Infection with the hepatitis C virus (HCV) has adverse liver, kidney, and cardiovascular consequences in patients with chronic kidney disease (CKD), including those on dialysis therapy or with a kidney transplant. Since the publication of the Kidney Disease: Improving Global Outcomes (KDIGO) HCV Guideline in 2018, advances in HCV management, particularly in the field of antiviral therapy and treatment of HCV-associated glomerular diseases, coupled with increased usage of HCV-positive kidney grafts, have prompted a reexamination of the 2018 guideline. As a result, the Work Group performed a comprehensive review and revised the 2018 guidance. This Executive Summary highlights key aspects of the updated guideline recommendations for 3 chapters: Chapter 2: Treatment of HCV infection in patients with CKD; Chapter 4: Management of HCV-infected patients before and after kidney transplantation; and Chapter 5: Diagnosis and management of kidney diseases associated with HCV infection.

A evolução da recorrência da hepatie C pós-transplante hepático pode ter um curso bastante variável. Raramente a doença pode progredir para uma forma conhecida como hepatite recorrente colestática grave, cuja patogenia ainda não é bem conhecida. Nós estudamos nesse trabalho alguns aspectos virológicos, histológicos e imunohistoquímicos de seis pacientes com essa forma rara de recorrência da doença, tendo como comparação um grupo pareado de seis pacientes transplantados com a forma leve de hepatite C recorrente, e como controle imunocompetente, cinco pacientes não transplantados com hepatite crônica pelo vírus C. Foram avaliados como possíveis fatores preditivos de gravidade da progressão da recorrência: viremia do VHC, evolução de quasispécies, parâmetros histopatológicos, e imunoreatividade para o antígeno core do VHC. Following liver transplantation (OLT) HCV-related disease severity is highly variable, with a minority of cases progressing to an extremely severe form of cholestatic hepatitis, in which the pathogenesis is not yet understood. We aim to compare virological, histological and immunohistological changes in patients developing mild and severe post-OLT HCV recurrence. Twelve patients with recurrent HCV infection were studied (6 with severe and 6 with mild disease). Five HCV-infected immunocompetent patients were used as controls. We looked at viral load, quasispecies evolution of HCV, several histological parameters and immuno-reactivity of core antigens at three time-points (pre-OLT, early post-OLT and late post-OLT) as predictors of severity of recurrence post-OLT.

Introdução: A doença hepática criptogênica é aquela cuja etiologia não pode ser explicada por achados clínicos, histológicos, laboratoriais e de imagem. Sua frequência é estimada em 5,4% da população com hepatopatia crônica. A deficiência de lipase ácida lisossomal (LAL-D) é uma doença rara, na qual os indivíduos acometidos cursam com ausência ou redução significativa da atividade da enzima lipase ácida lisossomal (LAL). Essa alteração enzimática causa uma desregulação no metabolismo lipídico, acarretando acúmulo de ésteres de colesterol e triglicérides dentro dos lisossomos, levando ao quadro de aterosclerose precoce, dislipidemia, esteatose hepática com progressão para cirrose e disfunção orgânica. Objetivos: Determinar se LAL-D pode ser uma causa de doença hepática criptogênica; caracterizar as populações de pacientes com LAL-D e com doença hepática criptogênica e avaliar, nessa última população, os pacientes que se enquadram na nova sugestão de nomenclatura, de doença hepática gordurosa associada à disfunção metabólica (MAFLD). Métodos: Estudo observacional e descritivo, baseado na ferramenta STROBE. As coletas de dados e de amostras de sangue foram realizadas no período de 2016 a 2018. Os critérios de inclusão foram: a) Idade acima de 18 anos; b) diagnóstico de hepatopatia criptogênica; c) em acompanhamento ambulatorial ou submetido a transplante hepático; e d) concordância do paciente em participar do estudo. Os critérios de exclusão foram: a) pacientes com outras etiologias da doença hepática; b) pacientes sem investigação completa das causas conhecidas de hepatopatia; e c) pacientes cuja análise do exame de LAL-D foi feita por outro método que não o dried blood spot (DBS). Foi utilizado material coletado de amostra de sangue periférico, pipetado em DBS. A dosagem da atividade da enzima LAL foi realizada utilizando-se um inibidor altamente específico da LAL, o Lalistat 2, em um dos dois laboratórios de referência do estudo. As amostras foram encaminhadas, aleatoriamente, para análise nos seguintes laboratórios: Seattle Childrens Hospital (Estados Unidos) e Afip Medicina Laboratorial (Brasil), cuja análise do exame é realizada no Laboratório de Erros Inatos do Metabolismo (LEIM). Resultados: Foram incluídos 295 pacientes no estudo, com doença hepática criptogênica. Após critérios de exclusão, 135 pacientes foram selecionados para o estudo, sendo que três desses (2,2%) apresentaram dosagem de LAL abaixo do valor de referência, compatível com LAL-D. A média de idade desses pacientes foi de 43,9 ± 10,1 anos, sendo 2 pacientes do sexo feminino. O índice de massa corporal médio foi de 23,1 ± 1,7 kg/m² e a glicemia média foi de 89,7 ± 3,2 mg/dl. As médias para colesterol total, HDL-c, LDL-c e triglicérides, foram, respectivamente, de 165,6 ± 32,1 mg/dl, 21,7 ± 3,2 mg/dl, 105 ± 33,9 mg/dl e 206,7 ± 25,5 mg/dl. Conclusões: Apesar da LAL-D ser uma doença rara e subdiagnosticada, deve-se considerar a sua investigação nos indivíduos com doença hepática sem etiologia definida e, principalmente, nos pacientes com IMC normal, com HDL-c reduzido e triglicérides elevados Introduction: Cryptogenic liver disease is one whose etiology cannot be explained by clinical, histological, laboratory and imaging findings. Its frequency is estimated at 5.4% of the population with chronic liver disease. Lysosomal acid lipase deficiency (LAL-D) is a rare disease in which the affected individuals have an absence or significant reduction in the activity of the lysosomal acid lipase (LAL) enzyme. This enzymatic deficiency causes a dysregulation in the lipid metabolism, causing accumulation of cholesterol esters and triglycerides within the lysosomes and may result in early atherosclerosis, dyslipidemia, hepatic steatosis with progression to cirrhosis and organ dysfunction. Aims: To determine if LAL-D may be a cause of cryptogenic liver disease; characterize the populations of patients with LAL-D and with cryptogenic liver disease and evaluate, in the latter population, patients who fit the new nomenclature suggestion of metabolic dysfunction-associated fatty liver disease (MAFLD). Methods: Observational and descriptive study, based on the STROBE. Data collection and blood samples were carried out from 2016 to 2018. Inclusion criteria were: a) age over 18 years; b) diagnosis of cryptogenic liver disease; c) outpatient follow-up or undergoing liver transplantation; and d) patient\'s agreement to participate in the study. Exclusion criteria were: a) patients with other etiologies of liver disease; b) patients without a complete investigation of the known causes of liver disease; and c) patients whose LAL-D examination analysis was performed using a method other than dried blood spot (DBS). The material was collected from a peripheral blood sample and pipetted in DBS. The measurement of LAL enzyme activity was performed using a highly specific LAL inhibitor, Lalistat 2, in one of the two reference laboratories in the study. The samples were randomly sent for analysis in the following laboratories: Seattle Children\'s Hospital (United States) and Afip Medicina Laboratorial (Brazil), whose examination is performed at the Laboratory of Inborn Errors of Metabolism (LEIM). Results: A total of 295 patients were included in the study, with cryptogenic liver disease. After exclusion criteria, 135 patients were selected for the study, and three of these (2.2%) had LAL dosage below the reference value, compatible with LAL-D. The mean age of these patients was 43.9 ± 10.1 years, 2 female patients. The mean body mass index was 23.1 ± 1.7 kg/m² and the mean blood glucose was 89.7 ± 3.2 mg/dL. The means for total cholesterol, HDL-c, LDL-c and triglycerides were respectively 165.6 ± 32.1 mg/dL, 21.7 ± 3.2 mg/dl, 105 ± 33.9 mg/dL and 206.7 ± 25.5 mg/dL. Conclusions: Although LAL-D is a rare and underdiagnosed disease, its investigation should be considered in individuals with liver disease without defined etiology and especially in patients with a normal BMI, with low HDL-c and high triglycerides

