRK in the speed and stability of visual acuity recovery, but the early postoperative pain of SMART is lighter, the corneal epithelium is healed faster, and the visual quality is better.

AIM: To compare the difference of corneal curvature, astigmatism, central corneal thickness and anterior chamber depth between the Pentacam anterior segment analyzer and anterior segment optical coherence tomography(AS-OCT)in young myopic preoperative and senile cataract patients.METHODS: Prospective clinical study.Totally 64 preoperative examinations(124 eyes)and 61 senile cataract patients(85 eyes)were selected. Pentacam and AS-OCT were used to measure the people's parameters of the ophthalmic anterior segment. Paired sample t-test was used to compare the differences of the two devices' measurement results of the two groups. The correlation of the obtained data was analyzed by Pearson correlation analysis. The consistency of the measurement results was analyzed by Bland-Altman plot analysis.RESULTS: There were significant differences in measuring steep Keratometry(Ks), flat Keratometry(Kf)and mean Keratometry(Km)between the two devices of myopia(PP>0.05). The astigmatic parameters J0, central corneal thickness(CCT), and anterior chamber depth(ACD)measured by the devices and there were statistically significant differences between the two devices(P45(P>0.05). The results of Ks, Kf, Km, J0, J45, CCT and ACD measured by the two devices in both groups were linearly related(P0, J45, CCT and ACD. The consistency of Ks, Kf and Km measured by the myopia group was good, while the consistency of Ks, Kf and Km measured by the cataract group was poor.CONCLUSION: Pentacam and AS-OCT measurements of the ophthalmic anterior segment are consistent with astigmatic parameters, CCT and ACD, and can be used interchangeably. The consistency of Ks, Kf and Km is affected by age related factors, and Pentacam is preferred.

AIM: To explore the effect of miR-22 on viability, migration, invasion and apoptosis in retinoblastoma (RB) Y79 cells and to further detect the potential mechanism. METHODS: Plasmids were constructed to change the expression level of miR-22 in Y79 cells. Real-time reverse transcription polymerase chain reaction (RT-PCR) was conducted to test the expression level of miR-22. After changing the expression of miR-22, the mRNA and protein levels of high-mobility group box 1 (HMGB1) were investigated using RT-PCR and Western blotting. The effect of miR-22 on viability was analyzed by using cell counting kit-8 (CCK-8) assay and the effect on apoptosis was detected by the flow cytometry. Wound healing migration assay and Transwell invasion assay were used to detect the effects of miR-22 on cell motility. RESULTS: miR-22 inhibited viability, migration and invasion, while promoting apoptosis, in RB Y79 cells. The inhibition rate of miR-22 overexpression group at 12, 24, 48h was 11.71%±2.54%, 21.36%±1.39% and 29.44%±1.15%, respectively. Cellular apoptosis was higher in miR-22 overexpression group (17.00%±0.39%) compared with negative control (4.38%±0.38%). miR-22 negatively mediated the expression of HMGB1. Furthermore, decreased HMGB1 significantly attenuated viability, migration and invasion, while promoting apoptosis. Enforced expression of HMGB1 partially rescued the effects of miR-22 overexpression on cell viability, migration, invasion and apoptosis. Moreover, the phosphorylated protein kinase B (p-AKT) was significantly downregulated in the HMGB1 shRNA group and miR-22 overexpression group and elevated in the HMGB1 overexpression group compared with the normal control. CONCLUSION: miR-22 inhibites viability, migration and invasion and increases apoptosis in Y79 cells by targeting HMGB1. These findings may provide a therapeutic strategy for RB.

There are severe challenges for slurry pressure balance tunnel boring machine (TBM) tunnelling in sandy cobble soil of Beijing, Chengdu, and Lanzhou in China. And the problems caused by tunnelling from silty clay to sandy cobble stratum are more serious. With the change of stratum, the key parameters and surface settlement will change correspondingly. Controlling the key parameters and predicting the surface settlement accurately and efficiently is important for hazard mitigation and risk management. In this study, based on the Tsinghuayuan Tunnel project in Beijing, the key parameters and surface settlement while tunnelling from silty clay to sandy cobble stratum are studied. Firstly, the difference of key parameters while tunnelling in two different strata is analyzed. The analysis shows that immediate responses to changes in the stratum are recommended in order to ensure construction efficiency. Then, a refined 3D finite difference model is developed to simulate the slurry TBM tunnelling in different strata. For refined simulation, three key parameters obtained from measurement data were applied to the 3D models, and the simulation results were compared with the field data. Results show that the refined model has good performance in terms of the accuracy and efficiency. This study provides a good engineering practice reference for slurry TBM tunnelling in mixed strata.

