Background: A divergent fracture pattern is seen in patients living with type 1 diabetes (T1D). These patients are at risk for fractures at unusual distal sites over and above the fractures occurring at major osteoporotic sites. Avascular necrosis (AVN) involving the talus has not been reported previously in T1D. Case Presentation: We hereby report an exceptional case of a 24-year old patient of T1D, who reported to us 3 months post-partum with swelling and pain over her right ankle. In the third trimester of her pregnancy, she encountered trivial trauma to her right ankle. One month after delivery, she developed fragility fractures over her left 2nd to 4th metatarsal heads. On further workup, she was found to have low bone mass and avascular necrosis of right talus on magnetic resonance imaging. On follow up 1 year later, she continued to have low bone mass although her bone mineral density (BMD) increased at the lumbar spine and hip. She was diagnosed with right talar AVN due to fragility fracture sustained in the third trimester with low bone mass consequent to T1D. A possible role of pregnancy- and lactation-related osteoporosis was considered in view of the chronological association with pregnancy. The effect of preoperative first metatarsal pronation on postoperative prognosis of hallux valgus (HV) surgery is under investigation. Utilizing semi-weight-bearing computed tomography, the preoperative pronation angle was assessed to quantify its impact on postoperative prognosis. In a retrospective analysis of 31 feet, those with re-increased hallux valgus angle postoperatively were classified as the non-maintained group, and the remainder as the maintained group. Preoperative pronation angles were compared to establish a threshold. Subsequently, feet were re-classified into high or low-pronation categories. The relative risk of non-maintenance in high-pronation category was calculated. The non-maintained group exhibited a significantly higher preoperative pronation angle (p = 0.021), with a 28.4º threshold. The high-pronation category had a relative risk of 2.34 for non-maintenance. Increased preoperative first metatarsal pronation angle is associated with correction loss after HV surgery. Fracture is the act of breaking or dissolution in the continuity of a bone, with or without displacement of bone fragments. A 7-month-old Balami-Ouda cross Ram was presented at the Large Animal Clinic of the Veterinary Teaching Hospital of Usmanu Danfodiyo University Sokoto with a complaint of lameness on the left hind limb which was noticed 3 days prior to presentation. Radiography of the limb, revealed a swollen left metatarsophalangeal joint with an impacted fracture at the distal end of the metatarsus and a valgus deviation of the phalanges. History further disclosed that the ram might be a victim of stampede by some cows in the farm, as it was managed semi-intensively with other animals (8 ewes, 6 rams and 5 cattle). The fracture was managed by closed reduction and external fixation using Plaster of Paris (POP) and a splint to redirect the valgus deviated limb back to its original position. This POP was removed on the 6th week after complete healing was ascertained on a radiograph and complete weight bearing was achieved. The ram was fully discharged at week 7. Background: This systematic review and meta-analysis aimed to investigate the differences in hallux valgus angle (HVA), intermetatarsal angle (IMA), American Orthopedic Foot and Ankle Society (AOFAS) scores, and complication rates between chevron osteotomy and scarf osteotomy for correcting hallux valgus. Methods: Two investigators independently searched for randomized controlled trials (RCTs) published from 2007 to 2018 on PubMed, Web of Science, and Cochrane Library databases. Next, chevron and scarf osteotomies were compared for their postoperative outcomes using HVA, IMA, and AOFAS scores and their complication rates. Meta-analysis was performed using Review Manager (version 5.3). Results: Six RCTs—comprising 507 feet, of which 261 and 246 underwent chevron and scarf osteotomies, respectively—were included. The meta-analysis revealed that chevron osteotomy led to significantly smaller postoperative HVAs than scarf osteotomy (weighted mean difference [WMD] = -1.94, 95% CI = − 2.65 to − 1.29, P <.00001). However, the differences in postoperative IMA (WMD = − 0.44, 95% CI = − 1.10 to 0.22, P =.19), postoperative AOFAS scores (WMD = 0.75; 95% CI = − 5.32 to 6.82; P =.81), and complication rates (risk ratio = 1.22, 95% CI = 0.65–2.27, P =.53) between feet that underwent chevron and scarf osteotomies were nonsignificant. Conclusions: Compared with scarf osteotomy, chevron osteotomy had significantly more favorable postoperative outcomes in terms of HVA correction, but not in terms of IMA, AOFAS scores, or complication rates. AbstractHallux valgus is a common foot deformity characterized by outward tilting and twisting of the big toe, often accompanied by a medial prominence at the base. Minimally invasive surgical techniques are widely utilized for treating metatarsus adductus due to their advantages of smaller incisions, faster recovery, and early weight-bearing. However, due to individual variations and limited sample size, the biomechanical effects of different Kirschner wire fixation methods and the underlying mechanisms of postoperative metatarsalgia remain unclear. In this study, a finite element method was employed to develop a biomechanical model of metatarsus adductus. The influence of various Kirschner wire entry points and angles on foot loading characteristics was investigated. Six different Kirschner wire fixation models, including two entry methods (along the adjacent fracture line and proximal-biased entry at the midshaft of the metatarsal) with different entry angles, were analyzed. Mechanical parameters such as metatarsal stress distribution, plantar pressure distribution, and displacement of the first metatarsal osteotomy plane were assessed. This research aims to enhance understanding of minimally invasive surgery and its fixation methods for metatarsus adductus. By providing scientific support and reliable evidence, it seeks to contribute to the development of minimally invasive surgical techniques and the improvement of clinical practice in metatarsus adductus surgery. A new monotypic genus of Prodidominae, Paleotoca gen. n., is proposed to include one cave species collected in a paleoburrow and ferruginous caves from Quadrilátero Ferrífero, Minas Gerais, Brazil: Paleotoca diminassp. n. (♂♀). The new genus is closely related to other Neotropical Prodidominae by sharing the classic claw tuft clasper. Paleotoca gen. n. is diagnosed by the absence of a dorsal abdominal scutum, a ventral parallel rows of strong spines on the tibia and metatarsus I–II, a lack of a conductor, a discrete median apophysis on the bulb and a bifid retrolateral tibial apophysis in the male palp, a posterior extension that is beak-shaped, and folds of a copulatory duct ventrally visible in the female epigyne. Like other Prodidominae species from caves, P. diminassp. n. is a troglobitic spider with morphological characteristics that indicate specialization to live in subterranean environments, including reduction in cuticular pigments, eye loss, heavy spination and trichobothria. Recurrent deformity following hallux valgus surgery can be technically challenging to treat. In cases of revision surgery, a surgical technique with greater corrective power is often chosen compared to the primary surgery. Therefore, minimally invasive surgery is not commonly performed. On the other hand, minimally invasive surgery minimizes soft tissue damage and allows for greater correction of deformity compared to traditional open approaches. This paper reports four cases of recurrent hallux valgus treated with a minimally invasive distal transverse metatarsal osteotomy - Akin osteotomy (MITA), resulting in significant improvements in the clinical and radiographic outcomes. Background: Previous research in sport populations has demonstrated that abnormal magnetic resonance imaging (MRI) findings may be present in individuals without symptoms or known pathology. Extending this understanding to ballet, particularly in relation to the foot and ankle, is important to guide medical advice given to dancers. Purpose: To assess foot and ankle MRI scans in asymptomatic ballet dancers focusing on bone marrow edema and the posterior ankle and to investigate whether these MRI findings would become symptomatic within 1 year. Study Design: Case series; Level of evidence, 