Objective: The objective of this study is to provide a description of a group of retrospective cohort outcomes in patients with systemic lupus erythematosus (SLE) complicated with immune thrombocytopenia (ITP) receiving belimumab. Methods: This study reports on the treatment of 10 female patients (mean age 34.3 ± 14.0 years, mean weight 58.7 ± 18.2 kg) with both SLE and ITP who received belimumab in addition to basic drug therapy. The belimumab treatment regimen consisted of a dosage of 10 mg/kg, with an initial infusion every 2 weeks for the first 3 doses, followed by an infusion every 4 weeks. Results: Ten patients were included in the study. The overall response rate of thrombocytopenia was 90% after treatment. The parameters such as platelet count, lymphocyte count, erythrocyte count, hemoglobin, dsDNA, C3, and C4 were significantly improved (p <.05). The SLE Disease Activity Index (SLEDAI), British Islet lupus Assessment Group 2004 (BILAG-2004), and Physician Global assessment (PGA) scores were significantly decreased (p <.05). There were no significant differences in glutamic pyruvic transaminase (ALT), glutamic oxaloacetic transaminase (AST), and serum creatinine (Scr) before and after treatment (p >.05). Conclusion: Belimumab shows promising clinical outcomes in the treatment on patients with both SLE and ITP. Further studies are needed to validate these findings in larger patient populations and compare the efficacy of belimumab with other treatments for SLE complicated with ITP. The carbonate sedimentary microfacies characteristic and evolution model of the Middle and Lower Ordovician have been established through the field work of Bachudabantage, Bachudawuzitage, Kepingshuinichang outcrop as well as abundant hand specimen and slice observation in the northwestern of Tarim Basin. Based on macro sedimentary structure and micro characteristics, 11 kinds of limestone microfacies and 6 kinds of dolomite microfacies have been identified. Different microfacies were combined into 7 kinds of microfacies combination with particular facies sequence to reflect the specific sedimentary environment and reef building characteristic, which contains high energy thick-bedded platform edge shallow reef, tidal flat intraclast beach, open platform intraclast shoal, moderate-low energy thin-interbedded intraclast beach and interbank sea, open platform bioclastic beach, low energy thin-bedded interbank sea, arid and semi-arid climate thick bedded restricted platform dolomite tidal sequence. The study area mainly changes from restricted platform, dolomite tidal to open platform and platform margin with large set of shoal and reef development. Based on the discussion above, platform margin reef and shoal, inner shoal and dolomitic flat are the favorable reservoir facies belts to breakthrough in priority. [ABSTRACT FROM AUTHOR]

Based on thin sections, cathodoluminescence (CL), scanning electron microscope (SEM), X-diffraction and imaging logging data analysis, Serpukhovian carbonate reservoir space types and properties, together with its diagenetic types, environments and evolution both in the Lower Carboniferous high frequency sequences and in the sequence boundaries were studied in Marsel block of southern Kazakhasian.The results show that in middle-lower part of Serpukhovian high frequency sequences (the 1st to 3rd forth-order sequences), penecontemporaneous and burial dolomitization combined with tectonic processes made great contribution to early-middle Serpukhovian carbonate reservoirs. While in middle-upper part of Serpukhovian high frequency sequences (the 4th to 6th forth-order sequences), peneconlemporaneous and seepage-reflux dolomitization combined with strong peneconlemporaneous and meteoric water dissolution at early diagenetic stage had significant impact on late Serpukhovian carbonate reservoirs quality. However, strong cementation and compaction with weak dissolution in burial diagenetic environment of middle Serpukhovian high frequency sequence (the third forth-order sequence) are key factors leading to poor reservoir proper-ties. Based on the comprehensive correlation of outcrops data，logging data and drilling data，we established the Cretaceous sequence stratigraphic framework in the piedmont zone of the south Tianshan and western Kunlun mountains in Southwest Depression，Tarim Basin. According to the regional unconformities and regional mutations in the interface of sedimentary facies，two second-order sequences of the Cretaceous in the study area were recognized，which can be further divided into four third-order sequences based on the change of sedimentary facies，transforming surface of stratigraphic accumulation type and the structure of sedimentary cycle. Based on the analysis from outcrops and wireline logging datasets in the study area，we recognized five sedimentary facies including alluvial fan，braided river，meandering river，braided river delta，meandering river delta. The distribution pattern of sedimentary systems for each sequence is systematically studied，through which the spatial distribution and vertical evolution of depositional systems are revealed. Studies show that the deposition scope and the depositional systems had changed significantly in Cretaceous in Southwest Depression. The large-scale sea transgression events were generally proved by the evidence that the sedimentary scope changed from piedmont belt to intra-basin in the study area. The NW-trending pre-existing faults and the NE-trending early faults controlled the structural evolution