AIM: To report long-term outcomes of secondary glaucoma due to uveitis treated with Ahmed glaucoma valve (AGV) implantation in a series of Chinese patients. METHODS: The retrospective study included 67 eyes from 56 patients with uveitic glaucoma who underwent AGV implantation. Success of the treatment was defined as patients achieving intraocular pressure (IOP) levels between 6 and 21 mm Hg with or without additional anti-glaucoma medications and/or a minimum of 20% reduction from baseline IOP. The main outcome measurements included IOP, the number of glaucoma medications at 1, 3, 6, 12, 24, 36, 48 and 60mo after surgery, surgical complications, final best-corrected vision acuity (BCVA), visual field (VF) and retinal nerve fiber layer (RNFL). RESULTS: The mean follow-up was 53.3±8.5 (range 48 to 60)mo. The cumulative probability of success rate was 98.5%, 95.5%, 89.6%, 83.6%, 76.1%, 70.1%, 65.7% and 61.2% at 1, 3, 6, 12, 24, 36, 48 and 60mo, respectively. IOP was reduced from a baseline of 30.8±6.8 to 9.9±4.1, 10.1±4.2, 10.9±3.7, 12.9±4.6, 13.8±3.9, 13.2±4.6, 12.3±3.5 and 13.1±3.7 mm Hg at 1, 3, 6, 12, 24, 36, 48 and 60mo, respectively (P

AIM: To compare the effect of compound electrolyte solution and lactated Ringer's solution on corneal function in cataract phacoemulsification, and to provide scientific basis for clinical selection of appropriate perfusion fluid. METHODS: The patients with senile cataract were randomly divided into control group with lactated Ringer's solution as anterior chamber perfusion and experimental group with compound electrolyte as anterior chamber perfusion. Surgical removal of cataract and phacoemulsification with intraocular lens implantation were taken. The corneal endothelial cell density, central corneal thickness, hexagonal cell ratio and endothelial cell coefficient of variation were measured at preoperative and postoperative points. RESULTS: Totally 60 patients successfully completed all follow-ups, the experimental group of 30 cases, the control group of 30 cases. The density of corneal endothelial cells in experimental group was significantly higher than those in the lactated Ringer's solution group at 1 and 3d after operations(P=0.030, 0.046). The coefficient of variation of corneal endothelial cells in lactated Ringer's solution group was higher than that in compound electrolyte group at 1 and 14d after operation(P=0.025, 0.014). The visual acuity of the compound electrolyte group was better than that of the lactated Ringer's solution on the first day after operation(P=0.04). CONCLUSION: In the phacoemulsification of senile cataract, the compound electrolyte perfusion has better histocompatibility, which can maintain the stability of corneal endothelial cell structure and reduce corneal endothelial cell injury. The compound electrolyte perfusion solution is more suitable for senile cataract phacoemulsification surgery.

AIM: To find new biomarkers for uveal melanoma (UM) by analyzing the serum peptidome profile. METHODS: Proteomic spectra in patients with UM before and after operation were analyzed and compared with those of healthy controls. Magnetic affinity beads were used to capture serum peptides and matrix-assisted laser desorption/ionization time-of-flight (MALDI-TOF) mass spectrometer were used to compile serum peptide profiles. RESULTS: A panel of 49 peptides were differentially expressed between UM patients and controls, of which 33 peptides were of higher intensities in patient group and 16 peptides were of higher intensities in control group. Based on combined use of these potential markers, peptides with mean molecular masses of 1467 and 9289.0 Da provide high sensitivity (83.3%), specificity (100%) and accuracy rate (93.0%) together to differentiate melanoma patients from healthy controls. At the time point of 6mo postoperatively, the levels of many peptides differentially expressed before surgery showed no more statistical difference between the patients and the control group. Fibrinogen α-chain precursors were identified as potential UM markers. CONCLUSION: We have shown that a convenient and fast proteomic technique, affinity bead separation and MALDI-TOF analysis combined with bioinformatic software, facilitates the identification of novel biomarkers for UM.

In this review, we summarize the progression of several parameters assessed by spectral-domain optical coherence tomography (SD-OCT) in recent years for the detection of glaucoma. Monitoring the progression of defects in the retinal nerve fiber layer (RNFL) thickness is essential. Imaging and analysis of retinal ganglion cells (RGCs) and inner plexiform layer (IPL), respectively, have been of great importance. Optic nerve head (ONH) topography obtained from 3D SD-OCT images is another crucial step. Other important assessments involve locating the Bruch’s membrane opening (BMO), estimating the optic disc size and rim area, and measuring the lamina cribrosa displacement. Still other parameters found in the past three years for glaucoma diagnosis comprise central retinal artery resistive index, optic disc perfusion in optical coherence tomography angiography (OCTA) study, peripapillary choroidal thickness, and choroidal area in SD-OCT. Recently, several more ocular fundus parameters have been found, and compared with the earlier parameters to judge the accuracy of diagnosis. While a few of these parameters have been widely used in clinical practice, a fair number are still in the experimental stage.

