Abstract

La terapia con radioligandi (Radioligand therapy, RLT, o Peptide Receptor Radionuclide Therapy, PRRT) è una delle opzioni terapeutiche più efficaci per i tumori neuroendocrini (NET) metastatici, in progressione e non operabili. Come dimostrato dallo studio NETTER-1, la PRRT con [177Lu]Lu-DOTA-peptidi è infatti in grado di migliorare la sopravvivenza libera da progressione e la sopravvivenza globale del paziente. Grazie a questo studio, nel 2018 il [177Lu]Lu-DOTA-TATE (Lutathera®) è stato approvato dalla Food and Drug Administration (FDA) e dall’Agenzia Europea del Farmaco (EMA) per il trattamento dei NET gastro-entero-pancreatici ben differenziati inoperabili o metastatici con progressione di malattia. Tuttavia, la valutazione della risposta a tale terapia rimane tuttora un tema complesso e gli esperti del settore non hanno ancora raggiunto un consenso univoco sui criteri da utilizzare e sulle tempistiche di rivalutazione dalla somministrazione del trattamento. Questa rassegna ha l’obiettivo di illustrare gli approcci interpretativi e i criteri di valutazione della risposta attualmente più rilevanti per la previsione dell’efficacia della RLT, con particolare attenzione al ruolo svolta dall’imaging funzionale e bio-molecolare.

Abstract

DYT1 mutation is characterized by focal to generalized dystonia and incomplete penetrance. To explore the complex perturbations in the different neural networks and the mutual interactions among them, we studied symptomatic and asymptomatic DTY1 mutation carriers by resting-state functional MRI.A total of 7 symptomatic DYT1, 10 asymptomatic DYT1, and 26 healthy controls were considered. Resting-state functional MRI (Oxford Centre for Functional MRI of the Brain) [FMRIB] Software Library) (FSL) MELODIC, dual regression, (as a toolbox of FSL, with Nets is referred to "networks") (FSLNets) (http://fsl.fmrib.ox.ac.uk/fsl/fslwiki/FSLNets) was performed on 9 resting-state neural networks.DYT1 mutation signature (symptomatic DYT1 and asymptomatic DYT1) was characterized by increased connectivity in the dorsal attention network and in the left fronto-parietal network. Functional correlates of symptomatic DYT1 patients (symptomatic DYT1 vs healthy controls) showed increased connectivity in the sensorimotor network.This study argues that DYT1 dystonia is a network disorder, with crucial nodes in sensory-motor integration of posterior parietal structures. A better characterization of cortical networks involved in dystonia is crucial for possible neurophysiological therapeutic interventions. © 2016 International Parkinson and Movement Disorder Society.

41. Sympathetic overdrive and unrestrained adipose lipolysis drive alcohol-induced hepatic steatosis in rodents.

Abstract

Hepatic steatosis is a key initiating event in the pathogenesis of alcohol-associated liver disease (ALD), the most detrimental organ damage resulting from alcohol use disorder. However, the mechanisms by which alcohol induces steatosis remain incompletely understood. We have previously found that alcohol binging impairs brain insulin action, resulting in increased adipose tissue lipolysis by unrestraining sympathetic nervous system (SNS) outflow. Here, we examined whether an impaired brain–SNS–adipose tissue axis drives hepatic steatosis through unrestrained adipose tissue lipolysis and increased lipid flux to the liver. We examined the role of lipolysis, and the brain–SNS–adipose tissue axis and stress in alcohol induced hepatic triglyceride accumulation in a series of rodent models: pharmacological inhibition of the negative regulator of insulin signaling protein-tyrosine phosphatase 1β (PTP1b) in the rat brain, tyrosine hydroxylase (TH) knockout mice as a pharmacogenetic model of sympathectomy, adipocyte specific adipose triglyceride lipase (ATGL) knockout mice, wildtype (WT) mice treated with β3 adrenergic agonist or undergoing restraint stress. Intracerebral administration of a PTP1b inhibitor, inhibition of adipose tissue lipolysis and reduction of sympathetic outflow ameliorated alcohol induced steatosis. Conversely, induction of adipose tissue lipolysis through β3 adrenergic agonism or by restraint stress worsened alcohol induced steatosis. Brain insulin resistance through upregulation of PTP1b, increased sympathetic activity, and unrestrained adipose tissue lipolysis are key drivers of alcoholic steatosis. Targeting these drivers of steatosis may provide effective therapeutic strategies to ameliorate ALD. • Alcohol binging induced fatty liver is reduced by CNS inhibition of the negative regulator of insulin signaling PTP1b. • Reduction of adipose tissue lipolysis prevents alcohol induced hepatic steatosis. • Reducing sympathetic outflow improves alcohol induced fatty liver. • Stress and β3 adrenergic agonism synergize with alcohol in increasing lipolysis and driving hepatic triglyceride accumulation. Abstract

