Supplemental Digital Content is Available in the Text., Objectives: To (1) demonstrate that the AO Spine Sacral Classification System can be reliably applied by general orthopaedic surgeons and subspecialists universally around the world and (2) delineate those injury subtypes that are most difficult to classify reliably to refine the classification before evaluating clinical outcomes. Design: Agreement study. Setting: All-level trauma centers, worldwide. Participants: One hundred seventy-two members of the AO Trauma and AO Spine community. Intervention: The AO Sacral Classification System was applied by each surgeon to 26 cases in 2 independent assessments performed 3 weeks apart. Main Outcome Measurements: Interobserver reliability and intraobserver reproducibility. Results: A total of 8097 case assessments were performed. The kappa coefficient for interobserver agreement for all cases was 0.72/0.75 (assessment 1/assessment 2), representing substantial reliability. When comparing classification grading (A/B/C) regardless of subtype, the kappa coefficient was 0.84/0.85, corresponding to excellent reliability. The kappa coefficients for interobserver reliability were 0.95/0.93 for type A fractures, 0.78/0.79 for type B fractures, and 0.80/0.83 for type C fractures. The overall kappa statistic for intraobserver reliability was 0.82 (range 0.18–1.00), representing excellent reproducibility. When only evaluating morphology type (A/B/C), the average kappa value was 0.87 (range 0.18–1.00), representing excellent reproducibility. Conclusion: The AO Spine Sacral Classification System is universally reliable among general orthopaedic surgeons and subspecialists worldwide, with substantial interobserver and excellent intraobserver reliability.

Study Design: Cross-sectional, international survey. Objectives: To identify factors influencing pharmacologic anticoagulation initiation after spine surgery based on the AOSpine Anticoagulation Global Survey. Methods: This survey was distributed to the international membership of AOSpine (n = 3805). A Likert-type scale described grade practice-specific factors on a scale from low (1) to high (5) importance, and patient-specific factors a scale from low (0) to high (3) importance. Analysis was performed to determine which factors were significant in the decision making surrounding the initiation of pharmacologic anticoagulation. Results: A total of 316 spine surgeons from 64 countries completed the survey. In terms of practice-specific factors considered to initiate treatment, expert opinion was graded the highest (mean grade ± SD = 3.2 ± 1.3), followed by fellowship training (3.2 ± 1.3). Conversely, previous studies (2.7 ± 1.2) and unspecified guidelines were considered least important (2.6 ± 1.6). Patient body mass index (2.0 ± 1.0) and postoperative mobilization (2.3 ± 1.0) were deemed most important and graded highly overall. Those who rated estimated blood loss with greater importance in anticoagulation initiation decision making were more likely to administer thromboprophylaxis at later times (hazard ratio [HR] = 0.68-0.71), while those who rated drain output with greater importance were likely to administer thromboprophylaxis at earlier times (HR = 1.32-1.43). Conclusion: Among our global cohort of spine surgeons, certain patient factors (ie, patient mobilization and body mass index) and practice-specific factors (ie, expert opinion and fellowship training) were considered to be most important when considering anticoagulation start times.

Study Design: Cross-sectional, international survey. Objectives: This study addressed the global perspectives concerning perioperative use of pharmacologic thromboprophylaxis during spine surgery along with its risks and benefits. Methods: A questionnaire was designed and implemented by expert members in the AO Spine community. The survey was distributed to AO Spine’s spine surgeon members (N = 3805). Data included surgeon demographic information, type and region of practice, anticoagulation principles, different patient scenarios, and comorbidities. Results: A total of 316 (8.3% response rate) spine surgeons completed the survey, representing 64 different countries. Completed surveys were primarily from Europe (31.7%), South/Latin America (19.9%), and Asia (18.4%). Surgeons tended to be 35 to 44 years old (42.1%), fellowship-trained (74.7%), and orthopedic surgeons (65.5%) from academic institutions (39.6%). Most surgeons (70.3%) used routine anticoagulation risk stratification, irrespective of geographic location. However, significant differences were seen between continents with anticoagulation initiation and cessation methodology. Specifically, the length of a procedure ( P = .036) and patient body mass index ( P = .008) were perceived differently when deciding to begin anticoagulation, while the importance of medical clearance ( P < .001) and reference to literature ( P = .035) differed during cessation. For specific techniques, most providers noted use of mobilization, low-molecular-weight heparin, and mechanical prophylaxis beginning on postoperative 0 to 1 days. Conversely, bridging regimens were bimodal in distribution, with providers electing anticoagulant initiation on postoperative 0 to 1 days or days 5-6. Conclusion: This survey highlights the heterogeneity of spine care and accentuates geographical variations. Furthermore, it identifies the difficulty in providing consistent perioperative anticoagulation recommendations to patients, as there remains no widely accepted, definitive literature of evidence or guidelines.

