International audience; Mini-EUSO is a high-sensitivity imaging telescope that observes the Earth from the ISS in the near ultraviolet band (290÷ 430 nm), through the nadir-facing, UV-transparent window in the Russian Zvezda module. The instrument, launched in 2019, has a field of view of 44∘, a spatial resolution on the Earth’s surface of 6.3 km and a temporal sampling rate of 2.5 microseconds. Thanks to its triggering and on-board processing, the telescope is capable of detecting UV emissions of cosmic, atmospheric, and terrestrial origin on different time scales, from a few microseconds up to tens of milliseconds. The optics is composed of two Fresnel lenses focusing light onto an array of 36 Hamamatsu Multi-Anode PhotoMultiplier Tubes, for a total of 2304 pixels. The telescope also contains two cameras in the near-infrared and visible, an 8-by-8 array of Silicon-PhotoMultipliers and a series of UV sensors to manage night-day transitions. The scientific objectives range from the observation of atmospheric phenomena [lightning, Transient Luminous Events (TLEs), ELVES], the study of meteoroids, the search of interstellar meteoroids and strange quark matter, mapping of the Earth’s nocturnal emissions in the ultraviolet range, and the search of cosmic rays with energy above 1021 eV. The instrument has been integrated and qualified in 2019, with the final tests in Baikonur prior to its launch. Operations involve periodic installation in the Zvezda module of the station with observations during the crew night time, with periodic downlink of data samples, with the full data being sent to the ground via pouches containing the data disks. Mission planning involves the selection of the optimal orbits to maximize the scientific return of the instrument. In this work, we will describe the various phases of construction, testing, and qualification prior to the launch and the in-flight operations of the instrument on board the ISS.

Mini-EUSO (Multiwavelength Imaging New Instrument for the Extreme Universe Space Observatory) is a telescope observing the Earth from the International Space Station since 2019. The instrument employs a Fresnel-lens optical system and a focal surface composed of 36 multi-anode photomultiplier tubes, 64 channels each, for a total of 2304 channels with single photon counting sensitivity. Mini-EUSO also contains two ancillary cameras to complement measurements in the near infrared and visible ranges. The scientific objectives of the mission range from the search for extensive air showers generated by Ultra-High Energy Cosmic Rays (UHECRs) with energies above 10$^{21}$ eV, the search for nuclearites and Strange Quark Matter (SQM), up to the study of atmospheric phenomena such as Transient Luminous Events (TLEs), meteors and meteoroids. Mini-EUSO can map the night-time Earth in the near UV range (between 290-430 nm) with a spatial resolution of about 6.3 km (full field of view of 44{\deg}) and a maximum temporal resolution of 2.5 $\mu$s, observing our planet through a nadir-facing UV-transparent window in the Russian Zvezda module. The detector saves triggered transient phenomena with a sampling rate of 2.5 $\mu$s and 320 $\mu$s, as well as continuous acquisition at 40.96 ms scale. In this paper we discuss the detector response and the flat-fielding and calibration procedures. Using the 40.96 ms data, we present $\simeq$6.3 km resolution night-time Earth maps in the UV band, and report on various emissions of anthropogenic and natural origin. We measure ionospheric airglow emissions of dark moonless nights over the sea and ground, studying the effect of clouds, moonlight, and artificial (towns, boats) lights. Mini-EUSO is part of the JEM-EUSO program and operates on board the International Space Station (ISS). It is a UV-telescope with single-photon counting capability looking at nighttime downwards to the Earth through a nadir-facing UV-transparent window. As part of the pre-flight tests, the Mini-EUSO engineering model, a telescope with 1/9 of the original focal surface and a lens of 2.5 cm diameter, has been built and tested. Tests of the Mini-EUSO engineering model have been made in laboratory and in open-sky conditions. Laboratory tests have been performed at the TurLab facility, located at the Physics Department of the University of Turin, equipped with a rotating tank containing different types of materials and light sources. In this way, the configuration for the observation of the Earth from space was emulated, including the Mini-EUSO trigger schemes. In addition to the qualification and calibration tests, the Mini-EUSO engineering model has also been used to evaluate the possibility of using a JEM-EUSO-type detector for applications such as observation of space debris. Furthermore, observations in open-sky conditions allowed the studies of natural light sources such as stars, meteors, planets, and artificial light sources such as airplanes, satellites reflecting the sunlight, and city lights. Most of these targets could be detected also with Mini-EUSO. In this paper, the tests in laboratory and in open-sky conditions are reported, as well as the obtained results. In addition, the contribution that such tests provided to foresee and improve the performance of Mini-EUSO on board the ISS is discussed.

International audience; The trigger logic of the Tracking Ultraviolet Setup (TUS) and Multiwavelength Imaging New Instrument for the Extreme Universe Space Observatory (Mini-EUSO) space-based projects of the Joint Experiment Missions - EUSO (JEM-EUSO) program is summarized. The performance results on the search for ultra-high energy cosmic rays are presented.

