2. [Anisakidosis diagnosed after expulsion of worms in stools: a rare form of diagnosis in humans]

Anisakidosis is an accidental and cosmopolitan parasitic zoonosis of human beings, its definitive host being marine mammals such as sea lions, seals and dolphins, among others. Humans are infested by consumption in the larval stage of the nematode of Anisakis present in raw fish and shellfish such as hake or conger eel. The infestation is preferably located in the upper digestive tract, observing the regurgitation of the parasite to the mouth or becoming evident during an upper gastrointestinal endoscopy. In rare cases, the larvae can migrate to the peritoneum or continue their passage through the small intestine and colon, with expulsion in stools being unusual. We present a case of expulsion of two larvae of the Anisakidae family in feces.

- Abstract
Se trata en esta oportunidad de una respuesta a un proyecto investigativo que reflexiona acerca de la prueba del dolo y culpa grave de los interventores contractuales en procesos de responsabilidad fiscal, a partir de la revisión analítica de decisiones emanadas de la Contraloría General de la República –Gerencia Bolívar–, en el período de gerencia 2012-2018, su objetivo general fue analizar la forma como se ha aplicado a los interventores contractuales, el sistema probatorio subjetivo, en los procesos de responsabilidad fiscal adelantados por esta institución. Para tales fines se desarrolló una investigación jurídica, de enfoque cualitativo y tipo documental, haciendo uso del método hermenéutico. En consonancia con ello, la técnica predominante fue el análisis documental y la triangulación hermenéutica de los resultados. Se presenta así una exposición de casos legales, que constituyen una muestra de procesos de esta entidad que habiendo iniciado bajo la Ley 1474 de 2011, fueron fallados antes de culminar el segundo semestre de 2018, en todos ellos se vinculó al interventor como presunto responsable fiscal.

La parasitosis ilusoria, más conocida como delusión parasitaria, es un síndrome neuropsiquiátrico donde los pacientes tienen el convencimiento de padecer una infestación parasitaria, que no puede ser demostrada tras un exhaustivo estudio médico. Estos pacientes se caracterizan por ser policonsultantes en distintas especialidades médicas y, muchos de ellos, poseen antecedentes de trastornos psiquiátricos, algunos de ellos no diagnosticados. Conocer la existencia del cuadro, diagnosticar precozmente y empatizar con el paciente, pueden dar al médico clínico algunas claves para una derivación psiquiátrica oportuna y asertiva, y mejorar la adherencia del paciente al tratamiento propuesto. Se presentan cuatro casos clínicos que consultaron por esta extraña condición.

5. Improving eighth graders’ Lexical Competence by using Translation as a Teaching Technique

- Abstract
The birth of the Communicative Approach in 1956 and the tedious repetition that learners had to undergo with the Grammar-Translation method implied the stigmatization of the use of the first language in classroom environments. Nevertheless, nowadays, there are authors who support the first language use in foreign language lessons and propose new ways to include it in order to develop the four communicative skills. In this work, the researchers studied the impact of pedagogical translation with an 8 th grade class to develop their lexical competence in English. The perception of the students and the language interaction were established by means of data collection instruments such as interviews and observations. In addition, the strategy was adapted and integrated into a pedagogical intervention taking into account previous works like those of Leonardi (2011), Ballester & Chamorro (1991), and Koletnik (2012), and topics from the Suggested Curriculum for the English subject (2016). The results presented in the chapter of data analysis determined that Pedagogical Translation works as a tool to develop lexical knowledge causing a significant improvement, and the positive general perception shown by students displays the acceptance and the benefits of its implementation in foreign language teaching environments., El nacimiento del Enfoque Comunicativo en 1956 y la tediosa repetición que los alumnos tuvieron que experimentar con el método de Gramática-Traducción implicaron la estigmatización del uso de la primera lengua en la clase de lengua extranjera. Sin embargo, hoy en día, existen autores que apoyan el uso de la lengua materna en las lecciones de idiomas extranjeros y proponen nuevas formas de incluirlo para desarrollar las cuatro habilidades comunicativas. En este trabajo, los investigadores estudiaron el impacto de la traducción pedagógica con una clase de 8º grado para desarrollar su competencia lexical en inglés. El tipo de percepción de los alumnos y la interacción lingüística se establecieron mediante instrumentos de recopilación de datos, como entrevistas y observaciones. Además, la estrategia se adaptó e integró en una intervención pedagógica teniendo en cuenta trabajos anteriores como los de Leonardi (2011), Ballester & Chamorro (1991) y Koletnik (2012), y temas del Currículo Sugerido para el tema en inglés (2016). Los resultados presentados en el capítulo de análisis de datos determinaron que la traducción pedagógica funciona como una herramienta para desarrollar conocimiento lexical. Illusory parasitosis, better known as delusional parasitosis, is a neuropsychiatric syndrome in which patients have the belief of suffering a parasitic disease, that can not be demonstrated after an exhaustive medical study. These patients are characterized by being polyconsultants in different medical specialties and, many of them, have antecedents of psychiatric disorders, some of them undiagnosed. Knowing the existence of the clinical picture, diagnosing early and empathizing with the patient, could give to clinician some clues for a timely and assertive psychiatric referral, and improve patient adherence to the proposed treatment.

