Abstract

Background: Data on the prognostic role of the TRI-SCORE in patients undergoing transcatheter tricuspid valve intervention (TTVI) are limited. Objectives: The aim of this study was to evaluate the performance of the TRI-SCORE in predicting outcomes of patients undergoing TTVI. Methods: TriValve (Transcatheter Tricuspid Valve Therapies) is a large multicenter multinational registry including patients undergoing TTVI. The TRI-SCORE is a risk model recently proposed to predict in-hospital mortality after tricuspid valve surgery. The TriValve population was stratified based on the TRI-SCORE tertiles. The outcomes of interest were all-cause death and all-cause death or heart failure hospitalization. Procedural complications and changes in NYHA functional class were also reported. Results: Among the 634 patients included, 223 patients (35.2%) had a TRI-SCORE between 0 and 5, 221 (34.8%) had 6 or 7, and 190 (30%) had ≥8 points. Postprocedural blood transfusion, acute kidney injury, new atrial fibrillation, and in-hospital mortality were more frequent in the highest TRI-SCORE tertile. Postprocedure length of stay increased with a TRI-SCORE increase. A TRI-SCORE ≥8 was associated with an increased risk of 30-day all-cause mortality and all-cause mortality and the composite endpoint assessed at a median follow-up of 186 days (OR: 3.00; 95% CI: 1.38-6.55; HR: 2.17; 95% CI: 1.78-4.13; HR: 2.08, 95% CI: 1.57-2.74, respectively) even after adjustment for procedural success and EuroSCORE II or Society of Thoracic Surgeons Predicted Risk of Mortality. The NYHA functional class improved across all TRI-SCORE values. Conclusions: In the TriValve registry, the TRI-SCORE has a suboptimal performance in predicting clinical outcomes. However, a TRISCORE ≥8 is associated with an increased risk of clinical events and a lack of prognostic benefit after successful TTVI.

Abstract

In this work, we perform molecular dynamics simulations to investigate the structural properties of water at the interface with K-feldspar. We aim at understanding the modifications induced on water structure by the presence of this mineral. We analysed both the hydration layer of K-feldspar in supercooled water and the premelting layer at the interface between K-feldspar and hexagonal ice. We studied the density profile, the orientational tetrahedral order parameter, the distribution of the $ \cos (\gamma) $ cos ⁡ (γ) function, the hydrogen bond distribution, the number of water molecules belonging to different phases close to the surface of the feldspar. We focused on the interplay between high-density water and low-density water to inquire on local modifications of water structure with respect to the bulk phase. We investigated four different pressures where the structure of bulk water spans from high-density liquid (locally more disordered) to low-density liquid (locally more ordered). The investigation showed that both the hydration and the premelting layers present a low-density liquid water component and similarities as a function of pressure with the corresponding bulk. We also find that the premelting layer shows structural features intermediate between the liquid and the crystalline state acting as a matching layer. [ABSTRACT FROM AUTHOR]

Abstract

Background: Acute myocarditis (AM) is thought to be a rare cardiovascular complication of COVID-19, although minimal data are available beyond case reports. We aim to report the prevalence, baseline characteristics, in-hospital management, and outcomes for patients with COVID-19-associated AM on the basis of a retrospective cohort from 23 hospitals in the United States and Europe. Methods: A total of 112 patients with suspected AM from 56 963 hospitalized patients with COVID-19 were evaluated between February 1, 2020, and April 30, 2021. Inclusion criteria were hospitalization for COVID-19 and a diagnosis of AM on the basis of endomyocardial biopsy or increased troponin level plus typical signs of AM on cardiac magnetic resonance imaging. We identified 97 patients with possible AM, and among them, 54 patients with definite/probable AM supported by endomyocardial biopsy in 17 (31.5%) patients or magnetic resonance imaging in 50 (92.6%). We analyzed patient characteristics, treatments, and outcomes among all COVID-19-associated AM. Results: AM prevalence among hospitalized patients with COVID-19 was 2.4 per 1000 hospitalizations considering definite/probable and 4.1 per 1000 considering also possible AM. The median age of definite/probable cases was 38 years, and 38.9% were female. On admission, chest pain and dyspnea were the most frequent symptoms (55.5% and 53.7%, respectively). Thirty-one cases (57.4%) occurred in the absence of COVID-19-associated pneumonia. Twenty-one (38.9%) had a fulminant presentation requiring inotropic support or temporary mechanical circulatory support. The composite of in-hospital mortality or temporary mechanical circulatory support occurred in 20.4%. At 120 days, estimated mortality was 6.6%, 15.1% in patients with associated pneumonia versus 0% in patients without pneumonia (P=0.044). Abstract

The increasing number of patients with heart failure HF and comorbidities is due to aging population and increase of life expectancy of patients with cardiovascular disease. Encouraging results derived by recent trials may suggest some comorbidities as new targets for new drugs, highlighting the need for a better understanding of the comorbidities’ effects in HF patients and the need of a multidisciplinary approach for the management of chronic HF with comorbidities. We report a brief review about main cardiovascular and non-cardiovascular comorbidities in HF patients in order to update physicians and researchers engaged in the HF research or in “fight against heart failure.”

