Aim: Hepatocellular carcinoma (HCC) is a major cause of cancer‐related death, with low survival rates worldwide. Fatty liver disease (FLD) significantly contributes to HCC. We studied the screening performance of different methods for identifying HCC in patients with FLD or with metabolic risk factors for FLD. Methods: Korean adults (n = 340 825) without a prior HCC diagnosis were categorized into four groups: normal (G1), ≥2 metabolic risk factors (G2), FLD (G3), and viral liver disease or liver cirrhosis (G4). The National Cancer Registry data were used to identify HCC cases within 12 months. We assessed the area under the receiver operating characteristic curve, sensitivity, specificity, and positive and negative predictive values of individual or combined screening methods. Results: In 93 HCC cases, 71 were identified in G4, whereas 20 cases (21.5%) in G2 and G3 combined where ultrasound and Fibrosis‐4 performed similarly to alpha‐fetoprotein and ultrasound. In G2, Fibrosis‐4 and ultrasound had the highest area under the receiver operating characteristic curve (0.93 [0.87–0.99]), whereas in G3, the combined screening methods had the highest area under the receiver operating characteristic curve (0.98 [0.95–1.00]). The positive predictive value was lower in G2 and G3 than in G4, but was >5% when restricted to a high Fibrosis‐4 score. Conclusions: More than 21% of HCC cases were observed in patients with diagnosed FLD or at risk of FLD with metabolic risk factors. Nevertheless, screening for HCC in individuals without cirrhosis or viral hepatitis yielded very low results, despite the potential value of the Fibrosis‐4 score in identifying individuals at high risk of HCC. ABSTRACT: The causes of variation in the milk mineral profile of dairy cattle during the first phase of lactation were studied under the hypothesis that the milk mineral profile partially reflects the animals' metabolic status. Correlations between the minerals and the main milk constituents (i.e., protein, fat, and lactose percentages), and their associations with the cows' metabolic status indicators were explored. The metabolic status indicators (MET) that we used were blood energy-protein metabolites [nonesterified fatty acids, β-hydroxybutyrate (BHB), glucose, cholesterol, creatinine, and urea], and liver ultrasound measurements (predicted triacylglycerol liver content, portal vein area, portal vein diameter and liver depth). Milk and blood samples, and ultrasound measurements were taken from 295 Holstein cows belonging to 2 herds and in the first 120 d in milk (DIM). Milk mineral contents were determined by ICP-OES; these were considered the response variable and analyzed through a mixed model which included DIM, parity, milk yield, and MET as fixed effects, and the herd/date as a random effect. The MET traits were divided in tertiles. The results showed that milk protein was positively associated with body condition score (BCS) and glucose, and negatively associated with BHB blood content; milk fat was positively associated with BHB content; milk lactose was positively associated with BCS; and Ca, P, K and S were the minerals with the greatest number of associations with the cows' energy indicators, particularly BCS, predicted triacylglycerol liver content, glucose, BHB and urea. We conclude that the protein, fat, lactose, and mineral contents of milk partially reflect the metabolic adaptation of cows during lactation and within 120 DIM. Variations in the milk mineral profile were consistent with changes in the major milk constituents and the metabolic status of cows.

