This essay examines various aspects of alienation in Karl Ove Knausgaard's autobiographical novel My Struggle and its cultural contexts. The overall purpose of the essay is twofold. The aim is to analyze both the depiction and the causes of alienation in My struggle, and to examine how the cultural contexts contribute to the author Knausgaard's experience of alienation. To accomplish this the analysis is divided into two sections. The first part focuses on the literary character Karl Ove and aims to analyze the experience of alienation in the novel. The aim of the second part is, on the other hand, to look at how the cultural context affects the author Knausgaard in an alienating way, mainly how the media landscape and the digitalization of society increases Knausgaard's self-exploitation and the reification of his name. The second part, thereby, reads the novel as a work of autofiction where the author (Knausgaard) and the protagonist (Karl Ove) are treated as one and the same. The theoretical framework of the essay is based upon Rahel Jaeggi's and Hartmut Rosa's definitions of alienation. Altogether, the analysis indicates multiple examples of alienation in My Struggle. The protagonist, Karl Ove, is indifferent to the people and the world around him, and he feels as if he has lost control over his own life. Karl Ove is also unable to combine his two most important social roles in life – as a father and as a writer. Consequently, this leads to an internal division between his conflicting ideals and gender notions. Furthermore, Karl Ove is described as a rootless and formless subject, who is alienated from time and space. Another form of alienation occurs when Knausgaard becomes famous, and his authorship is reified. The reification of Knausgaard's name and face implies that he, as a commodified subject, is linked to the trademark Knausgaard™ over which he has limited control. Generative Artificial Intelligence (AI) in education has been a hot topic since the release of ChatGPT in late 2022. Schools worldwide have had to adapt to the use of the new technology because of the fear of their students using it to cheat. The Swedish authority, Skolverket, has released guidelines to help schools manage the use of generative AI. The guidelines include eliminating the use of graded essays, unless the teacher can ensure it was written without the use of generative AI. Opinions surrounding AI and especially the use of generative AI in schools are divided because of the challenges and opportunities it presents. This includes adapting learning to fit each student and supporting teachers in their work. However, it also means students are able to use generative AI for cheating, which currently is nearly impossible to prove as the traditional tools used for detecting plagiarism cannot detect AI-generated text. Multiple studies point to having a clear set of rules surrounding the use of generative AI, as well as teachers' knowledge and competence surrounding AI, as two of the most important aspects of a successful implementation of the technology in schools. In this paper we explore how generative AI is used at Swedish high schools today and its effect on student learning by answering the questions: What do teachers think of the use of generative AI in schools? How have they adapted their teaching and what measures have they taken? What consequences can be seen as a result of this technology?

Background: Intestinal protozoa are a major causative agent of gastrointestinal problems in humans and animals. They are detected worldwide, mainly in developing countries where environmental sanitation is inadequate. Infections can affect people of all ages, but children are particularly vulnerable. Here we investigated intestinal protozoa and bacteria in Mozambique.Methods: We analysed the occurence of Giardia intestinalis and other intestinal parasites in stool samples from 291 children under five years of age, by microscopy. Most of the samples (279) were also analysed by multiplex PCR for the occurrence of intestinal parasites and common bacteria. Positive Giardia samples were further subtyped Results: G. intestinalis was detected in 72.2% (n=210/291) of study participants. Assemblage identification was possible in 45 cases by PCR. We identified assemblages; A and B, and four sub-assemblages.Sub-assemblage AI, which may have zoonotic potential, was identified in 37.8% of the analysed samples,. Enteroaggregative E. coli (EAEC) was detected in 77.8% of the faeces samples analysed. PCR was more sensitive for parasite detection than microscopy. Conclusions: Our results indicated that microscopic diagnosis is likely sufficient for identifying symptomatic intestinal infections.The presence of G. intestinalis AI warrants special attention in future studies, and the attention of the Mozambique public health authority.

Monoclonal antibodies (mAbs) are an important treatment option for COVID-19 caused by SARS-CoV-2, especially in immunosuppressed patients. However, this treatment option can become ineffective due to mutations in the SARS-CoV-2 genome, mainly in the receptor binding domain (RBD) of the spike (S) protein. In the present study, 7950 SARS-CoV-2 positive samples from the Uppsala and Örebro regions of central Sweden, collected between March 2022 and May 2023, were whole-genome sequenced using amplicon-based sequencing methods on Oxford Nanopore GridION, Illumina MiSeq, Illumina HiSeq, or MGI DNBSEQ-G400 instruments. Pango lineages were determined and all single nucleotide polymorphism (SNP) mutations that occurred in these samples were identified. We found that the dominant sublineages changed over time, and mutations conferring resistance to currently available mAbs became common. Notable ones are R346T and K444T mutations in the RBD that confer significant resistance against tixagevimab and cilgavimab mAbs. Further, mutations conferring a high-fold resistance to bebtelovimab, such as the K444T and V445P mutations, were also observed in the samples. This study highlights that resistance mutations have over time rendered currently available mAbs ineffective against SARS-CoV-2 in most patients. Therefore, there is a need for continued surveillance of resistance mutations and the development of new mAbs that target more conserved regions of the RBD.

