Objective To investigate the efficacy and safety of the targeted drug poly ADP-ribose polymerase (PARP) inhibitor niraparib combined with low-dose chemotherapy drugs cisplatin and etoposide in the treatment of pediatric recurrent PFA-type ependymoma. Methods A total of 12 pediatric patients with recurrent PFA - type ependymoma, treated in Children's Hospital of Fudan University from January 2022 to January 2024, were included in the study. All patients received a treatment regimen combining niraparib with low-dose cisplatin and etoposide chemotherapy. The objective response rate (ORR) of the tumor was statistically evaluated to assess treatment efficacy, and patient tolerability and safety indicators, including liver and kidney function, cardiac function, nausea and vomiting, fatigue, bone marrow suppression, gastrointestinal reactions, and infections, were recorded. Results Among 12 pediatric patients, 10 cases showed a tumor reduction of ≥ 30%, in which 2 cases showed a tumor reduction of 100%, while one case showed no change in tumor size, and one case showed tumor progression. The overall ORR was 10/12. Adverse reactions included 5 cases of severe vomiting, primarily occurring during chemotherapy period, and 8 cases of varying degrees of bone marrow suppression, all of which were alleviated after symptomatic treatment. Conclusions Niraparib combined with low-dose chemotherapy drugs shows good efficacy and safety in the treatment of pediatric recurrent PFA-type ependymoma.

AIM: To explore the relationship between retinal exudative changes in neonates and perinatal toxoplasmosis, others, rubella, cytomegalovirus, and herpes simplex virus(TORCH)infections, as well as the characteristics of TORCH infection in neonates with retinal exudative changes.METHODS: Retrospective study. A total of 612 neonates with retinal exudative changes detected during ophthalmic screening in our hospital from May 2019 to March 2023 were selected. TORCH tests were performed on these neonates, and the results were subjected to statistical analysis to determine the infection characteristics. The neonates with retinal exudative changes were grouped by sex and age, the characteristics of TORCH infection were analyzed, and the positive rates were compared.RESULTS: Among the 612 neonates with retinal exudative changes, the highest positive rate was observed for cytomegalovirus(CMV-IgG)(96.7%), followed by rubella virus(RV-IgG)(73.9%). Mixed infections with two or three viruses were also observed, with the highest positive rate for mixed infection of RV-IgG and CMV-IgG reaching 71.2%. There was no statistically significant difference in TORCH infection among neonates of different sex(P>0.05). ObjectiveTo use the maternal morbidity WOICE Tool(Chinese version)to investigate the maternal morbidity in Shanghai, and to examine the current situation and associated factors of adverse symptoms and impaired functioning in the third trimester and postpartum period.MethodsPregnant women who made their initial visit and established a medical record at a tertiary obstetrics and gynecology hospital in Shanghai from March to August 2021 were recruited and a baseline survey was completed. The prevalence of maternal morbidity was surveyed in the third trimester and first 6 weeks postpartum.ResultsSelf-reported adverse physical symptoms were observed in 89.8% of women in the third trimester and 86.1% in first 6 weeks postpartum. The prevalence rates of anxiety and depression were 4.1% and 6.2% in the third trimester, and 6.2% and 7.5% in first 6 weeks postpartum, respectively. The proportions of women reporting impaired functioning were 80.3% in the third trimester and 64.1% in first 6 weeks postpartum, respectively. Physical symptoms were associated with parity, education, the newborn’s health status, and risk of experiencing violence. Psychological symptoms were associated with age, household responsibilities, employment status, the newborn’s health status, pre-pregnancy body mass index (BMI), and risk of experiencing violence. Functionality was associated with the type of registered residence, education level, household responsibilities, and risk of experiencing violence.ConclusionThe physical health status of pregnant and postpartum women in Shanghai is better than that in the middle-income regions abroad, with anxiety and depression at average levels compared to national statistics in China. Physical, psychological, and functional status are affected by multiple factors including personal, health, family, and work conditions. Preventive measures should be taken from various perspectives to mitigate adverse symptoms and impaired functioning, and to improve the positive experience of pregnancy and childbirth.