Background Pulmonary vein isolation by cryoablation (PVI-C) is a standard therapy for the treatment of atrial fibrillation (AF) with high safety and efficacy profiles. The CHA2DS2 -VASc score is also a well-established predictor of AF stroke. Importantly, the role of CHA2DS2-VASc score is still unknown regarding the long-term clinical outcomes following PVI-C. Purpose The aim of this analysis is to evaluate the role of CHA2DS2-VASc score in predicting AF recurrence during long term follow up after PVI-C. Methods Patients with recurrent and symptomatic AF underwent an index PVI-C. Data were collected prospectively in the framework of 1STOP ClinicalService® project. Patients were categorized into two groups Low risk (LR) and high risk (HR) (CHA2DS2-VASc score = 0-1 and CHA2DS2 -VASc score ≥2, respectively). Results Out of 3313 patients, 1910 (57.6%) had CHA2DS2-VASc score = 0-1, while 1403 (42.3%) were in the HR score group (CHA2DS2 -VASc score ≥2). As expected, patient baseline characteristics were significantly different between the two cohorts including: age, sex, BMI, percentage of paroxysmal AF, history of stroke, diabetes, ischemic cardiomyopathy. On the contrary, procedural times and the incidence of acute complications were comparable between the two groups. The 36-month freedom from AF after a single procedure was 72.5% (95% CI 69.8% - 75.0%) in the LR group and 65.9% (62.3% - 69.2%) in the HR score group (HR: 1.26, 95% CI 1.08-1.47, p= 0003, Figure 1). At multivariate analysis, higher CHA2DS2 -VASc score was still a significant predictor of the risk of AF recurrence [HR: 1.33 (1.10 - 1.60, p=0.003] Conclusions PVI-C is highly effective in the treatment of AF over the long term. A CHA2DS2 -VASc score ≥2 is an independent predictor of AF recurrence during the follow-up and should be taken into account in the clinical management after the index procedure.

International audience; The Mini-EUSO telescope was launched for the International Space Station on August 22nd, 2019 to observe from the ISS orbit (∼400 km altitude) various phenomena occurring in the Earth’s atmosphere through a UV-transparent window located in the Russian Zvezda Module. Mini-EUSO is based on a set of two Fresnel lenses of 25 cm diameter each and a focal plane of 48×48 pixels, for a total field of view of 44°. Until July 2021, Mini-EUSO performed a total of 41 data acquisition sessions, obtaining UV images of the Earth in the 290 nm – 430 nm band with temporal and spatial resolution on ground of 2.5 μs and 6.3×6.3 km2, respectively. The data acquisition was performed with a 2.5 μs sampling rate, using a dedicated trigger looking for signals with a typical duration of tens of μs. In the present paper the analysis of the performance of the 2.5 μs trigger logic is presented, with a focus on the method used for the analysis and the categories of triggered events. The expected functioning of the trigger logic has been confirmed, with the trigger rate on spurious events that remains within the requirements in nominal background conditions. The trigger logic detected several different phenomena, including lightning strikes, elves, ground-based flashers and events with EAS-like characteristics.

Mini-EUSO is a small orbital telescope with a field of view of $44^{\circ}\times 44^{\circ}$, observing the night-time Earth mostly in 320-420 nm band. Its time resolution spanning from microseconds (triggered) to milliseconds (untriggered) and more than $300\times 300$ km of the ground covered, already allowed it to register thousands of meteors. Such detections make the telescope a suitable tool in the search for hypothetical heavy compact objects, which would leave trails of light in the atmosphere due to their high density and speed. The most prominent example are the nuclearites -- hypothetical lumps of strange quark matter that could be stabler and denser than the nuclear matter. International audience; The second generation Extreme Universe Space Observatory on a Super-Pressure Balloon (EUSO-SPB2) mission is on track to be approved by NASA for a long duration flight (up to 100 days) in 2023 from Wanaka, New Zealand. EUSO-SPB2 will monitor the night atmosphere of the Southern hemisphere from 33 km altitude to detect Ultra-High Energy Cosmic Rays (UHECRs) and search for astrophysical and cosmogenic neutrinos. EUSO-SPB2 features two telescopes, a Cherenkov (CT) and a Fluorescence (FT) Telescope. The hardware implementation and performance of the FT trigger logic were tested at the TurLab facility, hosted at the University of Turin. The tests proved the correct hardware implementation of the trigger logic and its ability to detect μs timescale transient signals.

Mini-EUSO is the first detector of the JEM-EUSO program deployed on the ISS. It is a wide field of view telescope currently operating from a nadir-facing UV-transparent window on the ISS. It is based on an array of MAPMTs working in photon counting mode with a 2.5 μs time resolution. Among the different scientific objectives it searches for light signals with time duration compatible to those expected from Extensive Air Showers (EAS) generated by EECRs interacting in the atmosphere. Although the energy threshold for cosmic ray showers is above $E > 10^{21} eV$, due the constraints given by the size of the UV-transparent window, the dedicated trigger logic has been capable of the detection of other interesting classes of events, like elves and ground flashers. An overview of the general performance of the trigger system is provided, with a particular focus on the identification of classes of events responsible for the triggers.

