Se observan diferencias estadísticamente significativas, según el género, respecto a los resultados de la media del índice de actitud global (IAG) para algunas cuestiones del Cuestionario de Opiniones sobre la Ciencia, la Tecnología y la Sociedad (COCTS), en los tres grupos que integraron la muestra: profesoras y profesores en actividad y en formación, y estudiantes del último año de escuela media y primer año de universidad. Estas diferencias aparecen no sólo en las cuestiones referidas a los Efectos de género y a la Infrarrepresentación de las mujeres, sino también en algunas incluidas en los temas Definiciones, Sociología externa e interna de la ciencia y Epistemología. Cuanto mayor es el nivel de formación de los grupos, mayor es su IAG.

Objective To assess the diagnostic performance of ultrasound (US), x-rays, and microscopic analysis of synovial fluid (SF) for calcium pyrophosphate dihydrate crystal deposition disease (CPPD) using histology as a reference standard Methods We enrolled consecutive patients with osteoarthritis waiting to undergo knee replacement surgery. Each patient underwent US of the knee, focusing on menisci and the hyaline cartilage, the day before surgery. During surgery, SF, menisci and condyles were retrieved and examined microscopically. For the meniscus and cartilage microscopic analysis, 8 samples were collected from each specimen and knee radiographs, performed up to 3 months before surgery, were also assessed. A dichotomous score was given for the presence/absence of CPP for each method. Microscopic findings of the specimens were considered the reference standard. All the procedures followed were in accordance with the ethical standards of the local responsible committee. Results 42 patients (14 males) were enrolled. All patients underwent US, 34 had eligible radiographs and 32 had SF analysis. 25 patients (59.5%) were positive for CPP at US, 15 (44.1%) at X-ray and 14 (43.7%) at SF. Sensitivity and specificity values were 96% and 87% for US, 75% and 93% for radiography and 77% and 100% for SF respectively. There were no statistically significant differences between the diagnostic performance across single tests. Conclusion US proved to be at least as accurate as SF analysis for the diagnosis of CPPD. US, which is feasible and harmless, could be considered the first exam of choice for CPPD diagnosis.

Background & Aims: Transcatheter arterial chemoembolization (TACE) currently is used as a palliative treatment for patients with unresectable hepatocellular carcinoma (HCC), but its efficacy still is debated. Our aim was to assess the impact of TACE on patient survival and to identify prognostic factors for survival. Methods: Fifty-six cirrhotic patients with unresectable HCC undergoing at least :1 course of TACE were matched 1:1. for sex, age (in 5-year periods), parameters of Child-Pugh score, Okuda stage, and tumor type with a control group who had received only supportive care. Results: The 2 groups were comparable for cause of cirrhosis, alpha-fetoprotein serum levels, and Cancer of the Liver Italian Program (CLIP) score. The 56 patients in the TACE group received a total of 123 treatment courses. The median follow-up period was 16 months (range, 1-67 mo) in the TACE group and 5 months (range, 1-77 mo) in the supportive care group. Survival rates at 12, 24, and 30 months in patients receiving TACE were 74.3%, 52.1%, and 38.8% respectively, with a median survival time of 25 month whereas in supportive care patients the rates we 39.4%, 25.4%, and 19%, respectively, with a median survival time of 7 months (P =.0004). At univariate analysis, TACE, tumor type, presence of ascites, alpha-fetoprotein serum level, CLIP score, and Okuda stage were associated significantly with survival. Only TACE and CLIP score proved to be independent predictors of survival at multivariate analysis. Conclusions: TACE is an effective therapeutic option for cirrhotic patients with unresectable HCC and a CLIP score of 3 or less.

Se con “genere” ci si riferisce al carattere socialmente costruito dell’esperienza della maschilità e della femminilità e delle conseguenti disparità che spesso segnano le relazioni tra uomini e donne, non si può prescindere dall’individuare nell’infanzia e nelle relazioni con gli adulti in famiglia e nei servizi educativi e scolastici i tempi, i luoghi le interazioni all’origine di tali processi. La promozione dell’evoluzione di una cultura delle differenze tra generi e di un autentico e rispettoso dialogo tra le differenze non può che passare attraverso l’educazione nella prima infanzia, e dunque necessariamente attraverso processi di consapevolezza critica circa la forma e i contenuti che assumono, nel vivere quotidiano, le rappresentazioni sulle differenze di genere. Il volume presenta e analizza le rappresentazioni, stereotipate e non, sui generi e sui rapporti tra essi in adulti educativamente (e affettivamente) significativi per i bambini e le bambine emiliano-romagnoli in età 0-6 anni (madri, padri, nonni, nonne, educatrici/ori di nido e nei centri per bambini e genitori, insegnanti di scuola dell’infanzia, operatori di centri per le famiglie). In particolare le analisi e le riflessioni offerte si concentrano su temi quali l’idea di educazione al femminile e al maschile che viene veicolata a scuola e in famiglia; le rappresentazioni di bambino e bambina veicolate nel contesto educativo e familiare; l’influenza dei giochi e giocattoli proposti all’infanzia rispetto alla costruzione dell’identità di genere; la prospettiva dalla quale i padri e le madri di oggi vedono le relazioni di genere; le rappresentazioni che i genitori e il personale educativo hanno della figura professionale dell’educatore maschio. Tali aspetti vengono affrontati dalle autrici da una prospettiva interdisciplinare che intreccia visioni antropologiche, pedagogiche e sociologiche. Le evidenze empiriche mettono in luce un quadro complesso nel quale modelli vicini a visioni stereotipate e tradizionali si affiancano ad altri che interpretano in modo differente ruoli e comportamenti legati al genere. Quello che però appare comune ai diversi interlocutori e contesti indagati è la tendenza ad appellarsi ad un’indifferenziazione di genere, come una sorta di velo che cela le differenze, anziché valorizzarle. L’educazione di genere dunque viene agita, ma non viene sufficientemente messa a tema nelle famiglie e neppure nei servizi educativi. Tali risultati portano a supporre che per compiere un percorso intenzionalmente educativo in merito al genere occorrerebbe ripartire dai tanti significati ad esso attribuiti da parte delle diverse figure educative, molto spesso non pienamente presenti alla loro consapevolezza, per creare un percorso di riflessione che consenta di renderli maggiormente espliciti e condivisi.

