To evaluate and synthesize the evidence from the individual reviews that evaluated the efficacy of PDT therapy in root canal disinfection by undertaking an umbrella review. The protocol of the review was registered in the PROSPERO database under number CRD42021214056. The literature search was conducted using the following electronic databases: PubMed, Scopus, Web of Science, BVS, Cochrane Database of Systematic Reviews, Embase, and Epistemonikos, from inception to July 2021. Systematic reviews that evaluated the efficacy of PDT for root canal disinfection were included. Two authors independently performed a literature search, data extraction, and quality assessment of the included studies. Any disagreements were resolved by a third reviewer. The quality of the reviews was assessed using the AMSTAR 2 tool and the final categorization of each systematic reviews was classified as of "high," "moderate," "low," or "critically low" quality. Six systematic reviews were included in the current umbrella review and all of them were graded as critically low quality. From the critically low-quality evidence available, this umbrella review showed that the efficacy of PDT in root canal disinfection remains yet undetermined.

To compare the shaping ability of a heat-treated centric reciprocating file system (WaveOne Gold), a heat-treated eccentric rotary multifile system (TRUShape 3D Conforming Files), and a heat-treated expandable one-file rotary system (XP-endo Shaper) extending its activation time, in preparing oval-shaped root canals in extracted mandibular molars by means of microcomputed tomography (micro-CT) analysis.Thirty moderately curved oval-shaped distal roots of mandibular molars were selected. The normality of canal length, curvature angle, volume, surface area, structure model index, and aspect ratio were confirmed. The samples were randomly divided into three groups (n = 10). Micro-CT scans were taken before and after canals were instrumented using WaveOne Gold (size 35, .06 taper), or TRUShape (size 30, .06v taper), both following the manufacturer's instructions, or XP-endo Shaper following a new protocol with extended activation time. The mechanical preparation time for each sample was recorded. Pre- and postoperative images were analysed for the percentage of unprepared canal areas and the percentage of removed dentine. This systematic review (PROSPERO - CRD42020150722) was performed to answer the following question: Does the use of chelating agents affect the dislodgment resistance of epoxy resin-based root canal sealers? A search was conducted in PubMed, EMBASE, Scopus, LILACS and Web of Science, hand searching of two endodontic journals and grey literature until September 2020. Extracted data included: teeth used, sample size, root canal preparation, irrigant, chelating solution, neutralisation solution, type of resin-based sealer and obturation approach, storage method and duration, root third and slice thickness, plunger dimension and loading direction and the push-out testing parameters and values. The search retrieved a total of 2.389 studies. After analysis, 12 studies fulfilled the eligibility criteria and were included. Most of the final irrigation protocols with chelating agents had a positive impact and promoted an improvement in the dislodgment resistance of epoxy resin-based sealers to the root dentin.

Objective The aim of this study was to evaluate the influence of the incorporation of chlorhexidine-hexametaphosphate nanoparticles (CHX-HMP NPs) on antibacterial, cytotoxic and physicochemical properties of AH Plus (AH), MTA Fillapex (MTA) and Pulp Canal Sealer (PCS). Methods The NPs were synthesized and characterized by Scanning Electron Microscopy (SEM), Dynamic Light Scattering (DLS), zeta potential, Atomic Force Microscopy (AFM) and Energy-Dispersive X-ray Spectroscopy (EDS). The incorporation was made by weight, 2% and 5% of NPs. The antimicrobial activity, cytotoxicity, flow, radiopacity, setting time, solubility and pH were evaluated. The statistical analysis was performed by two-way analysis of variance test and Tukey post hoc test (P Results SEM analysis showed the tendency for CHX-HMP NPs to cluster, the effective mean diameter measured by DLS: 169.39 nm and the zeta potential: -10.18 mV. The NPs were individually measured by AFM: 22.99–52.75 nm. EDS analysis identified the presence of C, N, O, Na, P, Cl. After incorporation: The Direct Contact Test showed an increase in the antimicrobial action of AH, PCS and MTA; the sealers showed a decrease in flow and at 24 h of immersion also an increase in solubility, but did not affect the radiopacity of the samples; AH setting time increased and MTA did not reach setting under any of the conditions tested. All samples showed a decrease in pH value as the immersion time progressed. Significance The incorporation of NPs can improve the antimicrobial performance of endodontic sealers without impairing other biological and physicochemical properties.

To evaluate the impact of root canal preparation in teeth with ultraconservative access cavities (UltraAC) on the development of dentinal microcracks using a nondestructive longitudinal micro-computed tomography (micro-CT) imaging experimental model. Root canal preparation in teeth with traditional access cavities (TradAC) was used as control.Forty mandibular molars were scanned in a micro-CT device, anatomically matched, and distributed into 4 groups according to the type of access cavity and instrumentation system: traditional/Reciproc (TradAC/RC), traditional/XP-endo Shaper (TradAC/XP), ultraconservative/Reciproc (UltraAC/RC), and ultraconservative/XP-endo Shaper (UltraAC/XP). After root canal preparation, the teeth were rescanned. After reconstruction and co-registration procedures, the images were screened from the furcation level to the apex to identify the presence of dentinal microcracks.From a total of 15,340 cross-section images, 19.65% (3014 slices) had some dentinal microcracks. The qualitative analysis demonstrated the presence of some dentinal microcracks in 11%, 33%, 19%, and 15% of the images of cross-sections in TradAC/RC, TradAC/XP, UltraAC/RC, and UltraAC/XP groups, respectively. All dentinal microcracks observed after root canal preparation were already present in the corresponding images before canal instrumentation. Therefore, no new microcracks were detected, regardless of the access cavity and root canal instrumentation system.Root canal preparation with Reciproc or XP-endo Shaper under traditional or ultraconservative access cavities did not create dentinal microcracks in extracted mandibular molars.

This study aimed to evaluate the antibacterial efficacy and discolouration potential of antibiotic pastes containing macrogol. Seventy single-rooted teeth were contaminated with a culture of Enterococcus faecalis for 30 days. A Extensão Universitária é uma importante ferramenta de interação entre o estudante universitário e a comunidade, sendo ambos beneficiados. O projeto de extensão “Tratamento endodôntico simplificado visando sua aplicação no SUS” tem como objetivo somar forças junto ao SUS no atendimento à comunidade, de forma a reduzir a demanda reprimida e ao mesmo tempo proporcionar aos pacientes tratamentos endodônticos com novas tecnologias. Este projeto tem contribuído para a formação de recursos humanos, para a produção de conhecimento científico e para a prestação de um tratamento com maior eficiência para a comunidade. Por meio de resultados parciais da pesquisa desenvolvida no projeto, foi possível verificar que a inclusão da instrumentação acionada a motor e outras tecnologias durante o tratamento endodôntico, permitiu redução no tempo de atendimento e consequentemente contribui para a diminuição da demanda reprimida de pacientes encaminhados para o setor de Endodontia.