Natural gas liquefaction requires massive cold energy, but the single liquefaction process can not fully utilize the cold energy, so that much of it is wasted. Besides, storing LNG generates a large amount of BOG that demands urgent care in order to prevent production accidents. Therefore,for the purpose of energy saving, consumption reduction and full utilization of the cold energy, this study proposes an innovative co-production process combining nitrogen cyclic expansion refrigeration based natural gas liquefaction and BOG cryogenic helium extraction, This study uses the HYSYS software to simulate the nitrogen cyclic expansion refrigeration based natural gas liquefaction, the BOG cryogenic helium extraction and the co-production process, and analyzes the key parameters relating to the comprehensive energy consumption and the crude helium concentration. While maintaining the crude helium concentration at 76.8%, the co-production system, with the goal of minimizing the comprehensive energy consumption, was optimized by applying the response surface method and the genetic algorithm into the process. The optimal parameters obtained were as follows: the high pressure of the refrigerant was 7 005 kPa while the low pressure was 489 kPa and the flow rate was 4 000 kmol/h. The feed temperature and the feed pressure of the cryogenic were-130 ℃ and 2 390 kPa respectively. In the co-production process, the natural gas liquefaction rate exceeded 90%, the helium recovery rate exceeded 95%, and the crude helium volume fraction exceeded 76.8%. Compared with the single liquefaction process, the total compression power consumption in the co-production process was reduced by about 23.6%, 4 022.71 kW in number; and the overall energy consumption was reduced by about 18.00%, 3 482.4 kW in number. The simulation results suggest that the co-production process is more cost-effective and energy-efficient. This study could provide a reference for the engineering application of the newly designed co-production process.

Based on Germany’s practical experience, this research offers a strategy idea for other countries with a social health insurance system, aiming to ensure that the health care system does not collapse rapidly due to medical resource shortage in the event of a pandemic. Firstly, this research conducted a documentary analysis to systematically collate the temporary and additional coverage measures provided by the health insurance fund from January to the end of July 2020, which sheds light on the pivotal role of these funds in epidemic prevention and control. Secondly, this research used comparative analysis to examine the time sequence of implementing these different types of coverage measures in the progression of the epidemic to illustrate how the health insurance fund adjusted its response measures. The health insurance fund was actively involved in the development of core strategies for combating the epidemic when it broke out, by taking part in joint multisectoral consultations. By using payment instruments flexibly, the fund led the implementation of epidemic prevention and control measures, as it could allocate health resources quickly and efficiently in emergencies. Furthermore, the health insurance fund played a critical role in transmitting information on the epidemic and guiding the insured to take appropriate protective measures. By fulfilling its role in health promotion, particularly in the area of health education, the fund provided important complementary and synergistic contributions to the prevention and control of the spread of infectious diseases. In summary, this research provides a new model for other countries for mobilizing a multi-sectoral response to infectious disease prevention and control, and emphasises the key role of the health insurance fund in responding to major public health crises.Keywords: health insurance fund, the epidemic prevention and control, decision-making, payment instrument, cooperative participation

The Cu–15Ni–8Sn alloy ingot was prepared in the medium-frequency induction melting furnace. The hot extruded alloy was treated by solid solution at 840 °C for 60 min and aging at 400 °C for different times (0 miñ480 min). The effect of aging time on the evolution of the nanophase of Cu–15Ni–8Sn alloy and the strengthening-toughening mechanism were studied. The results show that at the beginning of aging, the alloy undergoes spinodal decomposition, the wavelength of the modulated structure is approximately 3∼7 nm, the ordered phase transformation of D022 and L12 occurs simultaneously, and the ordered phase size is approximately 5 nm. With the extension of aging time, the modulated structure became coarser, and the wavelength of the modulated structure increased to 5–9 nm. At the same time, the D022 ordered phase changed to the L12 ordered phase, and a continuous distribution of elliptical discontinuous precipitation appeared at the grain boundary. The ratio of the long and short axes of the discontinuous precipitation was approximately 4.7. In the late aging period, the D022 ordered phase was basically transformed into the L12 ordered phase, and discontinuous precipitation, which is incompatible with the matrix, extended into the crystal. After 120 min of aging at 400 °C, the hardness, strength, and elongation of the alloy are 105.4 HRB, 839.8 MPa and 9.6 %, respectively. The peak-aging states D022 and L12 coexist in an orderly phase at the nanoscale, and this nanoscale strengthening phase is completely integrated with the matrix, which improves the strength and toughness of the alloy. The research results can provide support for the development of high-performance copper alloys in the ocean.

