The implantable cardioverter-defibrillator (ICD) has become an integral part of treatment for a variety of patients with symptomatic, or at risk for, ventricular tachyarrhythmias. The ICD's effectiveness is attributed to its ability to promptly detect and terminate ventricular tachycardia (VT) and fibrillation (VF). The clinical trials that established the positive role of ICD therapy were based on patients who underwent some form of defibrillation testing at the time of implantation. Therefore, since its advent, intraoperative defibrillation testing of the ICD to assure reliable detection and termination of VT/VF has been a standard practice. But because of advances in defibrillator and lead technology, which now facilitates successful device implantation (i.e., low defibrillation energy requirement to allow for an adequate programmed safety margin) in the majority of patients, the necessity of defibrillation testing has been called into attention. Despite substantial progress, it is not altogether clear whether a wholesale abandonment of intraoperative ICD testing is appropriate at this point. We review pertinent data regarding pros and cons of ICD testing and offer a suggestion as to when, how, and who should test ICDs.

3. Coupled hydro‐mechanical pore‐scale modeling of biopore‐coated clods for upscaling soil shrinkage and hydraulic properties

Abstract Earthworms and plant roots are vital for macropore formation and stabilization. The organo‐mineral coating of biopore surfaces also regulates macropore‐matrix mass exchange during preferential flow. The influence of finer‐textured burrow coatings on macroscopic soil properties during shrinkage could potentially be assessed by upscaling pore‐scale hydraulic and mechanical simulations. The aim was to investigate the influence of micro parameters (particle size, stiffness, and bond strength) on macro parameters (i.e., shrinkage curve and soil hydraulic properties). Drainage experiments and simulations were carried out using biopore‐coated clod‐size samples compared to those without coating. Simulations were performed using a two‐phase pore‐scale finite volume coupled with discrete element model (DEM‐2PFV). The structural dynamics was characterized by analyzing the pore volume and soil shrinkage curve obtained from numerically determined data. The soil hydraulic parameters were described using uni‐ and bimodal van Genuchten (vG) functions. The drainage simulations revealed hydro‐mechanical dynamics characterized by Braudeau‐shrinkage curve subdomains: The matrix‐only samples, with lower particle bond strength, exhibited relatively higher shrinkage. The coated samples, with higher particle stiffness and bond strength, displayed greater hydro‐mechanical stability. The numerically determined initial value of the saturated hydraulic conductivity (Ks) was about 70 times larger for matrix‐only samples than for coated samples. As expected, for the nonrigid soil structures, constant Ks, α, and n values for bimodal vG model resulted in prediction errors. Upscaling DEM‐2PFV pore‐scale model outcomes quantifies micro‐coating effects on macro hydro‐mechanics. This yields void ratio‐based soil water retention and hydraulic conductivity functions, advancing macroscopic soil hydraulic models and enhancing structured soil flow and transport descriptions.

4. Neutron imaging of exchange flow between biopore and matrix for Bt versus C horizons

Abstract The surface coating of biopores plays a pivotal role in mediating mass exchange between the biopore and the soil matrix, ultimately governing preferential flow in structured soils. However, quantitative experiments of the flow exchange dynamics in the soil around an earthworm burrow (drilosphere) are limited. Here, we developed an experimental setup to mimic preferential flow within a biopore while monitoring the water distribution by neutron radiography. The objective was to quantify the evolving water content distribution in the biopore with surrounding drilosphere samples from Bt and C horizons. We hypothesize that the distribution of clay and organic material across biopore surfaces in Bt and C horizons impacts the biopore coating material formation (i.e., composition and thickness), thereby influencing its role in controlling mass exchange during preferential flow events. The experimental setup was proposed for in situ quantification of the preferential flow dynamics in an earthworm burrow (biopore) passing through Bt and C horizons of a Luvisol developed on glacial till. The neutron radiographies were obtained at the NEUtron Transmission Radiography facility of the Paul Scherrer Institut, Villingen. The volumetric water content was quantified by calibrating gray scale values from radiography images with defined water contents using stepped aluminum cuvettes attached to the samples; image analysis was used to determine sorptivity at the biopore–soil matrix interface. The water exchange flow was found to be smaller for coated biopore surface in the more clayey Bt horizon, as compared to the coarser textured carbonaceous subsoil C horizon, where higher sorptivity intensified mass exchange. The method most realistically captured the dynamics of the vertical preferential flow process in a biopore, including lateral exchange flow, by determining the water sorptivity of the drilosphere. In contrast to mini‐infiltrometer data, biopore–matrix exchange flow was here more restricted for Bt in contrast to carbonaceous C horizon. Since both horizons showed the presence of coating material on their burrow walls, the results suggest further analysis of their contrasting properties and structures.

