Martina Fumeo, Sabrina Manfredi, Antonella VoltaDepartment of Veterinary Science, University of Parma, 43126 Parma, ItalyAbstract: Emphysematous cystitis (EC) is a rare disease in human as well as in veterinary medicine; in both it is defined as an uncommon form of complicated urinary tract infection (UTI), characterized by the presence of gas within the bladder wall and lumen. While female dogs are at increased risk of developing an UTI, patients with EC are not subject to gender or age predisposition and may present with variable clinical manifestations. Diabetes mellitus (DM) appears as the most common risk factor for the development of EC, but many other underlying conditions have been mentioned in literature and should be taken into consideration. In case of EC, Escherichia coli appears to be the most common pathogen isolated from urine cultures. A conclusive diagnosis of EC requires necessarily the recourse to imaging methods, such as abdominal radiography and ultrasonography. An early diagnosis and appropriate medical therapy, consisting in protracted antibiotic treatment associated with control over underlying diseases, could lead to avoid surgical intervention.Keywords: emphysematous cystitis dog, dog, cat, gas, urinary, bladder, infection

L’ultrasonografia è una metodica di diagnostica per immagini routinariamente utilizzata sia in medicina umana che in medicina veterinaria. Da numerosi anni le applicazioni sono innumerevoli sia in campo umano che veterinario e, in particolare nell’imaging muscolo-scheletrico, rappresenta la tecnica che si sta sviluppando più rapidamente. In medicina umana, la displasia congenita dell’anca compare in un modesto ma significativo numero di bambini, e l’intervallo di tempo per trattare la patologia è compreso nei primi sei mesi di vita. L’ecografia è il metodo diagnostico maggiormente utilizzato nella diagnosi di malattia. Come noto, la displasia dell’anca del cane è la patologia ortopedica non traumatica del periodo di accrescimento di più frequente riscontro. Ad oggi sono ancora limitate le conoscenze in relazione all’esame ultrasonografico delle anche in medicina veterinaria. Alla luce di queste considerazioni, e revisionando l’amplissima bibliografia sull’argomento sviluppata in medicina umana, in questo lavoro sono state studiate le articolazioni coxo-femorali di 78 cuccioli di cane di razza Labrador Retriever ecograficamente durante le prime settimane di vita e seguite nel loro sviluppo a 6 e 12 mesi d’età, previo esame radiografico delle stesse. Gli scopi della presente ricerca sono: - definire una metodica standardizzata per l’indagine ecografica delle articolazioni coxo-femorali e valutarne le caratteristiche ecografiche durante le prime settimane di vita; - standardizzare una metodica per valutare le possibili variazioni ecografiche tra pazienti sani e pazienti displasici, nel tentativo di effettuare una diagnosi precoce di malattia. Le misurazioni effettuate sono state: ampiezza dell’angolo alfa, percentuale di copertura acetabolare, spessore della cartilagine acetabolare, area e volume della testa femorale e area del centro di ossificazione della testa del femore. L’esame ecografico è stato utile nel valutare la morfologia delle articolazioni coxo-femorali nel cucciolo. La finestra acustica ideale è compresa tra i 10 e i 20 giorni di età. L’utilizzo dell’angolo alfa nella scansione coronale non si è mostrato essere utile nella diagnosi precoce di malattia, come già riportato negli studi precedenti. Nessuno studio in medicina veterinaria ha invece preso in considerazione i valori e l’utilità dello spessore della cartilagine acetabolare e la percentuale di copertura dell’acetabolo. Anche questi parametri non si sono mostrati utili nella diagnosi di displasia. Invece l’area e il volume della testa del femore, e soprattutto l’area del centro di ossificazione sono risultati essere correlati al grado di malattia. Pertanto, è possibile che una componente genetica sia correlata alla dimensioni della testa ma è ancora poco chiaro se una testa piccola predisponga allo sviluppo della malattia. Lo stesso discorso vale per il nucleo di ossificazione della testa femorale. The aim of the present study is to determine the feasibility of sonographical examination of hip joints in non-sedated Labrador Retriever puppies and to assess the value of static ultrasonography for the early diagnosis of canine hip dysplasia. Prospective study was carried out. 78 puppies, 7 and 39 days old, underwent sonographical determination of the -angle, acetabular coverage of the femoral head, thickness of acetabular cartilage, area and volume of the femoral head and area of the centre of ossification of the femoral head at 19 days. Sonographical parameters were correlated to conventional radiographic hip dyspalsia classification in the adult dogs ( 12 months). Mean -Angles of 71,45° ± 8,2 were measured. Despite the feasibility of sonographical hip assessment in puppies, no statistically significant correlation was found between sonographical determination of the -angle and conventional radiographic hip joint classification at the age of 12 months. The mean thickness of acetabular cartilage in the hips classified as normal was 0.115 cm, as compared with 0.125 cm for the dysplastic hips in the referred group. No statistically significant correlation was found between sonographical determinations of the thickness of acetabular cartilage and conventional radiographic hip classification. No statistically significant correlation was present between the percentage of acetabular coverage and hip dysplasia at 12 months of age. A mild correlation was found between the area and the volume of the femoral head and the grade of disease at 12 months of age. And finally, a moderate correlation between the area of the centre of ossification of the femoral head and the radiographic conventional hip joint classification in the mature dog was found. Results of the present study suggest that static ultrasonography of hip joints in puppies between 7 and 39 days of age is technically feasible but the measurements of -angle, acetabular coverage of the femoral head and thickness of acetabular cartilage cannot be recommended for detecting puppies that will develop hip dysplasia at the age of 12 months. It is possible that the size of the femoral head and the centre of ossification can be an early sign or a concomitant cause in the development of hip dysplasia in dogs of this breed. Further investigations are needed.
