The objective of this research was to construct and to evaluate a portable fiber tester (PFT) for in field fleece and fiber test use. The standard wool top (SWT) samples were used for comparative measurement of fiber diameter using PFT in three laboratories without restricted ambient temperature or relative humidity control. Linear model with three factors was used to derive the least square means and variance components were determined. The PFT is constructed to be a compact and lightweight device of high-speed fiber testing. It works using digital image technology that measure and evaluate the fiber diameter variability of each wool sample within 45 seconds. The results show that the average fiber diameters measured by PFT at three labs are within IWTO defined tolerance values. There were no significant lab effect whereas, variance of labs remained as low. It can be concluded that PFT is a new instrument with high precision and accuracy to measure the fiber diameter of wool fleeces, which may be operated in differing field conditions., Bu araştırmanın amacı, saha testi kullanımı için taşınabilir bir lif test (PFT) cihazı oluşturmak ve özelliklerini değerlendirmektir. Standart yün tops (SWT) numuneleri, sınırlı sıcaklık veya nem kontrolleri olmayan üç laboratuvarda PFT kullanılarak lif çapının değerlendirilmesinde kullanılmıştır. Üç faktörlü doğrusal model en küçük kareler ortalamasını elde etmek için kullanılmıştır ve varyans bileşenleri de belirlenmiştir. Buna ek olarak, ölçümde doğruluk ve hassasiyet de belirlenmiştir. Dijital görüntü teknolojisini kullanan ve her numune için lif çapını 45 saniyede değerlendirebilen PFT, kompakt ve hafif olacak şekilde üretilmiştir Sonuçlar, tüm laboratuvarlarda PFT ile ölçülen ortalama lif çapının IWTO tarafından tanımlanan tolerans değerleri dahilinde yüksek bir hassasiyetle olduğunu göstermektedir. Önemli bir laboratuvar etkisi bulunmazken, laboratuvarların varyansları düşük kalmıştır. PFT'nin farklı koşullarda çalıştırılabilen, yün elyaf çapını ölçmek için yüksek hassasiyet ve ölçüm doğruluğunda bir alet olduğu sonucuna varılabilir.

Maintenance; Olaparib; Ovarian cancer Manteniment; Olaparib; Càncer d'ovaris Mantenimiento; Olaparib; Cáncer de ovarios Objective The phase IIIb OPINION trial (NCT03402841) investigated olaparib maintenance monotherapy in patients without a deleterious or suspected deleterious germline BRCA1/BRCA2 mutation (gBRCAm) who had platinum-sensitive relapsed ovarian cancer (PSROC) and had received ≥2 previous lines of platinum-based chemotherapy. Methods In this single-arm, open-label, international study, patients who had responded to platinum-based chemotherapy received maintenance olaparib tablets (300 mg twice daily) until disease progression or unacceptable toxicity. The primary endpoint was investigator-assessed progression-free survival (PFS) (modified RECIST version 1.1). A key secondary endpoint was PFS by homologous recombination deficiency (HRD) and somatic BRCAm (sBRCAm) status. The primary analysis of PFS was planned for 18 months after the last patient received their first dose. Results Two hundred and seventy-nine patients were enrolled and received olaparib. At data cutoff (October 2, 2020), 210 PFS events had occurred (75.3% maturity) and median PFS was 9.2 months (95% confidence interval [CI], 7.6–10.9) in the overall population. At 12 and 18 months, 38.5% and 24.3% of patients were progression-free, respectively. In the predefined biomarker subgroups, median PFS was 16.4, 11.1, 9.7, and 7.3 months in sBRCAm, HRD-positive including sBRCAm, HRD-positive excluding sBRCAm, and HRD-negative patients, respectively. The most common treatment-emergent adverse events (TEAEs) were nausea (48.4%) and fatigue/asthenia (44.1%). TEAEs led to dose interruption, dose reduction, and treatment discontinuation in 47.0%, 22.6%, and 7.5% of patients, respectively. Conclusion Maintenance olaparib demonstrated clinical benefit in patients without a gBRCAm, and across all subgroups, compared with historical placebo controls. There were no new safety signals. Objective To study the effectiveness and limitations of photodynamic therapy (PDT) as treatment of choice in patients with symptomatic circumscribed choroidal hemangioma. Methods A retrospective study was conducted on 16 patients (13 men and 3 women, with mean age of 54.88 years) with circumscribed choroidal hemangioma, who attended our center and were treated with PDT in the last 7 years. Results All patients had circumscribed choroidal hemangioma, which caused a decrease in visual acuity (VA) secondary to the presence of intraretinal microcystic edema or neurosensory detachment. The mean initial VA was 0.23, and the final mean VA after performing PDT was 0.38 (all the VA were measured in decimal scale). It should be noted that patients needed a mean of 1.69 PDT sessions. Three of the patients needed rescue treatment with trans-pupillary thermotherapy, intravitreal injection of anti-vascular endothelial growth factor (ranibizumab, aflibercept) or a dexamethasone intravitreal implant (Ozurdex ® ). The indication for a change of treatment was the persistence of intraretinal microcystic edema and/or neurosensory detachment (or incomplete resolution) after 3 PDT sessions. As overall results, 62.5% of patients evolved into anatomical and functional (increase in AV or stability) resolution. Conclusions PDT is a straight forward and fast procedure, with a good anatomical and functional response, causing minimal damage to adjacent vessels.