4. Methods: In total, 31 healthy dancers (62 feet/ankles; 15 male and 16 female; age, 26.5 ± 4.3 years) who were dancing in full capacity were recruited from an elite professional ballet company. Orthogonal 3-plane short tau inversion recovery imaging of both feet and ankles was obtained using 3T MRI and the images were reviewed using a standardized evaluation form by 2 musculoskeletal radiologists. Injuries in the company were recorded and positive MRI findings were assessed for correlation with any injuries requiring medical attention during the subsequent 12 months. Results: A total of 51 (82%) of the 62 feet and ankles had ≥1 area of bone marrow edema. The most common locations of bone marrow edema were the talus (n = 41; 66%), followed by first metatarsal (n = 14; 23%). Os trigonum and Stieda process were seen in 5 (8%) and 8 (13%) ankles, respectively. Among them, 2 os trigona showed bone marrow edema. Fluid in the anterior and posterior talocrural joints and the subtalar joint was observed in 48%, 63%, and 63% of these joints, respectively. Fluid around foot and ankle tendons was observed, with the most prevalent being the flexor hallucis longus tendon (n = 13; 21%). Two dancers who had positive findings on their MRI subsequently developed symptoms during the next 12 months. Conclusion: Positive MRI findings are commonplace in the foot and ankle of asymptomatic professional ballet dancers. The majority do not result in the development of symptoms requiring medical attention within 12 months. Objective: The aim of the study is to evaluate the effect of minimally invasive ultrasound-guided fascial release and a foot orthoses with first metatarsal head cutout on the biomechanics of the medial longitudinal arch of the foot in cadaveric specimens. Design: A cross-sectional study was designed (20 body donors). Anthropometric measurements of the foot, foot posture index, and the windlass test and force were measured in different conditions: unloaded, loaded position, with foot orthoses, after a 25% plantar fascia release and after a 50% release. Results: For the anthropometric measurements of the foot, differences were found in foot length (P = 0.009), arch height (P < 0.001), and midfoot width (P = 0.019) when comparing the unloaded versus foot orthoses condition. When foot orthoses were compared with 25% plantar fascial release, differences were found in foot length (P = 0.014) and arch height (P < 0.001). In the comparison with 50% plantar fascial release, differences were found in the arch height (P < 0.001). A significant interaction between foot orthoses condition and grades was found in the arch height during the windlass test (P = 0.021). Conclusions: The results indicate that the presence of foot orthoses leads to a significant increase in arch height compared with other conditions. Purpose: The minimally invasive Chevron-Akin (MICA) is considered the third generation of minimally invasive hallux valgus (HV) surgery, and its original description included fixation of the Akin osteotomy with a screw. The aim of this study is to evaluate a series of patients undergoing HV correction using the MICA technique without screw fixation of the Akin osteotomy. Methods: We retrospectively evaluated 58 consecutive patients who underwent surgical correction for HV between August 2018 and March 2020. A total of 69 feet were evaluated with a minimum follow-up of 2 years. Clinical outcomes such as pain (VAS), function (AOFAS), range of movement, criteria personal satisfaction and complications were evaluated. Results: The AOFAS score (mean ± standard deviation) significantly improved from 57.0 ± 8.6 preoperatively to 93.9 ± 8.7 postoperatively (p <.001) with a minimum follow-up of 2 years. The VAS score improved from 6.0 ± 1.8 preoperatively to 0.6 ± 1.4 at 2-year follow-up (p <.001), and the hallux valgus angle reduced from 39.7 ± 6.9 to 8.9 ± 9.0 (p <.001). The majority of patients (95.6%) reported the result as excellent or good, and the most common complication was the need to remove the screw (7.2%). Conclusions: The use of MICA without Akin osteotomy fixation resulted in successful correction of hallux valgus with improvements in clinical and radiographic parameters. No evidence has been found to support the hypothesis that there is a correlation between hallux valgus (HV) and intermetatarsal (IM) angles in HV with metatarsus adductus (MA) and that IM angle in HV with MA is lower than that in HV without MA. The present study aimed to analyze the radiographic characteristics of HV with MA compared to matched controls and to clarify the differences between HV with MA and without MA. Preoperative radiographs of 126 female patients (164 feet) who underwent hallux valgus surgery were reviewed. The HV, IM, and MA angles were measured. The MA was defined as MA angle of 20° or greater. Of all the feet, 37 (22.6%) had HV with MA (MA group). Control A (111 feet) having HV without MA was matched by age, gender, and BMI to MA group; Control B (79 feet) having HV without MA was matched by age, gender, BMI, and HV angle to the sub-MA group (31 feet) having HV with MA. The correlation coefficient between the HV and IM angles in the MA group was considered negligible (r = 0.08, p = 0.63), whereas the correlation coefficient in Control A was considered moderate (r = 0.57, p < 0.00001). The correlation coefficient in the MA group was significantly smaller than in Control A (p < 0.01). There was no significant difference in the HV angle between the sub-MA group and Control B (p = 0.23), but the IM angle was significantly smaller than in Control B (p = 0.002). There is no significant correlation between the HV and IM angles in HV with MA, as there is in HV without MA. Background: This case series reported the outcomes of severe hallux valgus treated with first-ray distal metatarsal and proximal phalangeal osteotomies without soft tissue procedure. Methods: The medical records of patients who underwent this surgery from February 2018 to December 2021 were reviewed, including patients with a hallux valgus angle (HVA) ≥40 degrees who were followed up for >2 years. The analyzed data included age, sex, height, weight, and body mass index at the surgery, HVA and intermetatarsal angle (IMA) on the weighted anteroposterior radiograph of the affected foot, the Japanese Society for the Surgery of the Foot score, visual analog scale (VAS) score, and passive plantarflexion and dorsiflexion angles of the first metatarsophalangeal joint 1 month before surgery and at final follow-up. Results: The study group included 35 feet in 29 patients (26 females) with a mean age of 67 ± 10.6 years and mean follow-up of 3.5 ± 0.8 years. Average preoperative and final follow-up measures were HVA, 46.8 to 7.7 degrees; IMA, 18.8 to 9.5 degrees; and VAS score, 61.5 ± 29.6 to 2.7 ± 4.6. Range of motion decreased on average: dorsiflexion, 83.6 ± 14.7 to 71.3 ± 12.0 degrees; and plantarflexion, 63.0 ± 14.7 to 53.0 ± 11.8. All changes were statistically significant (P <.001). Background: Hallux valgus deformity affects more than 35% of people aged ≥65 years. Surgical correction in this population can be more complicated because of poor bone quality, worse deformity, and postoperative recovery challenges. The purpose of this study was to compare the radiographic and clinical outcomes of patients aged ≥65 years who underwent either open Lapidus or minimally invasive chevron Akin osteotomy for bunion correction. Methods: A retrospective review identified 62 patients aged ≥65 years who were treated surgically for hallux valgus with at least 1-year postoperative Patient-Reported Outcomes Measurement Information System (PROMIS) scores (physical function and pain interference). Preoperative and at least 6-month postoperative radiographs were measured for the hallux valgus angle and intermetatarsal angle. PROMIS scores were obtained preoperatively and at 1 and/or 2 years postoperatively. Differences in demographic, clinical, and radiographic outcomes were assessed using the Mann Whitney U test and P values were adjusted for a false discovery rate of 5%. Results: There was no difference between the MIS and open cohorts in pre- or postoperative radiographic measurements or clinical outcomes at any time point. At 1 year postoperatively, both groups had statistically significant improvements in the PROMIS pain interference domain but only the MIS group had a statistically significant improvement in the PROMIS physical function domain. Clinical significance was equivocal. At 2 years postoperatively, there were clinically and statistically significant