The herpetofauna of São Tomé and Príncipe consists of nine species of amphibians, all endemic, and 21 species of terrestrial reptiles, of which 17 are endemic. Our current knowledge regarding its natural history, ecology, and distribution is limited. Here two important tools are provided to support researchers, conservationists, and local authorities in the identification of the country’s herpetofauna: an illustrated key to the herpetofauna of the two islands and surroundings islets and a DNA barcode reference library. The keys allow a rapid and unambiguous morphological identification of all occurring species. The DNA barcodes for the entire herpetofauna of the country were produced from 79 specimens, all of which are deposited in museum collections. The barcodes generated are available in online repositories and can be used to provide unambiguous molecular identification of most of the species. Future applications and use of these tools are briefly discussed.

8. Design and Implementation of a Low-Power Device for Non-Invasive Blood Glucose

Glucose is a simple sugar molecule. The chemical formula of this sugar molecule is C6H12O6. This means that the glucose molecule contains six carbon atoms (C), twelve hydrogen atoms (H), and six oxygen atoms (O). In human blood, the molecule glucose circulates as blood sugar. Normally, after eating or drinking, our bodies break down the sugars in food and use them to obtain energy for our cells. To execute this process, our pancreas produces insulin. Insulin “pulls” sugar from the blood and puts it into the cells for use. If someone has diabetes, their pancreas cannot produce enough insulin. As a result, the level of glucose in their blood rises. This can lead to many potential complications, including blindness, disease, nerve damage, amputation, stroke, heart attack, damage to blood vessels, etc. In this study, a non-invasive and therefore easily usable method for monitoring blood glucose was developed. With the experiment carried out, it was possible to measure glucose levels continuously, thus eliminating the disadvantages of invasive systems. Near-IR sensors (optical sensors) were used to estimate the concentration of glucose in blood; these sensors have a wavelength of 940 nm. The sensor was placed on a small black parallelepiped-shaped box on the tip of the finger and the output of the optical sensor was then connected to a microcontroller at the analogue input. Another sensor used, but only to provide more medical information, was the heartbeat sensor, inserted into an armband (along with the microprocessor). After processing and linear regression analysis, the glucose level was predicted, and data were sent via the Bluetooth network to a developed APP. The results of the implemented device were compared with available invasive methods (commercial products). The hardware consisted of a microcontroller, a near-IR optical sensor, a heartbeat sensor, and a Bluetooth module. Another objective of this experiment using low-cost and low-power hardware was to not carry out complex processing of data from the sensors. Our practical laboratory experiment resulted in an error of 2.86 per cent when compared to a commercial product, with a hardware cost of EUR 8 and a consumption of 50 mA.