Additive Manufacturing (AM), applied to metal industry, is a family of processes that allow complex shape components to be realized from raw materials in the form of powders. The compaction of the powders is achieved by local melting of bed. Electron Beam Melting (EBM) is an additive manufacturing process in which a focalized electron beam is the heat source that allows the powders to be compacted. By EBM it is possible to realize complex shape components; this feature is of particular interest in titanium industry where numerous efforts are done to develop near net shape processes. One of the limits of EBM based AM process is the difficulty to realize large dimension parts. Due to this limit the study of joining processes of different parts is of great interest. In the present work the microstructure evolution of sheets of TI6Al4V made by EBM and joined by Linear Friction Welding (LFW) is analyzed in details. The bulk microstructure of the specimen is fine lamellar; lamellae are enclosed in alpha colonies. Different types of porosities are observed. In the joint a Thermo Mechanical Affected Zone (TMAZ) and Weld Bead (WB) are evident. In TMAZ a partial recrystallization occurs and the parent microstructure is deformed. Complete recrystallization occurs in WB whose structure is martensitic. [ABSTRACT FROM AUTHOR]

Abstract

This paper is about the optimal selection of process parameters for Wire Arc Additive Manufacturing technology, an emerging solution for additive production of metal parts. In particular, the selection of the process parameters is based on the evolution of the microstructure and on the mechanical properties of the final samples obtained through the successive deposition weld beads of a ER70S-6 steel, according to the AWS legislation. The feed rate and the heat input during the deposition of the weld beads have been varied, in order to understand how the temperature reached by the samples can affect the final product mechanical characteristics. The final cooling has been carried in calm air at room temperature and between the deposition of a weld bead and the following one it has been imposed a pause of 60s. The tests on mechanical properties carried out have been: A full experimental campaign that includes: macrographic observations, micrographic observations and Vickers microhardness. The analysis of these tests has highligthed that by varying the process parameters, the samples do not have substantial differences between them. Instead, a microstructure that evolves from pearlitic-ferritic grains until bainitic lamellae along the vertical direction of the samples has been observed by micrographic analysis and confirmed by microhardness measurements. Abstract

Frontotemporal dementia (FTD) is the second commonest form of dementia after Alzheimer's disease, but its clinical and biological features are less well known. To uncover its earliest signs, we studied the main clinical, neuroimaging, and biochemical findings in an asymptomatic carrier from a three generation FTD family, bearing the P301L pathogenic mutation in the tau gene. Except for selective impairment on the Verbal Fluency Test for letters, all cognitive tests were normal. The brain computed tomography scan was normal, but the brain single photon emission computed tomography and statistical parametric mapping (SPECT-SPM) scan revealed bilateral frontal lobe hypoperfusion. Levels of total tau, 181P-tau, and Abeta1-42 in the cerebrospinal fluid were increased compared with control values. We conclude that detection of these distinctive abnormalities should improve early diagnostic accuracy for FTD and help distinguish it from Alzheimer's disease.
