1. Solid organ transplant patient management undergoing oral surgery: a review of literature

Abstract: A relationship between various specialties and adverse effects of post-transplant treatments gives rise to a permanent search for ways to reduce these complications. Aim of the study was to investigate the guidelines for the management of solid organ transplant patients after oral surgery treatment, considering medication employed. A literature review of scientific articles was carried out with systematic search. Terms used were “Oral Surgery” AND “Solid Organ Transplanted Patient” OR “Organ Transplanted”. Medline, Web of Science, and Scopus databases were explored, with no time restriction and in Spanish and English languages, complemented by a manual search. A total of 162 titles were retrieved. Out of these, six underwent full data extraction and were included in analysis. Data of 784 patients, with age ranging from 18 to 80 years, and a history of liver, heart, kidney, or pancreas transplants were investigated. Patients who underwent surgical dental treatment, such as extractions, biopsies, and scaling and root planning, to which different medications were administered were considered. Antibiotic prophylaxis was the most common medication administered, in addition to other medicines applied in the treatment of complications, such as post-operative bleeding. In conclusions, there is not enough scientific evidence to establish clear and universal recommendations for the management of solid organ transplant patients after oral surgery.

2. Projection neurons are necessary for the maintenance of the mouse olfactory circuit

Abstract: The assembly and maintenance of neural circuits is crucial for proper brain function. Although the assembly of brain circuits has been extensively studied, much less is understood about the mechanisms controlling their maintenance as animals mature. In the olfactory system, the axons of olfactory sensory neurons (OSNs) expressing the same odor receptor converge into discrete synaptic structures of the olfactory bulb (OB) called glomeruli, forming a stereotypic odor map. The OB projection neurons, called mitral and tufted cells (M/Ts), have a single dendrite that branches into a single glomerulus, where they make synapses with OSNs. We used a genetic method to progressively eliminate the vast majority of M/T cells in early postnatal mice, and observed that the assembly of the OB bulb circuits proceeded normally. However, as the animals became adults the apical dendrite of remaining M/Ts grew multiple branches that innervated several glomeruli, and OSNs expressing single odor receptors projected their axons into multiple glomeruli, disrupting the olfactory sensory map. Moreover, ablating the M/Ts in adult animals also resulted in similar structural changes in the projections of remaining M/Ts and axons from OSNs. Interestingly, the ability of these mice to detect odors was relatively preserved despite only having 1–5% of projection neurons transmitting odorant information to the brain, and having highly disrupted circuits in the OB. These results indicate that a reduced number of projection neurons does not affect the normal assembly of the olfactory circuit, but induces structural instability of the olfactory circuitry of adult animals.

Abstract: Abstract Background Literature suggests colonic resection and primary anastomosis (RPA) instead of Hartmann’s procedure (HP) for the treatment of left-sided colonic emergencies. We aim to evaluate the surgical options globally used to treat patients with acute left-sided colonic emergencies and the factors that leading to the choice of treatment, comparing HP and RPA. Methods This is a prospective, international, multicenter, observational study registered on ClinicalTrials.gov. A total 1215 patients with left-sided colonic emergencies who required surgery were included from 204 centers during the period of March 1, 2020, to May 31, 2020. with a 1-year follow-up. Results 564 patients (43.1%) were females. The mean age was 65.9 ± 15.6 years. HP was performed in 697 (57.3%) patients and RPA in 384 (31.6%) cases. Complicated acute diverticulitis was the most common cause of left-sided colonic emergencies (40.2%), followed by colorectal malignancy (36.6%). 4. Accuracy of Different Systems of Guided Implant Surgery and Methods for Quantification: A Systematic Review

Abstract: The aim of this systematic review was to assess the scientific literature on the accuracy of guided implant surgery and the methods used for its quantification. Two reviewers searched PubMed and the Cochrane Library to identify relevant articles published before 2023. Two methodological quality-assessment tools were used to assess the quality of the studies included. Inter-investigator reliability was verified using kappa statistics. Twenty-three clinical studies out of the 3299 articles initially identified met the inclusion criteria. The “radiographic matching method” and “digital registration method” were used to assess accuracy in guided surgery. The mean angular, coronal, and apical deviations of mucosa-supported guides ranged from 2.7° to 5.14°, 0.87 mm to 2.05 mm, and 1.08 mm to 2.28 mm, respectively. With bone-supported guides, these ranged from 2.49° to 5.08°, 0.71 mm to 1.60, and 0.77 mm to 1.65 mm, respectively. In tooth-supported guides, deviations were from 2.5° to 5.62°, 0.39 mm to 1.63 mm and 0.28 mm, and 1.84 mm, respectively. Regardless of the evaluation method, all systems exhibit some error. The minimum and maximum deviation ranges found between the planned and placed implants show that, although deviations occur, guided surgery is not far from accurate.

5. Estimación y características del riesgo meteoro-trópico que afecta a la población cubana

Abstract: Los efectos desfavorables de las condiciones meteorológicas sobre la salud de la población cubana están aumentando en frecuencia e intensidad, debido a los impactos ya visibles del cambio climático sobre Cuba, problema que demanda realizar la evaluación de la vulnerabilidad de la población cubana ante los peligros regionales asociados a los procesos meteorológicos que influyen sobre el país. Se realiza una adaptación de la metodología utilizada en Cuba para los estudios de peligro, vulnerabilidad y riesgo, aplicada en este caso a la estimación del riesgo meteoro-trópico, a partir de establecer la línea base de la mortalidad general diaria en exceso para el período 2001-2020, como indicador de la vulnerabilidad de las poblaciones residentes en las mitades occidental y oriental de Cuba; para después estimar el peligro asociado a los procesos sinópticos de escala global y regional que influyen sobre esas mismas porciones del territorio. Se demuestra que existe una relación bien definida entre la mortalidad en exceso y los peligros analizados, lo cual permitió estimar satisfactoriamente la magnitud del riesgo meteoro-trópico diario y llegar a un modelo estadístico que facilita su pronóstico a corto plazo. Se comprueba la génesis meteoro-trópica de los días con mayor mortalidad en exceso, tanto en invierno como en verano, demostrando el relevante papel que tienen algunos procesos y condiciones meteorológicas sobre la salud de la población cubana, hecho que justifica la necesidad de su pronóstico y mitigación.