Irritable bowel syndrome (IBS) is a functional disorder of the gastrointestinal tract characterised by multi-factorial aetiology. In IBS physiopathology are involved diverse factors between them biological, psychosocial, and environmental components which affect the immune activation status of gut mucosa. Among these factors is recognized the intestinal parasitosis. Post-infection IBS (PI-IBS) is recognised as a subgroup of functional disorders whose symptoms onset appear after a symptomatic intestinal infection caused by microbial agents. There are few studies regarding of relationship between IBS and intestinal parasitosis in Chile. However, is has been well described a positive association between IBS and Blastocystis hominis infections, one of prevalent parasites in Chile. In other countries, is also described a relationship between IBS and amebiasis and giardiasis. Both, characterized by a common mode of transmission through water as well as contaminated food. Because the high prevalence of parasitosis in our country it is necessary to expand the association studies to clarify the strength of the parasites ethiology in IBS.

9. Itraconazole or allopurinol in the treatment of chronic American trypanosomiasis: the results of clinical and parasitological examinations 11 years post-treatment

Eleven years after they had been given itraconazole or allopurinol for the treatment of chronic American trypanosomiasis, 109 adult patients were checked for electrocardiographic abnormalities and evidence of Trypanosoma cruzi infection. The parasitological investigations included xenodiagnosis, in which the faeces of Triatoma infestans that had fed on the patients were checked under the microscope for flagellates. In addition, a PCR-based assay and a hybridization assay were used to test blood samples from the patients, and faeces from the Tri. infestans that had fed on the patients, for Try. cruzi DNA. For the data analysis, the patients were divided into four groups known as normal/normal, abnormal/normal, normal/abnormal and abnormal/abnormal, according to whether the patients had been found to have normal or abnormal electrocardiograms (ECG) shortly before the first treatment and to have normal or abnormal ECG when checked at the 11-year follow-up. The 51 normal/normal and 24 normal/abnormal patients were assumed to have been in the 'indeterminate' phase of the disease when they were treated, whereas the 16 abnormal/normal and 18 abnormal/abnormal patients all had evidence of chagasic cardiopathy at that time. When checked 11 years post-treatment, 40 (78.4%), 17 (70.8%), 14 (87.5%) and 17 (94.4%) of these patients, respectively, were each found positive for Try. cruzi in at least one of the parasitological tests. In chronic cases, its diagnosis is confirmed by direct visualization of parasitic eggs in stool examination, by positive testing for Fasciola hepatica antigens in stools, or by direct observation of parasites by endoscopic retrograde cholangiography or surgery. In acute cases, serological reactions as immunoblothing or detection of parasite antigens in the blood are useful. The treatment of choice is triclabendazole. However, parasite resistance in animals, as well as in man, has been reported to this drug. We report four patients in whom the parasitic infection persisted despite a course of treatment with triclabendazole.

In the xenodiagnosis (XD) of American trypanosomiasis (Chagas disease), Trypanosoma cruzi in the triatomine bugs fed on the patient can now be detected using PCR (XD-PCR) as well as by microscopy (XD-M). In a study to compare XD-PCR with XD-M, triatomine bugs were fed on 50 cases of chronic American trypanosomiasis, of whom only 25 were ever found positive by XD-M. Overall, the bugs fed on 34 of the patients (all 25 cases found positive by XD-M and nine of the other patients) were found PCR-positive, giving a 330-bp fragment corresponding to part of the hyper variable region of the kinetoplast DNA of T. cruzi. Of the 25 patients who were ever found positive by XD-M, 20 gave bugs that were smear-positive on day 90 and a similar number (24; P=0.125) gave bugs that were PCR-positive at this time. On day 30, however, the bugs fed on only 11 of these 25 patients were found positive by microscopy, whereas 23 of these patients were found positive by XD-PCR (P=0.0016). Thus, not only was XD-PCR more sensitive than XD-M but it was also quicker, revealing more cases within 30 days than detected using XD-M over a period of 90 days.