Objectives: To evaluate how hormonal therapy can impact breast cancer patients sexual quality of life and compare two widely used therapeutic agents: anastrozole and tamoxifen. Studies so far have evaluated the side effects of such therapy on patients general quality of life, but literature remains scarce regarding the impact it has on sexual aspects. We believe there is a demand for a detailed view of these aspects since most patients undergo these treatments for at least five years. Material and Methods: Transverse observational study evaluated in 2019, 41 women with a history of breast cancer, all of them undergoing hormonal therapy. Group presented a mean age of 55.4 years (35 to 77 years); those in menopause with a mean time of menopause of 10.92 years (2 to 28 years). Thirty-eight women lived maritally and/or were sexually active. The mean duration of treatment was 36.84 months. We analyzed data in pre-and postmenopausal women, evaluating the results of questionnaires with general parameters (age, treatment time, general quality of life, adaptation to therapy) as well as specific instruments for evaluation of sexual dysfunction (FSDS-R) and quality of life with specific aspects for breast cancer (FACT-B). The results were placed in 2×2 contingency tables comparing the group receiving tamoxifen versus anastrozole. Results and Conclusion: Tamoxifen compared to anastrozole is a drug with apparent less impact on most common sexual dysfunctions (orgasm, dyspareunia, and feeling good quality of sex life), following those already published in international literature. We found no impact on physical, socio-familiar, and emotional well-being. Finally, we conclude that the results of this study significantly contribute to the choice of adequate therapeutic agent and highlight the need to bring this topic during routine consults and to the decision with the patient for the best suited treatment option.

Objective: Analyze data regarding patients in need of hospital care admitted in our oncology infirmary aiming for better clinical practices, clinical support and scope for financial and structural needs for institutions. Methods: A prospective study of 191 patients admitted for in-hospital care during a year, who were stratified by diagnosis, age, sex, admission reason, duration of stay, re-admission rate and outcomes. Results: 97 male and 94 female patients were admitted, with a 61-year-old mean age; the mean stay was 13,4 days; and a re-admission rate of 9,4%. Main admission causes: terminality [defined by PSECOG=4, with a 30 day or less estimated life expectancy]: 30 patients (15,7%); total pain: 23 (12,0%); oncological emergencies [febrile neutropenia, malignant hypercalcemia, medullar compression, superior vena cava syndrome]: 20 (10,4%); clinical deterioration [defined as worsening of patients performance or malnutrition]: 15 (7,8%); abdominal pain [without acute abdomen]: 14 (7,3%). Most common primary tumor sites were: colorectal=30 (15,7%), breast=28 (14,6%), stomach=20 (10,4%), lung=14 (7,3%), ovary=12 (6,2%) and prostate=10 (5,2%). Results: 99 deaths (mortality rate 52,1%). Patients in terminality presented a 90% ratio of mortality within an average of 6,6 days of hospital care. Febrile neutropenia (42,8%) and abdominal pain (35,7%) both achieved above expected mortality rates. 91 patients were discharged from the hospital and went back to ambulatory care. Conclusion: Our analysis pointed at an admission rate of 1,2%, pointing at good clinical ambulatory practices. The mean in-hospital stay comparison between patients who were discharged and those who died, not considered terminal ill, was unexpectedly similar, 11,7 versus 13,4 days, respectively, suggesting that in-hospital care should be focused on symptoms - not outcomes. Patients with febrile neutropenia and gastrointestinal cancer should be evaluated with caution due to higher than expected mortality rates

O processo de volatilidade pode reduzir a eficiência de controle de plantas daninhas por herbicidas e causar contaminação ambiental e fito intoxicação de culturas sensíveis, sendo que as condições ambientais no momento das aplicações podem influenciar neste processo. O objetivo desta pesquisa foi avaliar o efeito de duas condições controladas de umidade relativa do ar na volatilidade de herbicidas. O experimento foi realizado em delineamento inteiramente casualizado, com cinco repetições. Foi avaliada a volatilidade dos herbicidas 2,4-D, Trifluralina, Haloxyfope-P metil ester, Glyphosate e Clomazona em sete tempos (1h, 2h, 3h, 4h, 5h, 6h e 7h após a deposição), em duas condições de umidade relativa do ar 47 e 70% (±2%) e temperatura 25 °C (±2°C). A volatilidade foi determinada conforme a Associação Brasileira de Normas Técnicas-ABNT (NBR 13238:2014) e expressa em percentual de material volatilizado. Os resultados indicaram que a volatilidade aumenta ao longo do tempo e se estabilizou para todos os herbicidas após três horas da deposição. Na metodologia de determinação da volatilidade de herbicidas será necessário a indicação da umidade relativa do ar, pois com a elevação de 47% para 70% houve redução da volatilidade da Trifluralina. A volatilidade foi decrescente para os herbicidas Haloxyfope > Glyphosate > 2,4-D > Trifluralina = Clomazona.