Abstract: Resumen El Real Decreto-ley 2/2023, de 16 de marzo, de medidas urgentes para la ampliación de derechos de los pensionistas, simplifica las actuaciones que conducen al alta médica en los trabajadores que permanecen en situación de incapacidad temporal entre un año y año y medio de duración. Aunque puede ser eficiente en muchos casos, en los procesos de incapacidad temporal de especial complejidad, el hecho de perder el trabajo de un equipo colegiado como el Equipo de Valoración de Incapacidades, podría dificultar la toma de decisiones eficaces, justas y eficientes. Abstract: Lithium, a natural element, has been employed as a mental stabilizer in psychiatric treatments; however, some reports indicate it has an anticancer effect, prompting the consideration of repurposing lithium for cancer treatment. The potential anticancer use of lithium may depend on its form (salt type) and the type of cancer cells targeted. Little is known about the effects of Li2CO3 or LiCl on cancer cells, so we focused on exploring their effects on proliferation, apoptosis, migration, and cell cycle as part of the hallmarks of cancer. Firstly, we established the IC50 values on HeLa, SiHa, and HaCaT cells with LiCl and Li2CO3 and determined by crystal violet that cell proliferation was time-dependent in the three cell lines (IC50 values for LiCl were 23.43 mM for SiHa, 23.14 mM for HeLa, and 15.10 mM for HaCaT cells, while the IC50 values for Li2CO3 were 20.57 mM for SiHa, 11.52 mM for HeLa, and 10.52 mM for HaCaT cells.) Our findings indicate that Li2CO3 and LiCl induce DNA fragmentation and caspase-independent apoptosis, as shown by TUNEL, Western Blot, and Annexin V/IP assay by flow cytometry. Also, cell cycle analysis showed that LiCl and Li2CO3 arrested the cervical cancer cells at the G1 phase. Moreover, lithium salts displayed an anti-migratory effect on the three cell lines observed by the wound-healing assay. All these findings imply the viable anticancer effect of lithium salts by targeting several of the hallmarks of cancer.

Abstract: Abstract Pancreatic cancer cells with mutant KRAS require strong basal autophagy for viability and growth. Here, we observed that some processes that allow the maintenance of basal autophagy in pancreatic cancer cells are controlled by protein methylation. Thus, by maintaining the methylation status of proteins such as PP2A and MRAS, these cells can sustain their autophagic activity. Protein methylation disruption by a hypomethylating treatment (HMT), which depletes cellular S-adenosylmethionine levels while inducing S-adenosylhomocysteine accumulation, resulted in autophagy inhibition and endoplasmic reticulum stress-induced apoptosis in pancreatic cancer cells. We observed that by reducing the membrane localization of MRAS, hypomethylation conditions produced an imbalance in KRAS signaling, resulting in the partial inactivation of ERK and hyperactivation of the PI3K/AKT–mTORC1 pathway. Interestingly, HMT impeded CRAF activation by disrupting the ternary SHOC2 complex (SHOC2/MRAS/PP1), which functions as a CRAF-S259 holophosphatase. The demethylation events that resulted in PP2A inactivation also favored autophagy inhibition by preventing ULK1 activation while restoring the cytoplasmic retention of the MiT/TFE transcription factors. Since autophagy provides pancreatic cancer cells with metabolic plasticity to cope with various metabolic stress conditions, while at the same time promoting their pathogenesis and resistance to KRAS pathway inhibitors, this hypomethylating treatment could represent a therapeutic opportunity for pancreatic adenocarcinomas.

Abstract: Introducción: La Vigilancia Centinela de las Infecciones Respiratorias Agudas Graves (IRAG) incluye el monitoreo y caracterización de casos hospitalizados. Objetivo: Describir las características clínico-epidemiológicas y agentes etiológicos de los adultos con IRAG en un hospital centinela en Paraguay. Metodología: Estudio observacional, descriptivo, de corte transversal, con recolección de datos retrospectivos de los pacientes adultos con IRAG hospitalizados en el Hospital Nacional de Itauguá entre junio de 2022 a agosto de 2023. La información se tomó de la base de datos del Sistema Nacional de Vigilancia Epidemiológica. Resultados: Se analizaron los datos de 1598 pacientes con IRAG, 820 varones (51,3%), 875 (54,8%) con 60 o más años de edad, 1054 (66,0%) del departamento Central. El 79.2% (n=1265) de los pacientes presentaba algún factor de riesgo de gravedad o comorbilidad, 764 (47,8%) con hipertensión arterial. El 38.5% (n=632) fue admitido a UCI y la mortalidad fue de 34,7% (n=554). En el 13,8% (n=353) se confirmó SARSCoV-2, rhinovirus (1,7%), influenza A (1,5%), influenza B (0,6%), parainfluenza 3 (0,6%), VSR (0,2%), bocavirus (0,4%) e influenza AH1N1 (0,6%). Hubo significativamente mayor frecuencia de fallecidos en los de 60 años y más (40,7%), con algún factor de riesgo de gravedad (35,5% vs 25,9%) y en los confirmados con COVID-19 (38,2% vs 30,5%). Conclusión: SARSCoV-2 sigue siendo el agente etiológico principal de las IRAG. El conocimiento de la epidemiología y los agentes virales es crucial para desarrollar estrategias de prevención y terapéuticas efectivas.