To evaluate the effects of alcohol and nicotine, when used alone or simultaneously, in the development of apical periodontitis induced in rats, using a correlative analytic approach with micro-CT, histological and immunohistochemical analysis.Twenty-eight male Wistar rats were arranged into four groups: Control, Nicotine, Alcohol and Alcohol + Nicotine. The alcohol groups were exposed to self-administration of a 25% alcohol solution, whilst the other groups drunk only filtered water. The nicotine groups received daily intraperitoneal injections of a solution with 0.19 μL of nicotine per mL, whilst the other groups received saline solution. The pulps of the left mandibular first molars were exposed for 28 days to induce periapical lesions. Throughout the experiment, drug administration was maintained, and the animals had their weight and solid and liquid consumption measured. After euthanasia, the mandibles were removed and the area, volume and major diameter of the periapical lesions were measured using micro-computed tomography images. The samples were submitted to histopathological evaluation and immunohistochemistry for RANKL and PTHrP. Statistical analysis was undertaken with a significance level of 5%. Nonparametric data were analysed using the Kruskal-Wallis test followed by Dunn's test, whilst one-way anova followed by Tukey's test was performed for parametric data.The alcohol groups had lower solid and liquid consumption and gained less weight when compared to the nonalcohol groups (P 0.05). The Alcohol + Nicotine group had lesions with significantly larger volume and area when compared to the other groups (P 0.05), whilst the Alcohol or Nicotine groups had significantly larger lesions than the control group (P 0.05). There was no significant difference in the largest diameter of the lesions amongst groups (P 0.05). The experimental groups had greater inflammatory response scores than the control group (P 0.05), and the representative samples had more pronounced immunoreaction against RANKL and PTHrP antibodies.Alcohol and nicotine consumption exacerbated the inflammatory response and the development of periradicular lesions in rats. The association of both substances enhanced their harmful effects.

This study evaluated the antimicrobial effectiveness of 6.5% Vitis vinifera grape seed extract (GSE) against Enterococcus faecalis biofilm using confocal laser scanning microscopy (CLSM). Saline solution (SS), 5.25% sodium hypochlorite (NaOCl) and 2% chlorhexidine (CHX) were used for comparison. Dentin discs were inoculated with E. faecalis strain establishing a 3-week-old biofilm. This study aimed to evaluate the preservation of periradicular dentin and the enlargement of the apical canal of mandibular molars with TruNatomy (Dentsply Sirona, Ballaigues, Switzerland) and ProTaper Gold (Dentsply Sirona) instruments.Twenty mandibular molars were scanned in a micro-computed tomographic device, anatomically paired, and distributed into 2 groups (n = 10). In the ProTaper Gold group, mesial and distal canals were prepared up to F2 (25/.08v) and F3 (30/.09v) instruments, whereas in the TruNatomy group, mesial and distal canals were enlarged up to the prime (26/.04v) and medium (36/.03v) instruments, respectively. After a new scan, the surface area, volume, unprepared areas, transportation, percentage of dentin removal, and dentin thickness parameters were calculated. Data were compared between groups using the Mann-Whitney test, the Student t test, and the nonmetric multidimensional scaling test with alpha set at 5%.No difference was found between groups regarding unprepared canal areas and the reduction of dentin thickness (P.05). Transportation was lower than 0.1 mm in all groups, and statistical differences were observed only at the apical third of the mesiobuccal canal with lower values in the TruNatomy group. ProTaper Gold removed more dentin than TruNatomy at the coronal level of mesial roots (1.8% and 1.0%, respectively) (P.05).TruNatomy and ProTaper Gold were efficient for performing canal preparation in mandibular molars. Methods: Sixty-seven dentin cylinders of single-rooted teeth were contaminated with a culture of Enterococcus faecalis (ATCC 29212; American Type Culture Collection, Manassas, VA) for 5 days. Samples were divided into 1 control group and the following experimental groups according to the medication applied: traditional triple antibiotic paste (TAP), clindamycin-modified TAP (mTAP), triple antibiotic medication with macrogol (3Mix-MP), clindamycin-modified 3Mix-MP (m3Mix-MP), calcium hydroxide (CH), and ethanol extract of propolis (EEP). After 14 days, the medications were removed, and the samples were submitted to confocal laser scanning microscopic analysis to quantify the percentage of viable bacteria. The distribution of data was confirmed by the Shapiro-Wilk test. Objectives The aim of this study was to evaluate the shaping ability of the TruShape and Reciproc Blue systems and the apical extrusion of debris after root canal instrumentation. The ProTaper Universal system was used as a reference for comparison. Materials and Methods Thirty-three mandibular premolars with a single canal were scanned using micro-computed tomography and were matched into 3 groups (n = 11) according to the instrumentation system: TruShape, Reciproc Blue and ProTaper Universal. The teeth were accessed and mounted in an apparatus with agarose gel, which simulated apical resistance provided by the periapical tissue and enabled the collection of apically extruded debris. During root canal preparation, 2.5% sodium hypochlorite was used as an irrigant. The samples were scanned again after instrumentation. The percentage of unprepared area, removed dentin, and volume of apically extruded debris were analyzed. The data were analyzed using 1-way analysis of variance and the Tukey test for multiple comparisons at a 5% significance level. Results No significant differences in the percentage of unprepared area were observed among the systems (p > 0.05). ProTaper Universal presented a higher percentage of dentin removal than the TruShape and Reciproc Blue systems (p < 0.05). The systems produced similar volumes of apically extruded debris (p > 0.05). Conclusions All systems caused apically extruded debris, without any significant differences among them. TruShape, Reciproc Blue, and ProTaper Universal presented similar percentages of unprepared area after root canal instrumentation; however, ProTaper Universal was associated with higher dentin removal than the other systems.

Objectives This study aimed to analyze the main features of the 25 most-cited articles in minimally invasive access cavities. Materials and Methods An electronic search was conducted on the Clarivate Analytics' Web of Science ‘All Databases’ to identify the most-cited articles related to this topic. Citation counts were cross-matched with data from Elsevier's Scopus and Google Scholar. Information about authors, contributing institutions and countries, year and journal of publication, study design and topic, access cavity, and keywords were analyzed. Results The top 25 most-cited articles received a total of 572 (Web of Science), 1,160 (Google Scholar) and 631 (Scopus) citations. It was observed a positive significant association between the number of citations and age of publication (r = 0.6907, p < 0.0001); however, there was no significant association regarding citation density and age of publication (r = −0.2631, p = 0.2038). The Journal of Endodontics made the highest contribution (n = 15, 60%). The United States had the largest number of publications (n = 7) followed by Brazil (n = 4), with the most contributions from the University of Tennessee and Grande Rio University (n = 3), respectively. The highest number of most-cited articles were ex vivo studies (n = 16), and ‘fracture resistance’ was the major topic studied (n = 10). Conclusions This study revealed a growing interest for researchers in the field of minimally invasive access cavities. Future trends are focused on the expansion of collaborative networks and the conduction of laboratory studies on under-investigated parameters.

