39. Adoption of video laryngoscopy by a major academic anesthesia department

Abstract

Aim:To describe the adoption patterns of intubating devices used at a major teaching and research facility. Materials & methods:Retrospective analysis of 2012–2019 data on frequency and trends in airway management devices collected from our anesthesia information management system. Results:Use of direct laryngoscopy was more frequent, but there was a downward trend in use over time (p < 0.008) in favor of video laryngoscopy (VL), which increased significantly (p < 0.008). The largest growth among devices was the McGrath VL, which increased from 0.2% in 2012 to 36.2% of cases in 2019. Conclusion:Our study shows a clear increase in VL usage which has implications in quality of care and medical education.

40. Core temperature monitoring during cesarean delivery under spinal anesthesia: a quality improvement descriptive study

Abstract

Aim:To analyze intraoperative temperature change over time following spinal anesthesia for cesarean delivery using temperature enabled Foley catheters. Materials & methods:512 records of women who underwent scheduled cesarean deliveries were retrospectively identified from January 1, 2018 through September 9, 2018 using our anesthesia information management system. Results:Median minimum temperature at min 1 following foley insertion was 35.24°C (interquartile range: 1.43), with an average of 12 minutes until temperature equilibration at median maximum temperature of 36.54°C (interquartile range 0.39). Temperature dropped to a nadir of 35.9°C at the 45 min mark, reflecting an average 0.64°C decline in temperature. Conclusion:Bladder temperature is a useful surrogate for core temperature and offers a practical solution to continuous temperature monitoring in awake patients.

Abstract

Background/objective: Opioid overdose deaths have been on the rise in the last two decades in the United States, yet the economic burden remains unclear. Although prior studies have assessed the economic impact of total overdose deaths at a regional level, there remains a paucity of information surrounding the financial burden due to specific types of opioids on a national level. This analysis aims to examine the economic burden of the opioid overdose crisis by quantifying years of potential lives lost (YPLL) due to premature mortality and the value of those statistical lives (VSL) to estimate monetary losses., Methods: Death counts for overdose deaths due to fentanyl, hydrocodone, hydromorphone, oxycodone, and oxymorphone from 2018 to 2020 were obtained from the National Vitals Statistics System. Period life expectancy was extracted from the Social Security Administration. Resultant YPLL and VSL were then calculated., Results: Fentanyl accounted for an overwhelming proportion of opioid-induced deaths (77 percent of deaths in 2018 and 86 percent in 2020 resulting in 2.2 million YPLL and USD 527 billion in financial losses). The largest percentage of overdose deaths from fentanyl and oxymorphone occurred in the 25-34 age range; nearly two-thirds of all fatalities were men. There was also a significant increase in the number of opioid overdose deaths in the year 2020, compared to prior years., Conclusions: Our analysis suggests that fentanyl contributed a significant proportion of economic losses in this public health crisis. Understanding the distribution of opioid deaths by drug type and subsequent economic losses could guide targeted intervention strategies.