Purpose: To evaluate incidence of hepatic hemangiomas in patient affected by endometriosis. Methods: retrospective analysis of prospectively collected data. 252 consecutive patients, referred to our Center for suspected endometriosis, were studied and underwent surgery according to the same technique. In the preoperative phase, the patients, in addition to the initial evaluation and standard preparation for the operation, were subjected to upper abdominal ultrasonography. Subsequently, all women underwent surgical treatment and laparoscopy was attempted as first-line surgical approach. After surgery, findings at preoperative investigations were compared with surgical and histological results. Results: of the 252 operated patients, 188 (74.60%) had histologically confirmed genitopelvic endometriosis and 64 (25.39%) did not have endometriosis. The comparison of these data with the results of the preoperative abdominal ultrasound showed that in the group of patients with endometriosis 42 (22.34%) were carriers of hepatic hemangiomas while in the group without endometriosis only 7 (10.93%) were found to have hepatic hemangiomas. The prevalence of liver disease was significantly higher in patients with endometriosis (22.3% vs 12.3, P < 0.05). Conclusion: The prevalence of hepatic hemangiomas in patients with endometriosis appears to be significantly higher than in patients without endometriosis. This data, if confirmed by larger series, could suggest the systematic execution of a liver ultrasound in patients with endometriosis and imply important limitations in the hormonal treatment of these patients. Non-alcoholic fatty liver disease (NAFLD) is a common chronic liver condition with significant risk of progression to steatohepatitis and cirrhosis. Therapeutic strategies in NAFLD include lifestyle changes mainly related to dietary interventions and use of drugs or nutritional components that could improve plasma lipid profiles and insulin sensitivity and decrease the local inflammatory response. In this study, we tested the effects of monacolin K, an inhibitor of HMCoA reductase. In a prospective, uncontrolled, open study, we treated 24 patients with NAFLD and mild hypercholesterolemia with 10 mg/day of monacolin K. At baseline and after 26 weeks, we measured in plasma liver tests, lipids, malondialdehyde, and oxidized glutathione, and assessed biochemical steatosis scores, liver elastography, and body composition with bioimpedance analysis. Monacolin K significantly reduced plasma alanine aminotransferase, cholesterol, triglycerides and the homeostatic model assessment (HOMA) index that indicated improved insulin sensitivity. No significant changes were found in body fat mass and visceral fat, nor in liver elastography, while the fatty liver index (FLI) was significantly decreased. Plasma levels of both malondialdehyde and oxidized glutathione were markedly reduced by monacolin K treatment, suggesting a reduction in oxidative stress and lipid peroxidation. In summary, this pilot study suggests possible benefits of monacolin K use in NAFLD patients that could be linked to a reduction in oxidative stress. Hepatic portal venous gas (HPVG) detected by ultrasound (US) following liver transplantation or in suppurative cholangitis was described previously. To our knowledge, there have been no descriptions of HPVG detected by US in acute mesenteric ischemia. Here we present diagnostic images of a 52-year-old female who was admitted to the intensive care unit (ICU) following successful embolization of a ruptured saccular aneurysm of the right vertebral artery. During their stay in the ICU, the patient developed hypotension with low systemic vascular resistance and hypovolemia. Based on physical examination of the abdomen and laboratory results, preliminary diagnosis of intra-abdominal sepsis was made. Early abdominal US was performed to find the source of sepsis. The preliminary diagnosis of stomach/small intestine ischemia was made by ultrasonic detection of HPVG. Other less likely diagnoses were pneumobilia due to cholangitis, hepatic micro-abscesses, and punctuate calcifications. The diagnosis was confirmed by multi-phase abdominal computed tomography. The explorative laparotomy revealed necrosis of the stomach, small intestine, and liver. Due to the severity of necrosis, surgical treatment was abandoned. Provided sonographic images show HPVG as an ominous sign of small intestine and stomach ischemia. Early liver US should be performed whenever intra-abdominal pathology is suspected. Familial combined hyperlipidemia (FCH) is a very common inherited lipid disorder, characterized by a high risk of developing cardiovascular (CV) disease and metabolic complications, including insulin resistance (IR) and type 2 diabetes mellitus (T2DM). The prevalence of non-alcoholic fatty liver disease (NAFLD) is increased in FCH patients, especially in those with IR or T2DM. However, it is unknown how precociously metabolic and cardiovascular complications appear in FCH patients. We aimed to evaluate the prevalence of NAFLD and to assess CV risk in newly diagnosed insulin-sensitive FCH patients. From a database including 16,504 patients, 110 insulin-sensitive FCH patients were selected by general practitioners and referred to the Lipid