BackgroundDepression presents a significant risk to the physical and mental health of the elderly. The relationship between sleep and depressive symptoms has become a major research focus. However， previous research findings on this relationship have yielded inconsistent conclusions.ObjectiveTo explore the relationship between sleep duration and depressive symptoms in the elderly， in order to provide insights into the prevention of depressive symptoms and delaying their progression in the elderly.MethodsData analysis involved 8 210 adults aged 60 years or older， utilizing data from the 2018 China Health and Retirement Longitudinal Study （CHARLS） survey. Participants were categorized into two groups those with depressive symptoms and those without- based on the Center for Epidemiologic Studies Depression-10 Scale （CESD-10） criteria. Logistic regression and restricted cubic spline models were used to analyze the association between sleep duration and depressive symptoms.ResultsThe 318 people with depressive symptoms were detected among the 8 210 elderly participants， with an average sleep duration of （6.14±2.05） hours per night. Restricted cubic spline models showed a non-linear association between sleep duration and depressive symptoms （χ2=412.670，P0.05）.ConclusionThere was an approximately nonlinear association between sleep duration and depressive symptoms， with differences by gender and age.［Funded by Sichuan Applied Psychology Research Center， Chengdu Medical College （number， CSXL-22226）］

Ying-Nan Bi, Yu-An Liu School of Political Science and Public Administration, Shandong University, Qingdao, People’s Republic of ChinaCorrespondence: Ying-Nan Bi, Email 201720089@mail.sdu.edu.cnAbstract: After 75 years of reformed practice, general practitioners (GPs) in the UK have transformed from health gatekeepers who simply provide medical decision-making such as diagnostic and treatment services and referral services, to health agents who proactively provide more relevant health services such as immunizations, health monitoring and health management, etc. In order to discuss this transformation of the role of the general practitioner and the conditions for the evolution of the role, this study chose the documentary analysis method to provide a comprehensive overview of the legal and normative documents related to the general practitioner. Furthermore, this study uses a comparative analysis method to conclude the definition and role characteristics of GPs as health agents. This study summarises the general pattern of evolution of GPs into health agents. The transformation into a health agent relies on the interpersonal trust and rigorous institutional of society on the general practitioner system. The expansion of GPs’ clientele and range of services, together with the motivation to proactively provide services, have combined to push for a “qualitative change” in the GP’s role as health agent. The transformation of the role of the general practitioner to a health agent is a historical necessity. It responds to the evolution of society’s understanding of health and the need for higher levels of health. Therefore, recognizing the role of GPs as health agents is important for optimizing the use of health care resources and improving the health of society by taking advantage of this role.Keywords: general practitioner, health agent, health gatekeeper

Fermentation is the key to the formation of characteristics and quality of black tea. It is very important to evaluate the fermentability of black tea quickly and accurately. In this paper, the application of artificial sensory evaluation technology, biochemical component detection technology, intelligent biomimetic technology, data fusion technology and intelligent biomimetic equipment in the evaluation of black tea fermentation quality in recent years was reviewed, and the development trend of intelligent biomimetic technology in the evaluation of black tea fermentation in the future was prospected.

ObjectiveTo compare the effects of cell lysis method and magnetic beads method in forensic DNA identification and to explore these two methods in forensic DNA identification.MethodsThe genome DNA of THP-1 cells in different quantities was extracted by the cell lysis method and magnetic beads method, and the DNA content was quantified by real-time quantitative PCR. The cell lysis method and magnetic beads method were used to type the STR of human blood with different dilution ratios.ResultsWhen the numbers of THP-1 cell were 100, 400 and 800, the DNA content extracted by cell lysis method were （1.219±0.334）, （5.081±0.335）, （9.332±0.318） ng, respectively; and the DNA content extracted by magnetic beads method were （1.020±0.281）, （3.634±0.482）, （7.896±0.759） ng, respectively. When the numbers of THP-1 cells were 400 and 800, the DNA content extracted by the cell lysis method was higher than that by the magnetic beads method. The sensitivity of cell lysis method and magnetic beads method was similar in STR typing of human blood at different dilution ratios. Complete STR typing could be obtained at 100, 300 and 500-fold dilutions of blood samples, but could not be detected at 700-fold dilution. STR typing of undiluted human blood could not be detected by cell lysis method.ConclusionThe cell lysis method is easy to operate and can retain template DNA to the maximum extend. It is expected to be suitable for trace blood evidence tests.