ResumoA doação de tecidos ósseos e tendíneos é uma importante fonte de material biológico empregado em diversas técnicas cirúrgicas. A remoção destes tecidos gera uma importante sequela estética no doador, sendo necessária a reconstrução dos membros antes da devolução do corpo aos familiares. A técnica utilizada pelo Banco de Ossos do Paraná faz uso de vergalhões metálicos, tubos de silicone e abraçadeiras de náilon para reconstruir os membros, uma técnica de fácil execução e com resultado estético satisfatório.

STUDY DESIGN: Literature review. OBJECTIVE: The aim of this review is to describe the injuries associated with sacral fractures and to analyze their impact on patient outcome. METHODS: A comprehensive narrative review of the literature was performed to identify the injuries associated with sacral fractures. RESULTS: Sacral fractures are uncommon injuries that result from high-energy trauma, and that, due to their rarity, are frequently underdiagnosed and mistreated. Only 5% of sacral fractures occur in isolation. Injuries most often associated with sacral fractures include neurologic injuries (present in up to 50% of sacral fractures), pelvic ring disruptions, hip and lumbar spine fractures, active pelvic/ abdominal bleeding and the presence of an open fracture or significant soft tissue injury. Diagnosis of pelvic ring fractures and fractures extending to the lumbar spine are key factors for the appropriate management of sacral fractures. Importantly, associated systemic (cranial, thoracic, and abdominopelvic) or musculoskeletal injuries should be promptly assessed and addressed. These associated injuries often dictate the management and eventual outcome of sacral fractures and, therefore, any treatment algorithm should take them into consideration. CONCLUSIONS: Sacral fractures are complex in nature and often associated with other often-missed injuries. Study Design: Prospective survey-based study. Objectives: The AO Spine thoracolumbar injury classification has been shown to have good reproducibility among clinicians. However, the influence of spine surgeons’ clinical experience on fracture classification, stability assessment, and decision on management based on this classification has not been studied. Furthermore, the usefulness of varying imaging modalities including radiographs, computed tomography (CT) and magnetic resonance imaging (MRI) in the decision process was also studied. Methods: Forty-one spine surgeons from different regions, acquainted with the AOSpine classification system, were provided with 30 thoracolumbar fractures in a 3-step assessment: first radiographs, followed by CT and MRI. Surgeons classified the fracture, evaluated stability, chose management, and identified reasons for any changes. The surgeons were divided into 2 groups based on years of clinical experience as 10 years (n = 29). Results: There were no significant differences between the 2 groups in correctly classifying A1, B2, and C type fractures. Surgeons with less experience had more correct diagnosis in classifying A3 (47.2% vs 38.5% in step 1, 73.6% vs 60.3% in step 2 and 77.8% vs 65.5% in step 3), A4 (16.7% vs 24.1% in step 1, 72.9% vs 57.8% in step 2 and 70.8% vs 56.0% in step3) and B1 injuries (31.9% vs 20.7% in step 1, 41.7% vs 36.8% in step 2 and 38.9% vs 33.9% in step 3). In the assessment of fracture stability and decision on treatment, the less and more experienced surgeons performed equally. The selection of a particular treatment plan varied in all subtypes except in A1 and C type injuries. Conclusion: Surgeons’ experience did not significantly affect overall fracture classification, evaluating stability and planning the treatment. Surgeons with less experience had a higher percentage of correct classification in A3 and A4 injuries. Despite variations between them in classification, the assessment of overall stability and management decisions were similar between the 2 groups.

