Eighty-six edentulous patients provided with Brånemark implants were randomly selected for fixed prostheses with modified framework designs. This study describes alternative laboratory techniques in which premachined titanium components are welded together to form the framework. Clinical experience, following the patients for 1 year after placement, indicates that it is a predictable technique with a similar pattern of complications as experienced by patients with cast frameworks supported by implants. Fiberoptic intubation for a difficult airway requires significant experience. Traditionally only normal airways were available for high fidelity bronchoscopy simulators. It is not clear if training on difficult airways offers an advantage over training on normal airways. This study investigates the added value of difficult airway scenarios during virtual reality fiberoptic intubation training. A prospective multicentric randomized study was conducted 2019 to 2020, among 86 inexperienced anesthesia residents, fellows and staff. Two groups were compared: Group N (control, n = 43) first trained on a normal airway and Group D (n = 43) first trained on a normal, followed by three difficult airways. All were then tested by comparing their ORSIM® scores on 5 scenarios (1 normal and 4 difficult airways). The final evaluation ORSIM® score for the normal airway testing scenario was significantly higher for group N than group D: median score 76% (IQR 56.5-90) versus 58% (IQR 51.5-69, p = 0.0039), but there was no difference in ORSIM® scores for the difficult intubation testing scenarios. A single exposure to each of 3 different difficult airway scenarios did not lead to better fiberoptic intubation skills on previously unseen difficult airways, when compared to multiple exposures to a normal airway scenario. This finding may be due to the learning curve of approximately 5-10 exposures to a specific airway scenario required to reach proficiency.

Weaning success of pond-cultured pikeperch and wild-caught perch (mean length 51 and 48 mm respectively) was evaluated using different weaning techniques and different formulated feeds. Juveniles that were fed formulated feed grew as well as or better than juveniles that were weaned successively using zooplankton or yolk. Four different formulated feeds (agglomerated marine larvae feed, marine larvae feed, trout feed and a semi-moist feed) were evaluated regarding specific growth rate (SGR), condition factor and a subjective stomach fullness estimate. The agglomerated marine larvae feed gave significantly better weaning performance than the other feeds regarding all parameters (SGR = 7.3% day[sup -1] and 3.4% day[sup -1] for pikeperch and perch respectively). The distribution and the genetic similarity of Prevotella intermedia and Prevotella nigrescens in saliva and in subgingival samples recovered from the same subject were studied in 16 subjects with different periodontal status. The isolates (4 salivary and 4 subgingival P. intermedia/nigrescens group isolates per subject) were identified to species level by hybridization with species-specific oligonucleotide probes, and the clonal analysis was performed using arbitrarily primed polymerase chain reaction (AP-PCR) (all isolates) and ribotyping (isolates from 5 subjects). In addition, the applicability of AP-PCR in differentiating between P. intermedia and P. nigrescens species was tested using 18 P. intermedia and 20 P. nigrescens isolates from 34 subjects. P. intermedia was detected in 7 and P. nigrescens in 14 of the 16 subjects. In all subjects the same species was found both in saliva and in subgingival plaque. In 15 of the 16 subjects, similar AP-PCR types of P. intermedia and/or P. nigrescens between salivary and subgingival samples were found. The salivary and subgingival isolates that were similar by AP-PCR were indistinguishable also by ribotyping. The AP-PCR analysis revealed a P. intermedia or P. nigrescens species-specific AP-PCR product in most isolates. This study indicates that both P. intermedia and P. nigrescens were found both in salivary and in subgingival samples, and both sampling sites within the same individual were usually colonized with identical AP-PCR types of the species. Thus, in addition to a subgingival sample a salivary sample seems to be suitable for detection and clonal analysis of these species. A total of 344 Prevotella intermedia and nigrescens group isolates from 59 subjects were identified by hybridization with nonradioactively labeled species-specific oligonucleotide probes. Identification of 20 P. intermedia and 46 P. nigrescens isolates was confirmed by analyzing the electrophoretic mobilities of malate and glutamate dehydrogenase enzymes. A total of 111 isolates (32%) were identified as P. intermedia and 233 isolates (68%) as P. nigrescens. Identification performed with oligonucleotide probes and with malate and glutamate dehydrogenase electrophoresis matched perfectly. The distribution of oral P. intermedia and P. nigrescens in various periodontal status groups was investigated in periodontally healthy or diseased individuals. To reveal intra- and interindividual genetic diversity and possible intrafamilial transmission, P. intermedia and P. nigrescens isolates from 16 to 59 subjects, representing 8 married couples, were ribotyped. The stability of colonization was examined in 12 of the 59 subjects, of whom 6 received periodontal treatment and 6 were untreated. All children and periodontally healthy adults and most subjects with initial periodontitis (13/21) harbored only P. nigrescens. Of the 20 subjects with advanced periodontitis, 7 harbored both P. intermedia and P. nigrescens, 7 only P. intermedia and 6 only P. nigrescens. One or two ribotypes of P. intermedia and/or P. nigrescens were found intraindividually. The spouses in 5 of the 8 married couples shared an identical ribotype of P. intermedia or P. nigrescens, whereas ribotypes from unrelated subjects were mostly unique. This study reports a genetic characterization of Actinobacillus actinomycetemcomitans strains in relation to serotypes by using rRNA gene restriction patterns. Eighty-eight clinical strains were isolated from 20 unrelated subjects at one or several occasions. The strains were serotyped by using serotype-specific rabbit antisera against serotypes a, b, c, d or e. Three subjects harbored 2 A. actinomycetemcomitans serotypes, 15 subjects 1 serotype and 2 subjects untypable strains. Chromosomal DNA was digested with restriction endonuclease Cla I, BamHI, BglI or HindIII and hybridized to the rrnB ribosomal RNA operon of the Escherichia coli chromosome. Isolates belonging to the same serotype were genetically identical in the same individual but nonidentical if they belonged to different serotypes. Isolates of the same or different serotypes were genetically nonidentical in different individuals. The banding patterns of A. actinomycetemcomitans isolates recovered from the same individuals during several years always remained identical. The hybridization method using pKK3535 as a probe seemed suitable as an epidemiological tool for comparing the clonal identity of A. actinomycetemcomitans strains. [ABSTRACT FROM AUTHOR]

Two Tanzanian patients with konzo were severely disabled by a non-progressive spastic paraparesis, since the sudden onset during an epidemic six years earlier. At the time of onset they had a high dietary intake of cyanide from exclusive consumption of insufficiently processed bitter cassava roots. MRI of brain and spinal cord were normal but motor evoked potentials on magnetic brain stimulation were absent, even in the only slightly affected upper limbs. Other neurophysiological investigations were largely normal but the more affected patient had central visual field defects. Meloxicam 15 mg once daily (n = 128) was compared with piroxicam 20 mg (n = 127) in this 6 week, double-blind, parallel-group, randomized, multicentre study in out-patients with symptomatic osteoarthritis (OA) of the hip. Assessments of pain, global efficacy and global tolerance were made on a 10 cm horizontal visual analogue scale; severity of OA was evaluated by Lequesne's index. Efficacy results showed significant improvement compared with baseline, with no significant difference between meloxicam 15 mg and piroxicam 20 mg. The type and frequency of adverse events were comparable for the two drugs. The most frequent events reported were gastrointestinal (GI) disorders, occurring in 21 and 23% of meloxicam and piroxicam patients respectively. The global tolerance assessment by patients at the end of treatment favoured meloxicam. In conclusion, meloxicam at a dose of 15 mg/day is comparable in efficacy and safety to piroxicam 20 mg.