The quality and safety of children’s textiles, especially chemical safety, is very important. The harm to children caused by using unsafe products is immeasurable. At present, the chemical safety research of textiles usually focuses on the specific target substances specified in the regulations. The detection methods are mainly to detect the content of target substances in the sample to determine whether it meets the regulatory requirements. In fact, although the known chemicals in the product meet the regulatory requirements, other unknown chemicals may exist potential hazards to children’s health. Therefore, it is necessary to screen potential chemicals in children’s textiles by non-targeted method in order to more comprehensively screen the chemical risks in product. Non-targeted analysis of potential chemical substances in imported children’s textiles using gas chromatography-Orbitrap high-resolution mass spectrometry (GC-Orbitrap MS) was established. Unknown substances were qualitatively assessed by using the comprehensive score and retention index, identifying the molecular formula by chemical ionization, comparing fragment ion data to distinguish isomers, and verifying according to chemical standards. A total of 48 substances were identified in 40 children’s textiles, including 21 esters, 8 ketones, 6 amines, 4 ethers, 3 alcohols, 3 phenols and 3 other substances. The identified substances were statistically and semi-quantitatively analyzed. Dibutyl phthalate, tolylene-2,4-diisocyanate, isopropyl palmitate and 1,2,4-butanetriol were identified in the imported children’s textiles, and their detection rates were greater than or equal to 10%. The identified substances were further filtered and screened according to detection rates, toxicities and response intensities. Then the high-risk chemicals in the children’s textiles were found. This method can provide a new way for screening potential chemicals in textiles and other products, and play a positive role in carrying out product chemical risk monitoring and promoting product quality and safety.

Abstract The registered dietitian system started late in China, and since 2014 a working committee of registered dietitians has been formed by the Chinese Nutrition Society to establish a national accreditation system in accordance with the international standard academic and internship requirements. The data from the certification examinations and registrations were collected and analyzed. 8,328 registered dietitians were included in the study, with 5,673 registered dietitians (RD) and 2,655 registered dietetic technicians (DTR). Among them, 45.27% of RDs have master degree or above, and 66.44% of DTRs have bachelor or above. 53.25% RDs work in medical institutions, 14.77% in colleges and universities, and 14.56% in enterprises; 48.21% DTRs work in the medical institutions, 30.28% in enterprises, and 7.34% in the community. Although six years of work has initially established a foundation of standardized registered dietitian accreditation system, the problems of limited resources for academic education and the lack of legislative protection for the profession remain prominent.

A stable and reliable time keeping system depends on the integrity monitoring of the atomic frequency standard. This paper reports a scheme for autonomous integrity monitoring of multiple atomic clocks, which combines the frequency standard comparison method and the frequency jump detection method. The frequency standard comparison method uses multi-channel synchronous acquisition technology and digital frequency measurement technology to realize the precise measurement of multiple atomic frequency standards. The frequency jump detection method uses adaptive filtering to predict the relative frequency difference and give an accurate and timely alarm for the abnormal of frequency jump. The results show that the noise floor frequency standard comparator is better than 6.5×10−15 s. For a relative frequency deviation of 2.0×10−6 Hz, the probability of anomaly detection is almost 100 %. The system has high frequency measurement resolution and fast alarm of frequency jump, which can meet the real-time requirements of a time keeping system for the integrity monitoring of multiple atomic clocks.

The accumulation and incineration of waste tires have caused great damage to the environment. Therefore, recycling waste tires efficiently and economically has become an important issue in protecting world resources and the environment. In this study, 5%, 10%, and 15% rubber particles were used to replace fine sand in concrete, and basalt fibers (0.1%, 0.2%, and 0.3%) and polypropylene fibers (0.1%, 0.15%, and 0.2%) were added to rubber concrete to prepare hybrid fiber rubber concrete (HFRC). The experiments investigated the changes of mechanical properties of basalt (BF), polypropylene (PPF) fibers, and rubber particles when they were blended into concrete respectively, and the degree of influence of each factor on the mechanical and carbonation resistance of HFRC was investigated based on the orthogonal test. The results show that the rubber concrete’s mechanical and carbonation properties are improved to varying degrees after adding hybrid fibers. When the basalt fiber content is 0.2% and the polypropylene fiber content is 0.15%, the overall mechanical properties of HFRC reach the best state. When the basalt fiber content is 0.2%, the polypropylene fiber content is 0.15%, and the rubber content is 5%, the carbonation resistance of HFRC is the most ideal. In addition, the microstructure of HFRC was observed by scanning electron microscope.