Abstract: Background: The usefulness of thiopurines has been poorly explored in pouchitis and other pouch disorders. Objective: To evaluate the effectiveness and safety of azathioprine as maintenance therapy in inflammatory pouch disorders. Design: This was a retrospective and multicentre study. Methods: We included patients diagnosed with inflammatory pouch disorders treated with azathioprine in monotherapy. Effectiveness was evaluated at 1 year and in the long term based on normalization of stool frequency, absence of pain, faecal urgency or fistula discharge (clinical remission), or any improvement in these symptoms (clinical response). Endoscopic response was evaluated using the Pouchitis Disease Activity Index (PDAI). Results: In all, 63 patients were included [54% males; median age, 49 (28–77) years]. The therapy was used to treat pouchitis ( n = 37) or Crohn’s disease of the pouch ( n = 26). The rate of clinical response, remission and non-response at 12 months were 52%, 30% and 18%, respectively. After a median follow-up of 23 months (interquartile range 11–55), 19 patients (30%) were in clinical remission, and 45 (66%) stopped therapy. Endoscopic changes were evaluated in 19 cases. PDAI score decreased from 3 (range 2–4) to 1 (range 0–3). In all, 21 patients (33%) presented adverse events and 16 (25%) needed to stop therapy. Conclusion: Azathioprine may be effective in the long term for the treatment of inflammatory pouch disorders and could be included as a therapeutic option.

Abstract: Abstract Background Establishing the level of surgical difficulty pre-operatively is an essential step in ensuring correct treatment planning. This study set out to determine whether the knowledge and experience acquired by dentists who had received different levels of training influenced, firstly, the perceived levels of difficulty of a variety of cases of mandibular third molar (MTM) extraction and, secondly, the perceived difficulty deriving from a series of factors (patient-related factors, anatomical and radiographic factors, operative factors). Methods This cross-sectional, descriptive, observational study took the form of a survey. Using a visual analog scale (VAS), participants evaluated both the perceived difficulty of 30 cases of MTM extraction described by means of digital panoramic radiographs and the perceived difficulty deriving from a series of factors conditioning MTM extraction. The results underwent statistical analysis with SPSS Statistics 28.0 software. Non-parametric tests (Mann Whitney test for independent samples and the Kruskal–Wallis test) were applied. Results A total of 389 surveys were available for analysis. Dental practioners with no surgical training saw the intervention as presenting greater difficulty. Professionals with postgraduate training in oral surgery considered patient-related factors more important than operative factors, in contrast to dentists who had not received oral surgery training. Conclusions Dental training has a signficant influence on the perceived difficulty of MTM extraction and also affects opinions about which factors have greater or lesser influence on surgical difficulty.

13. Trampa de fondo como una alternativa en el control integrado del ácaro Varroa destructor en colmenas de abejas africanizadas

Abstract: Se midió la efectividad de la trampa de fondo en el control del ácaro Varroa destructor en colmenas de abejas africanizadas, de junio a setiembre 2017, en Atenas, Alajuela. Se registró la caída natural de ácaros durante un período de 30 días en la trampa, la cual consistió en un fondo de madera (50 x 42 cm), cubierto con cedazo metálico en la parte superior, con un tamaño de tamiz de seis agujeros por cm2. En el interior de la trampa, se colocó una lámina de cartulina blanca, impregnada con vaselina, previamente cuadriculada, para facilitar el conteo de los ácaros, la cual se reemplazó cada siete días. Luego del día 30, los ácaros remantes se eliminaron. Se determinó la efectividad del método, para lo cual se aplicó cuatro tiras de flumetrina a cada colmena, durante seis semanas. Adicionalmente, se evaluó la mortalidad de los diferentes estadios de varroa en la cría sellada (operculada) de obrera. La caída natural de ácaros, en 30 días, fue de 715 ± 531 (n= 5). La mayor cantidad de varroa se observó al día 22, colectándose 335 ± 285 ácaros, mientras que el menor número se obtuvo el primer día de muestreo con 30 ± 13. Al aplicar flumetrina, se eliminaron 2072 ± 1874 ácaros. Este procedimiento permitió establecer la cantidad de ácaros caídos naturalmente y eliminados de la colmena mediante la trampa de fondo, correspondió a un 25.7%. Por otra parte, se determinó que la mortalidad de los diferentes estadios de varroa, en la cría sellada de obrera, fue menor a un 10.0%. En conclusión, la trampa de fondo eliminó más de la cuarta parte de los ácaros presentes en las colmenas, por lo cual, este método puede ser considerado como una opción en el manejo integrado de varroa en colmenas de abejas africanizadas.

14. Maxillary Sinus Augmentation with Autogenous Tooth Grafting Material: A Systematic Review

Abstract: The aim of this systematic review was to determine whether autogenous tooth grafting material (ATGM) is as safe and effective as other bone substitutes used for maxillary sinus augmentation procedures, evaluating histomorphometric and/or histological data, implant primary stability, associated complications and radiographic bone height measurements. An automated electronic search was conducted using four databases (Medline/PubMed, Scopus, Web of Science and Cochrane Library), supplemented by a manual search, to identify clinical human studies using particulate ATGM for the aforementioned procedure. The included studies had a sample size of at least four patients and were published before 31st July 2024. The Newcastle–Ottawa scale (NOS) and Joanna Briggs Institute (JBI) Critical Appraisal Checklist were used to assess the risk of bias in cohort studies and case series, respectively. Seven studies were included in the descriptive analysis, obtaining 128 participants (46.8% only treated with ATGM) and 192 placed implants. Due to the heterogeneity of the studies, meta-analysis could not be performed. The authors concluded that ATGM appears to be a feasible and safe alternative for maxillary sinus augmentation procedures. These results should be interpreted with caution due to the limited amount of scientific evidence on this topic and the heterogeneity between the included studies.

