Sepsis is a syndromic entity with high prevalence and mortality. The management of sepsis is standardized and exhibits time-dependent efficiency. However, the management of patients with sepsis is complex. The heterogeneity of the forms of presentation can make it difficult to detect and manage such cases, in the same way as differences in training, professional competences or the availability of health resources. The Advisory Commission for Patient Care with Sepsis (CAAPAS), comprising 7 scientific societies, the Emergency Medical System (SEM) and the Catalan Health Service (CatSalut), have developed the Interhospital Sepsis Code (CSI) in Catalonia (Spain). The general objective of the CSI is to increase awareness, promote early detection and facilitate initial care and interhospital coordination to attend septic patients in a homogeneous manner throughout Catalonia.

PurposeEnteral feeding intolerance (EFI) is a frequent problem in the intensive care unit (ICU), but current prokinetic agents have uncertain efficacy and safety profiles. The current study compared the efficacy and safety of ulimorelin, a ghrelin agonist, with metoclopramide in the treatment of EFI.MethodsOne hundred twenty ICU patients were randomized 1:1 to ulimorelin or metoclopramide for 5days. EFI was diagnosed by a gastric residual volume (GRV) 500ml. A volume-based feeding protocol was employed, and enteral formulas were standardized. The primary end point was the percentage daily protein prescription (%DPP) received by patients over 5days of treatment. Secondary end points included feeding success, defined as 80% DPP; gastric emptying, assessed by paracetamol absorption; incidences of recurrent intolerance (GRV500ml); vomiting or regurgitation; aspiration, defined by positive tracheal aspirates for pepsin; and pulmonary infection.ResultsOne hundred twenty patients were randomized and received the study drug (ulimorelin 62, metoclopramide 58). Mean APACHE II and SOFA scores were 21.6 and 8.6, and 63.3% of patients had medical reasons for ICU admission. Ulimorelin and metoclopramide resulted in comparable %DPPs over 5days of treatment (median [Q1, Q3]: 82.9% [38.4%, 100.2%] and 82.3% [65.6%, 100.2%], respectively, p=0.49). Five-day rates of feeding success were 67.7% and 70.6% when terminations unrelated to feeding were excluded, and there were no differences in any secondary outcomes or adverse events between the two groups.ConclusionsBoth prokinetic agents achieved similar rates of feeding success, and no safety differences between the two treatment groups were observed.

Background: Up-to-date identification of local trends in sepsis incidence and outcomes is of considerable public health importance. The aim of our study was to estimate annual incidence rates and in-hospital mortality trends for hospitalized patients with sepsis in a European setting, while avoiding selection bias in relation to different complexity hospitals. Methods: A large retrospective analysis of a 5-year period (2008–2012) was conducted of hospital discharge records obtained from the Catalan Health System (CatSalut) Minimum Basic Data Set for Acute-Care Hospitals (a mandatory population-based register of admissions to all public and private acute-care hospitals in Catalonia). Patients hospitalized with sepsis were detected on the basis of ICD-9-CM codes used to identify acute organ dysfunction and infectious processes. Results: Of 4,761,726 discharges from all acute-care hospitals in Catalonia, 82,300 cases (1.72%) had sepsis diagnoses. Annual incidence was 212.7 per 100,000 inhabitants/year, rising from 167.2 in 2008 to 261.8 in 2012. Length of hospital stay fell from 18.4 to 15.3 days (p < .00001), representing a relative reduction of 17%. Hospital mortality fell from 23.7 to 19.7% (p < .0001), representing a relative reduction of 16.9%. These differences were confirmed in the multivariate analysis (adjusted for age group, sex, comorbidities, ICU admission, emergency admission, organ dysfunction, number of organ failures, sepsis source and bacteraemia). Conclusions: Sepsis incidence has risen in recent years, whereas mortality has fallen. Our findings confirm reports for other parts of the world, in the context of scarce administrative data on sepsis in Europe.

