Introduction: malnutrition is a frequent finding among cancer patients. Despite its prognostic significance, there are still few studies evaluating the nutritional status of patients with hepatocellular carcinoma (HCC). Objectives: to evaluate the nutritional status of patients with HCC by different methods. Methods: patients with HCC were evaluated in an outpatient clinic at the Hospital Santa Casa de Misericórdia de Porto Alegre. The methods used for the nutritional assessment were body mass index (BMI), tricipital skinfold (TSF), arm circumference (AC), arm muscle circumference (AMC), hand grip strength (HGS), adductor pollicis muscle (APM), patient-generated subjective global assessment (PG-SGA) and phase angle (PA). Results: forty-three patients with HCC were evaluated; all of them were cirrhotic. The mean age was 64.0 ± 5.8 years and the prevalent gender was male (72.1%). APM, PA, PG-SGA and AMC were the most sensitive methods for the diagnosis of malnutrition. There was a negative correlation between TSF and Child-Pugh score (p = 0.004) and a positive correlation between APM and BCLC stage (Barcelona Clinic Liver Cancer Group) (p = 0.006). Conclusions: there was high variation in the diagnosis of malnutrition among the methods studied. APM, AMC, PA and PG-SGA can be indicated as tools of choice in the nutritional assessment of the HCC patient because they were the most sensitive methods in the diagnosis of malnutrition. TSF and APM correlate with disease severity. Resumen Introducción: la desnutrición es un hallazgo frecuente entre pacientes oncológicos. A pesar de su significancia pronóstica, todavía son escasos los estudios que evalúan el estado nutricional de pacientes con carcinoma hepatocelular (CHC). Objetivos: evaluar el estado nutricional de pacientes con CHC por diferentes métodos. Métodos: se evaluaron pacientes con CHC en seguimiento ambulatorio en el Hospital Santa Casa de Misericordia de Porto Alegre. Los métodos empleados para la evaluación nutricional fueron índice de masa corporal (IMC), pliegue cutáneo tricipital (PCT), circunferencia del brazo (CB), circunferencia muscular del brazo (CMB), fuerza del apretón de manos (FAM), músculo aductor del pulgar (MAP), evaluación subjetiva global producida por el paciente (ASG-PPP) y ángulo de fase (AF). Resultados: se evaluaron 43 pacientes con CHC, todos cirróticos. El promedio de edad fue de 64,0 ± 5,8 años y el sexo prevalente, el masculino (72,1%). MAP, AF, ASG-PPP y CMB fueron los métodos más sensibles para el diagnóstico de desnutrición. Se observó una correlación negativa entre la PCT y la puntuación Child-Pugh (p = 0,004) y una correlación positiva entre MAP y la estadificación BCLC (p = 0,006). Conclusiones: hubo gran variación en el diagnóstico de desnutrición entre los métodos estudiados. MAP, CMB, AF y ASG-PPP se pueden indicar como herramientas de elección en la evaluación nutricional del paciente con CHC por haber sido los métodos más sensibles en el diagnóstico de desnutrición. PCT y MAP se correlacionan con la gravedad de la enfermedad.

BACKGROUND: Insulin resistance, especially that induced by obesity, plays a central role in the development of non-alcoholic fatty liver disease. Although the evaluation of overweight patients is important, the nutritional assessment tools used in clinical practice have limitations. Neck circumference (NC), from this, becomes a viable and low-cost alternative, which seems to be related to the accumulation of fat in the hepatic tissue. OBJECTIVE: To evaluate the association between NC and metabolic alterations in patients with non- alcoholic fatty liver disease. METHODS: A cross-sectional study performed in 82 patients, of whom 76 underwent liver biopsy. We performed weight, height, abdominal circumference and NC measures. Values of NC ≥42 cm and ≥36 cm were considered as altered for men and women, respectively. Laboratory tests and liver biopsy result were collected in the participants’ charts. We evaluated fasting blood glucose levels, insulin, glycosylated hemoglobin, triglycerides, total cholesterol, high density lipoprotein (HDL-C), low density lipoprotein (LDL-C), ferritin, alkaline phosphatase, gamma glutamyltransferase, albumin, total bilirubin, direct bilirubin, glutamic-oxalacetic transaminase, glutamic-pyruvic transaminase and the HOMA-IR index. RESULTS: We evaluated eighty-two patients. Patients with altered NC had increased body mass index (P=0.043), abdominal circumference (P=0.007), insulin (P=0.003) and HOMA-IR (P=0.029) when compared to those with adequate NC. Background Patients with liver disease often present protein-energy malnutrition. The assessment of food intake is very important in the investigation