Abstract: Objectives To assess factors influencing the choice and effectiveness of biological disease-modifying antirheumatic drugs (DMARDs) following failure of rituximab (RTX) in rheumatoid arthritis (RA), taking patient profile into account. Methods In a retrospective, multicenter study, data about RA patients starting a new biologic during the year after RTX discontinuation were collected at baseline (when the biologic was introduced after RTX), and during follow-up (3, 6, and 12 months). Characteristics of patients receiving tocilizumab (TCZ), abatacept (ABA), or a TNFα inhibitor (TNFi), EULAR response, and retention rate were compared using multidimensional factorial analysis for patient profiles and multivariate analysis including propensity score built on the patient profile. Results Among 152 patients analyzed (37.5% TCZ, 31.6% ABA, 30.9% TNFi), sex, disease characteristics and activity, concomitant DMARDs or glucocorticoids, and previous use of RTX and TNFi were similar at baseline. Patients receiving ABA were slightly older. Multimorbidity index was higher but not significantly different. Multidimensional factorial analysis showed a distinct profile of patients receiving ABA, characterized by older age, more men, more smokers, more comorbidities, and higher anti-cyclic citrullinated peptide antibody. At 1 year, drug retention was higher for ABA than TNFi after adjustment for disease duration, concomitant DMARDs, glucocorticoids, and propensity score (P = 0.04). Tolerance and serious infections were similar among groups. Conclusions The profile of patients receiving ABA following failure of RTX differed from TNFi and TCZ using multidimensional factorial analysis. After adjustment for propensity score, drug retention rate remained higher with ABA than TNFi.

Abstract: International audience; Immunocompromised patients may experience prolonged viral shedding after their initial SARS-CoV-2 infection, however, symptomatic relapses after remission currently remain rare. We herein describe a severe COVID-19 relapse case of a kidney transplant recipient (KTR) following rituximab therapy, 3 months after a moderate COVID-19 infection, despite viral clearance after recovery of the first episode. During the clinical relapse, the diagnosis was established on a broncho-alveolar lavage specimen (BAL) by RT-PCR. The infectivity of the BAL sample was confirmed on a cell culture assay. Whole genome sequencing confirmed the presence of an identical stain (Clade 20A). However, it had an acquired G142D mutation and a larger deletion of 3-amino-acids at position 143-145. These mutations located within the N-terminal domain are suggested to play a role in viral entry. The diagnosis of a COVID-19 relapse should be considered in the setting of unexplained persistent fever and/or respiratory symptoms in KTRs (especially for those after rituximab therapy), even in patients with previous negative naso-pharyngeal SARS-CoV-2 PCR.

4. Le Rituximab en pneumologie : expérience au CHU de Montpellier

Abstract: Le Rituximab est une biothérapie fréquemment utilisée en pneumologie notamment dans les pneumopathies interstitielles diffuses (PID) associées aux connectivites malgré l’absence d’essai randomisé. Dans les PID idiopathiques, il existe un rationnel physiopathologique pour soutenir son utilisation mais aucune étude n’a été publiée à ce jour. Méthodes : cette étude rétrospective observationnelle était réalisée chez les patients traités par Rituximab entre janvier 2015 et juin 2020 dans le service de pneumologie au CHU de Montpellier. Le critère de jugement principal était l’évolution de la capacité vitale forcée (CVF) et de la DLCO exprimées en pourcentage de la normale à un an de l’introduction du Rituximab chez les patients atteints de PID idiopathiques et associées à une connectivite. Une analyse de la tolérance était également réalisée chez l’ensemble des patients ayant reçu du Rituximab. Résultats : sur 41 patients ayant reçu du Rituximab pour une PID idiopathique (n = 14) ou associée à une connectivite (n = 27), 26 patients étaient toujours sous Rituximab un an après son introduction. La CVF s’améliorait significativement à un an (mediane (IQ25-75) : 63,5 (49,5-74) vs 76 (49-85), à baseline vs 1 an, respectivement, p = 0.0116). La DLCO était stable après un an de traitement. 20/33 (61 %) patients ont présenté une amélioration de CVF > 10 % ou de DLCO > 15 % entre 6 et 12 mois après introduction du Rituximab et 8 patients avaient une fonction respiratoire stable. 32,7 % des patients ont présenté un évènement indésirable du Rituximab, en majorité de nature infectieuse. Conclusion : notre expérience montre une efficacité du Rituximab dans la prise en charge des PID idiopathiques ou de connectivites, avec une tolérance satisfaisante.

