Paragangliomas of the mediastinum are rare, with only approximately 150 cases reported in the literature. Surgical excision is the treatment of choice; however, these tumors often lie near critical vascular structures. Here we present the case of a patient with a mediastinal paraganglioma discovered during a diagnostic procedure.

OBJECTIVE: To compare the effect of depot medroxyprogesterone acetate (DMPA) and two types of oral contraceptives (OC) on bone mineral density (BMD) among women 18–33 years of age with those not using hormonal contraception. METHODS: Data from 155 women were analyzed. Depot medroxyprogesterone acetate was administered to 33 women; 63 women who chose oral contraception were randomly assigned to receive either a norethindrone-containing pill ( n = 28) or a desogestrel-containing pill ( n = 35). Fifty-nine women who did not use hormonal contraception served as controls. Lumbar spine BMD was determined using dual-energy x-ray absorptiometry at baseline and after 12 months of contraceptive use. We analyzed method-related percent change in BMD while controlling for body mass index, calcium intake, exercise, and smoking. We had approximately 90% power to detect a 2.5% difference between any two groups. RESULTS: Users of DMPA experienced a mean BMD loss of 2.74% over 12 months compared with controls who sustained a 0.37% loss ( P = .01). Users of OCs generally demonstrated a gain (2.33% for norethindrone-containing pills, 0.33% for desogestrel-containing pills), which was different from controls among users of norethindrone-containing pills ( P = .01), but not among users of desogestrel-containing pills ( P = .99). Observed changes in BMD among DMPA users differed from women who used either type of pill ( P CONCLUSION: Depot medroxyprogesterone acetate has an adverse effect on BMD, in comparison with OCs or nonhormonal methods, when used for 12 months. Results must be interpreted cautiously until it is determined whether these effects endure or are reversible.

5. Signing an Organ Donation Letter: The Prediction of Behavior From Behavioral Intentions1

The signing of an organ donation letter was examined among 149 unmarried university students. The letter was developed for the purpose of conveying one's wishes regarding posthumous donation to one's next of kin. Predictors of letter signing were examined that explain hypothesized inconsistencies between the behavioral intention to sign and actual letter-signing behavior. Respondents were offered the opportunity to sign and send the donation letter to their parents. As predicted, reported willingness to sign the letter was only moderately related to actual behavior. Decision confidence, organ donation knowledge, and perceived decision importance positively influenced signing, independent of willingness. Unfavorability toward defending one's donation decision to one's parents inhibited signing, even among those expressing a high initial willingness to sign.

This article presents a critical review of psychological perspectives on organ donation. The review considers individual decisions to donate organs posthumously and next-of-kin consent decisions. A theoretical analysis of intention to donate is presented for both types of donation decisions, and the literature is reviewed within the context of the proposed framework. Donation decisions are examined as a function of attitude toward donation and the religious, cultural, altruistic, normative, and knowledge-based beliefs that comprise the attitude. Consent decisions are primarily influenced by prior knowledge of the deceased individual's wishes. An alternative conceptual model is offered to explain the basis of consent decisions in the absence of this knowledge. Suggestions are offered to improve measurement strategy and to guide theoretically based organ donation research within selected disciplines of psychology.

The relationship between gender role identification and decision making orientations was examined. Decision making was analyzed in terms of decision making orientation (behavioral, cognitive, and emotional) and decision making skills (problem definition and formulation, generation of alternative solutions, choice process, solution implementation, and verification). The results revealed a 2-factor characterization of general decision making in terms of approach-avoidance tendencies and self-appraisals of decision skills. A masculinity model was supported for decision making orientation, with masculinity serving as the only significant predictor. In contrast, an androgyny model was supported for decision making skills, with masculinity and femininity serving as significant predictors.

BEACON is a Computer Aided Engineering environment which allows the user to graphically design a real time control system as a series of hierarchical block diagrams. The diagrams constitute a complete documentation of the designed system. These diagrams can thereafter be used to automatically generate both simulation and real-time computer code. BEACON has been used within the General Electric Company to generate production code since the first quarter of 1992. This paper will present the BEACON block-diagram editor, its graphical user interface, and its different modes of operation. The extensive hierarchical modeling capabilities of BEACON will be discussed.