13. [Anisakiasis in a patient with a small hiatal hernia. Report of one case]

We describe the presence of anisakiasis in a patient who had a small hiatal hernia. A 60 year-old women presented general malaise, burning pain, flatulence, persistent nausea and abdominal distension during five days before consulting. She referred that she ate a dish of marinated raw fish with lemon and pepper ("cebiche") and after a few hours the symptoms began. An esophagogastroscopy showed a white colour larva of approximately three cm with the cephalic end partially adhered and localized in the mucosa of the hiatal hernia. During the procedure the worm was easily extracted. The morphologic study of the specimen identified it as a stage IV larva of Pseudoterranova sp. The clinical condition of the patient improved after the extraction of the parasite.

Las diversas inquietudes planteadas por pacientes chagásicos crónicos en relación a su condición clínica, motivaron a un grupo de parasitólogos a diseñar un programa de intervención educativa. Participaron en forma voluntaria 72 pacientes, 32 hombres y 40 mujeres, procedentes de zona de alta y baja endemia chagásica de Chile. La metodología docente consistió fundamentalmente en: aplicación de test diagnóstico y de conocimientos adquiridos; sesiones teóricas que abordaron seis temáticas relacionadas; actividades prácticas tutoriales; utilización de material audiovisual e impreso; atención personalizada por médico parasitólogo (posterior a la intervención educativa y con los antecedentes serológicos, electrocardiográficos y parasitológicos previamente determinados) y evaluación de la actividad educativa por parte de los pacientes. Un mayor conocimiento previo y adquirido se observó en chagásicos crónicos procedentes de zonas endémicas. Todos demos-traron un alto grado de interés y participaron activamente de las actividades programadas. La metodología utilizada, el trabajo tutorial de pequeño grupo y la atención-orientación individual por parte del médico parasitólogo, facilitaron el proceso de enseñanza-aprendizaje. Se concluye que la intervención educativa responde a muchas de las interrogantes respecto de la afección, permite al paciente reconocer el rol que le cabe como protagonista de su propio bienestar y lo transforma en un eficaz agente en el control de la enfermedad de Chagas

Serum antibodies against Trypanosoma Cruzi have been observed in 19% of the Chilean population. Marked differences in organ involvement have been reported in patients with Chagas disease. Chagas disease is rarely an aetiological factor for achalasia in Chile, which is different from reports in other countries of South America. In contrast, a high incidence of megacolon among these patients have been reported. Aim: To study the incidence of gastric and small intestinal motor disorders among these patients and their relationship to esophageal and colon motility disorders. Patients and Methods: We studied 18 patients, 12 women (mean age 45 years); with positive antibodies against T Cruzi. Seven had radiological evidence of megacolon and no one had radiological or manometric evidence for achalasia. Non specific motor esophageal abnormalities were found in 11 patients. Nine had an abnormal electrocardiogram, suggesting a myocardial disease. A questionnaire for gastrointestinal symptoms, an electrogastrography and a small intestinal motility study, were performed in each patient. Results: All patients had evidences of abnormalities in at least one segment of the digestive tract. Twelve patients had an abnormal electrogastrographic study, with bradygastria as the most common finding. Nine had an abnormal small intestinal manometry with a myophatic pattern evidenced by a decreased amplitude of contractions (18.5±3 mmHg). Also an increased number of clustered contractions was observed. Conclusions: Gastric dysrhythmias and small intestinal motor abnormalities are frequently associated to non specific esophageal motor disorders and megacolon in patients with Chagas disease (Rev Méd Chile 2004; 132: 939-4)

En el año 1999, se certificó en Chile la interrupción de la transmisión vectorial de la enfermedad de Chagas. Por otra parte, el desarrollo de políticas habitacionales, que incluyen zonas rurales endémicas, han logrado importantes avances en grupos humanos que presentaban graves deficiencias de habitabilidad. En esta situación epidemiológica, se estudió 10 años después de la terapia, a 37 chagásicos crónicos procedentes de zonas rurales de la IV Región, mediante encuesta epidemiológica (pre y post-terapia) y evaluación parasitológica (post-terapia). La encuesta incluyó: conocimiento del vector, triatominos en el domicilio actual, antecedentes de haber sido picado por triatominos y material de la construcción de la vivienda. La evaluación parasitológica se realizó mediante xenodiagnóstico (XD) (37 casos) y Reacción en Cadena de la Polimerasa (PCR) en sangre periférica (34 casos). Los resultados de la encuesta epidemiológica evidenciaron cambios importantes en la calidad de la vivienda rural de los chagásicos tratados, mientras que, en relación a la parasitemia, se detectó Trypanosoma cruzi circulante en el 89,2% de los casos (37,8% y 88,2%, mediante XD y PCR, respectivamente). Se concluye que la persistencia del parásito en condiciones post-terapia, no está relacionada con la presencia de Triatoma infestans intradomiciliario