Introdução: Alguns estudos de imunogenicidade da vacina anti-HBV em pacientes com infecção crônica pelo vírus da hepatite C (VHC) demonstraram resposta anti-HBs diminuída, variando de 63,6% a 72,9%, em comparação com 90,9% a 93,9% em controles sadios. Objetivos: Avaliar a resposta antiHBs, com dose dupla comparando com a dose padrão da vacinação VHB e, em não respondedores administrar uma quarta dose de vacina (Butang®, Instituto Butantã, São Paulo) em pacientes VHC crônicos virgens de tratamento sem cirrose. Métodos: 141 adultos com VHC crônico sem cirrose foram randomizados em 2 grupos: A (dose dupla: 40 µg aos 0, 1 e 6 meses) ou B (dose padrão: 20 µg aos 0, 1 e 6 meses) para receber vacinação contra o VHB. A resposta à vacinação foi medida por títulos de anti-HBs 1 mês após a última dose de vacina VHB. 70 controles sadios (grupo C) foram também avaliados (anticorpos negativos para anti-HCV, anti-HBc, AgHBs e anti-HBs) que receberam dose padrão (20 µg em intervalos de 0, 1 e 6 meses). A resposta de vacina (soroconversão) foi definida por anti-HBs 10 U/L. Pacientes VHC crônicos que não respondem à imunização primária receberam a quarta dose de vacina de acordo com o grupo que foram previamente randomizados. Resultados: 128 pacientes (60 dose dupla, 68 dose padrão) completaram o estudo, sem abandono devido a eventos adversos. Os pacientes que concluíram tinham idade mediana de 52 anos, 61% mulheres, 52% brancos, 60% fibrose < 2 e 75% genótipo 1 com mediana HCV RNA de 6log10. A taxa global de soroconversão global foi de 76,7% (95% CI 65-87) na dose dupla e de 73,5% (95% CI 63-84) na dose-padrão e, em comparação, com 91,2% (95% IC 84-99) em controles sadios (p=0,02 e 0,008, respectivamente). Entre os pacientes infectados com VHC, a soroconversão não foi significativamente diferente na dose dupla versus dose-padrão (p = 0,69). Entre os respondedores, a média de títulos anti-HBs na dose dupla e na dose padrão dos grupos vacinais não foram significativamente diferentes dos controles (205 vs 432 UI/ L, p = 0,66). No modelo de regressão logística avaliando a associação entre os grupos vacinais (dose dupla vs padrão) e a soroconversão anti-HBs,o esquema vacinal não foi um preditor independente de resposta (OR = 0,85, p = 0,69). Entre os 25 não respondedores, após a administração da quarta dose, 8 soroconverteram (IC95% 13,2-52,9) e as taxas de soroconversão para os grupos de dose dupla e dose padrão foram 45,5% (n=5, IC 95% CI 16-74.9) e 21,4% (n=3, IC 95% 0.0-42,9) respectivamente (p = 0,19). Na análise multivariada, após controle de possíveis fatores de confusão, incluindo idade, sexo, etnia e genótipo do VHC, apenas a idade avançada (OR = 0,93, p = 0,0004), etnia não branca (OR = 0,34, p = 0,0245) e genótipo 1 do VHC (vs outros) (OR = 0,14, p = 0,0059 ) estavam associados com uma menor probabilidade de resposta anti-HBs em pacientes infectados com VHC. A mediana de idade após a primeira dose da vacina não mostrou diferença entre os pacientes que soroconverteram e os que falharam a imunização. Conclusões: Em pacientes infectados com VHC, sem cirrose, a resposta à vacinação contra o VHB é significantemente comprometida e esta resposta reduzida não pode ser superada pela utilização da dose dupla da vacina. Adicionar uma quarta dose da vacina para os não respondedores pode ser uma estratégia efetiva e justifica outros estudos. Outras medidas adjuvantes são necessárias para aperfeiçoar as taxas de soroconversão neste grupo de pacientes altamente susceptíveis Background and Aims: Some immunogenicity studies of anti-HBV vaccine in patients with chronic HCV infection have demonstrated a diminished anti-HBs response ranging from 63.6% to 72.9%, compared to 90.9% to 93.9% in healthy controls. Aims: To evaluate the anti-HBs response to double compared to standard dose HBV vaccination and in non-responders to administer the fourth dose of vaccine (Butang®; Butantã Institute, São Paulo) in treatment naive chronic HCV patients without cirrhosis. Methods: 141 adults with chronic HCV without cirrhosis were randomized in 2 groups A (double dose: 40µg at 0, 1 and 6 months) or B (standard dose :20µg at 0, 1 and 6 months) to receive HBV vaccination. Response to vaccination was measured by titers of anti-HBs at 1 month after the last dose of HBV vaccine. 70 healthy controls (group C) were also evaluated (negative to anti-HCV, anti-HBc, HBsAg and anti-HBs antibodies) who received standard dosing (20µg at intervals of 0, 1 and 6 months). Vaccine response (seroconversion) was defined by anti-HBs 10 U/L. HCV chronic patients who did not respond to primary immunization received the fourth dose of vaccine according to the group that were previously randomized. Results: 128 patients (60 double dose, 68 standard dose) completed the study, with no dropouts due to adverse events. The patients who completed were of median age 52 years, 61% female, 52% white, 60% fibrosis < 2, and 75% genotype 1 with median 6log10 HCV RNA. The overall seroconversion rate was 76.7% (95% CI 65-87) in double dose and 73.5% (95% CI 63-84) in standard dose groups compared to 91.2% (95% CI 84-99) in non-HCV controls (p=0.02 and 0.008, respectively). Among HCV infected patients, seroconversion were not significantly different in the double dose vs standard dose HBV vaccination regimen (p=0.69). Among responders, median anti-HBs titers in the double dose and standard dose vaccine groups were not significantly different from controls (205 vs 432 UI/L, p=0.66). In a logistic regression model evaluating the association between vaccine group (double dose vs standard dose) and anti-HBs seroconversion, vaccine regimen was not an independent predictor of response (OR=0.85, p=0.69). Among 25 HCV non responders after the administration of the fourth dose, 8 seroconverted (95%CI 13.2-52.9) and seroconversion rate for double and standard dose groups were 45.5% (n=5, 95% CI 16-74.9) and 21.4% (n=3, 95% CI. 0.0-42.9) respectively (p=0.19). In multivariate analysis, controlling for potential confounders including age, sex, ethnicity, and HCV genotype, only older age (OR=0.93, p=0.0004), non-white ethnicity (OR=0.34, p=0.0245), and HCV genotype 1 (vs others) (OR=0.14, p=0.0059) were associated with a decreased likelihood of anti-HBs response in HCV-infected patients. The median age after the first dose of the vaccine showed no difference between patients who seroconverted and failed to immunization. Conclusions: In HCV-infected patients without cirrhosis, responses to HBV vaccination are significantly impaired and this reduced response cannot be overcome by the use of double dose HBV vaccination. Adding a fourth dose of vaccine for non-responders may be an effective strategy and warrants further study. Other adjuvant measures are needed to enhance seroconversion rates in this high need group of patients

Introdução: A hepatite C crônica tem sido considerada doença de grande impacto em saúde pública, devido às altas taxas de morbimortalidade a ela associadas assim como aos elevados custos associados às suas complicações hepáticas ou extra-hepáticas. Várias estratégias de enfrentamento dessa doença têm sido propostas, com foco na prevenção, diagnóstico precoce e tratamento eficaz. As elevadas taxas de resposta virológica sustentada com os antivirais de ação direta trouxeram o conceito de potencial de eliminação do VHC pelo tratamento. Objetivo: descrever o perfil demográfico e clínico de uma coorte de pacientes com hepatite C crônica acompanhados em um serviço de complexidade terciária do Brasil e analisar: a prevalência de comorbidades e manifestações extra-hepáticas (MEH) a ela associadas; o grau de complexidade clínica dos casos analisados; a associação entre doença hepática avançada e algumas variáveis demográficas e clínicas; a associação entre presença de complexidade clínica e algumas variáveis selecionadas. Materiais e métodos: Trata-se de um estudo observacional transversal retrospectivo, de uma coorte de pacientes com hepatite C crônica, com idade acima de 18 anos, atendidos no ambulatório de doenças infecciosas de um serviço de referência terciário do Brasil, no período de janeiro de 2014 a outubro de 2016. Os dados analisados foram obtidos por meio de um banco de dados estruturado da instituição, criado para esse fim. As variáveis demográficas e epidemiológicas analisadas foram idade, etnia, gênero, mecanismos de exposição e hábitos de vida. As variáveis clínicas analisadas foram genótipo e grau de fibrose hepática. As MEH, as comorbidades e algumas coinfecções também foram analisadas. A complexidade dos pacientes foi definida pela presença de pelo menos uma das seguintes condições: Presença de fibrose hepática avançada (Metavir F3 ou F4); diagnóstico de hepatocarcinoma (HCC); presença de 3 ou mais comorbidades associadas ou não ao VHC e/ou MEH. As características qualitativas foram descritas por meio do uso de frequências absolutas e relativas e as idades e número de comorbidades foram descritas por meio do uso de medidas resumo (média, desvio padrão, mediana, mínimo e máximo). As MEH e as comorbidades associadas e não associadas ao VHC que se apresentaram com frequência acima de 1% na análise descritiva foram incluídas na análise bivariada. Foi investigada a existência de associação com fibrose hepática avançada e complexidade clínica por meio do uso de testes qui-quadrado ou testes exatos (teste exato de Fisher ou teste da razão de verossimilhanças). Foram estimados modelos de regressão logística múltipla com método de seleção stepwise backward para fibrose avançada e complexidade, selecionando-se as variáveis que nos testes bivariados apresentaram níveis de significância inferiores a 0,20 (p < 0,20) mantendo-se nos modelos finais apenas as variáveis com nível de significância inferior a 0,05 (p < 0,05). Para realização das análises foi utilizado o software IBM-SPSS for Windows versão 22.0 e para tabulação dos dados foi utilizado o software Microsoft Excel 2003. Resultados: 1574 pacientes foram incluídos. Entre eles, 401 (25,4%) não apresentavam nenhuma comorbidade ou MEH no momento do estudo. MEH, comorbidades associadas e não associadas ao VHC foram identificadas em 123 (7,83%), 574 (36,4%) e 916 (58,2%) pacientes respectivamente. Entre as MEH destacou-se a artrite reumatóide, sendo observada em 33 casos (2,09%). Entre as comorbidades associadas ao VHC destacaram-se a síndrome metabólica (n = 161; 10,2%), DM (n = 130; 8,3%) e depressão (n = 147; 9,3%). Dentre as comorbidades não associadas ao VHC destacaram-se as afecções cardiovasculares (n = 662; 42,1%), psiquiátricas (n = 78; 5%) e neurológicas (n = 66; 4,2%). Apenas 674 (42,8%) pacientes foram considerados complexos. Entre estes, 22,2% e 1,8% apresentavam cirrose hepática e HCC, respectivamente. Pela análise de regressão logística, estiveram independentemente associados à presença de fibrose hepática avançada: gênero masculino (p = 0,003), idade > 40 anos (p < 0,001), número de comorbidades (p < 0,001) e depressão (p = 0,010). Somente idade > 40 anos esteve associada à presença de complexidade clínica por meio de análise multivariada. Conclusões: Neste estudo, a presença de MEH e comorbidades associadas ou não ao VHC foi alta. Apesar disso, menos da metade dos pacientes foi considerada complexa. Acreditamos que as informações aqui descritas: Reforçam a necessidade de um plano de eliminação dessa doença em nível nacional, levando-se em consideração as altas taxas de RVS e a alta morbidade a ela associada no que tange às suas complicações clínicas a médio e longo prazo, na ausência de tratamento específico; Reforçam a necessidade de um delineamento das estratégias de atendimento aos pacientes com diagnóstico de hepatite C, no âmbito do SUS, levando-se em consideração as diferentes necessidades desses pacientes e seus graus de complexidade Introduction: Chronic hepatitis C virus infection has been considered a disease of great impact on public health due to the high rates of mortality and morbidity associated with it, as well as the high costs associated with the management of its hepatic and extrahepatic complications. Several strategies to deal with this disease have been proposed focusing on prevention, early diagnosis and effective treatment. Direct-acting antivirals (DAA) modified the natural history of hepatitis C, offering the possibility of viral elimination through treatment due to their high rates of sustained virological responses. Objectives: Our objective was to describe the demographic and clinical profile of a cohort of patients with chronic hepatitis C treated at a tertiary center in Brazil and analyze: 1-the prevalence of comorbidities and extrahepatic manifestations (EHM) associated with the infection; 2-the degree of clinical complexity of the cases analyzed; 3-the associations between advanced liver disease and demographic and clinical variables; 4- the association between clinical complexity and additional variables. Material and methods: This is a retrospective, cross-sectional observational study of a cohort of patients with chronic hepatitis C, aged over 18 years, treated at a tertiary reference service of infectious disease in Brazil, between January 2014 and October 2016. Data were collected from medical records and compiled in a structured database. We analyzed demographic and epidemiological variables: age, ethnicity, gender, exposure mechanisms and lifestyle habits. The clinical variables related to HCV were genotype and degree of liver fibrosis. Extrahepatic manifestations, comorbidities and coinfections were also analyzed. Complexity was defined by one the three conditions described as follows: 1-presence of advanced liver fibrosis (Metavir score F3 or F4); 2-presence of hepatocarcinoma; 3-presence of 3 or more comorbidities, whether or not directly associated with HCV and/or extrahepatic manifestations. We initially conducted an analysis of the frequency distribution for each selected variable. The qualitative characteristics of the patients were described using absolute and relative frequencies and the age and number of comorbidities were described using summary measures (mean, standard deviation, median, minimum and maximum). Extrahepatic manifestations and comorbidities that comprised more than one percent of the total number of reported extrahepatic manifestations and comorbidities were included in the bivariate analysis. The association between advanced liver fibrosis and clinical complexity was studied through chi-square tests or exact tests (Fisher´s exact tests ou likelihood ratio test). Two tailed p-values were calculated and considered statistically significant if < 0.05. Variables with p values less than 0.20 on bivariate analysis were selected for multivariate analysis. Multiple logistic regression analysis was performed to examine the association of variables with advanced liver disease and complexity. Statistical analyses were performed using the IBM-SPSS for Windows version 22.0 software. Results: 1574 patients were analyzed. Among all patients, 401 (25,4%) did not have any comorbidity or extrahepatic manifestation. Extrahepatic manifestations and comorbidities either associated with or independent of HCV were found in 123 (7,83%), 574 (36,4%) and 916 (58,2%) patients, respectively. Rheumatoid arthritis was the most prevalent EHM, with 33 cases (2,09%). Metabolic syndrome, diabetes mellitus and depression were the most prevalent comorbidities associated with HCV, 161 (10,2%), 130 (8,3%) and 147 (9,3%) respectively. Cardiovascular, psychiatric and neurological conditions were the most prevalent comorbidities not associated with HCV, 662 (42,1%), 78 (5%) and 66 (4,2%), respectively. Only 674 (42.8%) patients were considered clinically complex. Among these, 22.2% had liver cirrhosis and 1.8% had hepatocarcinoma. According to multivariate analysis, male gender (p = 0.003), age> 40 years (p < 0.001), number of comorbidities (p < 0.001) and depression (p = 0.010) were independently associated with the presence of advanced liver fibrosis. Only age > 40 years was associated with the presence of clinical complexity according to multivariate analysis. Conclusions: The presence of EHM and comorbidities associated with independent of HCV infection was high. Despite this, less than half of the patients treated, at this tertiary service were considered to be clinically complex. In conclusion, our findings reinforce:1-the necessity of a plan of elimination of HCV at a national level taking into account the high SVR and high morbidity associated with its complications without specific treatment; 2-the need for an improved delineation of patient care strategies with HCV in SUS, considering the heterogenicity of cases and their complexity