improvements in the PROMIS pain interference and physical function domains for the open and MIS groups. Conclusion: Patients in both surgical groups had improvement in radiographic measurements and 2-year PROMIS scores, although there was no clinical or statistical difference found between groups. MIS and open surgical techniques appear to be safe and effective in correcting hallux valgus in older patients; however, patients may need to be counseled that maximum improvement after surgery may take more than 1 year. Background: There has been increasing interest in the use of percutaneous or minimally invasive osteotomy techniques for bunionette correction. The aim of this systematic review was to investigate the clinical and radiographic outcomes following percutaneous or minimally invasive surgery for bunionette deformity correction. Methods: A systematic review following PRISMA guidelines was undertaken. All clinical studies published in MEDLINE, Embase, PubMed, and the Cochrane Library Database from inception until December 2023 reporting on the use of percutaneous or minimally invasive osteotomy techniques for bunionette deformity correction were included. The primary outcome was radiographic deformity correction. A meta-analysis of clinical and radiographic outcomes was performed to assess the mean difference following surgery. Risk of bias was assessed using the ROBINS-I tool. Results: A total of 942 potential studies were identified, of which 18 were included encompassing 714 feet in 580 patients. There were no comparative studies identified. The majority of studies (n = 14/18) used an unfixed distal osteotomy technique. All studies showed a statistically significant improvement in clinical outcomes (American Orthopaedic Foot & Ankle Society ankle-hindfoot score and visual analog scale for pain) and radiologic outcomes (fourth-fifth intermetatarsal angle and fifth metatarsophalangeal angle). Complication rates ranged from 0% to 21.4%. The nonunion rate was 0% to 5.6%. Overall risk of bias was low to moderate. The most common complication was development of a hypertrophic callus that tended to resorb over time without needing further surgical intervention. Conclusion: The results of this systematic review must be considered in light of the methodologic limitations of the studies analyzed—including additional procedures performed at the same time as the bunionette correction, lack of comparative studies, and heterogeneity of the case series included. Despite these limitations, our review suggests that percutaneous techniques for bunionette deformity correction are generally clinically safe and associated with improvement in radiographic alignment and patient-reported outcome measures. Introduction: Subtalar Arthroereisis (STA) is a surgical intervention for pediatric flexible flatfoot (PFF), primarily targeting hindfoot alignment by limiting excessive subtalar eversion. However, its effects on forefoot parameters remain underexplored. This study aims to investigate radiological changes following STA in pediatric patients. Materials and Methods: A retrospective analysis was conducted on consecutive patients treated with STA for PFF. First ray-related angles, including the Hallux Valgus Angle (HVA) and the Intermetatarsal Angle (IMA), alongside hindfoot radiological parameters such as the Meary, Calcaneal Pitch, and Costa Bartani angles, were assessed. Subgroup analysis by gender was performed, and correlations between demographic and preoperative radiological parameters were examined. Results: Forty-one patients (81 feet) with an average age of 11.6 years were included, with a mean follow-up duration of 6.4 months. No significant differences were observed in first ray-related angles pre-and postoperatively, with the mean IMA changing from 7.97° to 7.18° and the mean HV angles changing from 9.51° to 8.66°. Noteworthy improvements were seen in flat foot angles, including the Meary, Calcaneal Pitch, and Costa Bartani angles, postoperatively. The age subgroup analysis revealed similar trends in IMA and HVA changes between Group A (who underwent surgery before peak growth) and Group B (who underwent surgery after peak growth). Higher preoperative angles tended to improve, while lower preoperative IMAs and HVAs tended to worsen postoperatively, all remaining within normal ranges. Conclusion: STA showed positive radiological outcomes for PFF treatment, while negligible changes in first ray-related angles were observed. The age subgroup analysis indicated similar trends regardless of operation timing. Higher preoperative angles tended to improve, while lower preoperative angles tended to worsen postoperatively, despite all falling within non-pathological ranges. Background: Central metatarsal pressure is increased in patients with hallux valgus, but the pedographic outcomes after hallux valgus (HV) correction are inconclusive. No known literature has reported the pedographic outcomes after HV correction with Minimally Invasive Chevron and Akin Osteotomy (MICA). Methods: A prospective cohort of 31 feet from 25 patients with moderate-to-severe symptomatic HV but without metatarsalgia underwent MICA and was evaluated using radiographic parameters and pedographic measurements (Footscan®, RSscan International, Olen, Belgium). Data were collected preoperatively and 3 months after surgery. Results: The radiographic parameters of the hallux valgus angle, intermetatarsal angle, distal metatarsal articular angle, first metatarsal head lateral shape, and lateral sesamoid grade significantly improved after MICA. The corrected first metatarsal length was significantly shortened by 2.3 mm, with consistent second metatarsal protrusion distance, lateral Meary's angle, and calcaneal pitch angle. Max force, max pressure, cumulative force, and cumulative pressure on the central metatarsals did not show significant changes between pre- and post-operative measurements, while these parameters significantly decreased in the hallux and first metatarsal area. Background: Physiological changes in the foot may be influenced by external factors such as shoe types or demographic parameters, leading to podiatric conditions in adulthood. The aim of this study was to assess the changes in morphological measurements of the feet of boys and girls during childhood and adolescence. Methods: A total of 1678 Ecuadorian children aged 8 to 17 years participated in the study. The length, width, and height of the foot were analyzed using a 3D scanner to obtain the arch height ratio for the diagnosis of pediatric flat foot. Results: Statistical differences were observed for lengths, widths, and perimeters of the foot in boys aged 5 to 15–16 years and girls aged 5 to 12–13 years. Differences in the height of the navicular bone were found in all age groups, with changes from 2.8 to 4.1% in boys and from 1.3 to 1.5% in girls. The greatest differences between boys and girls of the same age were found at 14 years old onwards. The highest prevalence of flat foot was found in 8-year-old girls (64.9%) and in 12-year-old boys (82.5%). Conclusions: The feet of Ecuadorian children develop progressively during childhood and adolescence. Boys presented with longer and wider feet than girls of the same age. A DMA (dynamic mechanical analysis)-like device based on the principle of classical viscoelasticity testing is invented to investigate the in-situ/in-vivo shear-bearing mechanism of plantar soft tissue. Forty-three volunteers were recruited for the shear-strain test in the longitudinal and transverse directions at five anatomical spots on the plantar surface. Several encouraging observations indicated significant variances among different spots and individuals, implying that the outer forefoot surrounding the second, fifth metatarsal head is a more intensive shear-bearing region on the plantar surface compared to the inner forefoot under the first metatarsal head, and drawing the hypothesis of a significant effect of BMI on the shear-bearing property. The speculations agree with our expectations and other previous research. The feasibility and practical value of this novel approach are substantiated, and these intriguing discoveries provide foundational underpinnings for further in-depth investigations. Purpose: The primary aim is to assess the efficacy of the surgical callus distraction technique of the metatarsus in paediatric patients. Secondary objectives are to assess complications and treatment duration. We have also described the details of our surgical technique. Materials and methods: A case series review of paediatric patients