Center. Lipid profile, fasting plasma glucose and insulin were determined by standard methods. Based on the results of the hospital screening, 96 patients were finally included (mean age 52.2 ± 9.8 years; 44 males, 52 females). All participants underwent carotid ultrasound to assess carotid intima media thickness (cIMT), presence or absence of plaque, and pulse wave velocity (PWV). Liver steatosis was assessed by both hepatic steatosis index (HSI) and abdomen ultrasound (US). Liver fibrosis was non-invasively assessed by transient elastography (TE) and by fibrosis 4 score (FIB-4) index. Carotid plaque was found in 44 out of 96 (45.8%) patients, liver steatosis was found in 68 out of 96 (70.8%) and in 41 out of 96 (42.7%) patients by US examination and HSI, respectively. Overall, 72 subjects (75%) were diagnosed with steatosis by either ultrasound or HSI, while 24 (25%) had steatosis excluded (steatosis excluded by both US and HSI). Patients with liver steatosis had a significantly higher body mass index (BMI) compared to those without (p < 0.05). Steatosis correlated with fasting insulin (p < 0.05), liver stiffness (p < 0.05), BMI (p < 0.001), and inversely with high-density lipoprotein cholesterol (p < 0.05). Fibrosis assessed by TE was significantly associated with BMI (p < 0.001) and cIMT (p < 0.05); fibrosis assessed by FIB-4 was significantly associated with sex (p < 0.05), cIMT (p < 0.05), and atherosclerotic plaque (p < 0.05). The presence of any grade of liver fibrosis was significantly associated with atherosclerotic plaque in the multivariable model, independent of alcohol habit, sex, HSI score, and liver stiffness by TE (OR 6.863, p < 0.001). In our cohort of newly diagnosed, untreated, insulin-sensitive FCH patients we found a high prevalence of liver steatosis. Indeed, the risk of atherosclerotic plaque was significantly increased in patients with liver fibrosis, suggesting a possible connection between liver disease and CV damage in dyslipidemic patients beyond the insulin resistance hypothesis. Objective: The aim of this review is to determine the effect of curcumin on the liver ultrasonographic morphology, and the effectiveness of curcumin as adjuvant treatment for NAFLD. Methods: The Cochrane library and PubMed were searched systematically to identify randomized controlled trials from 2000 to January 2021. The primary outcomes were NAFLD severity, liver steatosis resolution, liver scarring, liver enzymes, also lipid profiles. 16 RCTs with a total of 1028 participants were included in the meta-analysis. Results: Curcumin improved NAFLD severity (RR: 3.52, 95 % CI 1.27–9.72; P = 0.02) and increased the liver steatosis resolution (RR 3.96, 95 % CI 1.54–10.17; P = 0.004) based on the liver ultrasonographic finding. During thermal ablation in a target tissue the information about temperature is crucial for decision making of successful therapy. An observable temporal and spatial temperature propagation would give a visual feedback of irreversible cell damage of the target tissue. Potential temperature features in ultrasound (US) B-Mode image sequences during radiofrequency (RF) ablation in ex-vivo porcine liver were found and analysed. These features could help to detect the transition between reversible and irreversible damage of the ablated target tissue. Experimental RF ablations of ex-vivo porcine liver were imaged with US B-Mode imaging and image sequences were recorded. Temperature was simultaneously measured within the liver tissue around a bipolar RF needle electrode. In the B-Mode images, regions of interest (ROIs) around the centre of the measurement spots were analysed in post-processing using average gray-level (AVGL) compared against temperature. The pole of maximum energy level in the time-frequency domain of the AVGL changes was investigated in relation to the measured temperatures. Frequency shifts of the pole were observed which could be related to transitions between the states of tissue damage.

This research presents a machine vision approach to detect lesions in liver ultrasound as well as resolving some issues in ultrasound such as artifacts, speckle noise, and blurring effect. The anisotropic diffusion is modified using the edge preservation conditions which found better than traditional ones in quantitative evolution. To dig for more potential information, a learnable super-resolution (SR) is embedded into the deep CNN. The feature is fused using Gabor Wavelet Transform (GWT) and Local Binary Pattern (LBP) with a pre-trained deep CNN model. Moreover, we propose a Bayes rule-based informative patch selection approach to reduce the processing time with the selective image patches and design an algorithm to mark the lesion region from identified ultrasound image patches. To train this model, standard data ensures promising resolution. The testing phase considers generalized data with a varying resolution and test the performance of the model. Exploring cross-validation, it finds that a 5-fold strategy can successfully eradicate the overfitting problem. Experiment data are collected using 298 consecutive ultrasounds comprising 15,296 image patches. This proposed feature fusion technique confirms satisfactory performance compared to the current relevant works with an accuracy of 98.40%.

