Fire tests are used to determine whether fire protection products meet the minimum performance criteria set out in codes and legislation, as well as to certify these products. Experimental large-scale fires are used to test the performance of safety systems in tunnels, which are confined environments with a high probability of accidents and significant consequences due to the evolution of the event and whether there is the capability of counteracting it by safety measures. In this study, we conducted a systematic literature review following PRISMA guidelines. We searched the Scopus and Web of Science databases for publications from 2013 to 2022, resulting in a selection of 72 articles. An analysis was conducted on the following main topics: tunnel fires, fire characteristics (measured variables, spread, and smoke), model-scale tests, automatic shutdown systems, and ventilation solutions. One of the most important contributions of this study is the suggestion that fire tests represent an effective method not only to prevent fire events in tunnels but also to ensure the resilience of the infrastructure. Based on this state-of-the-art literature review, future tunnels could be designed by linking new smart technology and artificial intelligence to create interactive and high-performing safety systems. The aim of this study is the risk analysis evaluation about argon release from the GERDA experiment in the Gran Sasso underground National Laboratories (LNGS) of the Italian National Institute for Nuclear Physics (INFN). The GERDA apparatus, located in Hall A of the LNGS, is a facility with germanium detectors located in a wide tank filled with about 70 m3 of cold liquefied argon. This cryo-tank sits in another water-filled tank (700 m³) at atmospheric pressure. In such cryogenic processes, the main cause of an accidental scenario is lacking insulation of the cryo-tank. A preliminary HazOp analysis has been carried out on the whole system. The risk assessment identified two possible top-events: explosion due to a Rapid Phase Transition - RPT and argon runaway evaporation. Risk analysis highlighted a higher probability of occurrence of the latter top event. To avoid emission in Hall A, the HazOp, Fault Tree and Event tree analyses of the cryogenic gas extraction and ventilation plant have been made. The failures related to the ventilation system are the main cause responsible for the occurrence. To improve the system reliability some corrective actions were proposed: the use of UPS and the upgrade of damper opening devices. Furthermore, the Human Reliability Analysis identified some operating and management improvements: action procedure optimization, alert warnings and staff training. The proposed model integrates the existing analysis techniques by applying the results to an atypical work environment and there are useful suggestions for improving the system reliability. Abstract: Syringocystadenoma papilliferum is a benign adnexal neoplasm frequently seen in association with other adnexal tumours. We report the dermoscopic features of three cases of syringocystadenoma papilliferum developing in naevus sebaceus. Clinically the lesions were characterised by exophytic papillary structures. Background: Some dermatopathological parameters are recognized as dominant indicators of high metastatic potential in melanoma, especially Breslow thickness, ulceration, Clark’s level of invasion and mitotic rate. Because NM23 protein is the product of a melanoma metastasis suppressor gene, the aim of this study was to compare such dermatopathological indicators of melanoma prognosis with NM23 protein expression in primary cutaneous melanoma. Methods: The immunohistochemical NM23 expression was semiquantitatively assessed in 30 primary cutaneous melanomas. Ten dermatopathological parameters were evaluated and compared with NM23 expression. Results: A significant inverse correlation was found for NM23 expression in comparison with Breslow thickness (p < 0.01), ulceration (p < 0.05), Clark’s level (p < 0.01), mitotic rate (p < 0.05), and vertical growth phase (p < 0.05). By contrast, no significant correlation was found for NM23 expression in comparison with cell morphology, presence of adjacent nevus, pigmentation, tumor-infiltrating lymphocytes, and regression was impossible to evaluate. Conclusions: The expression of NM23 protein in primary cutaneous melanoma is significantly inversely correlated with dermatopathological parameters currently recognized as powerful indicators of melanoma prognosis. Infantile acute hemorrhagic edema (AHE) is a benign condition characterized by a dramatic onset of inflammatory edema and ecchymotic purpura in a target or cockade pattern. It is considered an uncommon form of cutaneous vasculitis occurring in children younger than 2 years of age. The outbreak is frequently preceded by an immunization or various infections. We describe an 11-month-old girl with rosette-shaped purpuric plaques on the face and limbs, clinically consistent with a diagnosis of AHE of the skin, associated with fever and diarrhea. Laboratory investigations showed a rotavirus infection, which has not previously been reported in association with AHE of the skin. The disease had a benign course without relapses. Appropriate microbiologic investigations are advisable to confirm the possible etiologic role of rotavirus. [ABSTRACT FROM AUTHOR]

