2. Placental Oxidative Status throughout Normal Gestation in Women with Uncomplicated Pregnancies

The effects of gestational age on placental oxidative balance throughout gestation were investigated in women with uncomplicated pregnancies. Placental tissues were obtained from normal pregnant women who delivered at term or underwent elective pregnancy termination at 6 to 23 + 6 weeks of pregnancy. Placental tissues were analyzed for total antioxidant capacity (TAC) and lipid peroxide (malondialdehyde, MDA) levels using commercially available kits. Two hundred and one placental tissues were analyzed and the mean ± SD MDA (pmol/mg tissue) and TAC (µmol Trolox equivalent/mg tissue) levels for first, second, and third trimester groups were 277.01 ± 204.66, 202.66 ± 185.05, and 176.97 ± 141.61, P < 0.004 and 498.62 ± 400.74, 454.90 ± 374.44, and 912.19 ± 586.21, P < 0.0001 by ANOVA, respectively. Our data reflects an increased oxidative stress in the placenta in the early phase of normal pregnancy. As pregnancy progressed, placental antioxidant protective mechanisms increased and lipid peroxidation markers decreased resulting in diminution in oxidative stress. Our findings provide a biochemical support to the concept of a hypoxic environment in early pregnancy. A decrease in placental oxidative stress in the second and third trimesters appears to be a physiological phenomenon of normal pregnancy. Deviations from this physiological phenomenon may result in placental-mediated disorders.

This study was undertaken to assess the impact of obesity on fetal well-being in glucose-tolerant and nonhypertensive women. Medical charts of all patients admitted to the labor and delivery department at our institution between January, 2011 and July, 2011 were retrospectively reviewed. Patients with diabetes/impaired glucose tolerance or hypertension were excluded. A total of 100 women, 50 lean and 50 obese, were included. Background: Despite advances in surgery and pharmacotherapy, there remains significant residual ischemic risk after coronary artery bypass grafting surgery. Methods: In REDUCE-IT (Reduction of Cardiovascular Events With Icosapent Ethyl–Intervention Trial), a multicenter, placebo-controlled, double-blind trial, statin-treated patients with controlled low-density lipoprotein cholesterol and mild to moderate hypertriglyceridemia were randomized to 4 g daily of icosapent ethyl or placebo. They experienced a 25% reduction in risk of a primary efficacy end point (composite of cardiovascular death, myocardial infarction, stroke, coronary revascularization, or hospitalization for unstable angina) and a 26% reduction in risk of a key secondary efficacy end point (composite of cardiovascular death, myocardial infarction, or stroke) when compared with placebo. The current analysis reports on the subgroup of patients from the trial with a history of coronary artery bypass grafting. Results: Of the 8179 patients randomized in REDUCE-IT, a total of 1837 (22.5%) had a history of coronary artery bypass grafting, with 897 patients randomized to icosapent ethyl and 940 to placebo. Baseline characteristics were similar between treatment groups. Randomization to icosapent ethyl was associated with a significant reduction in the primary end point (hazard ratio [HR], 0.76 [95% CI, 0.63–0.92]; P =0.004), in the key secondary end point (HR, 0.69 [95% CI, 0.56–0.87]; P =0.001), and in total (first plus subsequent or recurrent) ischemic events (rate ratio, 0.64 [95% CI, 0.50–0.81]; P =0.0002) compared with placebo. This yielded an absolute risk reduction of 6.2% (95% CI, 2.3%–10.2%) in first events, with a number needed to treat of 16 (95% CI, 10–44) during a median follow-up time of 4.8 years. Safety findings were similar to the overall study: beyond an increased rate of atrial fibrillation/flutter requiring hospitalization for at least 24 hours (5.0% vs 3.1%; P =0.03) and a nonsignificant increase in bleeding, occurrences of adverse events were comparable between groups. Conclusions: In REDUCE-IT patients with a history of coronary artery bypass grafting, treatment with icosapent ethyl was associated with significant reductions in first and recurrent ischemic events. Contains fulltext : 215333.pdf (Publisher’s version ) (Closed access) BACKGROUND: Patients with stable coronary artery disease and diabetes with previous percutaneous coronary intervention (PCI), particularly those with previous stenting, are at high risk of ischaemic events. These patients are generally treated with aspirin. In this trial, we aimed to investigate if these patients would benefit from treatment with aspirin plus ticagrelor. METHODS: The Effect of Ticagrelor on Health Outcomes in diabEtes Mellitus patients Intervention Study (THEMIS) was a phase 3 randomised, double-blinded, placebo-controlled trial, done in 1315 sites in 42 countries. Patients were eligible if 50 years or older, with type 2 diabetes, receiving anti-hyperglycaemic drugs for at least 6 months, with stable coronary artery disease, and one of three other mutually non-exclusive criteria: a history of previous PCI or of coronary artery bypass grafting, or documentation of angiographic stenosis of 50% or more in at least one coronary artery. Eligible patients were randomly assigned (1:1) to either ticagrelor or placebo, by use of an interactive voice-response or web-response system. The THEMIS-PCI trial comprised a prespecified subgroup of patients with previous PCI. The primary efficacy outcome was a composite of cardiovascular death, myocardial infarction, or stroke (measured in the intention-to-treat population). FINDINGS: Between Feb 17, 2014, and May 24, 2016, 11 154 patients (58% of the overall THEMIS trial) with a history of previous PCI were enrolled in the THEMIS-PCI trial. Median follow-up was 3.3 years (IQR 2.8-3.8). In the previous PCI group, fewer patients receiving ticagrelor had a primary efficacy outcome event than in the placebo group (404 [7.3%] of 5558 vs 480 [8.6%] of 5596; HR 0.85 [95% CI 0.74-0.97], p=0.013). The same effect was not observed in patients without PCI (p=0.76, pinteraction=0.16). The proportion of patients with cardiovascular death was similar in both treatment groups (174 [3.1%] with ticagrelor vs 183 (3.3%) with placebo; HR 0.96 [95% CI 0.78-1.18], p=0.68), as well as all-cause death (282 [5.1%] vs 323 [5.8%]; 0.88 [0.75-1.03], p=0.11). The Texas Department of Transportation is implementing a smoothness specification based on inertial profile measurements. This specification includes a localized roughness provision to locate defects on the final surface based on measured surface profiles. To identify defects, the existing method checks deviations between the average profile and its 25-ft moving average against the specified threshold of 150 mils. Stations at which deviations exceed 150 mils in magnitude are considered defect locations. Although this method provides an objective approach to identification of defects on the basis of profile data, some districts have taken the additional step of using a bump rating panel to select bumps and dips to be corrected based on the panel's opinion of the defects' severity from the point of view of ride quality. Clearly, a standard method needs to be developed to maintain consistency in ride quality assurance testing. This paper presents results of a study in which bump rating panel data were used to investigate the need to correct defects identified from surface profile measurements.