BACKGROUND Several classification systems exist for sacral fractures; however, these systems are primarily descriptive, are not uniformly used, have not been validated, and have not been associated with a treatment algorithm or prognosis. The goal of the present study was to demonstrate the reliability of the AOSpine Sacral Classification System among a group of international spine and trauma surgeons. METHODS A total of 38 sacral fractures were reviewed independently by 18 surgeons selected from an expert panel of AOSpine and AOTrauma members. Each case was graded by each surgeon on 2 separate occasions, 4 weeks apart. Intrarater reproducibility and interrater agreement were analyzed with use of the kappa statistic (κ) for fracture severity (i.e., A, B, and C) and fracture subtype (e.g., A1, A2, and A3). RESULTS Seventeen reviewers were included in the final analysis, and a total of 1,292 assessments were performed (646 assessments performed twice). Overall intrarater reproducibility was excellent (κ = 0.83) for fracture severity and substantial (κ = 0.71) for all fracture subtypes. When comparing fracture severity, overall interrater agreement was substantial (κ = 0.75), with the highest agreement for type-A fractures (κ = 0.95) and the lowest for type-C fractures (κ = 0.70). Overall interrater agreement was moderate (κ = 0.58) when comparing fracture subtype, with the highest agreement seen for A2 subtypes (κ = 0.81) and the lowest for A1 subtypes (κ = 0.20). CONCLUSIONS To our knowledge, the present study is the first to describe the reliability of the AOSpine Sacral Classification System among a worldwide group of expert spine and trauma surgeons, with substantial to excellent intrarater reproducibility and moderate to substantial interrater agreement for the majority of fracture subtypes. These results suggest that this classification system can be reliably applied to sacral injuries, providing an important step toward standardization of treatment.

ResumoObjetivoAvaliar a influência da injeção de células‐tronco mononucleares autólogas sobre as alterações histológicas do colágeno no ânulo fibroso do disco intervertebral após lesão experimental.MétodosForam submetidos 32 coelhos New Zealand a punção do discos intervertebrais lombares seguida de injeção intradiscal de células mononucleares provenientes da crista ilíaca versus injeção de solução salina nos seguintes períodos tempo: dois meses após a lesão (CT2M e SS2M), duas semanas (CT2S e SS2S), imediatamente após a lesão (CTCP e SSCP) e sem induzir a degeneração (CTSP e SSSP). Após dois meses da terapia celular, os animais foram submetidos a eutanásia e as alterações do colágeno nos discos intervertebrais foram avaliadas histologicamente.ResultadosHouve diferença estatisticamente significativa na CEAF entre os grupos CT2S e SS2S (p=0,018). Objective: Spine surgery with a minimally invasive lateral approach and validate possible anatomical differences between the right and left sides. Methods: Four measurements (cm) were taken on 38 cadavers: the distance between the lumbar plexus and the transverse process (L4-L5) and the distance between the lumbar plexus and the midline of the lumbar spine, both on the right and left sides. Results: The mean distance between the lumbar plexus and the transverse process of L4-L5 was 1.03 cm and the distance to the midline was 3.99 cm for the right side. Die optimale Behandlung von Verletzungen des thorakolumbalen Ubergangs basiert auf einem guten Verstandnis der Wirbelsaulenverletzung unter Berucksichtigung der Frakturmorphologie, der ligamentaren Integritat und der neurologischen Ausfallsymptomatik. Ein Klassifikationssystem hilft dabei, diese Analyse zu strukturieren und sollte einen Leitfaden fur die Behandlung offerieren. Die gebrauchlichen Klassifikationssysteme, wie z. B. die Magerl-Klassifikation oder die TLICS-Klassifikation sind entweder sehr komplex, berucksichtigen nicht den neurologischen Status oder vereinfachen die Frakturmorphologie als Indikator der Instabilitat zu sehr. Die „AOSpine Classification Group“ hat daher eine neue Klassifikation entwickelt, die sowohl auf der Magerl- als auch auf der TLICS-Klassifikation basiert, und die Schwachen dieser beiden Klassifikationen uberwinden soll. Dabei bietet sie auch eine Quantifizierung des Verletzungsschweregrads und lasst damit eine Vergleichbarkeit von Wirbelsaulenverletzungen zu. Wie in der bisherigen AO-Klassifikation unterscheidet sie dabei 3 Grundtypen der Verletzung: Typ-A-Verletzungen sind knocherne Kompressionsverletzungen der Wirbelsaule. Typ-B-Verletzungen zeigen Zerreisungen entweder der hinteren oder der vorderen Zuggurtung und Typ-C-Verletzungen sind translatorisch instabile Verletzungen. Nur Typ-A- und Typ-B-Verletzungen werden in weitere Untergruppen unterteilt. Eine begleitende neurologische Lasion wird vom transienten neurologischen Defizit bis zur kompletten Querschnittslahmung in 5 Schweregrade unterteilt. Weitere Modifikatoren berucksichtigen Erkrankungen, welche die Therapie masgeblich beeinflussen, z. B. eine signifikante Osteoporose oder eine ankylosierende Wirbelsaulenerkrankung. Bisherige Untersuchungen zur Intra- und Interbegutachterkonsistenz haben vielversprechende Ergebnisse gezeigt, sodass die Einfuhrung der AOSpine-Klassifikation fur thorakolumbale Verletzungen in einer deutschen Ubersetzung gerechtfertigt sinnvoll ist.