AimTo examine the pattern and cumulative curve of descriptions of freshwater fishes world-wide, the geographical biases in the available information on that fauna, the relationship between species richness and geographical rarity of such fishes, as well as to assess the relative contributions of different environmental factors on these variables. LocationGlobal. MethodsModestR was used to summarize the geographical distribution of freshwater fish species using information available from data-based geographical records. The first-order jackknife richness estimator was used to estimate the completeness of such data in all terrestrial 1-degree cells world-wide. An -shape procedure was used to build range maps capable of providing relatively accurate species richness and geographical rarity values for each grid cell. We also examined the explanatory capacity of a high number of environmental variables using multiple regressions and Support Vector Machine. ResultsCumulative species description curves show that a high number of species of freshwater fishes remain to be discovered. Completeness values indicate that only 199 one-degree grid cells, mainly located in eastern North America and Europe, could be considered as having relatively accurate inventories. Range maps provide species richness values that are positively and significantly related to those resulting from the first-order jackknife richness estimator. The relationship between species richness and geographical rarity is triangular, so that these species-rich cells are those with a higher proportion of distributionally rare species. Species richness is predicted by climatic and/or productivity variables but geographical rarity is not. Main conclusionsIn general, species-rich tropical areas harbour a higher number of narrowly distributed species although comparatively species-poor subtropical cells may also contain narrowly distributed species. Historical factors may help to explain the faunistic composition of these latter areas; a supposition also supported by the low predictive capacity of climatic and productivity variables on geographical rarity values.

The Joaquín Costa reservoir contains a mixed fish assemblage of native and exotic species. Feeding habits and feeding relationships of species in the fish assemblage were analysed over a one year cycle. Differences in diet composition were found both between species and within species among seasons. Food overlap and trophic similarity among species also showed seasonal variations. Cluster analysis differentiated four groups of predominant diet: (1) macroinvertebrates (trout and largemouth bass), (2) detritus (nase), (3) cladoceran crustaceans and (4) an omnivorous feeding regime, with large seasonal variations in food habits. Food of fish species included in groups 3 and 4 (roach, white bream, barbel, common and mirror carp) varied seasonally. Using graphical models of feeding strategies, similarity indexes, cluster and multivariate analyses based on the relative importance of food categories in the diet of the species, we illustrate that the fish assemblage showed food resource partitioning according to food habits and foraging habitats within the reservoir.

Background A description of individuals with SARS-CoV-2 infection comparing the first and second waves could help adapt health services to manage this highly transmissible infection. Objective We aimed to describe the epidemiology of individuals with suspected SARS-CoV-2 infection, and the characteristics of patients with a positive test comparing the first and second waves in Catalonia, Spain. Methods This study had 2 stages. First, we analyzed daily updated data on SARS-CoV-2 infection in individuals from Girona (Catalonia). Second, we compared 2 retrospective cohorts of patients with a positive reverse-transcription polymerase chain reaction or rapid antigen test for SARS-CoV-2. The severity of patients with a positive test was defined by their admission to hospital, admission to intermediate respiratory care, admission to the intensive care unit, or death. The first wave was from March 1, 2020, to June 24, 2020, and the second wave was from June 25, 2020, to December 8, 2020. Results The numbers of tests and cases were lower in the first wave than in the second wave (26,096 tests and 3140 cases in the first wave versus 140,332 tests and 11,800 cases in the second wave), but the percentage of positive results was higher in the first wave than in the second wave (12.0% versus 8.4%). Among individuals with a positive diagnostic test, 818 needed hospitalization in the first wave and 680 in the second; however, the percentage of hospitalized individuals was higher in the first wave than in the second wave (26.1% versus 5.8%). The group that was not admitted to hospital included older people and those with a higher percentage of comorbidities in the first wave, whereas the characteristics of the groups admitted to hospital were more alike. Conclusions Screening systems for SARS-CoV-2 infection were scarce during the first wave, but were more adequate during the second wave, reflecting the usefulness of surveillance systems to detect a high number of asymptomatic infected individuals and their contacts, to help control this pandemic. The characteristics of individuals with SARS-CoV-2 infection in the first and second waves differed substantially; individuals in the first wave were older and had a worse health condition.
