Introduction: Spirometry devices, which are components of many anaesthesia machines, are commonly used to assess lung mechanics during anaesthesia. Spirometry calibration usually adheres to manufacturer recommendations without established guidelines. Although more accurate and less variable than inbuilt spirometry in certain General Electric anaesthesia ventilators, near-patient spirometry lacks adequate evaluation. Methods: We assessed near-patient spirometers' performance using Pedi-lite and D-lite flow sensors. Certified 1 L calibration syringes were used on 67 monitors located in 14 veterinary hospitals. Three consecutive inspired and expired volume values displayed by the monitors for each volume of the calibration syringe were recorded. Volumes studied were 50, 100, 150, 250, 300 mL for Pedi-lite and 150, 300, 450, 500, 750 mL for D-lite. Measured and targeted volumes were averaged, agreement error calculated. Accuracy was assessed plotting agreement errors against calibration volumes. A linear mixed-effects model was used to obtain linear regression between the error and the calibration volume. Mean, differential and proportional bias, limits of agreement, claimed accuracy and 10% clinical tolerance were calculated and displayed. Differences among monitors were evaluated using the Friedman rank sum test, differences between inspired and expired volumes using the Wilcoxon signed-rank. Results: Inter-monitor variability for inspired and expired volume readings using both sensors was high; intra-monitor variability was low. The error magnitude was independent of volumes evaluated. Using Pedi-lite, only a minority of measurements met manufacturer's specification or a 10% clinical tolerance; both inspired and expired volumes were significantly underestimated. Using D-lite, superior performance was demonstrated for volumes between 300 and 750 mL (mean biases close to zero and the majority of measurements meeting manufacturer's specifications and clinical tolerance). The difference between measured inspired and expired volumes with both sensors was significant. Discussion: These results support caution when interpreting clinical measurements of lung volumes and mechanics in anaesthetised patients when using these sensors. Introduction: In clinical practice, evaluating dynamic compliance of the respiratory system (Cdyn) could provide valuable insights into respiratory mechanics. Reference values of Cdyn based on body weight have been reported, but various factors may affect them and the evidence is scanty. This study aimed to establish a reference interval for Cdyn and identify associated variables. Methods: Data were collected from 515 client-owned dogs requiring anesthesia, excluding those with lower airway disease. The dogs were anesthetized, the tracheas intubated, and lungs ventilated at clinicians' discretion across 11 centers in six countries, with no restrictions on anesthesia protocols or ventilation settings, except avoiding inspiratory pauses. Three Cdyn measurements from three consecutive breaths per dog were recorded using a standardized form, which also documented factors affecting Cdyn identified through literature and an online survey. Various spirometry technologies were used. The substantial variance in Cdyn measurements led to a comprehensive analysis using a multiple linear regression model. Multicollinearity (variables highly correlated with each other) was addressed by investigating, transforming, or excluding factors. Initial simple linear regression assessed each variable's individual effect on Cdyn, followed by a multiple linear regression model constructed via stepwise forward selection and backward elimination. Results: The best-fitting model identified a linear relationship between Cdyn and body mass when the following conditions were met: high BCS (Body Condition Score), orotracheal tubes <7% smaller than predicted, the use of a D-lite flow sensor, and the absence of a high FIO2 (>80%) exposure for more than 10 minutes before Cdyn measurement. In cases where these conditions were not met, additional factors needed to be incorporated into the model. Low (1/9, 2/9, 3/9) and medium (4/9, 5/9) BCS, an orotracheal tube of the predicted size or larger and longer inspiratory times were associated with increased Cdyn. The use of alternative spirometry sensors, including Ped-lite, or prolonged exposure to high FIO2 levels resulted in decreased Cdyn. Conclusion and clinical relevance: Establishing a reference interval for Cdyn proved challenging. A single reference interval may be misleading or unhelpful in clinical practice. Nevertheless, this study offers valuable insights into the factors affecting Cdyn in healthy anesthetized dogs, which should be considered in clinical assessments. [ABSTRACT FROM AUTHOR]