5. Incidence et facteurs de risque d’infections sévères en cas de déficit acquis en immunoglobulines chez des patients traités par Rituximab pour une maladie inflammatoire à médiation immune

Abstract: Le rituximab est efficace pour traiter les maladies inflammatoires à médiation immune. Une hypogammaglobulinémie peut apparaitre sous traitement, et majorer le risque infectieux. La littérature à ce sujet étant limitée, une étude de cohorte rétrospective monocentrique a été réalisée, afin de déterminer l’existence d’un surrisque d'infection sévère en cas de survenue d'un déficit en immunoglobulines sous rituximab. 311 patients ont été inclus, avec un suivi rétrospectif de 1623,7 patients-années. 15% des patients ont présenté une infection sévère (2,77 / 100 PA). 68 patients ont développé une hypogammaglobulinémie, principalement pour les IgM (12%) et les IgG (8%). Aucun surrisque infectieux n’a été détecté en cas d'hypogammaglobulinémie acquise. Par contre, le taux d'infection sévère est plus élevé en cas de déficit prévalent au traitement. La corticothérapie, les comorbidités pulmonaires chroniques et le traitement d'une pathologie autre que la polyarthrite rhumatoïde majorent le risque. Après ajustement, les pathologies pulmonaires chroniques et la corticothérapie au cours du suivi sont associées au surrisque d'infection sévère. Au total, l'étude n'a pas montré de majoration du risque d'infection sévère en cas de déficit acquis en immunoglobuline, au contraire d'autres études de cohorte préexistantes. Ces données incitent à poursuivre la surveillance régulière du dosage des immunoglobulines sous rituximab. La conduite à tenir en cas de survenue d'une hypogammaglobulinémie doit être discutée au cas par cas, en prenant en compte les autres traitements associés, et les comorbidités notamment pulmonaires. La recherche d'un point d'appel infectieux doit rester une priorité avant la réalisation d'une perfusion de rituximab.

Abstract: The anti–von Willebrand factor nanobody caplacizumab was licensed for adults with immune-mediated thrombotic thrombocytopenic purpura (iTTP) based on prospective controlled trials. However, few data are available on postmarketing surveillance. We treated 90 iTTP patients with a compassionate frontline triplet regimen associating therapeutic plasma exchange (TPE), immunosuppression with corticosteroids and rituximab, and caplacizumab. Outcomes were compared with 180 historical patients treated with the standard frontline treatment (TPE and corticosteroids, with rituximab as salvage therapy). The primary outcome was a composite of refractoriness and death within 30 days since diagnosis. Key secondary outcomes were exacerbations, time to platelet count recovery, the number of TPE, and the volume of plasma required to achieve durable remission. The percentage of patients in the triplet regimen with the composite primary outcome was 2.2% vs 12.2% in historical patients (P = .01). One elderly patient in the triplet regimen died of pulmonary embolism. Patients from this cohort experienced less exacerbations (3.4% vs 44%, P < .01); they recovered durable platelet count 1.8 times faster than historical patients (95% confidence interval, 1.41-2.36; P < .01), with fewer TPE sessions and lower plasma volumes (P < .01 both). The number of days in hospital was 41% lower in the triplet regimen than in the historical cohort (13 vs 22 days; P < .01). Caplacizumab-related adverse events occurred in 46 patients (51%), including 13 major or clinically relevant nonmajor hemorrhagic events. Associating caplacizumab to TPE and immunosuppression, by addressing the 3 processes of iTTP pathophysiology, prevents unfavorable outcomes and alleviates the burden of care.

