Abstract

Heterocyclic aromatic amines (HAAs), formed during the cooking of meat, are potential human carcinogens, underscoring the need for long-lived biomarkers to assess exposure and cancer risk. Frequent consumption of well-done meats containing 2-amino-1-methyl-6-phenylimidazo[4,5-b]pyridine (PhIP), a prevalent HAA that is a prostatic carcinogen in rodents and DNA-damaging agent in human prostate cells, has been linked to aggressive prostate cancer (PC) pathology. African American (AA) men face nearly twice the risk for developing and dying from PC compared to White men. We previously demonstrated that scalp hair is a reliable biospecimen for measuring PhIP intake using liquid chromatography-mass spectrometry. This study aimed to determine whether PhIP dietary intake is higher in AA men, potentially contributing to this health disparity. Healthy AA men were found to have a significantly higher mean hair PhIP level (2.12-fold) than White men on free-choice diets. However, this difference was not statistically significant after adjusting for melanin content. Further research is needed to understand how hair pigmentation, follicular density, and other morphological features of hair influence PhIP accumulation. These insights can improve the accuracy of using hair PhIP levels as a biomarker for exposure and its potential associations with cancer risk. [ABSTRACT FROM AUTHOR]

10. Sex Differences in Ependymoma Methylation by Methylation‐Defined Subgroup.

Abstract

Ependymoma is the second most common malignant paediatric brain tumour composed of nine methylation‐defined, clinically relevant subgroups. It is unclear if there are sex differences in methylation profiles within these subgroups which could guide future treatment options. We obtained available methylation data from the National Center for Biotechnology Information Gene Expression Omnibus (GEO). Differentially methylated probes (DMPs) between sexes were identified in each ependymoma sample and mapped to genes. Reactome pathways resulting from genes were identified. Survival was estimated for each sex within molecular subgroups. There were 492 cases included in the main analysis: PF‐EPN‐A (n = 238) PF‐EPN‐B (n = 52), PF‐SE (n = 34), SP‐MPE (n = 26), SP‐EPN (n = 21), ST‐EPN‐RELA (n = 87), ST‐EPN‐YAP1 (n = 13) and ST‐SE (n = 21). Females were observed to have better, but statistically nonsignificant, 5‐year overall survival (OS) and better, marginally significant 5‐year progression‐free survival (PFS) than males. One subgroup, ST‐EPN‐RELA, showed significantly better OS in females. There was a difference in immune cell composition within tumour subgroups. One gene, RFTN1, was consistently differentially methylated by sex among all subgroups. There were biologic pathways identified from genes with differential methylation by sex in the following subgroups: PF‐EPN‐B, PF‐SE, ST‐EPN‐RELA and ST‐EPN‐YAP1. 13. PM2.5, vegetation density, and childhood cancer: a case-control registry-based study from Texas 1995-2011.

Abstract

Background Air pollution is positively associated with some childhood cancers, whereas greenness is inversely associated with some adult cancers. The interplay between air pollution and greenness in childhood cancer etiology is unclear. We estimated the association between early-life air pollution and greenness exposure and childhood cancer in Texas (1995 to 2011). Methods We included 6101 cancer cases and 109 762 controls (aged 0 to 16 years). We linked residential birth address to census tract annual average fine particulate matter <2.5 µg/m³ (PM2.5) and Normalized Difference Vegetation Index (NDVI). We estimated odds ratios (ORs) and 95% confidence intervals (CIs) between PM2.5/NDVI interquartile range increases and cancer. We assessed statistical interaction between PM2.5 and NDVI (likelihood ratio tests). Results Increasing residential early-life PM2.5 exposure was associated with all childhood cancers (OR = 1.10, 95% CI = 1.06 to 1.15), lymphoid leukemias (OR = 1.15, 95% CI = 1.07 to 1.23), Hodgkin lymphomas (OR = 1.27, 95% CI = 1.02 to 1.58), non-Hodgkin lymphomas (OR = 1.24, 95% CI = 1.02 to 1.51), ependymoma (OR = 1.27, 95% CI = 1.01 to 1.60), and others. Increasing NDVI exposure was inversely associated with ependymoma (0- to 4-year-old OR = 0.75, 95% CI = 0.58 to 0.97) and medulloblastoma (OR = 0.75, 95% CI = 0.62 to 0.91) but positively associated with malignant melanoma (OR = 1.75, 95% CI = 1.23 to 2.47) and Langerhans cell histiocytosis (OR = 1.56, 95% CI = 1.07 to 2.28). There was evidence of statistical interaction between NDVI and PM2.5 (P  < .04) for all cancers. Conclusion Increasing early-life exposure to PM2.5 increased the risk of childhood cancers. NDVI decreased the risk of 2 cancers yet increased the risk of others. These findings highlight the complexity between PM2.5 and NDVI in cancer etiology. [ABSTRACT FROM AUTHOR]