who had metatarsal lengthening at our unit between 2014 and 2022. Patient demographics, duration of time in frame, complications and metatarsal length achieved were recorded. The AOFAS Midfoot and the MOXFQ were taken pre-operatively and at final follow-up. Results: Sixteen metatarsals in 8 patients (14 feet) underwent lengthening between 2014 and 2022 using the MiniRail OrthoFix 100 (Orthofix Medical Inc, Lewisville, TX, USA). The mean age was 13.3 (12–17) years. The average duration between surgery and implant removal was 5.2 months. According to Paley's classification, there was one obstacle encountered in a patient who required a revision of their osteotomy and one problem in another patient who had an infected metatarsophalangeal joint stabilising k-wire treated with oral antibiotics. The Mean AOFAS Midfoot score improved from 53.10 to 86.40 (p < 0.0001) and the Mean MOXFQ improved from 32.5000 to 12.1250 (p < 0.05); these were statistically significant. Conclusion: Gradual metatarsal lengthening using the MiniRail external fixator is a safe and effective method to treat brachymetatarsia in paediatric patients. This preliminary report describes and supports metatarsal lengthening in appropriate patients. BACKGROUND: Hip osteoarthritis is a major civilizational challenge of the ageing population, usually due to the reduced function of gait. In the light of this fact, gait analysis has become an important issue for the doctor, the physiotherapist and the patient, as it serves as a useful tool in diagnosis, recovery, and rehabilitation. Pedobarography is one of the most modern gait analysis methods. OBJECTIVE: The aim of this study is to assess the distribution of pressure forces of the foot on the ground during gait in patients with hip osteoarthritis. METHODS: Dynamic foot tests were performed by means of a two-meter RSscan® International platform, equipped with 16.384 sensors with the scanning frequency level exceeding 500 Hz. Patients were divided into two groups: the research group of N = 60 individuals with hip osteoarthritis, aged 52–84; and the control group of N = 32 individuals without hip osteoarthritis, aged 50–74. Data distribution analysis was performed with the Shapiro-Wilk test, followed by a non-parametric Mann-Whitney U test. RESULTS: The differences between the mean results of maximal peak values of pressure and force in such areas as the big toe, metatarsal I, III, IV, V, lateral and medial heel are significantly lower in the research group than in the control group. Only in the area of metatarsal II, the mean value of pressure is higher in the research group than in the control group. For all metatarsals, the midfoot and medial heel mean results of the contact area are significantly higher in the research group than in the control group. The differences between mean load rate were also tested. The test revealed statistical significance of metatarsal II and III, and the medial heel while metatarsal II displayed a higher mean value in the research group than in the control group. CONCLUSION: The distribution of the pressure forces of the foot can indicate imbalances which cause degenerative changes. Introduction: The anatomy of the forefoot is complex, and the sonographic assessment to image the plantar digital nerves and exclude, diagnose or discriminate between a Morton's neuroma and intermetatarsal bursitis can be challenging. Topic description and discussion: A good appreciation of the sonographic anatomy, technique, normal and abnormal appearances is required to undertake a sonographic assessment of the forefoot and its interspaces, particularly the plantar digital nerves. This is unpacked in this paper with associated pictorial aids. Muscles, tendons, and ligaments of the interspaces and the nearby metatarsophalangeal joints and their associated soft-tissue structures are helpful sonographic landmarks to guide imaging and assessment of the common and proper plantar digital nerves and the intermetatarsal bursa. These need to be appreciated from both dorsal and plantar sonographic approaches, in both short- and long-axis imaging planes. Conclusion: Improved understanding of the anatomy and sonographic appearances of the interspace structures can enhance the sonographic assessment of the forefoot and improve diagnosis of a Morton's neuroma and/or intermetatarsal bursitis when present to guide patient management. Background. The purpose of the study was to describe the most sensitive and specific radiological parameter to diagnose symptomatic adult acquired flatfeet deformity (AAFD) with weight bearing radiographs in Indian population. Methods. A retrospective study was conducted in which 50 consecutive patients with 100 flat feet were included at department of orthopedics from August 2021 to October 2022. Amongst them, 32 were bilateral (64 feet) and 18 were unilateral (18 flat feet and 18 normal feet) flat feet. Results. Out of these 50 subjects, 32 (64%) had bilateral flat feet, 18 (36%) were unilateral (11 left foot and 7 right foot). Out of 50 patients (100 feet), 24 had symptoms on left foot, whereas 26 had symptoms on right foot (total number of symptomatic patients were 50). Amongst our study population (50 subjects), 23 (46%) patients did not have any co-morbidities. 18 (36%) patients had type 2 diabetes, 5 (10%) patients had hypertension, 3 (6%) patients were associated with hypothyroidism and one individual (2%) showed psychiatric disorder. The talar 1st metatarsal angle in lateral view, talar uncoverage angle and the talar 1st metatarsal angle in anteroposterior (AP) view, were found to be statistically significant. Conclusions. Three radiologic parameters-talar 1st metatarsal angle in lateral view and talar uncoverage angle and talar-1st metatarsal angle in anteroposterior view can be the most relevant radiological parameters in the assessment of AAFD in Indian population. We also conclude that the talar uncoverage angle has a higher sensitivity of 82% to predict symptomatic flat foot in adults. [ABSTRACT FROM AUTHOR]

Taxonomic studies have evolved greatly since their early stages and new techniques have been incorporated to improve species descriptions. Those involving the comparison of traits, either quantitatively or qualitatively, can be difficult because the identification of a species must rely on the experience of the observer and errors can occur when cryptic species are involved. Molecular methods have been used to fill these gaps, but morphological methods are still needed to match the recognized molecular species with an adequate taxonomic description. Focusing on the trapdoor spider genus ProshermachaSimon, 1908, we provide a case study using Geometric Morphometrics (GM) techniques to identify morphological divergence between species found in the south-western Australia region. We used GM to identify morphological divergence from museum-preserved specimens by examining shape variation of sexual characters from 39 male specimens from five different localities on a single mountain range. Variation in the shape of both the palpal bulb and tibia provided strong evidence to distinguish two morphotypes, while metatarsus shape showed fewer between-locality differences. Our results illustrate the utility of GM methods, when applied to a few taxonomically-informative structures, as a quantitative species delimitation tool for taxonomic studies. Featured Application: This article aims to analyze and contextualize osteotomies to treat hallux valgus pathology to find gaps in this area where engineering can significantly help. Hallux valgus is one of the most common deformities of the forefoot. When addressing hallux valgus, surgical management plays a central role, with the majority of techniques consisting of cutting (osteotomy) and reshaping the overall anatomy of the metatarsal and phalanx. Understanding and analyzing the results of many osteotomies to treat hallux valgus is essential. This systematic review aims to summarize a structure of the different practices used through osteotomies to correct hallux valgus in the metatarsal, the results obtained, and the methodologies used to provide a follow-up. This systematic search was carried out using three databases: the National Library of Medicine, Science Direct, and Sage Journals. This research mainly focused on analyzing the outcomes and post-operative results of the various osteotomies of the first metatarsal manuscripts that addressed more than just the procedure analysis or its results, which were excluded, as in the case of combined pathologies. Fifteen manuscripts were included