Matrix metalloproteinases (MMPs), specifically MMP-9 plays a role in human placentation. The enzyme confers an invasive ability to cytotrophoblasts and degrades the endometrial matrix as the cells infiltrate the decidua to keep up with placental growth. Since tumor necrosis factor-α (TNF-α) can induce the synthesis of MMP-9, we investigated the patterns of changes in and correlation between placental villous MMP-9 and TNF-α expressions throughout normal human gestation. Placentas were obtained from 179 normal pregnant women who underwent elective abortion or term delivery. Chorionic villi isolated from placental samples were grouped as first, second, and third trimester (70/7-130/7, 131/7-236/7, and 370/7-424/7 weeks, respectively). Chorionic villous TNF-α and MMP-9 proteins were assayed using enzyme immunoassay kits. There were significant differences in MMP-9 and TNF-α protein expressions among the trimester groups ( P = .001). The MMP-9 protein increased progressively with an increase in gestational age (GA), but TNF-α peaked in the second trimester. Within each trimester group, we searched for the effects of variation of GA in days on the 2 variables. A significant positive correlation between MMP-9 and GA was noted in the first trimester ( r = 0.364, P = .005). No other comparisons were significant. When GA was controlled for, partial correlation revealed a significant positive correlation between TNF-α and MMP-9 only in the second trimester ( r = 0.300, P = .018). We hypothesize that the TNF-α peak and the positive correlation between TNF-α and MMP-9 in the second trimester of normal human gestation could contribute toward a successful pregnancy outcome.

The Texas Department of Transportation (DOT) is implementing a smoothness specification based on inertial profile measurements. For quality assurance testing, this smoothness specification includes pay adjustment schedules that are tied to the average international roughness index. The acceptance criteria used in this specification are based on surface profile measurements collected with the traditional single-point lasers, which the department has been using to monitor pavement smoothness on the state highway network. Since the Texas DOT implemented its smoothness specification, developments in sensor technology have included multipoint and wide-footprint lasers for noncontact height measurements. There is a need to verify ride quality measurements from these newer lasers to assess their impact on quality assurance tests conducted under the Texas DOT's existing ride specification. This paper presents a comparison of ride quality measurements from tests conducted with various lasers on hot-mix asphalt concrete pavements. Of particular importance to this comparison is the verification of ride quality measurements determined from the lasers tested with corresponding reference profile measurements.