Introdução: O conceito atual sobre a coagulopatia na cirrose indica que existe um reequilíbrio da hemostasia com tendência à hipercoagulabilidade plasmática. A infecção bacteriana pode romper este reequilíbrio, pois promove alterações na coagulação e induz à liberação endotelial de heparinoides endógenos. Entretanto, o impacto desta condição na geração de trombina é desconhecido. O objetivo do estudo foi avaliar o efeito da infecção bacteriana na geração de trombina em pacientes com cirrose. Métodos: pacientes com cirrose e infecção bacteriana (grupo infectado) foram incluídos até 24h, após o início da terapia antibiótica e reavaliados, no mínimo, 5 dias após o término do tratamento. Pacientes com cirrose descompensada e sem infecção (grupo não infectado) também foram incluídos e reavaliados, sem intervenção entre as coletas. O desfecho principal foi o impacto da infecção bacteriana no parâmetro da geração de trombina (GT), ETP com trombomodulina (TM). TM é um ativador da proteína C adicionado ao ensaio para reproduzir as condições in vivo. Todos os parâmetros da GT (com e sem TM) também foram analisados. Os ensaios ROTEM, INTEM e HEPTEM (modificados com heparinase), foram utilizados na avaliação do efeito heparinoide endógeno. Foram realizados ensaios de anti-Xa, proteína C (PC), antitrombina(AT), RNI / TP e TTPa. Todos os resultados foram comparados em cada grupo entre os tempos de avaliação. Resultados: foram incluidos e reavaliados 36 pacientes com cirrose infectados e 28 com cirrose sem infecção. Os valores do ETP TM durante a infecção foram significativamente maiores, após a resolução da infecção (de 1.145,4 ± 360,7 nmol / L * min a 958,1 ± 254,8 nmol / L * min, p = 0,005). A ETP razão (com / sem TM) permaneceu praticamente inalterada (0,78 ± 0,15 vs 0,77 ± 0,09, p = 0,89). A frequência de GT normal a elevada no parâmetro ETP TM foi a mesma durante e após a resolução da infecção (97,2%) e no parâmetro ETP a razão foi idêntica (respectivamente, 97,2% e 100%). O efeito heparinoide foi visto somente nos pacientes com infecção, com duração do CTINTEM significativamente maior que a CTHEPTEM (p = 0,004) neste grupo. Este efeito desapareceu, após a resolução da infecção (p = 0,75). As deficiências de PC e AT foram significativamente mais graves nos pacientes infectados (p < 0,01). RNI / TP, aPTT que pioraram na infecção ativa (p < 0,05). No grupo não infectado, nenhum destes parâmetros apresentou diferença significante entre a inclusão e a reavaliação. Conclusão: Nos pacientes com cirrose e infecção bacteriana, a quantidade total de trombina gerada é maior que a observada após a resolução da infecção, e está associada a um agravamento na deficiência de PC e AT. A despeito do efeito heparinoide endógeno durante a infecção na cirrose, a hipercoagulabilidade plasmática encontra-se preservada e não pôde ser avaliada pelos testes de coagulação convencionais Background and aims: The current concept of coagulopathy in cirrhosis indicates that there is a rebalancing of hemostasis with a tendency to plasma hypercoagulability. Bacterial infection can disrupt this rebalancing, as it promotes changes in coagulation and releases endothelial heparinoids. However, the effect of this condition on the thrombin generation is unknown. Our aim was to assess the effect of bacterial infection on thrombin generation in patients with cirrhosis. Methods: patients with cirrhosis and bacterial infection (infected group) were evaluated within 24 hours after start antibiotic therapy and at least 5 days after infection resolution. Patients with decompensated cirrhosis and not infected (not infected group) were also enrolled and reevaluated, without any intervention between evaluation times. Primary endpoint was the effect of bacterial infection on thrombin generation (TG) parameter ETP with TM (ETP TM). TM is a protein C activator added to mimic in vivo conditions. All TG parameters (with and without TM) were also analysed. ROTEM assays, INTEM and HEPTEM (heparinase-modified), was performed to evaluate the endogenous heparinoids effect. Anti-Xa, protein C (PC), antithrombin (AT), RNI/TP and TTPa assays were performed. All results were compared within each group between evaluation times. Results: we included and reevaluated 36 cirrhotic infected patients and 28 cirrhotics not infected. ETP TM values in infected cirrhotics were significantly higher than after resolution of infection (from 1145.4 ± 360.7 nmol/L * min to 958.1 ± 254.8 nmol / L * min, p = 0.005). ETP ratio (with/without TM) remained practically unchanged (0,78±0,15 vs 0,77 ± 0,09, p = 0,89). Frequency of normal or high TG during infection and after resolution of infection was the same by the ETP TM (97,2%) and was identical by the ETP ratio (respectively, 97,2% and 100%). A heparinoid effect was found only in infected cirrhotics, with CTINTEM duration significantly longer than CT HEPTEM (p = 0.004). This effect disappeared after resolution of infection (p = 0.75). PC and AT deficiencies were significantly more severe in infected patients (p < 0,01). RNI/TP, aPTT was worsen at active infection (p < 0.05). None of these parameters exhibited a significant difference between inclusion and reavaluation times in not infected group. Conclusion: patients with cirrhosis exhibits significant higher amount of thrombin generation during bacterial infection and it is associated with reduction of PC and AT levels. Despite the endogenous heparinoid effect during infection in cirrhosis, plasma hypercoagulability is preserved and cannot be assessed by conventional coagulation tests

Introdução: Apesar dos conhecimentos crescentes a respeito dos polimorfismos dos genes TM6SF2 e PNPLA3, a sua influência no espectro da da doença hepática crônica pelo vírus da hepatite C (HCV) ainda não está totalmente definida. Além disso, populações miscigenadas, como a brasileira, não foram incluídas na maioria dos estudos. O objetivo desde estudo foi descrever a prevalência destes polimorfismos em brasileiros com hepatite C crônica e avaliar a sua associação com fibrose hepática e outros componentes da síndrome metabólica. Métodos: Estudo transversal e unicêntrico que avaliou 365 pacientes com hepatite C crônica, virgens de tratamento, e 134 indivíduos saudáveis. Os polimorfismos do PNPLA3 (rs738409 c.444C>G) e do TM6SF2 (rs58542926 c.449C>T) foram avaliados. Resultados: Entre os saudáveis, as frequências do TM6SF2 CC e CT + TT foram de 93.28% e 6.72%, respectivamente; estas foram 88.95 % e 11.05% em pacientes com o HCV. Em relação ao PNPLA3, dentre os saudáveis, as frequências de CC e CG + GG foram 49.25% e 50.74%, respectivamente; estas foram 51.37% e 48.63% entre os pacientes com HCV. Em pacientes com hepatite C, pela análise univariada, os genótipos TM6SF2 CT + TT foram associados com fibrose hepática significativa (p=0,0471, OR 1,953, IC95% 1,009-3,788) em comparação com genótipo CC, enquanto este foi associado com idade mais avançada (p=0,0024), maior frequência de hipertensão arterial sistêmica (p=0,0327), obesidade (p=0,0309), síndrome metabólica (p=0,0141) e níveis mais elevados de colesterol total (p=0.0360), em comparação aos genótipos CT + TT. Portadores de VHC com o PNPLA3 GG tiveram menor IMC (p=0,0469) e menor frequência de síndrome metabólica (p=0,0137) do que os indivíduos CG + CC. Nenhum dos polimorfismos foi associado de forma independente com esteatose hepática ou fibrose. Conclusões: A presença dos genótipos TM6SF2 CT + TT foi associada com fibrose hepática significativa em pacientes com HCV, o mesmo não sendo observado para os polimorfismos do PNPLA3 Introduction: Despite the growing body of knowledge about TM6SF2 and PNPLA3 polymorphisms, their influence in the spectrum of HCV liver disease is not yet fully defined. Moreover, admixed populations, such as Brazilians, were not included in most studies. The aim of this study was to describe the prevalence of these polymorphisms in Brazilians with chronic hepatitis C, and to assess their association with liver fibrosis and other components of the metabolic syndrome. Methods: This single center cross-sectional study enrolled 365 patients with chronic hepatitis C, naïve for antiviral therapy, and 134 healthy individuals. PNPLA3 (rs738409 c.444C>G) and TM6SF2 (rs58542926 c.449C>T) polymorphisms were evaluated. PNPLA3 GG subjects had lower BMI (p=0,0469) and less metabolic syndrome (p=0.0137) than CG / CC individuals. None of the polymorphisms were independently associated with hepatic steatosis or fibrosis. Conclusions: The presence of the TM6SF2 CT + TT genotype was associated with significant liver fibrosis in patients with HCV, but not the PNPLA3 polymorphisms