The study was to report a new two-point scleral-fixation technique for foldable intraocular lenses with four haptics. Lenses were slid into the anterior chamber from a 2.8 mm corneal incision and fixed under two sclera flaps at two opposite points. The postoperative best-corrected visual acuities (BCVAs) of all patients were significantly better than their preoperative BCVA. The results demonstrate that two-point, scleral fixations of foldable, intraocular lenses might be practicable and effective.

AIM:To investigate the clinical value of optical coherence tomography(OCT)combined with ophthalmic B-type ultrasound in fundus examination before cataract surgeries.METHODS:Conventional ophthalmoscope, ophthalmic B-type ultrasound and OCT were used to exam the fundus of 176 cataract patients(278 eyes). The positive rate of fundus diseases by the three methods was calculated, and then McNemar test was used to analyze the data in pairs and the values of the three methods on diagnosis of fundus diseases were investigated.RESULTS:Thirty-two eyes were diagnosed with fundus diseases by conventional ophthalmoscope with positive rate of 11.5%, 18 eyes(6.5%)by ophthalmic B-type ultrasound, 6 eyes(2.2%)of those 18 eyes were serious lens turbidity. Fifty-one eyes(18.3%)were diagnosed with fundus diseases by OCT and 57 eyes(20.5%)by OCT combined with ophthalmic B-type ultrasound. The positive rate of fundus disease detected by OCT had significant difference at the level of α=0.05 compared with those by ophthalmic B-type ultrasound, conventional ophthalmoscope and OCT combined with ophthalmic B-type ultrasound. CONCLUSION:OCT is more valuable and more sensitive on detecting macular diseases than the other two. However, the structure of retinal layer of the patients who had serious lens turbidity cannot be observed, which can be remedied by ophthalmic B-type ultrasound. The examination of OCT combined with ophthalmic B-type ultrasound is the most valuable one on accurate and early diagnosis of fundus diseases and the prediction of the visual acuity after surgeries.

AIM: To compare the clinic effects of implanting two kinds of artificial lens(Bigbag and US)in patients of high myopia with cataract. METHODS: A total of 56 cases were studied retrospectively. Patients in group A(31 cases with 55 eyes)were implanted in Bigbag intraocular lens and patients in group B(25 cases with 40 eyes)were implanted in US intraocular lens. Best corrected visual acuity(BCVA), anterior chamber depth, and post operative complications were measured and compared 3 months after surgery. RESULTS: There was no significantly difference between group A and B in BCVA 3 months after surgery. Anterior depth of group A was deeper than group B, which was statistically significant difference(PPPCONCLUSION: Bigbag has obvious advantages such as reducing complications and the incidence of PCO than US for patients of high myopia with cataract.

AIM: To describe the application and evaluate the effects of internal limiting membrane(ILM)peeling in patients underwent primary vitrcetomy for complicated proliferative vitreoretinopathy(PVR). METHODS: Totally 48 cases with serious PVR underwent vitrectomy were enrolled in this study. All these cases were divided into two groups: 23 cases in group A underwent vitrectomy with ILM peeling; 25 cases in group B underwent vitrectomy without ILM peeling. All patients underwent successful primary vitrectomy using silicone oil. After regular referral, silicone oil was removed, the retina reattached successfully. The results of slit lamp, optical coherence tomography(OCT), best corrected logarithm of minimal angle of resolution(logMAR)visual acuity pre and post operation and secondary epimacular membrane formation were observed and contrasted. Independent sample t-test, paired sample t-test, Pearson Chi-square test and Fisher's Exact test were used to analyze the significance of results with a P value ≤0.05 interpreted as statistically significance. RESULTS: Secondary epimacular membrane formation was found in one eye in group A, and seven eyes in group B after vitrectomy, the difference was significant. Central macular thickness was 188.36±45.53μm in group A, and 220.42±53.15μm in group B after the removal of silicone oil, the difference was significant. Macular edema was comparatively mild in group with ILM peeling. Image of macular morphology in OCT was more similar to the normal in group A. The final logMAR visual acuity was 1.38±0.60 in group A and 1.61±0.51 in group B, the difference was not significant between these two groups.CONCLUSION: ILM peeling may prevent the formation of secondary epiretinal membrane, is conducive to the recovery of retinal structure. This technology can be used in vitrectomy for PVR.