To evaluate the influence of pH variation on the solubility and water sorption of a premixed calcium silicate-based root canal sealer (EndoSequence BC Sealer) compared to the gold standard based on epoxy resin (AH Plus Jet) after immersion in distilled water and phosphate-buffered saline (PBS).Solubility and water sorption were evaluated after immersion in distilled water or PBS at several pHs (5, 7 and 12) and the values were calculated as percentages of the original mass after 24 h, 7 and 30 days of immersion. The crystalline structures present in the sealers and surface precipitates were assessed by X-ray diffraction. The Shapiro-Wilk's test revealed that data were normally distributed; thus, statistical analysis was performed using one-way anova and Tukey's tests or independent t-test, assuming a 5% α-error.EndoSequence BC Sealer was associated with significantly greater water sorption and solubility compared to AH Plus Jet in all tested conditions (P 0.05). The acid environment increased the 24 h solubility of EndoSequence BC Sealer immersed in PBS (P 0.05) and did not induce significative changes in the water sorption (P 0.05). Alkaline pH reduced the solubility of EndoSequence BC Sealer and increased that of AH Plus Jet at all experimental times and soaking media (P 0.05). Alkaline environment also significantly increased the water sorption of AH Plus Jet immersed in PBS (P 0.05). Immersion in PBS significantly reduced the solubility of EndoSequence BC Sealer and significantly increased that of AH Plus Jet (P 0.05). Precipitates on the surface of EndoSequence BC Sealer corresponding to hydroxyapatite and calcium carbonate were detected after immersion in PBS at pH 5 and 7 for 30 days.EndoSequence BC Sealer had significantly greater solubility and water sorption than AH Plus Jet. Although the alkaline pH and soaking media directly influenced the solubility and water sorption of the sealers, the solubility of AH Plus Jet remained within the limits recommended by ISO 6876, whilst the solubility of the EndoSequence BC Sealer did not comply with ISO recommendations in all the conditions tested.

To evaluate the use of a reduced taper endodontic instrument system (Bassi Logic™ .03 taper) and expandable heat-treated system (XP-endo Shaper) on the ability to shape canals in mandibular molars, by means of microcomputed tomography (micro-CT) analysis. The Reciproc system was used as the reference for comparison.Twenty-four mandibular molars were scanned in a micro-CT, matched based on similar anatomical features and sorted into three groups (n = 8 per group), according to root canal preparation system: Bassi Logic™ .03, XP-endo Shaper and Reciproc. The teeth were mounted onto a mannequin and the pulp chambers were accessed with traditional access cavities. In Bassi Logic™ .03 and Reciproc groups, mesial canals were prepared with size 25, .03 taper or R25 (size 25, .08v taper) instruments and the distal root canal with size 25, .03 taper and size 40, .03 taper or R25 and R40 (size 40, .06v taper), respectively. In XP-endo Shaper groups, all root canals were prepared with XP-endo Shaper (size 30, .04v taper). After root canal preparation, the teeth were rescanned. The percentage of untouched canal areas and the percentage of removed dentine were evaluated separately for mesial and distal root canals. The data were analysed using one-way anova and Tukey tests (P 0.05).The Bassi Logic™ .03 group was associated with a greater percentage of untouched canal areas when compared to XP-endo Shaper and Reciproc groups for mesial and distal root canals (P 0.05), but no differences were found between XP-endo Shaper and Reciproc groups (P 0.05). No significant difference was observed in the percentage of dentine removed amongst the groups for mesial and distal root canals (P 0.05).The use of a reduced taper system (Bassi Logic™ .03) during root canal preparation resulted in a greater percentage of untouched canal areas when compared to XP-endo Shaper and Reciproc instruments. BACKGROUND/AIMS Cross-country mountain biking and field hockey are two Olympic sports that pose a potential risk for dentofacial trauma. However, mouthguard use is not mandatory in either of these sports and knowledge about tooth rescue among athletes is often neglected. The aim of this cross-sectional epidemiological survey was to evaluate the prevalence of sports-related dentofacial injuries, mouthguard use, and attitudes regarding tooth rescue among cross-country mountain biking and field hockey athletes participating at pre-Olympic competitions held in Rio de Janeiro, Brazil. MATERIALS AND METHODS A convenience sample of 217 athletes from 33 countries participating in cross-country mountain biking (n = 82; mean age = 30.96 ± 8.52 years) and field hockey (n = 135; mean age = 19.72 ± 2.46 years) pre-Olympic competitions were examined clinically, and they answered a questionnaire regarding previous history of sports-related dentofacial injuries, attitudes toward mouthguard use, and tooth rescue. To evaluate the influence of ultraconservative endodontic access cavities (UEC) and the use of instruments with various tapers (.03 or .05) and tip diameters (25 or 40) on the ability to shape canals in mandibular molars and their fracture resistance in comparison with traditional endodontic access cavities (TEC).Thirty-two intact extracted mandibular molars were scanned in a micro-computed tomography device, matched based on similar anatomical features and assigned to UEC or TEC groups and to subgroups according to mechanical canal preparation with Bassi Logic™ rotary NiTi instruments with .03 or .05 taper (n = 8 per group). The teeth were mounted onto a mannequin head, and the pulp chambers were accessed and the root canals subjected to instrumentation. In the first stage, size 25 endodontic instruments were used, with .03 or .05 tapers. The teeth were re-scanned. Subsequently, a second canal instrumentation was performed with the size 40 instruments with .03 or .05 tapers and scanned again. The root canals were filled and then the teeth restored before being subjected to fracture resistance tests. The data on untouched canal areas, canal transportation, centring ability, the percentage of removed dentine and maximum load at fracture were analysed. One-way anova and Tukey's tests were performed for inter-group analysis. The paired-samples t-test was performed on the results between size 25 and size 40 apical preparations for each individual specimen (P 0.05).No significant difference was observed for the percentage of untouched canal area when comparing UEC and TEC groups or between the .03 and .05 tapers with the same instrument tip size (P 0.05). There was a significantly lower percentage of untouched canal area after instrumentation with size 40 instruments when compared to size 25 (P 0.05). No significant difference in the percentage of dentine removed was observed between the access cavities or the different tapers (P 0.05). Size 40 apical preparations removed significantly more dentine than size 25 preparations (P 0.05). No significant differences were observed in canal transportation, centring ability and fracture resistance in any of the tested groups (P 0.05).Ultraconservative endodontic access cavities did not offer any advantages in comparison with traditional endodontic access cavities on the ability to shape canals, nor the fracture resistance of mandibular molars. No differences were observed in shaping ability and fracture resistance between .03 and .05 taper canal preparations. Apical preparation with larger instruments resulted in significantly less untouched canal area in all groups.

