What predicts therapeutic outcome and recurrence is one of the most critical and less known aspects of addiction treatment. Prognostic studies have mostly examined individual based characteristics typically used in the biomedical literature such as age, principal drug of choice, personality traits or brain features (1, 2). However, there is growing recognition of the relevance of social determinants such as socioeconomic background, job and housing security or wealth (3). To date, very few studies have jointly examined the individual and social determinants of treatment prognosis, as illustrated by a limited number of systematic reviews on the topic over the last decade (4-7). Two of these reviews included participants with miscellaneous substance use disorders (4, 5), one focused on alcohol (7) and one on smoking (6). The Lewer et al. review (4) encompassed all treatment approaches and used health care utilisation as the primary outcome. Their findings showed that unstable housing, use of injected drugs and mental health problems were negatively associated with health care utilisation, whereas engagement with opioid substitution treatments was positively associated with the outcome. The Lappan et al review (5) focused on face-to-face psychosocial treatments, mostly in the United States, and used treatment retention as the primary outcome. Their findings showed that higher percentage of African Americans, lower income, inclusion of participants who used stimulant drugs, heavier cocaine use and treatments with more treatment sessions and longer session lengths were negatively associated with retention, whereas smoking more cigarettes per day and higher frequency of heroin use were linked to better retention. Among people with tobacco use disorder, greater dependence and having made a previous quit attempt were associated with lower quitting success, whereas higher affluence and social grade predicted better outcomes although they were comparatively less studied (6). In people with alcohol use disorder, severity of dependence, mental health problems, lower self-efficacy and motivation, and shallower treatment goals were prospectively associated with higher recurrence (selected studies had a minimum 3-month follow up) (7). Cognitive and socioeconomic variables were also negatively associated with recurrence although not as consistently. Overall, findings show that both individual (e.g. drug type, patterns of use), treatment (e.g. number and length of sessions) and social (e.g. affluence, housing) significantly predict clinical outcome and risk of recurrence, with the latter (social) variables consistently appearing across reviews but lacking sufficient evidence. This gap resonates with emerging evidence on and increased awareness of the importance of social determinants of health for addiction mechanisms (3). To our knowledge, there are no existing adequately sampled studies that have systematically assessed the cumulative, differential, and interactive contribution of both individual and social determinants in predicting addiction treatment outcome and risk of recurrence, nor attempts to generate predictive models that can optimise individualised risk prediction. The latter is critical for translation of research into clinical practice, as it may enable clinicians to use algorithms to estimate risk and personalise treatment for each client. In this project, we will leverage an existing dataset of circa 100,000 participants with substance use disorders who were registered and monitored through the digital health information system of a state-wide addiction treatment network between 2015 and 2023, to examine the prognostic contribution of both individual and social factors to clinical outcomes (treatment outcome and recurrence) over two years since treatment onset. Given the large sample size we will randomly split the sample in two subsamples (i.e., henceforth the training and testing subsamples) to build a predictive model in a training subsample and test the ability of the predictive model to forecast individualised risk using unseen data in a testing subsample. As the influence of social factors was likely intensified during the COVID-19 pandemic (8-10), we will use time-lagged data (pre- versus post-pandemic onset) to test this potential intensification as a secondary aim. Aims 1. To examine the prognostic contribution of individual and social determinants of addiction to treatment outcome and risk of recurrence over a period of two years in people with substance use disorders monitored through the health information system of a state-wide treatment network. 1a) To quantify both the cumulative and differential contribution of individual versus social determinants to therapeutic outcome and risk of recurrence. 1b) To explore meaningful interactions between individual and social determinants in prognostically predicting treatment outcome and risk of recurrence. 1c) To study if the contribution of individual vs social determinants to treatment outcomes and risk of recurrence shifted during the COVID-19 pandemic in Spain (2020-2022) relative to the immediately preceding period (2017-2019). 2. To test the accuracy of a data-driven model combining individual and social determinants (generated in a training subsample) to predict individual treatment outcome and recurrence in a testing subsample.