A Doença hepática gordurosa não alcoólica (DHGNA) é uma das formas mais comuns de doença hepática, relacionada primordialmente ao aumento progressivo da obesidade no mundo. Cerca de 10-25% dos pacientes com DHGNA desenvolvem Esteato-hepatite não alcoólica (EHNA) e os fatores responsáveis pela progressão de esteatose para EHNA ainda não estão totalmente elucidados. Antecedentes genéticos, microbiota intestinal (MI), fatores ambientais, tais como, sedentarismo e dieta inadequada, devem ser considerados fatores críticos para o desenvolvimento de distúrbios metabólicos, como obesidade, síndrome metabólica, diabetes e DHGNA. O objetivo do presente estudo foi comparar a MI de indivíduos com sobrepeso, obesos e magros com e sem EHNA a fim de delinear diferenças fenotípicas entre os grupos. Foi realizado um estudo transversal que incluiu 13 pacientes com EHNA comprovada por biópsia (4 magros com EHNA, 5 com sobrepeso e com EHNA e 4 obesos com EHNA) e 10 indivíduos (magros e obesos) sem DHGNA utilizados como controle (5 magros e 5 obesos) estratificados de acordo com o índice de massa corpórea (IMC). O DNA da MI foi extraído de amostras de fezes. A amplificação por reação em cadeia da polimerase (PCR) foi realizada utilizando primers para a região V4 do gene 16S rRNA. A amplificação de sequências alvo foi realizada utilizando oligonucleotídeos modificados e o sequenciamento foi executado na plataforma Ion PGM Torrent. Os dados foram analisados pelo software QIIME. O consumo de macronutrientes foi analisado por um recordatório alimentar de 7 dias. Os pacientes com EHNA mostraram diferenças significativas na quantidade de Faecalibacterium, Ruminococcus, Lactobacillus e Bifidobacterium em comparação aos indivíduos (magros e obesos) sem DHGNA utilizados como controle. Em particular, os pacientes com EHNA magros apresentaram uma quantidade 3 vezes menor de Faecalibacterium e Ruminococcus (p=0,004), enquanto os pacientes com EHNA obesos apresentaram quantidades abundantes de Lactobacillus (p= 0,048) e pacientes com EHNA com sobrepeso apresentaram quantidades reduzidas de Bifidobacterium (p= 0,041). Além disso, os pacientes com EHNA magros mostraram redução na quantidade de Ruminococcus e Lactobacillus em comparação aos pacientes com EHNA com sobrepeso e obesidade. No entanto, a análise da alfa diversidade da MI não demonstrou diferença estatística entre os pacientes com EHNA magros quando comparados aos indivíduos sem DHGNA utilizados como controle (p= 0,265). Índices de fibrose maior ou igual a 2 foram associados ao aumento de Lactobacillus (p= 0,015). Não houve diferenças na ingestão calórica e lipídica entre os grupos. Nossos dados sugerem que os pacientes com EHNA magros têm composição diferente da MI em comparação aos pacientes com sobrepeso e, ou também, obesos e aos indivíduos sem DHGNA. Escores de fibrose maior ou igual a 2 também foram associados à composição da MI; mas a ingestão de macronutrientes e calorias não foi associada às diferenças específicas na composição dos microrganismos intestinais fecais Nonalcoholic fatty liver disease (NAFLD) is one of the most common types of liver disease, primarily related to the progressive increase of obesity throughout the world. Despite the knowledge of NAFLD pathogenesis, the reason why some patients developed NASH with fibrosis and the progression of liver disease still remain unclear. It has been reported that intestinal dysbiosis plays an important role in metabolic disorders such as obesity, metabolic syndrome, diabetes and cardiovascular disease. About 10-25% of patients with NAFLD develop nonalcoholic steatohepatitis (NASH) and the factors responsible for the progression from steatosis to NASH are not clear yet. Genetic background, gut microbiota, environmental factors, such as sedentary lifestyle and inadequate diet, should be considered critical factors for the development of metabolic disorders such as obesity, metabolic, diabetes and also NAFLD. The goal of the present study was to compare the gut microbiome of lean, overweight and obese patients with or without NASH, in order to outline phenotypic differences. We performed a cross sectional study comprising 13 NASH patients with liver biopsy (4 lean, 5 overweight and 4 obese) and 10 individuals (5 lean and 5 obese) without NAFLD which were used as stratified controls according to their BMI. The Microbiome DNA was extracted from stool samples and PCR amplification was performed using primers for the V4 region of the 16S rRNA gene. The enlargement of sequenced targets was accomplished using modified oligonucleotides, the sequencing was executed on the Ion PGM Torrent platform and data analyzed using QIIME software. The macronutrients consumption was analyzed by a 7-day food intake record. Patients with NASH showed significant differences in the quantity of Faecalibacterium, Ruminococcus, Lactobacillus and Bifidobacterium compared to the individuals (lean and obese) without NAFLD used as controls. In particular, when compared to controls, lean NASH patients possessed a quantity 3 times fewer of Faecalibacterium and Ruminococcus (p=0.004), obese NASH patients showed an abundant microbiome in Lactobacillus (p=0.048). Overweight NASH patients had fewer quantities of Bifidobacterium (p=0.041). Moreover, lean patients with NASH showed a reduction in the quantity of Ruminococcus and Lactobacillus compared to overweight and obese NASH patients. However, the analysis of microbiome alpha diversitydid not show statistic difference between lean NASH patients and controls without NAFLD (p=0.265). A fibrosis score greater than or equal to 2 was associated with the increase of Lactobacillus (p=0.015). There were no differences in caloric and lipid intake among groups. Our data suggests that lean NASH patients have different microbiome composition compared to their overweight/obese counter part\'s. Hepatic fibrosis score greater or equal to 2 in NASH patients had association with their microbiome composition. On the other hand, macronutrients intake and calories were not linked to specific differences in composition of the gut stool microorganisms

Introducao: A infeccao pelo virus da hepatite C (HCV) e um importante problema de saude publica e tem sido associada a um consideravel risco de mortalidade. Assim, o objetivo deste estudo e investigar a contribuicao da infeccao pelo HCV na mortalidade por todas as causas e por causas hepaticas, em uma grande coorte de doadores de sangue no Brasil. Metodos: Trata-se de um estudo de coorte retrospectiva de doadores de sangue, no periodo de 1994 a 2016, na Fundacao Pro-Sangue - Hemocentro de Sao Paulo (FPS). Esta coorte incluiu 2.892 e 5.784 doadores soropositivos e soronegativos para anticorpos do HCV, respectivamente. Os registros da base de dados da FPS e do Sistema de Informacoes sobre Mortalidade (SIM) foram vinculados por meio de um relacionamento probabilistico de registros. Os desfechos de mortalidade analisados foram definidos com base nos codigos da CID-10 (Classificacao Internacional de Doencas e Problemas Relacionados a Saude) listados como a causa da morte na declaracao de obito. Hazard Ratios (HRs) foram estimados para os resultados usando modelos de regressao multipla de Cox. Resultados: Quando se consideraram todas as causas de morte, identificaram-se 209 mortes entre doadores de sangue soropositivos e 190 entre doadores de sangue soronegativos. Os doadores soropositivos para infeccao pelo HCV tiveram um risco 2,15 vezes maior de morte por todas as causas (IC 95%: 1,76-2,62; p < 0,001). Quando consideradas apenas as causas hepaticas de morte, identificaram-se 73 mortes entre doadores de sangue soropositivos e apenas 6 entre doadores de sangue soronegativos. Os doadores soropositivos para infeccao pelo HCV tiveram um risco 23,41 vezes maior de morte por causas relacionadas ao figado (IC 95%: 10,15-53,94; p < 0,001). Os doadores soropositivos para HCV tiveram 29,52 (IC 95%: 3,93-221,73), 2,75 (IC 95%: 1,38-5,47) e 1,86 (IC 95%: 1,17-2,97) vezes maior risco de morte por carcinoma hepatocelular, infeccao e trauma, respectivamente, em comparacao com doadores soronegativos nesta coorte. Em relacao aos individuos coinfectados, HCV-HIV e HCV-HTLV, o risco de obito por todas as causas foi ainda maior, correspondendo a um HR de 14,57 (IC 95%: 8,42-25,22) e 3,65 (IC 95%: 1,93-6,91), respectivamente. Conclusoes: Nesta coorte retrospectiva, a taxa de mortalidade relacionada a todas as causas e as causas hepaticas foi maior entre doadores de sangue soropositivos para o HCV em comparacao com a taxa de mortalidade entre doadores de sangue soronegativos. Alem disso, aqueles individuos coinfectados, HCV-HIV e HCV-HTLV, apresentaram risco de obito ainda maior. Nossos dados confirmam o HCV como uma causa relevante de mortes no Brasil e tambem sugerem que intervencoes direcionadas ao seguimento destes pacientes, mesmo apos o acesso ao tratamento medicamentoso especifico, sao urgentes e necessarias Introduction: Hepatitis C virus (HCV) infection is a major health problem associated with considerable risk of mortality in different regions of the world. The purpose of this study was to investigate the contribution of HCV infection on all-cause and liver-related mortality, in a large cohort of blood donors in Brazil. Methods: This is a retrospective cohort study of blood donors from 1994 to 2013, at Fundação Pró-Sangue - Hemocentro de São Paulo (FPS). This cohort included 2,892 and 5,784 HCV antibody seropositive and seronegative donors, respectively. Records from the FPS database and the Mortality Information System (SIM: a national database in Brazil) were linked through a probabilistic record linkage (RL). Mortality outcomes were defined based on ICD-10 (10th International Statistical Classification of Diseases and Related Health Problems) codes listed as the cause of death on the death certificate. Hazard ratios (HRs) were estimated for outcomes using Cox multiple regression models. Results: When all causes of death were considered, RL identified 209 deaths among seropositive blood donors and 190 among seronegative blood donors. Donors seropositive for HCV infection had a 2.15 times higher risk of death due to all causes (95% CI: 1.76-2.62; p < 0.001). When only liver-related causes of death were considered, RL identified 73 deaths among seropositive blood donors and only 6 among seronegative blood donors. Donors seropositive for HCV infection had a 23.4 times higher risk of death due to liver related causes (95% CI: 10.2- 53.9; p < 0.001). Donors seropositive for HCV had a 29.5 (95% CI: 3.9-221.7), 2.8 (95% CI: 1.4-5.5) and a 1.9 (95% CI: 1.2-3.0) times higher risk of death due to hepatocellular carcinoma, infection or trauma, respectively compared to seronegative donors. Regarding co-infection, HCV-HIV and HCV-HTLV, the risk of death from all causes was even higher, corresponding to a HR of 14.57 (95% CI: 8.42-25.22) and 3.65 (95% CI: 1.93- 6.91), respectively. Conclusions: In this retrospective cohort, all-cause and liver-related mortality was highest among blood donors seropositive for HCV compared with the mortality rate among seronegative blood donors. In addition, those co-infected individuals, HCV-HIV and HCV-HTLV, presented an even greater risk of death. Our data confirms HCV as a relevant cause of death in Brazil and also suggest that interventions directed at following patients even after access to specific drug treatment are urgent and necessary