We report of a series of laboratory experiments and numerical simulations of freely-decaying rotating turbulent flows confined in domains with variable height. We show that the vertical confinement has important effects on the formation of large-scale columnar vortices, the hallmark of rotating turbulence, and in particular delays the development of the cyclone-anticyclone asymmetry. We compare the experimental and numerical results face-to-face, showing the robustness of the results., 12 pages, 10 figures

International audience; Mini-EUSO is a mission of the JEM-EUSO program flying onboard the International Space Station since August 2019. Since the first data acquisition in October 2019, more than 35 sessions have been performed for a total of 52 hours of observations. The detector has been observing Earth at night-time in the UV range and detected a wide variety of transient sources all of which have been modeled through Monte Carlo simulations. Mini-EUSO is also capable of detecting meteors and potentially space debris and we performed simulations for such events to estimatetheir impact on future missions for cosmic ray science from space. We show here examples of the simulation work done in this framework to analyze the Mini-EUSO data. The expected response of Mini-EUSO with respect to ultra high energy cosmic ray showers has been studied. The efficiency curve of Mini-EUSO as a function of primary energy has been estimated and the energy threshold for Cosmic Rays has been placed to be above $10^{21}$ eV. We compared the morphology of several transient events detected during the mission with cosmic ray simulations and excluded that they can be due to cosmic ray showers. To validate the energy threshold of the detector, asystem of ground based flashers is being used for end-to-end calibration purposes. We therefore implemented a parameterization of such flashers into the JEM-EUSO simulation framework and studied the response of the detector with respect to such sources.

EUSO-SPB1 was a balloon-borne mission of the JEM-EUSO (Joint Experiment Missions for Extreme Universe Space Observatory) Program aiming at the ultra-high energy cosmic ray (UHECR) observations from space. We operated the EUSO-SPB1 telescope consisting of 1 m2 Fresnel refractive optics and multi-anode photomultiplier tubes. With a total of 2304 channels, each performed the photon counting every 2.5 µs, allowing for spatiotemporal imaging of the air shower events in an ~ 11°× 11° field of view. EUSO-SPB1 was the first balloon-borne fluorescence detector with a potential to detect air shower events initiated by the EeV energy cosmic rays. On 24 April 2017 UTC, EUSO-SPB1 was launched on the NASA’s Super Pressure Balloon that flew at ~16 – 33 km flight height for ~12 days. Before the flight was terminated, ~27 hours of data acquired in the air shower detection mode were transmitted to the ground. In the present work, we aim at evaluating the role of the clouds during the operation of EUSO-SPB1. We employ the WRF (Weather Research and Forecasting) model to numerically simulate the cloud distribution below EUSO-SPB1. We discuss the key results of the WRF model and the impact of the clouds on the air shower measurement and the efficiency of the cosmic ray observation. The present work is a part of the collaborative effort to estimate the exposure for air shower detections.

The Plateau Rosa Alpine station (Italy) has collected atmospheric concentrations of carbon dioxide since 1989. If the complete set of hourly data is observed, two distinct and exceptional very high concentration events are evident for February 2004. Similar and almost contemporary peaks were registered at the European high–altitude stations of Zugspitze–Schneefernerhaus and Sonnblick in the Alps, and at Mt. Cimone in the Northern Apennines. A regional meteorological model (the Weather Research and Forecast) was applied over a medium–high resolution grid to study the evolution of the meteorological fields and to identify the trajectories of the polluted air masses during the CO2 observed peaks. The results show that, during both episodes, atmospheric circulation conveyed highly polluted air from the European plains to the Alpine stations. This conclusion has been also confirmed through concentration measurements of the atmospheric trace gases in the same area.

The probability of occurrence of rogue waves in wind-generated fields is investigated experimentally in an annular wind-wave flume. Unlike many experiments on rogue waves, where waves are mechanically generated, here the wave field is forced naturally by wind as it is in the ocean. The peculiar geometry of the flume makes waves propagating circularly in an unlimited-fetch condition. Water surface elevation was measured at specific cross-sections under the effect of different wind speeds to monitor the temporal evolution of the wave field. Results show that the kurtosis of the surface elevation, the fourth order moment of the probability density function and a measure of the percentage of extremes in a wave record, gradually increases in time with the evolution of the wave field. Deviations from Normal statistics are observed to be a function of wind speed. The maximum departure from Normality resembled the one induced by quasi-resonant wave-wave interactions and it is observed at the final stage of wave growth and immediately before reaching the fully development regime.

International audience; The TurLab facility is a laboratory, equipped with a 5 m diameter and 1 m deep rotating tank,located in the Physics Department of the University of Turin. Originally built mainly to study problems where system rotation plays a key role in the fluid behaviour such as in atmospheric and oceanic flows at different scales, in the past few years the TurLab facility has been used to perform experiments related to observation of Extreme Energy Cosmic Rays from space using the fluo-rescence technique, as in the case of the JEM-EUSO mission, where the diffuse night brightness and artificial light sources can vary significantly in time and space inside the Field of View of the telescope. The description of the EUSO@TurLab project and its first results have been presented in the past. During the last two years many upgrades have been performed on the instrumenta-tion mainly related to the read-out electronics: SPACIROC-1 (employed in EUSO-Balloon and EUSO-TA prototypes) and SPACIROC-3 (EUSO-SPB and Mini-EUSO) which allowed to test a fully equipped Elementary Cell of JEM-EUSO. This phase has been named Phase II. Moreover, the Focal Surface of EUSO-Balloon with the level 1 trigger logic implemented in the Photo-Detector Module has been tested at TurLab after the Canada flight. Finally, tests related to the possibility to employ a EUSO-like detector for other type of applications such as wave monitor-ing and imaging detector have been pursued. The tests and results obtained in EUSO@TurLab Project - Phase II are described.