of the Shaleitian uplift margin. According to the characteristics of the main faults, the combination of the main faults, the early synsedimentary faults and the companion faults, Shaleitian uplift margin accommodates the following six kinds of the Paleogene faulting structure styles: concordant fault terrace gentle-slope margin, major low-angle faulted gentle-slope margin, straight normal faulted gentle-slope margin, listric normal faulted steep-slope margin, ramp-flat normal faulted steep-slope margin and platy normal faulted steep-slope margin. Different types of faulted slope-breaks and palaeogeomorphology formed by fault structural styles with early valley developed on the uplift controlled the input and distribution of material source and resulted in different types of raised edge sedimentary filling characteristics. As a result, the plane distribution and vertical evolution of sedimentary facies belts from the uplift to the depression have certain regularity. In concordant fault terrace gentle-slope margin, the low fan-delta developed in early period and the thick braided delta front developed in later period in control of the early basin-margin slope. The main fault which separates Shaleitian uplift and the adjacent depression controlled the development of the high fan-delta in early period and the braided delta in later period. The syndepositional fault which developed on the major low-angle normal fault controlled the deposition of the low fan delta and turbidite fan in the Es3 member and then developed braided river delta and shore shallow lake in the major low-angle faulted gentle-slope margin. The transgressive and high braided river deltas developed in Shahejie Formation in the straight normal faulted gentle-slope margin, the thick braided deltas front developed below the slope break and the delta plain and small filled valleys developed on the slope break in Dongying Formation. The sedimentary filling has a layered structure in the vertical direction in the edge of the steep slope. From early to late stage,the early alluvial fan or fan delta facies evolved into braided river delta, the steep slope controls the development of the low fan delta,and the delta plain and small filled valleys developed on the slope break. Objective: To compare the effects of tofacitinib and adalimumab on the risk of adverse lipidaemia outcomes in patients with newly diagnosed rheumatoid arthritis (RA)., Methods: Data of adult patients newly diagnosed with RA who were treated with tofacitinib or adalimumab at least twice during a 3-year period from 1 January 2018 to 31 December 2020, were enrolled in the TriNetX US Collaborative Network. Patient demographics, comorbidities, medications, and laboratory data were matched by propensity score at baseline. Outcome measurements include incidental risk of dyslipidemia, major adverse cardiac events (MACE) and all-cause mortality., Results: A total of 7,580 newly diagnosed patients with RA (1998 receiving tofacitinib, 5,582 receiving adalimumab) were screened. After propensity score matching, the risk of dyslipidaemia outcomes were higher in the tofacitinib cohort, compared with adalimumab cohort (hazard ratio [HR] with 95% confidence interval [CI], 1.250 [1.076-1.453]). However, there is no statistically significant differences between two cohorts on MACE (HR, 0.995 [0.760-1.303]) and all-cause mortality (HR, 1.402 [0.887-2.215])., Conclusion: Tofacitinib use in patients with RA may increase the risk of dyslipidaemia to some extent compared to adalimumab. Objective: To assess the therapeutic efficacy and safety of Kunxian Capsule (KXC) in treatment of rheumatoid arthritis (RA)., Methods: Randomized positive parallel controlled and multi-center open test method was adopted. 240 RA patients of mild/moderate degree were randomly assigned to three groups equally, i.e., KXC group (who took KXC), the methotrexate (MTX) group (who took MTX), and the KXC + MTX group (who took KXC and MTX simultaneously), respectively. The therapeutic course for them all was 12 weeks. The effect of the treatment was assessed in items of DAS28, ACR20, and ACR50; number of joints with pain and swelling; VAS score of pain, tiredness, and general condition; time of morning stiffness; bilateral grip strength; HAQ score, as well as blood levels of erythrocyte sedimentation rate (ESR), C-reactive protein (CRP), rheumatoid factor (RF), anti-CCP antibody, and platelet count., Results: By the end of the 4th week, the improvement of ACR20, ACR50, DAS28 efficacy judgment, and DAS28 score in the KXC + MTX group were much better than those in the other two groups, with statistical difference (P<0.05). The total effective rate was 88. 6% and the markedly effective rate was 51.8% in the KXC + MTX group at the 12 th week. The Improvement was more obviously shown in all groups after treatment (all P<0.05). Better effects in reducing VAS scores of pain and tiredness were shown in the KXC group and the KXC + MTX group. The effects of KXC + MTX were superior to the other two groups in terms of swollen joint numbers, pain joints, grip strength (assessed by researcher), as well as VAS score of general condition and HAQ score (assessed by both patients and researcher, P<0.05). But the differences among groups in improving morning stiffness and the incidence rate of adverse events were in- significant., Conclusions: KXC could relieve symptoms, improve joint functions, physical signs, and laboratory indices of RA patients with less adverse reaction. It was synergistic with MTX.
