Abstract

Background: The Italian Society of Echography and Cardiovascular Imaging (SIECVI) conducted a national survey to understand the volumes of activity, modalities and stressors used during stress echocardiography (SE) in Italy. Methods: We analyzed echocardiography laboratory activities over a month (November 2022). Data were retrieved through an electronic survey based on a structured questionnaire, uploaded on the SIECVI website. Results: Data were obtained from 228 echocardiographic laboratories, and SE examinations were performed in 179 centers (80.6%): 87 centers (47.5%) were in the northern regions of Italy, 33 centers (18.4%) were in the central regions, and 61 (34.1%) in the southern regions. We annotated a total of 4057 SE. We divided the SE centers into three groups, according to the numbers of SE performed: <10 SE (low-volume activity, 40 centers), between 10 and 39 SE (moderate volume activity, 102 centers) and ≥40 SE (high volume activity, 37 centers). Dipyridamole was used in 139 centers (77.6%); exercise in 120 centers (67.0%); dobutamine in 153 centers (85.4%); pacing in 37 centers (21.1%); and adenosine in 7 centers (4.0%). We found a significant difference between the stressors used and volume of activity of the centers, with a progressive increase in the prevalence of number of stressors from low to high volume activity (P = 0.033). The traditional evaluation of regional wall motion of the left ventricle was performed in all centers, with combined assessment of coronary flow velocity reserve (CFVR) in 90 centers (50.3%): there was a significant difference in the centers with different volume of SE activity: the incidence of analysis of CFVR was significantly higher in high volume centers compared to low - moderate - volume (32.5%, 41.0% and 73.0%, respectively, P < 0.001). Abstract

Aim: Epidemiology of wild-type transthyretin cardiac amyloidosis (ATTRwt-CA) remains poorly defined. A better characterization of pathways leading to ATTRwt-CA diagnosis is of key importance, and potentially informative of disease course and prognosis. The aim of this study was to describe the characteristics of contemporary pathways leading to ATTRwt-CA diagnosis, and their potential association with survival. Methods and results: This was a retrospective study of patients diagnosed with ATTRwt-CA at 17 Italian referral centres for CA. Patients were categorized into different ‘pathways’ according to the medical reason that triggered the diagnosis of ATTRwt-CA (hypertrophic cardiomyopathy [HCM] pathway, heart failure [HF] pathway, incidental imaging or incidental clinical pathway). Prognosis was investigated with all-cause mortality as endpoint. Overall, 1281 ATTRwt-CA patients were included in the study. The diagnostic pathway leading to ATTRwt-CA diagnosis was HCM in 7% of patients, HF in 51%, incidental imaging in 23%, incidental clinical in 19%. Patients in the HF pathway, as compared to the others, were older and had a greater prevalence of New York Heart Association (NYHA) class III–IV and chronic kidney disease. Survival was significantly worse in the HF versus other pathways, but similar among the three others. In multivariate model, older age at diagnosis, NYHA class III–IV and some comorbidities but not the HF pathway were independently associated with worse survival. Conclusions: Half of contemporary ATTRwt-CA diagnoses occur in a HF setting. These patients had worse clinical profile and outcome than those diagnosed either due to suspected HCM or incidentally, although prognosis remained primarily related to age, NYHA functional class and comorbidities rather than the diagnostic pathway itself.

Abstract

Background: The Italian Society of Echocardiography and Cardiovascular Imaging (SIECVI) conducted a national survey to understand better how different echocardiographic modalities are used and accessed in Italy. Methods: We analyzed echocardiography laboratory activities over a month (November 2022). Data were retrieved via an electronic survey based on a structured questionnaire, uploaded on the SIECVI website. Results: Data were obtained from 228 echocardiographic laboratories: 112 centers (49%) in the northern, 43 centers (19%) in the central, and 73 (32%) in the southern regions. During the month of observation, we collected 101,050 transthoracic echocardiography (TTE) examinations performed in all centers. As concern other modalities there were performed 5497 transesophageal echocardiography (TEE) examinations in 161/228 centers (71%); 4057 stress echocardiography (SE) examinations in 179/228 centers (79%); and examinations with ultrasound contrast agents (UCAs) in 151/228 centers (66%). We did not find significant regional variations between the different modalities. The usage of picture archiving and communication system (PACS) was significantly higher in the northern (84%) versus central (49%) and southern (45%) centers (P < 0.001). Lung ultrasound (LUS) was performed in 154 centers (66%), without difference between cardiology and noncardiology centers. The evaluation of left ventricular (LV) ejection fraction was evaluated mainly using the qualitative method in 223 centers (94%), occasionally with the Simpson method in 193 centers (85%), and with selective use of the three-dimensional (3D) method in only 23 centers (10%). 3D TTE was present in 137 centers (70%), and 3D TEE in all centers where TEE was done (71%). The assessment of LV diastolic function was done routinely in 80% of the centers. Abstract

