Abstract

IntroductionPhysical activity (PA) is inversely associated with risk of heart failure and cardiovascular disease (CVD), whereas increased left ventricular (LV) mass and mass to volume (m:v) ratio are unfavorable CVD risk factors. We assessed whether changes in leisure time PA were associated with longitudinal changes in cardiac structure in a community-based population.MethodsWe included 2779 Multi-Ethnic Study of Atherosclerosis participants, free of baseline CVD, who had available data on PA and cardiac magnetic resonance imaging at examinations 1 (2000-2002) and 5 (2010-2012). Physical activity was measured by a Typical Week PA Survey and converted to MET-minutes per week of moderate+vigorous activity. We used linear mixed effect models to estimate the associations of baseline and change in PA with baseline and change in cardiac structure, adjusting for CVD risk factors and body size.ResultsAt baseline, the mean age was 59 yr, 53% were women, and 58% of nonwhite race/ethnicity. During average 10-yr follow-up, and after accounting for baseline PA levels, the highest quintiles of PA increase were significantly associated with increases in LV mass (2.3 g; 95% confidence interval [CI], 0.4-4.2), LV end-diastolic volume (4.7 mL; 95% CI, 2.4-7.0), and stroke volume (3.3 mL; 95% CI, 1.6-5.1), but lower M:V ratio (-2.9; 95% CI, -5.0 to -0.8) compared with the lowest quintiles. Increasing exercise PA was associated with increases in LV diameter and reductions in M:V ratio, whereas occupational PA was associated with increases in m:v ratio. Increasing PA over 10 yr was also associated with greater risk of eccentric dilated LV hypertrophy at examination 5.ConclusionsAfter accounting for baseline PA, greater positive changes in leisure-time PA levels were associated with a more eccentric-type of LV remodeling pattern over 10 yr. The clinical implications of such findings remain to be determined.

Abstract

Objective The contribution of hepatitis C virus (HCV) infection to the risk of heart failure in human immunodeficiency virus (HIV)-coinfected persons is unknown. The objective was to characterize cardiac function and morphology in HIV-treated coinfected persons. Methods In a cross-sectional study, HIV-infected patients virologically suppressed on antiretroviral therapy without known cardiovascular disease or diabetes mellitus underwent cardiac magnetic resonance imaging and spectroscopy for measures of cardiac function, myocardial fibrosis, and steatosis. Results The study included 18 male patients with a median age of 44 years. Of these, 10 had untreated HCV coinfection and eight had HIV monoinfection. Global systolic and diastolic function in the cohort were normal, and median myocardial fat content was 0.48% (interquartile range 0.35-1.54). Left ventricular (LV) mass index and LV mass/volume ratio were significantly greater in the HIV/HCV-coinfected group compared with the HIV-monoinfected group. In the HIV-monoinfected group, there was more myocardial fibrosis as measured by extracellular volume fraction. Conclusions There were differences between HIV/HCV-coinfected and HIV-monoinfected patients in cardiac structure and morphology. Larger studies are needed to examine whether HIV and HCV independently contribute to mechanisms of heart failure.

Abstract

BackgroundEvaluation of impaired left ventricular (LV) filling has focused on intrinsic causes of LV dysfunction; however, pulmonary vascular changes may contribute to reduced LV filling and dyspnea. We hypothesized that lower total pulmonary vascular volume (TPVV) on computed tomography (CT) would be associated with dyspnea and decrements in LV end-diastolic volume, particularly among ever-smokers.MethodsThe Multi-Ethnic Study of Atherosclerosis recruited adults without clinical cardiovascular disease in 2000-02. In 2010-12, TPVV was ascertained as the volume of arteries and veins in the lungs detectable on non-contrast chest CT (vessels ≥1 mm diameter). Cardiac measures were assessed by magnetic resonance imaging (MRI). Dyspnea was self-reported.ResultsOf 2303 participants, 53% had ever smoked cigarettes. Abstract

ImportanceMyocardial scarring leads to cardiac dysfunction and poor prognosis. The prevalence of and factors associated with unrecognized myocardial infarction and scar have not been previously defined using contemporary methods in a multiethnic US population.ObjectiveTo determine prevalence of and factors associated with myocardial scar in middle- and older-aged individuals in the United States.Design, setting, and participantsThe Multi-Ethnic Study of Atherosclerosis (MESA) study is a population-based cohort in the United States. Participants were aged 45 through 84 years and free of clinical cardiovascular disease (CVD) at baseline in 2000-2002. In the 10th year examination (2010-2012), 1840 participants underwent cardiac magnetic resonance (CMR) imaging with gadolinium to detect myocardial scar. Cardiovascular disease risk factors and coronary artery calcium (CAC) scores were measured at baseline and year 10. Logistic regression models were used to estimate adjusted odds ratios (ORs) for myocardial scar.ExposuresCardiovascular risk factors, CAC scores, left ventricle size and function, and carotid intima-media thickness.Main outcomes and measuresMyocardial scar detected by CMR imaging.ResultsOf 1840 participants (mean [SD] age, 68 [9] years, 52% men), 146 (7.9%) had myocardial scars, of which 114 (78%) were undetected by electrocardiogram or by clinical adjudication. Abstract