International audience; The JEM-EUSO Collaboration aims to study Ultra-High-Energy Cosmic Ray (UHECR) using a novel approach of looking down from space and using the atmosphere as a large detector to achieve a large effective area and therefore high-statistics on these events for the first time. For this purpose we have been developing a series of pathfinders operating from the ground, high altitude balloons and space, all sharing the same electronics, in particular the central data acquisition system. We report on the implementation and successful testing of the first-level trigger (L1) within the FPGA of the photo-detection module (PDM) board, which processes the signals from 36 64-pixel MAPMTs with a time unit of 2.5 μs to detect the passage of light from cosmic ray generated air-showers.

Through a retrospective study concerning the experience of our center in patients affected by Neurally Mediated reflex Syncope (NMS) we wanted to verify not only the diagnostic yield of the Implantable Loop Recorder (ILR) but its possible placebo therapeutic effect. In the context of patients affected by a severe clinical presentation of NMS identified through a careful clinical evaluation, we selected those who followed a diagnostic iter using the ILR. We analysed 84 patients (39 male and 45 female, mean age 71 years), during the period 2009-2013. 34 patients (40.5%) had no recurrences after a mean follow-up (FU) of 35 months, among these 17 concluded a FU of 4 years. 50 patients (59.5%) had recurrences and a specific diagnosis after an average period of 7 months. We found an important number of patients who showed a disappearance of syncope during an observation period of 2-3 and 4 years. At first glance this results could be explained considering the possible placebo therapeutic effect of ILR.

Vertigo and dizziness are common symptoms in the general population, with an estimated prevalence between 20% and 56%. The aim of our work was to assess the point prevalence of these symptoms in a population of 2672 subjects. Patients were asked to answer a questionnaire; in the first part they were asked about demographic data and previous vertigo and or dizziness. Mean age of the sample was 48.3 ± 15 years, and 46.7% were males. A total of 1077 (40.3%) subjects referred vertigo/dizziness during their lifetime, and the mean age of the first vertigo attack was 39.2 ± 15.4 years; in the second part they were asked about the characteristics of vertigo (age of first episode, rotational vertigo, relapsing episodes, positional exacerbation, presence of cochlear symptoms) and lifetime presence of moderate to severe headache and its clinical features (hemicranial, pulsatile, associated with phono and photophobia, worse on effort). An age and sex effect was demonstrated, with symptoms 4.4 times more elevated in females and 1.8 times in people over 50 years. In the total sample of 2672 responders, 13.7% referred a sensation of spinning, 26.3% relapsing episodes, 12.9% positional exacerbation and 4.8% cochlear symptoms; 34.8% referred headache during their lifetime. Subjects suffering from headache presented an increased rate of relapsing episodes, positional exacerbation, cochlear symptoms and a lower age of occurrence of the first vertigo/dizziness episode. In the discussion, our data are compared with those of previous studies, and we underline the relationship between vertigo/dizziness from one side and headache with migrainous features on the other.La vertigine e l’instabilità sono sintomi molto comuni nella popolazione la cui prevalenza è stimata tra il 20 e il 56%. L’obiettivo del nostro lavoro è stato quello di determinare la prevalenza di questi sintomi in una popolazione di 2672 soggetti. È stato somministrato loro loro un questionario; nella prima parte sono stati richiesti i dati demografici e se avessero mai sofferto di vertigine o instabilità nella loro vita. L’età media del campione è stata di 48,3 ± 15 anni, il 46,7% erano maschi. Sul totale della popolazione 1077 (40,3%) hanno riferito di aver sofferto di vertigine o instabilità nella loro vita, con un primo episodio occorso all’età di 39,2 ± 15,4 anni. Nella seconda parte del questionario sono state indagate le caratteristiche delle vertigini (età del primo episodio, il tipo di vertigine, presenza di più episodi, esacerbazione posizionale della vertigine, presenza di sintomi cocleari infine la presenza di cefalea da moderata o severa nel corso della vita e le sue caratteristiche cliniche (riferita a un emicrania, pulsante, associata a fono o fotofobia, peggiore con l’attività fisica). È stata osservata una correlazione della vertigine con l’età e con il sesso, essendo la prima 4,4 volte più frequente nelle donne e 1,8 volte nei soggetti con oltre 50 anni. Sul campione complessivo di 2672 soggetti, 13,7% hanno riferito vertigine rotatoria, 26,3% episodi recidivanti, 12,9% esacerbazione correlata alla posizione e il 4,8% presenza sintomi cocleari; il 34,8% ha lamentato cefalea nel corso della loro vita. I soggetti affetti da cefalea presentavano un’incidenza aumentata di vertigini recidivanti, di esacerbazione correlata alla posizione, di sintomi cocleari e un’età più giovane di comparsa del primo episodio di vertigine/instabilità. Nella discussione i nostri dati sono stati confrontati con quelli di precedenti studi. Gli autori sottolineano la correlazione tra vertigine/instabilità da un lato e cefalea con caratteristiche emicraniche dall’altro.