We calculated unpaired t-test and Pearson’s coefficient, and areas under the receiver-operating characteristic curves (AUROC) for the Fatty Liver Index (FLI), Lipid Accumulation Product (LAP) indexes, right liver index diameter, and for predictive models constructed with discriminant analysis. Results: One hundred patients in groups with and without NAFLD. FLI, LAP, right and caudate liver lobe diameters, and congestion index were higher in NAFLD group (p = 0.011, p = 0.011, p = 0.001, p = 0.027, p = 0.009). The right liver lobe diameter had the highest AUROC. Predictive models that combined sensitivity and specificity for the clinical indexes and liver ultrasound had an AUROC over 0.7. Conclusion: The ultrasonography measure of right liver lobe diameter by itself can reliably identify patients with NAFLD with a good sensitivity and specificity, however, this can be improved by adding the LAP mathematical index in our population.

Introduction: Leptospirosis is an important zoonotic disease commonly found in tropical or sub-tropical countries. The most severe form is Weil's syndrome which presents with jaundice, renal failure, and bleeding diatheses. Although jaundice occurs in 38% of patients with leptospirosis, no studies in Asia have focused on the liver biochemical profile of these patients. Characterization of liver biochemical profile and ultrasonographic findings may shed more light on the disease process. Identification of liver biochemical parameters that portend a poor prognosis may also allow for early aggressive intervention., Objective: To describe the liver biochemical profile and liver ultrasonographic findings in adult patients with laboratory-confirmed leptospirosis, admitted at a tertiary hospital in Manila, Philippines. The association of clinical and laboratory features with clinical outcomes (i.e., severe liver injury, Weil's syndrome, and mortality) was also investigated., Methods: This retrospective cross-sectional study reviewed all available cases of adult patients with laboratory-confirmed leptospirosis admitted in the Philippine General Hospital from January 2009 to August 2018. The clinical features, liver biochemical profiles, and ultrasound findings were recorded and analyzed. Comparison between the means of each group based on clinical outcome (i.e., mortality, Weil's syndrome) was done via Students' t-test for continuous variables, and calculation of the Odds Ratio for categorical variables., Results: Total and direct bilirubin levels were elevated in patients with leptospirosis compared to serum amino-transferases and alkaline phosphatase levels which were only mildly elevated. Abdominal ultrasound showed typically un-enlarged livers with normal parenchymal echogenicity, normal spleens, and non-dilated biliary trees. Dyspnea was associated with an increased odds for mortality. Although jaundice was present in 39.5% of patients and significantly associated with severe liver injury, this was not associated with mortality. Liver biochemical test values did not differ among patients who expired and those who survived to discharge. The presence of myalgia and abdominal pain increased the odds for Weil's syndrome., Conclusion: To date, no local studies have fully described the liver biochemical profile of patients with leptospirosis. Our findings are compatible with previous studies showing that leptospirosis typically presents with predominantly elevated direct bilirubin from cholestasis and systemic infection. Aims: To evaluate and compare lipid accumulation product (LAP) with alanine aminotransferase (ALT), aspartate aminotransferase (AST), visceral adiposity index (VAI) and triglyceride-glucose index (TyG) as biomarkers for hepatic steatosis and nonalcoholic fatty liver disease (NAFLD). Methods: LAP, ALT, AST, VAI and TyG were measured in 52 biopsy-proven NAFLD patients and 21 control subjects. Additionally, LAP was also measured in 448 ultrasound-proven NAFLD patients and 1009 control subjects. Results: LAP was positively associated with hepatic steatosis and inflammation in biopsy-proven NAFLD. The risk of NAFLD was positively related to LAP and TyG, but LAP showed a better area under the receiver operating characteristic curve for hepatic steatosis and NAFLD. LAP also performed well in recognizing ultrasound-proven NAFLD. Conclusion: LAP is an ideal biomarker of hepatic steatosis and NAFLD.

