An experimental platform for laser-driven ion (sub-MeV) acceleration and potential applications was commissioned at the HiLASE laser facility. The auxiliary beam of the Bivoj laser system operating at a GW level peak power (~10 J in 5–10 ns) and 1–10 Hz repetition rate enabled a stable production of high-current ion beams of multiple species (Al, Ti, Fe, Si, Cu, and Sn). The produced laser–plasma ion sources were fully characterized against the laser intensity on the target (1013–1015 W/cm2) by varying the laser energy, focal spot size, and pulse duration. The versatility and tuneability of such high-repetition-rate laser–plasma ion sources are of potential interest for user applications. Such a statistically accurate study was facilitated by the large amount of data acquired at the high repetition rate (1–10 Hz) provided by the Bivoj laser system.

The placement of a feeding jejunostomy can be indicated in malnourished patients with gastric and oesophagogastric junction cancer to allow for enteral nutritional support. In these patients, the jejunostomy tube can be suitably placed at the time of staging laparoscopy. Several techniques of laparoscopic jejunostomy (LJ) have been described, yet the Witzel approach remains neglected, due to the perceived difficulty of suturing the bowel around the tube and securing them to the abdominal wall. Here, we describe a novel technique for LJ, using a single barbed suture for securing the bowel and tunnelling the jejunostomy catheter according to the Witzel approach.

Abstract Background Acute calculus cholecystitis (ACC) has a high incidence in the general population. The presence of several areas of uncertainty, along with the availability of new evidence, prompted the current update of the 2016 WSES (World Society of Emergency Surgery) Guidelines on ACC. Materials and methods The WSES president appointed four members as a scientific secretariat, four members as an organization committee and four members as a scientific committee, choosing them from the expert affiliates of WSES. Relevant key questions were constructed, and the task force produced drafts of each section based on the best scientific evidence from PubMed and EMBASE Library; recommendations were developed in order to answer these key questions. The quality of evidence and strength of recommendations were reviewed using the Grading of Recommendations Assessment, Development and Evaluation (GRADE) criteria (see https://www.gradeworkinggroup.org/ ). All the statements were presented, discussed and voted upon during the Consensus Conference at the 6th World Congress of the World Society of Emergency Surgery held in Nijmegen (NL) in May 2019. A revised version of the statements was voted upon via an online questionnaire until consensus was reached. Results The pivotal role of surgery is confirmed, including in high-risk patients. When compared with the WSES 2016 guidelines, the role of gallbladder drainage is reduced, despite the considerable technical improvements available. Early laparoscopic cholecystectomy (ELC) should be the standard of care whenever possible, even in subgroups of patients who are considered fragile, such as the elderly; those with cardiac disease, renal disease and cirrhosis; or those who are generally at high risk for surgery. Subtotal cholecystectomy is safe and represents a valuable option in cases of difficult gallbladder removal. Conclusions, knowledge gaps and research recommendations ELC has a central role in the management of patients with ACC. The value of surgical treatment for high-risk patients should lead to a distinction between high-risk patients and patients who are not suitable for surgery. Further evidence on the role of clinical judgement and the use of clinical scores as adjunctive tools to guide treatment of high-risk patients and patients who are not suitable for surgery is required. The development of local policies for safe laparoscopic cholecystectomy is recommended.

Abstract Background Treatment of esophageal perforations and postoperative anastomotic leaks of the upper gastrointestinal tract remains a challenge. Endoluminal vacuum-assisted closure (E-Vac) therapy has positively contributed, in recent years, to the management of upper gastrointestinal tract perforations by using the same principle of vacuum-assisted closure therapy of external wounds. The aim is to provide continuous wound drainage and to promote tissue granulation, decreasing the needed time to heal with a high rate of leakage closure. Cases presentation A series of two different cases with clinical and radiological diagnosis of esophageal fistulas, recorded from 2018 to 2019 period at our institution, is presented. The first one is a case of anastomotic leak after esophagectomy for cancer complicated by pleuro-mediastinal abscess, while the second one is a leak of an esophageal suture, few days after resection of a bronchogenic cyst perforated into the esophageal lumen. Both cases were successfully treated with E-Vac therapy. Conclusion Our experience shows the usefulness of E-Vac therapy in the management of anastomotic and non-anastomotic esophageal fistulas. Further research is needed to better define its indications, to compare it to traditional treatments and to evaluate its long-term efficacy.