Abstract: International audience; Venetoclax (Ven), an orally administered, potent BCL-2 inhibitor, has demonstrated efficacy in chronic lymphocytic leukaemia (CLL) in combination with rituximab (R) or obinutuzumab (G). Our aim was to investigate the addition of bendamustine (B) to these Ven-containing regimens in relapsed/refractory (R/R) or first-line (1L) CLL. This multi-arm, non-randomized, open-label, phase 1b study was designed to evaluate the maximum tolerated dose (MTD) and safety/tolerability of Ven with BR/BG, with 3+3 dose-escalation followed by safety expansion. Patients received Ven (schedule A) or BR/BG first (schedule B) to compare safety and determine dose/schedule for expansion. Six Ven-BR/-BG cycles were to be administered, then Ven monotherapy until disease progression (R/R) or fixed-duration 1-year treatment (1L). Overall, 33 R/R and 50 1L patients were enrolled. No dose-limiting toxicities were observed (doses 100–400-mg), and the MTD was not reached. Safety was similar between schedules; no tumour lysis syndrome (TLS) occurred during dose-finding. Schedule B and Ven 400-mg were chosen for expansion. The most frequent grade 3–4 toxicity was neutropenia: R/R 64%, 1L Ven-BR 85%, 1L Ven-BG 55%. Grade 3–4 infection rate was: R/R 27%, 1L Ven-BR 0%, 1L Ven-BG 27%. During expansion, one clinical and two laboratory TLS cases occurred. Fewer than half the patients completed six combination therapy cycles with all study drugs; rates of bendamustine discontinuation were high. Overall response rate was 91% in R/R and 100% in 1L patients (16/49 1L patients received Ven for >1 year). In conclusion, addition of bendamustine to Ven-R/-G increased toxicity without apparent efficacy benefit.

Abstract: International audience; To investigate the frequency and risk factors of postoperative complications in RA patients treated with tocilizumab (TCZ).METHODS: the French registry REGATE recruited 1496 RA patients receiving TCZ in routine care. Data from patients treated with TCZ who underwent surgery were reviewed. Frequency of post-surgery complications was collected and compared in patients with and without complications in order to identify factors associated with complications. Similar analysis was performed in patients with postoperative infection.RESULTS: we identified 167 patients who underwent 175 surgical procedures including 103 orthopaedic surgeries (58.9%). The patients were mainly women (84%) with a mean disease duration of 14.96±11.29 years. The mean delay between surgery and the last TCZ infusion was 4.94±1.74 weeks. Fifteen patients experienced 15 complications (8.6%) with 10 severe infections including 5 surgical site infections (33.3%). There was no significant difference between patients with and without complications. In multivariate analysis, previous treatment with rituximab in the previous year tended to be associated with postoperative complications (OR: 3.27, IC95% 0.92–11.49, p=0.06). Concerning postoperative infections, diabetes mellitus tended to be associated with this complication (OR: 3.73, IC95% 0.88–15.79, p=0.06) in multivariate analysis.CONCLUSIONS: in RA patients treated with TCZ in perfusion, the rate of surgical complications was low: 8.6%. The median time between surgery and last infusion was relatively short according to half-life of TCZ but did not influence the rate of postoperative complications.

10. Risk of diverticulitis and gastrointestinal perforation in rheumatoid arthritis treated with tocilizumab compared to rituximab and abatacept: a prospective propensity-matched cohort study