El objetivo de la investigación fue diseñar un cuestionario para la evaluación de creencias nucleares en torno al consumo de drogas y el craving. La muestra estuvo compuesta por 215 pacientes, de los que el 85.8% eran hombres y el 14.2% mujeres en tratamiento por su adicción al alcohol (40%), cocaína (36.3%) y cánnabis (23.7%). Se realizó un análisis descriptivo de la muestra, análisis de varianza, de regresión y análisis factorial con la finalidad de indagar en la estructura del cuestionario y su relación con variables como la abstinencia o deseo de consumo. El apartado de creencias relacionadas con las drogas mostró una estructura de cuatro factores: lo que la persona cree que no podrá realizar en ausencia del consumo de drogas, la falta de renuncia al consumo, las condiciones que deben darse para volver a consumir y el consumo como única vía para sentirse bien. El apartado relacionado con el craving mostró tres factores: las emociones negativas como precipitantes del consumo, las emociones positivas y las dificultades atribuidas al afrontamiento del deseo. A su vez, se constata que las creencias tienen más peso en la predicción del deseo en comparación al tiempo de abstinencia. El cuestionario permite evaluar un conjunto de creencias que muestras factores significativos para el diseño de programas de prevención de recaídas., This study was aimed at designing a questionnaire for the assessment of addiction-related core beliefs and craving. The sample comprised 215 patients (85.8% males and 14.2% females) in treatment for dependence to alcohol (40%), cocaine (36.3%) and cannabis (23.7%). Descriptive statistics were used to characterize the sample. Variance, regression and factorial analyses were conducted to study the questionnaire structure and its relation with variables such as abstinence and craving. Items about drug-related beliefs yielded a four-factor structure: what patient think that they could not do without drug use, lack of withdrawal, conditions required to use drugs again, and use of drugs as the only way to feel good. Items related to craving yielded three factors: negative emotions as precipitants of drug use, positive emotions, and difficulties attributed to coping with craving. Furthermore, beliefs were more important to predict craving than abstinence time. The present questionnaire allows to assess a set of significant factors to design prevention relapse programs.

El elevado consumo de drogas que se produce en las raves y las características propias de estas fiestas las convierten en un contexto recreativo de alto riesgo para la salud de los participantes. El objetivo de esta revisión es establecer una categorización de las investigaciones sobre raves y consumo de drogas a partir de sus objetivos y principales resultados. Se discuten las necesidades de investigación/conocimiento detectadas como fruto de esta revisión. Para ello, se realizó una revisión sistemática de la literatura científica a través de las bases de datos Medline, Psycinfo y Psicodoc. Una vez aplicados los criterios de inclusión se obtuvieron 36 artículos, clasificados en seis categorías. Los resultados han mostrado que 23 estudios tienen por objetivo el análisis del perfil psicosocial, la prevalencia y los patrones de consumo de drogas. Nueve estudios se centran en el riesgo asociado al consumo de drogas y otros nueve en sus efectos. Dado el elevado riesgo asociado a las raves y la escasez de estudios empíricos detectado, se señala la necesidad de realizar más estudios empíricos. Las evidencias que permitan guiar las estrategias de prevención y reducción de daños con ravers son insuficientes. Se detecta la necesidad de estudiar: el policonsumo, las estrategias de reducción de daños, los efectos positivos y motivaciones., The high drug use that occurs at raves and the specific characteristics of these parties make them a high risk recreational context the health of participants. The aim of this paper is to establish a categorization of research on drug use and raves according to their objectives and main results. Knowledge and research needs identified as a result of this review are discussed. To this end, a systematic review of scientific literature through Medline, Psycinfo and Psicodoc was conducted. After applying the inclusion criteria, 36 papers were obtained, classified into six categories. The results show that 23 studies aim psychosocial profile analysis, and the prevalence and patterns of drug use. Nine studies focus on risks related to drug use, and nine in the drug effects. Given the high risk associated with raves and the scarcity of empirical studies identified, the need to develop further empirical studies is addressed. There is still insufficient evidence to guide intervention strategies to prevent risks and harms among ravers. There is also a need to explore: polydrug use, harm reduction strategies, positive effects and motivations.