To determine the rate and factors associated with the successful Induction of Labor (IOL) in nulliparous patients undergoing scheduled IOL at 41 weeks of gestational age (GA) with an unfavorable cervix. This was a retrospective analysis that included nulliparous patients who presented to the Labor and Delivery unit at the Bronx Lebanon Hospital Center between 2011 and 2012 for elective IOL at 41 weeks of GA. The Bishop score was assessed upon admission and IOL agents were used in compliance with ACOG guidelines in different combinations, based on the obstetrical team preference. Labor and Delivery Unit of the Bronx Lebanon Hospital. Nulliparous patients with 41 weeks of pregnancy for elective induction of labor. Seventy-six patients were included in the study. GA was confirmed using a combination of the last menstrual period and a dating sonogram during pregnancy. This was a retrospective chart review that included nulliparous patients who presented to the Labor and Delivery unit at the Bronx Lebanon Hospital Center between October 2011 and October 2012 for elective IOL at 41 weeks of gestational age with an unfavorable cervix defined as a Bishop score of 6 or less. The overall successful rate of IOL in a combination of different maternal factors with different agents for induction in nulliparous patients undergoing scheduled IOL with an unfavorable Bishop score at 41 weeks of GA was 51.32 %. Factors associated with successful IOL were younger age [22.3 years vs. 25.1(p = 0.015)], lower BMI [25 vs. 28.1(p = 0.46)] and lower maternal weight [64.75 kg vs. 74.02 (p = 0.28)]. Maternal height was not a contributing factor; the artificial rupture of membranes, epidural anesthesia and the prostaglandins used did not contribute. Use of cervical balloon and oxytocin was associated with failed IOL. Patients undergoing IOL at 41 weeks with an unfavorable cervix had a successful rate of 51.32 %. Younger maternal age, lower weight, and lower BMI were associated with successful IOL.

Pavement texture is an important indicator of road serviceability. Pavement texture is directly linked to pavement and tire skid resistance and to traffic safety for the public. Therefore, most transportation agencies require texture data for their pavement management information systems. Current methods for measuring pavement texture that use high-speed distance sensors or three-dimensional line scan devices have shown limitations for data stability. Problems caused by vehicle vibration, bad data from optical sensors, and, most important, speed dependency related to the sensor sampling time or exposure time either make the texture results unreliable or reveal noticeable errors. The influence of speed on network-level texture data collection was studied, and a highspeed, high-precision three-dimensional texture system was developed. The experiment confirmed that improper selection of a sensor with large exposure time would result in speed dependency on measured texture data. Because the system developed in the study adopted a very small exposure time for each sampling, it could produce accurate texture data that are largely independent of vehicle speed.

This paper presents results from tests conducted with fixed-slip and locked-wheel friction measurement systems to investigate relationships between the friction numbers obtained from both systems.

Placental tumor necrosis factor-α (TNF-α) is a cell signaling protein. During pregnancy, TNF-α induces synthesis of matrix metalloproteinases (MMPs) which allows cytotrophoblasts to reach the spiral arteries deeper within the uterine decidua. TNF-α also augments apoptosis of vascular smooth muscle cells surrounding these arteries. In this study, chorionic villi TNF-α protein expression throughout normal human gestation were investigated.Placental chorionic villi tissues obtained from elective surgical terminations of pregnancy and from uncomplicated term births were assayed using EIA kits (Cayman Chemicals, Ann Arbor, MI, Item # 589201).The median, 25th percentile and 75th percentile values in the first (N = 99), second (N = 58) and third trimester (N = 42) were: 36.46, 27.25, 45.90 pg/100 mg tissue; 55.43, 40.09, 110.88 pg/100 mg tissue; and 16.63, 9.32, 31.92 pg/100 mg tissue, respectively.Variations in placental TNF-α protein expression noted at different trimesters may suggest gestational age specific roles for the cytokine. The increase in TNF-α protein expression observed in the second trimester may be involved in upregulating synthesis of MMP and in augmenting apoptosis of vascular smooth muscle cells of the spiral arteries. A failure in this second trimester increase in TNF-α protein could contribute to gestational compromise.

