11. Patterns of urban land use in a developing country: the role of transport infrastructure and natural amenities in Brazil

Abstract

The paper investigates the determinants of urban land use for the city of Recife, Brazil. Using microdata on 98,198 individual parcels and a conditionally parametric spatial multinomial logit model, it is shown that there are considerable spatial heterogeneities in the studied intra-urban context. Additionally, some general patterns of land use are identified: transport infrastructure tends to attract commercial activities, while natural amenities tend to encourage residential development. Finally, on average, the probability of unimproved lots is greater in the immediate vicinity of the historic city centre, and verticalization is greater in areas near the city centre and natural amenities.

Abstract

Purpose of Review : This review summarizes the pathophysiology of calcific aortic valve stenosis (CAVS) and surveys relevant clinical data and basic research that explain how CAVS arises. Recent Findings : Lipoprotein(a) [Lp(a)], lipoprotein-associated phospholipase A2 (Lp-PLA2), oxidized phospholipids (OxPL), autotaxin, and genetic driving forces such as mutations in LPA gene and NOTCH gene seem to play a major role in the development of CAVS. These factors might well become targets of medical therapy in the coming years. Summary : CVAS seems to be a multifactorial disease that has much in common with coronary artery disease, mainly regarding lipidic accumulation and calcium deposition. No clinical trials conducted to date have managed to answer the key question of whether Lp(a) lowering and anti-calcific therapies confer a benefit in terms of reducing incidence or progression of CAVS, although additional outcome trials are ongoing.

Abstract

OBJECTIVES: This study sought to evaluate the impact of patient–prosthesis mismatch (PPM) on the risk of perioperative, early-, mid- and long-term mortality rates after surgical aortic valve replacement. METHODS: Databases were searched for studies published until March 2018. The main outcomes of interest were perioperative mortality, 1-year mortality, 5-year mortality and 10-year mortality. RESULTS: The search yielded 3761 studies for inclusion. Of these, 70 articles were analysed, and their data were extracted. The total num- ber of patients included was 108 182 who underwent surgical aortic valve replacement. The incidence of PPM after surgical aortic valve re- placement was 53.7% (58 116 with PPM and 50 066 without PPM). Perioperative mortality [odds ratio (OR) 1.491, 95% confidence interval (CI) 1.302–1.707; P < 0.001], 1-year mortality (OR 1.465, 95% CI 1.277–1.681; P < 0.001), 5-year mortality (OR 1.358, 95% CI 1.218–1.515; P < 0.001) and 10-year mortality (OR 1.534, 95% CI 1.290–1.825; P < 0.001) were increased in patients with PPM. Both severe PPM and moderate PPM were associated with increased risk of perioperative mortality, 1-year mortality, 5-year mortality and 10-year mortality when analysed together and separately, although we observed a higher risk in the group with severe PPM. CONCLUSIONS: Moderate/severe PPM increases perioperative, early-, mid- and long-term mortality rates proportionally to its severity. The findings of this study support the implementation of surgical strategies to prevent PPM in order to decrease mortality rates.

METHODS: Databases were searched for studies published until April 2018 to carry out a systematic review followed by meta-analysis of results. RESULTS: The search yielded 1468 studies for inclusion. Of these, 10 articles were analysed and their data extracted. A total of 13 174 patients (AVR with ARE: 2819 patients; AVR without ARE: 10 355 patients) were included from studies published from 2002 to 2018. The total rate of ARE was 21.4%, varying in the studies from 5.7% to 26.3%. The overall odds ratio (OR) [95% confidence interval (CI)] for periop- erative mortality showed a statistically significant difference between the groups (among 10 studies), with a higher risk in the ‘AVR with ARE’ group (OR 1.506, 95% CI 1.209–1.875; P < 0.001), but not when adjusted for isolated AVR + ARE without any concomitant procedures such as mitral valve surgery, coronary artery bypass surgery, etc. (OR 1.625, 95% CI 0.968–2.726; P = 0.066—among 6 studies). The ‘AVR with ARE’ group showed an overall lower risk of significant patient–prosthesis mismatch among 9 studies (OR 0.472, 95% CI 0.295–0.756; P = 0.002) and a higher overall difference in means of indexed effective orifice area among 10 studies (random-effect model: 0.06 cm2/m2, 95% CI 0.029–0.103; P < 0.001). CONCLUSIONS: Surgical ARE seems to be associated with increased perioperative mortality but with lower risk of patient–prosthesis mismatch.

