Il paziente nefropatico cronico facilmente presenta alterazioni morfologiche e funzionali dell'apparato gastroenterico. I segni più comuni e precoci nella sindrome uremica cronica sono rappresentati dai disturbi gastrointestinali. Da alcuni decenni abbiamo a disposizione dei farmaci con potente azione inibente la secrezione acida gastrica: gli inibitori di pompa protonica (IPP) hanno una struttura chimica affine, uno stesso meccanismo d'azione e sono molto importanti per il trattamento delle patologie acido correlate, per l'eradicazione dell'Helicobacter Pylori, per la prevenzione e la cura della gastropatia da farmaci antinfiammatori non steroidei (FANS). Somministriamo ai nostri pazienti questa classe di farmaci, con terapie che continuano nel tempo, nonostante la risoluzione della malattia (gastroprotezione). Ma gli IPP possono essere utilizzati indistintamente nei nefropatici cronici oppure sarebbe utile conoscere il profilo del farmaco per una corretta scelta? In questo articolo si argomenta che i loro effetti collaterali non sono molto rilevanti e sono abbastanza simili: il loro impiego nel lungo termine è sicuro. La potenza e l'efficacia dei vari IPP, dall'analisi comparativa dei vari trial clinici, risulta essere molto simile sulla base dei milligrammi di sostanza utilizzata. L'unica eccezione illustrata in questo lavoro è rappresentata da 6 pazienti in emodialisi, trattati con lansoprazolo (15 mg), che presentavano gastriti e ulcere peptiche complicate da gravi episodi di ematemesi e melena con conseguente anemia. Tutti gli IPP hanno dimostrato un'efficacia clinica sovrapponibile, tuttavia vanno valutati di volta in volta i vantaggi (relativi) di ciascun IPP. I criteri di scelta di un IPP sembrano basati, principalmente sulle indicazioni autorizzate, sulle formulazioni disponibili, sul profilo di sicurezza del farmaco.

The aim of this study was to investigate the phytochemical content of Poncirus trifoliata (L.) Raf. (Rutaceae) and to assess its hypoglycaemic and antioxidant effects. Hesperidin, naringin and chlorogenic acid were the most abundant compounds of the juice. Peel essential oil showed that limonene, myrcene, p-cimene and β-pinene were its main components. The antioxidant activity was studied through different in vitro tests. The hypoglycaemic effects were analysed via inhibition of carbohydrate-hydrolysing enzymes. IC50 values in the range 30.38–39.25 µg/mL were found respectively for juice and peel oil in the DPPH test. Juice inhibited α-amylase and α-glucosidase with IC50 values respectively of 138.14 and 81.27 µg/mL. Polyphenolic compounds identified in the juice were investigated. While neoeriocitrin displayed the strongest antioxidant activity and inhibited α-amylase (IC50 of 4.69 vs 77.45 µM for acarbose), didymin was found to be the most bioactive flavonoid against alpha-glucosidase (IC50 of 4.20 vs 54.99 µM for acarbose).

A comparative study of phytochemicals content and biological properties of eight Rosmarinus officinalis (rosemary) populations (RO1–RO8) collected in different areas of Tunisia was carried out. Two of the main rosemary constituents, rosmarinic and carnosic acids, were quantified by an NMR technique. Carnosic acid content was higher than that of rosmarinic acid. The non-polar constituents were examined by GC and GC–MS. Total phenols and flavonoids content were also determined in order to discuss the possible correlation between these phytochemicals and bioactivity. Antioxidant activity was investigated through different in vitro assays. Sample RO3 from a sub-humid area showed the highest potency in 1,1-diphenyl-2-picrylhydrazyl (DPPH) assay (IC50 of 6.74 μg/mL) while RO5 from arid area exhibited the highest radical 2,2′-azino-bis-3-ethylbenzthiazoline-6-sulphonic acid (ABTS) scavenging activity (IC50 of 5.92 μg/mL). The most active sample in Ferric Reducing Antioxidant Power (FRAP) assay was RO4 (62.21 μM Fe(II)/g), followed by RO1–RO3 and RO8 harvested in sub-humid areas with values in the range 49.61–58.72 μM Fe(II)/g. Extracts were able to inhibit acethylcholinesterase (AChE) and butyrylcholinesterase (BChE). Sample RO5 exhibited a promising AChE inhibitory activity (IC50 of 4.47 μg/mL). Thus, we suggest rosemary extracts as a functional food ingredient for preventing AD.