Recent events, including the floods in central Europe in May 2021, have highlighted how climate change is giving rise to scenarios that were neither foreseen nor predictable. One problem this poses is the need to rethink the logic of various environmental constraints that are often based on return times of 20-50 years or 100-200. A single event does not change the statistical expectations for the recurrence of the event itself, but the recurrence of several extraordinary events in a few years is a clear indication of a changing trend. The prevention of the effects of such events is based on the definition of the areas at greater or lesser risk specifically based on the return times of the exceptional events, so it is foreseeable that a series of territorial plans, mostly carried out a few decades ago, will have to be updated or re-executed from scratch. These reworkings will be able to take advantage of the opensource software and open spatial data that have become available in the meantime, facilitating the entire process, and making it more open and shareable. In this paper we tested on a real case (the May 1999 pyroclastic flows in Campania, southern Italy) the actual possibility of implementing a model for forecasting such events using only open-source software and open data. It has been demonstrated that the entire process can be carried out using only open-source resources and it has been verified that the predictions of the hazard and risk model obtained with only input data prior to the event, give an output prediction that is significantly coincident with the events that actually occurred as documented by the authorities. The increasing incidence of wildfires in the Mediterranean region has raised significant scientific and environmental concerns. This study focuses on a retrospective analysis of wildfire ignition and propagation within the context of the Monte Catillo Natural Reserve in Italy. After conducting a comprehensive review of the current state-of-the-art wildfire susceptibility mapping, propagation modeling, probability assessment, forest vulnerability models, and preventive silvicultural measures, we examine the regulatory framework surrounding wildfires in the national context, with a specific focus on prevention, prediction, and active firefighting measures. A geostatistical model of wildfire occurrence was developed, starting with the characterization of the area vegetation and anthropogenic factors influencing wildfire ignition. After that, wildfire observations from the period between 2010 and 2021 were included. The objective is to generate a wildfire hazard map for two distinct vegetation communities. To accomplish this, a statistical analysis was applied using the Poisson Model, assessing its goodness-of-fit by comparing observed frequencies with experimental data through the chi-square test. In conclusion, this model serves as a valuable tool for characterizing wildfire hazards, including ignition probabilities and propagation scenarios, within the Monte Catillo Natural Reserve. The research significantly contributes to enhancing our understanding of wildfire dynamics and plays a crucial role in the development of effective strategies for wildfire risk management. This paper proposes a methodology for sentiment analysis with emphasis on the emotional aspects of people visiting the Herculaneum Archaeological Park in Italy during the period of the COVID-19 pandemic. The methodology provides a valuable means of continuous feedback on perceived risk of the site. A semantic analysis on Twitter text messages provided input to the risk management team with which they could respond immediately mitigating any apparent risk and reducing the perceived risk. A two-stage approach was adopted to prune a massively large dataset from Twitter. In the first phase, a social network analysis and visualisation tool NodeXL was used to determine the most recurrent words, which was achieved using polarity. This resulted in a suitable subset. In the second phase, the subset was subjected to sentiment and emotion mapping by survey participants. This led to a hybrid approach of using automation for pruning datasets from social media and using a human approach to sentiment and emotion analysis. Whilst suffering from COVID-19, equally, people suffered due to loneliness from isolation dictated by the World Health Organisation. The work revealed that despite such conditions, people's sentiments demonstrated a positive effect from the online discussions on the Herculaneum site. [ABSTRACT FROM AUTHOR]

