As a side effect, they can alter intraocular pressure (IOP). The purpose of this study is to describe the incidence, severity, and management of ocular hypertension following the administration of multiple intravitreal injections of dexamethasone implants.Materials and Methods: A total of 78 eyes of 78 subjects (males 62%; females 38%; mean age 67 ± 13 years SD) received a total of 152 intravitreal injections of 0.7 mg dexamethasone implants over 4 years. Indications included retinal vein occlusion (87%), diabetic macular edema (9%), wet-type age-related macular degeneration (4%). Ocular hypertension was defined as intraocular pressure above 23 mmHg or any pressure increase of 10 mmHg or more from baseline values. IOP was measured by applanation tonometry before the injection (T0), as well as one week (T1), one month (T2), and three months (T3) afterwards.Results: Five percent (4/78) of subjects developed ocular hypertension after the 1st injection. On the second and third rounds, additional 7.2% (3/42) and 4.2% (1/24) of subjects developed the same side effect. Among the 8 subjects who received a fourth injection, none was found with OHT. Pressure elevations were detected at T2 and T3. In all patients, topical medical therapy was sufficient to lower the IOP below threshold. Mean pressure variations following the first injection as compared to previous recorded values were +0.97 mmHg (T1), +0.92 mmHg (T2), and − 0.41 mmHg (T3) (p < 0.05). Mean pressure variations following the second injection were +0.54 mmHg (T1), +0.23 mmHg (T2) and − 0.66 mmHg (T3) (p < 0.05).Conclusion: Ocular hypertension is a recognized side effect of intravitreal dexamethasone. Some patients develop it right after the first injection, while others develop it subsequently, on the 2nd or 3rd round. This side effect becomes most apparent 30– 90 days following the implantation procedure and responds well to topical pressure-lowering medications.Keywords: intravitreal dexamethasone implant, Ozurdex injection, hypertension, side effect, complications, retinal vein occlusion

Abstract

Background Acutely ill medical patients experience deep venous thrombosis (DVT) during the hospitalization, however the time course of DVT is still unclear. Objectives To evaluate risk factors in acutely ill hospitalized medical patients for proximal asymptomatic DVT (ADVT) and symptomatic DVT (SDVT) at admission and discharge. Patients/Methods In this prospective observational study, consecutive acutely ill medical patients (hospitalized mainly for acute medical disease as infections, neoplasm, anemia, heart failure) underwent compression ultrasonography (CUS) of proximal lower limb veins within 48 h from admission and at discharge to diagnose ADVT and SDVT. Covid-19 patients, anticoagulant therapy, surgical procedures, acute SDVT, and acute pulmonary embolism, were exclusion criteria. Biographical characteristics at hospitalization, D-Dimer (assessed by ELISA)) and DD-improve score. Results Of 2,100 patients (1002 females, 998 males, age 71 ± 16 years) 58 (2.7%) had proximal ADVT at admission. Logistic regression analysis showed that age, and active cancer were independently associated with ADVT at admission. The median length of hospitalization was 10 days [interquartile range: 6–15]. During the hospital stay, 6 patients (0.3%) with a negative CUS at admission experienced DVT (2 SDVT and 4 ADVT). In the subgroup of patients (n = 1118), in whom D-dimer was measured at admission, D-Dimer and IMPROVE-DD score were associated with ADVT at admission (n = 37) and with all DVT (n = 42) at discharge. ROC curve defined an IMPROVE-DD score of 2.5 as the optimal cut-off for discriminating patients with and without thrombotic events. Conclusions We provide evidence of early development of ADVT in unselected acutely ill medical patients suggesting the need of investigating patients by CUS immediately after hospital admission (within 48 h). Advanced age, active cancer, known thrombophilia and increased IMPROVE-DD score may identify patients at risk. The benefit of anticoagulation needs to be investigated in patients with these specific risk factors and negative CUS at admission. Trial registration NCT03157843.

Abstract

Background and aims: Vascular disease (VD), as assessed by history of myocardial infarction or peripheral artery disease or aortic plaque, increases stroke risk in atrial fibrillation (AF), and is a component of risk assessment using the CHA2DS2-VASc score. We investigated if systemic atherosclerosis as detected by ultrasound carotid plaque (CP) could improve the predictive value of the CHA2DS2-VASC score. Methods: We analysed data from the ARAPACIS study, an observational study including 2027 Ialian patents with non-valvular AF, in whom CP was detected using Doppler Ultrasonography. Results: VD was reported in 351 (17.3%) patients while CP was detected in 16.6% patents. Adding CP to the VD definition leaded to higher VD prevalence (30.9%). During a median [IQR] follow-up time of 36 months, 56 (2.8%) stroke/TIA events were recorded. Survival analysis showed that conventional VD alone did not increase the risk of stroke (Log-Rank: 0.009, p = 0.924), while addition of CP to conventional VD was significantly associated to an increased risk of stroke (LR: 5.730, p = 0.017). Cox regression analysis showed that VD + CP was independently associated with stroke (HR: 1.78, 95% CI: 1.05-3.01, p = 0.0318). Reclassification analysis showed that VD + CP allowed a significant risk reclassification when compared to VD alone in predicting stroke at 36 months (NRI: 0.192, 95% CI: 0.028-0.323, p = 0.032). Conclusions: In non-valvular AF patients the addition of ultrasound detection of carotid plaque to conventional VD significantly increases the pedictive value of CHA2DS2-VASc score for stroke.