Objective : To evaluate the accuracy of free-hand pedicle screws placement at the seventh cervical vertebra. Methods : The authors have exposed the cervicothoracic junction of 9 adult cadavers (7 male and 2 female) preserved in formalin from the Faculty of Medicine of the Universidad Andina Néstor Cáceres Velásquez, city of Juliaca, Puno - Peru, locating the C7 vertebra based on anatomical parameters. According to previous publications, the entry point for the C7 pedicle was determined as 3-4mm lateral and 5-6mm superior to the center of the lateral mass, and the pedicle was drilled manually and instrumented with 3.5mm screws. After the screws placement, the C7 vertebrae were removed for radiographic analysis. Results : The authors were able to adequately locate the C7 entry point in 12 pedicles (66.6% accuracy), finding a great variability both laterally (2-5mm) and cranially (3-10mm). The angulation in the coronal plane was correct in 13 pedicles (72.3%), despite the incorrect location of the entry point. Angle values in the coronal plane ranged from 38 to 62 degrees. In the sagittal plane angulation, 2 screws were placed in the C6-C7 disc. The midtransversal diameter of the 18 pedicles ranged from 4 to 7mm. Conclusions : The location of the entry point for placement of C7 pedicle screws with pure free-hand technique is very variable due to anatomical differences and the authors recommend some type of guidance for increased safety and accuracy. Objetivo : Avaliar a acurácia do posicionamento de parafusos pediculares na sétima vértebra cervical pela técnica à mão livre. Métodos : Os autores dissecaram a junção cervicotorácica de nove cadáveres de adultos da Faculdade de Medicina da Universidad Andina Néstor Cáceres Velásquez, da cidade de Juliaca, Puno, Peru, localizando a sétima vértebra cervical com base em parâmetros anatômicos. De acordo com publicações prévias, o ponto de entrada do pedículo de C7 foi determinado como 3-4 mm lateral e 5-6 mm superior ao centro da massa lateral; o pedículo foi perfurado manualmente e instrumentado com parafusos de 3,5 mm. As vértebras foram removidas para avaliação radiográfica. Resultados : Os autores foram capazes de localizar precisamente o ponto de entrada para C7 em 12 pedículos (acurácia de 66,6%), encontrando grande variabilidade tanto no sentido lateral (2 a 5 mm) quanto no craniano (3 a 10 mm). A angulação no plano coronal foi correta em 13 pedículos (72,3%), apesar da localização incorreta do ponto de entrada. Os valores dos ângulos no plano coronal variaram de 38 a 62 graus. Na angulação do plano sagital, foram colocados dois parafusos no disco de C6-C7. O diâmetro mesotransversal dos 18 pedículos variou de 4 a 7 mm. Conclusão : A localização do ponto de entrada dos parafusos pediculares em C7 com a técnica à mão livre é muito variável em decorrência das diferenças anatômicas e os autores recomendam algum tipo de orientação auxiliar para aumentar a segurança e a acurácia. Objetivo : Evaluar la exactitud de la colocación de tornillos pediculares enla séptima vértebra cervical con la técnica de manos libres. Métodos : Los autores disecaron la unión cervicotorácica de 9 cadáveres adultos (7 hombres y 2 mujeres) conservados en formol de la Facultad de Medicina de la Universidad Andina Néstor Cáceres Velásquez de la ciudad de Juliaca, Puno - Perú, localizando la vértebra C7 con base en los parámetros anatómicos. De acuerdo con publicaciones anteriores, el punto de entrada para la colocación de tornillos pediculares en la vértebra C7 se determinó como 3-4 mm lateral y 5-6 mm superior al punto central de la masa lateral, y se procedió al drilado del pedículo manualmentey a la colocación de tornillos pediculares de 3,5 mm. Luego de la colocación de los tornillos las vértebras C7 fueron retiradas del cadáver para el análisis radiográfico. Resultados : Los autores fueron capaces de localizar adecuadamente el punto de entrada de C7 en 12 pedículos (66,6% de exactitud), se encontrando gran variabilidad en lo que se refiere tanto en la distancia hacia lateral (2-5 mm) como hacia craneal (3-10 mm). La angulación en el plano coronal estuvo correcta en 13 pedículos (72,3%), a pesar de la ubicación incorrecta del punto de entrada. Los valores de los ángulos en el plano coronal variaron de 38 a 62 grados. En la angulación del plano sagital, dos tornillos se colocaron en el disco C6-C7. El diámetro medio transversal de los 18 pedículos osciló entre 4 a 7 mm. Conclusiones : La localización del punto de entrada para la colocación de tornillos pediculares C7 con la técnica de manos libres es muy variable debido a las diferencias anatómicas. Los autores recomiendan utilizar algún tipo de guía para incrementar la precisión y seguridad.