Resumen Objetivo Estudiar la eficacia y las limitaciones de la terapia fotodinamica (TFD) como tratamiento de primera eleccion en pacientes con hemangioma coroideo circunscrito sintomatico. Metodos Se realizo un estudio retrospectivo de 16 pacientes (13 varones y 3 mujeres, con una media de edad de 54,88 anos) con hemangioma coroideo circunscrito visitados en nuestro centro y tratados con TFD en los ultimos 7 anos. Resultados Todos los pacientes presentaban hemangioma coroideo circunscrito, que ocasionaba sintomatologia secundaria a la presencia de edema microquistico intrarretiniano o desprendimiento neurosensorial. La agudeza visual (AV) media inicial del grupo era de 0,23 y la AV media posterior a la realizacion de TFD fue de 0,38 (todas las AV fueron medidas en escala decimal). Cabe destacar que los pacientes necesitaron una media de 1,69 sesiones de TFD. Tres de los pacientes necesitaron un tratamiento de rescate con termoterapia transpupilar, inyeccion intravitrea de antifactor de crecimiento endotelial vascular (ranibizumab, aflibercept) o implante intravitreo de dexametasona (Ozurdex ® ). La indicacion de cambio de tratamiento fue la persistencia de edema microquistico intrarretiniano y/o desprendimiento neurosensorial (o su resolucion incompleta) tras 3 sesiones de TFD. Como resultados generales, destacamos que un 62,5% de los pacientes evoluciono hacia la resolucion anatomica y funcional (incremento de AV o estabilidad). Conclusiones La TFD es un procedimiento dirigido y rapido, con buena respuesta anatomica y funcional, que ocasiona un minimo dano a los vasos adyacentes a la lesion.

