Résumé: L'érythème noueux est une hypodermite aiguë septale secondaire à une réaction d'hypersensibilité médiée par les cytokines Th1. S'il est de diagnostic clinique facile, la recherche étiologique n'en est pas toujours aisée. Contrairement à l'érythème noueux idiopathique, dont l'évolution est généralement bénigne, celle de l'érythème noueux secondaire se fait en parallèle à la maladie causale. Parmi les étiologies rares d'érythème noueux secondaire se trouvent les hémopathies malignes. Nous rapportons le cas d'une patiente de 84 ans, hospitalisée pour syndrome anémique, douleurs oligoarticulaires aiguës des grosses articulations des membres inférieurs et éruption cutanée douloureuse en lien avec un érythème noueux évoluant depuis deux semaines. L'hémogramme a mis en évidence une tricytopénie et des blastes circulants. Le myélogramme a confirmé le diagnostic de leucémie aiguë myéloblastique sur myélodysplasie prévalente, et la biopsie cutanée a confirmé l'érythème noueux. Le bilan étiologique clinique et paraclinique de l'érythème noueux était par ailleurs négatif. Les lésions cutanées n'ont pas régressé malgré le repos bien conduit puis la mise en route d'une corticothérapie orale (1 mg/kg), tout comme la maladie hématologique dont la prise en charge a finalement été palliative compte tenu du mauvais état général de la patiente et du sombre pronostic. À notre connaissance l'association érythème noueux et leucémie est rare. Cette relation reste peu documentée par la littérature et son étiopathogénie mal connue. The erythema nodosum is an acute septal hypodermitis due to a hypersensibility reaction that is "mediated" by the th1 cytokines patheway. The diagnosis may be easy but the etiological research can in certain circumstance represent a real problem. In opposition to the evolution of the idiopathic erythema nodosum who's generally benign, the secondary erythema nodosum follow the evolution of the causative disease. Malignant hemopathies figures in the rare aetiology of erythema nodosum. We report the case of an 84 years old woman hospitalised for anemia and an acute oligoarticular pain in the joint of inferior limb associated with an algic cutaneous eruption linked with an erythema nodosum that evolued for 2 weeks. The hemogram showed tricytopenia and circulating blasts. The myelogram confirmed the diagnosis of acute myeloblastic leukemia on prevalent myelodysplasia, and the skin biopsy confirmed the erythema nodosum. The clinical and para-clinical etiological assessment of the erythema nodosum was negative. The skin lesions did not regress despite the well-conducted rest and then the initiation of oral corticosteroid therapy (1 mg/kg), as did the hematological disease, the management of which was ultimately palliative given the poor general condition of the patient and the poor prognosis. To our knowledge, the association of erythema nodosum and leukemia is rare. This relationship remains poorly documented in the literature and its etiopathogenesis is poorly understood.