Successful inter-organisational new product development (NPD) is a key source of competitive advantage, yet many NPD projects are still failing. To generate new insights on the phenomenon, we examine the performance implications of knowledge inputs among collaborating organisations, both in terms of external input ratio and input concentration. We test a set of hypotheses using multi-industry primary and secondary data on 210 inter-organisational NPD projects. Results support our hypotheses about the negative effect of input concentration on design quality, which in turn is positively associated with product market performance. Although we surprisingly find a negative linear effect of external input ratio on design quality, our post-hoc analysis confirms the relationship to be an inverse U-shaped relationship. We also find support for the roles of technology interdependence in moderating the performance implications of external input ratio and input concentration. By clarifying how knowledge inputs affect project outcomes under different levels of technology interdependence, the study provides managerial implications regarding how to select innovation partners in order to create a knowledge portfolio that best improves NPD project performance. We present the design and performance of the LIGO Input Optics subsystem as implemented for the sixth science run of the LIGO interferometers. The Initial LIGO Input Optics experienced thermal side effects when operating with 7 W input power. We designed, built, and implemented improved versions of the Input Optics for Enhanced LIGO, an incremental upgrade to the Initial LIGO interferometers, designed to run with 30 W input power. At four times the power of Initial LIGO, the Enhanced LIGO Input Optics demonstrated improved performance including better optical isolation, less thermal drift, minimal thermal lensing and higher optical efficiency. The success of the Input Optics design fosters confidence for its ability to perform well in Advanced LIGO.

The gravitational-wave (GW) sky may include nearby pointlike sources as well as astrophysical and cosmological stochastic backgrounds. Since the relative strength and angular distribution of the many possible sources of GWs are not well constrained, searches for GW signals must be performed in a model-independent way. To that end we perform two directional searches for persistent GWs using data from the LIGO S5 science run: one optimized for pointlike sources and one for arbitrary extended sources. The latter result is the first of its kind. Finding no evidence to support the detection of GWs, we present 90% confidence level (CL) upper-limit maps of GW strain power with typical values between 2-20x10^-50 strain^2 Hz^-1 and 5-35x10^-49 strain^2 Hz^-1 sr^-1 for pointlike and extended sources respectively. The limits on pointlike sources constitute a factor of 30 improvement over the previous best limits. We also set 90% CL limits on the narrow-band root-mean-square GW strain from interesting targets including Sco X-1, SN1987A and the Galactic Center as low as ~7x10^-25 in the most sensitive frequency range near 160 Hz. We present the first modeled search for gravitational waves using the complete binary black hole gravitational waveform from inspiral through the merger and ringdown for binaries with negligible component spin. We searched approximately 2 years of LIGO data taken between November 2005 and September 2007 for systems with component masses of 1-99 solar masses and total masses of 25-100 solar masses. We did not detect any plausible gravitational-wave signals but we do place upper limits on the merger rate of binary black holes as a function of the component masses in this range. We constrain the rate of mergers for binary black hole systems with component masses between 19 and 28 solar masses and negligible spin to be no more than 2.0 per Mpc^3 per Myr at 90% confidence., Comment: 19 pages, 3 figures

The physical mechanisms responsible for pulsar timing glitches are thought to excite quasi-normal mode oscillations in their parent neutron star that couple to gravitational wave emission. In August 2006, a timing glitch was observed in the radio emission of PSR B0833-45, the Vela pulsar. At the time of the glitch, the two co-located Hanford gravitational wave detectors of the Laser Interferometer Gravitational-wave observatory (LIGO) were operational and taking data as part of the fifth LIGO science run (S5). We present the first direct search for the gravitational wave emission associated with oscillations of the fundamental quadrupole mode excited by a pulsar timing glitch. No gravitational wave detection candidate was found. We place Bayesian 90% confidence upper limits of 6.3e-21 to 1.4e-20 on the peak intrinsic strain amplitude of gravitational wave ring-down signals, depending on which spherical harmonic mode is excited. The corresponding range of energy upper limits is 5.0e44 to 1.3e45 erg., Comment: 13 pages, 5 figures