regarding the "health-disease" process. Objective To assess dietary intake of cirrhotic patients through food record during 3 days, correlating it with the nutritional status of the patient and the stage of the disease. Methods Cirrhotic outpatients from the Santa Casa de Misericórdia Hospital, RS, Brazil, were assessed. Nutritional assessment was performed by anthropometry; non-dominating handgrip strength; adductor pollicis muscle thickness; phase angle by bioelectrical impedance analysis; and Subjective Global Assessment. For analysis of food consumption we used the food records of 3 days using scales for weighing of all foods. Results We evaluated 25 (68%) patients, in which there was a prevalence of cirrhosis by hepatitis C virus. The upper arm circumference, handgrip strength and phase angle by bioelectrical impedance analysis diagnosed 56% of malnourished. BACKGROUND: Carriers of hepatitis C virus have lower levels of total cholesterol, high density lipoprotein-cholesterol, low density lipoprotein- cholesterol and triglycerides compared to uninfected patients. With the progression of liver disease, the values for cholesterol and its fractions reduce linearly, with reduction ratio of lipid profile and markers Child-Pugh and MELD. AIM: To determine the relationship between decrease dlipid profile with clinical outcome presented (liver transplantation or death pre-transplant). METHODS: Was conducted a cross sectional analytical study of a follow-up study performed by reviewing medical records. Cirrhotic patients treated at theClinic of Gastroenterology from a large tertiary hospital with cirrhosis of viral etiology and/or alcohol were studied. The clinical characteristics (gender, age and etiology of cirrhosis) and lipid profile data from150 patients were collected in the year 2010.To analyze the occurrence of clinical outcomes (liver transplantation or death pre-transplant) patients were evaluated after four years. RESULTS: The prevalent cause was hepatitis C virus (53,3%), followed by alcohol (32%) and hepatitis C and alcohol (14,6%). Males represented 62% of the sample and the average age was 63.1±9.11 years. The prevalent lipid changes were hypocholesterolemia associated with hypotriglyceridemia (36,6%) and isolated hypocholesterolemia (34,6%). CONTEXT: Cirrhosis is a progressive chronic hepatopathy which constitutes an irreversible stage of liver dysfunction. OBJECTIVES: To evaluate the oxidative stress in the blood of cirrhotic rats treated with the antioxidant melatonin. METHODS: Cirrhosis was induced through inhalation of carbon tetrachloride. Liver integrity was evaluated by measuring serum enzymes, oxidative damage measured by lipoperoxidation, and antioxidant enzyme activity in erythrocytes. Lipoperoxidation, total nitrates, collagen, and histology by picrosirius staining were evaluated in the livers of these animals (n = 15), which were divided in three groups: control, carbon tetrachloride, and carbon tetrachloride + melatonin. Melatonin (20 mg/kg) was administered intraperitoneal from week 10 of carbon tetrachloride inhalation. In order to shorten the cirrhosis induction time, phenobarbital (0.3 g/L) was added to the animals' drinking water. RESULTS: A significant impairment in the liver integrity of melatonin-treated animals as compared to cirrhotic animals was observed. In rat erythrocytes and liver, lipoperoxidation was significantly increased in the cirrhotic rats as compared to controls, as measured through thiobarbituric acid reactive substances, and significantly decreased in melatonin-treated animals as compared to cirrhotic ones. In blood, a decrease in superoxide dismutase and glutathione peroxidase enzymes was detected in the cirrhotic group as compared to the control group, with increased superoxide dismutase activity when melatonin was administered. A reduction in the levels of total nitrates was detected in the hepatic tissue of the animals in the carbon tetrachloride group as compared to the control group and an increase of these levels in the carbon tetrachloride + melatonin group. As for hepatic collagen, we found a significant increase in the carbon tetrachloride group as compared to the controls and a regression of these values in the treated group. In histology, the rats in the carbon tetrachloride group showed fibrosis and formation of fibrotic nodules, characterizing liver cirrhosis; there was reduction of nodules and fibrosis in the melatonin treated group. CONCLUSION: The data allow us to suggest that the observed oxidative stress is related to the damages caused by carbon tetrachloride and that the use of melatonin can minimize these damages CONTEXTO: A cirrose é uma hepatopatia crônica