Objectives: The aim of this study was to report the incidence of anaesthetic complications associated with feline bronchoscopy. Methods: This was a retrospective analysis of anaesthetic records and electronic case logs of feline bronchoscopies at two university hospitals (centres B and L) between January 2013 and December 2015. A two-tailed Fisher's exact test was used for comparison of variables and outcomes between centres. Results: Seventy-nine cases were included. Desaturation (SpO2 <90%) was the most frequently encountered complication, reported in 24 cats (30.3%); centre B reported significantly less desaturation than centre L (22.4% vs 52.4%; P = 0.014). The use of an endotracheal tube or laryngeal mask airway resulted in a lower incidence of desaturation (22.9% vs 22.2%) than the use of a tracheal catheter through which oxygen was insufflated (48.0%). The latter method was associated with an increased incidence of desaturation (P = 0.034). Patients to which terbutaline was administered had a lower incidence of desaturation (27.5%) than those that did not receive it (35.7%), although this was not statistically significant (P = 0.46). Airway management method and the use of terbutaline differed significantly between centres, as did the profile of complications. Pneumothorax was encountered in two cats (2.5%) and cardiac arrest resulting in the death in one of these cats (1.3%). Conclusions and relevance: Desaturation is a frequently encountered complication during and after bronchoscopy. Airway management method and the use and timing of terbutaline warrant prospective evaluation for their role in decreasing the incidence of desaturation. Bronchoscopy is a high-risk procedure with frequent requirement for post-procedure oxygen supplementation. Pneumothorax and cardiac arrest are potential complications. [ABSTRACT FROM AUTHOR]

The article presents case studies of the use of continual positive airway pressure (CPAP) ventilation in adult horses for a variety of orthopedic procedures in right lateral recumbency. It notes that such procedure may improve oxygenation while having limited deleterious effects on cardiovascular function of horses. The authors discuss the need for further investigation on the impact of CPAP on the cardiovascular system using different anesthetic protocols.

Introduction: Refugee and migrant communities carry histories wrought with trauma. These traumas and trauma-related responses often contribute to feelings of mistrust towards Western health care systems and can additionally foster negative clinician biases (conscious or unconscious) and attitudes towards these communities. Consequently, clinicians often fail to uncover underlying causes of poor health and distress for their refugee and migrant patients., Methods: To dismantle biases and cultivate empathy towards refugee and migrant patients, we developed a 1-hour simulation-based educational workshop to train clinicians to think critically about incorporating trauma-informed care (TIC) for these patients' unique needs. We introduced this tool to the GME curricula at Texas Tech University ( n = 36) and to the UME curricula at Albany Medical College ( n = 43). Using pre- and postquestionnaires, we analyzed the impact of this workshop on participants' knowledge, attitudes, and practices regarding TIC for migrant and refugee populations ( n = 44)., Results: This tool positively influenced students' and residents' knowledge and attitudes regarding TIC and displaced peoples, and learners expressed greater willingness to incorporate TIC into clinical practice ( p < .001). Additionally, residents self-reported percentage increases in behaviors that promote equitable care for refugee and migrant patients 6 months postintervention., Discussion: This training enhanced clinicians' trauma-informed practices towards refugee and migrant patients, with learners qualitatively expressing a greater sense of empathy towards this community. While measuring the impact on learners' empathy requires further assessment, this educational innovation's preliminary success provides a foundation for the role of simulation-based learning in medical education., (© 2024 Palnati et al.)

This case series is the first report of the use of CPAP (continuous positive airway pressure) ventilation in adult horses. Two horses and 3 ponies anesthetized for orthopedic procedures in lateral recumbency received 10 cm H(2)O CPAP. During anesthesia, arterial oxygen partial pressure tended to increase and arterial carbon dioxide pressure tended to increase despite increased minute ventilation index. The measured cardiovascular parameters were within physiologic limits.
