2. Assessing the clinical advantage of opioid-reduced anesthesia in thoracoscopic sympathectomy: a prospective randomized controlled trial

Abstract

Abstract Background Opioid-reduced multimodal analgesia has been used clinically for many years to decrease the perioperative complications associated with opioid drugs. We aimed to assess the clinical effects of opioid-reduced anesthesia during thoracoscopic sympathectomy. Methods Surgical patients (n = 151) with palmar hyperhidrosis were randomly divided into control (Group C, 73 patients) and test (Group T, 78 patients) groups. All patients were administered general anesthesia using a laryngeal mask. In Group C, patients received propofol, fentanyl, and cisatracurium for anesthesia induction, and maintenance was achieved with propofol and remifentanil, along with mechanical ventilation during the operation. In Group T, anesthesia was induced with propofol, dezocine, and dexmedetomidine (DEX) and maintained with propofol, DEX, and an intercostal nerve block, along with spontaneous breathing throughout the operation. Perioperative complications related to opioid use include hypotension, bradycardia, hypertension, tachycardia, hypoxemia, nausea, vomiting, urine retention, itching, and dizziness were observed. To assess the impact of these complications, we recorded and compared vital signs, blood gas indices, visual analogue scale (VAS) scores, adverse events, and patient satisfaction between the two groups. Results Perioperative complications related to opioid use were similar between groups. There were no significant differences in the type of perioperative sedation, analgesia index, respiratory and circulatory indicators, blood gas analysis, postoperative VAS scores, adverse reactions, propofol dosage, postoperative recovery time, and patient satisfaction. Conclusions In minimally invasive surgeries such as thoracoscopic sympathectomy, opioid-reduced anesthesia was found to be safe and effective; however, this method did not demonstrate clinical advantages. Abstract

Probabilistic programming languages (PPLs) are receiving widespread attention for performing Bayesian inference in complex generative models. However, applications to science remain limited because of the impracticability of rewriting complex scientific simulators in a PPL, the computational cost of inference, and the lack of scalable implementations. To address these, we present a novel PPL framework that couples directly to existing scientific simulators through a cross-platform probabilistic execution protocol and provides Markov chain Monte Carlo (MCMC) and deep-learning-based inference compilation (IC) engines for tractable inference. To guide IC inference, we perform distributed training of a dynamic 3DCNN--LSTM architecture with a PyTorch-MPI-based framework on 1,024 32-core CPU nodes of the Cori supercomputer with a global minibatch size of 128k: achieving a performance of 450 Tflop/s through enhancements to PyTorch. We demonstrate a Large Hadron Collider (LHC) use-case with the C++ Sherpa simulator and achieve the largest-scale posterior inference in a Turing-complete PPL., Comment: 14 pages, 8 figures

9. Mothers’ perceptions of children’s emotional behaviour problems and parenting sense of competence: The mediating role of maternal meta-emotion philosophy.

Abstract

We investigated the relationship between mothers’ perceptions of their children’s emotional behaviour problems and their parenting sense of competence, as well as the mediating effect of maternal meta-emotion philosophy in this connection. Participants were 349 mothers of 4- to 17-year-old children (female children = 40.7%, mean age = 8.24 years, SD = 2.95 years). The mothers completed the Strengths and Difficulties Questionnaire, the Parental Meta-Emotion Philosophy Scale, and the Parenting Sense of Competence Scale. Structural equation modelling and simple mediation analyses results showed that mothers’ perceptions of their children’s problem behaviours were linked to higher emotion dimissing and lower parenting sense of competence. Specifically, emotion dimissing mediated the relationship between children’s problem behaviours and parenting sense of competence by the mothers. Furthermore, mothers’ perceptions of their children’s prosocial behaviours were associated with both higher emotion coaching and parenting sense of competence. Emotion coaching mediated the relationship between children’s prosocial behaviours and parenting sense of competence for higher sense of competence by the mothers. Our findings suggest that mothers’ perceptions of children’s emotional behaviour problems may explain their parenting sense of competence through their parenting philosophy. By prioritising their maternal meta-emotion philosophy when dealing with their children’s emotional behaviour problems, mothers can enhance their parenting sense of competence. Abstract

Urban greenbelts are considered a key instrument for shaping sustainable urban growth and protecting the environment in a large number of cities in the world. In most cities, there is a widely shared belief that urban greenbelts are beneficial to the natural environment. By contrast, there is little understanding of the underlying economic impacts of greenbelts and other green space configurations in fast growing cities. The unprecedented rate and scale of urbanisation in the emerging economies has brought the role of greenbelts into an even sharper focus. In cities within these fast growing economies, the urban population is expected to double in the coming decades, which means that greenbelts are under great pressures to adapt to the large forthcoming growth. Few existing urban models are capable of addressing the dynamic nature of the urban transformations and predicting the impacts of urban greenbelts in the developing world. This prompts us to develop a new modelling method that is capable of assessing the impacts of different configurations, scales and locations of green spaces. We then use it to examine alternative futures to the greenbelt through a case study of Greater Beijing. The method we developed is a new variant in the land use-transport interaction (LUTI) model family. This model is capable of addressing the non-equilibrium nature of urban land use and transport development and the equilibrium nature of the day-to-day adaptations made by businesses and citizens. This LUTI model aims to answer the following questions: what are the short-term and long-term economic impacts of a greenbelt on a fast growing city? Which alternative green space configuration performs better in terms of economic well-being and travel costs? Where and how much should the greenbelt land be progressively reshaped or released as the city grows? The new LUTI model is calibrated and validated using data collected for 1990, 2000 and 2010 for Greater Beijing, The model is first tested retrospectively through revisiting the past greenbelt policies in Beijing from 1990 to 2010. Then the impacts of different future green space configurations from 2010 to 2030 are predicted and assessed through quantifying economic costs/benefits and travel costs for socio-economic groups. The model results suggest that under rapid transformative urban change, the configuration, scale and location of a greenbelt have a significant impact on a city’s economic efficiency. Such impacts will transcend the greenbelt boundary, and even the boundary of Beijing Municipality, onto the entire city region. A narrow greenbelt launched in the early age of urban expansion could lead to spatial mismatch of residents and jobs. A wide and strictly controlled ring-shaped greenbelt is not the highest performing intervention either, in terms of economic well-being. The green-wedges configuration is a remedial policy that balances the economic efficiency and environmental benefits. Intensive development around metro/rail stations in the designated greenbelt could reduce spatial costs and promote sustainable travel modes. This implies that a careful siting of new development within existing designations of the greenbelt can be beneficial in terms of economic well-being and sustainable transport.