e progressiva que constitui estágio irreversível de disfunção hepática. É associada a alterações na circulação sistêmica. OBJETIVOS: Avaliar o estresse oxidativo no sangue de ratos cirróticos e tratados com antioxidante melatonina. MÉTODOS: A cirrose foi induzida através da inalação de tetracloreto de carbono. Foram avaliadas as provas de integridade hepática através das medidas das enzimas séricas, o dano oxidativo medido pela lipoperoxidação e a atividade das enzimas antioxidantes no eritrócito. No fígado desses animais, foram avaliados a lipoperoxidação, os nitratos totais, colágeno e histologia através de picrosíruis. Os animais (n = 15) foram divididos em três grupos experimentais: controle, tetracloreto de carbono e tetracloreto de carbono + melatonina. A melatonina foi administrada por via intraperitonial após a 10ª semana de inalação na concentração de 20 mg/kg. Com o objetivo de abreviar o tempo de indução, foi administrado para todos animais, fenobarbital na água de beber na concentração de 0,3 g/L. RESULTADOS: Observou-se redução significativa nas provas de integridade hepática nos animais tratados com melatonina, em relação aos animais cirróticos. Nos eritrócitos e fígados dos ratos, foi observado aumento significativo da lipoperoxidação nos ratos cirróticos em comparação com os controles, através da medida das substâncias que reagem ao ácido tiobarbitúrico, e redução nos animais tratados com melatonina. No sangue, observou-se diminuição dos valores das enzimas superóxido dismutase e glutationa peroxidase do grupo cirrótico em comparação ao grupo controle, elevando a atividade da superóxido dismutase quando administrada melatonina. Na avaliação dos nitratos totais, no tecido hepático, observou-se redução dos valores nos animais do grupo tetracloreto de carbono em comparação ao grupo CO e um aumento desses valores nos ratos do grupo tratado com melatonina. Na medida do colágeno hepático, encontrou-se aumento significativo do grupo tetracloreto de carbono em relação ao controle e regressão desses valores no grupo tratado. Através da histologia, os ratos do grupo tetracloreto de carbono apresentaram presença de fibrose e formação de nódulos fibróticos, caracterizando a cirrose hepática; após o tratamento com a melatonina, houve redução dos nódulos e da fibrose. CONCLUSÃO: Com esses dados, pode-se sugerir que a análise do estresse oxidativo no sangue está relacionado ao dano hepático causado pelo tetracloreto de carbono e que o uso de melatonina pode minimizar esses danos

RACIONAL: A recurrência da hepatite C no período pós-operatório do transplante hepático é um dos principais desafios atualmente enfrentados pela comunidade transplantadora. O tratamento com interferon peguilado e ribavirina tem sido associado à resposta virológica sustentada em 21% a 45% dos casos e constitui a única conduta eficaz, até o momento, para mudar o curso da progressão acelerada da doença no período pós-operatório do transplante hepático, que pode levar à perda do enxerto e à necessidade de retransplante. No entanto, a portaria n° 863 do Ministério da Saúde não disponibiliza esse tratamento para pacientes com recidiva da hepatite C após o transplante hepático. OBJETIVOS: Avaliar as condutas de acompanhamento e tratamento de pacientes com vírus C submetidos a transplante hepático em diferentes centros nacionais. MÉTODOS: Foram enviados questionários à maioria dos centros de transplante hepático nacionais de adultos sobre condutas acerca de acompanhamento e/ou tratamento de pacientes com hepatite C. RESULTADOS: Dezenove centros nacionais que congregam mais de 2 800 pacientes (51% com hepatite C) submetidos a transplante hepático responderam o questionário. A maioria (53%) usa tacrolimus e prednisona como esquema de indução e 32% usam esquemas diferenciados para hepatite C. Treze centros fazem biopsia protocolar para seguimento, sendo empregados diferentes critérios histológicos para diagnóstico de recurrência. Todos os centros indicam tratamento com interferon peguilado e ribavirina apenas na recurrência histológica da hepatite C e 46% empregam estádios de fibrose inferiores a F2 de acordo com a classificação METAVIR, mais leves do que aqueles preconizados para tratamento de pacientes não transplantados. A duração do tratamento é de 1 ano em 32% dos centros, baseada no genótipo em 21% e a la carte em 47%. A maioria dos centros (84%) não interrompe o tratamento na 12ª semana, na ausência de resposta virológica. CONCLUSÕES: A despeito da portaria nº. 863 do Ministério da Saúde, todos os centros estão viabilizando tratamento mediante acordo com secretarias regionais de saúde ou por intermédio de liminares concedidas aos pacientes pela justiça comum.