Study Design. Empirical cross-sectional multicenter study.Objective. To identify the most commonly experienced problems by patients with traumatic spinal column injuries, excluding patients with complete paralysis.Summary of Background Data. There is no disease or condition-specific outcome instrument available that is designed or validated for patients with spine trauma, contributing to the present lack of consensus and ongoing controversies in the optimal treatment and evaluation of many types of spine injuries. Therefore, AOSpine Knowledge Forum Trauma started a project to develop such an instrument using the International Classification of Functioning, Disability and Health (ICF) as its basis.Methods. Patients with traumatic spinal column injuries, within 13 months after discharge from hospital were recruited from 9 trauma centers in 7 countries, representing 4 AOSpine International world regions. Health professionals collected the data using the general ICF Checklist. The responses were analyzed using frequency analysis. Possible differences between the world regions and also between the subgroups of potential modifiers were analyzed using descriptive statistics and Fisher exact test.Results. In total, 187 patients were enrolled. A total of 38 (29.7%) ICF categories were identified as relevant for at least 20% of the patients. Categories experienced as a difficulty/impairment were most frequently related to activities and participation (n = 15), followed by body functions (n = 6), and body structures (n = 5). Furthermore, 12 environmental factors were considered to be a facilitator in at least 20% of the patients.Conclusion. Of 128 ICF categories of the general ICF Checklist, 38 ICF categories were identified as relevant. Loss of functioning and limitations in daily living seem to be more relevant for patients with traumatic spinal column injuries rather than pain during this time frame. This study creates an evidence base to define a core set of ICF categories for outcome measurement in adult spine trauma patients. Level of Evidence: 4

The authors aim to evaluate the ability of spine surgeons to subjectively identify patients with psychological distress in a subset of lumbar fusion candidates, and the influence of such factors on surgical outcomes. From a cohort of 85 patients who had received a surgical indication for lumbar fusion and were subjectively evaluated for psychological distress, 60 were included in the study and underwent objective evaluation using the Distress Risk Assessment Method (DRAM) evaluation for depressive/distress symptoms, VAS and Oswestry scores pre- and postoperatively. Fifty-six patients were available with a minimum 6-month follow-up: 20 presented with normal DRAM scores, and 36 with abnormal DRAM (28 at risk; 4 distressed somatic; 4 distressed depressive). Although the group improved significantly with surgery regarding VAS and Oswestry, it was not the case for the DRAM score. The abnormal DRAM group had inferior VAS, Oswestry and satisfaction rates in comparison with the normal DRAM group. A significant number of patients in the at-risk group reduced their DRAM scores and were classified as normal patients at the end of the study. This study emphasizes the need for objective psychological screening on chronic low back pain patients and that although patients with abnormal DRAM scores benefit from surgery, they report inferior outcomes and satisfaction in comparison with the normal DRAM group.

Study Design Survey of spine surgeons. Objective To determine the reliability with which international spine surgeons identify a posterior ligamentous complex (PLC) injury in a patient with a compression-type vertebral body fracture (type A). Methods A survey was sent to all AOSpine members from the six AO regions of the world. The survey consisted of 10 cases of type A fractures (2 subtype A1, 2 subtype A2, 3 subtype A3, and 3 subtype A4 fractures) with appropriate imaging (plain radiographs, computed tomography, and/or magnetic resonance imaging), and the respondent was asked to identify fractures with a PLC disruption, as well as to indicate if the integrity of the PLC would affect their treatment recommendation. Results Five hundred twenty-nine spine surgeons from all six AO regions of the world completed the survey. The overall interobserver reliability in determining the integrity of the PLC was slight (kappa = 0.11). No substantial regional or experiential difference was identified in determining PLC integrity or its absence; however, a regional difference was identified ( p Conclusion The results of this survey indicate that there is only slight international reliability in determining the integrity of the PLC in type A fractures. Although the biomechanical importance of the PLC is not in doubt, the inability to reliably determine the integrity of the PLC may limit the utility of the M1 modifier in the AOSpine Thoracolumbar Spine Injury Classification System.