The keys allow a rapid and unambiguous morphological identification of all occurring species. The DNA barcodes for the entire herpetofauna of the country were produced from 79 specimens, all of which are deposited in museum collections. The barcodes generated are available in online repositories and can be used to provide unambiguous molecular identification of most of the species. Future applications and use of these tools are briefly discussed.

Fruit colour is considered an important feature mediating interactions between plants and frugivorous birds. Despite that, colour mediated interactions are context-dependent, and habitat disturbances may affect how frugivorous birds perceive fruit colours. This study assessed the influence of fruit colour and edge effect on the consumption of artificial fruits by frugivorous birds in three disturbed semideciduous forests in southeastern Brazil. In each of those areas, we performed a field experiment in which we placed artificial fruits of three different colours on plants and recorded their consumption by birds. Red-coloured fruits were ingested more often than yellow, but neither of them was consumed differently from dark-blue. Edge effect only affected consumption of yellow fruits. Our data neither support the hypothesis of preferential consumption of the highest contrasting colours nor of increased fruit consumption in the forest interior. These findings indicate that colour and edge effects, as well as the interaction between them, may not be strong predictors of fruit choice by birds in disturbed environments, especially because generalist species, which are less sensitive to the physical alterations in forests, are favoured in these areas.

- Abstract
A 3ª edição do Congresso Internacional de Estudantes e Profissionais da Saúde – DELTA SAÚDE, realizou-se de forma presencial no período de 04 a 06 de novembro de 2022, no Centro de Convenções do Hotel SESC Praia, Luis Correia, Piauí. O Delta Saúde 2022 trouxe como tema geral: “O pós-pandemia e os principais desafios da saúde global”; e as discussões foram norteadas pelos eixos centrais, a saber: a) Pandemia de COVID-19, b) Vigilância em Saúde, c) Saúde Pública e a Atenção Primária, d) Saúde Mental, e) Doenças Crônicas e Doenças Transmissíveis, f) Educação, Formação e Trabalho na Saúde, g) Inovação em Saúde, h) Eixos Transversais. O evento foi criado em 2017 e nesta edição alcançamos pelo menos 10 estados brasileiros em participação direta, palestrantes, convidados, além dos conferencistas internacionais on-line. Esta diversidade aliada a qualidade dos trabalhos apresentados, contribuiu de forma decisiva para o alcance dos objetivos do congresso que primam pela qualificação do ensino na saúde de acadêmicos e pósgraduandos, pelo fortalecimento das práticas profissionais nos serviços de saúde, e pelo incentivo à produção científica. A programação do evento foi composta por palestras nacionais e internacionais, mesas multidisciplinares, cursos livres, seminários, rodas de conversas, encontros e sessões de trabalhos com apresentações nas modalidades: Comunicações Orais e Pôsteres; e com publicação das pesquisas científicas nos Anais do congresso. O evento contou com 692 inscritos, e destes 42 foram palestrantes que abrilhantaram a programação do congresso. O fenômeno paisagístico é um processo cultural que se estabelece a partir da interação entre sujeitos e espaço percebido. A paisagem é um fenômeno vivo, contínuo, que expressa diferentes tempos, materialidades e arranjos sociais. É a metonímia de um território. Tendo isso em vista, um grupo de pesquisa interdisciplinar buscou experimentar diferentes ferramentas teórico-metodológicas ao realizar o exercício de intervir em uma determinada paisagem urbana, e assim, estimular o debate sobre as controvérsias que lhe são inerentes. A problemática escolhida foi a proposta de cercamento do Parque da Redenção, oficialmente denominado Parque Farroupilha, discutida na Câmara de Vereadores de Porto Alegre (RS) e na mídia local desde a década de 1990. Em uma área próxima ao lago Guaíba e ao Centro Histórico da cidade, os 40 hectares de área verde com diversos ambientes são um importante marco da paisagem de Porto Alegre. Caso fossem cercados, quais os aspectos da paisagem e outros que se agregam a ela que estariam sendo alterados? Como realizar uma intervenção para sensibilizar a população sobre as implicações do cercamento, ou não, do parque? Desde essas questões realizou-se o exercício, em 08 de novembro de 2014, cujos registros orientaram o debate e a produção deste texto e de um vídeo. Neste texto, procuramos inicialmente realizar um diagnóstico a paisagem estudada e a tensão evocada pela proposta de alterá-la mediante a instalação de uma cerca. A seguir relatamos uma intervenção realizada no local pelo grupo, como ferramenta metodológica para estimular diferentes reações e assim registrar as sensações e opiniões da população frente à alteração provocada naquela paisagem. Posteriormente, apresentamos reflexões sobre diferentes aspectos e implicações que o fechamento do parque poderá acarretar.

