Purpose: To compare the long-term ocular findings of children that were operated of congenital cataract before the age of two and that received an intraoperative intracameral triamcinolone injection or used postoperative oral prednisolone to modulate ocular inflammation. Methods: All patients who had previously participated in a clinical trial that analyzed the 1-year surgical outcomes of congenital cataract surgery utilizing intracameral triamcinolone (study group) or oral prednisolone (control group) were eligible to participate in this prospective cohort research. Patients’ medical records were reviewed, and the children underwent a complete ophthalmologic exam on final follow-up. Biomicroscopic findings, intraocular pressure, central corneal thickness, the need for additional surgical interventions, and findings compatible with glaucoma were the primary end measures. Results: Twenty-six eyes (26 patients) were included (study group = 11 eyes; control group = 15 eyes). The mean follow--up was 8.2 ± 1.2 years and 8.1 ± 1.7 years in the study and control groups, respectively (p=0.82). All eyes presented a centered intraocular lens. There was no statistically significant difference between the groups with regards to the presence of posterior synechia (p=0.56), intraocular pressure (p=0.49), or central corneal thickness (p=0.21). None of the eyes fulfilled the glaucoma diagnostic criteria, presented secondary visual axis obscuration, or were reoperated. Conclusion: The long--term ocular findings of children that underwent congenital cataract surgery and received an intraoperative intracameral triamcinolone injection were similar to those that used postoperative oral prednisolone to modulate ocular inflammation. This suggests that intracameral triamcinolone may substitute oral prednisolone in congenital cataract surgery, facilitating the postoperative treatment regimen and compliance. RESUMO Objetivo: Comparar os achados oculares em longo prazo de crianças que se submeteram à cirurgia de catarata congênita antes dos dois anos de idade e receberam uma injeção intracameral de triancinolona no intraoperatório ou usaram prednisolona oral no pós-operatório para modular a inflamação ocular. Métodos: Neste estudo prospectivo de coorte, todos os pacientes que participaram de um ensaio clínico anterior, que analisou os resultados cirúrgicos de 1 ano da cirurgia de catarata congênita usando triancinolona intracameral (Grupo de Estudo) ou prednisolona oral (Grupo Controle), eram elegíveis para participar. Os prontuários médicos dos pacientes foram revisados e as crianças foram submetidas a um exame oftalmológico completo no acompanhamento final. As principais medidas de desfecho foram: achados biomicroscópicos, pressão intraocular, espessura central da córnea, a necessidade de intervenções cirúrgicas adicionais e achados compatíveis com glaucoma. Resultados: Vinte e seis olhos (26 pacientes) foram incluídos (Grupo de Estudo = 11 olhos; Grupo de Controle = 15 olhos). O seguimento médio foi de 8,2 ± 1,2 anos e 8,1 ± 1,7 anos nos Grupos de Estudo e Controle, respectivamente (p=0,82). Todos os olhos apresentavam lente intraocular centrada. Não houve diferença estatisticamente significativa entre os grupos com relação à presença de sinéquia posterior (p=0,56), pressão intraocular (p=0,49) ou espessura central da córnea (p=0,21). Nenhum dos olhos preencheu os critérios diagnósticos para glaucoma, apresentou opacificação secundária do eixo visual ou foi reoperado. Conclusão: Os achados oculares em longo prazo de crianças que se submeteram à cirurgia de catarata congênita e receberam uma injeção intracameral de triancinolona no intraoperatório foram semelhantes aos que usaram prednisolona oral no pós-operatório para modular a inflamação ocular, sugerindo que a triancinolona intracameral pode substituir a prednisolona oral na cirurgia de catarata congênita, facilitando o tratamento pós-operatório e a adesão ao mesmo.