BACKGROUND: Recurrence of hepatitis C after orthotopic liver transplantation is one of the major clinical challenges faced by the liver transplantation community. Treatment of hepatitis C recurrence with peguilated interferon and ribavirin has been associated with sustained virological response in 21% to 45% of treated patients. Furthermore, it has been shown to halt disease progression after orthotopic liver transplantation and to prevent graft failure and the need for retransplantation at least in those subjects with sustained virological response. However, treatment of hepatitis C recurrence after orthotopic liver transplantation in Brazil was not recommended according to ministerial Law number 863. AIMS: To assess the management and treatment options of hepatitis C recurrence after orthotopic liver transplantation in different liver transplantation centers in Brazil. METHODS: Inquiries were sent to active liver transplantation centers throughout the country. RESULTS: Nineteen centers accepted to participate and answered the questionnaire. Altogether they transplanted around 2,800 subjects, half of them with hepatitis C. Immunosuppressive regimen is comprised by tacrolimus and short-term prednisone in 53% of the centers. One third of them claim to use different schedules for hepatitis C patients. Protocol biopsies for diagnosis of recurrence are employed by 13 centers. Different histological criteria are used for the either diagnosis or decision for treatment in most of the centers. Approximately half of them (42%) indicate treatment in subjects with less severe stages of fibrosis (less than F2 according to METAVIR classification). All centers are referring patients for treatment with peguilated interferon and ribavirin, for 1 year, for 6 months or 1 year based on the genotype, or a la carte based on response, respectively, in 32%, 21% and 47% of the centers. Most of them (84%) do not stop treatment in early non-responders at the 12th week. CONCLUSIONS: Even in the absence of national guidelines and federal support, most of the liver transplantation centers in Brazil are treating patients with hepatitis C recurrence after orthotopic liver transplantation.

RACIONAL: Os sintomas gastrointestinais são freqüentes no diabetes mellitus e podem estar relacionados com o estresse oxidativo, que é definido pelo desequilíbrio entre os sistemas pró-oxidante e o antioxidante. OBJETIVO: Avaliar algumas das alterações gastrointestinais no modelo de diabetes mellitus, como o estresse oxidativo no estômago e no fígado de animais diabéticos e o fluxo sangüíneo na artéria mesentérica superior em diferentes tempos de estudo. MATERIAL E MÉTODOS: Os parâmetros utilizados para verificar o estresse oxidativo no fígado e no estômago foram a mensuração da lipoperoxidação, através das técnicas das substâncias reativas ao ácido tiobarbitúrico e da quimiluminescência e a avaliação da atividade das enzimas antioxidantes catalase, superóxido dismutase e glutationa transferase. Utilizaram-se ratos machos Wistar, pesando entre 250-350 g, que foram divididos em quatro grupos: grupo I - 7 dias de diabetes, grupo II- 30 dias de diabetes, grupo III - 60 dias de diabetes e grupo IV - 90 dias de diabetes. O diabetes foi induzido por administração de estreptozotocina 70 mg/kg intraperitonialmente. RESULTADOS: Houve aumento significativo na lipoperoxidação no estômago e no fígado de animais diabéticos somente no tempo de 90 dias. No estômago, foi encontrada significativa diminuição na atividade das enzimas antioxidantes catalase e glutationa transferase. No fígado, somente a enzima glutationa transferase apresentou diminuição significativa. Houve aumento no fluxo da artéria mesentérica superior dos animais diabéticos com 90 dias, quando comparados aos animais-controle. CONCLUSÕES: É possível supor que o aumento no estresse oxidativo no estômago e no fígado e a alteração no fluxo sangüíneo da artéria mesentérica superior sejam influenciados pelo tempo de diabetes e pela hiperglicemia encontrada nos animais estudados, o que determinaria as alterações gastrointestinais. BACKGROUND: Gastrointestinal symptoms are frequent in diabetes mellitus and may be related to the oxidative stress, which is defined by an imbalance between the pro-oxidant and antioxidant systems. AIMS: To assess some of the gastrointestinal changes in the diabetes mellitus model, such as the oxidative stress in the stomach and liver of diabetic animals and the blood flow in the superior mesenteric artery at different times of study. MATERIAL AND METHODS: Male Wistar rats weighing 250-350 g were used and ascribed to four different groups as follows: group I - 7 days of diabetes, group II - 30 days of diabetes, group III - 60 days of diabetes, and group IV - 90 days of diabetes. Diabetes was obtained by administering streptozotocin (70 mg/kg) intraperitoneally. RESULTS: A significant increase in liver and stomach lipoperoxidation of diabetic animals was found only at 90 days. A significant decrease in the activity of antioxidant enzymes catalase e glutathione transferase was found in stomach, whereas in liver only glutathione transferase was significantly decreased. The blood flow in the superior mesenteric artery of animals with 90 days of diabetes was increased as compared to controls. CONCLUSIONS: It is possible to suppose that the increase in the oxidative stress in stomach and liver as well as the change in the blood flow in the superior mesenteric artery are affected by the length of diabetes and the hyperglycemia found in the animals studied, which would determine gastrointestinal changes.