To assess the impact of conservative endodontic access cavities (CEC) and truss access cavities (TAC) during root canal treatment performed on mandibular molars in terms of: ability to shape and fill root canals, microbial reduction in canals, and cleaning of the pulp chamber. In addition, the fracture resistance of the teeth after coronal restoration was assessed. Traditional endodontic cavities (TEC) were used as a reference technique for comparison.Thirty extracted intact mandibular molars were scanned in a microcomputed tomography device (micro-CT), matched based on similar anatomical features and assigned to TEC, CEC or TAC groups (n = 10). The specimens were accessed accordingly, and root canals were contaminated with bacterial suspensions of Enterococcus faecalis (21 days). Subsequently, the first microbial sample was collected from root canals (S1). The canals were initially prepared with Reciproc Blue R25 instrument followed by a second instrumentation using Reciproc Blue R40. Eight mL of 0.5% NaOCl were used as an irrigant for each instrument. A final irrigation protocol was performed with 2 mL of 0.5% NaOCl, 2 mL of 17% EDTA and another 2 mL of 0.5% NaOCl. Microbial samples were collected from root canals after R25 (S2), R40 (S3) and final irrigation (S4). The teeth were rescanned after S4. Then, root canals were filled, rescanned, restored and the teeth subjected to fracture resistance tests. The statistical analysis was performed with type I negative binomial and beta 0-1 inflation regression models for microbiological analysis. Instrumentation, filling and resistance to fracture results were subjected to anova and Tukey tests (P 0.05).S4 revealed no significant variations in microbial reduction amongst the groups (P 0.05). TEC had a significantly lower percentage of unprepared surface area than CEC (P 0.05). No differences were found regarding the percentage of dentine removed, transportation, centring ability and filling voids amongst the groups (P 0.05). The TEC group had a significantly lower volume of remaining root filling material within the pulp chamber than CEC and TAC groups (P 0.05). There was no difference regarding fracture resistance amongst the groups (P 0.05).Conservative access cavities did not offer any advantage in comparison with the traditional endodontic cavities in any of the parameters considered. Furthermore, conservative methods were associated with larger percentages of unprepared canal surface area and larger volumes of remaining root filling material within the pulp chamber.

Objective: This study aimed to assess the antibacterial effect of Easy Clean®, passive ultrasonic irrigation and sonic irrigation against Enterococcus faecalis biofilm in oval-shaped canals. Methods: Fifty-five extracted human teeth with similar dimensions were selected. Access cavities were prepared and the root canals were instrumented using Wave One® Primary files (25.08). The root canals were then contaminated with an E. faecalis suspension following incubation for thirty days. The contaminated roots were divided into three experimental groups (n=15) according to the irrigant agitation protocol (Easy Clean, passive ultrassonic irrigation and sonic irrigation), in addition to a positive control group (n=5) and a negative control group (n=5). Microbiological samples were taken from the root canals before instrumentation (S1), after instrumentation (S2) and after the final irrigation protocol (S3). The samples were assayed and incubated for 48 hours in order to obtain the residual titer of E. faecalis cells. Viable cells were quantified by colony-forming units (CFU) measurement. The collected data was submitted to statistical analysis by using Shapiro-Wilk`s test, Wilcoxon paired test, Kruskal–Wallis test and Dunn’s post-hoc test. The level of significance was set at 5%. Results: All experimental groups presented significant reduction on bacterial load after instrumentation (P0.05). Conclusion: The results showed that all tested groups exhibited similar disinfection effectiveness. However, none of them was able to render all root canals free from microorganisms.

Aim: This study investigated the sealing ability produced by AH Plus (Dentsply DeTrey, Konstaz, Germany), EndoSequence BC sealer (Brasseler USA, Savannah, GA, USA), GuttaFlow (Coltène/Whaledent, Altstätten, France) and MTA Fillapex (Angelus, Londrina, Brazil). Methdos: A total of forty-six single-root human canines were prepared and randomly divided into four experimental groups (n=10): MTA Fillapex, EndoSequence BC Sealer, AH Plus or GuttaFlow. Teeth with intact crowns served as negative controls (n=3) and teeth filled with only gutta-percha served as positive controls (n=3). Teeth were mounted in a two-chamber apparatus and exposed to Enterococcus faecalis. The number of days over a 60-days period was recorded for the appearance of turbidity in the lower chamber. Kaplan-Meier method was used to estimate the survival curves. The nonparametric log-rank test was used to compare the survival curves using a significance level of 0.05. Results: The results at the end of the observation time were further analyzed by chi-square testing. All positive controls leaked within 24 h, whereas none of the negative controls leaked after 60 days. EndoSequence BC Sealer and MTA Fillapex had significant less bacterial leakage (P0.05). Conclusion: In conclusion, calcium-silicate based root canal sealers promoted improved sealing ability when compared to other endodontic sealers.

Objective Enterococcus faecalis is a gram-positive facultative anaerobic bacterium, which is present in 30–89% of teeth with postendodontic treatment failures. E. faecalis is capable of penetrating dentinal tubules and surviving as a monoculture after conventional endodontic therapy, indicating that it is resistant to commonly used endodontic disinfection protocols. Different E. faecalis strains have shown resistance to several antibiotics, and have been associated with both dental pathology and systemic infections. The aim of this study was to evaluate the efficacy of a genetically engineered bacteriophage to disinfect dentin infected with antibiotic resistant strains of E. faecalis. Methods Extracted human dentin root segments were cemented into sealable two-chamber devices, fabricated from syringe needle caps to form in vitro infected-dentin models. The models were inoculated with an overnight suspension of either E. faecalis V583 (vancomycin resistant strain) or E. faecalis JH2-2 (fusidic acid and rifampin resistant, vancomycin sensitive strain). After 7 days of incubation at 37 °C, a suspension of a genetically engineered phage, ϕEf11/ϕFL1C(Δ36)PnisA, was added to the root canal of each infected dentin segment, and the incubation was continued for an additional 72-h. Dentin was harvested from the walls of each root canal and assayed for the residual titer of E. faecalis cells. Results The recovered E. faecalis titer was reduced by 18% for the JH2-2 infected models, and by 99% for the V583 infected models. Conclusion Treatment: of E. faecalis-infected dentin with bacteriophage ϕEf11/ϕFL1C(Δ36)PnisA consistently resulted in a decrease in the residual bacterial population of both vancomycin-sensitive and resistant strains.