for full-text analysis based on comparative/explanatory studies of surgical procedures for hallux valgus metatarsal osteotomies and their clinical outcomes. The clinical results were analyzed in two aspects: the dimensional analysis of the metatarsal before and after the surgical procedure and the technologies used to obtain the follow-up. This review will guide future research into the more comprehensive use of the metatarsal osteotomy process and where engineering can and should intervene and make the procedures more precise and straightforward. Background: Degenerative changes at the sesamoid-metatarsal joints (SMJs) may be a source of pain following hallux valgus surgery. The aims of this study were to describe degenerative changes at the SMJs on weightbearing computed tomography (WBCT) scans and, secondarily, investigate their association with 1-year patient-reported outcome scores following a modified Lapidus procedure for hallux valgus. We hypothesized that reduced joint space in the SMJs would correlate with worse patient-reported outcomes. Methods: Fifty-seven hallux valgus patients who underwent a modified Lapidus procedure had preoperative and minimum 5-month postoperative WBCT scans, and preoperative and at least 1-year postoperative PROMIS physical function (PF), pain interference, and pain intensity scores were included. Degenerative changes at the SMJs were measured using distance mapping between the sesamoids and first metatarsal head on preoperative and postoperative WBCT scans. The minimum and average distances between the first metatarsal head and tibial sesamoid (tibial-SMJ) for each patient preoperatively and postoperatively were measured. Sesamoid station was measured on WBCT scans using a 0 to 3 grading system. Linear regression was used to investigate the correlations between minimum preoperative and postoperative tibial-SMJ distances and 1-year postoperative PROMIS scores. Results: The median minimum and average tibial-SMJ distances increased from 0.82 mm (interquartile range [IQR] 0.40-1.03 mm) and 1.62 mm (IQR 1.37-1.75 mm) preoperative to 1.09 mm (IQR 0.96-1.23 mm) and 1.73 mm (IQR 1.60-1.91 mm) postoperative (P < .001 and P < .001), respectively. In a subset of patients with complete sesamoid reduction, we found an association between preoperative minimum tibial-SMJ distance and 1-year postoperative PROMIS PF scores (coefficient 7.2, P = .02). Conclusion: Following the modified Lapidus procedure, there was a statistically significant increase in the tibial-SMJ distance. Additionally, in patients with reduced sesamoids postoperatively, reduced preoperative tibial-SMJ distance correlated with worse PROMIS PF scores. [ABSTRACT FROM AUTHOR]

Transmetatarsal amputation (TMA) is a common surgical procedure for addressing severe forefoot pathologies, such as peripheral vascular disease and diabetic foot infections. Variability in research methodologies and findings within the existing literature has hindered a comprehensive understanding of healing rates and complications following TMA. This meta‐analysis and systematic review aims to consolidate available evidence, synthesising data from multiple studies to assess healing rates and complications associated with closed TMA procedures. Following Preferred Reporting Items for Systematic Reviews and Meta‐Analysis (PRISMA) guidelines, a systematic search of Medline, Embase, and Cochrane databases was conducted for articles published from January 1st, 1988, to June 1st, 2023. Inclusion criteria comprised studies reporting healing rates in non‐traumatic transmetatarsal amputation patients with more than 10 participants, excluding open TMAs. Two independent reviewers selected relevant studies, with disagreements resolved through discussion. Data extracted from eligible studies included patient demographics, healing rates, complications, and study quality. Among 22 studies encompassing 1569 transmetatarsal amputations, the pooled healing rate was 67.3%. Major amputation rates ranged from 0% to 55.6%, with a random‐effects pooled rate of 23.9%. Revision rates varied from 0% to 36.4%, resulting in a pooled rate of 14.8%. 30‐day mortality ranged from 0% to 9%, with a fixed‐effects pooled rate of 2.6%. Post‐operative infection rates ranged from 3.0% to 30.7%, yielding a random‐effects pooled rate of 16.7%. Dehiscence rates ranged from 1.7% to 60.0%, resulting in a random‐effects pooled rate of 28.8%. Herein, we redescribe Neostenotarsus guianensis (Caporiacco, 1954) nearly seven decades after its original description. In the original description of Neostenotarsus scissistylus Tesmoingt & Schmidt, 2002, we found characters incongruent with N. guianensis, namely, the purported presence of serration on the prolateral keels of the palpal bulb; a narrower apical third of the embolus; the absence of a patch of bristles on the retrolateral face of the palpal tibia and of a baso-retrolateral protuberance on metatarsus I; and a shorter and more apically situated megaspine on the retrolateral branch of the tibial apophyses. The characters from its original description are discussed. Brachymetatarsia is marked by the shortened length of one or more metatarsal bones. Several operative options have been suggested without demonstrating the superiority of one treatment over another. This study aims to assess the main available treatment, bone lengthening achieved, clinical outcomes, and complications pertaining to congenital brachymetatarsia interventions. A literature search of PubMed, Embase, Scopus, and Cochrane databases was performed according to PRISMA guidelines. After the screening process, 13 articles were included in the review. Brachymetatarsia mainly occurs in females at a ratio of 14.8:1, and bilateral involvement in 25.8 % of cases. The fourth metatarsal is most affected, followed by the first metatarsal. The main surgical procedures are one-stage approach which results in limited length gain but fewer complications, and gradual lengthening which achieves greater length but has a higher complication rate, including metatarsophalangeal or interphalangeal subluxation, fractures, and infections. The first metatarsal reported better values concerning lengthening rate and healing index compared to fourth metatarsal, while no differences were observed in distraction rates or clinical outcomes. More complications, such as stiffness, fractures, and cavus foot were associated with first metatarsal lengthening. It is not possible to identify a gold standard technique for addressing this condition. The two predominant surgical techniques bear advantages and drawbacks. A judicious case-by-case assessment is mandatory to determine the optimal surgical approach. Exploring the combination of techniques could be promise in mitigating the limitations associated with the main two methods and achieving better forefoot balance. Level II, systematic review. Objective: We aimed to explore the relationship between bone shape and radiographic severity in individuals with first metatarsophalangeal joint osteoarthritis (first MTP joint OA). Methods: Weightbearing lateral and dorsoplantar radiographs were obtained for the symptomatic foot of 185 participants (105 females, aged 22 to 85 years) with clinically diagnosed first MTP joint OA. Participants were classified into none/mild, moderate, or severe categories using a standardized atlas. An 80‐point model for lateral radiographs and 77‐point model for dorsoplantar radiographs was used to define independent modes of variation using statistical shape modeling software. Odds ratios adjusted for confounders were calculated using ordinal regression to determine the association between radiographic severity and mode scores. Results: After assessment and grading of radiographs, 35 participants (18.9%) were included in the none/mild first MTP joint OA severity category, 69 (37.2%) in the moderate severity category, and 81 (43.7%) in the severe category. For lateral‐view radiographs, 16 modes of variation were included, which collectively represented 83.2% of total shape variance. Of these, four modes were associated with radiographic severity. For dorsoplantar‐view radiographs, 15 modes of variation were included, representing 82.6% of total shape variance. Of these, six modes were associated with radiographic severity. Conclusions: Variations in the shape and alignment of the medial cuneiform, first metatarsal, and proximal and distal phalanx of the hallux are significantly associated with radiographic severity of first MTP joint OA. Minimally invasive foot surgery (MIS) has become a common procedure to