Non-Fermi-liquid (NFL) behavior in the f-sublattice-diluted alloy system U_{1-x}Th_xPd_2Al_3 has been studied using ^{27}Al nuclear magnetic resonance (NMR). Impurity satellites due to specific U near-neighbor configurations to ^{27}Al sites are clearly resolved in both random and field-aligned powder samples. The spatial mean Kbar and rms spread delta K of impurity satellite shifts, which are related to the mean chibar and rms spread delta chi of the inhomogeneous susceptibility, have been measured in field-aligned powders with the crystalline c axis both perpendicular and parallel to the external field. The relatively narrow lines observed at low temperatures suggest that disorder- induced inhomogeneity of the f-ion--conduction-electron hybridization is not the cause of NFL behavior in these alloys: at low temperatures the experimental values of delta chi(T)/chibar(T) are much smaller than required by disorder- driven models. This is in contrast to results in at least some alloys with disordered non-f-ion nearest neighbors to f ions ("ligand disorder"), where disorder-driven theories give good accounts of NFL behavior. Our results suggest that f-ion dilution does not produce as much inhomogeneity of the hybridization strength as substitution on ligand sites., Comment: 10 pages, 12 figures, REVTeX. Abstract

We report measurements of the bulk magnetic susceptibility and ^{29}Si nuclear magnetic resonance (NMR) linewidth in the heavy-fermion alloy CeRhRuSi_2. The linewidth increases rapidly with decreasing temperature and reaches large values at low temperatures, which strongly suggests the wide distributions of local susceptibilities \chi_j obtained in disorder-driven theories of non-Fermi-liquid (NFL) behavior. The NMR linewidths agree well with distribution functions P(\chi) which fit bulk susceptibility and specific heat data. The apparent return to Fermi-liquid behavior observed below 1 K is manifested in the vanishing of P(\chi) as \chi \to \infty, suggesting the absence of strong magnetic response at low energies. Our results indicate the need for an extension of some current theories of disorder-driven NFL behavior in order to incorporate this low-temperature crossover., Comment: 10 pages, 8 figures, submitted to Phys. Abstract

Signal intensity (SI) drop has been proposed as an indirect stenosis assessment in non-contrast coronary MRA (NC-MRCA) but it uses unproven assumptions.We aimed to clarify the mechanisms that govern the SI in vitro and develop a stenosis detection method in vivo. Flow phantom tubes with/without stenosis were scanned under two spatial resolutions (0.5/1.0 mm3) on a 3.0 T MRI. Thirty-two coronary arteries from 11 volunteers were prospectively scanned with an EKG- and respiratory-gated 3D NC-MRCA with a resolution of 1.0 mm3, with coronary computed tomography angiography (CTA) as reference. The normalized SI along the centerline of the tubes or the coronary arteries was assessed against the distance from the orifice using a linear regression model. Its coefficient (SI decay slope) and goodness-of-fit (R2) were extracted to assess the effect of flow velocity and stenosis on the SI profile curve. The R2 was utilized for the stenosis detection. Phantom study: A slow flow velocity caused a steep SI decay slope. The SI drop revealed only at the inlet and outlet of stenosis due to the flow turbulence/vortex and yielded low R2, in which shape changed by the resolution. Clinical study: The R2 cutoff to detect ≥ 50% stenosis for the left and right coronary arteries were 0.64 and 0.20 with a sensitivity/specificity of 71.5/71.5 and 66.7/100 (%), respectively. The SI drop did not reflect the actual stenosis position and not suitable for the stenosis localization. The R2 cutoff represents an alternative method to detect stenoses on NC-MRCA at vessel level.

Abstract

We report measurements of the bulk magnetic susceptibility and (Formula presented) nuclear magnetic resonance (NMR) linewidth in the heavy-fermion alloy (Formula presented) The linewidth increases rapidly with decreasing temperature and reaches large values at low temperatures, which strongly suggests the wide distributions of local susceptibilities (Formula presented) obtained in disorder-driven theories of non-Fermi-liquid (NFL) behavior. The NMR linewidths agree well with distribution functions (Formula presented) which fit bulk susceptibility and specific-heat data. The apparent return to Fermi-liquid behavior observed below 1 K is manifested in the vanishing of (Formula presented) as (Formula presented) indicating the absence of strong magnetic response at low energies. Our results point out the need for an extension of some current theories of disorder-driven NFL behavior in order to incorporate this low-temperature crossover. © 2000 The American Physical Society.

Abstract

Adolescent idiopathic scoliosis (AIS) is a three-dimensional spinal curvature deformity that appears in the adolescent period. In this study, we performed whole-exome sequencing on 11 unrelated Taiwanese patients with a Cobb's angle greater than 40 degrees. Our results identified more than 200 potential pathogenic rare variants, however, most of which were carried only by one individual. By in silico pathogenicity annotation studies, we found that TTN, CLCN1, and SOX8 were the most important genes, as multiple pathogenic variants were within these genes. Furthermore, biological functional annotation indicated critical roles of these AIS candidate genes in the skeletal muscle. Importantly, a pathogenic variant on SOX8 was shared by over 35% of the patients. These results highlighted TTN, CLCN1, and SOX8 as the most likely susceptibility genes for severe AIS.
