16. Comparing the First and Second Wave of COVID-19 in Kidney Transplant Recipients: An East-European Perspective.

Abstract

Background: The present study examined whether patient characteristics, management, and outcome of kidney transplant recipients (KTx) with COVID-19 changed in the second versus the first pandemic wave. Methods: We reviewed all available data (demographics, medical history, comorbidities, therapeutic interventions, and outcome) on our KTx with COVID-19 during the first wave (March–September 2020, n = 33) and the second wave (October 2020–February 2021, n = 149) of the COVID-19 pandemic. Results: One hundred eighty-two out of our 1,503 KTx in active follow-up got COVID-19 during 12-month period, corresponding to a prevalence of 12.1%. No difference was found in age, gender distribution, comorbidities, body mass index, or baseline immunosuppression between the 2 COVID-19 waves. Bilateral COVID pneumonia was more frequent during the first wave. More KTx were managed as outpatients during the second wave (15 vs. 39%, p < 0.01). Calcineurin inhibitors were more sparingly reduced during the second wave, whereas antimetabolites were similarly reduced (91 vs. 86, p = ns). Admission to intensive care units was comparable between the first (27%) and second waves (23%). During the first wave, 8 out of 9 patients (89%) requiring intensive care died, whereas the mortality of the ICU patients in the second wave was 68% (23 deaths) (p = 0.2). The overall mortality was 24% during the first wave and 16% during the second wave (p = 0.21), while in-hospital mortality was identical between the CO-VID-19 waves (27%). Increasing age and poor allograft function were significant predictors of mortality. Conclusions: Most patient characteristics and outcome were comparable between the first 2 COVID-19 waves. Anaplasma phagocytophilumand spotted fever group Rickettsiaare obligate intracellular Gram-negative tick-borne bacteria, among which several may cause clinical infections in humans. Several Rickettsiaspp. and A. phagocytophilumare transmitted in Europe by Ixodes ricinus, the most common tick species feeding on humans in this area. The aim of this study was to evaluate the annual prevalence of Rickettsiaspp. and A. phagocytophilumin I. ricinuscollected from humans during three consecutive years.

26. Real-time PCR-based identification of Borrelia burgdorferisensu lato species in ticks collected from humans in Romania

Abstract

The aims of our study were to determine (i) which tick species bite humans in Romania and (ii) the prevalence of Borrelia (B.) burgdorferigenospecies in these ticks. All ticks collected from patients who presented to the Clinic of Infectious Diseases Cluj Napoca in spring/summer 2010 were morphologically identified by an entomologist and tested for B. burgdorferigenospecies prevalence by a real-time PCR assay targeting the hbbgene and melting curve analysis. Out of 532 ticks, 518 were Ixodes ricinus, 10 Dermacentor marginatus, and 3 Haemaphysalisspp. ticks, and one unidentified tick due to destruction. Since evaluation of the hbbPCR revealed that it was not possible to differentiate between B. spielmanii/B. valaisianaand B. garinii/B. bavariensis, sequencing of an 800-bp fragment of the ospAgene was performed in these cases. Out of 389 investigated ticks, 43 were positive by hbbPCR for B. burgdorferisensu lato. The positive samples were 42 Ixodes ricinus(11.1% B. burgdorferisensu lato prevalence) and the one unidentified tick. Species identification revealed the presence of mainly B. afzelii, but also of B. garinii, B. burgdorferisensu stricto, B. valaisiana, and B. lusitaniae. In 4 samples, differentiation between B. spielmanii/B. valaisianawas impossible. Our study shows that the most relevant human pathogenic B. burgdorferigenospecies – predominantly B. afzelii– are present in ticks collected from Romanian patients.

Abstract

Generalized tuberculosis (characterized by the presence in several organs of small tuberculous granulations surrounded by normal tissue) is becoming a more and more seldom disease, having an unpredictable evolution and prognosis. The diagnosis is often difficult. We present the case of a young woman, 29 years old, in which the diagnosis established in a late stage could have been fatal.