INTRODUÇÃO: O Transplante de fígado evoluiu nas últimas décadas, sempre em busca de melhorar a sobrevida do paciente e do enxerto. Importante causa de morbi-mortalidade é a disfunção precoce do enxerto (DPE) e o não funcionamento primário do enxerto (NFP). Diversos biomarcadores vem sendo estudados, porém ainda não há um consenso. Com isso tivemos a hipótese científica de avaliar e quantificar a função hepática avaliada pele verde de indocianina (VI) após o transplante de fígado. OBJETIVO: Avaliar a disfunção precoce do enxerto pela taxa de depuração plasmática do (VI) no pós-operatório imediato de transplante hepático. MÉTODO: Estudo clinico, de julho de 2014 a junho de 2015, prospectivo e observacional. Um total de 40 pacientes fizeram parte desta análise pela pulso-densitometria, usando o sistema de Limon (Impulse Medical System, Munique, Alemanha). Foram avaliados também o índice de risco de doadores (DRI), os critérios de Wagener e de Olthoff e preditores prognósticos pós-transplante de fígado. Todos os testes realizados levaram em consideração um alfa bidirecional de 0,05 e intervalo de confiança (IC) de 95% e foram realizados com apoio computacional dos softwares IBM SPSS 25 (Statistical Package for the Social Sciences) e Excel 2016® (Microsoft Office). RESULTADOS: Um total de 40 pacientes foram avaliados. A idade média foi de 53 anos e a maioria do sexo masculino (70%). A etiologia da cirrose mais comum foi hepatite por vírus C (42,5%). Os pacientes eram Child C em 45% dos casos. A taxa de retenção o verde de indocianina em 15 minutos (R15) permaneceu aumentada nos dias 1 e 3 de pós operatório ( > 10%) e normalizou no 7º dia de pós operatório ( < 10%). A taxa de depuração manteve valores normais, com 18,5% no 1º dia; 20,3 no 3º e 20,4 no 7º dia pós operatório. A comparação com os critérios de Olthoff e Wagener não mostrou diferença estatística (p=0,467 e p=0,178). Na comparação com DRI > 1,5 encontrou-se p=0,066, e com desfecho negativo (Perda do enxerto ou óbito) em p=0,063. A depuração do verde de indocianina mostrou relação significativa com o grau de lesão histológica pós isquemia e reperfusão (p=0,030). CONCLUSÃO: A reserva funcional hepática apresenta-se diminuída no pós operatório recente de transplante de fígado com melhora ao final da primeira semana. A depuração hepática do verde de indocianina não relaciona-se com a disfunção precoce do enxerto avaliada pelos critérios de Oltoff e Wagener. Por outro lado ela tem uma relação significativa inversamente proporcional ao grau da lesão hepática pós isquemia e reperfusão INTRODUCTION: Liver transplantation has evolved in the last decades, alway seeking to improve patient and graft survival. Important cause of morbidity and mortality is early graft dysfunction (EGD) and primary non-graft function (NGF). Several biomarkers have been studied, but there is still no consensus. With this we had the scientific hypothesis to evaluate and quantify the hepatic function evaluated by indocyanine green (IG) after liver transplantation. OBJECTIVE: To evaluate the early graft dysfunction by the plasma clearance rate of (IG) in the immediate postoperative period of liver transplantation. METHOD: Clinical study, from July 2014 to June 2015, prospective and observational. A total of 40 patients were part of this analysis by pulse-densitometry, using the Limon system (Impulse Medical System, Munich, Germany). Donor risk index (DRI), Wagener and Olthoff criteria, and prognostic predictors after liver transplantation were also evaluated. All the tests performed into account a bidirectional Alpha of 0.05 and a 95% confidence interval (CI) and were performed with computational support of the software IBM SPSS 25 (Statistical Package for the Social Sciences) and Excel 2016 (Microsoft Office). RESULTS: A total of 40 patients were evaluated. The mean age was 53 years and the majority of them was male (70%). The most common etiology of cirrhosis was C virus hepatitis (42.5%). The patients were Child C in 45% of cases. The indocyanine green retention rate in 15 minutes (R15) was increased on days 1 and 3 postoperatively ( > 10%) and normalized on the 7th postoperative day ( < 10%). The ICG clearance rate maintained normal values, with 18.5% in the 1st day; 20.3 in the 3rd and 20.4 in the 7th postoperative day. The comparison with Olthoff and Wagener criteria showed no statistical difference (p=0,467 e p=0,178). In the comparison with DRI > 1.5 a p = 0.066 was found; and with negative outcome (Loss of graft or death) a p = 0.063 was found. The clearance of indocyanine green showed a significant relation with the degree of histological lesion after ischemia and reperfusion (p = 0.030). CONCLUSION: The liver functional reserve is decreased in the recent postoperative period of liver transplantation with improvement at the end of the first week. Hepatic clearance of indocyanine green is not related to early graft dysfunction assessed by Oltoff and Wagener criteria. On the other hand, it has a significant relationship inversely proportional to the degree of ischemia and reperfusion hepatic injury

Introdução e Objetivos: A Doença hepática gordurosa não alcóolica (DHGNA) é claramente marcada por influência ambiental, no entanto, fatores genéticos têm sido associados a progressão para formas mais graves de DHGNA. O estresse oxidativo tem sido cada vez mais enfatizado nesta evolução e o sistema NADPH parece ser uma importante fonte de produção de espécies reativas de oxigênio. O papel dos polimorfismos do sistema NADPH nessa população e sua possível relação com progressão de doença e síndrome metabólica são desconhecidos. Desta maneira, investigamos dois polimorfismos de nucleotídeo único (SNPs) na região reguladora dos genes que codificam a NADPH oxidase 4 subunidade catalítica (NOX4) e sua subunidade regulatória p22phox (CYBA) e sua relação com variáveis histológicas e bioquímicas em pacientes com DHGNA. Métodos: 207 pacientes com DHGNA com biópsia hepática (esteatose=27; esteato-hepatite=180), sendo (70% do sexo feminino), foram genotipados para SNPs da Nox4 (rs3017887) e CYBA -675 T/A. O DNA genômico foi extraído a partir de células do sangue periférico e a genotipagem foi realizada utilizando primers específicos e sondas fluorescentes marcadas por sequenciamento. A análise histológica foi baseada na classificação de Kleiner et al (2005). Todos os pacientes eram negativos para os marcadores de hepatites virais, doença de Wilson, hemocromatose, doenças auto-imunes e tinham ingestão diária de álcool < 100 g/semana. Resultados:Associação do CYBA -675 T/A com triglicerídeos foi observada: 176,87 ± 12,93 (TT) versus 228,70 ± 21,66 (XA), p = 0,007 . Quando a população é estratificada, a associação aparece apenas entre os pacientes que possuem esteatohepatite não alcoólica (EHNA). Foi observada uma associação do CYBA-675 T / A com HDL: 48,05 ± 1,71 (TT) vs 41,70 ± 2,91 (TA) vs 28,03 ± 9,47 mg / dL (AA), p = 0,001. Quando a população é estratificada, a associação aparece apenas entre os pacientes que possuem EHNA. Quando se estudou o SNP no gene da NOX4, observou-se associação com ALT (CA + AA: 60,10 ± 6,01 U/L vs CC: 44,23 ± 4,36 U/L; P = 0,02). Porém, quando a população foi estratificada em esteatose e EHNA não se verificou diferença estatisticamente significante na frequência dos genótipos. Não houve associação de SNPs de genes que codificam proteínas do sistema NADPH oxidase nos genes CYBA (não registrado) e Nox4 (rs 3017887) e a presença de EHNA, nos portadores de DHGNA. Quanto aos resultados clínicos, observou-se que os graus de fibrose mais avançados ocorreram em pacientes com diagnóstico de Diabetes Mellitus tipo 2 (66,9% vs 37,5%; p= 0,007), além de serem mais obesos (32,2% vs 29%; p=0,003). Além disso, os níveis séricos de glicose e insulina aumentaram significantemente, de acordo com a presença de EHNA. A frequência da SM foi significativamente maior em pacientes com EHNA. Conclusões: 1) Houve associação entre a presença do genótipo CC na Nox4 SNP com uma maior concentração de ALT na população em geral 2) Houve associação entre a presença do genótipo AA no polimorfismo do gene -675 T/A CYBA com uma maior concentração de TGL e menor de HDL, nos pacientes com EHNA 3) Houve associação entre a presença de síndrome metabólica e graus avançados de fibrose hepática em portadores de DHGNA. No entanto, a amostra deve ser ampliada para confirmar estas associações, bem como, para melhor explorar possíveis relações com escores de fibrose e inflamação Background/Aims: Oxidative stress has been implicated in progression to severe forms of non-alcoholic fatty liver disease (NAFLD). NADPH oxidase appears to be an important source of production of reactive oxygen species. We investigated by the first time two single nucleotide polymorphisms (SNPs) in the regulatory region of genes encoding the NADPH oxidase 4 (NOX4) and p22phox (CYBA) in NAFLD. Methods: 207 biopsy-proven NAFLD patients (27 = steatosis; 180= NASH) were evaluated. Genomic DNA was extracted from peripheral blood cells and on Nox4 and CYBA polymorphisms were determined by direct sequencing of PCR. All the patients were negative for markers of viral hepatitis, Metabolic diseases, autoimmune diseases and had daily intake of alcohol < 100 g/week. Results: An association of CYBA -675 T / A with triglycerides was observed: 176.87 ± 12.93 (TT) versus 228.70 ± 21.66 (XA), mean ± SEM, p = 0.007. When the population is stratified, the association appears only among patients who have non-alcoholic steatohepatitis (NASH). An association of CYBA-675 T/A with HDL was observed: 48.05 ± 1.71 (TT) vs 41.70 ± 2.91 (TA) vs 28.03 ± 9.47 mg/dL (AA), mean ± SEM, p = 0.001. When the population is stratified, the association appears only among patients who have NASH. When the NOX4 (rs 3017887) gene was studied, there was association with ALT (CA + AA: 60.10 ± 6.01 U / L vs CC: 44.23 ± 4.36 U/L; P = 0, 02). However, when the population was stratified in steatosis and NASH, there was no statistically significant difference in the frequency of genotypes. There was no association of SNPs from genes encoding NADPH oxidase system proteins in the CYBA (unregistered) and Nox4 (rs 3017887) genes and the presence of NASH in the DHGNA carriers. Regarding the clinical results, it was observed that the most advanced degrees of fibrosis occurred in patients diagnosed with Type 2 Diabetes Mellitus (66.9% vs 37.5%, p = 0.007), in addition to being more obese (32.2% % vs 29%, p = 0.003). In addition, serum glucose and insulin levels increased significantly, according to the presence of NASH. The frequency of MS was significantly higher in patients with NASH. Conclusions: 1) There was an association between the presence of the CC genotype in the Nox4 SNP with a higher concentration of ALT in the general population. 2) There was an association between the presence of the AA genotype in the CYBA gene -675 T / A polymorphism with a higher concentration of TGL and lower HDL in patients with NASH. 3) There was an association between the presence of metabolic syndrome and advanced degrees of hepatic fibrosis in patients with NAFLD. However, the sample should be expanded to confirm these associations, as well as to better explore possible relationships with fibrosis and inflammation scores