treat various pathologies, and accuracy in the angle of metatarsal osteotomies is crucial to ensure optimal results. This randomized controlled trial with 37 patients investigates whether the implementation of a digital inclinometer can improve the accuracy of osteotomies compared to traditional freehand techniques. Patients were randomly allocated to group A (n = 15) receiving inclinometer-assisted surgery or group B (n = 22) receiving conventional surgery. Osteotomies were performed and outcomes were evaluated using an inclinometer. The inclinometer group showed a significant decrease in plantar pressure from 684.1 g/cm2 pretreatment to 449.5 g/cm2 post-treatment (p < 0.001, Cohen's d = 5.477). The control group decreased from 584.5 g/cm2 to 521.5 g/cm2 (p = 0.001, Cohen's d = 0.801). The effect size between groups was large (Cohen's d = −2.572, p < 0.001). The findings indicate a significant improvement in accuracy and reduction in outliers when using an inclinometer, suggesting that this technology has the potential to improve surgical practice and patient outcomes in minimally invasive metatarsal osteotomies. This study, based on the discovery and examination of male specimens of Liomma laeve Roewer, 1959, proposes the reclassification of the monotypic genus Liomma Roewer, 1959. Currently placed in Grassatores incertae sedis, we hereby transfer it to the Nomoclastidae family. In this work, we present a reevaluation of the genus and a comprehensive redescription of its type species. Additionally, we introduce a novel anatomical feature, seemingly exclusive to Nomoclastidae, which we term 'metatarsal carinate setae (MtCS)'. MtCS comprises specialised striate setae organised in a comb-like structure located on the prolateral distal part of metatarsus I in males. The presence of MtCS has been observed not only in Liomma laeve but also in several as-yet-undescribed species within the Nomoclastidae family. Pressure measurement technology is being increasingly used by physical therapists, podiatric physicians, and surgeons in both clinical practice and research. Plantar pressure was recently proposed as a strong predictor of adverse outcomes. Few reviews have been conducted to identify a standard for the plantar pressure in healthy participants, older people, and children. The aims of this study were to explore currently available normative values of static-dynamic plantar pressure and its cutoff values. The SCOPUS, Rehabilitation, MEDLINE, EBSCO, PEDro, and PubMed electronic databases were systematically searched from January 2000 to December 2022. All articles were analyzed using a standardized template to extract various data, such as the first author's details, publication year, country of origin, participant age and gender, sample size, population characteristics, instrument used for measurement, plantar pressure distribution (PPD) type, normative data, and cutoff values. In literature review, the researchers selected nine studies, and in all studies, they selected different areas of foot for normative values. Areas selected for PPD were forefoot, hallux, big toe, second toe, third through fifth toes, toes 2-5, metatarsal 1-5, lateral forefoot, middle forefoot, medial forefoot, midfoot, lateral midfoot, medial forefoot, lateral area of the heel, medial area of the heel, heel, right foot total, left foot total, and right + left total. Age, gender, and body mass index (BMI) affect the results of the plantar pressure. It is necessary to take age, gender, and BMI into consideration when conducting stratified physical evaluations for the evaluation of older people individuals' screening and outcome. Aim: Accessory and sesamoid bones are hidden anatomical structures that fulfil a wide variety of functions in the human body. Therefore, studying the nature, formation and dimensions of these structures is of great importance for the medical literature. In our study, we aimed to measure the presence and area (size) of these ossicles. Material and Method: Our study was performed retrospectively on the images of individuals who applied to Ordu University and whose radiological images were obtained. The presence of accessory and sesamoid ossicles (present/absent) and their sizes (measured using the PACS system) were evaluated on radiographs and Computed Tomography (CT) images. The results of the ossicles were divided into gender, age, and bilateral groups. Statistical analyzes were performed with the SPSS program. Results: The most common and largest accessory ossicles in the foot were os peroneum (18.2%), os naviculare accessoria (17.4%), os trigonum (12.1%) and os intermetatarsarum (7.8%). Sesamoid ossicles at the level of the first metatarsophalangeal (mtp) joint of the foot and the first metacarpopgalangeal (mcp) joint of the hand were found to be 100% common. The second most common sesamoid bone in the hand and foot was at the fifth mtp (22.22%) and mcp (68.53%) joints. In the hand, the most common accessory ossicles were os triangulare (6.08%), os radiale externum (2.60%) and os centrale (1.73%). Accessory ossicles (fabella: 19%, cyamella: 17.5%) were found in 25.5% of the knee. Conclusion: Accessory and sesamoid ossicles are most commonly found in the hand, foot, and knee. The presence and size of these ossicles can be both beneficial and harmful. Sesamoids provide mechanical benefit, whereas accessory ossicles can be mistaken for fractures and may lead to unnecessary medical conditions. Background: Recognizing preoperative first-ray hypermobility is important to planning hallux valgus (HV) surgery. A recent study showed the minimally invasive chevron Akin (MICA) osteotomy increased varus displacement of the proximal fragment of the first metatarsal osteotomy. The present study aims to evaluate the ability of the radiographic firstray squeeze test to predict the varus displacement of the proximal fragment of the first metatarsal osteotomy when performing the MICA procedure. Methods: A prospective case series of patients with moderate to severe HV who underwent MICA was performed. The HV deformity correction was analyzed by comparing the preoperative and 12-week postoperative hallux valgus angle (HVA) and the intermetatarsal angle between the first and second rays (1-2 IMA). The ability of the radiographic firstray squeeze test to predict the varus displacement of the first metatarsal was done by comparing the preoperative 1-2 IMA measured in the AP radiographic first-ray squeeze test (IMA-ST) with the intermetatarsal angle between the second metatarsal and the axis of the first metatarsal osteotomy proximal fragment (IAPF) taken 12 weeks postoperatively. Results: Between July 2022 and May 2023, a total of 39 feet in 28 patients underwent MICA. The mean IMA improved from 13.8 (SD = 2.2) to 3.8 degrees (SD = 1.5) (P < .001), and the mean HVA improved from 27.8 (SD = 6.1) to 4.9 degrees (SD = 2.5) (P < .001). A linear regression analysis revealed that IMA-ST is highly associated with the 12 week assessed IAPF (P < .001). Conclusion: The preoperative radiographic first-ray squeeze test appears to predict with high fidelity the varus displacement of the proximal fragment of the first metatarsal that can occur after the MICA procedure. Limb salvage is a difficult path for patients to travel as there is no guarantee of the outcome, often the major factor is perfusion. For patients who underwent transmetatarsal amputation (TMA), success rate is crucial as the next option is most likely a major amputation. We performed a 10 years (2010–2020) retrospective review of patients that underwent a TMA and had an angiogram or computed tomography angiography (CTA) perioperatively at the Dallas VA Medical Center. Failure after TMA was defined as a patient requiring a proximal amputation within 1 year. There were 125 TMAs performed between 2010 and 2020 at the institution. Forty‐four (35.2%) patients had an angiogram/CTA peri‐operative and met the inclusion criteria. Seventeen subjects (38.6%) had a higher level of amputation. Of the 17 failures, 2 (11.8%) patients had no patent vessel runoff to the foot, 9 (52.9%) had one vessel, 4 (23.5%) had two vessels, and 2 (11.8%) had three vessels runoff. One vessel runoff to the foot yielded a high rate of poor outcomes (56.3%) defined as a higher level of amputation. OBJECTIVE: To evaluate skin perfusion in cats receiving dexmedetomidine compared to a placebo. ANIMALS: 9 healthy adult research cats. METHODS: A randomized, blinded, placebo-controlled study design was used. Two sites, the dorsal metatarsus (site: limb) and lateral flank (site: flank), were