Results: The research yielded 2,985 studies for inclusion. Of these, 16 articles were analyzed, and their data extracted. The total number of patients included was 10,239, who underwent mitral valve replacement. The incidence of prosthesis-patient mismatch after mitral valve replacement was 53.7% (5,499 with prosthesis- patient mismatch and 4,740 without prosthesis-patient mismatch). Perioperative (OR 1.519; 95%CI 1.194–1.931, P<0.001) and 10-year (OR 1.515; 95%CI 1.280–1.795, P<0.001) mortality was increased in patients with prosthesis-patient mismatch. Patients with prosthesis-patient mismatch after mitral valve replacement had higher systolic pulmonary artery pressure and transprosthethic gradient and lower indexed effective orifice area and left ventricle ejection fraction. Conclusion: Prosthesis-patient mismatch increases perioperative and long-term mortality. Prosthesis-patient mismatch is also associated with pulmonary hypertension and depressed left ventricle systolic function. The findings of this study support the implementation of surgical strategies to prevent prosthesis- patient mismatch in order to decrease mortality rates.

22. Patterns of land use, zoning and economies of scale in cities: three essays on urban economics for Brazil

Abstract

FACEPE This thesis is divided into three empirical essays in the field of urban economics, analyzing the case of Brazilian cities. The first essay uses a novelty microdata of 98,198 individual parcels in the city of Recife to investigate the determinants of the urban land use. Through a semi-parametric spatial econometric approach, it is observed that there are considerable local spatial effects. In addition, we note some regularities in the spatial distribution of urban land use. Transport infrastructures tend to attract the development of commercial activities, while natural amenities encourage residential land use. The objective of the second essay is to evaluating the impact of zoning ordinances on the housing market. Using matching methods and a dataset covering all Brazilian municipalities, we evidenced that zoning generates an increment ranging from 5.4% to 6.3% on the average rents, but do not affect the house growth. In addition, we conducted sensitivity analyses that suggests that this result is not simply driven by unobservable confounders. Finally, the third essay investigates the impact of municipal secession on local public expenditures through a Differences-in-Differences approach. The results indicate that those municipalities that experienced a secession process increased their per capita capital expenditures by 14.7%. In addition, we show evidence that suggests that this increase in expenses can be explained by a reduction in economies of scale and rent-seeking behavior. Essa tese está dividida em três ensaios empíricos no campo de economia urbana, analisando o caso das cidades brasileiras. O primeiro ensaio utiliza uma base de dados inédita contendo informações sobre 98.198 lotes individuais da cidade do Recife para investigar os determinantes dos diferentes usos do solo urbano. Através de uma abordagem espacial não paramétrica, observa-se que existem consideráveis efeitos espaciais locais. Além disso, notamos algumas regularidades na distribuição espacial do uso do solo urbano. As infraestruturas de transportes tendem a atrair o desenvolvimento de atividades comerciais, enquanto as amenidades naturais incentivam o uso residencial. Já o segundo ensaio, concentra-se em avaliar o impacto das políticas de zoneamento urbano sobre o mercado habitacional. Utilizando o método de pareamento e um conjunto de dados que abrangem todos os municípios brasileiros, evidencia-se que as leis de zoneamento provocam um acréscimo que varia entre 5.4% a 6.3% nos aluguéis médios, mas não afetam o crescimento no número de casas. Adicionalmente, conduzimos uma série de análises de sensibilidade que mostram que esse resultado não é simplesmente guiado por variáveis não observáveis. Por último, o terceiro ensaio investiga o impacto da secessão de municípios sobre os gastos públicos locais através de uma abordagem de Diferenças em Diferenças. Os resultados indicam que os municípios que experimentaram um processo de secessão aumentaram as suas despesas per capita com capital em cerca de 14.7%. Além disso, mostramos evidências que sugerem que esse aumento pode ser explicado por perda de economias de escala e pelo aumento do comportamento rent-seeking.

