5. Satisfying Product Features of a Fall Prevention Smartphone App and Potential Users’ Willingness to Pay: Web-Based Survey Among Older Adults

Abstract

BackgroundProhibiting falls and fall-related injuries is a major challenge for health care systems worldwide, as a substantial proportion of falls occur in older adults who are previously known to be either frail or at high risk for falls. Hence, preventive measures are needed to educate and minimize the risk for falls rather than just minimize older adults’ fall risk. Health apps have the potential to address this problem, as they enable users to self-assess their individual fall risk. ObjectiveThe objective of this study was to identify product features of a fall prevention smartphone app, which increase or decrease users’ satisfaction. In addition, willingness to pay (WTP) was assessed to explore how much revenue such an app could generate. MethodsA total of 96 participants completed an open self-selected Web-based survey. Participants answered various questions regarding health status, subjective and objective fall risk, and technical readiness. Seventeen predefined product features of a fall prevention smartphone app were evaluated twice: first, according to a functional (product feature is implemented in the app), and subsequently by a dysfunctional (product feature is not implemented in the app) question. On the basis of the combination of answers from these 2 questions, the product feature was assigned to a certain category (must-be, attractive, one-dimensional, indifferent, or questionable product feature). This method is widely used in user-oriented product development and captures users’ expectations of a product and how their satisfaction is influenced by the availability of individual product features. ResultsFive product features were identified to increase users’ acceptance, including (1) a checklist of typical tripping hazards, (2) an emergency guideline in case of a fall, (3) description of exercises and integrated workout plans that decrease the risk of falling, (4) inclusion of a continuous workout program, and (5) cost coverage by health insurer. Participants’ WTP was assessed after all 17 product features were rated and revealed a median monthly payment WTP rate of €5.00 (interquartile range 10.00). ConclusionsThe results show various motivating product features that should be incorporated into a fall prevention smartphone app. Results reveal aspects that fall prevention and intervention designers should keep in mind to encourage individuals to start joining their program and facilitate long-term user engagement, resulting in a greater interest in fall risk prevention.

41. Das komplexe regionale Schmerzsyndrom – eine interdisziplinäre Betrachtung aus der chirurgischen Sprechstunde

Abstract

Chronische Schmerzen sind häufig. Treten diese in Verbindung mit sensomotorischen, vaskulären und trophischen Störungen auf, kann ein komplexes regionales Schmerzsyndrom („complex regional pain syndrome“ [CRPS], Morbus Sudeck) zugrunde liegen. Für betroffene Patienten ist das CRPS mit einer erheblichen Alltagseinschränkung verbunden. Die Diagnosestellung erfolgt anhand der Budapest-Kriterien nach klinischen Merkmalen. Ziele der multimodalen Therapie, bestehend aus medikamentösen, rehabilitativen und psychosomatischen Ansätzen, sind die Reduktion von Schmerzen und ein Rückgewinn an Funktionalität. In der medikamentösen Therapie haben sich Bisphosphonate, Steroide und Antiepileptika etabliert, diese sollten aber immer in Kombination mit einer funktionellen Therapie eingesetzt werden. Interventionelle Verfahren sollten nur bei chronischen Verläufen und in spezialisierten Zentren angewendet werden. Insgesamt zeigt das CRPS individuell sehr unterschiedliche Krankheitsverläufe und erfordert häufig eine interdisziplinäre Langzeitbehandlung.

42. Traumatische periprothetische Frakturen bei einliegender Hüftprothese

Abstract

Aufgrund der Zunahme der endoprothetischen Versorgungen wächst auch die Zahl der periprothetischen Hüftfrakturen. Diese Frakturen benötigen ein abgestuftes und individuelles Therapiekonzept, dessen Ziel eine möglichst schnelle Mobilisation des Patienten sein muss. Im Rahmen der Diagnostik liegt eine besondere Herausforderung in der Identifikation schon vor dem Unfall bestehender Lockerungszeichen. Femurfrakturen bei noch fest verankerter Prothese können osteosynthetisch versorgt werden. Hierbei spielt die winkelstabile Plattenosteosynthese die wichtigste Rolle. Ein gelockerter Schaft muss meist gewechselt werden, oft in Kombination mit einer Osteosynthese. Auch Frakturen des Acetabulums können bei festem Pfannensitz konservativ oder osteosynthetisch versorgt werden. Gelockerte Pfannen müssen ebenfalls gewechselt werden; hierbei ist die Stabilisierung des hinteren Pfeilers für das Ergebnis besonders wichtig.

49. Strategies for the treatment of femoral fractures in severely injured patients: trends in over two decades from the TraumaRegister DGU ®

Abstract

PURPOSE Treatment strategies for femoral fracture stabilisation are well known to have a significant impact on the patient's outcome. Therefore, the optimal choices for both the type of initial fracture stabilisation (external fixation/EF, early total care/ETC, conservative treatment/TC) and the best time point for conversion from temporary to definitive fixation are challenging factors. PATIENTS Patients aged ≥ 16 years with moderate and severe trauma documented in the TraumaRegister DGU$^{®}$ between 2002 and 2018 were retrospectively analysed. Demographics, ISS, surgical treatment strategy (ETC vs. EF vs. TC), time for conversion to definitive care, complication (MOF, sepsis) and survival rates were analysed. RESULTS In total, 13,091 trauma patients were included. EF patients more often sustained high-energy trauma (car: 43.1 vs. 29.5%, p < 0.001), were younger (40.6 vs. 48.1 years, p < 0.001), were more severely injured (ISS 25.4 vs. 19.1 pts., p < 0.001), and had higher sepsis (11.8 vs. 5.4%, p < 0.001) and MOF rates (33.1 vs. 16.0%, p < 0.001) compared to ETC patients. A shift from ETC to EF was observed. The time until conversion decreased for femoral fractures from 9 to 8 days within the observation period. Sepsis incidences decreased in EF (20.3 to 12.3%, p < 0.001) and ETC (9.1-4.8%, p < 0.001) patients. CONCLUSIONS Our results show the changes in the surgical treatment of severely injured patients with femur fractures over a period of almost two decades caused by the introduction of modern surgical strategies (e.g., Safe Definitive Surgery). It remains unclear which subgroups of trauma patients benefit most from these strategies.
