10 Páginas.-- 7 Figuras.-- 5 Tablas, This work explores the feasibility of using waste cooking oils (WCO) as eco-lubricants. Five WCO from different food facilities were studied. Three of them were fractionated into both lighter and heavier fractions by molecular distillation. A comprehensive chemical characterization (fatty acids distribution, polar compounds and acidity) was carried out on all WCOs and their fractions, which led to set relationships with the oils’ properties (such as viscosity index, low temperature viscous flow behavior, oxidation resistance, etc.). It is worth mentioning the high viscosity index values found in waste cooking oils with both low total polar compounds and acidity level, as well as the benefit which acidity had on their fluidity at low temperature and their lubricity. Moreover, it was also noteworthy that the lighter fractions, merely constituted by free fatty acids (FFA), presented an improved oxidative resistance. The largest OOT enhancement, 12.4%, was found for the light fraction of a non-segregated oil. Moreover, a better thermal stability was shown by the heavier fractions. Finally, both fractions exhibited enhanced friction-reducing capability as compared to their parent WCO. The light fractions from a non-segregated oil, a fast food restaurant oil and a deep-fried food establishment oil yielded wear reductions of 11.7%, 44.3% and 36.8%, respectively. The Mexican Society of Pediatric Endocrinology presents recommendations for the diagnosis and treatment of precocious puberty (PP), a condition defined as the development of sexual characteristics due to an increase in pituitary gonadotropin secretion before 8 or 9 years of age in girls and boys, respectively.Three systematic reviews were conducted: controlled clinical trials on interventions for PP treatment, diagnostic tests, and observational studies on the long-term effects of PP. The quality evaluation and data extraction from the studies were conducted by two independent reviewers. The Scottish Intercollegiate Guidelines Network and the Oxford Center for Evidence-Based Medicine systems were used for grading the quality of evidence for recommendations on intervention and diagnosis, respectively. Recommendations were submitted to a consensus by a Delphi method and were validated by another 143 certified pediatric endocrinologists through an online questionnaire.The group generated 12 recommendations on the diagnosis of PP, seven on the diagnosis of secondary causes of PP, eight on interventions for inhibition of puberty, five on other interventions for PP treatment, and 14 for the monitoring and follow-up of these patients. The online questionnaires submitted to certified pediatric endocrinologists showed more than 90% of approval for each one of the recommendations.Although a high degree of consensus for the recommendations for diagnosis, treatment, and monitoring of PP among pediatric endocrinologists was achieved, most of these recommendations showed a low level of evidence.La Sociedad Mexicana de Endocrinología Pediátrica presenta recomendaciones para el diagnóstico y el tratamiento de la pubertad precoz (PP), condición definida como el desarrollo de caracteres sexuales por incremento en la secreción hipofisiaria de gonadotropinas antes de los 8 años en las niñas y de los 9 años en los niños.Se realizaron tres revisiones sistemáticas de ensayos clínicos controlados sobre intervenciones para el tratamiento de la PP, pruebas diagnósticas y estudios observacionales sobre efectos a largo plazo de la PP. La evaluación de la calidad de los estudios y la extracción de datos se realizó por pares. La evidencia se graduó con el sistema de la Scottish Intercollegiate Guidelines Network (SIGN) y del Oxford Centre for Evidence-Based Medicine (OCEBM) para las recomendaciones sobre la intervención y el diagnóstico, respectivamente. Las recomendaciones generadas se sometieron a un consenso por el método Delphi y fueron validadas por otros 143 endocrinólogos pediatras certificados mediante un cuestionario en línea.Mediante consenso se generaron 12 recomendaciones para el diagnóstico de PP, siete sobre diagnóstico de causas secundarias de PP, ocho sobre intervenciones para inhibición de la pubertad, cinco sobre otras intervenciones en PP y 14 para la monitorización del tratamiento y el seguimiento de estos pacientes. Se obtuvo más del 90% de aprobación para cada una de las recomendaciones por el grupo de endocrinólogos certificados que respondieron el cuestionario en línea.Si bien se logró un alto grado de consenso para las recomendaciones para el diagnóstico, el tratamiento y la monitorización de la PP entre los endocrinólogos pediatras, el nivel de evidencia para la mayoría de estas recomendaciones resultó bajo.

