Abstract

Insulin regulation and diabetes mellitus are complex topics that involve a variety of biological processes and environmental factors. One important factor that has been implicated in both insulin regulation and the development of diabetes mellitus is vitamin D. Vitamin D is a fat-soluble vitamin that is produced by the body in response to sunlight exposure, and it is also found in certain foods. Studies have shown that vitamin D deficiency is associated with an increased risk of developing type 2 diabetes mellitus, which is a chronic metabolic disorder characterized by elevated blood sugar levels. Vitamin D is thought to play a role in insulin sensitivity and secretion, and it may also help to reduce inflammation in the body, which is a risk factor for diabetes mellitus. In this context, understanding the role of vitamin D in insulin regulation and diabetes mellitus is important for the development of strategies to prevent and treat this condition. In this regard, research has shown that vitamin D supplementation may improve insulin sensitivity and glucose metabolism in individuals with type 2 diabetes mellitus. Overall, a better understanding of the relationship between vitamin D and diabetes mellitus may lead to new therapeutic approaches to manage this condition. There is some evidence to suggest that there may be a correlation between vitamin D levels and blood glucose levels in individuals with and without diabetes mellitus. Abstract

Changes in stature reflect the creep behaviour of the intervertebral discs when loaded and unloaded. Running, weight-training and bounding are exercises which load the spine and cause a loss in stature. Gravity-facilitated traction (inversion) has been shown to unload the spine and increase stature. This study examined the use of a gravity-inverted posture for attenuating the shrinkage induced by a bounding exercise regimen. Eight male subjects, aged 20–26, performed the experimental protocol twice, at the same time of day (14·00 hours) with an alteration only to their 10min pre-exercise behaviour. The first occasion involved standing and the second involved gravity inversion at an angle of 50 degrees to the vertical. Prior to testing, each subject stood for a period of 20 min. Measurements of stature were taken at the beginning of the experiment, before and after the pre-exercise treatments, after the exercise period and at 5 min intervals during a 20 min standing recovery. A stadiometer, accurate to within 0·5 mm, was used to record alterations in stature. The exercise regimen consisted often sets of five standing broad jumps with 15 s recovery between each set. Standing pre-exercise caused little change in stature, whereas, gravity inversion caused a mean increase in stature of 2·7mm (p<0·001). Exercise caused a mean shrinkage of 1·7 mm and 3·5 mm when it followed standing and gravity inversion, respectively (p<0·05). 24. Spinal shrinkage in females: circadian variation and the effects of circuit weight-training.

Abstract

Compressive loading of the spine leads to spinal shrinkage and a loss of stature. The aim of this study was to measure the circadian variation in the stature of females and to examine differential effects of spinal loading with the time of day. Ten females aged 20–30 years were studied in nine measurements on a 24-hour period. Between rising and retiring to bed, the mean peak-to-trough variation was 15·4 mm (0·92% of stature). Subjects underwent two sequences of 20 min circuit-training with weights, followed by 20 min reclining in Fowler's position and 20 min standing erect, these being first thing in the morning after 7 hours of sleep and before retiring at midnight. Mean loss of height from circuit-training was 5·4 mm in the morning and 4·3 mm in the evening (P < 0·001): mean height regained in Fowler's position was 4·5 mm and 3·4 mm respectively (P<0·05). There were significant height losses in the subsequent standing posture but no time of day effect. Height losses with exercise were related to the isometric strength of the back muscles and to the perceived exertion. Results support the hypothesis that height losses from a given spinal load will be of smaller amplitude when body height is in the trough of circadian variation. [ABSTRACT FROM PUBLISHER]

Abstract

Spinal shrinkage, measured by changes in stature, is used as an index of spinal loading as alterations reflect changes in intervertebral disc height. Shrinkage induced by various physical activities may be reversed using gravity inversion. The present purpose was to examine the shrinkage induced by a drop jumping regimen and evaluate gravity inversion post-exercise. Eight males, aged 20–31, performed two separate experimental protocols, each on different dates at 1400 h. Subjects stood for 30 min before undertaking an exercise regimen, consisting of five sets of five drop jumps from a height of 1 m, rebounding over a hurdle 0.5 m high. For 20 min, directly following the exercise regimen, subjects on one occasion stood and on a second occasion undertook gravity inversion. Shrinkage was monitored for 40 min after this post-exercise treatment. The stadiometer used to measure shrinkage was accurate to 0.05 mm. The exercise regimen caused a mean shrinkage of 1.68 and 1.81 mm for the two testing sessions. Post-exercise inversion and standing for 20 min increased stature by 5.18 and 0.76 mm, respectively (P< 0.01). The 40-min standing period following inversion caused a rapid loss in stature (4.07 mm). At 30 min into this recovery period, there was no significant difference in shrinkage for either of the regimens. Results suggest that effects of an inversion treatment are short-lasting.

28. Measurement of transcutaneous oxygen pressure in normal and ischaemic skin

Abstract

The transcutaneous oxygen pressure (tcPO2) was measured by a polarographic technique in the legs of 161 volunteers and compared with the levels found in 62 patients with ischaemic skin due to peripheral vascular disease. The results show that the tcPO2 was related to the degree of ischaemia and, in many cases, was a more accurate guide to the viability of the skin than clinical assessment. Measurement of the transcutaneous oxygen pressure in the leg at the site of amputation in 24 patients with peripheral vascular disease showed that a preoperative level greater than 40 millimetres of mercury at an electrode temperature of 44 degrees Celsius was necessary for the skin of the stump to heal. The technique is simple, non-invasive and reliable. The tcPO2 accurately reflects the physiological and pathological changes in the circulation of the skin. It has potential in many fields of surgery where careful assessment of the viability of the skin is necessary.