The speed of sound in moist air is discussed and a more accurate value for the coefficient of the linear dependence of sonic temperature on specific humidity is proposed. An analysis of speed-of-sound data measured by three sonic anemometers in a climate chamber and in the field shows that the temperature response of each instrument significantly influences not only the determination of sonic temperature, but also its fluctuations. The corresponding relative contribution to the error in the evaluation of the temperature fluctuations and the turbulent heat fluxes can be as high as 40%. The calibration procedure is discussed and a method of correction is proposed.

The lapse rates of high-resolution temperature profiles during nearly neutral, saturated conditions are compared with the saturated adiabatic lapse rate and with that proposed by Richiardone and Giusti. A good agreement between the latter and the mean value of the observed lapse rate is found, whereas the saturated adiabatic lapse rate differs significantly, confirming experimentally that it is not completely correct to assess the moist neutrality from a comparison with the saturated adiabatic lapse rate. The lapse-rate distribution supports the hypothesis that the lapse-rate statistics is a local collection of saturated adiabatic lapse rates in a background normal distribution centered around the neutrality.

We describe the coordinate transformations that can be used to convert the velocity components measured by a set of sonic anemometers with time-dependent tilt fluctuations into a single, time-independent coordinate system. By applying the planar fit method (PFM) to each anemometer dataset, it is possible, for planar flows, to locate the flow plane at each measurement point and compare its orientation with the topography. Installation on a ship is also considered. An application of this method to intercomparison data has led to the detection of an instrument error due to a misalignment between the assembly of the sonic transducers and the anemometer pedestal. If this error occurs, pedestal levelling does not guarantee that measurements are unbiased. A correction method is proposed and the results of two experiments are shown. Flow planarity at different levels and flow distortion caused by the mast are highlighted. The influence of the error on the evaluation of the Reynolds stresses using PFM or the double rotation method and the triple rotation method is discussed and the tilt corrected stresses calculated using the three methods compared.

The EUSO@TurLab project is an on-going activity aimed to reproduce atmospheric and luminous conditions that JEM-EUSO will encounter on its orbits around the Earth. The use of the TurLab facility, part of the Department of Physics of the University of Torino, allows the simulation of different surface conditions in a very dark and rotating environment in order to test the response of JEM-EUSO's sensors and sensitivity. The experimental setup currently in operation has been used to check the potential of the TurLab facility for the above purposes, and the acquired data will be used to test the concept of JEM-EUSO's trigger system.

The turbulent velocity field of a dense fluid release has been measured in a water tank experiment carried out in the TURLAB laboratory in Turin (Italy). A vertical density driven current was created releasing a saline solution in a water tank with no mean flow. The experiment reproduces in physical similarity, based on the density Froud number, the release of a dense gas in the atmosphere and the PIV technique has been used to analyse the buoyancy generated velocity field. The high temporal and spatial resolution of the measurements gives a deep insight to the problems of the bouncing of dense gases and of the creation of the outflow velocity at the ground. The experimental findings are used to test and improve an original method for the dispersion of a positively and negatively buoyant plume. The method is based on the idea of Alessandrini and Ferrero (Phys. A 388:1375–1387, 2009) for the treatment of a background substance entrainment into the plume and it consists on the introduction of two fictitious scalars inside the Lagrangian Model SPRAY. The fictitious scalars represent the density and momentum difference between the plume and the environment air that naturally takes into account the interaction between the plume and the environment. As a consequence, no more particles than those inside the plume are released to simulate the entrainment of the background air temperature. In this way the entrainment is properly simulated and the plume sink is calculated from the local property of the flow. The approach is wholly Lagrangian.

A laboratory simulation of atmospheric microbursts is presented. The physical model of the atmospheric phenomenon is reproduced at a reduced scale in a rotating tank (TURLab, Italy), the similitude is based on the Froude number. Different experiments were carried out varying the Rossby number, and the analysis of four significant cases is presented. The velocity, vorticity, and turbulent kinetic energy fields are evaluated together with the swirling strength analysis. The comparison with the natural prototype is eventually shown and discussed. Two simulations of the response of Adriatic Sea to severe wind performed by an atmosphere-ocean coupled model and the comparisons with observed data and modelled fields published in literature are presented. The model RAMS-DieCAST was applied to simulate the variations of sea currents and temperature profiles, from surface to bottom, induced by two episodes of intense wind over the Adriatic sea: a Bora wind event that occurred in January 1995 and a Sirocco wind event in November 2002. The results of the simulations are compared with observed data at the sea surface. In the Bora episode, the computed surface temperatures are compared with satellite SSTs and in situ observed temperatures; in the Sirocco event the simulated surface currents and temperatures are compared with experimental data collected by surface drifters released in different regions of the Adriatic Sea during the same Sirocco event. In both episodes the simulated temperature trends agree with the observed values and during the Sirocco episode the current fields are in quite good agreement with the drifter data. The modelled sea temperature and velocity fields show also a good concordance with other simulation results in literature., Dvije simulacije jakog vjetra nad Jadranom simuliraju se združenim atmosfersko-oceanografskim modelom te se analiziraju i uspoređuju s mjerenjima i drugim objavljenim modeliranim poljima. U tu svrhu se koristi model RAMS-DieCAST za simulaciju promjena morskih struja i temperaturnih profila po vertikali, uzrokovanih dvjema epizodama intenzivnog vjetra nad Jadranom: burom u siječnju 1995. godine i jugom u studenom 2002. godine. Rezultati simulacija se uspoređuju s opažanjima na morskoj površini. Tijekom bure, modelirana površinska temperatura uspoređuje se s površinskom temperaturom mora dobivene satelitskim mjerenjima i mjerenjima temperature u određenim točkama prostora. Tijekom juga, simulirane površinske struje i temperature se uspoređuju s eksperimentalnim mjerenjima dobivenih na temelju površinskih driftera koji su pušteni u različitim dijelovima Jadrana za istu epizodu. U obje epizode trendovi simulirane temperature dobro se podudaraju s opažanjima. Također se tijekom juga, površinske struje dobro podudaraju s mjerenjima driftera. Modelirana temperatura mora kao i vektorsko polje brzina pokazuju dobro slaganje s rezultatima sličnih simulacija u literaturi.