Abstract

Simple Summary: In our study we found that the overall meta-analysis of all cancer related fatigue (CRF) treatment studies showed significant reduction of CRF. The meta-analysis did not show significant reduction of CRF with treatment ginseng, guarana, megestrol, mistletoe, psychostimulants, selective serotonin reuptake inhibitors/antidepressants. Metanalysis of Corticosteroids studies showed significant reduction in CRF. Further studies are needed. Purpose: In this study we aimed to estimate the effectiveness of pharmacological, nutraceutical, and phytopharmaceutical treatments on CRF. Methods: Ovid MEDLINE, Ovid Embase, Ovid Psych info, CINHAHL and Cochrane Library databases were searched up to 30 September 2021. Randomized controlled trials of pharmacological, nutraceutical and phytopharmaceutical interventions for treatment of CRF for at least one week duration and have used valid tool to assess severity of CRF as a primary or secondary outcome were considered. Results: 32 eligible studies (4896 patients) were reviewed. For the overall meta-analysis, the random effect models yielded the treatment effect (95% CI) of −0.29 (−0.48,−0.09), p < 0.001. The meta-analysis did not show significant reduction of CRF with treatment with ginseng (n = 6), guarana (n = 3), megestrol (n = 2), mistletoe (n = 3), psychostimulants (n = 14), SSRI/antidepressants (n = 2). Corticosteroids (n = 2) showed significant reduction in CRF with treatment effects of 0.94 (−1.21, −0.67), p <0.0001, respectively. Conclusions: In this study, overall meta-analysis of all studies demonstrates significant reduction of CRF using Pharmacological, Nutraceutical and Phytopharmaceutical interventions with a pooled standardized treatment effect of −0.29. Abstract

Background: The effect of high‐flow oxygen (HFOx) and high‐flow air (HFAir) on dyspnea in nonhypoxemic patients is not known. We assessed the effect of HFOx, HFAir, low‐flow oxygen (LFOx), and low‐flow air (LFAir) on dyspnea. Subjects, Materials, and Methods: This double‐blind, 4×4 crossover clinical trial enrolled hospitalized patients with cancer who were dyspneic at rest and nonhypoxemic (oxygen saturation >90% on room air). Patients were randomized to 10 minutes of HFOx, HFAir, LFOx, and LFAir in different orders. The flow rate was titrated between 20–60 L/minute in the high‐flow interventions and 2 L/minute in the low‐flow interventions. The primary outcome was dyspnea numeric rating scale (NRS) "now" where 0 = none and 10 = worst. Results: Seventeen patients (mean age 51 years, 58% female) completed 55 interventions in a random order. The absolute change of dyspnea NRS between 0 and 10 minutes was −1.8 (SD 1.7) for HFOx, −1.8 (2.0) for HFAir, −0.5 (0.8) for LFOx, and − 0.6 (1.2) for LFAir. In mixed model analysis, HFOx provided greater dyspnea relief than LFOx (mean difference [95% confidence interval] −0.80 [−1.45, −0.15]; p =.02) and LFAir (−1.24 [−1.90, −0.57]; p <.001). HFAir also provided significantly greater dyspnea relief than LFOx (−0.95 [−1.61, −0.30]; p =.005) and LFAir (−1.39 [−2.05, −0.73]; p <.001). HFOx was well tolerated. Seven (54%) patients who tried all interventions blindly preferred HFOx and four (31%) preferred HFAir. Conclusion: We found that HFOx and HFAir provided a rapid and clinically significant reduction of dyspnea at rest in hospitalized nonhypoxemic patients with cancer. Larger studies are needed to confirm these findings (Clinicaltrials.gov: NCT02932332). Implications for Practice: This double‐blind, 4×4 crossover trial examined the effect of oxygen or air delivered at high‐ or low‐flow rates on dyspnea in hospitalized nonhypoxemic patients with cancer. High‐flow oxygen and high‐flow air were significantly better at reducing dyspnea than low‐flow oxygen/air, supporting a role for palliation beyond oxygenation. This randomized trial assessed the effect of high‐flow oxygen, high‐flow air, low‐flow oxygen, and low‐flow air on dyspnea in hospitalized cancer patients without hypoxemia. [ABSTRACT FROM AUTHOR]