Introdução e Objetivos: A Hepatite pelo Vírus E (HEV) é uma infecção conhecida mundialmente por ter seu curso assintomático e autolimitado na maioria dos casos. Nos últimos anos, casos de evolução para cronicidade têm sido descritos envolvendo população de pacientes imunocomprometidos, particularmente os transplantados de órgãos sólidos. A prevalência da infecção pelo HEV na população de transplantados de fígado é ainda desconhecida em nosso meio. Pacientes e Métodos: Nós avaliamos parâmetros de infecção pelo HEV (HEV RNA, anti-HEV IgM e anti-HEV IgG) em uma coorte de 294 adultos transplantados de fígado seguidos no HCFMUSP. Para pesquisa dos anticorpos foi utilizada a metodologia ELISA (RecomWell HEV IgM/ IgG da Mikrogen®), sendo os resultados indeterminados e com IgM isoladamente positivo confirmados por Immunoblotting (RecomLine IgM/ IgG da Mikrogen®). Para pesquisa do HEV RNA utilizou-se a metodologia de PCR em tempo real One-Step com primers e sondas que amplificam um fragmento da ORF3 do genoma viral. Foram avaliados dados demográficos e laboratoriais de toda população de transplantados. Posteriormente, foram analisadas apenas as biópsias hepáticas de 122 pacientes transplantados devido à Hepatite pelo Vírus C (VHC) possuidores ou não de marcadores sorológicos ou moleculares do HEV de acordo com a classificação METAVIR. Resultados: Amostras de 8,2% (24/294) dos pacientes foram positivas para anti-HEV IgG, 2% (6/294%) positivas para anti-HEV IgM e 5,8% (17/274) mostraram positividade para o HEV RNA. Apenas um paciente mostrou positividade tanto para anti-HEV IgM quanto para o anti-HEV IgG. Setenta e sete vírgula oito por cento (95/122) dos pacientes transplantados pelo VHC foram tratados com esquema que incluía ribavirina por um período mínimo de 6 meses antes da coleta do sangue. Dentre os transplantados pela cirrose por VHC que apresentaram positividade para o anti-HEV IgG apenas 37,5% (3/122) mostraram níveis de fibrose maior que 2, enquanto que 41,7% (5/122) dos positivos para o HEV RNA demonstraram níveis de fibrose maior que 2. Em geral, a prevalência do HEV no cenário pós transplante hepático parece ser baixa podendo não trazer um dano histológico significativo no cenário pós transplante. Conclusão: Nós concluímos que apesar de alguns estudos demonstrarem riscos de cronificação pelo HEV, esse vírus tem baixa prevalência em nosso meio e os pacientes transplantados de fígado pelo VHC que apresentaram marcadores sorológicos ou moleculares para o HEV não tiveram níveis mais acentuados de fibrose quando comparados com os pacientes que não apresentaram indícios de infecção pelo HEV no momento da análise Background and Aims: Hepatitis E Virus (HEV) is an infection known worldwide for its asymptomatic and self-limited course in most of the cases. In the last five years, cases evolving to chronicity have been described involving immunosuppressed patients, especially in recipients of solid organs transplants. The HEV infection prevalence in liver transplanted patients has yet to be fully assessed by our community. Patients and Methods: We evaluated laboratory parameters for the HEV infection (HEV RNA, anti-HEV IgM and anti-HEV IgG) in a cohort of 294 adult patients who had liver transplants on the HCFMUSP (Clinical Hospital of University of São Paulo School of Medicine). In order to investigate the antibodies, we used Elisa methodology (RecomWell HEV IgM/ IgG by Mikrogen®) and the indeterminate results and isolates positive anti-HEV IgM were confirmed by immunoblotting (RecomLine IgM/ IgG by Mikrogen®). For screening of HEV RNA, One-Step PCR in real time was used with primers and probes that amplify a fragment of the ORF3 from the viral genome. Laboratory and demographic data were collected in the entirety of the transplanted population. Following that, the hepatic biopsies of 122 patients transplanted due to Hepatitis C (HCV) were analysed with or without serological or molecular markers of HEV according to METAVIR score. Results: Serological samples of 24 (8.2%) of the patients tested positive for anti-HEV IgG, 06 (2%) were positive for anti-HEV IgM and 17 (5.8%) showed positive results for HEV RNA. Only one patient showed positive results for both anti- HEV IgM and anti-HEV IgG. Also, 95 (77.8%) of the patients transplanted because of the HCV infection received treatment including Ribavirin for at least 6 months before the blood sample collection. Among the transplanted patients due to HCV cirrhosis who presented positive results for anti-HEV IgG, only 3 (37.5%) showed fibrosis beyond stage 2, while 5 (41.7%) of the HEV RNA positive patients had liver fibrosis higher than 2. Conclusion: Overall, the prevalence of HEV in the post-hepatic transplant scenario appears to be low, seemingly not harmful histologically in the post-transplant cases. We have concluded that although some studies showed risks of HEV chronification, patients who had their livers transplanted due to HCV who showed serological or molecular markers for HEV did not have higher levels of fibrosis when compared to patients who showed no indications of HEV infection at the time of the analysis

O objetivo principal deste estudo foi avaliar fatores virais associados com a evolução para o carcinoma hepatocelular (CHC) em pacientes com hepatite B crônica. Para tanto caracterizamos os subgenótipos do HBV, investigamos a ocorrência de mutações nos genes pré-core/core do HBV associadas à presença de CHC avaliamos por análise filogenética a associação de linhagens virais com a ocorrência de CHC e por fim a associação de outros fatores de risco com o desenvolvimento de CHC. Foram incluídos 119 amostras de soro de pacientes com infecção crônica pelo HBV, destas amostras 60 pertencem ao grupo 1 (CHC), que são pacientes com diagnóstico confirmado de carcinoma hepatocelular e 59 amostras pertencem ao grupo 2 (sem CHC) que são pacientes com hepatite crônica sem detecção prévia de nódulos hepáticos. Foram obtidas informações acerca da idade, sexo e naturalidade. Além disso, os pacientes responderam a um questionário sobre fatores de riscos associados ao desenvolvimento de CHC. Foram realizados exames bioquímicos, sorológicos, determinação da carga viral, e amplificação por nested PCR e sequenciamento das regiões S/polimerase e pré-core/core do genoma viral para posterior caracterização dos genótipos/subgenótipos do HBV e pesquisa de mutações associadas com evolução da doença hepática. Em relação à idade e sexo não houve grande variação entre os grupos. Quanto à naturalidade a maioria era procedente da região sudeste, seguido pela região nordeste; e por fim seis pacientes eram procedentes de outros países. Com base no sobrenome dos pacientes avaliou-se também a frequência de etnia oriental na casuística estudada, que foi similar nos 2 grupos. O perfil sorológico HBeAg negativo foi o mais frequente nos dois grupos de pacientes, assim como níveis de carga viral abaixo de 2.000 UI/mL. Em relação aos exames bioquímicos foram observadas diferenças estatisticamente significantes nos níveis séricos de AFP (p= 0,0013), FA (p= 0,0003) e GGT (p= 0,005). Dentre os fatores de risco analisados neste estudo, o consumo de amendoim foi o único que apresentou significância estatística (p= 0,003). A região S/pol foi amplificada e sequenciada com sucesso em 58 amostras (28 do grupo 1 e 30 do grupo 2). Entre as 58 amostras analisadas 4 genótipos e 8 subgenótipos do HBV foram identificados, sendo o subgenótipo A1 o mais frequente nos dois grupos. Não se observou diferença estatisticamente significante na distribuição dos subgenótipos entre os dois grupos de pacientes. Na topologia da árvore filogenética construída com sequências do HBV isoladas dos pacientes incluídos neste estudo e sequências disponíveis no GenBank não se observou padrões de agrupamento associados com o perfil clinico do paciente (com e sem CHC). Foram obtidas sequências de boa qualidade da região précore/ core em 44 amostras, sendo 20 amostras do grupo 1 e 24 do grupo 2. Diversas das mutações investigadas foram identificadas na região précore/ core, as quais foram avaliadas estatisticamente para verificar a existência de diferença na frequência das mesmas entre os grupos de pacientes estudados. Entre as mutações identificadas se destacaram com significância estatística as seguintes mutações: T1768A (p= 0,006), a combinação das mutações C1766T + T1768A (p= 0,043) e G1888H (p= 0,05). Na análise de regressão logística simples foi possível identificar que a chance de um paciente do grupo 2 desenvolver CHC aumenta 14,7 vezes na presença de infecção por cepas do HBV com a mutação T1768A, enquanto que a infecção com cepas do HBV que albergam a mutação G1888H reduz tal chance 2,5 vezes The aim of this study was to evaluate viral factors associated with the progression to hepatocellular carcinoma (HCC) in patients with chronic hepatitis B. For this goal, we characterized HBV subgenotypes, investigated the occurrence of mutations in pre-core/core genes associated with progression to HCC, characterized HBV strains through phylogenetic analyzes and evaluated risk factors associated with HCC. Were included 119 serum samples from patients with chronic HBV infection that were classified in 2 groups: 60 patients with confirmed HCC diagnosis (group 1) and 59 patients with advanced hepatitis B liver disease without the detection of nodular liver lesions and without HCC (group 2). Data about the age, sex and geographic precedence were obtained from medical records. The patients also answered a questionnaire on risk factors for developing HCC. Biochemical, serological and viral load testing were performed in all samples. Moreover, S/polymerase and precore /core regions of HBV DNA were amplified by nested PCR and sequenced by Sanger method. Sequences were analyzed to identify HBV genotypes and subgenotypes and to detect mutations in the precore/core gene. Patient\'s age and sex did not differ between the two groups. Most of the patients came from the Southeast region, followed by the Northeast region; and six patients were from other countries. Based on the patient\'s surname, they were evaluated concerning Eastern ethnicity, which was similar in the 2 groups. Most of the patients included in this study were HBeAg negative and showed viral load bellow 2,000 IU/mL. Concerning the biochemistry assays, statistically significant differences in serum levels of AFP (p = 0.0013), AP (p = 0.0003) and GGT (p = 0.005) were found. Among the risk factors analyzed in this study, peanut consumption was the only one statistically significant (p = 0.003). The S/pol region was successfully amplified and sequenced in 58 samples (28 from Group 1 and 30 from Group 2). Among the 58 samples analyzed, 4 genotypes and 8 subgenotypes were identified, subgenotype A1 was the most frequent in both groups and there was no statistically significant difference in the distribution of them between the two groups. In the phylogenetic tree topology built with HBV sequences isolated from patients included in this study and sequences available in GenBank, it was not observed any clustering associated with the clinical profile of the patients (with or without HCC). Sequences of good quality from pre-core/core region were obtained from 44 samples, 20 from group 1 and 24 from group 2. These sequences were analyzed and several mutations were found among which stood out with statistical significance: T1768A (p = 0.006) C1766T + T1768A (p = 0.043) and G1888H (p = 0.05). In addition to the comparative analysis, the changes were subjected to a simple logistic regression analysis which found that the chance of a patient in group 2 developed HCC increases 14.7 times in the presence of HBV infection strains with the T1768A mutation, while infection with HBV strains harboring the mutation G1888H reduces this chance by 2.5 times