La actividad física es la estrategia más adecuada para disminuir las Enfermedades No Transmisibles dado que promueve mayores beneficios estar activo que inactivo. La actividad física regular proporciona bienestar más allá del alcance personal; no sólo puede mejorar la salud y ayudar a mantenerla, sino además presenta importantes ventajas sociales y económicas. Por ello, es indispensable destinar esfuerzos para el esclarecimiento del problema público generado por la inactividad física donde las relaciones entre lo social, la salud, y la Actividad Física juegan un papel determinante para la formulación y priorización de una política social, para que el país que reflexione sobre los condicionantes asociados a la desigualdad por género de la población colombiana. La Enfermedad de Alzheimer presenta como uno de los primeros síntomas, alteraciones en la recuperación y comprensión de las palabras. Estos déficits no se producen en todas las funciones lingüísticas de la misma forma sino que algunas permanecen mejor preservadas y durante más tiempo que otras. Para poder averiguar qué tipo de palabras son las que primero se deterioran, en este trabajo se han utilizado tareas de lectura en las que se evaluará el procesamiento léxico y semántico. Los estímulos de las tareas se organizan en relación a dos variables psicolingüísticas: por un lado, la edad de adquisición, por ser una de las más estudiadas y al parecer una de las que más influencia tiene en el procesamiento léxico y semántico y por otro, la imaginabilidad de las palabras, por ser una variable de tipo semántico y por presentar los pacientes con Enfermedad de Alzheimer, déficits en este tipo de procesamiento. Los modelos teóricos en los que se basa este trabajo son el modelo de la doble ruta en cascada (Coltheart, Rastle, Perry, Langdon, & Ziegler, 2001) y los modelos conexionistas de Steyvers & Tenenbaum (2005), de Monaghan y Ellis (2010) y de Ellis y Lambón Ralph (2000). El estudio que se presenta contiene dos objetivos generales. El primero de ellos consiste en comparar la influencia de la variable AoA en el procesamiento léxico y semántico en pacientes con Enfermedad de Alzheimer con diferente grado de deterioro. El segundo objetivo se centra en detectar si es la variable AoA o Imaginabilidad la que presenta un mayor efecto en la EA en el procesamiento léxico a medida que el deterioro cognitivo es mayor. Se utilizaron tres grupos de estudio: un grupo control constituido por 25 personas mayores de 55 años que no presentaban deterioro cognitivo ni historial psiquiátrico y dos grupos experimentales constituidos por 25 pacientes con diagnóstico de Enfermedad de Alzheimer con diferente grado de deterioro (leves y moderados). Para llevar a cabo los objetivos del trabajo se utilizó una batería de pruebas neuropsicológicas y se diseñaron tres tareas experimentales. En lo que respecta a las tareas experimentales, dos de ellas se utilizaron para evaluar el procesamiento léxico y consistían en una variante de la tarea de decisión léxica, denominada tarea de selección léxica. En esta tarea se utilizaban, a diferencia de la tarea de decisión léxica clásica, cuatro estímulos en cada ítem experimental: un estímulos objetivo (palabra) y tres distractores (pseudopalabras).En una de ellas se manipuló la variable edad de adquisición y en la otra la imaginabilidad de las palabras. La segunda de las tareas experimentales consistía en un ejercicio de emparejamiento definición-palabra y evaluaba el procesamiento semántico de los sujetos. En esta prueba se presentaba una definición y seguidamente cuatro palabras: un target y tres distractores, relacionados semánticamente. Las palabras objetivo se organizaron en función de la variable edad de adquisición. En este caso, los target coincidían con los utilizados en la tarea de selección léxica en la que se manipulaba la variable edad de adquisición. Los resultados obtenidos muestran un peor rendimiento en la tarea semántica que en las de selección léxica, tanto en los grupos controles como en los Enfermos de Alzheimer. Estas diferencias se amplían progresivamente a medida que el deterioro cognitivo de los sujetos aumenta. En lo que respecta al efecto de la variable edad de adquisición, se observa un mantenimiento del patrón encontrado en el grupo control (mejor rendimiento en palabras tempranas que tardías) tanto en los enfermos de Alzheimer que se encuentran en las fases leves como en los moderados diferencias que se incrementan a medida que aumenta el