En mayo de 2003 se efectuó una encuesta epidemio-parasitológica en pre-escolares y escolares de la Escuela San Vicente de Lo Arcaya de Colina, comuna semirural de la Provincia de Santiago ubicada en la Región Metropolitana de Chile. Hay que destacar que los niños estudiados eran asintomáticos. Los elementos parasitarios más frecuentemente encontrados fueron: Blastocystis hominis - en hombre y mujeres respectivamente - 38,8% y 44,4%, Giardia intestinalis con 9,5% y 16,2% y Enterobius vermicularis con 12,9% y 10,0%. Cryptosporidium parvum no fue encontrado y ELISA para Strongyloides stercoralis resultó positiva en un niño.

A case of massive trichuriasis in a 37-year-old female from a rural locality of the Metropolitan Region of Chile, with antecedents of alcoholism, chronic hepatic damage and portal cavernomatosis, is presented. Since 12 year ago she has had geophagia. In the last six months she has frequently presented liquid diarrhea, colic abdominal pains, tenesmus and sensation of abdominal distention. Clinical and laboratory tests confirmed her hepatic affection associated with a celiac disease with anemia and hypereosinophilia. Within a week diarrhea became worse and dysentery appeared. A colonoscopy revealed an impressive and massive trichuriasis. The patient was successfully treated with two cures of 200 mg tablets of mebendazole twice daily for three days with a week interval. After the first cure she evacuated a big amount of Tricuris trichiura, fecal evacuations became normal, geophagia disappeared and recovered 4 kg of body weight.

This policy s management, exerted by Associações de Apoio à Escola (School Support Associations), following specific composition and participation rules, was researched in five Centros de Educação de Jovens e Adultos CEJA (Youth and Adult Education Centers), in the metropolitan region of the Rio de Janeiro state. The study aimed to comprehend the means by which this social control is enforced, and if this enforcement and the participation it requires contribute to the school public management s quality this being the thesis that supported the executed investigation. Some questions have guided the investigation, namely: what is the understanding of the subjects that usually attend these spaces in the topics of social control and participation? How do they exert this social control in the daily life in a CEJA? Does this exercise of participation contribute to the quality of public management? I intended to comprehend how in a CEJA s daily life the AAE make use of social control its exercise to manage the received financial resources. The methodological route set up a sample under criteria, defining five investigation loci. Through questionnaires, I collected conceptions from student, teacher and employee subjects; additionally, I also interviewed managers, looking to know from the total of 1589 sources how, why and to what end there is participation and the exercise of social control in these spaces. From the empirical field, a lot of content relating to the participation and social control categories has surfaced, and has been grouped as follows: belonging, pedagogical, political, taking the floor and public space occupation. The motives for not partaking have also been closely observed. Cues found amidst the content and in some observed practices in the empirical field support the validity of the devised hypothesis, increasing the comprehension of the adopted theoretical referential, not without noticing the limits that reality imposes on this exercise, what compromises the effectiveness of the practices, specifically relating to the financial management, which remains the current challenge to be faced. The possible synthesis of the investigation is that the researched concepts and the way they operate in the school institutions urge the need of boosting and valorizing mechanisms for the reinforcement of popular initiatives, which constitution itself allows, expanding and qualifying for the use of the public, specially by Youth and Adult Education subjects O controle social sobre recursos financeiros descentralizados repassados diretamente às escolas foi o objeto de estudo investigado, por constituir parte integrante da política pública de financiamento da educação no país. A gestão dessa política, exercida por Associações de Apoio à Escola, segundo regras específicas de composição e participação, foi pesquisada em cinco Centros de Educação de Jovens e Adultos (CEJA), na região metropolitana do estado do Rio de Janeiro. O estudo objetivou compreender como se faz o exercício desse controle social, e se esse exercício e a participação que ele exige contribuem para a qualidade da gestão pública escolar esta a tese que sustentou a investigação realizada. Algumas perguntas guiaram a investigação, a saber: qual a compreensão dos sujeitos que circulam nesses espaços sobre controle social e participação? Como fazem o exercício desse controle social no cotidiano de um CEJA? Esse exercício de participação contribui para a qualidade da gestão pública? Intencionei compreender como no cotidiano de um CEJA as AAE fazem uso do controle social seu exercício para gestionar recursos financeiros recebidos. O percurso metodológico construiu uma amostra sob critérios, definindo cinco loci de investigação. Por meio de questionários, captei concepções de sujeitos estudantes, professores e funcionários; e complementarmente entrevistei, também, gestores, buscando saber do total de 1589 informantes como, por que e para que se participa e se exerce o controle social nesses espaços. Do campo empírico emergiram diversos conteúdos relacionados às categorias participação e controle social, agrupados do seguinte modo: pertencimento, pedagógico, político, tomada da palavra e ocupação de espaço público. Também os motivos para não participar foram atentamente observados. Indícios encontrados nos conteúdos e em algumas práticas observadas no campo empírico sustentam a validade da hipótese formulada, ampliando a compreensão do referencial teórico adotado, não sem perceber os limites que a realidade impõe a esse exercício, o que compromete a efetividade das práticas, especificamente quanto à gestão financeira, que permanece sendo o desafio a ser enfrentado. A síntese possível da investigação é que os conceitos investigados e o modo como operam na instituição escolar impõe a necessidade de impulsionar e valorizar mecanismos de reforço a iniciativas populares que a própria legislação possibilita, ampliando e qualificando espaços que se ponham a serviço da população, especialmente entre sujeitos da educação de jovens e adultos