Study Design Literature review. Objective The aim of this review is to highlight challenges in the development of a comprehensive surgical algorithm to accompany the AOSpine Thoracolumbar Spine Injury Classification System. Methods A narrative review of the relevant spine trauma literature was undertaken with input from the multidisciplinary AOSpine International Trauma Knowledge Forum. Results The transitional areas of the spine, in particular the cervicothoracic junction, pose unique challenges. The upper thoracic vertebrae have a transitional anatomy with elements similar to the subaxial cervical spine. When treating these fractures, the surgeon must be aware of the instability due to the junctional location of these fractures. Additionally, although the narrow spinal canal makes neurologic injuries common, the small pedicles and the inability to perform an anterior exposure make decompression surgery challenging. Similarly, low lumbar fractures and fractures at the lumbosacral junction cannot always be treated in the same manner as fractures in the more cephalad thoracolumbar spine. Although the unique biomechanical environment of the low lumbar spine makes a progressive kyphotic deformity less likely because of the substantial lordosis normally present in the low lumbar spine, even a fracture leading to a neutral alignment may dramatically alter the patient's sagittal balance. Conclusion Although the new AOSpine Thoracolumbar Spine Injury Classification System was designed to be a comprehensive thoracolumbar classification, fractures at the cervicothoracic junction and the lumbosacral junction have properties unique to these junctional locations. The specific characteristics of injuries in these regions may alter the most appropriate treatment, and so surgeons must use clinical judgment to determine the optimal treatment of these complex fractures.

The aims of this study were (1) to demonstrate the AOSpine thoracolumbar spine injury classification system can be reliably applied by an international group of surgeons and (2) to delineate those injury types which are difficult for spine surgeons to classify reliably.A previously described classification system of thoracolumbar injuries which consists of a morphologic classification of the fracture, a grading system for the neurologic status and relevant patient-specific modifiers was applied to 25 cases by 100 spinal surgeons from across the world twice independently, in grading sessions 1 month apart. The results were analyzed for classification reliability using the Kappa coefficient (κ).The overall Kappa coefficient for all cases was 0.56, which represents moderate reliability. Kappa values describing interobserver agreement were 0.80 for type A injuries, 0.68 for type B injuries and 0.72 for type C injuries, all representing substantial reliability. The lowest level of agreement for specific subtypes was for fracture subtype A4 (Kappa = 0.19). Intraobserver analysis demonstrated overall average Kappa statistic for subtype grading of 0.68 also representing substantial reproducibility.In a worldwide sample of spinal surgeons without previous exposure to the recently described AOSpine Thoracolumbar Spine Injury Classification System, we demonstrated moderate interobserver and substantial intraobserver reliability. These results suggest that most spine surgeons can reliably apply this system to spine trauma patients as or more reliably than previously described systems.

STUDY DESIGN Reliability and agreement study, retrospective case series. OBJECTIVE To develop a widely accepted, comprehensive yet simple classification system with clinically acceptable intra- and interobserver reliability for use in both clinical practice and research. SUMMARY OF BACKGROUND DATA Although the Magerl classification and thoracolumbar injury classification system (TLICS) are both well-known schemes to describe thoracolumbar (TL) fractures, no TL injury classification system has achieved universal international adoption. This lack of consensus limits communication between clinicians and researchers complicating the study of these injuries and the development of treatment algorithms. METHODS A simple and reproducible classification system of TL injuries was developed using a structured international consensus process. This classification system consists of a morphologic classification of the fracture, a grading system for the neurological status, and description of relevant patient-specific modifiers. Forty cases with a broad range of injuries were classified independently twice by group members 1 month apart and analyzed for classification reliability using the Kappa coefficient (κ). RESULTS The morphologic classification is based on 3 main injury patterns: type A (compression), type B (tension band disruption), and type C (displacement/translation) injuries. Reliability in the identification of a morphologic injury type was substantial (κ= 0.72). CONCLUSION The AOSpine TL injury classification system is clinically relevant according to the consensus agreement of our international team of spine trauma experts. Final evaluation data showed reasonable reliability and accuracy, but further clinical validation of the proposed system requires prospective observational data collection documenting use of the classification system, therapeutic decision making, and clinical follow-up evaluation by a large number of surgeons from different countries.
