1. Hépatite herpétique avec syndrome d’activation macrophagique chez un patient immunocompétent

Abstract

Resume Introduction L’hepatite herpetique est une cause rare d’hepatite aigue chez les patients immunocompetents. La triade fievre, augmentation des transaminases et leucopenie est caracteristique d’une hepatite herpetique. Le retard diagnostique et de prescription du traitement antiviral sont des facteurs majeurs d’evolution defavorable. Observation Nous rapportons une observation rare d’hepatite aigue grave d’origine herpetique chez un patient immunocompetent âge de 50 ans, qui survenait a la suite de la reactivation d’une infection par virus herpes simplex. Cette hepatite se compliquait d’un syndrome d’activation macrophagique avec coagulation intravasculaire disseminee grave. L’evolution etait favorable grâce a un traitement precoce par acyclovir. Conclusion Malgre sa faible incidence, l’hepatite herpetique devrait etre evoquee chez les patients immunocompetents presentant une insuffisance hepatique aigue. Etant donne l’interet d’un traitement precoce par acyclovir dans cette situation, il est important que les cliniciens evoquent ce diagnostic rare et realisent rapidement une PCR-HSV serique.

Abstract

Resume Les troubles du rythme supraventriculaires, frequents en reanimation, sont d’origine multifactorielle. Ils sont associes a une morbimortalite importante et necessitent une prise en charge precoce et precise. La demarche diagnostique et therapeutique pour ces troubles du rythme supraventriculaires commence par l’evaluation de leur retentissement hemodynamique qui peut conduire a la realisation urgente d’un choc electrique externe. Cependant, si l’etat hemodynamique le permet, differents traitements medicamenteux peuvent etre entrepris en fonction de l’analyse precise des donnees de l’ECG apres prise en compte et correction d’un certain nombre de facteurs favorisants (hypokaliemie, hypoxie, hypovolemie…). Le traitement antiarythmique ideal n’existe pas, mais de nombreux antiarythmiques sont en cours de developpement dans le but de minimiser les effets secondaires ventriculaires et de limiter la toxicite extracardiaque.

Abstract

Herpes simplex hepatitis is a rare cause of acute hepatitis in immunocompetent patients. The triad of fever, increase in liver enzymes and leucopenia is suggestive of herpes simplex hepatitis. Delayed diagnosis without antiviral therapy contributes significantly to an unfavorable outcome.We report a 50-year old immunocompetent male patient, who presented with acute severe hepatitis due to a reactivation of a herpes simplex infection with a complicated course including macrophage activation syndrome and severe coagulopathy. Outcome was finally favorable with early acyclovir therapy.Despite its relatively low occurrence rate, diagnosis of herpetic hepatitis should be discussed in immunocompetent patients with acute liver failure. The benefit of an early acyclovir treatment should lead clinicians to consider this uncommon diagnosis in unexplained cases of hepatitis and to test rapidly HSV DNA levels by PCR in plasma.

Abstract

In this report, we review two cases of brain infection due to Dialister pneumosintes in previously healthy patients. The bacterium was isolated from the first patient by blood culture and directly from a brain abscess in the second patient. In both cases, the infection was suspected to be of nasopharyngeal or dental origin. The patients had favorable outcomes following surgical debridement and antibiotic treatment. After in vitro amplification and partial sequencing of the 16S rRNA gene, two strains were classified as D. pneumosintes . However, traditional biochemical tests were not sufficient to identify the bacteria. In addition to causing periodontal and opportunistic infections, D. pneumosintes, contained in mixed flora, may behave as a clinically important pathogen, especially in the brain. In addition to phenotypic characterization, 16S rRNA partial sequencing was used to identify D. pneumosintes definitively.