Abstract: The increasing demand for optimizing the use of agricultural resources will require the adoption of cutting-edge technologies and precision farming management. Unmanned Aerial Vehicle (UAV) sprayers seem promising due to their potential to perform precision or spot spraying, particularly in woody crop environments where total surface spraying is unnecessary. However, incorporating this technology is limited by the lack of scientific knowledge about the environmental risks associated with UAV sprayers and the strict legal framework. Nonetheless, these spraying systems’ characteristic downwash airflow and the limited swath width can potentially mitigate drift in hedgerow crops. During our study we performed comparative studies aimed to compare the airborne drift, soil, and crop depositions between a conventional orchard sprayer and a UAV sprayer in a commercial superhigh-density orchard in the South Iberian Peninsula in 2022. Our findings reveal that, in superhigh-density olive orchards, the UAV sprayer presents a substantial reduction in airborne drift, while soil depositions showed no significant differences compared to those of a conventional terrestrial orchard sprayer. Crop depositions were significantly lower when utilizing the UAV sprayer. These results suggest that introducing UAV spraying technology in Mediterranean agricultural systems, under specific scenarios, can effectively reduce the environmental impact of crop spraying and encourage the responsible use of plant protection products (PPPs).

16. Cobertura de vacunación contra COVID-19 e influenza en personal de salud y administrativo en Paraguay

Abstract: Introducción: A nivel mundial, la cobertura de vacunación contra el COVID-19, así como contra la influenza es baja tanto en la población general como en los profesionales de la salud a pesar de que la vacuna es gratuita y obligatoria en el personal sanitario. Objetivo: Describir la cobertura de vacunación contra el COVID -19, y la influenza en personal de salud y administrativo de un hospital de referencia del Ministerio de Salud Pública y Bienestar Social en el periodo 2021-2022. Metodología: Estudio observacional descriptivo de corte trasverso. Se hizo la revisión de los registros del personal sanitario y administrativo del centro vacunatorio del Hospital Nacional de Itauguá de la campaña vacunal contra el COVID-19 y de anti-influenza en el periodo 2021- 2022. Resultados: De los 3.586 funcionarios, 999 (27,9 %) eran médicos, 1494 (41,7 %) personal de enfermería, 366 (10,2 %) otra categoría de personal sanitario, y 727 (20,3 %) personal administrativo. En forma global, el 86,5 % de los funcionarios recibió por lo menos las dos dosis que constituyen el esquema primario y el 73 % la dosis de refuerzo. El 2,1 % del personal no recibió ninguna dosis de vacuna anti covid-19, la cifra fue mayor en el personal administrativo (4,8 %). La cobertura de vacunación contra la influenza fue de 20 % en el 2021 y 25 % en el 2022. Discusión: Si bien cobertura de vacunación anti-COVID-19 fue comparable a otros países, la vacunación contra la influenza fue muy baja. Es urgente implementar estrategias dirigidas a aumentar la percepción de riesgo y aceptabilidad de las vacunas obligatorias para el personal sanitario.

El objetivo es presentar dos pacientes del sexo masculino con neuroparacoccidiodomicosis internados en el Departamento de Medicina Interna del Hospital Nacional, uno en el año 2017 y el otro en el 2021. Ambos pacientes presentaron síntomas neurológicos con mejoría de las lesiones con anfotericina B. Los granulomas cerebrales de PCM pueden tener comportamiento pseudotumoral. La regresión de las lesiones fue completa con anfotericina B y trimetoprin sulfametoxasol en el primer caso y anfotericina B e itraconazol en el segundo caso.

18. Impact of hypertension diagnosis on morbidity and mortality: a retrospective cohort study in primary care

Abstract: Abstract Background Hypertension is responsible for a huge burden of disease. The aim of this study was to evaluate the impact of newly diagnosed hypertension on the occurrence of kidney or cardiovascular events (K/CVEs) and on mortality among community dwellers. Methods Retrospective cohort study, conducted from January, 2007, to December, 2018. All patients (age > 18) newly diagnosed with hypertension and no previous K/CVEs in 2007 and 2008, in the primary care centers of Madrid (Spain) (n = 71,770), were enrolled. The control group (n = 72,946) included patients without hypertension, matched by center, sex and age. The occurrence of kidney or CV events, including mortality from these causes and total mortality were evaluated using Cox regression and multistate models. Data were collected from three sources: personal data from administrative records, clinical data from medical records, and mortality data from regional and national databases. Results The median follow-up was 138.61 months (IQR: 124.68–143.97 months). There were 32,896 K/CVEs (including 3,669 deaths from these causes) and 12,999 deaths from other causes. Adjusted for sex, smoking, diabetes and socioeconomic status, K/CVEs HR was 4.36 (95% CI: 3.80–5.00) for diagnoses before 45 years of age, 2.45(95% CI: 2.28- 2.63) for diagnosis between 45 to 54 years, and HR decreased to 1.86 (95% CI: 1.64–210) for diagnoses over age 85. Total mortality risk was only higher for hypertension diagnosed before 55 years of age (HR: 2.47, 95% CI: 1.90–3.19 for ages 18 to 44; and HR: 1.14, 95% CI: 1.02–1.28 for ages 45 to 54). Conclusion The diagnosis of hypertension in the community environment, in patients without evidence of previous kidney or CV disease, is associated with a large increase in the risk of K/CVEs, but especially in individuals diagnosed before the age of 55. This diagnosis is only associated with an increase in kidney or cardiovascular mortality or overall mortality when it occurs before age 55.