Introduction: metabolic steatopathy or nonalcoholic fatty liver disease (NAFLD) is frequently associated with type 2 diabetes mellitus (T2DM) with an increased risk of progression to advanced fibrosis. The purpose of our study was to determine the interest of hepatic steatosis index (HSI) in the detection of hepatic steatosis in patients with type 2 diabetes in order to establish an appropriate screening program of this disease in our population. Methods: cross-sectional study involving 281 type 2 diabetics hospitalized in the Department of Endocrinology in collaboration with the Hepatogastroenterology Department at the University Hospital Ibn Rochd Casablanca between January 2018 and June 2018. Anthropometric variables studied were, biological, hepatic steatosis index (HSI) and liver ultrasound. The HSI score of> 36 predicted the presence of fatty liver. The HSI score (fatty liver index) was calculated for all patients using the following formula: 8 × (ALT / AST) + BMI + 2 (if type 2 diabetes) + 2 (if female). Statistical analysis was performed with SPSS Version 19 software. The sensitivity and the specificity of the HSI score were calculated by 2x2 contingency table. The area under the receiver operating characteristic curve (AUROC) was also analyzed. Results: average age of patients was 54.15 ± 13.14 years with a female predominance (76.9% of cases), and a sex ratio of 3.32. Mean duration of diabetes of 10.5 ± 8.03 years with an average glycated hemoglobin of 10.23 ± 1.96%. BMI was 29.53 ± 4.55 kg/m², the average waist circumference was 99.51 ± 10.98 cm. 39.1% of patients were hypertensive, 58% were dyslipidemic. Abnormalities in transaminases were found in 6% of patients. Prevalence of NAFLD was 45.2% based on the HSI score > 36. This prevalence is consistent with the findings made by the liver ultrasound (47.7% of cases). Hepatic steatosis was significantly correlated with dyslipidemia (P=0.006), overweight (P=0.00015), obesity (P=0.001) and hypertriglyceridemia (P=0.0003). The sensitivity of HSI was 89.55%, negative predictive value (NPV) was 90.91%, specificity was 95.24%, and positive predictive value (PPV) was 94.49%. AUROC for HSI was at 0.979 (95% CI, 0.962-0.997). Conclusion: hepatic steatosis is common among our patients; it is correlated with dyslipidemia, obesity and hypertriglyceridemia. BACKGROUND: Contrast-enhanced ultrasound (CEUS) has been used as an additional imaging technique in order to evaluate focal liver lesions. CEUS is easy and fast to perform, overcomes the limitations of B-mode US. OBJECTIVE: To evaluate the diagnostic potential of contrast-enhanced ultrasound (CEUS) in focal liver lesions (FLL) with unclear findings on computed tomography (CT) or magnetic resonance imaging (MRI). METHODS: In this single-center retrospective study, 146 patients with at least one FLL underwent additional CEUS of the liver to clarify inconclusive cross-sectional imaging findings. Ultrasonography was performed using B-mode imaging and CEUS after injection of sulphur hexafluoride microbubbles (second-generation contrast agent) within two months after ceMRI or ceCT and interpreted by an experienced radiologist (EFSUMB level 3). Histopathological reports, long-term follow-up or clinical course served as reference standard. RESULTS: Thirty-eight of the 146 patients had malignant and 108 benign FLL. Overall, CEUS had 92% sensitivity (95% -CI, 79–97) and 98% specificity (95% -CI, 93–99) with an AUC of 0.95 in correctly characterizing the lesion as malignant or benign. Sensitivity increased to 98% (95% -CI, 96–100) for benign lesions and decreased to 92% (95% -CI, 78–98) for malignant lesions. CEUS showed the highest diagnostical accuracy in lesion <1 cm with an AUC of 1.000, while the lowest accuracy was achieved in lesions >2 cm with an AUC of 0.924 due to a decreasing specificity of 86% (95% -CI, 87–100). CONCLUSION: CEUS correctly distinguished malignant from benign FLL in cases with inconclusive cross-sectional imaging findings and achieved high levels of diagnostic accuracy. • A novel anatomy-preserved GAN (ApGAN) is developed for generating the simulated intraoperative ultrasound of liver without any preprocessing steps. • The simulated intraoperative ultrasound has high-level precision of anatomical structures and focal areas. • Empirical results of clinical data showed that the proposed method obtained higher accuracy compared to existing methods. In ultrasound-guided liver surgery, the lack of large-scale intraoperative ultrasound images with important anatomical structures remains an obstacle hindering the successful application of AI to ultrasound guidance. In this case, intraoperative ultrasound (iUS) simulation should be conducted from