evaluated with laser speckle contrast imaging (LSCI) at baseline and following administration of dexmedetomidine (1, 3, or 5 mcg/kg, IV) or a placebo (0.9% saline, IV). Mean speckle contrast (MSC), a surrogate for perfusion, was obtained from LSCI and compared between treatments. Heart rate, sedation score, and body temperature were recorded. Skin perfusion to the flank and limb, reported as MSC, was assessed via LSCI at baseline and at 5, 10, and 15 minutes posttreatment. RESULTS: There was a significant decrease in heart rate (P < .001) in cats receiving 1, 3, and 5 mcg/kg dexmedetomidine compared to placebo. There was a significant increase in median sedation score at all time points postsedation compared to baseline (P < .018). Changes in MSC for the metatarsus were not significantly different between treatments at any time point (P = .12). For the flank, MSC was significantly higher for cats treated with dexmedetomidine compared to baseline (P = .01). Skin perfusion to the flank decreased as early as 5 minutes posttreatment with dexmedetomidine and persisted for at least 15 minutes, regardless of dexmedetomidine dose. CLINICAL RELEVANCE: Dexmedetomidine decreased skin perfusion in cats, even at low doses. Veterinarians may elect for an alternative sedative medication when decreased skin perfusion is a concern. The open-holed trapdoor spiders of the Teyl damsonoides-group (formerly included in the now synonymized genus MerrediniaMain, 1983) are revised, and 20 new species are described from the biodiversity hotspot of south-western Western Australia. The only previously described species, T. damsonoides (Main, 1983), is further re-described and re-illustrated, and molecular data from seven genes for a subset of nine species are analyzed using Bayesian methods. The damsonoides-group of Teyl are unusual for their stunning bi-colored appearance and the bowed morphology of the male metatarsus I, and we here document the known diversity and biology of these little-known spiders. Introduction: Overcorrection is a possible complication of clubfoot treatment, whose prevalence varies from 5 to 67%. Overcorrected clubfoot usually presented as a complex flatfoot with different degrees of hindfoot valgus, flat top talus, dorsal bunion, and dorsal navicular subluxation. The management of clubfoot overcorrection is challenging, and both conservative and surgical treatments are available. This study aims to present our experience in the surgical management of overcorrected clubfoot and to provide an overview of actual treatment options for each specific sub-deformity. Materials and methods: A retrospective cohort study of patients surgically treated for an overcorrected clubfoot from 2000 to 2015 at our Institution was conducted. Surgical procedures were tailored to the type and symptomatology of the deformity. A medializing calcaneal osteotomy or subtalar arthrodesis was performed for hindfoot valgus. Subtalar and/or midtarsal arthrodesis were considered in cases of dorsal navicular subluxation. The first metatarsus elevatus was addressed through a proximal plantarflexing osteotomy, sometimes associated with a tibialis anterior tendon transfer. Clinical scores and radiographic parameters were obtained pre-operatively and at the last follow-up. Results: Fifteen consecutive patients were enrolled. The series included 4 females and 11 males, with a mean age at surgery of 33,1 (18–56) years, and a mean follow-up of 4,46 (2–10) years. Seven medializing calcaneal osteotomies, 5 subtalar arthrodesis, 11 first metatarsal plantarflexing osteotomies, and 7 anterior tibialis tendon transfers were performed. A statistically significant improvement in both clinical and radiographic scores was observed. Conclusions: Management of overcorrected clubfoot involves many surgical techniques because of the high interpersonal variability of the deformities. The surgical approach showed positive results, as long as the indication is based on clinical symptoms and functional impairment rather than morphological alterations and radiographic findings. Context: Amino acid supplementation and immunocastration are two husbandry practices with an increasing interest in the worldwide growing industry of deer farming. Amino acids (AAs) optimise nutrition and feed costs and improve the quality of products such as venison, velvet or antlers. Immunocastration (IC) reduces agonistic behaviours, which produce carcass damage and reduced growth. Thus, both treatments have positive effects on body growth, but may interfere with hormonal regulation, subsequently affecting antler growth. Aims: This study aimed to evaluate the combined use of both practices and their impact on body and antler growth. Methods: Forty-four yearling male fallow deer, approximate 10 months old, were subjected to the following four combinations based on both treatments: control–control, AA–control, IC–control, and AA–IC. Immunocastration treatment started in March 2020 and was repeated after 50 and 130 days. All groups received 250 g per animal and day of grains, and those under AA supplementation received ruminally protected lysine 6.3 g and ruminally protected methionine 2.1 g per animal and day. Biometric data, antlers and metatarsus were collected at slaughter in November. Antlers and metatarsus were analysed through computer-tomography scanning and mineral spectrometry. Key results: Supplementation of AAs scarcely affected antler growth, although an indirect effect through improved body condition cannot be rejected. Immunocastration showed significant but not dramatic effects: IC animals had lighter antlers, with a lower amount of cortical bone and less mineralisation (density and calcium and phosphorus concentrations) in the base of the antler than did control animals. In contrast, the effects were scarce at the top of the antler and the metatarsus. Conclusions: The results suggest a reduction but not total suppression of testosterone, with almost negligible effects on body growth and internal bones, thus not raising animal welfare issues. Implications: Both techniques can be used simultaneously in deer farming, especially in farms with venison-production goals. Future research should focus on optimising the vaccination schedule for the main farmed deer species to ensure that the welfare benefits are well balanced with productivity. Objective: To design appropriate orthosis for hallux valgus, a difficult foot condition that affects a quarter of the body's bones, we need to clarify the numerical biomechanical features, which have not been established in previous biomechanical studies. Therefore, we constructed a finite element model of the bunion foot to investigate the orthopaedic force compensation mechanism. Methods: A patient with moderate hallux valgus was recruited. CT imaging data in DICOM format were extracted for three‐dimensional foot model reconstruction. In conjunction with the need for rapid design of bunion orthosis, a metatarsal force application sizing method based on an orthogonal test design was investigated. The orthogonal test design was used to obtain the hallux valgus angle (HVA) and the inter metatarsal angle (IMA) data for different force combinations. Based on the extreme difference analysis and analysis of variance of the test results, the influence of different force combinations on the bunion angle was quickly determined. Results: The results showed that the stress concentration occurred mainly in the first metatarsal bone. The distribution trend was in the medial and lateral middle of the bone and gradually decreased to the dorsal base of the bone body. The greatest stress occurs in the cartilage between the phalanges and metatarsals. In 25 groups of simulation experiments, HVA was reduced from 27.7° to 13°, and IMA was reduced from 12.5° to 7.3°. Conclusion: Applying detailed orthopaedic force collocation to the first metatarsal column can effectively restore the mechanics and kinematics of hallux valgus, and provide a reference for the treatment of bunion valgus and the design of orthopaedic devices. Introduction: Overcorrection is a possible complication of clubfoot treatment, whose prevalence varies from 5 to 67%. Overcorrected clubfoot usually presented as a complex flatfoot with different degrees of hindfoot valgus, flat top talus, dorsal bunion, and dorsal navicular subluxation. The management of clubfoot overcorrection is challenging, and both conservative and surgical treatments are available. This study aims to present our experience in the surgical management of overcorrected clubfoot and to provide an overview