ObjectiveThis study aimed to assess the fairness of medical resource allocation in the Yangtze River Economic Belt, based on the Healthy China strategy. It aimed to identify the issues with resource allocation fairness and provide optimization suggestions.MethodsTo assess the allocation fairness from a geographical population perspective, the study used the Health Resource Concentration and Entropy Weight TOPSIS methods. Additionally, the study analyzed the allocation fairness from an economic level angle, using the Concentration Curve and Concentration Index.ResultsThe study found that the downstream area had higher resource allocation fairness than the midstream and upstream areas. The middle reaches had more resources than the upper and lower reaches, based on population concentration. The Entropy Weight TOPSIS method found that Shanghai, Zhejiang, Chongqing, and Jiangsu had the highest comprehensive score index of agglomeration. Furthermore, from 2013 to 2019, the fairness of medical resource distribution gradually improved for different economic levels. Government health expenditure and medical beds were distributed more equitably, while general practitioners had the highest level of unfairness. However, except for medical and health institutions, traditional Chinese medicine institutions, and primary health institutions, other medical resources were mostly distributed to areas with better economic conditions.ConclusionThe study found that the fairness of medical resource allocation in the Yangtze River Economic Belt varied greatly based on geographical population distribution, with inadequate spatial accessibility and service accessibility. Although the fairness of distribution based on economic levels improved over time, medical resources were still concentrated in better economic areas. The study recommends improving regional coordinated development to enhance the fairness of medical resource allocation in the Yangtze River Economic Belt.

ObjectiveTo explore the influencing factors of the horizontal distance of bodies in the high falling scene and the feasibility of inferring the falling mode based on it.MethodsA total of 614 high falling deaths and 15 cases of corpse dumping from high altitudes were collected. The relationship between the horizontal distance and the falling height, as well as the sex, age and manner of death （suicide, accident and corpse dumping） were observed.ResultsThe horizontal distance increased with the increase of falling height, and the difference among the height groups was statistically significant. The horizontal distance decreased with the increase of the age of the deceased, in each height group, the difference between the group over 60 years old and other age groups was statistically significant （P20-30 m group, there was no significant difference in horizontal distance between suicide high falls and accidental high falls in other height groups.ConclusionThe horizontal distance is affected by the falling height, the sex and age of the victim, and the spatial characteristics of the falling starting point.

As the most important bearing component of the hydraulic support, the bearing performance of the column has a huge influence on the supporting effect of the whole hydraulic support. Especially, when the coal bump disaster occurs, it may cause accidents such as bending, fracture and cylinder explosion of the hydraulic support column. The fluid-structure interaction model of ZF10000/25/38 hydraulic support column is established by Solid Works and Design Modeler software. The hydraulic support column is regarded as a spring, and the equivalent stiffness mathematical model of single telescopic column is derived. Then, the bidirectional transient fluid-structure interaction model of column is simulated by ANSYS Workbench simulation software, and the impact characteristics of coal bump are simulated by introducing triangular wave impact load to study the anti-impact characteristics of the single telescopic column of the hydraulic support. The simulation results show that the maximum stress of hydraulic support column piston rod under impact load is 508 MPa, which occurs at the top, and the maximum stress of cylinder block is 254 MPa, which occurs at the bottom.

In this paper, the reaction kinetic mechanism of La3+ was studied in the LiCl-KCl eutectic salt at various temperatures. First, the diffusion coefficient of La3+(D) measured by cyclic voltammetry(CV) increases from 3.06×10-5 cm2/s to 6.08×10-5 cm2/s with the temperature ranging from 723 K to 873 K, and the activation energy of La3+ diffusion(ED) in the molten salts is 34.51 kJ/mol calculated by the Arrhenius equation. Subsequently, the kinetic parameters of La3+ on the W electrode were studied by the electrochemical impedance spectroscopy(EIS). In the temperature range of 723-873 K, the exchange current density(i0) is measured to be 0.48-1.39 A/cm2, and the reaction rate constant(k0) is determined as 2.04×10-4-5.90×10-4 cm/s. Moreover, the corresponding reaction activation energy(Ea) is found to be 35.04 kJ/mol. The Nyquist plots and the fitted circuit diagrams explored the reaction kinetic mechanism of La3+ on a W electrode. It was illustrated that the electrochemical reduction rate of La3+ in the LiCl-KCl eutectic salt is controlled by diffusion and charge transfer, and it is positively correlated with temperature.