Abstract

The Brazilian state of Tocantins was splinted from the state of Goiás in 1988. This was the most recent first-tier subnational border reform in Brazil and involved an area that corresponds to 7.6% of the whole national territory. Using the synthetic control method, this paper estimates that the split increased the per capita GDP of the affected region by an average of 8.26%, and the positive effect persisted over the years. Abstract

Prosthesis-patient mismatch (PPM) after surgical aortic valve replacement (SAVR) is an issue that has been overlooked (not to say neglected). Cardiac surgeons must bear in mind that this is a real problem that we must tackle. The purpose of this paper is to be a wake-up call to the surgical community by giving a brief overview of what PPM is, its incidence and impact on the outcomes. We also discuss the increasing role played by imaging for predicting and assessing PPM after SAVR (with which surgeons must become more acquainted) and, finally, we present some options to avoid PPM after the surgical procedure. Abstract

Objectives: This study sought to evaluate the impact of prosthesis-patient mismatch (PPM) on the risk of early-term mortality after transcatheter aortic valve implantation (TAVI). Methods: Databases (Medical Literature Analysis and Retrieval System Online [MEDLINE], Excerpta Medica dataBASE [EMBASE], Cochrane Controlled Trials Register [CENTRAL/CCTR], ClinicalTrials. gov, Scientific Electronic Library Online [SciELO], Latin American and Caribbean Literature on Health Sciences [LILACS], and Google Scholar) were searched for studies published until February 2019. PPM after TAVI was defined as moderate if the indexed effective orifice area (iEOA) was between 0.85 cm²/m² and 0.65 cm²/m² and as severe if iEOA = 0.65 cm²/m². Results: The search yielded 1,092 studies for inclusion. Of these, 18 articles were analyzed, and their data extracted. The total number of patients included who underwent TAVI was 71,106. The incidence of PPM after TAVI was 36.3% (25,846 with PPM and 45,260 without PPM). One-year mortality was not increased in patients with any PPM (odds ratio [OR] 1.021, 95% confidence interval [CI] 0.979-1.065, P=0.338) neither in those with moderate PPM (OR 0.980, 95% CI 0.933-1.029, P=0.423). Severe PPM was separately associated with high risk (OR 1.109, 95% CI 1.041-1.181, P=0.001). Conclusion: The presence of severe PPM after TAVI increased early-term mortality. Although moderate PPM seemed harmless, the findings of this study cannot not rule out the possibility of it being detrimental, since there are other registries that did not address this issue yet. Abstract

In this paper, a flash flood that occurred in the Brazilian state of Santa Catarina in 2008 is used to investigate the existence of spatial spillovers from natural disasters in geographically‐linked areas. In order to do so, we estimated a Difference‐in‐Differences model that explicitly allows for the existence of spatial interactions within affected and unaffected regions. Our results show that municipalities directly affected by the flood suffered an 7.6% decrease in GDP per capita in the year of the disaster. Three years after the flood, however, GDP per capita rebounded back to pre‐disaster levels in all sectors but the Agricultural sector. Abstract