Background While early stage endometrial cancer (EC) is associated with a favorable prognosis, prognosis for advanced or recurrent EC is poor. Paclitaxel and carboplatin chemotherapy (CT) is often used as first-line treatment for these patients; however, there exists an imperative need for more effective and tolerable therapies. In KEYNOTE-146, combination therapy with the PD-1 inhibitor pembrolizumab (pembro) and the tyrosine kinase inhibitor lenvatinib resulted in an ORR of 39% in patients with EC (Makker et al. ASCO 2018). Based on these promising data, the ENGOT-EN9/LEAP-001 study was initiated to assess this combination therapy in women with recurrent or advanced EC. Trial design The ENGOT-EN9/LEAP-001 study (NCT03884101) is a phase 3, randomized, open-label, active-controlled trial comparing combination therapy with pembro and lenvatinib to paclitaxel and carboplatin CT in patients with newly diagnosed stage III-IV or recurrent EC (NCT03884101). Approximately 720 patients not previously treated with systemic chemotherapy (except as part of a chemoradiation regimen), antiangiogenic agents, PD-1 or PD-L1 inhibitors, or other T-cell receptor–targeted agents will be randomized 1:1 to pembro (200 mg Q3W) plus lenvatinib (20 mg daily) or paclitaxel and carboplatin CT (pac 175 mg/m2 plus carb AUC 6 Q3W). Patients will first be stratified by proficient vs deficient mismatch repair status (pMMR vs dMMR), and pMMR patients will be further stratified by ECOG performance status (0 vs 1), measurable disease (yes vs no), and prior chemoradiation (yes vs no). Treatment will continue until disease progression, initiation of new anticancer treatment, unacceptable adverse events, or withdrawal of consent for up to 35 cycles for pembro or 7 cycles of paclitaxel and carboplatin CT. Primary endpoints are PFS per RECIST v1.1, assessed by blinded independent central review, and OS. Secondary endpoints are ORR, health-related quality of life, safety and tolerability, and pharmacokinetics of lenvatinib. [EN] The article presents the results of a study that measured the attitudes of students of Education of the University of Barcelona to the Statistics, through scale EAE by Auzmendi (1992). Research has shown neutral-low level of general attitude towards the matter, but has also identified groups of students with different profiles: a group with an attitude unfavorably, but not especially anxious, just knowledge of statistics and very few numerical skills; another group is characterized by a positive attitude, who like statistics, but that worry and anxiety also shows; and a third group that stands out above all for anxiety before statistics. Given the potential relationship between academic achievement and attitude, the findings point to the need to explore strategies for Adaptive education for different groups., [ES] El artículo presenta los resultados de un estudio en el que se midieron las actitudes de los estudiantes de Pedagogía de la Universidad de Barcelona hacia la Estadística, a través de la escala EAE de Auzmendi (1992). La investigación ha evidenciado niveles neutros-bajos de actitud general hacia la materia, pero también ha identificado grupos de estudiantes con perfiles diferenciados: un grupo con una actitud desfavorable, pero no especialmente ansioso, con apenas conocimientos previos de Estadística y muy pocas habilidades numéricas; otro grupo caracterizado por una actitud positiva, al que le gusta la Estadística, pero que muestra preocupación y ansiedad también; y un tercer grupo que destaca sobre todo por la ansiedad ante la Estadística. Dada la potencial relación entre actitud y logro académico, las conclusiones apuntan hacia la necesidad de explorar estrategias de enseñanza adaptativa para los diferentes grupos.

Resumen Objetivo Presentar la deteccion de una recurrencia secundaria a un carcinoma gastrico difuso, con celulas en anillo de sello e indiferenciado mediante PET-FDG. Material y metodos Se trataba de un paciente con estadio III diagnosticado en 1994 y tratado con gastrectomia subtotal. En febrero del 2003 se diagnostica recurrencia mediastinica y supraclavicular izquierda, administrandose quimioterapia y radioterapia alcanzando respuesta completa. Tras 6 meses libre de enfermedad, presenta una elevacion de marcadores tumorales con pruebas complementarias (endoscopia digestive alta, gammagrafia osea y TC) negativas, efectuandose PET-FDG para descartar recurrencia. Resultados La PET-FDG identifico lesiones probablemente malignas en region supraclavicular derecha y en mediastino, confirmandose dichos hallazgos por seguimiento clinico y la realizacion de una TC 4 meses mas tarde. Conclusiones Destacamos la deteccion de recurrencia precoz por PET-FDG en un carcinoma no intestinal gastrico, circunstancia que creemos de interes dada la mayor dificultad de la PET-FDG en la deteccion de carcinomas productores y/o secretores de moco.

INTRODUCCIÓN: El cáncer de vejiga es un importante problema de salud y supone una parte muy significativa de la carga asistencial en los Servicios de Urología. Con este estudio ponemos de manifiesto las características epidemiológicas de esta neoplasia en la provincia de Cuenca en los últimos 17 años. PACIENTES Y MÉTODO: Desde enero de 1985 hasta diciembre del 2002 se registraron en la provincia de Cuenca todos los tumores de origen urológico. Los siguientes datos fueron recogidos al diagnóstico: el sexo, la edad, el tipo de tumor, su estadificación según la clasificación TNM, factores ocupacionales (consumo de alcohol, tabaco y café), procedencia social (rural/ urbana) y la presencia o no de metástasis demostrables. RESULTADOS: Encontramos diferencias significativas a lo largo del tiempo en cuanto a la histología, edad al diagnóstico, incidencia, procedencia social y algunos factores ocupacionales. CONCLUSIONES: Se diagnostican cada vez más tumores, cada vez en mayor número son infiltrantes, en población cada vez mayor y de origen eminentemente rural a pesar de la leve disminución poblacional de la provincia de Cuenca en los últimos 17 años.