This study examines risk factors for persistent cervical intraepithelial neoplasia (CIN) and examines whether human papillomavirus (HPV) testing predicts persistent lesions.: Women with histologically diagnosed CIN 1 or CIN 2 (n = 206) were followed up every 3 months without treatment. Human papillomavirus genotyping, plasma levels of ascorbic acid, and red blood cell folate levels were obtained. Cervical biopsy at 12 months determined the presence of CIN. Relative risk (RR) was estimated by log-linked binomial regression models.: At 12 months, 70% of CIN 1 versus 54% of CIN 2 lesions spontaneously regressed (p.001). Levels of folate or ascorbic acid were not associated with persistent CIN at 12 months. Compared with HPV-negative women, those with multiple HPV types (RRs ranged from 1.68 to 2.17 at each follow-up visit) or high-risk types (RRs range = 1.74-2.09) were at increased risk for persistent CIN; women with HPV-16/18 had the highest risk (RRs range = 1.91-2.21). Persistent infection with a high-risk type was also associated with persistent CIN (RRs range = 1.50-2.35). Typing for high-risk HPVs at 6 months only had a sensitivity of 46% in predicting persistence of any lesions at 12 months.: Spontaneous regression of CIN 1 and 2 occurs frequently within 12 months. Human papillomavirus infection is the major risk factor for persistent CIN. However, HPV testing cannot reliably predict persistence of any lesion.

Fatigue cracking is one of the predominant distresses that occur in hot-mix asphalt (HMA) pavements and often costs highway agencies millions of dollars in road maintenance and rehabilitation activities. Ways to minimize this distress include material screening and selecting appropriate mix designs that produce crack-resistant HMA mixes. However, no standardized laboratory tests for fatigue cracking have been universally adopted for routine mix design or screening purposes in the evaluation of HMA crack resistance. Four cracking test methods—overlay tester (OT), direct tension (DT), indirect tension (IDT), and semicircular bending (SCB)—were compared for their potential application as simple tests for the routine crack evaluation and screening of HMA mixes in the laboratory. The evaluation criteria, based on commonly used Texas mixes, included (a) rationality of the test concept and tie to field performance, (b) repeatability and variability, (c) simplicity and practicality, (d) specimen fabrication process, and (e) simplicity of data analysis. Compared with the repeated-loading OT test, preliminary findings indicated that although the monotonically increasing loading DT, IDT, and SCB tests were more repeatable, those tests could not adequately discriminate crack resistance among the mixes evaluated on the basis of the tensile strength and strain parameters. Thus none of these monotonic tests would be recommended as a simple fatigue cracking test. Additional development of the test protocols is strongly recommended, including exploration of the repeated-loading IDT and SCB tests, use of strain–fracture energy concepts, establishment of pass-or-fail criteria for screening mixes, and validation with field performance data.

Introduction Bicycling is associated with neurological impairment and impotence in men. Similar deficits have not been confirmed in women. Aim To evaluate the effects of bicycling on genital sensation and sexual function in women. Methods Healthy, premenopausal, competitive women bicyclists and runners (controls) were compared. Main Outcome Measures (1) Genital vibratory thresholds (VTs) were determined using the Medoc Vibratory Sensation Analyzer 3000. (2) Sexual function and sexually related distress were assessed by the Dennerstein Personal Experience Questionnaire (SPEQ) and the Female Sexual Distress Scale (FSDS). Results Forty‐eight bicyclists and 22 controls were enrolled. The median age was 33 years. The bicyclists were older, had higher body mass indices (BMIs), were more diverse in their sexual orientation, and were more likely to have a current partner. Bicyclists rode an average of 28.3 ± 19.7 miles/day (range 4–100), 3.8 ± 1.5 days/week, for an average of 2.1 ± 1.8 hours/ride. The mean number of years riding was 7.9 ± 7.1 years (range 0.5–30). Controls ran an average of 4.65 ± 2.1 miles/day (range 1.5–8) and 5.0 ± 1.2 days/week. On bivariate analysis, bicyclists had significantly higher VTs than runners, indicating worse neurological function at all sites ( P Conclusion There is an association between bicycling and decreased genital sensation in competitive women bicyclists. Negative effects on sexual function and quality of life were not apparent in our young, healthy premenopausal cohort. Anal incontinence (AI) is a significant problem that causes social and hygienic inconvenience. The true prevalence of AI is difficult to estimate due to inconsistencies in research methods, but larger studies suggest a rate of 2–6% for incontinence to stool. There is a significant association between sonographically detected anal sphincter defects and symptoms of AI. The intrapartum factors most consistently associated with a higher risk of AI include: forceps delivery, third or fourth degree tears, and length of the second stage of labor. Fetal weight of > 4,000 g is also associated with AI. Repair of the sphincter can be performed in either an overlapping or an end-to-end fashion, with similar results for both methods. The role of cesarean delivery for the prevention of AI remains unclear, and further study should be devoted to this question.