BACKGROUND Controversial findings exist in the literature regarding the association between tobacco smoking and development of apical periodontitis or need for root canal treatment, with some studies reporting an increase in the prevalence of these outcomes in smokers, whilst others reporting no association. AIM To evaluate if there is scientific evidence to support an association between tobacco smoking and a greater prevalence of apical periodontitis and/or root canal treatments. DATA SOURCES A systematic search was performed using MeSH terms and free terms in the PubMed, Scopus, Virtual Health Library (VHL), Cochrane library and Open Gray databases. STUDY ELIGIBILITY CRITERIA, PARTICIPANTS AND INTERVENTIONS Observational studies that evaluated the association between smoking and the development or healing of periapical lesion and/or the prevalence of root canal treatment in humans were included. STUDY APPRAISAL AND SYNTHESIS METHODS A quality assessment of included studies was performed, and the relevant information and findings were evaluated. A meta-analysis using nine included studies was performed through RevMan software, and the certainty of evidence was evaluated through GRADE. RESULTS Fifteen studies were included, of which 10 were classified as low risk of bias, 4 were considered as moderate risk of bias and 1 as high risk of bias. Ten studies reported that smoking was associated with a greater prevalence of periapical periodontitis and/or root canal treatment. Five studies reported no association. Nine studies were included in the pooled meta-analysis, comprising two subgroups: apical periodontitis and root canal treatment. From these nine studies, eight studies were included in the apical periodontitis subgroup meta-analysis and demonstrated that this condition was significantly more prevalent in smokers when compared to the nonsmokers (odds ratio = 2.78[CI: 1.60, 4.85], P

The purpose of this work was to evaluate the physicochemical properties, the cytotoxicity and in vivo biocompatibility of MTA Repair HP (MTA HP) and White MTA (WMTA). The setting time, flow, radiopacity and water solubility were assessed. To the cytotoxicity assay, primary human osteoblast cells were exposed to several dilutions of both materials eluates. MTT assay, apoptosis assay and cell adhesion assay were performed. The in vivo biocompatibility was evaluated through histological analysis using different staining techniques. No differences were observed between MTA HP and WMTA for setting time, radiopacity, solubility and water absorption (P > 0.05). However, MTA HP showed a significantly higher flow when compared to WMTA (P in vivo biocompatibility to the WMTA and the control group in all time-points. The MTA HP presented adequate physicochemical and biological properties with improved flow ability when compared to WMTA. Such improved flow ability may be a result of the addition of a plasticizing agent and should be related to an improvement in the handling of MTA HP.

Reducing the accumulation of microorganisms on an endodontic file during endodontic treatment is important to limit recontamination of the root canal and increase likelihood of successful treatment outcome. Objective: To compare the antimicrobial activity of peracetic acid (PA), isopropyl alcohol and acetone against a range of bacteria and also for disinfection of contaminated endodontic K-files. Material and Methods: Antimicrobial activities of PA, isopropyl alcohol and acetone were compared against Staphylococcus aureus, Enterococcus faecalis, Escherichia coli, Pseudomonas aeruginosa, vancomycin resistant E. faecalis (VRE) and meticillin resistant S. aureus (MRSA), using minimum bactericidal concentration (MBC) and time-kill assays. Test solutions at different exposure times (15 s and 30 s) were assessed for treatment of endodontic files acting as carriers of E. faecalis-contaminated dental debris. Results: All bacteria were susceptible to PA (MBC range 0.25-1%), acetone (MBC range 50-60%) and isopropyl alcohol (30-40%). Using a time-kill assay of the antimicrobials at the determined MBC, all test microorganisms, with the exception of E. faecalis (VRE) 7766 were killed after 15 s exposure. In the case of E. faecalis 7766, viable cells remained detectable after 120 s exposure to acetone. Testing disinfection of endodontic K-files, previously coated with dental debris containing E. faecalis, it was found that PA (2%) completely killed E. faecalis after 15 s exposure. However, even after 30 s exposure, isopropyl alcohol (80%) and acetone (80%) had limited disinfecting activity. Conclusion: Extrapolation of these results to clinical practice would suggest that PA would be the most effective agent for trans-operative disinfection of endodontic K-files during treatment of a single patient.

Aim To evaluate the effect of 17% ethylenediaminetetraacetic acid (EDTA), 2.25% peracetic acid (PA) or 10% citric acid (CA) on the push-out bond strength of calcium silicate-based endodontic sealers [MTA Fillapex (Angelus, Londrina, Brazil) and Total Fill BC Sealer (FKG, La Chaux-de-Fonds, Switzerland)] to root dentine after different periods of immersion in phosphate-buffered saline (PBS). AH Plus (Dentsply DeTrey, Konstaz, Germany) was used as a reference material for comparison. Methodology Thirty human maxillary incisors were selected and three 1-mm discs were obtained from the middle third of each root. On the axial surface of each disc, three 0.8-mm-diameter holes were drilled through the dentine. After standardized irrigation, each dentine disc from the same root was treated for smear-layer removal with one of the three tested chelating solutions: EDTA, PA or CA. After that, the holes were dried with paper points and filled with one of the three test root canal sealers. The filled dental slices were further immersed in PBS solution (pH 7.2) for 7 days (T1) or 30 days (T2) at 37 °C, before the push-out assay. Friedman, Kruskal–Wallis and Mann–Whitney tests were used respectively to estimate the impact of the independent variables (solutions, sealers and period of PBS immersion) on the push-out bond strength. The level for significance was set at α = 5% and data were further subjected to a Bonferroni correction. Results Solutions and periods of immersion did not significantly influence the displacement resistance (P = 0.285, and P = 0.104, respectively) of sealers to root dentine. On the other hand, the type of sealer significantly affected the push-out bond strength (P = 0.000). AH Plus was associated with greater resistance to dislodgment compared with Total Fill BC Sealer and MTA Fillapex (P = 0.000). MTA Fillapex resulted in the lowest push-out bond strength (P = 0.000). Conclusion The use of different chelating agents did not influence the push-out bond strength of endodontic sealers. Calcium silicate-based sealers had lower push-out bond strength values compared with a conventional epoxy resin-based sealer (AH Plus).