The results of laboratory wall turbulence experiments on a shear-driven rotating boundary layer are presented. The experiments were carried out in the Turin University Laboratory rotating water tank. The flow was generated by changing the rotation speed of the platform and measured by means of particle image velocimetry. In order to analyse the influence of the rotation and of surface roughness, different cases were examined. Several rotation periods were considered. The measurements were performed both over a smooth surface and over a rough-to-smooth transition. Mean flows and the higher-order moments of the velocity probability density function are shown and discussed together with a comparison of the different experimental cases, theory and large-eddy simulations.

A fully coupled atmosphere–ocean model, endowed with a feedback of the ocean model on the atmospheric one, was applied in order to simulate the variations of temperature and sea currents induced by the occurrence of a late-fall episode of intense Sirocco wind over the Adriatic Sea. The coupled model was made up of the RAMS atmospheric model coupled with the DieCAST ocean model with the purpose to obtain more realistic forcing conditions of the lower winds on the underlying sea surface. The two main phenomena highlighted by the simulation are: the genesis of a 10 cm s − 1 north- westward current along the Italian coastline and a general cooling of the entire basin of approximately 1 °C limited to the upper 40 m. The same current shows an offshore anti-cyclonic pattern at different latitude in the whole basin. The results generally agree with experimental data collected by surface drifters released in different regions of the Adriatic Sea as a part of the international DOLCEVITA project, which also includes the same Sirocco episode considered here. The simulated currents represent the drifter trajectories quite accurately, with only few exceptions in the northernmost region of the basin, characterized by shallow water conditions. The simulated SST fields fully agree with the temperature values observed by the same drifters.

PURPOSE: Invasive mechanical ventilation is associated with complications, and its abbreviation is desirable. The imbalance between increased workload, decreased inspiratory muscle strength and endurance is an important determinant of ventilator dependence. Low endurance may be present due to respiratory muscle atrophy, critical illness, or steroid use. Specific inspiratory muscle training may increase or preserve endurance. The objective of the study was to test the hypothesis that inspiratory muscle training from the beginning of mechanical ventilation would abbreviate the weaning duration and decrease reintubation rate. As a secondary objective, we described the evolution of inspiratory muscle strength with and without inspiratory muscle training. METHODS: Prospective, randomized clinical trial in an adult clinical-surgical intensive care unit. Twelve patients trained the inspiratory muscles twice a day, and 13 patients did not (control). Training was performed adjusting the sensitivity of the ventilator based on the maximal inspiratory pressure. Patients underwent daily surveillance of the maximal inspiratory pressure. RESULTS: The weaning duration (31 ± 22 hr, control and 23 ± 11 hr, training group; P = .24) and reintubation rate (5 control and 3 training group; P = .39) were not statistically different. The maximal inspiratory pressure of the control group showed a trend toward a modest increase. In contrast, the training group showed a small decrease (P = .34). CONCLUSIONS: In acute critically ill patients, inspiratory muscle training from the beginning of mechanical ventilation neither abbreviated the weaning duration, nor decreased the reintubation rate. Inspiratory muscle strength tended to stay constant, along the mechanical ventilation, with or without this specific inspiratory muscle training. OBJETIVO: A ventilação mecânica invasiva é associada a complicações, portanto sua abreviação é desejada. O desbalanço entre o aumento da carga sobre os músculos inspiratórios, a diminuição da força e a resistência muscular é determinante na dependência da ventilação. A baixa resistência muscular pode ser causada por atrofia muscular, pela doença grave ou pelo uso de corticoesteróides. O treinamento da musculatura inspiratória pode aumentar ou preservar a resistência. O objetivo principal do estudo foi testar a hipótese que o treinamento da musculatura inspiratória desde o início da ventilação iria abreviar o desmame da ventilação e diminuir a taxa de reintubação. Como objetivo secundário descrevemos a evolução da pressão inspiratória máxima com e sem treinamento da musculatura inspiratória. MÉTODOS: Estudo prospectivo e aleatorizado em unidade de tratamento intensivo Clínico-Cirúrgica. Doze pacientes treinaram a musculatura inspiratória duas vezes ao dia e treze não treinaram (controle). O treinamento foi realizado ajustando a sensibilidade do ventilador, baseando-se na pressão inspiratória máxima. Os pacientes tiveram sua pressão inspiratória máxima verificada diariamente. RESULTADOS: A duração do desmame (31 ± 22 controle e 23 ± 11 horas grupo treinamento; p=0.24) não foi estatisticamente diferente. A pressão inspiratória máxima do grupo controle teve leve tendência ao aumento, enquanto o grupo treinamento teve leve tendência à diminuição. CONCLUSÃO: Em pacientes graves, o treinamento da musculatura inspiratória desde o início da ventilação mecânica não abreviou o desmame, nem diminuiu a reintubação. A pressão inspiratória máxima tendeu a manter-se constante ao longo da ventilação mecânica, com ou sem o treinamento inspiratório aplicado.