The increase life expectancy led to an expected increase in skin cancer incidence in older patients. Their treatment can require a complex decision‐making process. Limited data are available on characteristics, management and outcome of skin tumours in nonagenarian and centenarian patients. The aim of our study was to describe epidemiology, clinical‐pathological features and treatment strategies of skin cancers in a cohort of patients aged ≧95 years. A total of 116 patients ≧95 years of age presented for the evaluation of 225 skin lesions (mean of 1.94 lesions per patient). The mean age was 97.4 years, 57.8% were women. Most patients had an ECOG score of 3 (49.3%) or 4 (32%). Lesions were mainly located on the head and neck area (74.2%), upper (7.1%) and lower (6,2%) limbs. The majority of patients presented with non‐melanoma skin cancers (183/225; 81.3%), 25/225 (11.1%) had actinic keratosis, 5 (2.2%) melanoma and 2 (0.9%) atypical fibroxanthoma. Forty‐eight lesions (21.3%) were treated with surgery, 58 (25.8%) with radiotherapy. The management of 73 lesion (32.4%) was discussed at the multidisciplinary tumour board meeting. One patient died for the progression of a squamous cell carcinoma; 74 patients died for causes unrelated to skin tumours, 36 are still alive after a mean follow‐up of 27.27 months. This cohort study confirms that age is not per se a contraindication for treatment of skin cancers in elderly patients. Our results support the importance of a patient‐centred care approach that should take into consideration patient's preferences, comorbidities, compliance and possible adverse events. During the Italian first wave of the COVID-19 pandemic, social restrictions and bad news spread daily by mass media inevitably had a huge influence on the mental state of the population. To assess how much the COVID-19 outbreak impacted the psychologic state of patients referring to our Skin Cancer Unit from March 9 to May 31, 2020, we administered to them a self-report questionnaire, the Impact of Event Scale-Revised (IES-R). To evaluate the trend of the IES-R score over time, we set a temporal cutoff of March 27 (the day with the highest number of deaths for COVID-19 in Italy during the first wave). Three hundred fifty-five patients completed the questionnaire, reporting an average IES-R score of 25.5 (±16.4); 32.4% of participants reached a total IES-R score >32. Patients who visited after March 27, 2020 reported a higher psychologic impact, since the IES-R score significantly increased from 23.6 (±15.6) to 28.3 (±17.2). A group reported higher scores (of participants reaching an IES-R score >32, 57.4% were women and 33.9% were men). We gathered that, at an early stage of events of this magnitude, it could be useful to submit the IES-R questionnaire in high-risk and oncologic patients: we could potentially identify individuals at risk of developing post-traumatic stress disorders, who might be tempted to postpone necessary medical consultations. The chlorella virus PBCV-1 was the first virus found to encode a functional potassium channel protein (Kcv). Kcv is small (94 aa) and basically consists of the M1-P-M2 (membrane-pore-membrane) module typical of the pore regions of all known potassium channels. Kcv forms functional channels in three heterologous systems. Internet-of-Things (IoT) innovations produce interesting opportunities, impact ethical and social aspects toward human society, and influence operational and technical solutions toward the structural systems. This article discusses the impact of some relevant characteristics of the IoT innovations. Significant debated aspects are the product discontinuity in the global market, their durability, the upcoming “social” virtual systems, new “socialized” equipment, the promotion of electrical microsystems, the human competence in the digital age, the safety, operability, and security of IoT. These innovations require an advancement of the technical culture and open new frontiers for electroforensic engineering. In the next future, forensic engineering can promote an international reference organization as a mediator between social, technical, and regulatory aspects. Innovations and efficient sustainability need adequate global regulatory changes to favor the spreading of advantages, with more cert success if old businesses are transferred into new businesses. Background: Nevus‐associated melanoma (NAM) has been regarded as a distinct biological entity from de‐novo melanoma (DNM); however, static dermoscopy often fails in differentiating these entities. Digital dermoscopic monitoring allows to identify dynamic changes occurring during follow‐up; this may improve diagnostic accuracy and potentially our knowledge on NAM biology. We aimed to define main independent factors associated with NAM diagnosis and those influencing follow‐up time in a population of melanomas excised at follow‐up. Methods: A cohort of melanomas excised at follow‐up was retrospectively and consecutively