The FAIR principles have become a data management instrument for the academic and scientific community, since they provide a set of guiding principles to bring findability, accessibility, interoperability and reusability to data and metadata stewardship. Since their official publication in 2016 by Scientific Data – Nature, these principles have received worldwide recognition and have been quickly endorsed and adopted as a cornerstone of data stewardship and research policy. However, when put into practice, they occasionally result in organisational, legal and technological challenges that can lead to doubts and uncertainty as to whether the effort of implementing them is worthwhile. Soon after their publication, the European Commission and other funding agencies started to require that project proposals include a Data Management Plan (DMP) based on the FAIR principles. This paper reports on the adherence of DMPs to the FAIR principles, critically evaluating ten European DMP templates. We observed that the current FAIRness of most of these DMPs is only partly satisfactory, in that they address data best practices, findability, accessibility and sometimes preservation, but pay much less attention to metadata and interoperability. Los principios FAIR se han convertido en una herramienta de gestión de datos para la comunidad académica y científica, ya que proporcionan un conjunto de principios rectores que facilitan la localización, accesibilidad, interoperabilidad y reutilización de la gestión de datos y metadatos. Desde su publicación oficial en 2016 por Scientific Data - Nature, estos principios han recibido reconocimiento mundial y fueron rápidamente respaldados y adoptados como pilares de la política de investigación y gestión de datos. Sin embargo, cuando se ponen en práctica, ocasionalmente presentan desafíos organizativos, legales y tecnológicos que pueden generar dudas e incertidumbres sobre el esfuerzo para implementarlos. Poco después de su publicación, la Comisión Europea y otras agencias de financiación comenzaron a exigir en sus propuestas de proyectos un Plan de Gestión de Datos (PGD) basado en los principios de FAIR. Este artículo informa sobre la adherencia de los PGD a los principios FAIR, evaluando críticamente diez modelos europeos de PGD. Observamos que el nivel de FAIRness de la mayoría de los PGD analizados sigue siendo parcialmente insatisfactorio, ya que abordan las mejores prácticas de datos, ubicación, accesibilidad y, a veces, preservación, pero prestan poca atención a los metadatos y la interoperabilidad. When developing a Learning Management System (LMS) using Scrum, we noticed that it was quite often necessary to redefine some system behaviour scenarios, due to ambiguities in the requirement specifications, or due to misinterpretations of stories reported by the Product Owners (POs). The definition of test suites was also cumbersome, resulting in test suites that were incomplete or did not at all comply with the system requirements. Based on this experience and to deal with these problems, in this paper, we propose the ScrumOntoBDD approach to agile software development, which combines Scrum, ontologies and Behaviour-Driven Development (BDD). This approach is centred on the concepts and techniques of Scrum and BDD and focuses on the planning and analysis phases of the software life cycle, since the BDD tools currently provide little support to these phases, while most of the problems during the LMS development were found exactly there. We claim that our approach improves the software development practices in this respect. Furthermore, ScrumOntoBDD employs ontologies in order to reduce ambiguities intrinsic to the use of a natural language as a BDD ubiquitous language. In this paper, we illustrate and systematically evaluate our approach, showing that it is beneficial since it improves the communication between members of an agile development team.