Abstract

Trehalose aqueous solutions are relevant in many technological applications, for example in cryopreservation of biomolecules. It is known that the presence of this disaccharide is able to slow down the dynamics of nearby water molecules and to modify their spatial rearrangement, nevertheless, a complete understanding of the properties of water-trehalose solutions and of trehalose cryoprotective properties is still lacking. Here we discuss recent molecular dynamics simulation results of water-trehalose solutions, performed at different temperatures upon cooling, and we compare the results with the behavior of the bulk phase. In particular we focus on the dynamical properties of hydration water, i.e., the water molecules in the hydration shell of the disaccharide. Hydration water shows a sub-diffusive behavior with respect to bulk water, the same structural relaxation typical of glass formers liquids, albeit slightly slower than in the bulk, and an additional relaxation process at longer timescales completely absent in the bulk. The analysis of hydrogen bond autocorrelation functions allows to connect the structural and long relaxation processes of hydration water to the dynamics of two distinct population of hydrogen bonds in the system, the water-water and water-trehalose hydrogen bonds.

Abstract

The longitudinal viscosity of diluted water-tert-butyl alcohol solutions in the 10 GHz frequency region has been measured by means of Brillouin light scattering and inelastic ultraviolet scattering. The main advantage of our hypersonic investigation compared to more traditional ultrasonic measurements is that in the gigahertz frequency range slow relaxation processes involving the alcohol dynamics are completely unrelaxed, so that the measured viscosity mainly originates from the hydrogen bond restructuring of water. In contrast with previous determinations, we estimate an activation energy which is independent from the alcohol mole fraction up to X = 0.1, and comparable to that of bulk water. A simple two-component model is used to describe the steep increase of viscosity with increasing alcohol mole fraction, and a retardation factor 1.7 ± 0.2 is found between the relaxation times of hydration and bulk water. These findings endorse a dynamic scenario where the slowing down of hydration water is mainly due to a reduction of configurational entropy and does not involve an arrested, icelike, dynamics. Abstract

Molecular dynamics and structural properties of water-tert-butyl alcohol (TBA) mixtures are studied as a function of concentration by extended depolarized light scattering (EDLS) experiments. The wide frequency range, going from fraction to several thousand GHz, explored by EDLS allows distinguishing TBA rotational dynamics from structural relaxation of water and intermolecular vibrational and librational modes of the solution. Contributions to the water relaxation originating from two distinct populations, i.e. hydration and bulk water, are clearly identified. The dynamic retardation factor of hydration water with respect to the bulk, ? ? 4, almost concentration independent, is one of the smallest found by EDLS among a variety of systems of different nature and complexity. This result, together with the small number of water molecules perturbed by the presence of TBA, supports the idea that hydrophobic simple molecules are less effective than hydrophilic and more complex molecules in perturbing the H-bond network of liquid water. At increasing TBA concentrations the average number of perturbed water molecules shows a pronounced decrease and the characteristic frequency of librational motions reduces significantly, both of which are results consistent with the occurrence of self-aggregation of TBA molecules. Abstract

Cities are increasingly becoming a focal point for economies and societies at large, particularly because of on-going urbanisation and the trend towards increasingly knowledge-intensive economies. Cities need to change and develop, but this change needs to be achieved in a smart way, making cities "smart cities". Information & Communication Technologies (ICT) present new opportunities for Smart WAter Networks (SWAN) as a key asset for Smart Cities. The Ctrl+SWAN (Cloud Technologies & ReaL time monitoring+Smart WAter Network) Action Group (AG) was created within the European Innovation Partnership on Water, in order to promote innovation in the water sector by advancing existing solutions. The integration of smart meters and biosensors with cloud computing and innovative software allows the development of new network management schemes. A more effective monitoring in terms of quantity and quality variables makes partitioning of large water networks possible, thus improving customer service, quality of maintenance and protection from accidental or intentional contamination. The vision of the AG consists of treating the water network not as a traditional system with devices trivially added, but as a SWAN featuring new applications that result in optimal management and protection. After providing the state of the art on the best technologies already available on the market, new perspectives for SWAN monitoring, partitioning and protection with innovative sensors are proposed. Each AG partner describes methodologies, best practices, novel devices and possible integrations among meters, biosensors, models and optimization algorithms.

Abstract

L'obiettivo di questo lavoro è quello di sviluppare un nuovo sistema real time per il monitoraggio di aree marine ad alto rischio di inquinamento, per l'immediato intervento e l'analisi della colonna d'acqua prima e dopo eventuali sversamenti. Ad oggi tali inquinanti sono individuati tramite reti di sensori su boe e ship survey. Le reti di sensori garantiscono un monitoraggio statico di aree marine prospicienti la boa: se lo sversamento avviene poco distante, e le correnti allontanano da essa l'inquinante, la rete non rileva la presenza di tali sostanze. Il secondo metodo implica costi maggiori, l'impiego prolungato di mezzi e personale e analisi svolte in un periodo ristretto di tempo: è quindi una soluzione non adatta al monitoraggio di aree a rischio. Per ottemperare a tali esigenze si è individuata nei Veicoli Autonomi Subacquei (AUV) la piattaforma più idonea da utilizzare. Essi offrono la possibilità di monitorare l'ambiente marino in particolari condizioni (es. nelle ore notturne), quando altre metodiche (es. le registrazioni satellitari) non sono applicabili. L'AUV può inoltre essere utilizzato per eseguire analisi dello stato delle acque in aree estese, tramite survey studiate ad hoc per la specifica area di interesse, grazie alla presenza di diversi tipi di sensore comprendenti un sistema, basato sul naso elettronico, in grado di rilevare la presenza di componenti organiche volatili (VOC) idrocarburiche sopra la superficie dell'acqua.