The methodology for port management in this study takes into account the stochastic nature of climate as well as of use and exploitation agents. It provides a probabilistic characterization of the performance of different alternatives in terms of a set of indicators that reflect the potential benefits for different interest groups. The joint distribution functions thus obtained for a set of pre-defined management alternatives are used to compare strategies and to make decisions, based on stochastic multi-criteria analysis. This method, which also addresses the uncertainty of decision-maker preferences, ranked the strategies, and at the same time, specified the typical decision-maker preferences for each strategy. In the analysis of a hypothetical port, four alternatives were designed as solutions for port congestion. The criteria selected were the service quality provided to vessels, the total goods handled and the profits accrued by the Port Authority. The criteria measuring the performance of each strategy were statistically analyzed first. Then, assuming the uniform distribution of decision-maker preferences, the alternatives were ranked, and the relative importance of each criterion in the decision-making process was obtained.

ResumenIntroducciónEl síndrome de Silver-Russell presenta restricción del crecimiento intrauterino y posnatal, macrocefalia relativa y asimetría, entre otras características. Es causado por mecanismos genéticos y epigenéticos en el cromosoma 11p15.5 en el 40% de los casos y por disomía uniparental materna del cromosoma 7 en el 10%.MétodosSe identificaron los pacientes con diagnóstico de síndrome de Silver-Russell del Hospital Infantil de México Federico Gómez atendidos de 1998 a 2012; se reevaluaron 20 pacientes según los criterios clínicos internacionales, y se confirmó el diagnóstico en nueve sujetos.ResultadosTodos los pacientes presentaron restricción del crecimiento intrauterino y talla baja, ambos criterios diagnósticos mayores. La macrocefalia relativa estuvo presente en el 78% y la asimetría corporal solo en el 33%. Otras características, como la acidosis tubular renal, estuvieron presentes en más del 50%.ConclusionesEl diagnóstico del síndrome de Silver-Russell es complejo, por lo que contar con criterios clínicos adecuados es fundamental. Dado que la talla baja es la principal solicitud de atención médica en este síndrome, es relevante establecer diagnósticos diferenciales y valorar el crecimiento y desarrollo de todos los pacientes para identificar a aquellos en quienes la talla baja forma parte de una entidad sindrómica y que serían candidatos para realizar estudios moleculares. Este abordaje tendrá implicaciones para su manejo, pronóstico y asesoramiento genético.AbstractBackgroundPatients with Silver-Russell syndrome suffer from severe intrauterine and postnatal growth retardation, relative macrocephaly and body asymmetry, among other characteristics. It is caused by several genetic and epigenetic mechanisms in 11p15.5 in 40% of the cases and maternal uniparental disomy of chromosome 7 in 10%.MethodsTwenty patients with a diagnosis of Silver-Russell syndrome who were seen at the HIMFG from 1998 to 2012, were evaluated according to international clinical criteria confirming the diagnosis in nine of the subjects.ResultsAll patients showed intrauterine and postnatal growth retardation and short stature, both considered as major criteria of Silver-Russell syndrome. Relative macrocephaly was present in 78% of the patients and asymmetry in 33%. Other characteristics such as renal tubular acidosis were present > 50% of the cases.ConclusionsThe clinical diagnosis of Silver-Russell syndrome is complex. Short stature is the main reason for seeking medical attention and is helpful in the identification of a differential diagnosis. This situation underlines the importance of growth and development evaluation of all patients and particularly in those with short stature to identify those cases that may require molecular studies, with implications in management, prognosis and genetic counseling.