The Laser Interferometer Gravitational Wave Observatory (LIGO) is a network of three detectors built to detect local perturbations in the space-time metric from astrophysical sources. These detectors, two in Hanford, WA and one in Livingston, LA, are power-recycled Fabry-Perot Michelson interferometers. In their fifth science run (S5), between November 2005 and October 2007, these detectors accumulated one year of triple coincident data while operating at their designed sensitivity. We present a search for periodic gravitational waves from the neutron star in the supernova remnant Cassiopeia A. The search coherently analyzes data in a 12-day interval taken from the fifth science run of the Laser Interferometer Gravitational-Wave Observatory. It searches gravitational wave frequencies from 100 to 300 Hz, and covers a wide range of first and second frequency derivatives appropriate for the age of the remnant and for different spin-down mechanisms. No gravitational wave signal was detected. Within the range of search frequencies, we set 95% confidence upper limits of 0.7--1.2e-24 on the intrinsic gravitational wave strain, 0.4--4e-4 on the equatorial ellipticity of the neutron star, and 0.005--0.14 on the amplitude of r-mode oscillations of the neutron star. These direct upper limits beat indirect limits derived from energy conservation and enter the range of theoretical predictions involving crystalline exotic matter or runaway r-modes. We present an up-to-date, comprehensive summary of the rates for all types of compact binary coalescence sources detectable by the Initial and Advanced versions of the ground-based gravitational-wave detectors LIGO and Virgo. Astrophysical estimates for compact-binary coalescence rates depend on a number of assumptions and unknown model parameters, and are still uncertain. The most confident among these estimates are the rate predictions for coalescing binary neutron stars which are based on extrapolations from observed binary pulsars in our Galaxy. These yield a likely coalescence rate of 100 per Myr per Milky Way Equivalent Galaxy (MWEG), although the rate could plausibly range from 1 per Myr per MWEG to 1000 per Myr per MWEG. We convert coalescence rates into detection rates based on data from the LIGO S5 and Virgo VSR2 science runs and projected sensitivities for our Advanced detectors. Using the detector sensitivities derived from these data, we find a likely detection rate of 0.02 per year for Initial LIGO-Virgo interferometers, with a plausible range between 0.0002 and 0.2 per year. The likely binary neutron-star detection rate for the Advanced LIGO-Virgo network increases to 40 events per year, with a range between 0.4 and 400 per year.

Astrophysics - High Energy Astrophysical Phenomena - Abstract
Progenitor scenarios for short gamma-ray bursts (short GRBs) include coalescenses of two neutron stars or a neutron star and black hole, which would necessarily be accompanied by the emission of strong gravitational waves. We present a search for these known gravitational-wave signatures in temporal and directional coincidence with 22 GRBs that had sufficient gravitational-wave data available in multiple instruments during LIGO's fifth science run, S5, and Virgo's first science run, VSR1. We find no statistically significant gravitational-wave candidates within a [-5, +1) s window around the trigger time of any GRB. Using the Wilcoxon-Mann-Whitney U test, we find no evidence for an excess of weak gravitational-wave signals in our sample of GRBs. We exclude neutron star-black hole progenitors to a median 90% CL exclusion distance of 6.7 Mpc., Comment: 8 pages, 2 figures, Accepted by ApJ

We present a search for gravitational waves from 116 known millisecond and young pulsars using data from the fifth science run of the LIGO detectors. For this search ephemerides overlapping the run period were obtained for all pulsars using radio and X-ray observations. We demonstrate an updated search method that allows for small uncertainties in the pulsar phase parameters to be included in the search. We report no signal detection from any of the targets and therefore interpret our results as upper limits on the gravitational wave signal strength. The most interesting limits are those for young pulsars. We present updated limits on gravitational radiation from the Crab pulsar, where the measured limit is now a factor of seven below the spin-down limit. This limits the power radiated via gravitational waves to be less than ~2% of the available spin-down power. For the X-ray pulsar J0537-6910 we reach the spin-down limit under the assumption that any gravitational wave signal from it stays phase locked to the X-ray pulses over timing glitches, and for pulsars J1913+1011 and J1952+3252 we are only a factor of a few above the spin-down limit. Of the recycled millisecond pulsars several of the measured upper limits are only about an order of magnitude above their spin-down limits. For these our best (lowest) upper limit on gravitational wave amplitude is 2.3x10^-26 for J1603-7202 and our best (lowest) limit on the inferred pulsar ellipticity is 7.0x10^-8 for J2124-3358., Comment: 39 pages, 5 figures, Accepted in The Astrophysical Journal

We present the results of a search for gravitational-wave bursts associated with 137 gamma-ray bursts (GRBs) that were detected by satellite-based gamma-ray experiments during the fifth LIGO science run and first Virgo science run. The data used in this analysis were collected from 2005 November 4 to 2007 October 1, and most of the GRB triggers were from the Swift satellite. The search uses a coherent network analysis method that takes into account the different locations and orientations of the interferometers at the three LIGO-Virgo sites. We find no evidence for gravitational-wave burst signals associated with this sample of GRBs. Using simulated short-duration (<1 s) waveforms, we set upper limits on the amplitude of gravitational waves associated with each GRB. We also place lower bounds on the distance to each GRB under the assumption of a fixed energy emission in gravitational waves, with typical limits of D ~ 15 Mpc (E_GW^iso / 0.01 M_o c^2)^1/2 for emission at frequencies around 150 Hz, where the LIGO-Virgo detector network has best sensitivity.