After a long period of human persecution that led it to extinction in most of its distribution range, the wolf is undergoing a fast recovery. Despite being described as an elusive species only living in remote areas, wolves are recently occupying also human‐dominated landscapes, increasing the frequency of direct contacts with humans. Nevertheless, it is unclear whether this situation is only caused by a numerical increase or partially facilitated by a higher tolerance of wolves with respect to human proximity. We focused on a European region offering an abundant, widespread and long‐term monitored wolf population to analyse wolf pack distribution and its relationship with human presence across areas recolonized across four different time periods (1972, 1996, 2005 and 2016). Throughout areas recolonized during different periods, wolves were initially located in mountains and hills, occupying plains only in the recent past. Although they consistently tended to be located as far as possible from urban settlements, especially from those with over 5000 inhabitants, in 2016, 70% of packs included at least one urban settlement within the expected home range. Moreover, the distance kept by wolves from the nearest urban settlement was more constrained in areas of recent recolonization (2005 and 2016) and in the mountainous altitudinal range, likely due to a reduced availability of territories. We showed that wolves tend to keep as far away as possible from humans, but they can also permanently occupy human‐dominated landscapes to cope with the lower availability of space induced by their remarkably successful recolonization. Abstract

With stress echo (SE) 2020 study, a new standard of practice in stress imaging was developed and disseminated: the ABCDE protocol for functional testing within and beyond CAD. ABCDE protocol was the fruit of SE 2020, and is the seed of SE 2030, which is articulated in 12 projects: 1-SE in coronary artery disease (SECAD), 2-SE in diastolic heart failure (SEDIA), 3-SE in hypertrophic cardiomyopathy (SEHCA), 4-SE post-chest radiotherapy and chemotherapy (SERA), 5-Artificial intelligence SE evaluation (AI-SEE), 6-Environmental stress echocardiography and air pollution (ESTER), 7-SE in repaired Tetralogy of Fallot (SETOF), 8-SE in post-COVID-19 (SECOV), 9: Recovery by stress echo of conventionally unfit donor good hearts (RESURGE), 10-SE for mitral ischemic regurgitation (SEMIR), 11-SE in valvular heart disease (SEVA), 12-SE for coronary vasospasm (SESPASM). The study aims to recruit in the next 5 years (2021–2025) ≥10,000 patients followed for ≥5 years (up to 2030) from ≥20 quality-controlled laboratories from ≥10 countries. In this COVID-19 era of sustainable health care delivery, SE2030 will provide the evidence to finally recommend SE as the optimal and versatile imaging modality for functional testing anywhere, any time, and in any patient.

Abstract

The detection of hazard exposure is a current priority, including the detection of traces of explosive molecules in different environments like luggage storage rooms and public places, and is becoming a major requirement for homeland security. In the present study we carried out a preliminary investigation on the binding capacities of four forms of the lipocalin odorant-binding protein (OBP) for the detection of explosive components such as diphenylamine, dimethyl-phthalate, resorcinol and dinitrotoluene. The experimental results, showing that OBP binds these compounds with affinity constants ranging between 80 nM and 10.6 mM, indicate that this protein can be used as a probe for the realization of a biosensor to sense explosive compounds.

Abstract

Angiotensin-converting enzyme (ACE) inhibitors reduce mortality, the development of remodeling, left ventricular (LV) dysfunction, and ischemic events, both when administered alone as long-term treatment in patients with impaired LV function and/or heart failure (HF) and as short-term treatment, early after acute myocardial infarction (AMI) and/or HF. The few data available on the use of ACE inhibitors in the elderly after AMI are conflicting. Nothing is known about the effects of ACE inhibitors in elderly postinfarction patients with preserved LV function: these patients have a remarkable medium- to long-term mortality and HF incidence after infarction. The aim of this study is to evaluate, in patients with AMI agedor =65 years, the effects of Perindopril on the combined outcome of death, hospitalization for HF, and heart remodeling, considered to be aor =8% increase in LV end-diastolic volume (LVEDV). Secondary objectives include the same factors listed in the primary end points but considered separately. In addition, safety of the drug, ventricular remodeling, and adaptation are being evaluated. A total of 1100 patients with AMI (first episode or reinfarction), agedor =65 years, and preserved or only moderately depressed LV (LV ejection fractionor =40%), are to be enrolled and randomly assigned to treatment (8 mg for 12 months of Perindopril or placebo, in double-blind conditions). Clinical assessment is performed at fixed times, and periodic evaluations of (1) ventricular shape, dimensions, and function by quantitative 2-D echocardiography, and (2) heart rate variability and arrhythmias by ambulatory electrocardiographic monitoring are anticipated. The results and conclusions will be available by 2002 year.