The prevalence of overweight and obesity in children and adolescents is increasing in both the United States and Mexico.The goal of this article was to assess the efficacy and safety of sibutramine in obese Mexican adolescents.This was a 6-month, randomized, double-blind, placebo-controlled, prospective clinical trial of sibutramine QD. Male and female patients aged 14 to 18 years with sex-specific body mass index (BMI) for age and sex85th percentile were eligible. The primary end points for the trial were the baseline versus end point absolute values for body weight, BMI, and percentage of the initial BMI (%BMI); secondary end points were waist circumference and percentage of the initial waist circumference (%waist). These were measured at days -15, 0, 30, 60, 90, 120, 150, and 180 of the study. Quality of life was assessed at the study start and end using the 36-Item Short-Form Health Survey (SF-36) questionnaire. Blood pressure and heart rate were assessed, and adverse events (AEs) were recorded. Both groups received individually tailored diet and exercise programs.Forty-six patients (age range, 14-18 years) with a BMI95th percentile for age were included (sibutramine group, n = 23 [14 females, 9 males]; placebo group, n = 23 [12 females, 11 males]). Twenty-one patients in the sibutramine group and 19 patients in the placebo group completed the 6-month trial. Using the intent-to-treat data, weight (mean [SD]) in the sibutramine group changed from 92.5 (14.6) kg to 85.7 (14.4) kg, for a net weight loss of 7.3 kg (95% CI 4.6-9.9), a waist circumference loss of 8.0 cm (95% CI, 4.7-11.3), and a % BMI loss of 9.2% (95% CI, 6.9-11.6). In the placebo group, weight changed from 98.9 (22.7) kg to 94.6 (22.5) kg, a weight loss of 4.3 kg (95% CI, 1.7-6.9), a waist circumference loss of 3.8 cm (95% CI, 0.7-7.0), and a %BMI loss of 5.2% (95% CI, 2.4-7.9) (P0.05 for all intragroup comparisons; P0.05 for the intergroup comparisons). Mean (SD) scores on the SF-36 scale in the sibutramine group changed from 78.0 (13.3) at baseline to 84.8 (7.4) at study end (P0.05); the respective values in the placebo group were 82.8 (10.3) and 87.3 (7.6) (P0.05). At base-line, systolic blood pressure (SBP) was 116.7 (5.9) mm Hg in the sibutramine group and 118.3 (7.6) mm Hg in the placebo group; at end point, the respective SBPs were 112.4 (9.6) mm Hg and 112.6 (6.5) mm Hg. At baseline, diastolic blood pressure (DBP) was 78.9 (4.5) mm Hg in the sibutramine group and 79.5 (5.2) mm Hg in the placebo group; at end point, the respective DBPs were 73.5 (6.3) mm Hg and 76.6 (6.2) mm Hg. At baseline, heart rate was 76.3 (6.4) beats/min in the sibutramine group and 81.1 (9.5) beats/min in the placebo group; at end point, the respective findings were 79.8 (8.8) beats/min and 77.6 (8.6) beats/min (P0.05 for all preceding intergroup comparisons). One patient in the sibutramine group had increased blood pressure (at month 3) and 3 had increased heart rate (at months 1, 2, and 4); 2 patients receiving placebo had increased blood pressure (month 3) and 2 had increased heart rate (at months 1 and 3). These changes disappeared in 1 week and did not require treatment or trial suspension. Additionally, in the sibutramine group, 3 patients experienced 4 mild AEs: headache, dry mouth, headache with nausea, and headache with weakness and paleness (P0.05). In the placebo group, 3 patients experienced 4 mild AEs: 2 cases of headache, as well as 1 case of headache with somnolence and 1 case of headache with dry mouth (P0.05).Sibutramine 10 mg QD in addition to diet and exercise was effective and generally well tolerated in this population of obese Mexican adolescents.

Objective The objectives of this paper are to compare sexual function and distress in women with systemic lupus erythematosus (SLE) and in healthy controls; to determine the association between disease characteristics, quality of life, psychopathology and sexual function; and to compare sexual function and distress of women according to age (reproductive and nonreproductive-age women). Methods We conducted a cross-sectional study of 120 participants; 65 women had SLE (aged 18–65), and 55 were healthy, age-matched controls. The assessment included the Female Sexual Function Index (FSFI), Symptom Checklist-90-Revised (SCL-90-R), Short Form 36 health survey (SF-36), socio-demographic characteristics and the Systemic Lupus International Collaborating Clinics (SLICC) and SLE Disease Activity Index (SLEDAI) in SLE patients only. Results Of 65 eligible patients with SLE, 61 (94%) responded; of 55 control subjects, 53 (96%) responded. The FSFI total score and subscale scores for desire, arousal, lubrication, orgasm and pain were significantly lower in patients with SLE. More somatization, obsessive-compulsive symptoms, interpersonal sensitivity, depression, anxiety, hostility, phobia, paranoid ideation, psychoticism, the Positive Symptom Total (PST), Positive Score Discomfort Index (PSDI), the use of psychotropic drugs, general health, vitality, social function, emotional role and mental health were significantly associated with changes in the patient group’s sexuality. Multivariate analysis indicated that depression, PSDI and vitality were the variables significantly associated with low sexual function in patients with SLE. Conclusions Women with SLE reported significantly impaired sexual function compared with healthy controls. Impaired sexual function was associated with somatization, obsessive-compulsive behavior, interpersonal sensitivity, depression, hostility, paranoid ideation, psychoticism, PST, higher scores in the PSDI subscale, vitality, social functioning and mental health. These results indicate that, in daily practice, inquiring about sexuality and quality of life and screening for psychopathology are important for every patient with SLE, irrespective of their clinical characteristics.