OBJETIVO: Avaliar o uso a longo prazo do flavonóide quercetina em ratos cirróticos por ligadura de ducto biliar comum (LDB). MÉTODOS: Foram utilizados 32 ratos machos Wistar, sendo submetidos à LDB ou simulação, e distribuídos em 4 grupos: 1) controle, 2) cirróticos, 3) cirróticos tratados com quercetina 50mg/kg, intraperitonealmente, desde o segundo dia após o procedimento cirúrgico; e 4) cirróticos tratados após o décimo quarto dia do procedimento cirúrgico. Analisou-se a função hepática por meio de testes bioquímicos (BT e BD) e atividade enzimática (ALT, AST, FA e GGT). Na análise anatomopatológica, utilizou-se a coloração de Hematoxilina & Eosina (H&E) e de Picrosírius para fibrose. A análise estatística para avaliação de sobrevivência foi realizada pelo teste Kaplan-Meier. RESULTADOS: Os resultados de sobrevivência dos oito animais de cada grupo foram: Grupo 1 = 200 dias de sobrevivência; Grupo 2 = 46 dias; Grupo 3 = 71 dias; e o Grupo 4 = 90 dias. Nos animais com ligadura de ducto biliar comum houve aumento das provas de função hepática e enzimáticas que se reduziu hipoteticamente com o tratamento com quercetina. Foram identificadas cirrose, congestão vascular porta e centrolobular na análise histopatológica por H&E e Picrosírius. CONCLUSÃO: O uso da quercetina diminuiu de maneira significante as alterações bioquímicas provocadas pela cirrose, aumentando o tempo de sobrevivência dos animais com cirrose biliar secundária à LDB, como verificado pelo teste de análise de sobrevivência. PURPOSE: The long term use of quercetin flavonoid was evaluated in cirrhotic rats by common biliary duct bondage (LDB). METHODS: 32 male Wistar rats were submitted to LDB or simulation, and distributed in 4 groups: 1) control, 2) cirrhotic, 3) cirrhotic treated with quercetin the second day after the surgical procedure; and 4) cirrhotic treated with quercetin after the fourteenth day of the surgical procedure. The hepatic function was analyzed through biochemical tests (TB and DB) and enzymatic activity (ALT, AST, AP and GGT). In the anatomopatological analysis, Hematoxilin & Eosin (H&E) coloration, and Picrosirius (for fibrosis) were used.The statistical analysis for survival evaluation was accomplished by the Kaplan-Meier test. RESULTS: The results of survival of the eight animals of each group were: Group 1 = 200 days of survival; Group 2 = 46 days; Group 3 = 71 days; and the Group 4 = 90 days. In the animals with bondage of common biliary duct, there was an increase of the hepatic and anzymatic function tests, which was reduced with the treatment with quercetin. Cirrhosis, portal and centrolobular vascular congestion, were identified in the histopatological analysis with H&E and Picrosirius. CONCLUSION: The use of quercetin decreased significantly the biochemical alterations caused by cirrhosis, increasing the time of survival of the animals with secondary biliary cirrhosis due to LDB.

As complicações neurológicas em transplante hepático (TxH) são importante causa de morbi-mortalidade, pois as intercorrências podem resultar de fatores prévios ao TxH, de alterações trans-operatórias, de dificuldades com os imunossupressores, coagulopatias e infecções do período pós-operatório. Nós analisamos , retrospectivamente, a cronologia, as causas e as frequências das alterações neurológicas de 30 pacientes adultos submetidos a TxH e constatamos que nossos resultados diferem ligeiramente dos apresentados em outras séries. Neurologic complications are important source of morbi-mortality, in liver transplantation. They result from previous factors, alterations during the surgical procedure, effects from immunosuppresor drugs, coagulopathy and infections. We analyzed, retrospectively, the chronology, causes, and frequencies of neurologic alterations in thirty adult patients submitted to liver transplantation, and our results differ slighty from those registered in other series.