Abstract: Objective: to investigate the risk of diverticulitis and gastrointestinal perforation (GIP) in patients with rheumatoid arthritis (RA) treated with tocilizumab (TCZ) compared to rituximab (RTX) and abatacept (ABA). Methods: we conducted a population-based study using three observational french registries evaluating the effectiveness and the safety of RTX, ABA, and TCZ in RA patients. Using a propensity score approach, we compared the risk of diverticulitis or GIP in these patients.Results: twenty-one vs 20 diverticulitis (corresponding incidence = 5.3 vs 2.3/1000 person-years) and 9 vs 10 GIP (2.3 vs 1.1/1000 person-years) occurred respectively in TCZ treated patients (n=1496) compared to RTX or ABA (n=3005). Based on inverse probability weighting, there were an increased risk of diverticulitis (OR 4.16 [95% CI 2.66-6.50]) and GIP (3.96 [2.18-7.20]) in TCZ treated patients compared to RTX and ABA. In a subgroup analysis, we observed an increased risk of GIP due to diverticulitis but not to any other etiology. Compared to RTX and ABA, diverticulitis and GIP among TCZ patients occurred earlier after the last perfusion (p=0.01), with atypical clinical presentation (slow transit in 30%, p=0.04) and lower acute phase reactants when the event occurred (C-reactive protein: 31.2±58.4 vs 88.2±89.6 mg/L, p=0.005).Conclusion: TCZ was associated with an increased risk of diverticulitis, GIP and GIP due to diverticulitis but not to other etiologies compared to RTX and ABA. Our study confirmed an increased risk of GIP in RA patients treated with TCZ that could be explained by an increased risk of diverticulitis with disconcerting clinical presentation.; Il existe un risque de perforation digestive chez les patients atteints de polyarthrite rhumatoïde (PR) traités par tocilizumab (TCZ) en comparaison aux traitements conventionnels synthétiques (csDMARDs) ou aux anti-TNFα (TNFi) (2,3). Néanmoins, une étude plus récente n’a pas retrouvé d’augmentation de ce risque en comparaison aux TNFi ni aux autres biothérapies (rituximab (RTX) ou abatacept (ABA)) (4). Notre objectif était de comparer le risque de diverticulite et de perforation digestive sous TCZ en comparaison au RTX et à l’ABA dans la PR à partir des données des registres de la société française de rhumatologie (SFR). Patients et méthodes : nous avons analysé les données de patients atteints de PR (répondant aux critères ACR 1987), suivis prospectivement dans 3 registres français évaluant l’efficacité et la tolérance du TCZ (REGATE), du RTX (AIR-PR) et de l’ABA (ORA) dans 107 centres français, de septembre 2005 à août 2013. Parmi les événements indésirables (EI) recensés, nous avons identifié les EI correspondant à des diverticulites ou des perforations digestives, puis nous avons collecté les données cliniques et paracliniques de ces EI à partir d’une fiche de recueil de donnée établie à priori et envoyée dans les différents centres. Nous avons comparé le risque de diverticulite et de perforation digestive sous TCZ en comparaison aux autres biothérapies (RTX et ABA « poolés »), après pondération sur l’inverse du score de propension visant à contrôler l’éventuel biais d’indication portant sur le possible choix préférentiel entre les biothérapies selon les caractéristiques et facteurs de risque propres du patient (variables inclues dans le score de propension : âge, sexe, diabète, néoplasie, score de comorbidité de Charlson, nombre de DMARDs et d'anti-TNF antérieurs, antécédent d'anti-TNF, durée d’évolution de la PR, corticothérapie à l’inclusion, association à un traitement de fond conventionnel, DAS28 moyen durant le suivi).Résultats : parmi 4501 patients inclus (exposition 12 700, 8 patients-années (PA)), nous avons identifié 41 diverticulites et 19 perforations digestives, correspondant respectivement à une incidence de 3,23 et 1,50/1000 PA dans la population totale, et plus spécifiquement à 5,3 et 2,3/1000 PA sous TCZ. Abstract: Objectifs : la fréquence et les conséquences des anticorps anti-médicaments contre le rituximab (RTX-HACA) sont mal connues dans la polyarthrite rhumatoïde (PR) et encore moins dans les autres maladies auto-immunes systémiques (sMAI). Notre objectif était d’évaluer la fréquence, les conséquences et les facteurs prédictifs des RTX-HACA dans la PR et les autres sMAI. Méthodes : tous les patients traités par rituximab (RTX) de 2012 à 2017 dans notre centre de référence tertiaire de maladies auto-immunes systémiques présentant une PR ou une autre sMAI ont été étudiés rétrospectivement. Les patients pour qui une recherche de RTX-HACA a été effectuée ont été identifiés. Résultats : 199 patients ont été traités par RTX (n=124 PR et n=75 autres sMAI). Parmi les 62 patients (31,1%) pour qui les RTX-HACA ont été dosés, la fréquence de ceux-ci étaient de 2,4% et 14,7% dans la PR et dans les sMAI respectivement (p=0,0026). La majorité des patients immunisés avaient présenté des réactions à partir de la deuxième perfusion de RTX (11 patients sur les 14 immunisés) ou une perte d’efficacité (2/14). Abstract: International audience; Rituximab (RTX), currently recommended as first-line treatment in moderate to severe pemphigus vulgaris (PV) and superficial pemphigus (PS) along with initial systemic steroids, may also be used as second-line or subsequent treatment, and this therapeutic strategy was investigated in a real-life monocentre retrospective survey.Material and methods: all patients treated between January 2010 and March 2018 with RTX as second-line or subsequent treatment for moderate to severe PV or PS and followed for at least one year were included. The main objective was to evaluate rates and times of complete clinical remission (CCR) after a first course of RTX. The secondary objectives consisted mainly of treatment safety, and frequency and time to relapse after the initial CCR.Results: the 24 patients selected received on average 2 cycles of RTX (i.e. 24 initial cycles and 24 additional cycles in all) over a mean follow-up period of 45 months. 18/24 (75%) patients achieved initial CCR within a mean 7.7 months. Despite at least one relapse in 13/18 initially responding patients regardless of relapse time, 59% (14/24) and 33% (8/24) were either in CCR and off treatment, or in partial remission, whether treated or untreated, according to the latest patient news, with an overall response rate of 92%. Safety was fair in these fragile patients.Discussion and conclusion: this survey of the practical use of RTX confirms its interest in moderate to severe pemphigus as a second-line or subsequent treatment, a situation that probably remains relevant even if this molecule is increasingly used as first-line therapy.; Le rituximab (RTX), actuellement recommandé en première ligne dans les formes modérées à sévères de pemphigus vulgaire (PV) ou superficiel (PS) en association à une corticothérapie initiale, peut également être utilisé en 2nde ligne ou plus. Ce schéma d’utilisation a été l’objet d’une analyse rétrospective monocentrique.Matériel et méthodes : tous les patients de notre centre traités entre janvier 2010 et mars 2018 pour PV ou PS modéré à sévère par du RTX en 2nde ligne ou plus et suivis au moins un an ont été inclus. L’objectif principal était évaluer le taux et le délai d’obtention d’une rémission clinique complète (RCC) après une première cure. Les objectifs secondaires étaient d’évaluer la tolérance du traitement, la fréquence et le délai de rechute après RCC initiale.Résultats : les 24 patients inclus ont reçu en moyenne 2 cycles de RTX (24 cycles initiaux et 24 cycles additionnels au total) au cours d’un suivi moyen de 45 mois. Dix huit sur 24 (75 %) ont obtenu une RCC initiale dans un délai moyen de 7,7 mois. Malgré l’apparition d’au moins une rechute chez 13/18 patients répondeurs tous cycles confondus, 59 % (14/24) étaient en RCC sans traitement aux dernières nouvelles pour un taux global de réponse de 92 %. La tolérance était satisfaisante chez ces patients fragiles.Discussion et conclusion : cette étude confirme l’intérêt du RTX dans le traitement du pemphigus modéré à sévère en 2nde ligne, une situation qui reste probablement d’actualité même si son emploi en première ligne est actuellement recommandé.