preoperative magnetic resonance (pMR), which not only helps doctors understand the characteristics of iUS in advance, but also expands the iUS dataset from various imaging positions, thereby promoting the automatic iUS analysis in ultrasound guidance. Herein, a novel anatomy preserving generative adversarial network (ApGAN) framework was proposed to generate simulated intraoperative ultrasound (Sim-iUS) of liver with precise structure information from pMR. Specifically, the low-rank factors based bimodal fusion was first established focusing on the effective information of hepatic parenchyma. Then, a deformation field based correction module was introduced to learn and correct the slight structural distortion from surgical operations. Meanwhile, the multiple loss functions were designed to constrain the simulation of the content, structures, and style. Empirical results of clinical data showed that the proposed ApGAN obtained higher Structural Similarity (SSIM) of 0.74 and Fr´echet Inception Distance (FID) of 35.54 compared to existing methods. Furthermore, the average Hausdorff Distance (HD) error of the liver capsule structure was less than 0.25 mm, and the average relative (Euclidean Distance) ED error for polyps was 0.12 mm, indicating the high-level precision of this ApGAN in simulating the anatomical structures and focal areas. During the transition period, the cow's body activates adaptive mechanisms aimed at adjusting to the changing demand for energy and nutrients, which are necessary for the growing fetus and the subsequent start of milk production. This time is also associated with an increased risk of metabolic diseases and reproductive disorders. Our study aimed to identify prepartum and postpartum biochemical markers and weight loss patterns that could differentiate cows that would exhibit ultrasonographic signs of liver fatty infiltration during the latter half of the transition period. The study was performed in a single herd of Holstein-Friesian cows and the animals were divided into two groups: CON (n=13) - cows without ultrasonographic signs of fatty liver, and FL (n=16) - cows with ultrasonographic signs of fatty liver. Backfat thickness and specific biochemical parameters were measured weekly from one week before parturition to 9 weeks postpartum. Our study highlights the importance of using a combination of monitoring methods to assess the metabolic status of transition dairy cattle. The results showed that ultrasound measurements of backfat thickness, blood NEFA levels, glucose concentration, and AST activity were all different (p<0.05) between the control and FL groups, indicating the usefulness of these parameters in monitoring the health status of transition cows. Additionally, the results suggest that high prepartum glucose levels (4.99 mmol/l) could serve as a potential marker for future FL, while the elevated NEFA levels (0.51 mmol/l) and decreased AST activity (80.56 u/l) in FL animals indicate their potential as indicators of lipid mobilization and liver structural damage, respectively.

Background and Hypotheses: High venous pressures and associated hepatic congestion are important drivers for Fontan-associated liver disease. The prognostic significance of hepatomegaly as a marker of congestion however is not well defined and is further explored in this research study., Methods: Fontan patients who have had liver ultrasound scans were identified from the Prince Sultan Cardiac Centre Fontan Database and had their anatomic, surgical, clinical histories abstracted from the electronic medical records following institutional ethics approval. Liver volumes were determined retrospectively from reviewing individual US images, and these, divided into tertiles, were analysed in the context of the predefined endpoints of (i) Primary - death or heart or liver transplantation, or (ii) Secondary - combined endpoint of death, transplantation, arrhythmia, or protein-losing enteropathy., Results: Mean indexed liver volumes for the entire cohort (n = 199) were 1065.1 ± 312.1 ml/m 2 , range 387 to 2071 ml/m 2 . Patients with the largest liver volumes (highest tertile) were less likely to have a functioning fenestration compared to those in the lowest tertile 44% versus 56% p = 0.016 and experienced the highest burden of mortality and heart or heart-liver transplantation, p = 0.016, and were more likely to reach the composite endpoint of death, protein-losing enteropathy, arrhythmia, or transplantation, p = 0.010. Liver volumes had an overall predictive accuracy for the combined outcome of 61% (CI 53%, 67%, p = 0.009)., Conclusions: Liver volumetry may serve as a potentially important congestion biomarker for adverse outcomes after the Fontan operation.