Using quantitative sensory testing and a validated questionnaire, we investigated the role of pudendal nerve integrity in sexual function among women.Participants completed the Pelvic Organ Prolapse/Urinary Incontinence Sexual Questionnaire (PISQ). Vibratory and pressure thresholds were measured at the S2 dermatome reflecting pudendal nerve distribution.A total of 56 women enrolled; 29 (51.8%) were asymptomatic and 27 (48.2%) had 1 or more forms of female sexual dysfunction (total sexual dysfunction) including: desire disorder 16.1%, arousal disorder 26.8%, orgasmic disorder 25%, and pain disorder 12.5%. Age, parity, menopausal status, and body mass index were similar between groups. PISQ scores were lower in symptomatic subjects compared with controls (P.001). Decreased tactile sensation was found at the clitoris for women with total sexual dysfunction, desire disorder, and arousal disorder. Women with arousal disorder also had decreased tactile sensation at the perineum.Pudendal nerve integrity may play a role in female sexual dysfunction.

Objective: The purpose of this study was to assess the urogenital effects of raloxifene, tamoxifen, conjugated equine estrogen, and placebo in healthy postmenopausal women. Study Design: This randomized, double-blind, placebo-controlled study compared the urogenital effects of 0.625 mg of conjugated equine estrogen (n = 15 women), 20 mg of tamoxifen (n = 14 women), 60 mg of raloxifene, (n = 15 women), and placebo (n = 13 women). Evaluations at baseline and evaluations after 20 weeks receiving the drug included a pelvic examination with cytologic evaluation of vagina and urethra, pelvic organ prolapse quantitation, and urethral axis deflection by cotton swab test (only in patients with incontinence [33%]). Results: Conjugated equine estrogen increased the maturation value of both urethral and vaginal cytologic condition ( P =.002, P =.032, respectively). There was a decrease in vaginal maturation value in the raloxifene group (not significant). Two of 8 women in the conjugated equine estrogen group showed evidence of worsening prolapse by pelvic organ prolapse quantitation; the condition of 2 of 8 women improved. In the raloxifene, tamoxifen, and placebo groups 8 of 12 women, 4 of 13 women, and 2 of 11 women had worsening in prolapse scores, respectively, whereas none of the women had improvement. Increased cotton swab deflection was found in 3 of 5 women in the raloxifene group, in 5 of 8 women in the tamoxifen group, in 0 of 4 women in the placebo group, and in 0 of 2 women in the conjugated equine estrogen group. Seventy-five percent of the patients who received raloxifene and 60% of the patients who received tamoxifen had increases in prolapse by any measure (ie, pelvic organ prolapse quantitation or cotton swab or clinical assessment) compared with 18% of the patients in the placebo group and 22% of the patients in the conjugated equine estrogen group ( P =.015), although symptoms did not differ among groups. Conclusion: Neither raloxifene nor tamoxifen improve cytohormonal effects in the vagina or urethra, whereas conjugated equine estrogen does. Raloxifene and tamoxifen appear to show worsening prolapse compared with conjugated equine estrogen and placebo. The clinical relevance of these effects is unknown and requires investigation. The effects of gestational age on placental oxidative balance throughout gestation were investigated in women with uncomplicated pregnancies. Placental tissues were obtained from normal pregnant women who delivered at term or underwent elective pregnancy termination at 6 to 23 + 6 weeks of pregnancy. Placental tissues were analyzed for total antioxidant capacity (TAC) and lipid peroxide (malondialdehyde, MDA) levels using commercially available kits. Two hundred and one placental tissues were analyzed and the mean ± SD MDA (pmol/mg tissue) and TAC (µmol Trolox equivalent/mg tissue) levels for first, second, and third trimester groups were 277.01 ± 204.66, 202.66 ± 185.05, and 176.97 ± 141.61,P< 0.004 and 498.62 ± 400.74, 454.90 ± 374.44, and 912.19 ± 586.21,P< 0.0001 by ANOVA, respectively. Our data reflects an increased oxidative stress in the placenta in the early phase of normal pregnancy. As pregnancy progressed, placental antioxidant protective mechanisms increased and lipid peroxidation markers decreased resulting in diminution in oxidative stress. Our findings provide a biochemical support to the concept of a hypoxic environment in early pregnancy. A decrease in placental oxidative stress in the second and third trimesters appears to be a physiological phenomenon of normal pregnancy. Deviations from this physiological phenomenon may result in placental-mediated disorders.