Abstract: International audience; Purpose:Progressive multifocal leukoencephalopathy (PML) is a rare but severe demyelinating disease caused by the polyoma-virus JC (JCV) in immunocompromised patients. We report a series of patients with primary immune deficiencies (PIDs) who developed PML. Methods:Retrospective observational study including PID patients with PML. Clinical, immunological, imaging features, and outcome are provided for each patient. Results:Eleven unrelated patients with PIDs developed PML. PIDs were characterized by a wide range of syndromic or genetically defined defects, mostly with combined B and T cell impairment. Genetic diagnosis was made in 7 patients. Before the development of PML, 10 patients had recurrent infections, 7 had autoimmune and/or inflammatory manifestations, and 3 had a history of malignancies. Immunologic investigations showed CD4 + lymphopenia (median 265, range 50-344) in all cases. Six patients received immunosuppressive therapy in the year before PML onset, including prolonged steroid therapy in 3 cases, rituximab in 5 cases, anti-TNF-α therapy, and azathioprine in 1 case each. Despite various treatments, all but 1 patient died after a median of 8 months following PML diagnosis. Conclusion: PML is a rare but fatal complication of PIDs. Many cases are secondary to immunosuppressive therapy warranting careful evaluation before initiation subsequent immunosuppression during PIDs.

Abstract: International audience; Tumor engraftment followed by monoclonal antibody (mAb) therapy targeting tumor antigens represents a gold standard for assessing the efficiency of mAbs to eliminate tumor cells. Mouse models have demonstrated that receptors for the Fc portion of immunoglobulin G (FcγRs) are critical determinants of mAb therapeutic efficacy, but the FcγR-expressing cell populations responsible remain elusive. We show that neutrophils are responsible for mAb-induced therapy of both subcutaneous syngeneic melanoma and human breast cancer xenografts. mAb-induced tumor reduction, abolished in neutropenic mice, could be restored in FcγR-deficient hosts upon transfer of FcγR+ neutrophils or upon human FcγRIIA/CD32A transgenic expression. Finally, conditional knockout mice unable to perform FcγR-mediated activation and phagocytosis specifically in neutrophils were resistant to mAb-induced therapy. Our work suggests that neutrophils are necessary and sufficient for mAb-induced therapy of subcutaneous tumors, and represent a new and critical focal point for optimizing mAb-induced immunotherapies that will impact on human cancer treatment.