La cirrosi epatica è un “processo diffuso caratterizzato da fibrosi e da trasformazione della normale architettura in noduli strutturalmente anormali”. Di questa definizione, fondata su criteri morfologici e fatta propria dall’Organizzazione Mondiale della Sanità, è opportuno sottolineare che 1) il processo patologico deve essere diffuso e coinvolgere tutto l’organo; le lesioni focali (ad es.: iperplasia nodulare focale, iperplasia rigenerativa) non costituiscono una cirrosi; 2) la fibrosi deve associarsi alla formazione di noduli ed al sovvertimento strutturale dell’organo; la fibrosi epatica congenita, ove la fibrosi circonda noduli nei quali l’architettura lobulare è conservata, non può essere definita cirrosi. Il sovvertimento strutturale del fegato rappresenta il fondamento patogenetico delle manifestazioni cliniche cardinali della cirrosi, conseguenti all’insufficienza funzionale dell’organo ed all’ipertensione portale. Entrambe, infatti, derivano dall’alterata organizzazione vascolare, con perdita dei rapporti fisiologici fra epatocita e rete sinusoidale e compressione delle venule a livello degli spazi portali e del centrolobulo. Una definizione morfologica è giustificata dal fatto che la cirrosi non costituisce un’entità nosologica unitaria, ma rappresenta l’esito di malattie diverse per eziologia, manifestazioni cliniche, decorso e prognosi. Su criteri morfologici è anche basata la tradizionale classificazione della malattia (Tab. 1), che, tuttavia, non assume un’effettiva importanza pratica, poiché manifestazioni cliniche ed implicazioni prognostiche delle diverse forme non differiscono sostanzialmente. La classificazione eziologico-patogenetica (Tab. 1), invece, riveste maggiori implicazioni cliniche, tali da influenzare decisioni sul piano terapeutico ed assumere importanza prognostica.

Background Treatment of hepatitis C virus (HCV) recurrence after liver transplantation (LT) is difficult with low response rates. Aim To assess the safety and efficacy of pegylated-interferon (PEG-IFN) alfa-2b + ribavirin (RBV) in patients with post-LT recurrent genotype-1 HCV and to establish stopping rules according to response. Methods Fifty-three patients with post-LT HCV recurrence were enrolled. Patients received PEG-IFN alfa-2b 1.0 μ/kg/week plus RBV 8–10 mg/kg/day for 24 weeks. Those with ‘early virological response at week 24’ (EVR24) continued treatment for 24 weeks (group A). Patients without EVR24 were randomized to continue (group B) or to discontinue (group C). Results Overall sustained virological response (SVR) was 26% (14/53). Alanine aminotransferase, rapid virological response, EVR12, EVR24, undetectable serum HCV-RNA at weeks 12 (cEVR12) and 24 (cEVR24) were related to SVR. cEVR12 and cEVR24 (OR: 14.7; 95% CI: 2.02–106.4) were independent predictors of SVR. All patients with SVR, had cEVR12. No patient in groups B and C achieved end-of-treatment response. One patient in group B had SVR. Conclusions Pegylated-interferon alfa-2b was effective in one of four of patients with HCV genotype 1 after LT. Treatment should be discontinued in patients with no virological response at week 12. Further studies are needed to evaluate whether a longer treatment period may be beneficial in patients with ≥2 log10 drop in HCV-RNA at week 24

Intestinal lymphangiectasia is defined as a dilatation of small bowel lymphatic capillaries and a loss of lymph into the bowel lumen. Clinically it is characterized by hypoproteinaemia and oedema. We present here a case of protein-losing enteropathy due to intestinal lymphangiectasia after liver transplantation in a 57-year-old man who was transplanted for hepatitis C virus. Four years after liver transplantation, the patient developed hypoalbuminaemia and ascites associated with recurrence of cirrhosis. The sudden fall in serum albumin led us to look for a cause of reduction other than or in addition to cirrhosis. Duodenal biopsies showed tall villi with dilated lymphatic vessels and widening of the villi caused by oedema, demonstrating intestinal lymphangiectasia. In this case a low-fat diet supplemented with medium-chain triacylglycerols achieved an early clinical improvement with increased serum albumin levels and ascites disappearance. Intestinal lymphangiectasia should be suspected in liver-transplanted patients developing hypoproteinaemia and hypoalbuminaemia after the recurrence of cirrhosis