During endodontic treatment, the maintennance of the aseptic chain is mandatory. Therefore, all substances, instruments, and medications inserted into root canals must be free of microorganisms. Objective: To evaluate in vitro and microbiologically the contamination of absorbent paper points. Material and methods:Absorbent paper points of different brands and under different conditions were evaluated and subdivided into the following groups: esterilized by the manufacturer (G1 to G5), not esterilized by the manufacturer (G6), esterilized in autoclave by the operator (G7), and intentionally contaminated (G8). The two last groups were the positive and negative controls, respectively. All paper points were unpacked and immersed into tubes with culture medium for 48 hours and then analyzed according to culture broth turbidity. Results: Only the samples esterilized by the manufacturers Dentsply® (G1), Endopoints®(G2), and Meta® (G3), and the samples esterilized in autoclave by the operator (G7) did not evidence microbial growth after 48 hours. Objetivo: O objetivo deste estudo foi avaliar a influência do consumo crônico de álcool no aumento da destruição óssea periapical em ratos. Material e métodos: Foram selecionados 12 ratos machos da linhagem Wistar, aleatoriamente divididos nos grupos controle e álcool (n=6). Os ratos do grupo álcool foram submetidos à auto-administração de solução alcoólica contendo 25% de álcool puro. O grupo controle recebeu apenas água filtrada durante o estudo. Após as 5 semanas de adaptação do grupo álcool, todos os ratos foram anestesiados e a polpa dos seus primeiros molares inferiores esquerdos foi exposta à cavidade oral para indução da lesão periapical. Após 28 dias de exposição pulpar, os ratos foram sacrificados por sobredose de anestesia, e suas mandíbulas foram removidas e seccionadas para avaliação microtomográfica. This study aimed to quantitatively assess the ability of two single-step restorative materials to avoid crown darkening caused by the use of minocycline as an intracanal medicament. After coronal access and instrumentation, 120 maxillary incisors were divided into four groups (n = 30). Two experimental groups according to the restorative material applied to the inner walls of the access cavity: OB, OptiBond All-In-One + minocycline intracanal paste; U200, RelyX U200 + minocycline intracanal paste. Two control groups without restorative materials: MIN, minocycline intracanal paste and SL, saline intracanal. Color determination was performed using a spectrophotometer at five time points, immediately after materials were applied (baseline), and at 7, 14, 21, and 28 days from the baseline. Decrease in the mean values of L* (luminosity) was observed after insertion of minocycline paste in all groups at all time points. Os materiais de referência utilizados para comparação foram os cimentos endodônticos MTA Fillapex e AH Plus. Vinte incisivos superiores humanos foram selecionados e um slice dentinário de 1 mm de espessura foi obtido do terço cervical de cada raiz. Na superfície coronária de cada slice, quatro orifícios com 0,9 mm de diâmetro foram confeccionados através da dentina. Uma irrigação padronizada foi realizada e os orifícios foram preenchidos com um dos quatro cimentos endodônticos avaliados: AH Plus, MTA Fillapex, MTAe, e MTAe-HA. Os slices preenchidos foram armazenados em solução PBS (pH 7,2) durante 7 dias a 37°C. O ensaio de push-out foi realizado por meio de um dispositivo com 0,7 mm de diâmetro. A carga foi aplicada com a velocidade de 0,5 mm/min até a obtenção de deslocamento do material obturador. Os resultados foram expressos em MPa. O teste de Kruskal-Wallis foi aplicado para avaliar o efeito da resistência de união de cada cimento endodôntico. O teste de Mann-Whitney com correção de Bonferroni foi utilizado para isolamento das diferenças. O erro do tipo-alfa foi fixado em 0,05. Abstract This study investigated the bond strength of two experimental root canal sealers based on MTA and butyl ethylene glycol disalicylate: MTAe and MTAe-HA. The reference materials used for comparison were AH Plus and MTA Fillapex. Twenty human upper incisors were selected and one 1 mm slice was obtained from the cervical third of each root. On the coronal surface of each slice, four 0.9 mm wide holes were drilled through the dentine. Standardized irrigation was performed and holes were filled with one of the four tested sealers: AH Plus, MTA Fillapex, MTAe, and MTAe-HA. The filled slices were stored in a PBS solution (pH 7.2) for 7 days at 37 °C. A push-out assessment was performed with a 0.7 mm plunger tip. Load was applied at a crosshead speed of 0.5 mm/min until sealer displacement. The results were expressed in MPa. The Kruskal-Wallis test was applied to assess the effect of each sealer on the push-out bond strength. Mann-Whitney with Bonferroni correction was used to isolate the differences. The alpha-type error was set at 0.05. Significant differences among medians values obtained by materials were observed (p0.05). The current external inflammatory root resorption treatment protocol, which uses calcium hydroxide dressing, usually comprises multiple and long-term applications. In addition to the need for multiple appointments for calcium hydroxide replacement, the long-term maintenance of this compound in the root canal weakens dental structures. A modification of this therapy would be advisable. In this clinical investigation, 3 patients with external inflammatory root resorption were submitted to revascularization therapy protocol usually used in teeth with necrotic pulp and open apices. The teeth were treated with revascularization therapy protocol, which consisted of disinfecting the root canal system with triantibiotic paste, filling it with blood clot, and sealing of the root canal with mineral trioxide aggregate and bonded resin restoration. During the follow-up, the pathologic process was arrested with tissue repair in pre-existing radiolucent areas. Reduced mobility was observed in the treated teeth. The 3 cases were followed up for 30, 18, and 15 months, respectively. All teeth remained asymptomatic and retained function and physiological mobility. The therapy used in the revascularization procedure was efficient in the treatment of external inflammatory root resorption, reducing the number of appointments and increasing patient compliance.

The aim of this in vitro study was to evaluate the cleaning and shaping efficacy of rotary nickel-titanium ProTaper® instrumentation in oval canals and the influence of ultrasonic irrigation on the final refinement. Material and methods: Twenty mandibular molars were accessed and divided in two groups. Only distal roots with oval canals were used. Group I was instrumented using only rotary nickel-titanium ProTaper® instruments. Group II received the same preparation followed by refinement with 3 minutes of ultrasonic passive irrigation. After preparation, the distal roots were sectioned for histological processing. Coronal, middle and apical thirds were analyzed. Uninstrumented perimeter in each third was measured by the software Image tool 3.0. Results: The percentage of uninstrumented perimeter was calculated for each third of each root and average percentage was calculated for each third in both groups. Statistical analysis was performed with t-test through the software SPSS 11.0.1 for Windows. The level of significance was set at 1%. Group I showed higher percentage of uninstrumented perimeter in all thirds. Group II, which received ultrasonic irrigation showed better results in all thirds (p < 0.01). Comparing the three thirds in the same group, no statistically significant differences were found (p > 0.05). Conclusion: Under the conditions of this ex vivo study, the three minutes use of ultrasonic irrigation after rotary instrumentation resulted in significantly more instrumented walls in the distal roots of mandibular molars.

Objective: This study evaluated the cleaning efficacy of the rotary nickel-titanium Mtwo ® by means of optical microscopy in oval-shaped root canals and compared refinement influence using ultrasonic files or Hedstroem files. Materials and Methods: Twenty mandibular incisors were accessed, included in epoxy resin blocks, and cut transversely at 3, 6, and 9 mm from the apex. Teeth were instrumented using rotary nickel-titanium Mtwo. Digital images of the root cross sections were obtained before and after instrumentation with a digital camera attached to an optical microscope. Subsequently, samples were randomly divided into two groups for refinement. Group I was refined using ultrasonic files #30 and group II with Hedstroem files #30. Final digital images of cross sections after refinement were captured and analyzed. The boundaries for the uninstrumented perimeter were measured by the software ImageTool 3.0. The data obtained were statistically analyzed by analysis of variance (ANOVA) and t -test. Results: Mtwo instrumentation produced 41.48% of the uninstrumented perimeter. The final refinement using Hedstroem and ultrasonic files decreased the uninstrumented perimeter. Significant differences ( t -test, P = 0.00) were found at the three thirds after the final refinement, except for the apical third that was enhanced by Hedstroem file. However, there was no statistical difference between the two refinement techniques used ( P > 0.05). Mtwo instrumentation was not capable of cleaning and shaping the entire perimeter of the root canals walls. Conclusions: The final refinement, either with ultrasonic files or with Hedstroem files, resulted in significantly less uninstrumented perimeter.

