Purpose: Over 6 million people in the United States participate in martial arts, and the focus on striking the face poses a significant risk of injury to the eye. The objective of this study is to characterize the nature and incidence of martial arts-related eye injuries in the United States., Methods: A retrospective analysis of the National Electronic Injury Surveillance System (NEISS) database served as the basis of the study. The NEISS database was queried for eye injuries related to martial arts from 2012 to 2021. The entries that resulted were coded for demographic information, mechanism of injury, martial arts discipline, and diagnosis. Data were analyzed using SPSS version 28 through descriptive and χ 2 analyses., Results: There were 162 reported cases of martial arts-related eye injuries in 4- to 62-years-old patients with most injuries occurring in males (84%). The most represented martial arts discipline was boxing (30.5%), and the most frequently reported mechanism of injury was punching (30.2%). The most frequently reported diagnosis resulting from eye injury was corneal abrasion (40.5%), while sight-threatening diagnoses were reported, including retinal detachment (n = 4), vitreous hemorrhage (n = 2), and open globe (n = 2)., Conclusions: Despite the significant risk of ocular injury from martial arts, corneal abrasion was the most commonly diagnosed with excellent prognosis. Sight-threatening diagnoses were also reported, indicating that martial arts athletes should be educated about the risks of eye injury and the importance of regular eye examination when practicing combat sports., (Copyright © 2024 Canadian Ophthalmological Society. Objective: To determine the best method of antimicrobial prophylaxis against implanted material-associated infections in the setting of scleral buckle surgery., Design: Experimental study., Participants: Scleral buckle elements were soaked in either gram-positive or polymicrobial broth, while control buckle elements were soaked in PBS only., Methods: Solid silicone and sponge scleral buckle elements were inoculated with common pathogens of the ocular surface, and then soaked in either 1% or 5% povidone-iodine, 1 mg/mL gentamicin solution, or sterile saline for 1, 5, 10, or 15 minutes. Bacteria were then isolated from the buckle elements and cultured for 24 hours., Results: In all gram-positive bacterial conditions, gentamicin solution decreased the bacterial load from 451,666.67 colony-forming units (CFU)/mL to 171,611.11 CFU/mL (p=0.0004). The fractional bacterial survival after soaking in gentamicin was higher for the silicone sponge than band (0.357 vs 0.079, p=0.038). Both 1% and 5% povidone-iodine were able to completely eradicate all gram-positive bacteria of both buckle elements. Only 5% povidone-iodine was able to completely sterilize all microbes on the buckle after soaking in a polymicrobial solution consisting of gram-positive, gram-negative bacteria, and fungi., Conclusion: Povidone-iodine solution was significantly more effective at bacterial eradication compared to gentamicin solution. For all scleral buckle procedures, we recommend soaking the buckle element in 2-3% povidone-iodine solution before placement and rinsing the ocular surface with the same solution after placement., Competing Interests: The authors declare that there are no conflicts of interest., (© 2021 Lin et al.)

Patients with photosensitive disorders of the skin may present with ocular manifestations that are evident at birth or may be manifested later with progression of the disorder. Dermatologists should be able to recognize these and appropriately refer patients for further management. Ocular involvement associated with immunologically mediated photodermatoses, drug- and chemical-induced photosensitivity, photodermatoses associated with defective DNA repair/chromosome instability, and photoaggravated dermatoses are reviewed. Photodermatoses are commonly classified into four general groups: (1) immunologically mediated photodermatoses; (2) drug- and chemical-induced photosensitivity; (3) photodermatoses associated with defective DNA repair/chromosome instability; and (4) photoaggravated dermatoses. Purpose: To determine how well patients understand the hierarchy of a teaching hospital, what factors influence patient knowledge, and how patients feel about their role in training physicians., Design: Prospective cross-sectional survey., Participants: A total of 312 patients attending 1 of 3 ophthalmology clinics at the Kresge Eye Institute in July 2006., Methods: Survey of knowledge about the training and responsibilities of medical students, residents, and attending physicians was conducted, as well as subjective evaluation regarding patient comfort with treatment by physicians in training. Knowledge Score was correlated with subjective Comfort Score and demographics using analysis of variance., Main Outcome Measures: Knowledge Score and subjective Comfort Score., Results: A total of 312 surveys were completed. Some 56% of questions were answered correctly. Higher education was the only significant factor for increased Knowledge Score (P<0.001). Respondents with a higher Knowledge Score had a correspondingly higher Comfort Score (P = 0.004) and are significantly more comfortable with treatment provided by physicians in training (residents). Clinic site (P = 0.002) and increasing age (P = 0.02) are also significant independent factors for increased comfort with treatment provided by physicians in training., Conclusions: Patients have moderate knowledge of the training and hierarchy of medical students, residents, and attending physicians. Increased patient knowledge is a significant independent factor for increased patient comfort with treatment by physicians in training. Objective: To determine the safety and efficacy of topical aminocaproic acid (Caprogel) in the management of traumatic hyphema., Design: Multicenter, randomized, double-masked, placebo-controlled clinical trial., Participants: A total of 51 patients participated in this trial (power = 36%, 2-tailed test)., Intervention: Patients presenting with traumatic hyphema were randomly assigned to 5-day treatment with topical aminocaproic acid or a placebo gel. Patients were monitored daily with ocular examination and vital sign testing for the 5 days of treatment and at 24 and 48 hours after treatment. General physical examination and laboratory testing were performed at baseline and day 5., Main Outcome Measures: The main efficacy variable was the rate of rebleeding. Secondary efficacy variables included time to hyphema clearance, intraocular pressure, time to secondary hemorrhage, and visual acuity. Safety variables included adverse events, vital signs, and laboratory measurements., Results: Rebleeding occurred in 30% of the placebo group (8 of 27; 95% confidence interval [CI] = 14-50%), versus 8% of the treatment group (2 of 24; 95% CI = 1-27%), for an estimated continuity-corrected difference in percentage of patients with bleeding of 17% (95% CI = -3-38%). Secondary efficacy variables were similar in the groups, except that there was a trend towards more visual improvement in the topical aminocaproic acid group (54%) than in the placebo group (30%) at the last measurement (P = 0.08). Adverse events were similar., Conclusions: This study provides evidence that topical aminocaproic acid is safe and demonstrates trends towards reducing the rebleeding rate in the management of traumatic hyphema. However, because the study was terminated before complete enrollment, more definitive recommendations will require a larger trial.

We studied the nonenzymatic glycosylation of lens epithelial basement membranes (LEBM) of senile cataractous lenses of both diabetic and nondiabetic patients. The human LEBMs were isolated from surgically removed senile cataracts and purified by osmotic lysis and detergent treatments. Glycosylation assay of LEBMs was done using the colorimetric method of Flückiger and Winterhalter. The glycosylation value ranged from 16.39 to 92.56 n mol/mg protein overall, with a mean of 63.54 +/- 24.56 n mol/mg protein for the diabetic specimens and a mean of 29.97 +/- 14.48 n mol/mg protein for the nondiabetic controls (P = 0.009). The study confirms our previous observation of in vivo glycosylation of the LEBM and further establishes that diabetic patients have a twofold increase in the amount of LEBM glycosylation when compared to their nondiabetic counterparts.