21. ROLUL DISBIOZEI INTESTINALE ÎN DISFUNCȚIA ENDOTELIALĂ LA PACIENȚII CU ANGINĂ MICROVASCULARĂ

Abstract

Scop. Evaluarea nivelurilor circulante ale lipopolizaharidei și zonulinei în contiguitate cu markerii disfuncției endoteliale, inflamației și stresului oxidativ la pacienții cu angină microvasculară (AM). Material și metode. Studiul s-a realizat pe un lot de 58 de pacienți cu AM spitalizați în Institutul de Cardiologie. Determinarea nivelurilor circulante ale 20 de biomarkeri s-a realizat în cooperare cu centrul de investigare de laborator din Universitatea Sapienza (Italia). Toți markerii funcționali și biochimici menționați au fost determinați și la 48 de persoane aparent sănătoase (lotul control) cu valorile cărora s-au comparat markerii pacienților cu AM. Rezultate. Disfuncția endotelială la pacienții cu AM a excelat prin creșterea grosimii complexului intimă-medie a a.carotide cu 41%, precum și reducerea valorii dilatării arterei brahiale mediate de flux (FMD) cu 31,6%. Prezența disbiozei s-a manifestat prin creșterea cu 80% a conținutului seric al lipopolizaharidelor și prin dublarea zonulinei (1,8±0,3 vs 3,6±0,7 ng/ml). Disfuncția endotelială și disbioza au evoluat pe fundalul activării stresului oxidativ estimat prin intermediul a 6 markeri și majorarea conținutului seric al 6 importanți markeri proinflamatori (hsCRP, IL-6, TNF-α, etc.) Concluzii. 1. La pacienții cu AM s-au constatat niveluri circulante elevate mai mult ca dublu ale LPZ și zonulinei față de valoarea control, fapt ce indică asupra prezenței disbiozei intestinale. 2. LPZ și zonulina se corelează robust cu markerii morfofuncționali și biochimici ai disfuncției endoteliale, precum și cu markerii factorilor patogenetici principali ai acesteia, inflamației și stresului oxidativ.

Abstract

OBJECTIVE: Carotid intima-media thickness (cIMT) is a surrogate marker of subclinical atherosclerosis and it is able to predict both coronary and cerebral vascular events. No data exist on the association between cIMT and non valvular atrial fibrillation (NVAF) type. We conduct this study with the aim to analyze the association between abnormal cIMT and NVAF type. METHODS: A cross-sectional study of the "Atrial fibrillation Registry for Ankle-brachial index Prevalence Assessment-Collaborative Italian Study (ARAPACIS)" has been performed. Among 2027 patients enrolled in the ARAPACIS, 673 patients, who underwent carotid ultrasound examination to assess cIMT, were included in the study. RESULTS: Among the entire population, 478 patients (71%) had cIMT > 0.90 mm. Patients with an abnormal cIMT (>0.90 mm) were significantly older and more likely hypertensive, diabetic and with a previous history of stroke than those with normal cIMT (≤0.90 mm). These patients had more permanent/persistent NVAF and CHA2DS2-VASc score ≥ 2 (p < 0.0001) compared to those with cIMT