Abdominal pregnancy is a very rare form of ectopic pregnancy, associated with high morbidity and mortality for both fetus and mother. It is, and often, seen in poor resource nations, where early diagnosis is often a major challenge due to poor prenatal care and lack of medical resources. An advanced abdominal pregnancy with a good fetal and maternal outcome is therefore a more extraordinary occurrence in the modern developed world. We present a case of an abdominal pregnancy at 33.4 weeks in an individual with no documented prenatal care, who arrived in a hospital in the Bronx, in June 25th 2014, with symptoms of generalized, severe lower abdominal pain. Upon examination it was found that due to category III fetal tracing an emergent cesarean section was performed. At the time of laparotomy the fetus was located in the pelvis covered by the uterine serosa, with distortion of the entire right adnexa and invasion to the right parametrium. The placenta invaded the pouch of Douglas and the lower part of the sigmoid colon. A massive hemorrhage followed, followed by a supracervical hysterectomy. A viable infant was delivered and mother discharged on postoperative day 4.

Resilient modulus is a critical material property required for the characterization of pavement layers used in modern mechanistic-based pavement design. Because of its fundamental nature, resilient modulus provides a meaningful basis for simulating the effect that an individual layer has on pavement response and performance. Resilient modulus can be determined by laboratory testing or by an analytical procedure known as backcalculation involving the interpretation of nondestructive testing. Unfortunately, resilient modulus results determined from laboratory testing and those determined from backcalculation generally do not agree. The issue of characterizing in situ moduli is addressed by presenting the results of an analysis and comparison of resilient modulus values determined from laboratory testing and those established through backcalculation of falling weight deflectometer (FWD) data from the WesTrack experiment in northern Nevada. These two basic methodologies for resilient modulus determination are reviewed, and many of their known weaknesses and limitations are identified. The backcalculated modulus values were determined using Washington State Department of Transportation’s EVERCALC computer program and FWD data obtained from WesTrack pavement sections at the onset of truck loading. The laboratory-based resilient modulus values were determined using a combination of test results on remolded and recompacted WesTrack materials and the KENLAYER computer program to estimate the modulus under the state of stress created by the FWD during testing. The results of this investigation continue to support the suspicion that significant differences do exist between resilient moduli determined from backcalculation and those determined through laboratory testing. Accordingly, some general observations are made concerning possible improvements in each methodology and concerning which of the two methodologies is better suited for a thorough pavement evaluation and design process. Overall, a better understanding of the problems associated with accurately characterizing pavement support under in-situ conditions is presented.

Genital human papillomavirus (HPV) infection is the major causal factor of cervical intraepithelial neoplasia (CIN). The potential role of nutrition as an additional, independent risk factor for CIN has not been appropriately addressed in the context of HPV. This case-control study evaluated the etiologic role of HPV in terms of viral type and load and examined the association between CIN and plasma levels of micronutrients adjusting for HPV. Cases (n = 378) with histo-pathologically confirmed CIN and controls (n = 366) with no history of abnormal Pap smears were recruited from colposcopy and gynecology clinics, respectively. Risk of CIN was significantly increased among women who were infected with multiple HPV types (odds ratio [OR] = 21.06), a high viral load (OR = 13.08) and HPV 16 (OR = 62.49). After adjusting for HPV positivity and demographic factors, there was an inverse correlation between plasma alpha-tocopherol and risk of CIN (OR = 0.15). Plasma ascorbic acid was protective at a high level of > or = 0.803 mg/dl (OR = 0.46). CIN was not associated with plasma retinol and beta-carotene levels. The effect of genital HPV infection on CIN development is highly influenced by oncogenic viral type and high viral load. Vitamins C and E may play an independent protective role in development of CIN that needs to be confirmed in prospective studies.