Aim To evaluate ex vivo, apical bacterial extrusion associated with two reciprocating single-file systems (WaveOne and Reciproc) compared with a conventional multifile rotary system (BioRace). Methodology Forty-five human single-rooted mandibular incisors were used. Endodontic access cavities were prepared, and root canals were contaminated with an Enterococcus faecalis suspension. Following incubation at 37 °C for thirty days, the contaminated teeth were divided into three groups of 15 specimens each (G1 – Reciproc, G2 – WaveOne and G3 – BioRace). Positive and negative controls consisted of 5 infected teeth and 3 uninfected incisors that were instrumented with one of the tested NiTi systems, respectively. Bacteria extruded from the apical foramen during instrumentation were collected into vials containing 0.9% NaCl. The microbiological samples were taken from the vials and incubated in brain heart agar medium for 24 h. The resulting bacterial titre, in colony-forming units (CFU) per mL, was determined, and these data were analysed by Wilcoxon matched-pairs signed rank test and Kruskal–Wallis H-test. The level of significance was set at α = 0.05. Results No significant difference was found in the number of CFU between the two reciprocating systems (P = 0.41). The conventional multifile rotary system group was associated with significantly higher CFU than both of the two reciprocating groups (P = 0.01). Conclusions All instrumentation systems extruded bacteria beyond the foramen. However, both reciprocating single-file systems extruded fewer bacteria apically than the conventional multifile rotary system.

This study evaluated the antimicrobial efficacy of 2.5% and 5.25% NaOCl, 2% gel and liquid CHX and MTAD against Enterococcus faecalis biofilms on human dentin. Material and methods: E. faecalis biofilms grown on dentin matrix of 216 root sections were submerged in test irrigants for 1, 5, 15 and 30 minutes. The antimicrobial activity of the test irrigants were assessed through CFU counts. Biofilm formation over the dentin surface was ensured by SEM analysis. Results: Results showed no statistic difference among CHX gel, 2.5% and 5.25% NaOCl. However, the CHX liquid and MTAD were less effective than 2.5% and 5.25% NaOCl. Only CHX liquid and MTAD had differences in its efficacy depending on the time. Conclusion: The most effective irrigants in eliminating E. faecalis biofilms were 2.5% and 5.25% NaOCl and 2% CHX gel, at all the tested time intervals, in comparison to CHX liquid and MTAD.

Objective: The present study aimed to evaluate the bending resistance and cyclic fatigue fracture resistance of a new single-file reciprocating instrument called WaveOne Gold. Reciproc and WaveOne instruments were used as references for comparison. Methods: Sixty 25-mm NiTi instruments (Reciproc R25, WaveOne Primary and WaveOne Gold Primary) were tested. Flexibility was determined by applying 45° bending tests using a universal testing machine (n=10). A custom-made device was used during cyclic fatigue test (n=10), comprising a stainless steel artificial canal measuring 1.4 mm in diameter, 19 mm in total length with an 86° angle and 6 mm radius of curvature. Objectives: The purpose of this study was to evaluate the capacity of BioPure MTAD to induce genetic damage in vitro. Genotoxicity was assessed by the single cell gel (comet) assay.Methods: Chinese hamster ovary (CHO) cells or murine fibroblasts cells were exposed to increasing final concentrations ranging from 0.1 a 10%. All treatments were performed for 1 hour at 37°C. The negative control group was treated with vehicle control (phosphate buffer solution - PBS) for 1 hour at 37°C and the positive control group was treated with methylmetanesulfonate (at 1 µM) for 1 hour at 37°C.Results: Present results showed that the BioPure MTAD was able to promote DNA breakage in CHO cells only at the highest concentration tested as well as to induce significant increase in tail moment at all tested concentrations in murine fibroblasts.Conclusions: In summary, our results indicate that BioPure MTAD is a genotoxic agent as depicted by the single cell gel (comet) assay. Aim To examine the microbiological status of primary endodontic infections in teeth with and without a sinus tract. Methodology Samples were collected by means of a size 15 H-type file and two sterile paper points from 30 cases of primary endodontic infections with (n = 15) or without (n = 15) a sinus tract. The presence of 40 bacterial species was determined by the checkerboard DNA-DNA hybridization method. Results The species found at the highest levels and prevalence were Fusobacterium nucleatum sp. vincentii, Porphyromonas gingivalis, Veillonella parvula, Enterococcus faecalis, Campylobacter gracilis and Neisseria mucosa. The aim of this study was to evaluate the antimicrobial capacity of sodium hypochlorite (1% and 5%) and chlorhexidine (0.12%, 0.5% and 1%) with or without the addition of organic material (bovine serum albumin, BSA) against some bacterial samples (Staphylococcus aureus, Enterococcus faecalis, Escherichia coli, Porphyromonas gingivalis and Fusobacterium nucleatum) using two activity tests (contact and diffusion agar tests). In the contact test (first model), bacterial samples were kept in contact with each irrigating solution for different time intervals: immediately (t(0)), 5 min (t(5)), 15 min (t(15)) and 30 min (t(30)). The agar diffusion test was the second model used. In half the specimens, 0.5% BSA was added to simulate organic tissue present in the root canal. Bacterial growth was evaluated for each microorganism and activity test. Each test was repeated 10 times. In the contact test, 0.12% chlorhexidine solution (CHX) did not eliminate E. faecalis at any tested time. CHX at 0.5% eliminated all strains except E. faecalis after immediate contact. All strains were eliminated by 1% CHX, 1% NaOCl and 5% NaOCl. BSA did not interfere with the antimicrobial activity of the irrigating solutions. In the agar diffusion test, all solutions exhibited zones of antimicrobial activity; however, BSA interfered with the antimicrobial activity of NaOCl and CHX. Under the condition of the contact test, the 0.12% CHX was ineffective in eliminating E. faecalis, while 0.5% CHX, 1% CHX, 1% NaOCl and 5% NaOCl showed antibacterial effectiveness against all the tested bacterial strains. The addition of an organic load interfered with the accuracy of the agar diffusion test.