Cardiogenic liver disease is a common yet poorly characterized complication of advanced heart failure (HF), and may impact clinical management in the setting of heart transplant evaluation. In this retrospective study, we describe clinical and histopathological features of liver injury in advanced HF, with a focus on the role of liver biopsy. Included were 45 HF patients, assessed for possible heart transplant, who underwent liver biopsy for suspected liver disease. Median duration of HF symptoms was 5 years. Most patients had stiff hepatomegaly and elevated bilirubin. Viral hepatitis (19 patients, 42.2%) was the most common cause of prior known liver disease. Sinusoidal dilatation was detected in the majority of patients (64.4%). Median necroinflammatory index was 3 and median fibrosis was 1, consistent with a small burden of histologically proven liver disease. Viral hepatitis was the only variable associated with a higher grade of necroinflammation and fibrosis. Nine of the 14 (64.3%) advanced fibrosis/cirrhosis patients had a viral hepatitis infection. Fibrosis was significantly associated with splenomegaly. The MELD score was not correlated with cardiac index. A coarse liver echo-pattern had a 29% positive and 63% negative predictive value for advanced fibrosis/cirrhosis. Severe liver disease is uncommon in patients with advanced HF in the absence of splenomegaly or primary causes of liver disease. Ultrasound data need to be carefully evaluated, as it may overstate the severity of liver disease. Purpose: The purpose of this study was to evaluate the relationship between final outcome of lesions indeterminate for malignancy on ultrasound (US) and patient and imaging characteristics.Methods: We identified all patients with indeterminate liver lesions on US between 9/1/2013 and 12/31/2014 using institutional codes based on radiologist opinion. Miscoded lesions (n = 30) and patients with no imaging, pathology, or clinical follow-up at our health system (n = 6) were excluded. Final diagnostic category of malignant, benign, pseudolesion, or indeterminate was assigned using imaging, pathology, and clinical follow-up. Differences in diagnostic categories were compared by patient (age, gender, race, known malignancy. or liver disease) and imaging characteristics (lesion size, echogenicity. and number). Independent likelihood of a benign final diagnostic category was adjusted for significant variables on univariate analysis.Results: Indeterminate liver lesions on US were found in 153/6813 patients (2%). Final diagnostic categories were malignant (11/153, 7%), benign (94/153, 61%), pseudolesion (42/153, 27%). and indeterminate (6/153, 4%). Nearly one-third of hypoechoic masses in patients with known malignancy or liver disease (i.e., high-risk status) ≥ 46 years of age were malignant (9/28, 32%). On multivariate analysis, patients of age ≥ 61 years and high-risk status were associated with decreased likelihood of benign diagnostic category (OR.19 (95% CI.07-.51) and OR.40 (95% CI.18-.88), p values.001 and.022, respectively).Conclusions: 2% of patients undergoing abdominal US have sonographically indeterminate liver lesions, of which 7% are malignant. Older, high-risk patients with hypoechoic lesions should receive short-term follow-up as one-third will have malignant lesions. Pemeriksaan ultrasonografi (USG) dilakukan untuk mendeteksi adanya kelainan organ seperti gambaran ekhostruktur, ukuran, permukaan hepar dan vesika felea. Tujuan penelitian ini adalah untuk mengetahui korelasi antara gambaran USG hepar dengan kadar alkali fosfatase pada pasien klinis hepatitis. Jenis penelitian ini observasional analitik dengan desain cross sectional, menggunakan data rekam medis. Data penelitian berjumlah 35. Analisis data menggunakan uji Spearman dan uji Lambda. Hasil penelitian menunjukkan terdapat korelasi antara kadar alkali fosfatase dengan gambaran USG hepar yang meliputi: ekhostruktur (r=0,094, p=0,590), ukuran (r=0,333, p=0,050) dan permukaan hepar (r=0,324, p=0 ,057), vesika felea (r=0,615, p=0 ,001). Disimpulkan bahwa tidak terdapat korelasi yang bermakna antara gambaran ekhostruktur, permukaan, dan ukuran hepar dengan kadar alkali fosfatase, tapi terdapat korelasi yang bermakna antara gambaran vesika felea dengan kadar alkali fosfatase pada pasien klinis hepatitis. It can concluded that there was no significant correlation between the image echostruktur, surface, and the size of the liver with levels of Alkaline Phosphatase, but there is a significant correlation between the vesica fellea features with Alkaline Phosphatase levels in patients with clinical hepatitis.