Bakgrund: I DSM-5 (2013) kombinerades de två diagnoserna ’Asperger’s syndrome’ och ’Autistic Disorder’, bland andra, till ’Autismspektrumtillstånd’ (AST) tillsammans med högre kriterier för att uppnå diagnosen. Denna uppsats är avsedd att undersöka utkomsten av denna förändring och dess konsekvenser för dess mottagare av diagnoserna tio år senare i en Svensk vårdkontext och vanliga uppfattningar om diagnosen.Resultat: En tematisk analys (TA) med fyra deltagare resulterade i en fördjupad analys av det praktiska kliniska arbetet kring klassificering av individer med AST som gav insikter om varför en del av oron inför förändringen inte besannats och hur samhället ser på mottagare av diagnosen. Slutsats: Diagnosen i sig är inte den huvudsakliga beståndsdelen i mottagande av bistånd i svensk vårdkontext. En individs funktionshinder ska vara den viktigaste faktorn i detta, och att inte uppnå en lämplig diagnos gör en person undantagen från insatser. Samhället uppfattas inte heller ha nått en nivå av förståelse för vad AST innebär, även om acceptansen för diagnosen verkar har ökat., Background: In DSM-5 (2013) the two diagnoses ‘Asperger’s Syndrome’ and ‘Autistic Disorder’, amongst others, were combined to ‘Autism Spectrum Disorder’ (ASD) along with higher criteria to achieve the diagnose. This paper is intended to investigate what came of this change and its implications for its recipients of the diagnoses ten years later in a Swedish caregiving context and common conceptions of the diagnose.Results: A Thematic Analysis (TA) with four participants resulted in an in-depth analysis of the practical clinical work regarding the classification of individuals with ASD providing insights as to why the change has not had some of the apprehension it entailed and how the society regards recipients of the diagnose. In this also, an investigation of the diagnosis throughout the history of DSM.Conclusion: The diagnose per se is not the main constituent in receiving of aid in the Swedish care context. An individual’s disability is supposed to be the main factor in this but not achieving a suitable diagnose makes one exempt. Society as well has not perceivably reached a level of understanding of what ASD entails though acceptance for it seemingly has increased.

We describe a flight-associated infection scenario of seven individuals with a B.1.617.2 (Delta) lineage, harbouring an S:E484Q point mutation. In Sweden, at least 10% of all positive SARS-CoV-2 samples were sequenced in each county; the B.1.717.2 + S:E484Q combination was not detected in Sweden before and was imported within the scenario described in this report. The high transmission rate of the delta lineage combined with the S:E484Q mutation, associated with immune escape in other lineages, makes this specific genetic combination a possible threat to the global fight against the COVID-19 pandemic. Even within the Omicron wave, the B.1.617.2 + S:E484Q variant appeared in community samples in Sweden, as it seems that this combination has an evolutionary gain compared to other B.1.617.2 lineages. The here described genomic combination was not detectable with the common fasta file-based Pango-lineage analysis, hence increasing the probability of the true global prevalence to be higher.

Background: Giardia and Cryptosporidium species are significant zoonotic parasites of humans and domesticated animals. Objectives: The study aimed to determine the prevalence of Giardia and Cryptosporidium in livestock and dogs of the Magude District. Method: The flotation technique (Willis), modified Ziehl-Neelsen (mZN) and direct and indirect immunofluorescence (DIF and IIF) techniques were applied to determine the prevalence of Giardia and Cryptosporidium species in faecal samples of dog pups (156), goat kids (60) and calves (480) from the Magude District of Mozambique from February to September 2015. Results: Using Willis, IIF and DIF, the prevalence of Giardia in calves was 0%, 8.1%, and 6.0%; in dogs 0.6%, 8.3% and 5.7% and for goats 0% and 13.3% (IIF was not performed), respectively. The prevalence of Cryptosporidium in calves using Willis, mZN, IIF and DIF was 0%, 3.8%, 4.7% and 0.4% and in dogs 0%, 0.6%, 6.4% and 0.6%, respectively. The parasite was not detected in goats. Conclusion: Results from the present study showed that IIF performed better diagnosis of Giardia and Cryptosporidium, and that the mZN can be used as an alternative for Cryptosporidium because of the high cost of IIF. There is a need for identification of genotypes or subtypes of these parasites through application of molecular techniques in order to determine their zoonotic potential, and we advocate a 'one health' approach in the control and prevention of these parasites.

