Abstract

This paper investigates the areal distribution of vowel systems in the Macro-Sudan Belt, an area encompassing most of the western and central parts of northern Sub-Saharan Africa. We report on a survey of 681 language varieties with entries coded for two phonological features: advanced tongue root (ATR) harmony and the presence of interior vowels (i.e. non-peripheral vowels [ɨ ɯ ɜ ə ʌ ... ]). Our results show that the presence of ATR harmony in the Macro-Sudan Belt is limited to three geographically unconnected zones: an Atlantic zone, a West African zone, and an East African zone. Between the West and East African ATR Zones is a genetically heterogeneous region where ATR harmony is systematically absent: we term this the Central African ATR-deficient zone. Our results show that in this same Central African zone, phonemic and allophonic interior vowels are disproportionately prevalent. Based on this distribution, we highlight two issues. First, ATR and interiority have an antagonistic relationship and do not commonly co-occur within vowel systems; this finding is supported through statistical tests. Second, our survey supports the existence of the Macro-Sudan Belt, but the discontinuous distribution of ATR harmony and its systematic absence in Central Africa challenges the proposal that this area represents the 'hotbed' of the Macro-Sudan Belt. 2. Late life suicide in Italy, 1980–2015.

Abstract

Aim: Suicide trends in older adults were examined in Italy from 1980 to 2015. Age groups considered were 65–74 years, 75–84 and 85 + years. Methods: Data on suicide deaths were obtained from the Italian Mortality Database (IMDB) collected by the Italian National Institute of Statistics (ISTAT) and processed by the Statistics Unit of the Italian National Institute of Health (Istituto Superiore di Sanità). Joinpoint regression analyses were carried out for age subgroups by sex. In addition, data related to four main macro-areas of Italy were considered: North-West, North-East, Centre, and South with Islands (Sicily and Sardinia). Results: Starting by 1985, declines were noted in all age groups with more noticeable decreases in older adults aged 75–84 and 85 + years. Female subjects demonstrated more ample falls in rates than male individuals. Declines were detected in all macro-areas of Italy. Conclusion: Improved health assistance and quality of life of older individuals have probably contributed to the decline of suicide rates of senior citizens, who also showed improved poverty indices in the country, differently from all other age groups. Abstract

Introduction: Systemic sclerosis (SSc) is characterized by a complex etiopathogenesis encompassing both host genetic and environmental -infectious/toxic- factors responsible for altered fibrogenesis and diffuse microangiopathy. A wide spectrum of clinical phenotypes may be observed in patients' populations from different geographical areas. We investigated the prevalence of specific clinical and serological phenotypes in patients with definite SSc enrolled at tertiary referral centres in different Italian geographical macro-areas. The observed findings were compared with those reported in the world literature.Materials and methods: The clinical features of 1538 patients (161 M, 10.5%; mean age 59.8 +/- 26.9 yrs.; mean disease duration 8.9 +/- 7.7 yrs) with definite SSc recruited in 38 tertiary referral centres of the SPRING (Systemic sclerosis Progression INvestiGation Group) registry promoted by Italian Society of Rheumatology (SIR) were obtained and clustered according to Italian geographical macroareas.Results: Patients living in Southern Italy were characterized by more severe clinical and/or serological SSc phenotypes compared to those in Northern and Central Italy; namely, they show increased percentages of diffuse cutaneous SSc, digital ulcers, sicca syndrome, muscle involvement, arthritis, cardiopulmonary symptoms, interstitial lung involvement at HRCT, as well increased prevalence of serum anti-Scl70 autoantibodies. In the same SSc population immunusppressive drugs were frequently employed. The review of the literature underlined the geographical heterogeneity of SSc phenotypes, even if the observed findings are scarcely comparable due to the variability of methodological approaches.Conclusion: The phenotypical differences among SSc patients' subgroups from Italian macro-areas might be correlated to genetic/environmental co-factors, and possibly to a not equally distributed national network of information and healthcare facilities.

Abstract

The Sustainable Healthcare project developed an original multidisciplinary evaluation tool, specifically designed to assess and improve a hospital’s global sustainability by considering together the environmental, social and economic issues, so to give a comprehensive evaluation of the hospital, according to an appropriate concept of sustainability. The system, which aimed to be simple, light and easy-to-use, includes the main weighting to enhance sustainability, organized in a hierarchical way: specific indicators are contained in a series of criteria, which represent the most critical factors and the most effective starting points for hospital’s sustainability improvement. They are finally divided into the three macro-areas of sustainability: social, economic and environmental sustainability. In order to take into account the different interrelationships among the various components of the system, a weighting process was carried out according to the Analytic Network Process method by Saaty in 2005. This allowed to take into consideration different users’ points of view and, most importantly, the human factor, thanks to the development of a weighting system based on the opinions of specific focus groups, which included experts and professionals from different healthcare sectors. The evaluation system’s application to a hospital allows the structure’s global sustainability to be assessed.

Abstract

Landslide hazard assessment at coarse scales (1:50,000 to 1:10,000) is often restricted to the analyses of landslide susceptibility by direct or indirect approaches, i.e. the spatial and static component of the hazard which is considered as a ‘relative hazard assessment', while the analyses of the probability of landslide occurrence, i.e. the temporal and dynamic component of the hazard, is restricted to detailed scale (1:10,000 to 1:2,000). Because the scales of work are different for both approaches, the thematic data are dissimilar especially in terms of nomenclature and investigation scale. Therefore, linking both methods to obtain realistic landslide hazard maps needs specific strategies. The objective of this study is the application of a statistical and a deterministic dynamic model on the same dataset of environmental factors to produce hazard maps at 1:10,000 scale.