INTRODUÇÃO: No Brasil, no período compreendido entre 2013 a 2015, a terapia tripla com boceprevir (BOC) ou telaprevir (TVR) foi o tratamento padrão para pacientes infectados pelo vírus da hepatite C (VHC) genótipo 1. OBJETIVOS: Avaliar a efetividade e a segurança da terapia tripla com BOC ou TVR, no contexto de vida real, em pacientes portadores de hepatite C crônica genótipo 1 tratados em centros de referência no Brasil. Foi também objetivo deste estudo, investigar os fatores preditores para obtenção da resposta viral sustentada (RVS) e fatores preditores da ocorrência de eventos adversos sérios (EAS) associados a essa modalidade terapêutica. MÉTODOS: Trata-se de um estudo multicêntrico, observacional e retrospectivo, que incluiu pacientes monoinfectados pelo VHC genótipo 1 que iniciaram o tratamento com interferon peguilado (Peg-IFN), ribavirina e BOC ou TVR . Dados demográficos, clínicos, virológicos e eventos adversos (EA) foram coletados durante o tratamento e seguimento. A análise dos fatores preditores da RVS e de EAS foi realizada por meio do modelo de regressão de Poisson com variância robusta. RESULTADOS: Dos 715 pacientes analisados, 56,1% eram do sexo masculino, com média de idade de 54,1 ± 10,1 anos, 59% eram portadores de cirrose hepática e 67,1% tinham sido tratados previamente. Foram tratados com TVR, 557 pacientes (77,9%), e com BOC, 158 (22,1%). Pela análise de intenção de tratamento, a RVS geral foi de 56,6% (IC 95%, 52,9 - 60,3), com uma efetividade semelhante em ambos os grupos (51,9% [BOC] versus (vs.) 58% [TVR], P = 0,190). Os pacientes cirróticos tiveram uma menor taxa de RVS comparado aos não cirróticos (46,9% vs. 70,6%, P < 0,001). Na análise múltipla, a obtenção de RVS foi associada com a ausência de cirrose hepática (P < 0,001), antecedente de recidiva viral após tratamento prévio (P < 0,001), contagem de plaquetas no pré-tratamento acima de 100.000/mm3 (P < 0,001) e obtenção de resposta viral rápida (P < 0,001). A taxa de descontinuação do tratamento foi de 34,8%, sendo a ocorrência de EA (16,1%) e a falha virológica (15,9%) as principais causas. Em relação à segurança do tratamento, 90,1% dos pacientes (IC 95%, 87,6 - 92,2) apresentaram algum EA durante o tratamento, sendo que 44,2% dos pacientes (IC 95%, 40,5 - 47,9) apresentaram EAS. A anemia foi o EA mais comum em ambos os grupos (59,5% [BOC] vs. 74,5% [TVR], P < 0,001). Houve maior ocorrência de EAS nos pacientes cirróticos comparados aos não cirróticos (50,7% vs. 34,8%, P < 0,001). Sexo feminino (P < 0,001), idade acima de 65 anos (P = 0,008), diagnóstico de cirrose hepática (P = 0,019), concentração de hemoglobina pré-tratamento alterada (P < 0,001) e contagem de plaquetas abaixo de 100.000/mm3 no pré-tratamento (P < 0,001) foram associados à ocorrência de EAS. Seis óbitos (0,8%) ocorreram. CONCLUSÃO: A terapia tripla com BOC ou TVR apresentou uma taxa de RVS superior ao tratamento padrão anterior (Peg-IFN e ribavirina). Contudo, este tratamento apresentou uma alta taxa de EAS, principalmente em pacientes com doença hepática avançada INTRODUCTION: In Brazil, from 2013 until 2015, triple therapy with boceprevir (BOC) or telaprevir (TVR) was the standard treatment for patients infected with hepatitis C virus (HCV) genotype 1. OBJECTIVES: To evaluate the effectiveness and safety of triple therapy with BOC or TVR, in real life context, in patients with chronic hepatitis C genotype 1 treated at reference centers in Brazil. It was also the objective of this study to investigate the predictive factors for obtaining sustained viral response (SVR) and factors predictive of the occurrence of serious adverse events (SAE) associated with this therapeutic modality. METHODS: This was a multicenter, observational, retrospective study that included HCV infected patients genotype 1 who started treatment with pegylated interferon, ribavirin and BOC or TVR from July 2013 until April 2014, from 15 centers of reference in Brazil. Demographic, clinical, virological, and adverse events (AE) data were collected during treatment and follow-up. The analysis of predictive factors of SVR and SAE was performed using the Poisson regression model with robust variance. RESULTS: Of the 715 patients analyzed, 56.1% were males, mean age was 54.1 ± 10.1 years, 59% had hepatic cirrhosis and 67.1% had been previously treated. They were treated with TVR, 557 patients (77.9%), and with BOC, 158 (22.1%). By intention-to-treat analysis, overall SVR was 56.6% (95%CI, 52.9 - 60.3), with similar effectiveness in both groups (51.9% [BOC] versus (vs.) 58% [TVR], P = 0.190). Cirrhotic patients had a lower SVR rate compared to non-cirrhotic patients (46.9% vs. 70.6%, P < 0.001). In the multiple analysis, SVR acquisition was associated with absence of hepatic cirrhosis (P < 0.001), previous viral relapse after previous treatment (P < 0.001), pre-treatment platelet count above 100,000/mm3 (P < 0.001) and rapid viral response (P < 0.001). The treatment discontinuation rate was 34.8%, with the occurrence of AE (16.1%) and virological failure (15.9%) being the main causes. Regarding treatment safety, 90.1% of the patients (95%CI, 87.6 - 92.2) presented some AE during treatment, and 44.2% of the patients (95%CI, 40.5 - 47,9) presented SAE. Anemia was the most common AE in both groups (59.5% [BOC] vs. 74.5% [TVR], P < 0.001). There was a higher occurrence of SAE in cirrhotic patients compared to non-cirrhotic patients (50.7% vs. 34.8%, P < 0.001). Female gender (P < 0.001), age above 65 years (P = 0.008), diagnosis of hepatic cirrhosis (P = 0.019), altered pre-treatment hemoglobin concentration (P < 0.001) and platelet count below 100,000/mm3 in the pre-treatment (P < 0.001) were associated with the occurrence of SAE. Six deaths (0.8%) occurred. CONCLUSION: Triple therapy with BOC or TVR showed a higher SVR rate than the previous standard treatment (Peg-IFN and ribavirin). However, this treatment had a high EAS rate, especially in patients with advanced liver disease