Se presenta un varón de 65 años de edad, hipertenso conocido, tratado con losartan, ingresa con una historia de 15 días de evolución con astenia, anorexia, pérdida de peso y cefalea holocraneana, y ptosis palpebral de una semana de evolución. Al examen físico se constata una protrusión ocular y parálisis del tercer par del ojo derecho. Presentó hiperglicemia que fue revertida con insulina. La resonancia magnética de órbita y senos paranasales mostró un engrosamiento del nervio óptico derecho, ocupación de los senos paranasales. El paciente fue sometido a un tratamiento quirúrgico de la lesión retrocular, la biopsia de la lesión mostró al examen directo hifas hialinas compatible con mucorales. Se inició tratamiento con amfotericina B.

Abstract: Introducción. La pandemia por COVID-19 desde su inicio hasta el 13 de agosto de 2022, ha causado 19.478 muertes en Paraguay, con una tasa de letalidad de 2,78%. El objetivo del estudio es comparar las características de los pacientes fallecidos por COVID-19 en dos años, enero-diciembre, 2021 y enero-julio 2022. Material y Método. Estudio analítico retrospectivo de corte transverso de pacientes adultos y pediátricos fallecidos por COVID-19 confirmados por RT-PCR en hisopados nasofaríngeos que estaban internados en el área respiratoria del Hospital Nacional de Itauguá. Resultados. De 5265 pacientes internados con COVID-19, fallecieron 1512, siendo la letalidad global de 28,7%, en el 2021 fue 26,3% (1164/4423) y 41,3% (348/842) en el 2022. Hubo mayor porcentaje de varones (53,6%) entre los fallecidos, significativamente mayor (p=0.045) en el 2022 (58,3%) que en el 2021 (52,2%). El 69% de los fallecidos eran mayores de 60 años en el 2022 y 48,3% en 2021. El 79,2% de los pacientes tenía algún factor de riesgo, que fue mayor en el 2022 (84,3%) que en el 2021 (77,7%), sobre todo, HTA (54,3% vs 42,2%), cardiopatía (15,8% vs 9,7%), ERC (14,4% vs 5,4%), EPOC (11,5% vs 5,4%), inmunodeficiencia (6,3% vs 2,5%) y enfermedad neurológica (3,4% vs 1,5%). Abstract: Las plantas del género Senna pertenecen a la familia Fabaceae, distribuidas, mundialmente, en zonas tropicales y subtropicales. Algunas especies del género contienen productos del metabolismo secundario, los cuales se han asociado con miopatía tóxica y necrosis hepática en diferentes especies animales. El objetivo de este reporte de caso fue documentar, por primera vez en Costa Rica, la miotoxicidad en terneros asociada con el consumo de Senna cobanensis, una especie de Senna que no ha sido reportada previamente como miotóxica en la literatura en la cual se observó, afectados, 4 de 18 animales, con muerte de 2 afectados. El caso se presentó en una finca de ganado bovino cebú, en el Pacífico Central de Costa Rica, durante diciembre de 2019. Se realizó examen clínico y análisis de la dieta. Se encontró problemas locomotores como paraparesia, y un aumento en la actividad de AST y CK-NAC, consistente con daño en el tejido muscular. El análisis histopatológico reveló degeneración y necrosis de las fibras musculares. Se descartó otros agentes miopáticos como la deficiencia de selenio y la intoxicación por ionóforos. De acuerdo con los resultados obtenidos en el estudio, se concluye que Senna cobanensis es otra especie del género que provoca miotoxicidad en bovinos.

22. Características clínicas y laboratoriales de pacientes adultos ambulatorios con Chikungunya del Hospital Nacional de Itauguá de Paraguay durante la epidemia 2022-2023.

Abstract: Introducción: La infección aguda por chikungunya produce una infección auto-limitada caracterizada principalmente por fiebre alta, artralgias, artritis, cefalea, mialgias y exantema pruriginoso; una minoría presenta síntomas persistentes que puede evolucionar a la cronicidad. Objetivo: describir las características clínicas y laboratoriales de pacientes adultos ambulatorios con chikungunya en un hospital de referencia durante la epidemia del 2022-2023 en Paraguay. Materiales y Método: estudio descriptivo de corte trasversal de pacientes adultos con chikungunya que consultaron en el Hospital Nacional de Itauguá entre enero-marzo de 2023. Resultados: Se incluyeron 140 pacientes entre 19 y 79 años (edad media: 46,6±13,6), 77,9% del sexo femenino y 89,3% del departamento Central. El 72,9% de los pacientes fueron confirmados por RT-PCR y el resto por la clínica o serología. El 48,6% tenía alguna comorbilidad, el 65% se encontraba en la fase aguda, y el resto en la fase subaguda (35%). Los síntomas más frecuentes fueron la poliartralgia (96,4%); artritis (98,6%), fiebre (95,7%), astenia (100%), mialgias (96,4%), cefalea (82,9%), exantema (95%). Los síntomas gastrointestinales fueron menos frecuentes; náuseas (37%), vómitos (15,7%) y diarrea (20%). Entre los principales hallazgos laboratoriales se encontraron la proteína C reactiva aumentada (45,7%), leucopenia (23,6%), eritrosedimentación aumentada (27,9%), citólisis hepática leve (13,6%). Todos fueron tratados con paracetamol; el 60,4% y el 87,8% de los pacientes en fase aguda y subaguda recibieron AINES, respectivamente. Conclusión: La combinación de fiebre alta y artralgia severa fue el rasgo característico de la infección por el virus chikungunya en la epidemia en Paraguay.