Abstract: Background Epstein-Barr virus (EBV) displays oncogenic properties, particularly in the immunocompromised host. Notably, hematopoietic stem cell transplantation (HSCT) recipients with a detectable blood EBV viral load (BEBVL) are considered at higher risk of post-transplant lymphoproliferative diseases (PTLD). Therefore, BEBVL is monitored after HSCT, and preemptive rituximab may be used in patients with high values. However, little is known about post-HSCT BEBVL dynamics, and the threshold that should lead to anti-CD20 therapy is poorly defined. Methods We retrospectively analyzed the post-HSCT BEBVL of 332 adult HSCT recipients in our center from 2005 to 2013, including the effect of rituximab. Results Detection of BEBVL > 100, 1000, 5000, 10,000, and 50,000 copies/mL occurred in respectively 77.7%, 69.6%, 37.0%, 27.1%, and 7.5% of the patients after a respective median time of 9, 14, 15, 16, and 14 weeks. No BEBVL threshold was associated with an overall survival difference. Seventy-eight patients received rituximab, with a BEBVL decrease in most. Among patients with detectable BEBVL, long-term survival did not differ in rituximab-treated and non-treated, except for patients with BEBVL ≥50,000. Only 1 case of PTLD was observed. Conclusions BEBVL is frequently detectable after HSCT, but suggests no strong association with survival. Preemptive rituximab therapy threshold remains to be defined. Abstract: International audience; Current preclinical dosimetric models often fail to take account of the complex nature of absorbed dose distribution typical of in vitro clonogenic experiments in targeted radionuclide therapy. For this reason, clonogenic survival is often expressed as a function of added activity rather than the absorbed dose delivered to cells/cell nuclei. We designed a multi-cellular dosimetry model that takes into account the realistic distributions of cells in the Petri dish, for the establishment of survival curves as a function of the absorbed dose. General-purpose software tools were used for the generation of realistic, randomised 3D cell culture geometries based on experimentally determined parameters (cell size, cell density, cluster density, average cluster size, cell cumulated activity). A mixture of Monte Carlo and analytical approaches was implemented in order to achieve as accurate as possible results while reducing calculation time. The model was here applied to clonogenic survival experiments carried out to compare the efficacy of Betalutin®, a novel 177Lu-labelled antibody radionuclide conjugate for the treatment of non-Hodgkin lymphoma, to that of 177Lu-labelled CD20-specific (rituximab) and non-specific antibodies (Erbitux) on lymphocyte B cells. The 3D cellular model developed allowed a better understanding of the radiative and non-radiative processes associated with cellular death. Our approach is generic and can also be applied to other radiopharmaceuticals and cell distributions.