The overall cost and cycle time for developing complex controllers is radically reduced if the analysis/simulation and/or real-time code is automatically generated from the engineering design. This eliminates time-consuming manual coding, and avoids the introduction of coding bugs. BEACON is a graphical Computer Aided Engineering environment which automatically translates block-diagrams into either simulation or real-time computer code. BEACON was developed at GE and has been in production use within the company since the first quarter of 1992. This paper will present the automatic code-generation capabilities of BEACON, including the ability to generate either FORTRAN simulation code or real-time Ada or 68020 code from the same diagram. The mapping between BEACON's graphical block-diagram "language" and the target eode will be explained.

This article presents a critical review of psychological perspectives on organ donation. The review considers individual decisions to donate organs posthumously and next-of-kin consent decisions. A theoretical analysis of intention to donate is presented for both types of donation decisions, and the literature is reviewed within the context of the proposed framework. Donation decisions are examined as a function of attitude toward donation and the religious, cultural, altruistic, normative, and knowledge-based beliefs that comprise the attitude. Consent decisions are primarily influenced by prior knowledge of the deceased individual's wishes. An alternative conceptual model is offered to explain the basis of consent decisions in the absence of this knowledge. Suggestions are offered to improve measurement strategy and to guide theoretically based organ donation research within selected disciplines of psychology.

Although studies of instructor response to student writing are numerous in both L1 and L2 contexts, the literature on student reaction to those comments is small in L2 research. Partly for this reason, we have examined ESL students' own preferences and views on feedback. We surveyed the attitudes of 59 students in four ESL-oriented classes. Our questionnaire elicited their opinions on the usefulness of various types of comments, the scope of teacher markings, responsibility in error marking and correction, and revision. Eight students were then selected for interview (either audio- or video-taped). The respondents can be divided into three categories, primarily according to their degrees of acceptance of revision and of teacher intervention in providing input: Receptors (46%), Semi-resistors (41%), and Resistors (13%). The survey revealed that as students progress from English language learners to apprentices in their chosen discipline, the more restricted is the role they assign to the language teacher.

In this Technical Design Report (TDR) we describe the SuperB detector that was to be installed on the SuperB e+e- high luminosity collider. The SuperB asymmetric collider, which was to be constructed on the Tor Vergata campus near the INFN Frascati National Laboratory, was designed to operate both at the Upsilon(4S) center-of-mass energy with a luminosity of 10^{36} cm^{-2}s^{-1} and at the tau/charm production threshold with a luminosity of 10^{35} cm^{-2}s^{-1}. This high luminosity, producing a data sample about a factor 100 larger than present B Factories, would allow investigation of new physics effects in rare decays, CP Violation and Lepton Flavour Violation. This document details the detector design presented in the Conceptual Design Report (CDR) in 2007. The R&D and engineering studies performed to arrive at the full detector design are described, and an updated cost estimate is presented. A combination of a more realistic cost estimates and the unavailability of funds due of the global economic climate led to a formal cancelation of the project on Nov 27, 2012.

- Abstract
One of the roles of a genetic counselor, when returning genetic test results for Parkinson's disease (PD), is to provide emotional support to the patient. However, whether or not these needs are being met in the genetic counseling setting is unknown. In this cross-sectional qualitative study, semi-structured interviews with 15 PD research participants were conducted to evaluate their emotional needs and expectations throughout the genetic counseling process. Interview questions assessed participants' background understanding of genetic counseling, informational and emotional expectations prior to the genetic counseling session, and experience with emotional support throughout the genetic counseling process. Through reflexive thematic analysis, we defined four major themes: (1) knowledge of genetic counselors' role in education but not in emotional support, (2) limited expectations for emotional support, (3) emotional support is not sought from genetic counselors, and (4) emotional support from genetic counselors was viewed ambivalently. One of the most important findings from this study was that regardless of whether participants had previously heard of genetic counseling, none of the participants knew that the provision of emotional support was an aspect of the genetic counselor's role. Although the majority of participants did not expect emotional support from their genetic counselor, over 60% of participants recognized that receiving emotional support is or could be important to them. Collectively, these findings highlight the necessity for enhanced patient education pertaining to the informational and emotional support that genetic counselors are trained to provide. We suggest that clarifying the role of emotional support with patients may increase comfort in disclosing their emotional needs with genetic counselors, improving patient-centered care overall., (© 2024 The Author(s). Abstract: Nurse practitioner faculty identified student struggles to gain diagnostic reasoning competence. The National Council of State Boards of Nursing developed a clinical judgment action model (CJAM) for the NCLEX (NextGen) that can be used in prelicensure nursing education to design educational interventions and has potential applicability toward diagnostic reasoning development in nurse practitioner students. The purpose of this project was to evaluate the impact of a didactic education intervention aligned with the CJAM on the development of clinical reasoning cognitive skills.