of actual treatment options for each specific sub-deformity. Purpose: Metatarsal fractures often occur around the diaphyseal nutrient foramina (NF) which vary in topography depending on population affinity. Topographical and morphometrical knowledge of the NF is crucial in understanding fracture development and fracture site healing patterns. The current study aimed to describe the topography and the morphometry of the metatarsal diaphyseal NF in South African Africans (SAA), South Africans of European descent (SAED) and South Africans of Mixed Ancestry (SAMA). Methods: The study examined 4284 dry cadaveric metatarsals from both sexes and sides of these populations for NF topography and morphometry, including the presence, number, location, position, size and direction of the NF on the metatarsal bones. Results: The NF was present in 99.4% of the metatarsals. Most (84.5%) metatarsals examined had a single NF. Most (97.4%) NF were located in the middle third of the metatarsal bones. The median foramina index (FI) of the second metatarsal exhibited population affinity and significant differences were found both on the left second metatarsal (P = 0.043), and the right second metatarsal (P = 0.046). The position of NF was predominantly lateral on the first (92.4%), second (64.9%) and third (59.1%) metatarsals, whilst the position was predominantly medial on the fifth (65.1%) metatarsals. The NF positions on the fourth metatarsals showed the greatest population variability. The first metatarsals had primarily dominant-sized and distally directed NF whilst the second through fifth had primarily secondary-sized and proximally directed NF. Conclusion: The topographical anatomy of the metatarsal diaphyseal NF appears similar across the South African populations. Background: Lisfranc injuries, if not accurately diagnosed, can result in chronic pain and instability. Previous studies have examined ultrasonographs, radiographs, magnetic resonance imaging (MRI), and conventional computed tomography (CT) scan to differentiate Lisfranc instability, but they focused on a healthy/injured scale without differentiating subtle injury. Weightbearing CT (WBCT) has emerged as a diagnostic tool for detecting subtle Lisfranc injuries. This systematic review aimed to compare WBCT with conventional CT in diagnosing Lisfranc injury, and the ability to differentiate injuries of varying severities. Methods: The review encompassed PubMed, CINAHL, MEDLINE, SPORTDiscus, and Web of Science databases from inception until July 5, 2023. Inclusion criteria involved studies on CT and/or WBCT for Lisfranc injuries and nonoperative studies. Exclusion criteria composed case reports, commentaries, postoperative imaging studies, pediatric patients, studies with nonobjective radiographic measurements, studies exclusively focused on injury classification, and studies with fewer than 5 patients because of poor statistical power. Data extraction focused on radiographic measurements of the Lisfranc complex, categorized into conventional CT, partial WBCT, and total WBCT. Results: Out of the initially retrieved 489 articles, 9 met the inclusion criteria. Several studies consistently demonstrate that WBCT provides a higher level of accuracy in measuring the Lisfranc area, offering enhanced sensitivity to detect subtle alterations in joint structure. Moreover, WBCT exhibits superior sensitivity in distinguishing between healthy Lisfranc joints and those with injuries, particularly when identifying dorsal ligament damage. This imaging modality allows for the detection of significant variations in critical measurements like first-second metatarsal (M1-M2) distance, first cuneiform (C1)-M2 distance, and joint volumes, enabling a more comprehensive assessment of Lisfranc joint health especially with subtle instability. Conclusion: This review evaluates the extant literature on WBCT's utility in diagnosing Lisfranc injuries and compares its effectiveness to CT in distinguishing between injuries of varying severity. Context: The lower quarter Y-Balance Test (YBT-LQ) is associated with injury risk; however, ankle range of motion impacts YBT-LQ. Arch height and foot sensation impact static balance, but these characteristics have not yet been evaluated relative to YBT-LQ. Objective: Determine if arch height index (AHI), forefoot sensation (SEN), and ankle dorsiflexion predict YBT-LQ composite score (CS). Design: Descriptive cohort. Setting: Athletic training laboratory. Participants: Twenty general population (14 females and 6 males; mean [SD]: age 35 [18] y, weight 70.02 [16.76] kg, height 1.68 [0.12] m) participated in this study. Interventions: AHI measurement system assessed arch height in 10% (AHI10) and 90% (AHI90) weight-bearing. Two-point discrim-a-gon discs assessed sensation (SEN) at the plantar great toe, third and fifth metatarsal heads. Biplane goniometer and weight-bearing lunge tests were used to measure static and weight-bearing dorsiflexion, respectively. The YBT-LQ assessed dynamic single-leg balance. Results: For right-limb dynamic single-leg balance, AHI90 and SEN were included in the final sequential prediction equation; however, neither model significantly (P = .052 and .074) predicted variance in YBT-LQ CS. For left-limb dynamic single-leg balance, both SEN and weight-bearing lunge test were included in the final sequential prediction equation. The regression model (SEN and weight-bearing lunge test) significantly (P = .047) predicted 22% of the variance in YBT-LQ CS. Conclusions: This study demonstrates that foot characteristics may play a role in YBT-LQ CS. The authors did not assess limb dominance in this study; therefore, the authors are unable to determine which limb would be the stance versus kicking limb. Hallux valgus is one of the most common deformities of the forefoot. When addressing hallux valgus, surgical management plays a central role, with the majority of techniques consisting of cutting (osteotomy) and reshaping the overall anatomy of the metatarsal and phalanx. Understanding and analyzing the results of many osteotomies to treat hallux valgus is essential. This systematic review aims to summarize a structure of the different practices used through osteotomies to correct hallux valgus in the metatarsal, the results obtained, and the methodologies used to provide a follow-up. This systematic search was carried out using three databases: the National Library of Medicine, Science Direct, and Sage Journals. This research mainly focused on analyzing the outcomes and post-operative results of the various osteotomies of the first metatarsal manuscripts that addressed more than just the procedure analysis or its results, which were excluded, as in the case of combined pathologies. Fifteen manuscripts were included for full-text analysis based on comparative/explanatory studies of surgical procedures for hallux valgus metatarsal osteotomies and their clinical outcomes. The clinical results were analyzed in two aspects: the dimensional analysis of the metatarsal before and after the surgical procedure and the technologies used to obtain the follow-up. This review will guide future research into the more comprehensive use of the metatarsal osteotomy process and where engineering can and should intervene and make the procedures more precise and straightforward.

Footwear plays an important role in worker safety.Work boots with safety toes are often utilized at mine sites to protect workers from hazards. Increasingly, mining operations require metatarsal guards in addition to safety toe protection in boots.While these guards provide additional protection, the impact of metatarsal guards on gait are unknown. This study aimed to measure the effects of 4 safety work boots, steel toe, and steel toe with metatarsal protection in wader- and hiker-style boots, on level and inclined walking gait characteristics, during ascent and descent. A total of 10 participants completed this study. A motion capture system measured kinematics that allowed for the calculation of key gait parameters. Results indicated that gait parameters changed due to incline, similar to previous literature.Wader-style work boots reduced ankle range of motion when ascending an incline. Hip, knee, and ankle ranges of motion were also reduced during descent for this style of boot. Wader-style boots with metatarsal guards led to the smallest ankle range of motion when descending an inclined walkway. From these results, it is likely that boot style affects gait parameters and may impact a miner's risk for slips, trips, or falls. [ABSTRACT FROM AUTHOR]