Although genital human papillomavirus (HPV) infection is well established as the etiologic agent for cervical intraepithelial neoplasia (CIN), little is known about the cofactors involved in the development of high-grade lesions or the progression of low-grade to high-grade lesions. In our study of HPV-infected women with CIN (163 CIN I, 51 CIN II and 44 CIN III), women with CIN II or III were compared with those with CIN I for risk factors associated with high-grade lesions. After controlling for age, education, ethnicity and frequency of Pap smear screening, infection with HPV 16, but not high viral load or infection with multiple types, was associated with high-grade lesions (OR for CIN II 5 11.96, OR for CIN III 5 23.74). Risk of CIN III, but not CIN II, increased with number of cigarettes smoked per day (ORs 5 1.49 and 3.35 for I10 and G10 cigarettes per day, respectively) and decreased with frequency of condom use during sex (ORs 5 0.60 and 0.32 for women who used condoms occasionally/sometimes and most/ all of the time, respectively). There were no associations between high-grade lesions and plasma levels of micronutrients (retinol, b-carotene, a-tocopherol and reduced ascorbic acid). Our results indicate that infection with HPV 16 is associated with high-grade lesions. Objective: Compare the Estimated Blood Loss (EBL) between residents performing primary cesarean sections based on their level of training and their associated variables, including the combination of surgeon and assistant experience, demographic variables and different techniques in the procedure. Methods: Retrospective analysis of patients who had a primary cesarean section performed by residents from the Obstetrics and Gynecology Residency Program that attended the Labor and Delivery between August 2011 through December of 2012. 278 patients were included. Demographic and pertinent data was extracted from patient’s electronic medical record. The surgeons and assistants were categorized depending on their level of training in the residency program (Post Graduate Year [PGY]1,2,3,4). Results: 278 patients were included in the study, of them 133 were considered emergent cesarean sections mean cEBL of 832 ± 490, 74 urgent with 829 ± 513 cc, 67 scheduled with 884±167cc and 4 elective with 825 ± 52cc with with an observed Estimated Blood Loss (oEBL) of 873cc ± 182, change in hematocrit of 5.14& ± 3.13 and a calculated (cEBL) of 832.5 ± 490 cc. time in minutes was different between 1 and 2 layers closure (57.4 vs. 67.7 minutes) and their cEBL 734 vs. 662cc. Conclusions: There is a difference in cEBL and change in hematocrit in primary cesarean sections done by residents based on their surgical experience. Time, cEBL, change in Hto is influenced by the years of training of the surgical team. Higher BMI was associated with more surgical time and change in Hto. 2 layer closures were found to consume more surgical time.

Women with histopathologically confirmed cervical intraepithelial neoplasia (CIN) were followed at 3-month intervals in a randomized double-blinded trial to evaluate the efficacy of beta-carotene to cause regression of CIN. Questionnaire data, plasma levels of micronutrients, and a cervicovaginal lavage for human papillomavirus (HPV) detection were obtained at each visit, and an endpoint biopsy was performed at 9 months. Sixty-nine subjects had a biopsy endpoint evaluation; 9 of 39 (23%) subjects in the beta-carotene group versus 14 of 30 (47%) in the placebo group had regression of CIN (P = 0.039). Independent risk factors for persistent CIN at 9 months included type-specific persistent HPV infection (OR = 11.38, P = 0.006) and continual HPV infection with a high viral load (OR = 14.25, P = 0.007) at baseline and 9 months, an initial diagnosis ofor =CIN II (OR = 6.74, P = 0.016), and older age (OR foror =25 years = 4.10, P = 0.072). After controlling for these factors, the beta-carotene and placebo groups did not differ in risk for having CIN at 9 months (OR = 1.53, P = 0.550). Resolution of baseline HPV infection was significantly correlated with non-high-risk HPV types (RR = 2.94, P = 0.015), age25 years (RR = 2.62, P = 0.014), and douching after sexual intercourse (RR = 3.02, P = 0.012), but not with randomization group. Our data indicate that a large proportion of mild CIN lesions regress; age and HPV infection play an important role in the natural course of CIN; and repeated HPV testing may have a value in distinguishing women who need aggressive treatment for CIN versus those who do not. Supplementation of beta-carotene does not appear to have a detectable benefit in treatment of CIN.

EXSPAV is an expert system program established for use in flexible pavement and overlay design. It helps transfer and facilitate the knowledge of experts and results of research and practice to the hands of the less experienced design engineer. It incorporates a versatile design approach, which also facilitates the prediction of the structural performance of pavements. EXSPAV was developed in the EXSYS environment and assimilates Ohio State University's predictive program of flexible pavement structural performance. It also has an overlay design module. EGYPAV is a knowledge-based pavement design program developed in the structure of EXSPAV. Its knowledge base was compiled through consultation with experts from the Roads and Bridges Authority, local universities, and construction companies. The base reflects experience, data, and research results for traffic, environmental conditions, pavements, and material characterization in Egypt. It also includes suggested designs of pavement structure for various combinations of design parameters. The program user may check the suggested design and find its structural performance through the inference engine–deterministic model interaction of the EGYPAV itself.

Objective To evaluate compliance with follow-up among inner-city patients with ectopic pregnancy treated with methotrexate, in addition to assessing outcome. Methods Retrospective analysis of all patients evaluated for first-trimester bleeding and diagnosed with ectopic pregnancy at the Bronx Lebanon Hospital Center, New York, USA, from October 2007 to March 2011. Compliance and complications were evaluated for all patients who received medical treatment with methotrexate. Results Ninety-nine patients with hemodynamically stable ectopic pregnancies were identified. Therapy with methotrexate was effective in 87 (87.9%) cases. Only 10 (10.1%) patients were fully compliant during follow-up after methotrexate administration. There were no differences according to ethnicity. Number of follow-up visits after treatment significantly affected compliance. Conclusion Improvements in selection criteria for therapy and simplification of follow-up for patients with medically managed ectopic pregnancy are required.