We investigated sick leave from work, studies, preschool, and kindergarten occurring between 1 November 2010 and 31 January 2011 and associated with a waterborne outbreak of diarrhea caused by Cryptosporidium hominis in late November 2010 in Ostersund, Sweden with 45.2% of 60,000 residents being symptomatic. A questionnaire defining acute watery diarrhoea and/or >= 3 diarrhea episodes/day as cryptosporidiosis was sent to 1,508 residents in late January 2011 (response rate 69.2%). Among adults aged 18-60 years, 24.0% took sick leave for a mean of 4.6 (SD +/- 4.0) days due to cryptosporidiosis, and an additional 10.6% were absent from work a mean of 4.0 (+/- 2.2) days to care for symptomatic children. Among children (aged <= 17 years), 35.0% stayed home sick from kindergarten/preschool or school/university for a mean of 5.2 (+/- 3.8) days resulting in 5.1 (+/- 4.4) days of absence from work per sick child shared between parents/guardians. The estimated total number of sick leave days was 50,000 for adults and 20,700 for children, with an estimated direct cost of (sic) 7 million for employers. The potential impact on society of sick leave caused by waterborne diseases must be considered in decisions regarding the quality of drinking water.

Background: In the winter and spring of 2011 a large outbreak of cryptosporidiosis occurred in Skelleftea municipality, Sweden. This study summarizes the outbreak investigation in terms of outbreak size, duration, clinical characteristics, possible source(s) and the potential for earlier detection using calls to a health advice line. Methods: The investigation included two epidemiological questionnaires and microbial analysis of samples from patients, water and other environmental sources. In addition, a retrospective study based on phone calls to a health advice line was performed by comparing patterns of phone calls between different water distribution areas. Results: Our analyses showed that approximately 18,500 individuals were affected by a waterborne outbreak of cryptosporidiosis in Skelleftea in 2011. This makes it the second largest outbreak of cryptosporidiosis in Europe to date. Cryptosporidium hominis oocysts of subtype IbA10G2 were found in patient and sewage samples, but not in raw water or in drinking water, and the initial contamination source could not be determined. The outbreak went unnoticed to authorities for several months. The analysis of the calls to the health advice line provides strong indications early in the outbreak that it was linked to a particular water treatment plant. Conclusions: We conclude that an earlier detection of the outbreak by linking calls to a health advice line to water distribution areas could have limited the outbreak substantially.

The diagnosis of neurocysticercosis (NCC) requires expensive neuroimaging techniques that are seldom affordable for people in endemic countries. Accordingly, there is a need for new low-cost diagnostic methods that offer high sensitivity and specificity. In this study, we evaluated Western blot analysis of the previously described recombinant antigen Tsol-p27 in relation to a commercial or in-house enzyme-linked immunosorbent assay (ELISA) for NCC, and compared the results with those provided by a commercial enzyme-linked immunoelectrotransfer blot (EITB) assay, which was regarded as the reference standard method. The analysed serum samples were obtained from 165 people, 18 of whom were confirmed to be NCC positive by EITB. Comparing our Western blot analysis of Tsol-p27 with a previous evaluation performed in Central America showed similar specificity (96.69% versus 97.8%) and sensitivity (85.71% versus 86.7%). The present results indicate that the recombinant Tsol-p27 antigen provides good sensitivity and specificity, and might be preferable as a diagnostic antigen in poorly equipped laboratories in endemic countries.