deterioro cognitivo. En las dos tareas de selección léxica, se observa un mayor efecto de la variable imaginabilidad a medida que aumenta el deterioro en la Enfermedad de Alzheimer que de la variable edad de adquisición. En resumen, este trabajo muestra una mayor preservación del procesamiento léxico que del semántico tanto en pacientes con EA leve como moderada, con un rendimiento más favorable en las palabras tempranas que en las tardías. Asimismo, se observa una mayor afectación de las variables psicolingüísticas de tipo semántico que de tipo léxico-semántico en este tipo de sujetos, efecto que va aumentando a medida que el deterioro cognitivo es mayor., Alzheimer's disease presents as one of the first symptoms, alterations in the recovery and understanding of words. These deficits are not produced in all languages functions in the same way but well preserved and some longer than others. To find out what kind of words are the first to be deteriorated, in this work we have used reading tasks in which lexical and semantic process will be evaluated. Tasks stimulus are organized in relation to two psycholinguistic variables. On one hand, the age of acquisition (AoA), being one of the most studied and apparently one of the most influenced on the lexical and semantic process. On the other hand, imageability of words, because of being a semantic variable and to present patients with Alzheimer's disease, deficits in this type of process. Theoretical models in which this work is based are the model of the dual-route cascaded (Coltheart, Rastle, Perry, Langdon, & Ziegler, 2001) and connectionist models of Steyvers & Tenenbaum (2005), Monaghan and Ellis (2010) and Ellis and Lambón Ralph (2000). The study presented has two overall objectives. The first consists in comparing the influence of AoA variable in the lexical and semantic process in subjects with Alzheimer's disease with different degrees of deterioration. The second objective focuses on detecting if the AoA or imageability variable is which present a greater effect on AD in lexical process as cognitive impairment is higher. Three study groups were used: a control group consisting of 25 people over 55 years old who did not have cognitive deterioration or psychiatric history and two experimental groups consisting of 25 patients diagnosed with Alzheimer's disease with different degrees of deterioration (mild and moderate). To carry out the objectives of the work a battery of neuropsychological tests was used and three experimental tasks were designed. With respect to the experimental tasks, two of them were used to assess the lexical process and it was consisted of a variant of the lexical response called lexical selection task. Were used in this work, unlike the classical lexical decision task four stimuli in each experimental item: one target stimulus (words) and three distractors (pseudowords) In one of the tasks, the AoA variable was manipulated and the imageability of words in the other. The second experimental task consisted of a matching exercise defmition-word and evaluating semantic processing subjects. In this test a definition was presented and then four words: one target and three distractors semantically related. The target words are organized according to age of acquisition variable. In this case, the targets coincide with those used in the lexical selection task in which the variable AoA is manipulated. The results show a worse performance in the semantic task than in the lexical selection in both control groups as in Alzheimer subjetcs. These differences are progressively extended as the cognitive decline of the subject increases. Regarding the effect of the variable age of acquisition, maintenance of the pattern found in the control group (better performance in early words that in the later) is observed in Alzheimer's patients too, who are in the mild and moderate stages, differences increased with increasing cognitive impairment. Increased imageability effect with increasing cognitive impairment in subjects with AD. This effect is greater than recorded in the AoA in the two tasks of lexical selection. In summary, this work shows a greater preservation of lexical than semantic processing in subjects with both mild and moderate AD, with a more favorable early words than in the later performance. Furthermore, greater involvement of the semantic variables than lexical-semantic type in this kind of subject, an effect that increases as the cognitive impairment is higher.