Abstract Background In recent years, the number of adolescents engaging in premarital sex has increased, and an increasing number of childhood sexual abuse (CSA) cases have been reported in China. Many studies have indicated that CSA has a well-established association with risky sexual activities. However, only a limited number of studies have explored possible reasons for this association among middle school students, a population that may engage in premarital sex, which is critical for the development of interventions to prevent risky sexual behavior. Based on random samples of middle school students from a Chinese city, this article investigated the relationship between CSA and students’ premarital sexual permissiveness (PSP). Methods In a cross-sectional study conducted between 2016 and 2017 in Luzhou, China, 2292 middle school and high school students aged 12–18 years were recruited by multistage random sampling. All students were administered anonymous questionnaires. Multiple linear regression and binary logistic regression analyses were conducted to analyze the relationship between CSA and PSP. Results The prevalence of CSA was 15.4% (354/2292; 95% CI: 14–16.9%). A higher percentage of male respondents (18.2%) than female respondents (12.9%) had experienced CSA. A positive association between CSA and PSP was found among students. Respondents who had suffered CSA exhibited greater PSP, and this relationship was observed in the male sample, female sample and the total student sample in Luzhou (β = − 3.76, P

Objective To explore the best time and method of lacrimal duct irrigation and probing in the treatment of children with congenital nasolacrimal duct obstruction (CNLDO). Methods A total of 3 558 children with CNLDO (5 514 eyes) who were treated in our hospital from April 2017 to December 2020 were enrolled and retrospectively analyzed. They were divided into groups according to their gender, delivery mode and treatment age, and the success rate of lacrimal duct irrigation was compared among the corresponding groups. In addition, 723 of the patients (723 eyes) who underwent lacrimal duct probing therapy were grouped according to their age and number of lacrimal duct irrigation, and the success rate of one-time patency was compared among the corresponding groups. Results In terms of lacrimal duct irrigation, the one-time patency rate was the highest in children aged < 2 months (66.36%), along with 46.25%, 38.05% and 28.64% in children aged 2~4 months, 4~6 months and >6 months, respectively, showing significant difference in age groups (P < 0.001). In addition, the patency rate of lacrimal duct irrigation was independent of the childbirth mode (P>0.05), and the patency rate of boys (46.38%) was higher than that of the girls (39.39%, P < 0.05). As for the lacrimal duct probing, the treatment Results showed that the younger the children and the fewer the times of duct irrigation were, the higher the success rate of one-time patency of probing achieved (P < 0.01). Conclusion Earlier lacrimal duct irrigation and probing in the treatment for CNLDO will lead to better therapeutic efficacy. CNLDO can be effectively cured by lacrimal duct probing when repeated lacrimal duct irrigation fails.

ObjectiveGram-negative bacilli (GNB) are common pathogens of infection in severe acute pancreatitis (SAP), and their occurrence increases the mortality of SAP. Early identification of SAP severity and prognosis is of great significance to SAP treatment. This study explored risk factors for mortality in patients with SAP and GNB infection and established a model for early prediction of the risk of death in GNB-infected SAP patients.MethodsPatients diagnosed with SAP from January 1, 2016, to March 31, 2022, were included, and their baseline clinical characteristics were collected. Univariate logistic regression analysis was performed to screen for death related variables, and concurrently, a Boruta analysis was performed to identify potentially important clinical features associated with mortality. The intersection of the two results was taken for further multivariate logistic regression analysis. A logistic regression model was constructed according to the independent risk factor of death and then visualized with a nomogram. The performance of the model was further validated in the training and validation cohort.ResultsA total of 151 patients with SAP developed GNB infections. Univariate logistic regression analysis identified 11 variables associated with mortality. The Boruta analysis identified 11 clinical features, and 4 out of 9 clinical variables: platelet counts (odds ratio [OR] 0.99, 95% confidence interval [CI] 0.99–1.00; p = 0.007), hemoglobin (OR 0.96, 95% CI 0.92–1; p = 0.037), septic shock (OR 6.33, 95% CI 1.12–43.47; p = 0.044), and carbapenem resistance (OR 7.99, 95% CI 1.66–52.37; p = 0.016), shared by both analyses were further selected as independent risk factors by multivariate logistic regression analysis. A nomogram was used to visualize the model. The model demonstrated good performance in both training and validation cohorts with recognition sensitivity and specificity of 96% and 80% in the training cohort and 92.8% and 75% in the validation cohort, respectively.ConclusionThe nomogram can accurately predict the mortality risk of patients with SAP and GNB infection. The clinical application of this model allows early identification of the severity and prognosis for patients with SAP and GNB infection and identification of patients requiring urgent management thus allowing rationalization of treatment options and improvements in clinical outcomes.