Abstract: Nowadays, there are plenty of data sources generating massive amounts of information that, combined with novel data analytics frameworks, are meant to support optimisation in many application domains. Nonetheless, there are still shortcomings in terms of data discoverability, accessibility and interoperability. Open Data portals have emerged as a shift towards openness and discoverability. However, they do not impose any condition to the data itself, just stipulate how datasets have to be described. Alternatively, the NGSI-LD standard pursues harmonisation in terms of data modelling and accessibility. This paper presents a solution that bridges these two domains (i.e., Open Data portals and NGSI-LD-based data) in order to keep benefiting from the structured description of datasets offered by Open Data portals, while ensuring the interoperability provided by the NGSI-LD standard. Our solution aggregates the data into coherent datasets and generate high-quality descriptions, ensuring comprehensiveness, interoperability and accessibility. The proposed solution has been validated through a real-world implementation that exposes IoT data in NGSI-LD format through the European Data Portal (EDP). Moreover, the results from the Metadata Quality Assessment that the EDP implements, show that the datasets’ descriptions generated achieve excellent ranking in terms of the Findability, Accessibility, Interoperability and Reusability (FAIR) data principles.

Abstract: The vast amount of information stemming from the deployment of the Internet of Things and open data portals is poised to provide significant benefits for both the private and public sectors, such as the development of value-added services or an increase in the efficiency of public services. This is further enhanced due to the potential of semantic information models such as NGSI-LD, which enable the enrichment and linkage of semantic data, strengthened by the contextual information present by definition. In this scenario, advanced data processing techniques need to be defined and developed for the processing of harmonised datasets and data streams. Our work is based on a structured approach that leverages the principles of linked-data modelling and semantics, as well as a data enrichment toolchain framework developed around NGSI-LD. Within this framework, we reveal the potential for enrichment and linkage techniques to reshape how data are exploited in smart cities, with a particular focus on citizen-centred initiatives. Moreover, we showcase the effectiveness of these data processing techniques through specific examples of entity transformations. The findings, which focus on improving data comprehension and bolstering smart city advancements, set the stage for the future exploration and refinement of the symbiosis between semantic data and smart city ecosystems.

Abstract: Tension and force propagation play a central role in tissue morphogenesis, as they enable sub- and supra-cellular shape changes required for the generation of new structures. Force is often generated by the cytoskeleton, which forms complex meshworks that reach cell–cell or cell–extracellular matrix junctions to induce cellular rearrangements. These mechanical properties can be measured through laser microdissection, which concentrates energy in the tissue of interest, disrupting its cytoskeleton. If the tissue is undergoing tension, this cut will induce a recoil in the surrounding regions of the cut. This protocol describes how one can perform laser microdissection experiments and subsequently measure the recoil speed of the sample of interest. While we explain how to carry out these experiments in Drosophila embryos, the recoil calibration and downstream analyses can be applied to other types of preparations. This flexibility and broad applicability make it an indispensable tool in reliability analysis and survival modeling. This manuscript explores five parameterizations of the Weibull distribution, each based on different moments, like mean, quantile, and mode. It meticulously characterizes each parameterization, introducing a novel one based on the model’s mode, along with its hazard and survival functions, shedding light on their unique properties. Additionally, it delves into the interpretation of regression coefficients when incorporating regression structures into these parameterizations. It is analytically established that all five parameterizations define the same log-likelihood function, underlining their equivalence. Through Monte Carlo simulation studies, the performances of these parameterizations are evaluated in terms of parameter estimations and residuals. The models are further applied to real-world data, illustrating their effectiveness in analyzing material fatigue life and survival data. Abstract: Ultrafast laser pulses provide unique tools to manipulate magnetization dynamics at femtosecond timescales, where the interaction of the electric field usually dominates over the magnetic field. Recent proposals using structured laser beams have demonstrated the possibility to produce regions where intense oscillating magnetic fields are isolated from the electric field. In these conditions, we show that technologically feasible tesla-scale circularly polarized high-frequency magnetic fields induce purely precessional nonlinear magnetization dynamics. This fundamental result not only opens an avenue in the study of laser-induced ultrafast magnetization dynamics, but also sustains technological implications as a route to promote all-optical non-thermal magnetization dynamics both at shorter timescales – towards the sub-femtosecond regime – and at THz frequencies.

Abstract: Many phenomena can be described by random variables that follow asymmetrical distributions. In the context of regression, when the response variable Y follows such a distribution, it is preferable to estimate the response variable for predictor values using the conditional median. Quantile regression models can be employed for this purpose. However, traditional models do not incorporate a distributional assumption for the response variable. To introduce a distributional assumption while preserving model flexibility, we propose new varying-coefficients quantile regression models based on the family of log-symmetric distributions. We achieve this by reparametrizing the distribution of the response variable using quantiles. Parameter estimation is performed using a maximum likelihood penalized method, and a back-fitting algorithm is developed. Additionally, we propose diagnostic techniques to identify potentially influential local observations and leverage points. Finally, we apply and illustrate the methodology using real pollution data from Padre Las Casas city, one of the most polluted cities in Latin America and the Caribbean according to the World Air Quality Index Ranking.

37. Further investigations on the Effective Width for district heating systems

Abstract: District Heating is a cornerstone for the decarbonisation of the heating and cooling sector in Europe. Nonetheless, this technology is currently absent in a majority of the continent’s urban areas, and hence the need for appropriate methods by which to estimate the cost, as well as underlying cost parameters, to assess the feasibility of developing district heating networks is of general interest. One key underlying cost parameter, the concept of Effective Width, which is the ratio between a land area and the trench length within that land area, is the focus quantity of this work. Effective Width enables a first order assessment of the total route length of pipes in a given urban area and, together with the average diameter of the pipes, allows an estimation of the investment cost of installing district heating pipes. However, initial implementations of the Effective Width have been based on rather limited empirical evidence, such as a small set of cases and often disregarding service pipes due to lack of data. Another shortcoming of previous studies is the extrapolation of established relations into more sparsely populated areas. By assembly of a richer database, which contains building data, heat consumption data in the supplied areas, as well as actual network information (numerical and geographical), provided by several district heating companies in Denmark and Sweden, the objectives of this study are twofold: first, to improve the general understanding of Effective Width and its relation to building density, and secondly, to study the particular case of sparse areas. The results of this study provide new insight to enhance our understanding of the Effective Width concept which may facilitate better assessments of future district heating systems.