Orientador: Carlos Eduardo Leite Arieta Tese (doutorado) - Universidade Estadual de Campinas, Faculdade de Ciencias Medicas Resumo: O objetivo principal deste estudo foi investigar se os exames complementares sistêmicos realizados rotineiramente, antes da cirurgia de catarata, com anestesia local, reduzem a taxa de complicações clinicas durante o período perioperatório em adultos. O estudo foi desenvolvido no Hospital de Clinicas da Universidade Estadual de Campinas Unicamp, Campinas, São Paulo, Brasil, entre 10 de fevereiro de 2000 e 10 de janeiro de 2001. As cirurgias de catarata foram aleatoriamente selecionadas para serem precedidas por uma bateria de testes pré-operatórios (grupo de testes rotineiros) ou não precedidas por testes rotineiros (grupo de testes seletivos). Se o paciente foi sorteado para o grupo de testes seletivos, foi solicitado que nenhum teste pré-operatório fosse executado a menos que o paciente apresentasse um novo problema médico, ou piora de uma doença preexistente, a qual requeresse os testes independentemente da realização da cirurgia. Para pacientes sorteados para o grupo de testes rotineiros, foram solicitados: eletrocardiograma, dosagem de hemoglobina e glicemia de jejum. A amostra de 1.025 pacientes programada para ser submetida à cirurgia de catarata incluiu 513 sorteados para o grupo de testes seletivos e 512 sorteados para o grupo de testes rotineiros. Os dois grupos tiveram proporções semelhantes de operações que foram canceladas e não foram remarcadas (2% em cada grupo). A :freqüência cumulativa de eventos adversos intraoperatórios foi semelhante nos dois grupos (9,6% no grupo de testes rotineiros e 9,7% no grupo de testes seletivos - p 0,923). Os tipos de eventos adversos foram semelhantes em ambos os grupos. Os resultados deste estudo sugerem que exames subsidiários clínicos rotineiros antes de cirurgia de catarata, sob anestesia local, não reduzem a taxa de complicações clinicas durante o período perioperatório. Uma conduta mais eficiente deve ser pedir apenas testes pré-operatórios indicados pela história clinica ou exame físico do paciente. A adoção dessa conduta pode permitir economia ao sistema de saúde do Brasil, que financia cerca de 250.000 facectomias ao ano Abstract: The objective of this study was to investigate whether routine medical testing before cataract surgery with local anesthesia reduces the rate of complications during the perioperative period in adults. The study was developed in an academic medical center in Brazil, between February 10, 2000 and January 10,2001. The scheduled cataract operation was random1y assigned either not to be preceded by routine medical testing (the selective-testing group) or to be preceded by routine testing (the routine-testing group). If the patient was assigned to the selective-testing group, it was requested that no preoperative testing be performed unless the patient presented with a new or worsening medical problem that would warrant medica! evaluation with testing. For patients assigned to the routine-testing group it was requested that a 12-lead electrocardiogram, measurements of hemoglobin and serum glucose be obtained. The sample of 1025 patients scheduled to undergo cataract surgery inc1uded 513 assigned to the selective-testing group and 512 assigned to the routine-testing group. The two groups had similar proportions of operations that were canceled and not rescheduled (2% in each group). The cumulative rate ofmedical events was similar in the two groups (9.6% in the routine-testing group and 9.7% in the selective-testing group - p 0.923). The types ofmedical events were similar in both groups. The results of this study suggest that routine medical testing before cataract surgery does not reduce the rate of complications during the perioperative period, and a more efficient approach should be only to request preoperative tests indicated by patient history or physical examination. Purpose: To describe microvascular changes in the maculas of individuals with type 2 diabetes observed on optical coherence tomography angiography (OCTA) images. We compared the maculas of diabetic subjects without diabetic retinopathy with those of healthy subjects and correlated the findings with the clinical profiles of diabetic subjects. Methods: One eye each of 30 patients with diabetes and 30 healthy individuals were examined. The patients with diabetes underwent funduscopy, retinography, and fluorescein angiography to rule out retinopathy. All subjects underwent optical coherence tomography angiography of a macular area (6×6 mm2), and the foveal and parafoveal vascular densities were analyzed in the superficial and deep retinal vascular plexus. The foveal and parafoveal thicknesses, foveal avascular zone of the superficial plexus, and choriocapillaris flow area were also examined. The optical coherence tomography angiography results were compared between the two study groups and correlated with the following parameters: visual acuity, time since diabetes diagnosis, glycemic control, lipid profile, and renal function of patients with diabetes. Results: A minimal increase in the choriocapillaris flow area was observed in the patients with diabetes (mean area, 22.3 ± 4.6 mm2 in controls; 22.6 ± 3.9 mm2 in patients with diabetes) (p=0.017). No significant differences were observed between other optical coherence tomography angiography parameters analyzed in the two groups. Glycosylated hemoglobin and fasting blood glucose levels were significantly negatively correlated with the foveal vascular density of both plexuses; conversely, fasting blood glucose levels were positively correlated with the choriocapillaris flow area (p=0.034). The other clinical parameters were not correlated with the optical coherence tomography angiography findings. Conclusion: Optical coherence tomography angiography may not be the most appropriate tool for detecting preclinical changes in patients with diabetes, moreover, optical coherence tomography angiography; does not replace clinical examinations. Glycemic control should be the primary clinical parameter considered during retinopathy screening. Larger studies are necessary to confirm these findings.