TC4/Ni60/Ni-graphite composite functional coatings with 25%(mass fraction), 35% and 45%Ni-graphite were prepared on TC4 titanium alloy surface by coaxial powder-feeding laser cladding technology. The forming quality, microstructure and mechanical properties of the composite coatings were studied by means of penetrant inspection, X-ray diffractometer, scanning electron microscope, energy spectrum analyzer, electron probe microanalyzer, white light interferometer, microhardness tester and friction and wear tester. The results show that the 35%Ni-graphite coating has the best forming quality, and different content Ni-graphite coatings have the same types of reaction precipitates, mainly Ti2Ni, TiC, TiB2, graphite and matrix α-Ti.The TiC coated graphite semi coherent composite phase and TiC, Ti2Ni cross growth coherent composite phase are formed in all coatings, the semi-coherent TiC coating can alleviate the melting of graphite in the molten pool, and the coherent TiC-Ti2Ni composite phase can make the brittle Ti2Ni structure uniformly refined. With the increase of Ni-graphite content, the average microhardness and wear resistance of the coatings decrease gradually, while the anti-friction performance first increases and then decreases, and the wear mechanism is abrasive wear.

Some typical network attacks,such as link-flooding attack,need to be carried out on critical links based on topology reconnaissance,which has strong destructiveness and stealthiness.In order to defense these attacks effectively,TopoObfu,a topology obfuscation mechanism against network reconnaissance,is proposed.TopoObfu can add virtual links to the real network according to the requirements of network topology obfuscation,and provide attacker with fake topology by modifying the forwar-ding rules of probing packets,and hide critical links in the network.To facilitate the implementation,TopoObfu maps the fake topology to the flow table entries used by SDN switches for packet processing,and can be deployed in the hybrid network where only part of the nodes are SDN switches.The simulation analysis based on several typical real network topologies shows that TopoObfu can effectively improve the difficulty of critical links analysis launched by attackers in terms of link importance,network structure entropy,path similarity and so on,and has high implementation efficiency in terms of the number of flow table entries in SDN switches,the generated time of fake topology,and can reduce the probability of critical links being attacked.

Objective To investigate whether miR-205 can regulate wingless-type MMTV integration site family member 5a (Wnt-5a) to inhibit the papillary thyroid carcinoma (PTC) cell proliferation. Methods RT-qPCR was used to detect the expression of miR-205 and Wnt-5a in PTC tissues and adjacent normal tissues.Further correlation analysis between miR-205 and Wnt-5a was carried out. The low expression of miR-205 related to the stage of PTC TMN and lymph node metastasis was also discussed. Meanwhile, RT-qPCR was also used to detect NTHY-OR-3-1 and PTC cell lines (K1 and BCPAP). In addition, the proliferation of PTC cells was detected by CCK-8 after over expression and inhibition of miR-205. Xenograft tumor formation test was used to observe whether inhibition of miR-205 acceler- ates the growth of tumor. The online target gene prediction software and dual luciferase reporter gene test were used to predict and verify whether Wnt-5a is the target gene of miR-205. Results The expression of miR-205 was significantly lower than that of surrounding normal tissues, and the expression of Wnt-5a was significantly higher. TNM stage and lymph node metastasis severity of PTC patients were significantly higher in patients with low miR-205 expression than in patients with high miR-205 expression. Low expression of miR-205 showed a poor survival rate. Expression of miR-205 in NTHY-OR-3-1 was significantly lower than that of K1 and BCPAP. miR-205 inhibited the proliferation of PTC cells. Over-expression of miR-205 also inhibited the growth of PTC. It was confirmed that Wnt-5a was the target gene of miR-205 by luciferase reporter assay. Conclusions miR-205 plays an anti-cancer role in PTC, which may be a potential therapeutic target.