Recently, a number of ontology-driven healthcare systems have been leveraged by the Internet-of-Things (IoT) technologies that offer opportunities to improve abnormal situation detection when integrating medical wearables and cloud infrastructure. Usually, these systems rely on standardised IoT ontologies to represent sensor data observations. The ETSI Smart Applications REFerence ontology (SAREF) is an extensible industry-oriented standard. In this paper, we explain the need for interoperability of IoT healthcare applications and the role of standardised ontologies to achieve semantic interoperability. In particular, we discuss the verbosity problem of SAREF when used for real-time electrocardiography (ECG), emphasizing the requirement of representing time series. We compared the main ontologies in this context, according to quality, message size (payload), IoT-orientation and standardisation. Here we describe the first attempt to extend SAREF for specific e-Health use cases related to ECG data, the SAREF4health extension, which tackles the verbosity problem. Ontology-driven conceptual modelling was applied to develop SAREF4health, in which an ECG ontology grounded in the Unified Foundational Ontology (UFO), which plays the role of a reference model. The methodology was enhanced by following a standardisation procedure and considering the RDF implementation of the HL7 Fast Healthcare Interoperability Resources (FHIR) standard. The validation of SAREF4health includes the responses to competency questions, as well as the development and tests of an IoT Early Warning System prototype that uses ECG data and collision identification to detect accidents with truck drivers in a port area. This prototype integrates an existing ECG wearable with a cloud infrastructure, demonstrating the performance impact of SAREF4health considering IoT constraints. Our results show that SAREF4health enables the semantic interoperability of IoT solutions that need to deal with frequency-based time series. Design decisions regarding the trade-off between ontology quality and aggregation representation are also discussed.

Biopore walls are often coated with exudates and finer soil particles such that the soil in the vicinity displays altered hydro-mechanical properties resulting in distinct wettability, permeability and diffusivity when compared to the soil matrix. The mechanical properties in the coating surface (i.e. stiffness and pore structure) may change rapidly during wetting or drying, thus affecting the biopore-matrix mass exchange of water and solutes during preferential flow and local non-equilibrium dynamics.Since the combined effects of hydro-mechanical properties on the soil structure dynamics are still poorly explored, we developed a cohesive structural model of the biopore - soil matrix interface and varied particle size and stiffness (i.e. Young’s modulus) in Discrete Element Method (DEM). Pore Finite Volume (PFV) was coupled in order to simulate water and air phases during pore scale drainage in 3D. Thus, the model take into account structural responses (i.e. deformations and stresses) during the change from saturated to funicular state. Considering water tension surface constant (0.0728 Pa) and the spherical particles perfectly wettable, four coated surfaces were simulated using a combination of particle radius of 0.1 and 0.2 mm and Young’s modulus of 700 GPa and 1100 GPa. The soil matrix particle properties were constant with radius of 0.3 mm and Young’s modulus of 700 GPa. Lateral drainage was simulated by decreasing the pressure head at the external coated surface, then the air phase invaded soil matrix in direction to the coated area. The retention curve showed higher dependence on particle size rather than particle stiffness. Simulated drainage started relatively slow followed by a rapid saturation decrement. For smaller particles with coated surfaces, the change from slow to rapid drainage was observed twice, for the soil matrix and for coated biopore surfaces. The shrinkage behavior was linear during slow drainage followed by swelling effect in rapid saturation decrement. With the combination of smaller particles and higher Young’s modulus, the plastic deformation and water retention was higher. This coupled effect of heterogeneous mechanical properties on shrinkage-swelling dynamic of biopore-matrix mass exchange bring about a new approach for more complex and realistic models.