Objective: This study sought to evaluate the impact of prosthesis-patient mismatch on the risk of perioperative and longterm mortality after mitral valve replacement. Methods: Databases were researched for studies published until December 2018. Main outcomes of interest were perioperative and 10-year mortality and echocardiographic parameters. Results: The research yielded 2,985 studies for inclusion. Of these, 16 articles were analyzed, and their data extracted. The total number of patients included was 10,239, who underwent mitral valve replacement. The incidence of prosthesis-patient mismatch after mitral valve replacement was 53.7% (5,499 with prosthesispatient mismatch and 4,740 without prosthesis-patient mismatch). Perioperative (OR 1.519; 95%CI 1.194-1.931, P<0.001) and 10-year (OR 1.515; 95%CI 1.280-1.795, P<0.001) mortality was increased in patients with prosthesis-patient mismatch. Patients with prosthesis-patient mismatch after mitral valve replacement had higher systolic pulmonary artery pressure and transprosthethic gradient and lower indexed effective orifice area and left ventricle ejection fraction. Conclusion: Prosthesis-patient mismatch increases perioperative and long-term mortality. Prosthesis-patient mismatch is also associated with pulmonary hypertension and depressed left ventricle systolic function. The findings of this study support the implementation of surgical strategies to prevent prosthesispatient mismatch in order to decrease mortality rates. [ABSTRACT FROM AUTHOR]

Abstract

O objetivo do trabalho é identificar os determinantes do crescimento econômico e analisar a dinâmica de renda considerando um painel de 1970-2010 com 522 microrregiões brasileiras. Tendo como base o modelo teórico de Mankiw-Romer-Weil com a expansão espacial proposta por Ertur e Koch (2006), emprega-se um Spatial Durbin Model (SDM) com efeitos-fixos, uma estratégia empírica que considera simultaneamente a dependência espacial e as heterogeneidades específicas de cada economia. Soma-se a isso a estimação dos impactos diretos e impactos indiretos (spillovers) dos determinantes do crescimento regional. Os resultados indicam uma forte dependência espacial entre as microrregiões brasileiras, além disso, há evidências que mostram que tanto o investimento em capital físico quanto o investimento em capital humano importam não só para o crescimento da própria economia, como também para o crescimento de economias vizinhas.

Abstract: In the 1990s, there was a growing process of administrative decentralization that culminated in the creation of 1,016 new municipalities in Brazil. The aim of this paper is to verify the impact of the municipal secessions on the public expenditures and its association with economies of scale. Based on a Differences‐in‐Differences methodology, the obtained set of evidence indicates that those municipalities that underwent a secession process increased their per capita capital expenditures by 14.7 percent. In addition, we show evidence that strongly suggests that this increase in expenses can be explained by a reduction in economies of scale and rent‐seeking behavior. [ABSTRACT FROM AUTHOR]

Abstract

This study aimed to evaluate the efficacy and safety of continuing versus stopping aspirin [acetylsalicylic acid (ASA)] preoperatively in patients undergoing coronary artery bypass graft surgery. MEDLINE, EMBASE, CENTRAL/Cochrane Controlled Trials Register (CCTR), ClinicalTrials.gov, Scientific Electronic Library Online (SciELO), Literatura Latino Americana em Ciências da Saúde (LILACS), Google Scholar and reference lists of relevant articles were searched for randomized controlled trials that reported efficacy outcomes ofmyocardial infarction and mortality, and safety outcomes of blood loss, packed red blood cell transfusion and surgical re-exploration were compared between groups. Fourteen studies fulfilled our eligibility criteria and included a total of 4499 patients (2329 for 'continuing ASA' and 2170 for 'stopping ASA'). In the pooled analysis, continuing aspirin therapy did not reduce the risk of myocardial infarction [risk ratio 0.834, 95% confidence interval (CI) 0.688-1.010; P = 0.063] or operative mortality (risk ratio 1.384, 95% CI 0.727-2.636; P = 0.323). Preoperative ASA increased postoperative chest tube drainage (mean difference 143 ml, 95% CI 39-248 ml; P = 0.007) and packed red blood cell transfusion (mean difference 142 ml, 95% CI 55-228; P = 0.001) but did not increase the risk of surgical re-exploration (risk ratio 1.316, 95% CI 0.910-1.905; P = 0.145). This meta-analysis found no statistically significant difference regarding the risk of operative mortality and myocardial infarction between the 'continuing ASA' and 'stopping ASA' strategies. On the other hand, the mean volume of blood loss and packed red blood cell transfusion was higher in the 'continuing ASA' group, but this finding did not translate into higher risk of reoperation for bleeding.
