Abstract

Patients with B-lymphoid malignancies have been consistently identified as a population at high risk of severe COVID-19. Whether this is exclusively due to cancer-related deficits in humoral and cellular immunity, or whether risk of severe COVID-19 is increased by anticancer therapy, is uncertain. Using data derived from the COVID-19 and Cancer Consortium (CCC19), we show that patients treated for B-lymphoid malignancies have an increased risk of severe COVID-19 compared with control populations of patients with non-B-lymphoid malignancies. Among patients with B-lymphoid malignancies, those who received anticancer therapy within 12 months of COVID-19 diagnosis experienced increased COVID-19 severity compared with patients with non-recently treated B-lymphoid malignancies, after adjustment for cancer status and several other prognostic factors. Our findings suggest that patients recently treated for a B-lymphoid malignancy are at uniquely high risk for severe COVID-19.SignificanceOur study suggests that recent therapy for a B-lymphoid malignancy is an independent risk factor for COVID-19 severity. These findings provide rationale to develop mitigation strategies targeted at the uniquely high-risk population of patients with recently treated B-lymphoid malignancies. Abstract

BackgroundOlder age is associated with poorer outcomes of SARS-CoV-2 infection, although the heterogeneity of ageing results in some older adults being at greater risk than others. The objective of this study was to quantify the association of a novel geriatric risk index, comprising age, modified Charlson comorbidity index, and Eastern Cooperative Oncology Group performance status, with COVID-19 severity and 30-day mortality among older adults with cancer.MethodsIn this cohort study, we enrolled patients aged 60 years and older with a current or previous cancer diagnosis (excluding those with non-invasive cancers and premalignant or non-malignant conditions) and a current or previous laboratory-confirmed COVID-19 diagnosis who reported to the COVID-19 and Cancer Consortium (CCC19) multinational, multicentre, registry between March 17, 2020, and June 6, 2021. Patients were also excluded for unknown age, missing data resulting in unknown geriatric risk measure, inadequate data quality, or incomplete follow-up resulting in unknown COVID-19 severity. The exposure of interest was the CCC19 geriatric risk index. The primary outcome was COVID-19 severity and the secondary outcome was 30-day all-cause mortality; both were assessed in the full dataset. Adjusted odds ratios (ORs) and 95% CIs were estimated from ordinal and binary logistic regression models.Findings5671 patients with cancer and COVID-19 were included in the analysis. Median follow-up time was 56 days (IQR 22-120), and median age was 72 years (IQR 66-79). The CCC19 geriatric risk index identified 2365 (41·7%) patients as standard risk, 2217 (39·1%) patients as intermediate risk, and 1089 (19·2%) as high risk. 36 (0·6%) patients were excluded due to non-calculable geriatric risk index. Compared with standard-risk patients, high-risk patients had significantly higher COVID-19 severity (adjusted OR 7·24; 95% CI 6·20-8·45). 920 (16·2%) of 5671 patients died within 30 days of a COVID-19 diagnosis, including 161 (6·8%) of 2365 standard-risk patients, 409 (18·5%) of 2217 intermediate-risk patients, and 350 (32·1%) of 1089 high-risk patients. High-risk patients had higher adjusted odds of 30-day mortality (adjusted OR 10·7; 95% CI 8·54-13·5) than standard-risk patients.InterpretationThe CCC19 geriatric risk index was strongly associated with COVID-19 severity and 30-day mortality. Our CCC19 geriatric risk index, based on readily available clinical factors, might provide clinicians with an easy-to-use risk stratification method to identify older adults most at risk for severe COVID-19 as well as mortality.FundingUS National Institutes of Health National Cancer Institute Cancer Center.

18. Grappling with Professional Ethics in Instructional Technology by Participating in an Online Service-Learning Course

Abstract

In this qualitative study, we engaged in a narrative inquiry to examine what graduate students in an online service-learning course grappled with while learning about professional ethics in instructional technology. This study took place in a service-learning partnership between community partners, students, and the course instructor. Students in the course worked collaboratively to address a design problem identified by community partners. Our research question was: In an online graduate-level instructional technology service-learning course, what do participants grapple with when learning about professional ethics through shared experiences? Eighteen students volunteered to participate in this study from three iterations of the course. Data included student narratives shared in synchronous and asynchronous discussions as well as written reflections. Narrative data was analyzed with the constant comparative method (Charmaz in Constructing grounded theory, SAGE Publications, Thousand Oaks, 2014; Corbin & Strauss in Basics of qualitative research: Techniques and procedures for developing grounded theory, SAGE Publications, Thousand Oaks, 2014) using NVivo 12, a qualitative research data analysis tool. The findings from this study indicated that, while engaging in authentic ethical problem-solving within the context of an online service-learning course, participants grappled with ethical challenges, leadership, reflecting on experiences, legal issues, and social responsibility.

Abstract

In this article, we studied net neutrality as a complex sociocultural phenomenon that can affect the works of distance education scholars and online learners. We decided to take part in this research because many distance education scholars and learners take net neutrality for granted. We engaged in a qualitative investigation of US public documents and explored the effects of net neutrality and public digital access, which can influence learner engagement with online educational materials. We focused on identifying tensions in the discourse about net neutrality, especially surrounding the Federal Communication Commission's (FCC) 2015 Open Internet Order. Our research question was: How do the complexities involved in the net neutrality debate in the United States affect public access to online information and services, and what implications does this hold for online learning? We relied on activity systems as an analytical framework for making sense of net neutrality.

34. The App Farm: Engaging Design Process as a Means for French Learning

Abstract

In this design case, we present an instructional design project that resulted in a French language learning game development system. What we describe here is not the game itself, but rather the pedagogical intervention that created what the design team termed a mobile "application farm," which in turn produced the game. The term "farm" was used due to the perspective of the designers to create a sustainable system rather than a single design. This design case also has another purpose beyond presenting precedent in instructional design; we interrogate the interview process and the protocol we used while documenting this project. We present the outcome of our interrogation in reflection notes posted throughout this design case. We do this because we believe that there is a value and need for design cases created by someone outside of the design arena but with access to members of the design team, in order to elicit where precedents might be found within complex designs.

42. Enacting Collective Design Intentions in an Online Graduate Level Introductory Instructional Technology Course

Abstract

This design case will introduce how collective design intentions shared by a group of three program faculty for an online Instructional Technology (IT) Master's program at the University of Tennessee (UT) were collaboratively identified and further acted upon within the context of the first course in the program. The course that is the focus of this case is "IT521 Proseminar 1: Instructional Technology as a Profession" in which we explored collective design intentions. The article begins with an introduction of the collective design intentions that program faculty shared. Then the article introduces how the first author enacted those design intentions in IT521 while working closely with the second author. The purpose for sharing this case is to document critical decisions that were made by one faculty member about a course within the context of shared design intentions for the program. The article ends with a discussion of lessons learned about communicating collective and personal design intentions to future designers who may be involved in similar situations.
