Device‐related pressure injury (DRPI) is a serious problem that is affecting professionals working on the front lines against COVID‐19 due to the prolonged use of personal protective equipment (PPE). In addition to the physical and psychological integrity of professionals, these injuries can compromise the quality of care. Therefore, using technologies to prevent this adverse effect is an urgent matter. This is a parallel two‐arm randomized clinical trial without the use of a control group to compare the use of foam and extra‐thin hydrocolloid in preventing DRPI associated with the use of PPE by health professionals working on the front lines against coronavirus. In total, 88 professionals were divided into two groups: foam and hydrocolloid. Data were collected using two instruments and related to demographic and professional characteristics and skin evaluation. Each volunteer received one of the dressings, both with the same dimensions and arranged over similar regions, and data were gathered at baseline and after 6 or 12 hours. Descriptive and inferential analytic statistical methods were used; the significance level adopted was 5%. No participant developed DRPI, but four areas with hyperemia were observed in the foam group (two in the forehead, one in the cheeks, and one in the nose bridge), as well as four areas with hyperemia in the hydrocolloid group (two in the nose bridge, one in the right ear, and one in the left ear). There was no difference between the groups regarding skin conditions and discomfort (P >.05). The average cost obtained was $ 5.8/person and $ 4.4/person in the foam group and the hydrocolloid group, respectively, considering the dressing measurements. Aim: To describe the alpha-linolenic (ALA) fatty acid effect on collagen deposition and tgfb1 modulation during wound healing. Method: Animal Ethics Committee (5635-1/2020). C57BL/6J isogenic mice were randomized in a diabetic group that received a daily intraperitoneal application, for 5 consecutive days, of streptozotocin (STZ) (50mg/kg) 70 mg/dL, in citrate buffer, and a nondiabetic group treated with citrate buffer. After the fourth week, fasting capillary blood glucose and metabolic characterization were performed. Animals were submitted to an insertion of an osmotic pump in the dorsal interscapular region, for continuous infusion of ALA 5mMol/kg/24h in the intervention groups, and ALA dilution vehicle in the control group. Two dorsal wounds were made with a punch (6mm). The wound healing process was evaluated by macroscopic, histological, gene, and protein modulation, on days 3, 7, and 12 post-wound. Groups were compared using two-way ANOVA (p≤0.05). Results / Discussion: The continuous supplementation with ALA fatty acids by 7 days before the injury can accelerate the wound healing processes and alteration de collagen deposit only in hyperglycemia conditions, to similar neuronal dopaminergic gene expression in skin. These changes occur concomitantly with de tgb1 gene modulation. Reducing the closer time is a therapeutic goal of wound healing care. The osmotic pump is a promissory method for chronic wounds therapeutic by minimizing the no adhesion of treatment. The TGFβ pathway is attenuated in nonhealing wounds, and its modulation may lead to the development of novel therapeutic advances for chronic wounds. Aim: To characterize and evaluate the Insulin Gel's in vitro toxicity and wound healing potential in a wound excisional model. Method: The insulin was evaluated in terms of physical characteristics, vitro toxicity and in vivo wound healing. The Insulin Gel's cytotoxicity on human dermal keratinocyte (HaCat) cells was studied by 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide (MTT) assay. The wound healing properties of Insulin gel were evaluated through an in vivo excisional wound model using C57BL/6JUnib mice. The animals were divided in three experimental groups: Insulin Gel, Placebo Gel, and Saline. Wound photographs were taken every 3-4 days and the wound contraction was calculated by the ImageJ® Software. The histopathological characteristics were analyzed on day 7th, 14th and 20th post wound by Hematoxilin and Eosin (H&E) staining. Results / Discussion: MTT assay revealed the HaCaT cells treated with different concentrations of Insulin Gel showed proliferative effects after 24 hours of exposure. The wound-healing rate closure was higher on Insulin group compared to other groups at 3, 10, 7, 14 days after injury. Histological evaluation showed that on the 7th day after injury, the Insulin group showed less inflammation and more organized granulation tissue compared to other groups. On the 14th day after injury, the tissues had similar characteristics in the three groups. On the 20th day after injury, all groups showed complete re-epithelialization, but the INS group showed hair follicle growth and a smaller scar area. The development of diabetic foot ulcers is a common and severe complication of diabetes that can significantly affect quality of life. The physiological healing cascade does not progress tissue repair in diabetic foot ulcerations in a timely manner. Serum markers from foot ulcers have been used to characterize the healing process of the diabetic foot using various collection techniques. This study aimed to compare the use of cervical brushes and the Levine technique to collect wound fluid from foot ulcers of people with diabetes in order to determine the presence of cytokines. The collected material was used for gene expression analysis of macrophage/monocyte-associated cytokines IL1-β, IL-6, TNF-α, regulatory cytokine IL-10 and growth factor TGFβ, via quantitative polymerase chain reaction (qPCR). Both collection methods produced sufficient amounts of RNA, but significantly more RNA was collected using a cervical brush (brush 224.82 ng/μL vs. Levine 80.90 ng/μL p = 0.0001). Significantly higher levels of expression of the following cytokine genes were detected in samples collected using a cervical brush: IL1-β (p = 0.0001), IL-6 (p = 0.0106), IL-10 (p = 0.0277) and TGFβ (p = 0.0002). Understanding why some wounds are difficult to heal is important for developing more effective treatments, and biomarkers may be useful for predicting the healing trajectory. Background: Burns are classified according to their mechanism of injury, depth, affected body area, affected region or part of the body, and extent of the lesions. Topical insulin modulates the healing process. However, studies evaluating the effects of topical insulin treatment on burns in human patients are lacking., Purpose: The purpose of this study was to investigate the effects of topical insulin on healing time of second-degree burns., Methods: In this nonrandomized clinical trial, patients with second-degree burns were allocated to a control group (CG) or an intervention group (IG) in which wounds were treated with 1% silver sulfadiazine and topical insulin, respectively., Results: Healing time was significantly shorter in the IG relative to the CG (9.1 ± 1.9 days and 12.7 ± 3.3 days, respectively; P < .05). The estimated burn area was similar in both groups (CG 1.44 ± 1.0%; IG 1.42 ± 0.53%)., Conclusion: In this study, topical insulin reduced healing time in second-degree burns. Further investigation is warranted to support wider use in clinical practice.

The acute and chronic complications that patients with diabetes and their families have to deal with after hospital discharge may be a consequence of the deficiencies in the educational process across hospitalization and the formal preparation for discharge. The objective of this study is to present a proposal of the plan for hospital discharge of adult patients with diabetes. A literature review on the hospital discharge of the assessed population was performed, including articles published between 2004 and February 2009. Taking the literature into consideration, a flier was created to guide the discharge process. The flier lists the information that should be collected and worked with the patient during the first four days of hospitalization, considering their individual needs and the Ineffective self health management. The discharge must be inserted in the Nursing Process, as nurses have an essential role in identifying the needs of patients and their families. The flier helps to identify the patient's needs and the actions to be performed by the team.