Production and advection of precipitating water in a prescribed, time dependent flow is investigated by means of a simple 2D model in order to predict the periodical behaviour of rain rate and pressure which was observed in the Po valley during IOP8 of MAP. The (x,z,t)-dependent wave perturbations on wind and temperature fields were derived from a linear stability analysis and are superimposed on z-dependent background fields. The wave influence on the hydrometeors production and advection is discussed.

We examined clinical aspects of Benign Paroxysmal Vertigo (BPV) in infancy and its most frequent differential diagnosis, in particular analogies and differences with forms of “migrainous vertigo” (MV) of later onset. During a long-term follow-up of 7 cases of BPV, diagnosed according to the Basser criteria, 5 of 7 BPV cases spontaneously resolved and 6 of 7 patients later developed migraine and other migraine-related symptoms. This course differs from that described for MV only in the age of onset of headache and in the chronological relationship with vertigo. The authors suggest that BPV can be interpreted as a migraine precursor and MV as a migraine equivalent.

The surgical treatment of type A aortic dissection is usually palliative and most surviving patients remain at considerable risk to develop late postoperative complications; consequently, there is the need for careful long-term follow-up. The present study reports on our experience in the postoperative follow-up of a consecutive series of patients with type A aortic dissection.Between January 1986 and December 1996, 89 patients underwent emergency surgery for type A acute aortic dissection; the overall hospital mortality rate was 22% (20/89). This study includes the 69 hospital survivors (49 men and 20 women). Forty-six patients had ascending aortic graft replacement, 13 patients underwent replacement of aortic valve and ascending aorta by a composite graft. The surgical repair was extended to the aortic arch in 5 patients. All patients were serially evaluated by clinical examination and imaging techniques (transthoracic echocardiography in all patients, magnetic resonance imaging in 40, transesophageal echocardiography in 33 and computed tomography in 25). Follow-up was complete in 97% of patients (two patients were lost to follow-up and excluded from the study) and extended to a maximum of 152 months (mean 74 +/- 39 months). The postoperative quality of life was assessed by a questionnaire in 51 current survivors. Risk factors for cardiovascular death, reoperation and poor quality of life were investigated with univariate and multivariate analysis.During the follow-up period 15 patients (22%) died; in 13 cases death was due to cardiovascular causes and in 6 of them it was related to aortic disease. The Kaplan-Meier survival was 92 +/- 3%, 87 +/- 5%, 78 +/- 6% and 70 +/- 8% at 2, 4, 6 and 8 years, respectively. A persistent aortic dissection was demonstrated in 50 patients (75%) and 42 of them showed the presence of flow in the false lumen. A dilatation of one or more aortic segments was found in 59 patients (88%), with a diameteror = 50 mm in 17 andor = 60 mm in 8. In 30 patients who underwent transesophageal echocardiography the relation between aortic dimensions and flow pattern in the false lumen was examined; the presence of aneurysmal dilatation with a diameteror = 50 mm was significantly correlated with a "high flow" pattern. Ten patients (15%) underwent reoperation from 13 to 83 months postoperatively. Reoperation was indicated for: sinus of Valsalva aneurysm and severe aortic regurgitation (2 patients), severe aortic regurgitation (2 patients), aneurysm of the arch (1 patient), thoracoabdominal aneurysm (1 patient), periprosthetic pseudoaneurysm (4 patients). The hospital mortality rate was 20% (2 patients). Sixty-two% of current survivors are asymptomatic; 30 patients returned to their predissection status. Quality of live is judged "good" by 23 patients, "fairly good" by 21 patients and "poor" by 7 patients. No significant independent risk factor for cardiovascular death, reoperation and poor quality of life was identified.The long-term prognosis after surgical treatment of type A aortic dissection is not satisfactory because of a significant risk of late complications. However, the results of our study can be judged fairly good, particularly if we consider the natural history of the disease.

Constrictive pericarditis may exceptionally present as pleural effusion of unknown origin and this form of presentation may cause diagnostic problems. We report a case of subacute constrictive pericarditis in which there were recurrent pleural effusion with no other signs of the disease and the initial echocardiographic study was nondiagnostic. For this reason the patient was initially considered to have primary pulmonary or pleural disease. On the basis of the subsequent development of signs of systemic congestion and the results of computed tomography, Doppler echocardiography and cardiac catheterization, which were consistent with constriction, it was concluded that the patient had constrictive pericarditis. A complete resolution of pleural effusion and signs of systemic congestion was observed following pericardiectomy.

