Abstract

In the medical field, accurate diagnosis of lung cancer is crucial for treatment. Traditional manual analysis methods have significant limitations in terms of accuracy and efficiency. To address this issue, this paper proposes a deep learning network framework based on the pre-trained MobileNetV2 model, initialized with weights from the ImageNet-1K dataset (version 2). The last layer of the model (the fully connected layer) is replaced with a new fully connected layer, and a softmax activation function is added to efficiently classify three types of lung cancer CT scan images. Experimental results show that the model achieves an accuracy of 99.6% on the test set, with significant improvements in feature extraction compared to traditional models.With the rapid development of artificial intelligence technologies, deep learning applications in medical image processing are bringing revolutionary changes to the healthcare industry. AI-based lung cancer detection systems can significantly improve diagnostic efficiency, reduce the workload of doctors, and occupy an important position in the global healthcare market. The potential of AI to improve diagnostic accuracy, reduce medical costs, and promote precision medicine will have a profound impact on the future development of the healthcare industry.

Abstract

The accurate prediction of B-cell epitopes is critical for guiding vaccine development against infectious diseases, including SARS and COVID-19. This study explores the use of a deep neural network (DNN) model to predict B-cell epitopes for SARS-CoVandSARS-CoV-2,leveraging a dataset that incorporates essential protein and peptide features. Traditional sequence-based methods often struggle with large, complex datasets, but deep learning offers promising improvements in predictive accuracy. Our model employs regularization techniques, such as dropout and early stopping, to enhance generalization, while also analyzing key features, including isoelectric point and aromaticity, that influence epitope recognition. Results indicate an overall accuracy of 82% in predicting COVID-19 negative and positive cases, with room for improvement in detecting positive samples. This research demonstrates the applicability of deep learning in epitope mapping, suggesting that such approaches can enhance the speed and precision of vaccine design for emerging pathogens. Future work could incorporate structural data and diverse viral strains to further refine prediction capabilities.

3. Dosimetric commissioning of a high-resolution CMOS 2D detector array for patient-specific QA of single-isocenter multi-target VMAT stereotactic radiosurgery.

Abstract

Stereotactic radiosurgery (SRS) using the single-isocenter-multiple-target (SIMT) technique by volumetric modulated arc therapy is increasingly popular for treating multiple brain metastases. However, the complex nature of SIMT SRS necessitates rigorous patient-specific quality assurance (PSQA). This study presents a multi-institutional dosimetric commissioning of a high-resolution complementary metal oxide semiconductor (CMOS) 2D detector array, the myQA SRS device for SIMT SRS PSQA. Basic dosimetric properties such as dose-rate, field-size, energy and angular dependencies were characterized for the CMOS detectors. Additionally, gamma index analyses were performed between the measured dose and the films for nine simulated and clinical plans. The results showed that the CMOS detector was dose-rate, field-size, energy and beam-angle dependent. Specific to SIMT SRS, angular dependence on gantry rotations was invariant to couch rotations but was sensitive to off-isocenter distances. With appropriate dose calibration and angular corrections, myQA SRS showed a high dosimetric correlation with films. The average gamma index pass rates were 99.9 ± 0.03% and 99.2 ± 1.1% at 3%/2 mm/10%thr(global) and 1 mm/1%/10%thr(local) criteria, respectively. The average dose difference between myQA SRS and films was 0.4 ± 1.3%. In conclusion, the CMOS 2D detector array has demonstrated its potential as a reliable tool for PSQA for SIMT SRS. The excellent dosimetric agreement with the films was consistent in multiple institutions, further validating the dosimetric accuracy and reproducibility. Abstract