Abstract: Objectives: We compared the ability of antibodies against cyclic citrullinated peptides (anti-CCP2), against mutated citrullinated vimentin (anti-MCV) and against citrullinated fibrinogen (AhFibA) to predict 1 year rapid radiographic progression (RRP; total Sharp score variation ≥5 points), in early rheumatoid arthritis (RA).Methods: We analysed 566 patients from the ESPOIR cohort with early RA fulfilling the 2010 American College of Rheumatology/European League Against Rheumatism (ACR/EULAR) criteria at year 1. We assayed the 3 anticitrullinated peptide antibodies (ACPA) tests on baseline sera. We compared the performance of these 3 ACPA tests to predict first-year RRP, by comparing areas under the receiver operating characteristic curves (ROCs). We assessed the 1 year RRP risk by ACPA titres. We used a logistic multivariate regression to analyse RRP risk in terms either of ACPA positivity or titre: high (>3 times the N cut-off) and low (1 to 3N).Results: 145 patients displayed RRP. Areas under the ROCs were similar (0.60) for the 3 tests. High ACPA titres were associated with 1 year RRP, whatever the test was, and with similar ORs. Low+ anti-MCV titres were not associated with 1-year RRP, whereas low+ anti-CCP2 titres (p=0.0226) and low+ AhFibA titres (p=0.0332) were significantly associated. Abstract: Context: Idiopathic childhood steroid-dependent nephrotic syndrome (SDNS) remains a therapeutic challenge, since remission is maintained thanks to steroid and immunosuppressants with significant drug toxicity. The aim of this study was to investigate the short and long-term efficacy and safety of Rituximab (RTX), an anti-CD20 monoclonal antibody, in idiopathic childhood SDNS. Abstract: International audience; B-cell depletion therapy (BCDT) based on rituximab (RTX) induces clinical remission in a majority of seropositive patients with Rheumatoid arthritis (RA). However, all patients eventually relapse. The aim of this study was to determine whether dynamic changes in combinations of serological measures of B-cell activation were associated over up to three cycles of BCDT. We included only RA patients who gave an adequate clinical response, as measured by DAS28. Twenty three patients were studied over 1 cycle, 21 over 2, and 15 over 3 cycles of BCDT. Serum analytes including isotypes of Rheumatoid factors (RhF) and anti-citrullinated protein/peptide antibodies (ACPA), B-cell activating factor (BAFF), serum free light chains (SFLC), soluble CD23 (sCD23), antibodies to tetanus toxoid (TT) and to pneumococcal capsular polysaccharide (PCP) were measured by ELISA at 4 key points in each cycle, namely: Baseline (pre-RTX in each cycle); when B-cell depleted (CD19+B-cells < 5/μl); at B-cell return (CD19+B-cells ≥ 5/μl); and at clinical relapse (ΔDAS28 > 1.2). SFLC were used as a measure of plasmablast activity. As sCD23 is cleaved from naïve B-cells coincident with attaining CD27 expression, levels were used as a novel measure of maturation of B-cells to CD27+. The most consistent changes between baseline and B-cell depletion within all 3 cycles were in SFLC, sCD23 and IgM-RhF which fell and in BAFF levels which rose. After 3 complete cycles of BCDT, both IgM autoantibodies and IgG-CCP had decreased, BAFF levels were higher (all p < 0.05); other analytes remained unchanged compared with baseline. Dynamic changes in λSFLC, sCD23, IgM-RhF and BAFF were also consistently associated with relapse in patients with longer clinical responses after B-cell return. Incremental rises in sCD23 levels in cycles 2 and 3 were correlated with time to relapse. Repopulation of the periphery after BCDT is initiated by naïve B-cells and precedes relapse. Our study showed that differentiation into plasmablasts, attended by sCD23 and SFLC production and IgM-RhF specificity may be required to precipitate relapse in patients experiencing longer responses after RTX. These studies also provide novel information related to the resumption of autoimmune responses and their association with B-cell kinetics following BCDT.

Abstract: International audience; OBJECTIVES: To assess the efficacy of the anti-CD20 antibody rituximab in improving physical function and health-related quality of life (HRQoL) in patients with active primary Sjögren's syndrome (pSS), as well as the duration and sources of HRQoL improvements. METHODS: Sixteen patients with pSS received rituximab infusions (375 mg/m2) at weeks 0 and 1 and were followed up for 36 weeks. All patients fulfilled 2002 American-European Consensus Group criteria for pSS and had active disease defined as scores >50 mm on two of four 100-mm visual analogue scales (VAS) evaluating global disease activity, fatigue, pain, and dryness. Standardised evaluations including the Short Form 36 Health Survey (SF-36) were conducted. SF-36 score changes from baseline to weeks 12, 24, and 36 were assessed. RESULTS: Baseline mean SF-36 scores were considerably decreased, compared to the general same-age population. Role-physical (14.1 ± 27.4), role-emotional (12.5 ± 29.9), vitality (26.2 ± 14.3), and general health (32.6 ± 11.2) were the dimensions with the worst scores. Twelve weeks after rituximab, the mental component summary score was improved in 15 patients (mean improvement, 31.2 ± 36.4, p=0.001) and the physical component summary score in 14 patients (mean improvement, 16.9 ± 26.2, p=0.049). Further improvements occurred from week 12 to week 24, and most of the gains were sustained at week 36. Improvements in the physical and mental component summary scores failed to correlate with improvements in the VAS scores. CONCLUSIONS: Substantial alterations in HRQoL were noted in patients with pSS. Rituximab infusions without corticosteroid therapy produced meaningful improvements in HRQoL. Controlled studies of rituximab are needed.