Several approaches have been used for noninvasive estimation of right atrial pressure (RAP), but, no currently available method has gained any definite validation. The purpose of this study was to evaluate the accuracy of two-dimensional and Doppler echocardiography in estimating mean RAP in patients with cardiac disease.We examined the relation of mean RAP to right atrial size and function, size and respiratory changes of inferior vena cava and Doppler parameters of tricuspid and hepatic vein flow in 114 consecutive patients (77 men, 37 women; mean age 57 +/- 12 years) with various cardiac diseases undergoing cardiac catheterization. Echocardiographic studies were performed within 24 hours before catheterization (mean interval 6 +/- 3 hours). Patients were assigned to 3 groups according to the values of mean RAP (group 1,or = 8 mmHg; group 2, between 9 and 12 mmHg; group 3,12 mmHg).Mean RAP ranged from 3 to 20 mmHg (mean 9.1 +/- 4.3 mmHg). It correlated most strongly with the collapsibility index of inferior vena cava (IVCCI) (r = -0.76), minimal inspiratory diameter of inferior vena cava (r = 0.72) and deceleration time of early tricuspid flow (DT) (r = -0.61). Discriminant analysis demonstrated that IVCCI and DT were major determinants of mean RAP with 81.6% of cases correctly assigned to study groups: 96% of patients of group 1 and 87% of patients of group 3 were identified, whereas the accuracy in identifying the patients of group 2 was lower (46%). An IVCCI45% was the best cutoff point in predicting a mean RAPor = 8 mmHg; an IVCCI35% and a DT150 msec were the best cutoff points in predicting a mean RAPor = 15 mmHg. The best multivariate equation predicting mean RAP was: mean RAP = 23.3 - 0.2 IVCCI -0.026 DT (r = 0.80, R2 = 0.64). This equation was 81% sensitive and 84% specific in detecting a mean RAPor = 8 mmHg and 74% sensitive and 97% specific in detecting a mean RAP12 mmHg.Mean RAP can be estimated noninvasively by two-dimensional and Doppler echocardiography. The combined analysis of IVCCI and DT provides an accurate prediction on mean RAPor = 8 mmHg and12 mmHg, whereas the prediction of intermediate values is less accurate.

We report the case of a 69-year-old woman with a history of hypertension and transient ischemic attack who was admitted because of a diagnosis of left atrial mass on transthoracic echocardiography. Monoplane transesophageal echocardiography showed an extracardiac mass compressing the left atrium posteriorly and an apparent intraatrial mass. As the procedure did not clarify the nature of the masses, a thoracic magnetic resonance imaging was performed. This technique revealed a large hiatal hernia impinging on the left atrium without evidence of an upper gastrointestinal tract. Our experience confirm that hiatal hernia may mimic an echocardiographic left atrial mass and should be considered in the differential diagnosis of this finding.

Patients who have undergone surgical repair of congenital heart diseases are usually not allowed to participate in competitive sports. In the present study we report our long-term experience with 9 male athletes aged 17 to 23 years who participate in competitive sports after undergoing surgical repair of ostium secundum atrial septal defect at a median age of 9 years; six of them play football and three of them volleyball. Competitive sport activities began 1 to 5 years after surgical repair. The mean duration of follow-up is 88 +/- 26 months. Sport fitness was granted on the basis of the following criteria: 1) a normal physical examination; 2) a normal working capacity on exercise test; 3) no arrhythmias on exercise test and Holter monitoring, recorded also during sport activities; 4) a normal M-mode and two-dimensional echocardiography, including the normalization of right ventricular size; the persistence of an abnormal ventricular septal motion did not exclude sport fitness. Recently we also performed Doppler and color Doppler echocardiography and gated equilibrium radionuclide angiography at rest and during exercise. We studied left ventricular diastolic filling through the pulsed wave Doppler evaluation of transmitral flow and measured cardiac output by continuous wave Doppler echocardiography during exercise test in the supine position. We also performed exercise test and M-mode, two-dimensional, Doppler and color Doppler echocardiography in a control group made up of 15 athletes (10 football players and 5 volleyball players). The exercise duration at graded treadmill exercise test (according to the Carù protocol), the maximal heart rate and the maximal systolic blood pressure were, respectively, 12.9 +/- 0.8 min, 192 +/- 10 beats/min and 198 +/- 12 mmHg. Left ventricular end-diastolic dimension, mass and ejection fraction (single-plane area-length method) were 50.3 +/- 2.8 mm, 210 +/- 38 g and 65 +/- 6%. M-mode right ventricular diastolic dimension was 23.4 +/- 1.6 mm; the right ventricular maximal diastolic diameter and area obtained on two-dimensional echocardiography from the apical four chamber view were 44.1 +/- 3.6 mm and 25 +/- 3.8 cm2 respectively. The evaluation of transmitral flow showed the following data: E velocity 77 +/- 12 cm/sec, A velocity 45 +/- 6 cm/sec, E/A ratio 1.7 +/- 0.3, the isovolumic-relaxation period 72 +/- 8 m/sec and the deceleration half-time of the early rapid filling 71 +/- 10 m/sec. A trivial tricuspid regurgitation was detected in 6 subjects; the peak velocity of the regurgitant jet was less than 2.1 m/sec.(ABSTRACT TRUNCATED AT 400 WORDS)