Abstract

Reproductive synchrony among gregarious mammals has a strong adaptive value and may lead to cooperative behaviors aimed at maximizing offspring survival. Additionally, temporal clustering of estrus has important implications on individual mating tactics and ultimately affects the degree of polygamy in a population. Although several studies have examined the reproductive biology of wild boar (Sus scrofa), much remains to be understood about the patterns of timing and synchrony of reproduction in natural populations. We analyzed the spatiotemporal distribution of conception dates in an Italian wild boar population taking into account the effects of environmental and individual factors, in order to determine the main variables influencing the timing of reproduction and to detect the signs of a socially-driven reproductive synchrony. Specifically, for each litter belonging to 354 pregnant sows culled between 2006 and 2013 in a mountain area of Tuscany, we determined the conception date (CD) from an estimate of the mean fetal age and the culling date. We then investigated which factors drove the variation in CD, by implementing linear mixed models, Mantel tests and spatial autocorrelation analyses. The selected model showed significant effects of rainfall, temperatures, and previous and current productivity on CD, as well as a strong correlation of CDs among sows culled in close spatial and temporal proximity (i.e., in the same hunting ground and hunting season). Likewise, autocorrelation analyses and Mantel tests consistently indicated that closer sows had similar conception dates. Overall, our results confirm the effect of resource availability and climate on wild boar reproductive phenology, and suggest socially-driven reproductive patterns, in spite of a high turn-over in social groups due to hunting. Abstract

We investigated the patterns of wild ungulate selection by wolves in an 860 km2 study area of the Northern Apennines (North Italy), in order to detect seasonal variation in wolf diet and changes of feeding habits in relation to the increased abundance of wild ungulates. From June 2007 to May 2008 we collected wolf scats and recorded the signs of presence of wild ungulate species along 25 transects, representative of the different habitat types within the study area. We analysed the scats to identify the main food items used by wolves in each season and we compared the proportions of biomass of wild ungulate species in the diet (use) to those calculated from the signs of presence (availability). We recorded significant seasonal changes in wolf feeding habits, but the main food items were wild ungulates, mainly wild boars, and medium-sized mammals. Livestock (sheep, goats, and calves) were used only in summer and early autumn, during the grazing period, in small quantities. Comparing the current diet of wolves to that recorded in the past in the same study area highlighted significant differences for wild ungulates and livestock, the former being more used nowadays and the latter less. Our study showed that, in the Northern Apennines, the feeding behaviour of wolves has evolved towards a greater use of wild prey, so reducing the impact on livestock and consequently wolf-human conflicts. Abstract

The grey partridge became extinct in the province of Siena (central Italy) in the late seventies, whereas the red-legged partridge had already disappeared by the beginning of the twentieth century. Some reintroduction attempts of both species carried out in the 1980s gave encouraging but not definitive results, and failed after an initial success. This was probably due to the low number of birds released, the small size of the re-introduction areas, their isolation, the farm-bred origin of the partridges, and hunting. In the province of Siena, for the first time in Italy, a large-scale reintroduction program of grey and red-legged partridges was experimented. The project started up in 1995 with seven reintroduction areas for grey and four for red-legged partridge, and was extended to 19 areas (22,562 ha) for grey and 7 (6858 ha) for red-legged partridge in 2002. Population viability analyses for both species showed that if reintroduced populations were isolated they would be extinct in a few years. Therefore, a metapopulation approach was adopted (contemporary releases in reintroduction areas close to each other). In each area, 100-1000 partridges per year were released for a minimum of 3 years, from different farms in order to achieve the maximum initial genetic diversity. Releases were effected in late summer (August-September) in acclimatization pens containing 10-20 aviaries. The reintroduced population showed marked variability of some demographic parameters, such as pair density and brood production rate; instead, average brood size was relatively constant across the study areas, but with annual variations. Reintroduction success was limited to a few areas only, mainly depending on the habitat characteristics of the areas, their surface area and isolation, and on the degree of care for the birds during the acclimatization period.