23. IMPACT OF HEAT–NOT–BURN CIGARETTE PASSIVE SMOKING ON CHILDREN‘S OXIDATIVE STRESS, ENDOTHELIAL AND PLATELET FUNCTION

Abstract

Growing global use of heat–not–burn cigarettes (HNBC) prompts investigation. Prior studies assessed HNBC‘s effects on cardiovascular health, revealing heightened oxidative stress, platelet activation, and endothelial dysfunction. However, limited understanding exists regarding passive smoking‘s impact on children exposed to HNBC.This study aims to assess levels of oxidative stress, endothelial and platelet function among children exposed to passive smoke from HNBC, traditional tobacco (TT) cigarettes and unexposed subjects. Seventy–eight children (2–18 years) were divided into three groups: HNBC passive smokers (n=26), TT cigarette exposed (n=26), and control (CNT) group (n=26, unexposed).Oxidative stress was evaluated by serum NADPH oxidase–2 (NOX2) activity, assessed by soluble Nox2–derived peptide (sNOX2–dp), isoprostanes, hydrogen peroxide (H2O2) production, hydrogen break–down activity (HBA) and NO bioavailability. Endothelial function was assessed by brachial flow–mediated dilation (FMD). Platelet function was evaluated by soluble CD40 ligand (sCD40L), soluble P–selectin (sP–selectin) and thrombus formation by T–TAS analysis. Passive smoking–exposed children (both HNBC and TT) exhibited significantly increased serum sNOX2–dp, isoprostanes, H2O2, sCD40L sP–selectin and thrombus formation versus controls. Conversely, exposed children displayed reduced brachial FMD and serum NO bioavailability. No significant differences were found between children exposed to passive smoking of HNBC vs TT.Multivariable regression linked sNOX2 (standardized coefficient β:–0.284; SE: 0.040; p=0.01) and H2O2 (standardized coefficient β:–0.243; SE: 0.0; p=0.02) as independent predictors of FMD, and isoprostanes (standardized coefficient β:0.388; SE: 0.022; p<0.001) and serum cotinine (standardized coefficient β:0.270; SE: 0.048; p=0.01) with sNOX2–dp levels.Exposure to HNBC smoke heightened oxidative stress, endothelial dysfunction, platelet activation, and thrombus formation in children. Findings suggest avenues for interventions to curb childhood passive smoking exposure.

Abstract

AIMS: The aim of this consensus paper is to review the available evidence on the association between moderate alcohol use, health and disease and to provide a working document to the scientific and health professional communities. DATA SYNTHESIS: In healthy adults and in the elderly, spontaneous consumption of alcoholic beverages within 30 g ethanol/d for men and 15 g/d for women is to be considered acceptable and do not deserve intervention by the primary care physician or the health professional in charge. Patients with increased risk for specific diseases, for example, women with familiar history of breast cancer, or subjects with familiar history of early cardiovascular disease, or cardiovascular patients should discuss with their physician their drinking habits. No abstainer should be advised to drink for health reasons. Alcohol use must be discouraged in specific physiological or personal situations or in selected age classes (children and adolescents, pregnant and lactating women and recovering alcoholics). Moreover, the possible interactions between alcohol and acute or chronic drug use must be discussed with the primary care physician. CONCLUSIONS: The choice to consume alcohol should be based on individual considerations, taking into account the influence on health and diet, the risk of alcoholism and abuse, the effect on behaviour and other factors that may vary with age and lifestyle. Moderation in drinking and development of an associated lifestyle culture should be fostered.

Abstract

BACKGROUND: NADPH oxidase is believed to modulate arterial tone, but its role in humans is still unclear. The objective of this study was to evaluate whether NADPH oxidase is involved in flow-mediated arterial dilation (FMD). METHODS AND RESULTS: Twenty-five patients with hereditary deficiency of gp91(phox), the catalytic core of NADPH oxidase, (X-CGD), 25 healthy subjects, and 25 obese patients matched for sex and age were recruited. Abstract

Visual acuity in congenital nystagmus has proven to be primarily related to the duration of foveation periods, during which the image of a target falls onto the fovea and eye velocity slows down. It was found that the longer the foveation time the higher the visual acuity. However, the cycle-to-cycle variability of the eye position and velocity during foveation periods also contribute to visual acuity. A high variability of the eye position during the foveations hinders a stable placement of the target image on the centralmost fovea and consequently decreases visual acuity. To investigate the relationship between different nystagmus features and visual acuity, infrared- oculographic and electro-oculographic eye position recordings of 20 patients affected by congenital nystagmus were analysed in different gaze positions. In several patients' recordings, a high variability of the eye position during foveations (i.e. greater than 0.5 degrees) was detected. Correspondingly, low visual acuity was measured, in spite of sufficiently long foveation periods. The standard deviation of eye positions during foveation periods was used to measure this variability and it was found to be correlated to visual acuity, in conjunction with the mean duration of the foveation periods. On the basis of the data analysis, an exponential relationship is proposed to relate visual acuity and the standard deviation of the eye position during foveations.

38. Design parameters for coastal dikes

Abstract

This paper presents results of the assessment of the design parameters leading to the definition of the crest level of a coastal dike along the German North Sea. Procedures to estimate the design water level have been proposed, distinguishing between comparative and single value procedures. The transformation of the wave characteristics from deep water towards the shallow foreshore was achieved through the application of a spectral wave model. To improve the wave parameter estimations, the existing model was nested to a grid with a higher resolution closer to the coast. The estimation of the wave run-up followed the Dutch procedure with some adjustments to the local wave characteristics and dike geometry. The computed maximum crest level of 8.4 m is below the crest height of the existing dike, which is 8.8 m. However a proposal for a more economical design should be carefully evaluated, paying attention to the uncertainties encountered in this research. The general recommendation is to enhance the reliability of the hindcasted wave parameters through calibration and validation of the wave model and to include in the design process an investigation of the effect of the medium term morphological developments.