Since 1987, when the Strategic Highway Research Program (SHRP) arose, rheometry has rapidly gained acceptance as a powerful technique for measuring mechanical properties of bitumen, which can be directly related to in-service performance. Neat bitumen and polymer-modified bitumen rheology is highly dependent on temperature. Thus, these binders should have low viscosity when handled and mixed with mineral aggregates at high temperatures. At intermediate temperature, binder linear viscoelastic behaviour is related to its resistance to traffic loading. Finally, at or below the glass transition temperature, thermal cracking is likely to occur under certain loading conditions. In this regard, this chapter first presents detailed information on general rheology and its application to the study of polymer-modified bitumen, which readers might find of great interest for the understanding of some other chapters in this book. In addition, two illustrative case studies are also envisaged. On the one hand, the rheological characterisation, and modelling, of bitumen modified by different elastomers and plastomers is described. on the other hand, the rheological behaviour of synthetic binders, mixtures of oil, resin and polymer with similar characteristics to bitumen but easily pigmented, is reviewed.

This chapter deals with the modification of petroleum bitumen with different types of waste polymers: EVA, EVA/LDPE blend, PE, PP, crumb tyre rubber (CTR) and ABS, all of them coming from recycling plants of waste plastic materials. In addition, the influence of processing conditions (temperature, mixing device, etc.) on the rheological properties and microstructure is also summarised. From the experimental results obtained, it can be deduced that CTR and polyolefins may enhance bitumen thermo-rheological behaviour. However, much more enhanced mechanical properties can be obtained using a combination of both CTR and polyolefin recycled polymers. In that sense, CTR would in the main improve material behaviour at low in-service temperatures and polyolefins would mainly improve high in-service temperature behaviour. Regarding the processing conditions, the mixing device used (low or high shear) strongly affects the mechanical behaviour and microstructure of the blend obtained, especially in the case of polyolefins. For modified bitumen with CTR, no influence of the processing device was observed at temperatures equal to or below 180°C, since the processing conditions used are not able to break up the crosslinked network of the rubber.

Classically, the non stationary lithofacies distribution inside reservoirs is described by the 3D distribution of their proportions. This approach is very attractive because proportions have a physical meaning. Moreover their 3D distribution reflects the qualitative information coming from geology or the quantitative constraints derived from seismic attributes. In models such as the truncated gaussian, the proportions are directly used in the simulation process. In object-based models, such as the Boolean model, the problem is more complex because the proportions are the results of two sets of parameters: the object description (shape and dimensions) and their 3D distribution. The non stationarity in an object-based method can be reproduced either by using a non stationary object description or through a regionalized distribution of the objects. In this paper, we focus on the latter approach. The main contribution of the proposed method is the fact that the fit of the intensity point distribution is obtained globally in one computation step for any non stationary facies distribution. The interest is to constrain, a priori, the lithofacies simulation by a given 3D proportion distribution and not by convergence during the simulation process.

A methodology for port management is proposed in this work. It takes into account the uncertainty of the port perfor- mance due to the action of random forcing agents, namely, climate agents and exploitation agents. It can be used to obtain optimal solutions for port management problems in which operationality and safety are involved. Multivariate simulation techniques are used to obtain realizations of the random processes that characterize the forcing agents. The port performance is then evaluated by a simulation model. The methodology is applied in a hypothetical port in which the influence of the climate thresholds on operational performance and safety is evaluated by the port simulation model. An optimization problem is then formulated and solved in order to reduce waiting times and costs without compromise the safety levels.