As the social construction is formed by various instances that compound the social universe, the University is one of these instances and cannot escape its role as a training institution; it must be attentive to the question that is a suitable "space" for reflection and for the beginning of change in the lives of young people who are part of it. The general objective of this study was to make a profile of the young students entering the University, attending a college located in an outlying municipality, in this case, São Gonçalo, whether residents of the municipality or not, taking a common course, the Teacher s Licentiate degree, which does not have high prestige in the labor market. Thus, I aim to understand the higher education expansion phenomenon in the country, as I question the population that enters it. And I seek to do so from the case study of the College of Teachers Education of São Gonçalo, UERJ. By using the technique of indirect collection of information from the socioeconomic questionnaire given by the UERJ Department of Academic Selection (presented in Annex 1), we carried out the profile analysis of entering students in six courses (Education, History, Mathematics, Language, Geography and Biology), in 2013, at the College of Teachers Education. We concluded that some courses have similarities, as Pedagogy and History, which, compared to other courses, have higher number of incoming students with advanced age; they come from families with less financial resources and when go to the university have better clarity on the labor market because, as we can see, most focuses on better wages and employment. We can also conclude that there are major differences among students of Biology and teaching courses related to age, family and socioeconomic status A proposta de compreensão do jovem a partir do entendimento da dinâmica das mudanças no mundo atual enfatiza que a construção de sua identidade é uma construção social, marcada por muitas significações que devem ser entendidas dentro do contexto que lhe confere sentido. Sendo, a construção social, formada por diversas instâncias que compõem o universo social, a Universidade é uma destas instâncias e não pode se furtar ao seu papel enquanto formadora, devendo estar atenta à questão de que representa um espaço adequado para a reflexão e o início de mudança na vida dos jovens que dela fazem parte. O objetivo geral do presente trabalho foi fazer um perfil dos jovens estudantes ingressantes na Universidade, que frequentam uma Faculdade localizada em um município periférico, no caso, São Gonçalo, sendo moradores do município ou não, e que fazem um curso em comum, o de Licenciatura, sendo este um curso que não possui grande prestígio no mercado de trabalho. Dessa forma, viso compreender o fenômeno da expansão do ensino superior no país, na medida em que interrogo a população que nele ingressa. E buscarei fazê-lo a partir do estudo do caso da Faculdade de Formação de Professores de São Gonçalo, da UERJ. A partir da técnica de coleta indireta de informações junto ao questionário socioeconômico cedido pelo Departamento de Seleção Acadêmica da UERJ (apresentado no anexo), fizemos a análise do perfil dos estudantes ingressantes nos seis cursos (Pedagogia, História, Matemática, Letras, Geografia e Biologia), no ano de 2013, da Faculdade de Formação de Professores. Chegamos à conclusão de que alguns cursos apresentam similaridades, como os de Pedagogia e História, sendo verificado que apresentaram, em comparação aos outros cursos, maior número de estudantes ingressantes com idade avançada; são oriundos de famílias com menos recursos financeiros e que, ao entrarem na universidade, possuem uma melhor clareza sobre o mercado de trabalho, pois, em sua maioria, observamos o foco de melhores salários e emprego. Pode-se concluir, também, que existem grandes diferenças entre os estudantes dos cursos de Biologia e Pedagogia, relacionadas à idade, família e situação socioeconômica.