RESUMO Objetivo: Descrever alterações microvasculares na mácula em diabéticos do tipo 2 sem retinopatia diabética e pacientes saudáveis, e correlacionar achados com perfil clínico nos diabéticos. Métodos: Foram incluídos 60 olhos de 30 diabéticos e 30 pacientes saudáveis. Diabéticos realizaram fundoscopia, retinografia® (CR2; Canon Inc., New York, New York, USA) e angiografia fluoresceínica® (TRC-50DXC; Topcon Inc., Tokyo, Japan) para descartar a presença de retinopatia. Os 60 pacientes realizaram a angiografia por tomografia de coerência óptica® (RTVue XR, Avanti, Optovue, Fremont, CA, USA) (área macular: 6 x 6 mm2) e foram analisados densidade vascular total, foveal e parafoveal no plexo capilar superficial e plexo capilar profundo, espessura foveal, espessura parafoveal, área da zona avascular da fóvea no plexo capilar superficial e área de fluxo da coriocapilar. Resultados da angiografia por tomografia de coerência óptica foram comparados entre os 2 grupos e correlacionados com acuidade visual, tempo de diabetes, controle glicêmico, perfil lipídico e função renal nos diabéticos. Resultados: Observou-se aumento mínimo da área de fluxo da coriocapilar nos diabéticos, média das áreas foi de 22,3 ± 4,6 mm2 no grupo controle e 22,6 ± 3,9 mm2 em diabéticos (p=0,017). Não foi observada diferença estatisticamente significante entre outras variáveis da angiografia por tomografia de coerência óptica analisadas nos dois grupos. Hemoglobina glicosilada e glicemia de jejum apresentaram correlação negativa estatisticamente significante com densidade vascular foveal de ambos os plexos e a glicemia de jejum se correlacionou positivamente com área de fluxo da coriocapilar (p=0,034). Outros dados clínicos avaliados não apresentaram correlação com achados da angiografia por tomografia de coerência óptica. Conclusão: Resultados sugerem que a angiografia por tomografia de coerência óptica pode não ser a melhor ferramenta na detecção de alterações pré-clínicas em diabéticos, não substituindo o exame clínico, e corroboram a ideia de que o controle glicêmico deve ser o principal parâmetro clínico a ser considerado na triagem da retinopatia. Estudos com amostras maiores são necessários para confirmar os achados.

Purpose: To determine the release profile of moxifloxacin encapsulated in liposomes in the aqueous humor as a controlled release system for intracameral application. Methods: Liposomes containing moxifloxacin were obtained using the lipid film hydration method and were characterized by particle size and encapsulation efficiency. Female rabbits were used for the in vivo profile release study. Liposomes containing moxifloxacin was injected into the anterior chamber of the right eye of each animal. The rabbits were divided into five groups, and a sample of aqueous humor was collected 2, 4, 8, 24, and 48 h after administration of liposomes containing moxifloxacin administration. Moxifloxacin concentrations in the aqueous humor were analyzed using high-performance liquid chromatography. Results: The average size of the liposomes containing moxifloxacin was 60.5 ± 0.72 nm with a particle size distribution of 0.307. The encapsulation efficiency of moxifloxacin in liposomes was 92.24 ± 0.24%. The results of an in vivo release study of liposomes containing moxifloxacin, showed that the maximum moxifloxacin concentration was achieved within the first 2 h after administration (5.27 ± 1.09 mg/mL) and was followed by a decrease in intracameral concentration (0.35 ± 0.05 mg/mL) until the 24 h mark. Conclusions: The in vivo experiments resulted in liposomes containing moxifloxacin that were homogenous in size and exhibited high drug encapsulation efficiency. The results indicate that liposomes containing moxifloxacin offers a satisfactory aqueous humor release profile after intracameral application. RESUMO Objetivo: Determinar o perfil de liberação, no humor aquoso, de moxifloxacino encapsulado em lipossomas como um sistema de liberação controlada para aplicação intracameral. Métodos: Lipossomas contendo moxifloxacino foram obtidos através do método de hidratação do filme lipídico e caracterizados por tamanho da partícula e eficiência de encapsulação. Utilizaram-se coelhos fêmeas foram para o estudo do perfil de liberação in