Objective Enterococcus faecalis is a Gram-positive, facultative anaerobic bacterium that is associated with failed endodontic cases and nosocomial infections. E. faecalis can form biofilms, penetrate dentinal tubules and survive in root canals with scarce nutritional supplies. These properties can make E. faecalis resistant to conventional endodontic disinfection therapy. Furthermore, treatment may be complicated by the fact that many E. faecalis strains are resistant to antibiotics. A potential alternative to antibiotic therapy is phage therapy. ϕEf11 is a temperate phage that infects strains of E. faecalis . It was previously sequenced and genetically engineered to modify its properties in order to render it useful as a therapeutic agent in phage therapy. In the current study, we have further genetically modified the phage to create phage ϕEf11/ϕFL1C(Δ36) PnisA . The aim of this study was to evaluate the efficacy of bacteriophage ϕEf11/ϕFL1C(Δ36) PnisA , to disrupt biofilms of two Enterococcus faecalis strains: JH2-2 (vancomycin-sensitive) and V583 (vancomycin-resistant). Methods 24 h static biofilms of E. faecalis strains JH2-2(pMSP3535 nis R/K) and V583 (pMSP3535 nis R/K), formed on cover slips, were inoculated with bacteriophage ϕEf11/ϕFL1C(Δ36) PnisA . After 24 and 48 h incubation, the bacterial biomass was imaged by confocal microscopy and viable cells were quantified by colony forming unit measurement. Results The results showed a 10-100-fold decrease in viable cells (CFU/biofilm) after phage treatment, which was consistent with comparisons of treated and untreated biofilm images visualized as max projections of the Z-series. Conclusion The biomass of both vancomycin-sensitive and vancomycin-resistant E. faecalis biofilms is markedly reduced following infection by bacteriophage ϕEf11/ϕFL1C(Δ36) PnisA .

The aim of this study was to quantitatively evaluate the amount of apically extruded debris by single-file reciprocating instruments with different working lengths and apical preparation sizes. Eighty human single-rooted mandibular incisors were used and conventional access cavities were prepared. Then, the specimens were divided into four groups (n=20), according to root canal instrumentation: Reciproc size 25, .08 taper and Reciproc size 40, .06 taper instruments were used at the foramen; Reciproc size 25, .08 taper and Reciproc size 40, .06 taper instruments were used 1 mm short of the foramen. Distilled water was used as an irrigant and the apically extruded debris were collected in pre-weighted glass vials and dried afterwards. The mean weight of debris was weighed with a microbalance and statistically analyzed using one-way analysis of variance and the post hoc Tukey multiple comparison test (p0.05). The present study demonstrated that the working length and the apical preparation size did not have a significant effect on debris extrusion when performing single-file reciprocating instrumentation. Resumo Este estudo avaliou quantitativamente a quantidade de material extruído apicalmente pela instrumentação com lima única reciprocante, aplicando diferentes comprimentos de trabalho e tamanhos de preparo apical. Para a análise de extrusão foram utilizados oitenta incisivos inferiores unirradiculares humanos. Cavidades de acesso convencionais foram preparadas e os espécimes foram divididos em quatro grupos (n=20), de acordo com o tipo de instrumentação do canal: Reciproc 25, 0.08 e Reciproc 40, 0.06 foram utilizadas na instrumentação até o forame; Reciproc tamanho 25, 0.08 e Reciproc 40, 0,06 foram utilizadas na instrumentação até 1 mm aquém do forame. Água destilada foi empregada como irrigante e o material extruído apicalmente foi coletado em frascos de vidro já pesados e posteriormente submetidos a secagem. O peso médio de detritos foi avaliado com uma microbalança de precisão e os dados submetidos a ANOVA e teste Tukey (p0,05). Este estudo demonstrou que o comprimento de trabalho e o tamanho do preparo apical não geraram efeito significativo sobre a extrusão de debris durante a instrumentação reciprocante.

Aim To evaluate bacterial growth after contact with sodium hypochlorite (NaOCl; 1 and 5%) and chlorhexidine (CHX; 0.12, 0.5 and 1%) in vitro with or without the addition of organic material (bovine serum albumin (BSA) 0.5%). Methodology Bacterial samples (American Type Culture Collection® (ATCC)) of Staphylococcus aureus, Enterococcus faecalis, Escherichia coli, Porphyromonas gingivalis and Fusobacterium nucleatum were kept in contact with each irrigating solution for varying intervals of time: immediately (t0), 5 min (t5), 15 min (t15) and 30 min (t30). Each test was repeated 10 times. In half of the specimens, 0.5% BSA was added as organic material in an attempt to simulate the organic tissue present in the root canal system. Bacterial growth under appropriate condition of incubation was evaluated and compared for each microorganism at all time intervals. Results A 0.12% CHX solution did not eliminate E. faecalis at any time interval. One percent CHX eliminated all strains, as did both NaOCl concentrations. BSA did not interfere substantially with the antimicrobial activity of any of the irrigating solutions. Conclusions Under the condition of this study, a 0.12% CHX solution was ineffective at killing E. faecalis.

These results permit us to conclude that to obtain better antimicrobial activity, chlorhexidine in a concentration greater than 0.12% should be used. O objetivo do presente trabalho foi avaliar in vitro a atividade antimicrobiana do hipoclorito de sódio (1% e 5%) e da clorexidina (0,12%, 0,5% e 1%). Cepas (ATCC) de Staphylococcus aureus, Enterococcus faecalis, Escherichia coli, Porphyromonas gingivalis e Fusobacterium nucleatum foram submetidas ao teste por contato. As amostras foram analisadas em diferentes intervalos de tempo: imediatamente, 5 minutos, 15 minutos e 30 minutos após o contato com cada solução e repetidas 10 vezes. Os resultados demonstraram que a solução de clorexidina a 0,12% não foi capaz de eliminar as amostras de E. faecalis em nenhum intervalo de tempo testado enquanto que a clorexidina a 0,5% e 1% eliminaram todas as cepas testadas assim como as 2 concentrações de hipoclorito de sódio. Estes resultados nos permitem concluir que para se obter melhores respostas quanto a atividade antimicrobiana, a solução de clorexidina deve ser utilizada numa concentração superior a 0,12%.

Objetivo: avaliar a eficácia do Sistema Reciproc, com ou sem solvente, para remoção do enchimento endodôntico de canais radiculares ovais. O tempo necessário para a remoção da guta-percha e a fratura do instrumento também foram avaliados. Material e Métodos: quarenta pré-molares unirradiculares retos foram confeccionados até o tamanho 30 e preenchidos com guta-percha e cimento, sendo aleatoriamente divididos em dois grupos de retratamento (n = 20), tratados com Sistema Reciproc com ou sem clorofórmio (RS + C e RS). Para todas as raízes, foram registrados erros de procedimento, tempo de retratamento e material apicalmente extrusado. Após o retratamento, as raízes foram divididas longitudinalmente e depois fotografadas. Todas as imagens foram avaliadas com o software Image Tool para calcular a porcentagem de material residual. Os testes de Kolmogorov Smirnov e t foram usados ??para determinar diferenças significativas em P 0,05).
