Superbugs in groundwater are posing severe health risks through waterborne pathways. An emerging approach for green disinfection lies at photocatalysis, which levers the locally generated superoxide radical (·O2−) for neutralization. However, the spin-forbidden feature of O2 hinders the photocatalytic generation of active ·O2−, and thus greatly limited the disinfection efficiency, especially for real groundwater with a low dissolved oxygen (DO) concentration. Herein, we report a class of strained Mo4/3B2-xTz MBene (MB) with enhanced adsorption/activation of molecular O2 for photocatalytic disinfection, and find the strain induced spin polarization of In2S3/Mo4/3B2-xTz (IS/MB) can facilitate the spin-orbit hybridization of Mo sites and O2 to overcome the spin-forbidden of O2, which results in a 16.59-fold increase in ·O2− photocatalytic production in low DO condition (2.46 mg L-1). In particular, we demonstrate an In2S3/Mo4/3B2-xTz (50 mg)-based continuous-flow-disinfection system stably operates over 62 h and collects 37.2 L bacteria-free groundwater, which represents state-of-the-art photodisinfection materials for groundwater disinfection. Most importantly, the disinfection capacity of the continuous-flow-disinfection system is 25 times higher than that of commercial sodium hypochlorite (NaOCl), suggesting the practical potential for groundwater purification. Photocatalytic disinfection is a known pollution-free water treatment strategy. However, its efficiency is limited due to the spin-forbidden feature of O2, which hinders photocatalytic ·O2− formation and limits its disinfection efficiency. Background: Patients with cardiogenic shock have a 25% to 50% mortality rate despite the introduction of mechanical circulatory devices and coordinated medical treatment. The use of shock teams has improved outcomes for these patients. Local Problem: A cardiovascular hospital with a multidisciplinary shock team had inconsistency and delays in recognition and diagnosis of patients with cardiogenic shock. Methods: A nurse-led, preintervention-postintervention quality improvement project was performed in April and May 2021 and in April and May 2022 within a cardiovascular hospital in north Texas. The 2 nursing staff interventions regarding shock team activation were education and shock alert implementation. Time from first signs to diagnosis (for inpatients) and time from initial transfer request to acceptance (for transferring patients) were measured. Descriptive and statistical analyses were conducted using R, version 4.0.0 (R Foundation for Statistical Computing). Results: The mean (SD) time to diagnosis of cardiogenic shock decreased significantly from 17.98 (28.39) hours in the preintervention group (n = 25) to 8.15 (12.26) hours in the postintervention group (n = 45; P =.045). For patients with cardiogenic shock transferring from referring hospitals, the median (IQR) time to acceptance was 1.55 (0.08-3.18) hours in the preintervention group and 0.35 (0.00-0.72) hours in the postintervention group (P <.001). Conclusions: Nursing staff interventions regarding shock team activation significantly improved the time of diagnosis and acceptance of patients with cardiogenic shock in a cardiovascular hospital, enhancing the overall quality of care provided to these patients. Summary: Background and Aims: Non‐selective beta‐blockers (NSBB) are a well‐established treatment in patients with clinically significant portal hypertension. However, their potential role after insertion of a transjugular intrahepatic portosystemic shunt (TIPS) still needs to be determined. Of note, recent studies suggested that favourable anti‐inflammatory effects of NSBB might be independent from pressure reduction. This study aimed to evaluate whether NSBB‐treatment is associated with amelioration of systemic inflammation (SI), hepatic decompensation and survival after TIPS‐insertion. Methods: In a retrospective study comprising 305 consecutive patients, we investigated the impact of NSBB‐intake at TIPS‐placement on periinterventional cirrhosis‐associated complications and continued NSBB‐treatment after discharge on complications including hepatic decompensation and mortality during 1‐year follow‐up, employing multivariable competing‐risk‐analyses. In a prospective cohort including 45 patients, we performed a comprehensive analysis of SI analysing 48 soluble inflammatory markers (SIMs) at baseline plus 3 and 6 months after TIPS‐insertion. Results: Overall, 175 (57.4%) patients received NSBB‐therapy prior to TIPS‐insertion; upon discharge, this decreased to 131 (22.9%), with 36 (27.5%) discontinuing NSBB within 1‐year follow‐up. Neither NSBB‐therapy at TIPS‐insertion nor treatment‐continuation after discharge were associated with lower risks for hepatic decompensation, individual cirrhosis‐associated complications or mortality neither in the periinterventional period nor during follow‐up. Similarly, in the prospective cohort NSBB‐intake was not linked to lower levels or a more prominent change of WBC, CRP or any other SIM at any of the investigated time points. Conclusion: NSBB‐therapy at the time of TIPS‐insertion and its (dis‐)continuation afterwards seems to have no significant impact on SI, development of hepatic decompensation and survival.