vivo. Lipossomas contendo moxifloxacino foram injetados na câmara anterior do olho direito de cada animal. Os coelhos foram divididos em cinco grupos, e uma amostra de humor aquoso foi coletada 2, 4, 8, 24 e 48 h após a administração de lipossomas contendo moxifloxacino. As concentrações de moxifloxacino no humor aquoso foram analisadas usando cromatografia líquida de alta eficiência. Resultados: O tamanho médio dos lipossomas contendo moxifloxacino foi de 60,5 ± 0,72 nm com uma distribuição de tamanho de partícula de 0,307. A eficiência de encapsulação de moxifloxacino nos lipossomas foi de 92,24 ± 0,24. Os resultados de um estudo de liberação in vivo de lipossomas contendo moxifloxacino, mostraram que a concentração máxima de moxifloxacino foi atingida dentro das primeiras 2 h após sua administração (5,27 ± 1,09 mg/mL) e foi seguida de um decréscimo na concentração intracameral (0,35 ± 0,05 mg/mL) até a marca de 24 h. Conclusão: Os experimentos in vivo resultaram em lipossomas contendo moxifloxacino que eram homogêneos em tamanho e exibiam alta eficiência de encapsulação do fármaco. Os resultados indicam que lipossomas contendo moxifloxacino oferecem um perfil de liberação de humor aquoso satisfatório após a aplicação intracameral.

Purpose: To identify the frequency of ocular diseases among recipients of disability benefits in the metropolitan region of Recife, Brazil. Methods: A review was performed of 217,221 cases of disability benefits granted between 2010 and 2015 by the executive managerial department of the Brazilian National Institute of Social Security (Instituto Nacional do Seguro Social [INSS]) in Recife, which encompasses 14 municipalities of the metropolitan region, including the capital. The frequencies of the identified cases of ocular morbidity were then determined according to their group in the International Classification of Diseases, 10th Revision (ICD-10), their cause, the age, sex, and income of the recipient, and the type and duration of the benefit. Results: Of all disability benefits granted, 5,324 (2.5%) were due to ocular disease, the majority (91.1%) consisting of sick pay. Most of the beneficiaries (64.6%) were males, were 20 to 59 years of age, and 61.2% earned the minimum wage or less. The principal ocular diseases for which sick pay benefits were granted were cataract (24.5%), conjunctivitis (21.1%), and pterygium (8.8%). Blindness and low vision were the principal ocular diseases in cases of accident indemnity and disability retirement. Conclusions: The results highlight the magnitude of the problem of ocular diseases to the social security system, with serious economic and social losses, and emphasize the need for measures aimed at their prevention. Moreover, integration between the national departments of health and social security needs to be improved. RESUMO Objetivo: Identificar a frequência das doenças oculares entre os segurados com benefícios por incapacidade. Métodos: Estudo retrospectivo de 217.221 benefícios por incapacidade concedidos pelo Instituto Nacional do Seguro Social (INSS) da gerência executiva de Recife, que engloba os 14 municípios da região metropolitana, incluindo a capital, no período de 2010-2015. Os casos de doenças oculares foram avaliados quanto à frequência considerando seu grupo na CID-10. Foi feita ainda uma análise do perfil dos benefícios concedidos por estas doenças com relação à causa, idade, sexo, faixa salarial, espécie e duração do benefício. Resultados Do total dos benefícios concedidos, 5.324 foram decorrentes de doenças oftalmológicas (2,5%) e entre estes, a maioria como auxíliodoença (91,1%). Houve predomínio do sexo masculino (64,6%), faixa etária dos 20 aos 59 anos e valor de até 01 salário mínimo (61,2%). Entre as concessões de auxíliodoença a principal causa foi a catarata (24,5%), seguida pela conjuntivite (21,1%) e pterígio (8,8%). A cegueira e baixa visão foi a principal causa de auxílio-acidente e aposentadoria por invalidez. Conclusões: Os resultados destacam a magnitude do problema das doenças oculares para a Previdência Social, com graves perdas econômicas e sociais, e enfatizam a necessidade de medidas que visem sua prevenção e maior integração entre os serviços de saúde e o INSS (Instituto Nacional do Seguro Social).