Abstract

Morphine is a potent opioid which is commonly used for relieving cancer pain with strong analgesic properties. Epithelioid hemangioendothelioma (EHE), a rare vascular tumor that can cause severe chronic pain, is usually diagnosed in a surgical biopsy. We describe ultrahigh doses of morphine used in a case of a 17-year-old male who suffered from severe chest pain with systemic multiple EHE. The total dosage of morphine was as high as 92,530 mg given within 164 days. However, we observed no indications that the patient was addicted.

33. Experimental and numerical study on cavitation under elastohydrodynamic lubrication point contact.

Abstract

As a common phenomenon in elastohydrodynamic lubrication, cavitation has an effect on the completeness of the oil film in the contact area. Many studies have therefore been conducted on cavitation. Experimental researches on cavitation usually rely on optical interference observation, which offers a limited resolution and observation range. In this paper, an infrared thermal camera is used to observe the cavity bubbles on a ball-on-disc setup under sliding/rolling conditions. The results show that the cavity length increases with an increases of the entrainment speed and the viscosity of the lubricants. These observations are explained by a numerical model based on Elrod's algorithm. Effects of entrainment speed and lubricant viscosity on the breakup of cavitation bubbles and the cavitation states are investigated. Both the simulation and experimental results show that a negative pressure area is present behind the Hertzian contact area. The ambient pressure plays a role in maintaining cavitation state 1. The cavitation pressure is close to the vacuum pressure when the entrainment speed is low and to the ambient pressure instead when the entrainment speed is high. [ABSTRACT FROM AUTHOR]

41. Does built form matter in a moving urban fringe? Evidence from a Land Use-Transport Interaction (LUTI) model

Abstract

Beijing has undergone rapid urbanisation from 1950-2010. The main built-up area expanded concentrically from 100 to 1210 km2. Its urban fringe became popular for new developments, some placed haphazardly. The government concentrated efforts in 'stopping concentric fringe expansion'. E.g. on neighbourhood and district levels, the government intended to densify roads, increase neighbourhood density and improve public transport. However, the effects remain unclear. The LUTI model is widely used to assess the effects of planning policies at entire city scale, and generate the whole metropolitan area's spatial pattern. However, it is insensitive to micro-level (neighbourhood or district) built form change. E.g. there are two policy scenarios with the same floorspace supply quantity, one case is that all houses concentrate around road nodes, the other that houses distribute sparsely. Given equal floorspace provisions, regardless of built form, the LUTI model will compute the same spatial patterns. However, in reality, different built forms will lead to different policy performances. Most LUTI models, to the best of our knowledge, are incapable of measuring this difference. Therefore, this paper augments existing LUTI models with a LUTI framework compliant built form component, allowing investigations into the impact of built form on whole city function. In our newly assembled model, we quantify the effects of built form change on route selection change. Then, the micro-level travel behaviour change is passed to the macro-level MEPLAN transport model (Echenique, 2004, 2011). Finally, we input the MEPLAN results, including generalised travel cost, distance and time, into a LUTI framework (Jin, Echenique, & Hargreaves, 2013) to simulate population distribution, changes on price level and spatial costs. We then apply this model to Beijing's case in order to: validate the micro built form component in the general LUTI framework; predict the impacts of alternative built forms in reshaping the city's fringe; gain insights into how built form at the urban fringe shapes the bigger scale spatial structure. We first calibrate a 130-zone recursive spatial equilibrium model using observed data for 1990-2010. Alternative built forms at the urban fringe for 2000 and 2010 that share the same floorspace provision are tested. These scenarios include densification around subway stations and decreasing road density in built-up area. The model results will show each area's spatial cost from its built form. The analyses suggest that under rapid transformative urban change, the built form has an indirect but significant impact on the economic performance of the entire city, through travel behaviour change. In densification scenarios, access trip length decreases, as does the spatial cost and price level. In road density restriction scenarios, spatial costs increase in the entire city. These results allow understanding of the impact to whole city economic performance from built form at the neighbourhood scale. They also provide insights into designing better built forms, as such design on neighbourhood and district scale are more feasible in practical implementation - not only for cities in emerging economies but also fast-growing cities in the developed economies that are revisiting the design.