Objective: To investigate the feasibility of standardizing RT simulation CT scanner protocols between vendors using target‐based image quality (IQ) metrics. Method and materials: A systematic assessment process in phantom was developed to standardize clinical scan protocols for scanners from different vendors following these steps: (a) images were acquired by varying CTDIvol and using an iterative reconstruction (IR) method (IR: iDose and model‐based iterative reconstruction [IMR] of CTp‐Philips Big Bore scanner, SAFIRE of CTs‐Siemens biograph PETCT scanner), (b) CT exams were classified into body and brain protocols, (c) the rescaled noise power spectrum (NPS) was calculated, (d) quantified the IQ change due to varied CTDIvol and IR, and (e) matched the IR strength level. IQ metrics included noise and texture from NPS, contrast, and contrast‐to‐noise ratio (CNR), low contrast detectability (d′). Area under curve (AUC) of the receiver operation characteristic curve of d′ was calculated and compared. Results: The level of change in the IQ ratio was significant (>0.6) when using IMR. The IQ ratio change was relatively low to moderate when using either iDose in CTp (0.1–0.5) or SAFIRE in CTs (0.1–0.6). SAFIRE‐2 in CTs showed a closer match to the reference body protocol when compared to iDose‐3 in CTp. In the brain protocol, iDose‐3 in CTp could be matched to the low to moderate level of SAFIRE in CTs. The AUC of d′ was highest when using IMR in CTp with lower CTDIvol, and SAFIRE in CTs performed better than iDose in CTp Conclusion: It is possible to use target‐based IQ metrics to evaluate the performance of the system and operations across various scanners in a phantom. Abstract

A growing body of evidence documents pervasive social and demographic factors relating to disparities in long-term care (LTC). In 2007, Taiwan implemented its Ten-year Long-Term Care Plan version 1.0 (TLTCP 1.0) that aimed to develop a home-and community-based (HCBS) LTC system. In 2016, Taiwan began to implement TLTCP 2.0. To continue providing effective LTC, this study aimed to assess the disparities in access to LTC services using Taiwan’s LTC claim database from 2010 to 2013. A total of 87,438 older adults who had applied for LTC services from the TLTCP 1.0 were included. The study assessed LTC disparities related to five sociodemographic factors, including age, gender, living status, urbanization, and income status. Sixteen types of LTC services, including HCBS, home-based professional care, meal services, transportation, and institutional services were assessed. After controlling for the level of disability, we found that those who were older-old (age 80> over), male, and low-income were less likely to use HCBS, but more likely to use institutions services (p < 0.001). We also found that those who lived in the city were more likely to use HCBS and transportation services (p < 0.001). Yet, older adults living alone were more likely to use home care and meal services but not other types of LTC services (p < 0.001). In conclusion, the social disparities in access to LTC services in Taiwan remains, suggesting LTC 2.0 should continue monitoring and placing the LTC equity issue on the top priority.

22. Optic disc dose reduction in ocular brachytherapy using 125I notched COMS plaques: A simulation study based on current clinical practice.

Abstract

Purpose: Although notched Collaborative Ocular Melanoma Study (COMS) plaques have been widely used, optic disc dose reduction by notched COMS plaques has not been discussed in the literature. Therefore, this study investigated optic disc dose reduction in ocular brachytherapy using 125I notched COMS plaques in comparison with optic disc dose for 125I standard COMS plaques. Methods: For this simulation study, an in‐house brachytherapy dose calculation program was developed using MATLAB software by incorporating the American Association of Physicists in Medicine Task Group‐43 Update (AAPM TG‐43U1) dosimetry formalism with a line source approximation in a homogeneous water medium and COMS seed coordinates in the AAPM TG 129. Using this program, optic disc doses for standard COMS plaques (from 12 to 22 mm in diameter in 2 mm increments) and notched COMS plaques with one seed removed (Case #1, from 12 to 22 mm) and with two seeds removed (Case #2, from 14 to 22 mm) were calculated as a function of tumor margin‐to‐optic disc distance (DT) for various tumor basal dimensions (BDs) for prescription depths from 1 to 10 mm in 1 mm intervals. A dose of 85 Gy for an irradiation time of 168 h was prescribed to each prescription depth. Then absolute and relative optic disc dose reduction by notched COMS plaques (Cases #1 and #2) was calculated for all prescription depths. Results: Optic disc dose reduction by notched COMS plaques (Cases #1 and #2) had five unique trends related to maximum optic disc dose reduction and corresponding optimal DT for each BD in each plaque. It increased with increasing prescription depth. Conclusions: The results presented in this study would enable the clinician to choose an adequate plaque type among standard and notched 125I COMS plaques and a prescription depth to minimize optic disc dose. 37. Copper(II) complexes of C-meso-1,5,8,12-tetramethyl-1,4,8,11-tetraazacyclotetradecane: syntheses, structures and properties †>