This case report describes the retinal optical coherence tomography (OCT) findings in a microcephalic infant with macular atrophy presumably caused by intrauterine Zika virus infection. OCT demonstrated atrophy of the outer retinal layers and choriocapillaris, including the outer nuclear layer and ellipsoid zone, associated with retinal pigment epithelium hyper-reflectivity and increased OCT penetration into deeper layers of the choroid and sclera. A major concern associated with this infection is the apparent increased incidence of microcephaly in fetuses born to mothers infected with the Zika virus. It is becoming increasingly difficult to ignore the upsurge in congenital microcephaly observed in Brazil. Recently, ocular findings in infants with microcephaly associated with intrauterine Zika virus infection have been described. This is the first report of OCT imaging of macular atrophy in a child with presumed Zika virus infection-associated microcephaly. Este relato de caso descreve os achados da tomografia de coerência óptica (TCO) da retina de uma criança com microcefalia e atrofia macular presumivelmente causada por infecção intra-uterina pelo vírus Zika. Tomografia de coerência óptica demonstrou camadas externas da retina e atrofia coriocapilar, incluindo a camada nuclear externa e zona de elipsóide, associada a hiperreflectividade do epitélio pigmentar da retina e aumento de penetração da tomografia de coerência óptica em camadas mais profundas da coróide e esclera. Uma grande preocupação associada com esta infecção é o aumento aparente da incidência de microcefalia em fetos nascidos de mães infectadas com o vírus Zika. É cada vez mais difícil ignorar o surto de microcefalia congênita observada no Brasil. Recentemente, foram descritos achados oculares em crianças com microcefalia associados à infecção pelo vírus Zika intra-uterino. Este é o primeiro relato de tomografia de coerência óptica com atrofia macular em uma criança com microcefalia associada a infecção presumida por vírus Zika.

Treatment for this affection remains palliative, so studies on its pathophysiology may contribute to the future development of novel therapies. SCR is more frequently observed in hemoglobin SC disease and derives from vaso-occlusion in the microvasculature of the retina leading to neovascularization and, eventually, to blindness. Circulating inflammatory cytokines, angiogenic factors, and their interaction may contribute to the pathophysiology of this complication. Angiopoietin (Ang)-1, Ang-2, soluble vascular cell adhesion molecule-1, intercellular adhesion molecule (ICAM)-1, E-selectin, P-selectin, IL1-beta, TNF-alpha, pigment epithelium derived factor (PEDF) and vascular endothelial growth factor plasmatic levels were determined in 37 SCD patients with retinopathy, 34 without retinopathy, and healthy controls. We observed that sICAM-1 is significantly decreased, whereas PEDF is elevated in HbSC patients with retinopathy (P = 0.012 and P = 0.031, respectively). Ang-1, Ang-2 and IL1-beta, levels were elevated in SCD patients (P = 0.001, P < 0.001 and P = 0.001, respectively), compared to controls, and HbSS patients presented higher levels of Ang-2 compared to HbSC (P < 0.001). A 46-year-old woman complained of blurred and distorted vision in both eyes. Ophthalmic examination showed that visual acuity was 20/200 for the right eye and counting fingers left eye. Fundoscopy revealed perimacular hemorrhages, aneurismal dilatation of the vessels in the posterior pole, and a white and elevated lesion adjacent to vascular changes. We report a case of idiopathic macular telangiectasia and epiretinal membrane that occurs concomitantly. To our knowledge, this is the first report that describes an association between idiopathic macular telangiectasia and epiretinal membrane formation.

Ao exame oftalmológico, sua acuidade visual era 20/200 no olho direito e conta dedos a 5 metros no olho esquerdo. A fundoscopia revelou hemorragias perimaculares, dilatação aneurismática dos vasos no polo posterior e uma lesão elevada e esbranquiçada ao lado das alterações vasculares. Relatamos um caso de telangectasia macular idiopática e membrana epirretiniana que ocorreram concomitantemente.