Calcium silicate/calcium alginate (PET-s-CaAlg/CaSiO3@SiO2) composites supported by polyethylene terephthalate fibers were prepared by chemical crosslinking. The PET-s-CaAlg/CaSiO3@SiO2 was characterized by optical microscope, Scanning Electron Microscopy (SEM), Fourier Transform infrared spectroscopy (FTIR) and BET.As can be seen in SEM, mesoporous SiO2 with the size of 150-200 nm appears on the surface of the material. FTIR shows that Si-OH structure has been formed on the surface of PET non-woven fabric.BET analysis showed that the specific surface area of the material increases significantly. The factors affecting the adsorption properties of Pb2+ were simultaneously discussed, such as the initial Pb2+ concentration, adsorption contact time, temperature and pH value. The results show excellent adsorption properties of PET-s-CaAlg/CaSiO3@SiO2for Pb2+ is obtained and the saturated adsorption amount is 58.33 mg/g. Its adsorption kinetics for Pb2+ is in accordance with the quasi second-order kinetic model. Low temperature is conducive to adsorption. The adsorption capacities of Pb2+on PET-s-CaAlg/CaSiO3@SiO2are affected by the pH of the solution through the ionization of surface functional groups and the degree of hydrolysis of Pb2+. It shows that the best adsorption capacities of Pb2+ is at the pH of 4.0-6.0.

As a teacher of medical genetics, the author attended both programs and proposes a new strategy of medical genetics teaching: Pairing andragogy and genomic advances in the era of precision medicine.

Nuclear receptor binding SET domain protein 1 (NSD1), which is a family member of histone methyltransferases, functions to methylate histone H3 on lysine 36 (H3K36). NSD1-related abnormalities are the major cause of Sotos syndrome, and also known to be associated with other human diseases. Inhibitors targeting histone methyltransferases DOT1L and EZH2 have been reported recently. However, no chemical probes targeting NSD1 have been found so far. Here, we identified three hits targeting the NSD1 SET domain using ligand-observed nuclear magnetic resonance (NMR) fragment-based screening. The binding affinities of the hit compounds to the NSD1 SET domain were determined by dose-dependent chemical shift perturbation analysis. Furthermore, the potential binding modes of the hit compounds to NSD1 were obtained by molecular docking. The hit compound 1 was found to bind to the binding pocket of S-adenosylmethionine (SAM), an endogenous ligand of the protein, in the NSD1 SET domain. The study provided valuable information for further structure-guided hit-to-lead evolution towards the potent and specific inhibitors of the NSD1 SET domain.

To improve steady-state and dynamic performance of sensorless control of permanent magnet synchronous motor with medium and high speed, and solve the problem that traditional quadrature phase-locked loop can not operate effectively when the motor is reversed, a sensorless control strategy of interior permanent magnet synchronous motor based on super-twisting sliding-mode observer and improved quadrature phase-locked loop was proposed. In the two-phase stationary coordinate system, the super-twisting function is used to replace switching function of the traditional sliding-mode observer to establish super-twisting sliding-mode observer, which can fully suppress chattering of sliding-mode and avoid using low pass filter, and can solve problems of phase delay and amplitude attenuation caused by low pass filter. The improved quadrature phase-locked loop algorithm avoids use of tangent function and uses a simple reverse electromotive force signal reconstruction strategy to obtain the error signal. Compared with the quadrature phase-locked loop algorithm based on tangent function, the improved algorithm is more simple and effective, and can make the motor reverse operation. In addition, the speed and position estimation error is small in operation process.The simulation results verify reliability and effectiveness of the control strategy.

In view of problems that traditional multi-objective predictive control method based on weighting method cannot effectively deal with contradiction of objectives and complex design of weight coefficient, a multi-objective optimization predictive control method of high-power explosion-proof permanent magnet motor based on two-layer structure compatible framework was proposed. The d-axis current tracking, q-axis current tracking and midpoint potential balance were divided into compatible optimization layers. By introducing boundary interval, compatible control of the three objectives was effectively realized. Switching loss was divided into optimal optimization layer. A switching frequency measurement method based on extension method was proposed. By approximately measuring switching time of multiple control cycles, average switching frequency of the system was effectively reduced. The simulation results show that the proposed method can achieve compatible control of current command tracking and midpoint potential, and can effectively reduce average switching frequency of the system.