Introdução e Objetivos: Hipovitaminose D, definida como nível sérico de 25(OH)D insuficiente é considerada pandêmica em muitas populações ao redor do mundo e está associada a comorbidades em hepatite C, infecção por HIV e coinfecção HIV/HCV. Os objetivos deste estudo são: 1) comparar a prevalência de deficiência de vitamina D (DVD) caracterizada por nível sérico de 25(OH)D < 20 ng/mL, entre pacientes monoinfectados pelo HCV, monoinfectados pelo HIV, coinfectados HIV/HCV e participantes do grupocontrole; 2) identificar fatores de risco específicos associados com DVD na população estudada. Pacientes e Métodos: Foram coletados dados clínicos e demográficos, 25(OH)D sérica, testes de função hepática e perfil metabólico durante os meses de inverno de 129 pacientes HCV monoinfectados, 118 pacientes HIV monoinfectados e 53 pacientes coinfectados HIV/HCV tratados em centros de referência na cidade de São Paulo, bem como, em 122 indivíduos saudáveis de um grupo-controle formado de pessoas não infectadas por HIV, HCV ou HBV, sem uso de suplementos de vitamina D. Resultados: A prevalência de deficiência de vitamina D ajustada por sexo, idade ( = 50), cor de pele (branco vs não branco), índice de massa corporal ( = 25), colesterol total (= 200), fração HDL colesterol ( = 40 em homens = 50 em mulheres), triglicérides (= 150), glicemia ( = 110), uso de efavirenz (sim vs não), uso de tenofovir (sim vs não) e índice HOMA-IR, foi menor no grupo HCV do que no controle e no grupo HIV (p < 0.001). Em todos os grupos, a razão de chance de DVD aumenta 1.21 [IC95%(1.01; 1.44) p=0.026] para cada ponto de aumento do índice HOMA. Efavirenz também esteve associado com maior razão de chance de DVD [3.49(IC95% 1.14-10.67) p=0.028]. Análise por regressão logística simples foi aplicada para avaliar fatores de risco associados a DVD dentro de cada grupo. Nesta análise, resultaram com associação significativa o sexo masculino, com uma menor razão de chance para DVD [RC 0,42(IC95% 0,18 - 0,96) p = 0,04] no grupo-controle e no grupo HCV [RC 0,42(IC95% 0,2 - 0,88) p = 0,02]; ainda no grupo HCV houve associação significativa entre DVD e HOMA-IR elevado [RC 5,59(IC 95% 1,37 - 22,8) p = 0,02]; e, no grupo HIV, os indivíduos que apresentaram nadir histórico de CD4 maior que 200 células/mm3 tiveram menos chance de DVD [RC 0,41 (IC95% 0,18 - 0,95) p = 0,04]. Conclusão: Uma alta prevalência de DVD foi observada em toda a população estudada, incluindo o grupo-controle, sugerindo que a apresentação de infecção por HIV e/ou HCV por si só não aumenta as chances de DVD. Por outro lado, o incremento do índice HOMA e o uso de efavirenz se destacaram como fatores de risco nesta população. Estes achados ressaltam a importância da associação da deficiência de vitamina D com outras duas condições; a resistência à insulina e o uso de terapia antirretroviral para o HIV, os quais, isoladamente ou em combinação, podem aumentar a incidência de comorbidades como o diabetes do tipo 2 Background and Aims: Hypovitaminosis D, defined as insufficient serum level of 25(OH)D, is considered pandemic in many populations worldwide and is associated with co-morbidities in hepatitis C, HIV and HIV/HCV co-infection. The aim of this study is to 1) compare the prevalence of 25-hydroxyvitamin D deficiency (VDD), defined as serum levels of 25(OH)D < 20 ng/mL, among HCV mono-infected, HIV mono-infected, HIV/HCV co-infected patients and control participants and 2) identify specific risk factors associated with VDD in each group. Patients and Methods: We collected demographic and clinical data, serum 25-hydroxyvitamin D, liver function parameters and metabolic profiles on 129 HCV mono-infected, 118 HIV mono-infected and 53 HIV/HCV co-infected patients treated at reference centers in São Paulo (Brazil) as well as on 122 volunteer controls, not infected by HIV, HCV, HBV or taking vitamin D supplements. Results: VDD prevalence adjusted for sex, age ( = 50), skin color (white vs not white), body mass index ( = 25), total cholesterol (= 200), HDL cholesterol ( = 40 in men and = 50 in women), triglycerides ( = 150), glycemia ( = 110), use of Efavirenz - EFV (yes vs no), use of Tenofovir -TDV (yes vs no) and HOMA-IR was lower in HCV group than control and HIV groups (p < 0.001). In all groups, adjusted odds of VDD increases by 1.21 [CI95% (1.01-1.44)] for each unit increase of HOMA-IR. Antirretroviral therapy regimens containing efavirenz were also associated to higher odds of VDD 3.49 [CI95% (1.14-10.67) p=0.028]. Logistic regression was applied to analyze risk factors associated to VDD within each group. In this analysis male sex resulted significantly associated to lower chance of VDD [OR 0,42(CI95% 0,18 - 0,96) p = 0,04] in control group and in HCV group [RC 0,42(CI95% 0,2 - 0,88) p = 0,02]; still in HCV group, elevated HOMA-IR was significantly associated to VDD [OR 5,59(CI 95% 1,37 - 22,8) p = 0,02]; and in HIV group, individuals presenting CD4 nadir higher than 200 cells/mm3 had less chance of VDD [OR 0,41 (IC95% 0,18 - 0,95) p = 0,04]. Conclusion: High prevalence of VDD was observed across all studied population, including control group, suggesting that being infected with HIV and/or HCV per se does not increase the chance of VDD. Otherwise, VDD was positively associated with HOMA-IR increase for controls and infected patients. It is also associated to use of Efavirenz in HIV/HCV patients. This finding highlights the relevance of vitamin D deficiency association with two other conditions; insulin resistance and antiretroviral therapy, which isolated or in combination, may contribute to the incidence of comorbidities, as Type 2 diabetes mellitus

Introdução: Tanto o vírus da hepatite crônica C (VHC) quanto o tratamento antiviral estão associados a transtornos do humor, piora da função cognitiva e da qualidade de vida. Objetivos: avaliar os pacientes com hepatite crônica pelo VHC antes, durante e após o tratamento antiviral em relação aos seguintes aspectos: depressão maior e sintomas depressivos, função cognitiva e qualidade de vida. Métodos: estudo clínico, prospectivo, cujos critérios de inclusão foram: idade 18 a 70 anos; clinicamente compensados. Critérios de exclusão: uso ativo de substâncias psicoativas ilícitas; confusão mental; encefalopatia hepática e recusa em participar do estudo ou em receber terapia antiviral. Utilizamos os seguintes instrumentos: 1) Diagnóstico de depressão maior: entrevista estruturada Primary Care Evaluation of Mental Disorders (PRIME-MD); 2) Sintomas depressivos: o Beck Depression Inventory (BDI). 3) Avaliação da qualidade de vida: questionários SF-36 e Liver Disease Quality of Life Questionnaire instrument (LDQOL). 4) Avaliação neuropsicológica: a) funções executivas - atenção seletiva e inibição (Stroop Teste Versão Victoria), atenção sustentada e alternada (Trail Making Test - partes A e B), fluência verbal fonológica (FAS) e fluência verbal semântica ou categórica. b) aprendizagem verbal e memória de longo-prazo (Hopkins Verbal Learning Test HVLT-R); c) memória de curtoprazo e memória operacional (Dígitos ordem direta e Dígitos de ordem inversa); d) teste de QI estimado. Os testes foram aplicados previamente ao início do tratamento, durante e após 6 meses do término do tratamento. Resultados: dos 78 pacientes avaliados, 36 completaram a avaliação de humor e qualidade de vida e 34, a avaliação neuropsicológica. Os 21 pacientes que completaram o tratamento foram considerados para análise final (n=16 com resposta virológica sustentada RVS e n=5, sem RVS). Quanto ao transtorno de humor, dentre os pacientes com RVS, observamos depressão maior em 9,5% antes do tratamento e 52,4% durante (p=0,012). Ao término do tratamento, a frequência reduziu-se a 6,3 % (p=1,0). Em relação aos sintomas depressivos, encontramos uma frequência de 19,1%, 62,0% e 25,0% antes, durante e após o tratamento, respectivamente. Diferença estatisticamente significante foi observada nas frequências antes e durante o tratamento (p=0,016). Dentre os 5 pacientes que cursaram sem RVS, um apresentou critério para depressão maior e 2 para sintomas depressivos. Quanto à função cognitiva, houve melhora no domínio da atenção seletiva (p=0,004) durante o tratamento. Ao seu término, houve melhora significante nos domínios: memória episódica verbal imediata (p=0,014), memória episódica verbal tardia (p=0,024), atenção seletiva (p=0,001) e fluência verbal fonológica (p=0,030). Comparando-se os pacientes com e sem RVS, observamos melhora significante nos domínios: memória episódica verbal imediata (p=0,045) e memória episódica verbal tardia (p=0,040). Em relação à qualidade de vida, houve piora significante da autopercepção em todos os domínios avaliados ao longo do tratamento, exceto no estado geral de saúde, no questionário SF-36. No questionário LDQOL, observou-se piora ao longo do tratamento nos domínios sintomas (p=0,000), efeitos da doença hepática (p=0,007), concentração (p=0,008), questões sociais (p=0,002) e função sexual (p=0,010). Ao término do tratamento, observou-se melhora significante nos domínios efeitos da doença hepática (p=0,024), preocupação com a doença (p=0,018) e estigma da doença hepática (p=0,008). Comparando-se os pacientes com e sem RVS, observamos melhora significante no domínio efeitos da doença hepática (p=0,005) naqueles pacientes com RVS. Conclusões: 1) Durante o tratamento, houve aumento da frequência de sintomas depressivos e de depressão maior, piora da qualidade de vida, sem haver piora importante dos domínios das funções cognitivas. 2) Ao término do tratamento, houve melhora do transtorno do humor, da função cognitiva (memória, atenção e fluência verbal) e da qualidade de vida. 3) Os pacientes que atingiram RVS apresentaram melhora da função cognitiva (memória) e da qualidade de vida quando comparados àqueles sem RVS Background: Both chronic hepatitis C virus (HCV) and antiviral therapy are associated with mood disorders, the deterioration of cognitive functions and worsening quality of life. Aims: To evaluate patients with chronic hepatitis C before, during and after antiviral treatment in relation to: major depression and depressive symptoms, cognitive function and quality of life. Methods: prospective study, whose inclusion criteria were: age between 18- 70 years; clinically compensated. Exclusion criteria: active use of illegal drugs; mental confusion; hepatic encephalopathy and refusal to participate or receive antiviral therapy. We used the following instruments: 1) Major depression diagnosis: Primary Care Evaluation of Mental Disorders (PRIME-MD); 2) Depressive symptoms: the Beck Depression Inventory (BDI). 3) Quality of life assessment: SF-36 and Liver Disease Quality of Life Questionnaire instrument (LDQOL). 4) Neuropsychological assessment: a) executive functions - selective attention and inhibition (Stroop Test Version Victoria), sustained and alternating attention (Trail Making Test - Parts A and B), phonological verbal fluency (FAS) and semantic verbal fluency or categorical. b) verbal learning and long-term memory (Hopkins Verbal Learning Test HVLT-R); c) short-term memory and working memory (Digits direct and reverse order of digits); d) estimated IQ test. The tests were applied before the start of treatment, during and after 6 months of the treatment. Results: of the 78 patients enrolled, 36 completed the mood disorder and quality of life workup and 34, the neuropsychological assessment. 21 patients who completed treatment were considered for final analysis (with sustained virologic response SVR n=16; without SVR n=5). As for the mood disorder among patients with SVR, we observed major depression in 9.5% before treatment, 52.4% for (p=0.012). After treatment, the frequency decreased to 6.3% (p=1.00). Regarding depressive symptoms, we found a frequency of 19.1%, 62.0% and 25.0% before, during and after treatment, respectively. Significant differences in frequency before and during treatment were observed (p=0.016). Among the 5 patients who did not achieve SVR, one fulfilled diagnostic criteria for major depression and 2 for depressive symptoms. Regarding cognitive function, there was an improvement in the field of selective attention (p=0.004) during treatment. Upon termination of treatment, there was a significant improvement in these domains: immediate verbal episodic memory (p=0.014), delayed verbal episodic memory (p=0.024), selective attention (p=0.001) and phonological verbal fluency (p=0.030). Comparing patients with and without SVR, we observed a significant improvement in these domains: immediate verbal episodic memory (p=0.045) and delayed verbal episodic memory (p=0.040). Regarding quality of life, there was a significant decline in self-perception in all domains assessed by the SF-36 questionnaire during treatment, except general health. The LDQOL questionnaire showed a worsening of the following domains during treatment: symptoms (p=0.000) effects of hepatic disease (p=0.007), concentration (p=0.008), social issues (p=0.002) and sexual function (p=0.010). After treatment, we observed a significant improvement in these domains: effects of hepatic disease (p=0.024), concern about the disease (p=0.018) and stigma of liver disease (p=0.008). Comparing patients with and without SVR, a significant improvement was evident in the effects of liver disease (p=0.005) in patients with SVR. Conclusions: 1) During treatment, there was an increase in the frequency of depressive symptoms and major depression, poor quality of life, with no significant deterioration of cognitive function domains. 2) After treatment, there was an improvement of mood, cognitive function (memory, attention and verbal fluency) and quality of life. 3) Patients who achieved SVR improved cognitive function (memory) and quality of life compared to those without SVR