The GR@LNPs were formulated to target brain border-associated macrophages (BAMs) as a central nervous system (CNS) therapy platform for silencing neuroinflammation-related genes. We specifically aim at inhibiting the expression of the gene encoding for lipocalin-type prostaglandin D synthase (L-PGDS), an anti-inflammatory enzyme expressed in BAMs, whose level of expression is altered in neuropsychopathologies such as depression and schizophrenia. The GR@LNPs are expected to demonstrate a bio-orthogonal genetic activity reacting with L-PGDS gene transcripts inside the living system without interfering with other genetic or biochemical circuitries. To facilitate selective BAM phagocytosis and avoid subsidiary absorption by other cells, they were functionalized with a mannosylated lipid as a specific MAN ligand for the mannose receptor presented by the macrophage surface. The GR@LNPs showed a high GR-packing density in a compact multilamellar configuration as structurally characterized by light scattering, zeta potential, and transmission electronic microscopy. As a preliminary biological evaluation of the mannosylated GR@LNP nanovectors into specifically targeted BAMs, we detected in vivo gene interference after brain delivery by intracerebroventricular injection (ICV) in Wistar rats subjected to gene therapy protocol. The results pave the way towards novel gene therapy platforms for advanced treatment of neuroinflammation-related pathologies with ASO@LNP nanovectors.

Abstract: Resumen Tras el alta médica finaliza un proceso de incapacidad temporal. se considera que existe recaída en un mismo proceso de incapacidad temporal cuando se produce una nueva baja médica, por la misma o similar patología, dentro de los ciento ochenta días naturales siguientes a la fecha de efectos del alta médica anterior. Cuando estas recaídas ocurren tras altas médicas emitidas por el Instituto Nacional de la Seguridad Social española quedan sometidas a especiales requerimientos reglamentarios que consisten en un sistema de inspección directa llevada a cabo por los inspectores médicos adscritos a la Seguridad Social, así como por los Equipos de Valoración de Incapacidades. Las recaídas afectan directamente a la delimitación de la duración máxima del subsidio de incapacidad temporal en cada trabajador. Su número, y por tanto el gasto económico que ocasionan al sistema de seguridad social, puede disminuir al colaborar con los Servicios de Prevención de las empresas y su control puede verse favorecido aplicando modelos de inteligencia artificial.

Abstract: Abstract Background The application of immune-based therapies has revolutionized cancer treatment. Yet how the immune system responds to phenotypically heterogeneous populations within tumors is poorly understood. In melanoma, one of the major determinants of phenotypic identity is the lineage survival oncogene MITF that integrates diverse microenvironmental cues to coordinate melanoma survival, senescence bypass, differentiation, proliferation, invasion, metabolism and DNA damage repair. Whether MITF also controls the immune response is unknown. Methods By using several mouse melanoma models, we examine the potential role of MITF to modulate the anti-melanoma immune response. ChIP-seq data analysis, ChIP-qPCR, CRISPR-Cas9 genome editing, and luciferase reporter assays were utilized to identify ADAM10 as a direct MITF target gene. Western blotting, confocal microscopy, flow cytometry, and natural killer (NK) cytotoxicity assays were used to determine the underlying mechanisms by which MITF-driven phenotypic plasticity modulates melanoma NK cell-mediated killing. Results Here we show that MITF regulates expression of ADAM10, a key sheddase that cleaves the MICA/B family of ligands for NK cells. By controlling melanoma recognition by NK-cells MITF thereby controls the melanoma response to the innate immune system. Consequently, while melanoma MITFLow cells can be effectively suppressed by NK-mediated killing, MITF-expressing cells escape NK cell surveillance. Conclusion Our results reveal how modulation of MITF activity can impact the anti-melanoma immune response with implications for the application of anti-melanoma immunotherapies.

Abstract: Summary: Background: Early death after cancer surgery is higher in low-income and middle-income countries (LMICs) compared with in high-income countries, yet the impact of facility characteristics on early postoperative outcomes is unknown. The aim of this study was to examine the association between hospital infrastructure, resource availability, and processes on early outcomes after cancer surgery worldwide. Methods: A multimethods analysis was performed as part of the GlobalSurg 3 study—a multicentre, international, prospective cohort study of patients who had surgery for breast, colorectal, or gastric cancer. The primary outcomes were 30-day mortality and 30-day major complication rates. Potentially beneficial hospital facilities were identified by variable selection to select those associated with 30-day mortality. Adjusted outcomes were determined using generalised estimating equations to account for patient characteristics and country-income group, with population stratification by hospital. Findings: Between April 1, 2018, and April 23, 2019, facility-level data were collected for 9685 patients across 238 hospitals in 66 countries (91 hospitals in 20 high-income countries; 57 hospitals in 19 upper-middle-income countries; and 90 hospitals in 27 low-income to lower-middle-income countries). The availability of five hospital facilities was inversely associated with mortality: ultrasound, CT scanner, critical care unit, opioid analgesia, and oncologist. 46. The Punta de la Mona Rhodolith Bed: Shallow-Water Mediterranean Rhodoliths (Almuñecar, Granada, Southern Spain)