Background: Acoustic radiation force impulse (ARFI) elastography has been used to assess liver stiffness non-invasively. However, its usefulness in alpha-1 antitripsin-deficient (AATD) individuals is unknown. Purpose: To assess if liver fibrosis is present in a cohort of AATD individuals using ARFI elastography. Material and Methods: Eighty-three participants aged 38-39 years, except for two who were aged 40 years, underwent ultrasound elastography of the liver with ARFI technique. Twenty-nine were homozygote ZZ genotype, PiZZ (14 men, 15 women); 12 were SZ genotype, Pi SZ (8 men, 4 women), and 42 were healthy volunteers, PiMM (16 men, 26 women). Three specific liver anatomical regions were examined: segments 2/3 (left lobe) in the subcostal plane, and 5/6 and 7/8 (right lobe) in the intercostal space. In each region, three measurements were registered. Results: There was no statistically significant difference between ARFI-median in the AATD group and the control group (P value=0.877) and neither between AATD groups (PiZZ and PiSZ) with a P value=0.259. The ARFI-median was lower in the right liver lobe than in the left lobe in all groups and the difference between both lobes was statistically significant (P=0.001). No statistically significant difference was found in ARFI-median of the right liver lobe between the AATD group and the control group (P=0.759), nor between the AATD group (P=0.384). Background. Liver disease continues to be a major cause of morbidity and mortality in sub-Saharan Africa, including Nigeria, due to the high endemicity of viral hepatitis B. However non-alcoholic fatty liver disease may be an important contributory factor. The impact of fatty liver disease in our region has not been evaluated.AIM. To determine the prevalence of non-alcoholic fatty liver disease (NAFLD) among a population of diabetic (DM) subjects attending the endocrine clinic of LASUTH compared with non-diabetic subjects; ascertain other contributing factors and compare the occurrence of the metabolic syndrome in subjects with and without NAFLD.Methodology. Consecutive patients who satisfy the study criteria were enrolled. An investigator-administered questionnaire was used to determine symptoms of liver disease, followed by physical examination to obtain anthropometric indices as well as signs of liver disease. Abdominal scan was performed to determine radiologic evidence of fatty liver and fasting blood samples were collected from for the measurement of fasting lipid profile, glucose, liver biochemistry and serology for hepatitis B and C markers.Results. One hundred and fifty subjects, mean age 56years (standard deviation = 9, range 20-80 yr) and gender ratio (F: M) of 83:67(55%:45%), were recruited. 106 were diabetics and 44 non-diabetics. The overall prevalence of NAFLD amongst all study subjects was 8.7%. The prevalence rate of NAFLD was higher in the DM cases than in the Control subjects but this difference was not statistically significant (9.5 vs. 4.5%, p = 0.2). Only one of the subjects with fatty liver disease had elevated transaminase levels (steatohepatitis) and also had type 2 DM. Central obesity as measured by waist circumference (WC) and SGPT levels were significantly higher in people with fatty liver. The mean body mass index (BMI) of diabetic and non-diabetic patients was similar (31 vs. 30 kg/m2). The prevalence of the metabolic syndrome was higher in the subjects with NAFLD than in those without fatty liver disease but this difference was not statistically significant (p = 0.8).Conclusion. Non-alcoholic fatty liver disease is present in Africa but is less than what one would expect based on American and European studies.

OBJECTIVE: The aim of this study is to analyze the correlation between liver ultrasonic quantitative detection indexes of patients with chronic hepatitis B and cirrhosis and hepatic hydrothorax. PATIENTS AND METHODS: We selected 56 cases of patients with chronic hepatitis B and cirrhosis combined with hepatic hydrothorax in our hospital from March 2013 to March 2015. Also, we selected 63 cases of patients with cirrhosis without hepatic hydrothorax at the same time. The comparison of liver ultrasonic quantitative detection indexes and positions, quantities and proportions of hepatic hydrothorax of patients in these two groups was conducted. RESULTS: In compare to the index of liver size and the level of ALP in two groups, there were not significant differences (p>0.05). In the observation group, the diameter of portal vein, the blood flow velocity of portal vein and the level of serum albumin significantly decreased in comparison to the control group (p<0.05). For patients with hydrothorax in both sides of the observation group, the diameter of portal vein, blood flow velocity of portal vein and level of serum albumin significantly decreased while comparing to the patients with hydrothorax in right (p<0.05). The volume of hydrothorax was negatively related to the diameter of portal vein, blood flow velocity of portal vein and the level of serum albumin, while the proportion of hydrothorax was not related to the diameter of portal vein, the blood flow velocity of portal vein and the level of serum albumin. CONCLUSIONS: The liver ultrasonic quantitative detection indexes of patients with chronic hepatitis B and cirrhosis, diameter of portal vein, blood flow velocity of portal vein and level of serum albumin might be related to the volume of hepatic hydrothorax.