selected. NAMs and DNMs were compared according to baseline features and main dermoscopic changes occurring during follow‐up. Univariate and multivariable logistic and Cox's regression analysis were performed to respectively define factors associated with NAM diagnosis and those influencing the risk for excision. Results: Eighty‐six melanomas were enrolled, of which 21 (24.4%) were nevus‐associated. During follow‐up NAMs mainly underwent atypical network modifications (47.6%), followed by inverse network (28.6%) and dermoscopic island (23.8%) worsening or appearance. DNMs were also mainly characterized by atypical network modifications (47.7%), however, a significant proportion of cases underwent irregular pigmentation/dots/globules or regression changes (29.2%), which were rarely seen among NAMs. Furthermore, both multivariable logistic and Cox's regression analysis demonstrated a significant association between NAM and a longer follow‐up. Conclusions: We demonstrated that among melanomas excised at follow‐up, different patterns of dermoscopic changes may be found between NAMs and DNMs. This finding, together with the association of NAM with a longer follow‐up time, supports the hypothesis of different biological behavior of these two entities. Background: Several studies have demonstrated the benefit of integrating clinical with pathologic information, to obtain a confident diagnosis for melanocytic tumors. However, all those studies were conducted retrospectively and no data are currently available about the role of a clinical-pathologic correlation approach on a daily basis in clinical practice. Aim of the Study: In our study, we evaluated the impact of a routine clinical-pathologic correlation approach for difficult skin tumors seen over 3 years in a tertiary referral center. Results: Interestingly, a re-appraisal was requested for 158 out of 2015 (7.7%) excised lesions because clinical-pathologic correlation was missing. Of note, in 0.6% of them (13 out of 2045) the first histologic diagnosis was revised in the light of clinical information that assisted the Pathologist to re-evaluate the histopathologic findings that might be bland or inconspicuous per se. Conclusion: In conclusion, our study demonstrated that an integrated approach involving clinicians and pathologists allows improving management of selected patients by shifting from a simply disease-focused management (melanoma versus nevus) to a patient-centered approach. The possibility of an inverse association between vitamin D and risk of cancer and, in particular, of cutaneous malignant melanoma has been suggested, but results of epidemiologic studies are still conflicting. We examined the relation between dietary vitamin D intake and melanoma risk through a population-based case-control study (380 cases, 719 controls) in a northern region of Italy, a country with an average vitamin D intake lower than that in northern Europe or the United States. We assessed average daily intake of vitamin D from foodstuffs using the European Prospective Investigation into Cancer and Nutrition (EPIC) semiquantitative food frequency questionnaire. In this population, levels of vitamin D intake were considerably lower than those observed in recent U.S. studies. We found an inverse relation between dietary vitamin D and melanoma risk in the sample as a whole, in both crude and adjusted analyses. In sex- and age-specific analyses, this association appeared to be stronger among males and among older subjects. These findings suggest that, at the relatively low levels of intake observed in this sample, an inverse relation between dietary vitamin D and risk of cutaneous malignant melanoma may exist. Although tumor-infiltrating lymphocytes (TILs) of primary cutaneous melanoma (PCM) include cytolytic T cells able to exert anti-PCM immunity, progression of PCM most frequently occurs, raising the hypothesis that the PCM microenvironment may also exert suppressive forces, for example, possibly developed by regulatory T (TREG) lymphocytes. The aim of this study was to investigate whether TILs of PCMs include lymphocytes bearing the transcription factor forkhead box protein P3 (FOXP3), which is the TREG lineage specification molecule in mice, and is debated to have a similar role in humans. Fourteen patients with PCM were selected, of which four had radial growth phase (RGP) stage I melanoma, five had vertical growth phase (VGP) stage I melanoma, and five had VGP stage III–IV melanoma. Formalin-fixed, paraffin-embedded sections were utilized for immunohistochemical single and double stainings. TILs of PCMs included FOXP3-bearing lymphocytes, which predominantly were CD20- and CD8-negative, but CD3-, CD4-, and CD25-positive, thus consistent with the standard immunophenotypical characteristics of “natural” TREG cells. Further, the proportions of FOXP3-bearing lymphocytes were higher in vertical than in RGP (P=0.001), as well as in late than in early melanoma stages (P<0.001).