Abstract: Shallow-water rhodolith beds are rare in the Mediterranean Sea and generally poorly known. The Punta de la Mona rhodolith bed extends for 16,000 square meters in shallow and oligotrophic waters at the southern coast of Spain, off Almuñecar in the Alborán Sea. We present a detailed analysis of the structure (rhodolith cover and density, rhodolith size and shape, sediment granulometry) and morphospecies composition of the bed along a depth gradient. A stratified sampling was carried out at six depths (9, 12, 15, 18, 21, and 24 m), estimating rhodolith cover and abundance; rhodoliths were collected from one 30 by 30 cm quadrat for each transect, resulting in 18 samples and a total of 656 rhodoliths. The collected rhodoliths were measured and the coralline algal components identified morphoanatomically through a stereomicroscope and SEM. Sediment on the seafloor mainly consisted of pebbles and cobbles; the highest rhodolith cover occurred between 15 and 18 m, and the lowest at the shallowest and deepest transects (9 and 24 m). Mean Rhodolith size was similar throughout the depth range (23–35 mm) with a slight increase at 24 m, although the largest rhodoliths occurred at 21 m. In monospecific rhodoliths, size depended more on the forming species than on depth. We found 25 non-geniculate coralline morphospecies, nearly all rhodolith-forming morphospecies reported in the Mediterranean Sea in recent accounts. The highest morphospecies richness (18–19) and proportional abundance were found at intermediate depths (15–18 m), where rhodolith cover is also highest. Lithophyllum incrustans and Lithophyllum dentatum dominated at shallow depths (9–12 m), whereas Lithothamnion valens was the dominant species at intermediate and greater depths. Overall, the latter species was the most common in the rhodolith bed. The shallow-water rhodolith bed in Punta de la Mona is probably the most diverse in the Mediterranean Sea. This highlights the importance of the conservation of this habitat and, in general, emphasizes the role of the Alborán Sea as a diversity center of coralline algae. The Punta de la Mona example contradicts the common assumption in the geological literature that rhodolith beds are indicative of oligophotic environments with high nutrients levels.

Abstract: Current asymmetric cryptography is based on the principle that while classical computers can efficiently multiply large integers, the inverse operation, factorization, is significantly more complex. For sufficiently large integers, this factorization process can take in classical computers hundreds or even thousands of years to complete. However, there exist some quantum algorithms that might be able to factor integers theoretically -- the theory works properly, but the hardware requirements are far away from what we can build nowadays -- and, for instance, Yan, B. et al. ([14]) claim to have constructed a hybrid algorithm which could be able even to challenge RSA-2048 in the near future. Abstract: Resumen: Introducción: Cada vez hay más niños escolarizados con enfermedades crónicas, por lo que los profesores deben ser conscientes de esta nueva realidad y de las necesidades especiales que pueden tener estos alumnos. Sin embargo, tenemos muy poca información sobre los conocimientos, actitudes y preocupaciones de los profesores ante una posible situación de riesgo en un escolar con alguna enfermedad crónica, motivo por el que se ha realizado el presente estudio. Métodos: Cuestionario anónimo con preguntas sencillas sobre crisis convulsivas, diabetes, anafilaxia y reanimación cardiopulmonar básica, realizado a 244 profesores de educación primaria y secundaria, previa a su participación en talleres breves centrados en el manejo de urgencias médicas en relación con dichas patologías. Resultados: El 60% de los profesores encuestados tenía en clase al menos un alumno con patología crónica, la más frecuente la epilepsia. Su principal preocupación era desconocer cómo actuar ante una situación potencialmente grave. En relación con las crisis hipoglucémicas y la anafilaxia, los profesores que tienen alumnos de riesgo, si bien conocen cuál es la medicación recomendada, no saben utilizarla correctamente. Conclusiones: Los profesores de nuestro medio muestran preocupación ante el desconocimiento de las actuaciones que deben acometer ante un evento urgente en un escolar con enfermedad crónica. Aunque algunos han recibido información sobre la medicación a administrar, no están seguros de ser capaces de hacerlo correctamente. Por tanto, se debería mejorar la formación, capacitación y autoconfianza de los educadores en la atención a posibles situaciones urgentes en escolares con enfermedades crónicas. 50. Effectiveness of the bone ring technique and simultaneous implant placement for vertical ridge augmentation: a systematic review

Abstract: Abstract Background Dimensional changes after dental extraction frequently lead to situations in which bone augmentation procedures are required prior to dental implant placement. Bone ring technique (BRT) has been described as a one-stage approach to restore vertical alveolar ridge defects, in which an autogenous or allogeneic cortico-cancellous bone block graft is stabilized with a dental implant inserted simultaneously. The objective of this systematic review was to evaluate the clinical performance of BRT. Materials and methods This review was conducted according to PRISMA guidelines. An electronic search was conducted in four databases: (1) The National Library of Medicine (MEDLINE/PubMed) via Ovid; (2) Web of Science (WOS); (3) SCOPUS; and (4) Cochrane Central Register of Controlled Trials (CENTRAL). The Newcastle-Ottawa Quality Assessment Scale and The Joanna Briggs Institute Critical Appraisal tool were used to assess the quality of evidence in the studies reviewed. Results Sixteen studies with a total of 186 patients treated with 219 bone rings bocks were included in the review. The studies showed a mean bone gain of 4.94 mm, mean bone resorption of 0.83 mm, and mean marginal bone loss of 0.57 mm after a mean follow-up period of 13.35 months. A mean bone ring survival rate of 97.26% and implant survival rate of 94.97% were recorded. Conclusions BRT would appear to be an adequate alternative technique for restoring single vertical alveolar ridge defects with simultaneous dental implant placement. However, further studies comparing this technique with other vertical ridge augmentation procedures in different clinical scenarios are needed to confirm the present results.