Abstract

A series of copper(II) complexes of C-meso-1,5,8,12-tetramethyl-1,4,8,11-tetraazacyclotetradecane L1 have been synthesized and characterized. The crystal structure of [CuL1(ClO4)]ClO4 1 showed the macrocyclic ligand in the unstable trans-II configuration, while those of [CuL1(N3)2] 2 and [CuL1(H2O)2]X2 (X = NO2 3, Cl 4, Br 5 or I 6) showed the macrocyclic ligand to be in the stable trans-III configuration. The metal ion is in a tetragonally elongated octahedral geometry in all the complexes. In 1 one of the perchlorate groups is free while the other bridges two metal ions with two of its oxygen atoms thus forming a linear chain, while in 2 the axial positions are occupied by the nitrogen atoms of two azide groups. The complexes 3–6 are isomorphous. The electronic spectral and the redox properties of the complexes are discussed. The acid dissociation kinetics of 1 has been studied in the temperature range 25.0 ± 0.1 to 45 ± 0.1 °C in aqueous HNO3–NaNO3 solutions (I = 5.0 M). The complex exhibited a first order dependence on the hydrogen ion concentration with kobs = k1H[H+] + k1d where k1H = 2.66 × 10−5 M−1 s−1 and k1d = 5.54 × 10−5 s−1 at 25.0 ± 0.1 °C. The temperature dependence of the rate constant for the reaction gave ΔH ‡1H = 64.7 kJ mol−1 and ΔS ‡1H = −117 J K−1 mol−1 for k1H while for k1d the parameters are ΔH ‡1d = 27.4 kJ mol−1 and ΔS ‡1d = −234 J K−1 mol−1. A possible mechanism of the reaction is discussed.

Abstract

Background: It has been reported that proximity of the tumor to the optic disc and macula, and radiation dose to the critical structures are substantial risk factors for vision loss following plaque brachytherapy. However, there is little dosimetry data published on this. In this study, therefore, the relationship between distance from tumor margin and radiation dose to the optic disc and macula in ocular brachytherapy using 125I Collaborative Ocular Melanoma Study (COMS) plaques was comprehensively investigated. From the information, this study aimed to allow for estimation of optic disc dose and macula dose without treatment planning.Methods: An in-house brachytherapy dose calculation program utilizing the American Association of Physicists in Medicine Task Group-43 U1 formalism with a line source approximation in a homogenous water phantom was developed and validated against three commercial treatment planning systems (TPS). Then optic disc dose and macula dose were calculated as a function of distance from tumor margin for various tumor basal dimensions for seven COMS plaques (from 10 mm to 22 mm in 2 mm increments) loaded with commercially available 125I seeds models (IAI-125A, 2301 and I25.S16). A prescribed dose of 85 Gy for an irradiation time of 168 h was normalized to a central-axis depth of 5 mm. Dose conversion factors for each seed model were obtained by taking ratios of total reference air kerma per seed at various prescription depths (from 1 mm to 10 mm in 1 mm intervals) to that at 5 mm.Results: The in-house program demonstrated relatively similar accuracy to commercial TPS. Optic disc dose and macula dose decreased as distance from tumor margin and tumor basal dimension increased. Dose conversion factors increased with increasing prescription depth. There existed dose variations (<8%) among three 125I seed models. Optic disc dose and macula dose for each COMS plaque and for each seed model are presented in a figure format. Dose conversion factors for each seed model are presented in a tabular format.Conclusions: The data provided in this study would enable clinicians in any clinic using 125I COMS plaques to estimate optic disc dose and macula dose without dose calculations. Abstract

On August 7th, 2009, the typhoon named Morakot hit Taiwan. The extreme amount of rain triggered enormous mudslides and severe flooding throughout the southern part and one of the mudslides buried the entire town of Xiaolin, a village built by indigenous peoples, killing an estimated over 400 people in the village alone. Most of the victims are minorities. What happened in Xiaolin is more than a natural disaster. Similar to the 1995 Chicago Heat Wave, it is also a social drama that makes a series of conditions to stand out that are always present but hardly to perceive by the society. The purpose of this roundtable discussion thus moves beyond the consequences of natural disasters. The context of provides an opportunity to uncover hidden social problems. One of the most salient problems is that the minority groups are particularly vulnerable to natural and human disasters due to limited access to resources and information. [ABSTRACT FROM AUTHOR]