Background: In 2010-2011, two large waterborne outbreaks caused by Cryptosporidium hominis affected two cities in Sweden, Ostersund and Skelleftea. We investigated potential post-infection health consequences in people who had reported symptoms compatible with cryptosporidiosis during the outbreaks using questionnaires. Methods: We compared cases linked to these outbreaks with non-cases in terms of symptoms present up to eleven months after the initial infection. We examined if cases were more likely to report a list of symptoms at follow-up than non-cases, calculating odds ratios (OR) and 95 % confidence intervals (CI) obtained through logistic regression. Results: A total of 872 (310 cases) and 743 (149 cases) individuals responded to the follow-up questionnaires in Ostersund and Skelleftea respectively. Outbreak cases were more likely to report diarrhea (Ostersund OR: 3.3, CI: 2.0-5.3. Skelleftea OR: 3.6, CI: 2.0-6.6), watery diarrhea (Ostersund OR: 3.4, CI: 1.9-6.3. Skelleftea OR: 2.8, CI: 1.5-5.1) abdominal pain (Ostersund OR: 2.1, CI: 1.4-3.3, Skelleftea OR: 2.7, CI: 1.5-4.6) and joint pain (Ostersund OR: 2.0, CI: 1.2-3.3, Skelleftea OR: 2.0, CI: 1.1-3.6) at follow-up compared to non-cases. Conclusions: Our findings suggest that gastrointestinal-and joint symptoms can persist several months after the initial infection with Cryptosporidium and should be regarded as a potential cause of unexplained symptoms in people who have suffered from the infection.

Commercial antigens used to diagnose human neurocysticercosis are obtained from either a soluble parasite extract or a parasite-derived glycoprotein fraction. The aim of the present study was to identify antigenic proteins as potential diagnostic candidates in Mozambique. Soluble proteins from Taenia solium cysticerci were separated by two-dimensional electrophoresis and blotted onto nitrocellulose membranes. Subtracted hybridization was performed with serum samples obtained from patients with neurocysticercosis (NCC) and from a NCC-negative control group. Six antigenic proteins were identified and sequenced by liquid chromatography-mass spectrometry. Among these we found Tsol-p27, which was previously identified as a diagnostic candidate in a study conducted in Nicaragua, Central America. Here, we evaluated Tsol-p27 and the antigen cC1 as potential recombinant diagnostic reagents, and also investigated the localization and partial function of Tsol-p27. Immunoblotting demonstrated that Tsol-p27 was recognized by all 10 serum samples from NCC-positive individuals, whereas cC1 was identified by only five of the 10 positive sera. None of the antigens were recognized by negative control sera. Despite the limited number of serum samples evaluated in this study, the results suggest that Tsol-p27 can be a suitable candidate for diagnosis of human NCC, not only in Central America but also in sub-Saharan Africa.

Background: The protozoan parasite Giardia intestinalis and the pathogenic bacterium Helicobacter pylori are well known for their high prevalences in human hosts worldwide. The prevalence of both organisms is known to peak in densely populated, low resource settings and children are infected early in life. Different Giardia genotypes/assemblages have been associated with different symptoms and H. pylori with induction of cancer. Despite this, not much data are available from sub-Saharan Africa with regards to the prevalence of different G. intestinalis assemblages and their potential association with H. pylori infections. Methodology/Principal Findings: Fecal samples from 427 apparently healthy children, 0-12 years of age, living in urban Kampala, Uganda were analyzed for the presence of H. pylori and G. intestinalis. G. intestinalis was found in 86 (20.1%) out of the children and children age 1<5 years had the highest rates of colonization. H. pylori was found in 189 (44.3%) out of the 427 children and there was a 3-fold higher risk of concomitant G. intestinalis and H. pylori infections compared to non-concomitant G. intestinalis infection, OR = 2.9 (1.7-4.8). No significant association was found in the studied population with regard to the presence of Giardia and gender, type of toilet, source of drinking water or type of housing. A panel of 45 G. intestinalis positive samples was further analyzed using multi-locus genotyping (MLG) on three loci, combined with assemblage-specific analyses. Giardia MLG analysis yielded a total of five assemblage AII, 25 assemblage B, and four mixed assemblage infections. The assemblage B isolates were highly genetically variable but no significant association was found between Giardia assemblage type and H. pylori infection. Network administrators typically employ di erent methods for authenticating and authorizing the access to their networks. A exible and scalable network access method is needed to combat the ever increasing network ubiquity brought on by technological advancements. The IEEE 802.1x Port-Based Network Access is a technology that allows transparent authentication to a network. It uses EAP-methods in order to authenticate against a server. There are a lot of di erent EAP-methods to choose from, and they vary in complexity and security. This report will bring up the di erences between the most commonly used authentication methods regarding the authentication time depending on di erent delay and network load. Results showed that EAP-methods that are less complex take less time to perform authentication than their counterparts. When there is no delay, or a very small delay, this might not matter, but when the delay is higher complex EAP-methods take signi cantly longer time to perform the authentication process. This is very negative considering the nature of transparent authentication, and could lead to users becoming annoyed. A general formula for determining how long time an EAP-authentication process will take is presented.