Objective We investigated the existence and regional distribution of sphingosine-1-phosphate regulatory enzymes and receptors in the lower urinary tract and determined the functional role of sphingosine-1-phosphate receptors in the bladder. Study Design Lower urinary tract tissue from 10 female rats was harvested for real-time reverse transcriptase-polymerase chain reaction or organ bath physiology, whereas blood serum was obtained for high-performance liquid chromatography determination of sphingosine-1-phosphate levels. Statistical analysis included the Student t test and analysis of variance. Results All 3 sphingosine-1-phosphate receptors and major enzymes were expressed throughout the lower urinary tract, but expression and physiologic force generation varied among regions. Sphingosine-1-phosphate was detected in serum. Conclusion We provide novel data that the sphingosine-1-phosphate signaling pathway regulatory proteins exist throughout the female rat lower urinary tract, but that relative expression exhibits regional heterogeneity corresponding with lower urinary tract contractile response to sphingosine-1-phosphate. Our study suggests that sphingosine-1-phosphate signaling is important in the lower urinary tract and identifies this pathway as a possible target for altering bladder smooth muscle tone.

Rupture of a tuboovarian abscess is a life-threatening emergency. The current standard of care is surgery to manage the onset of peritonitis and sepsis.A 34-year-old woman presented with gradually worsening abdominal pain over several weeks. She had fever, pelvic tenderness, an elevated white blood cell count and ultrasonographic evidence of a tuboovarian abscess. After 48 hours of triple antibiotic therapy without a clinical improvement, she underwent surgery, during which she was found to have an abscess that had ruptured into the abdominal wall. A supracervical hysterectomy/bilateral salpingo-oophorectomy and surgical debridement of the subcutaneous tissue was performed, with wet to dry dressing changes of the surgical wound. Over the following 18 months, the patient required extensive wound care for recurrent bouts of wound drainage.Rupture of a tuboovarian abscess usually presents with sudden worsening of the patient's condition. Delays in surgical debridement and drainage increase the rate of associated mortality. Atypical presentations may present as cases resistant to conventional medical therapy, with fewer of the associated risks of life-threatening peritonitis and subsequent sepsis. Our patient represents the first reported case of extraperitoneal spillage of abscess contents from rupture of a tuboovarian abscess into the anterior abdominal wall.

47. Genital sensation and sexual function in women bicyclists and runners: are your feet safer than your seat?

Bicycling is associated with neurological impairment and impotence in men. Similar deficits have not been confirmed in women.To evaluate the effects of bicycling on genital sensation and sexual function in women.Healthy, premenopausal, competitive women bicyclists and runners (controls) were compared.(1) Genital vibratory thresholds (VTs) were determined using the Medoc Vibratory Sensation Analyzer 3000. (2) Sexual function and sexually related distress were assessed by the Dennerstein Personal Experience Questionnaire (SPEQ) and the Female Sexual Distress Scale (FSDS).Forty-eight bicyclists and 22 controls were enrolled. The median age was 33 years. The bicyclists were older, had higher body mass indices (BMIs), were more diverse in their sexual orientation, and were more likely to have a current partner. Bicyclists rode an average of 28.3 +/- 19.7 miles/day (range 4-100), 3.8 +/- 1.5 days/week, for an average of 2.1 +/- 1.8 hours/ride. The mean number of years riding was 7.9 +/- 7.1 years (range 0.5-30). Controls ran an average of 4.65 +/- 2.1 miles/day (range 1.5-8) and 5.0 +/- 1.2 days/week. On bivariate analysis, bicyclists had significantly higher VTs than runners, indicating worse neurological function at all sites (P0.05). Multivariate analysis found significant correlations between higher VTs and bicycling at the left and right perineum, posterior vagina, left and right labia. Increasing VTs at the clitoris, anterior vagina, and urethra were associated with age. In bicyclists, there were no correlations between VTs and miles biked per week, duration of riding, or BMI. Composite SPEQ scores indicated normal sexual function in all sexually active subjects. Neither group suffered from sexually related distress.There is an association between bicycling and decreased genital sensation in competitive women bicyclists. Negative effects on sexual function and quality of life were not apparent in our young, healthy premenopausal cohort.

This paper reports of a statistical analysis done to evaluate the International Roughness Index (IRI) on Texas roads from 2000 to 2005. The objective of the analysis was to identify research focus areas to assist the Texas Department of Transportation (TxDOT) in improving the overall riding quality of the Texas road network. Analyses were done on Asphalt Concrete Pavement (ACP), Continuously Reinforced Concrete Pavement (CRCP) and Jointed Concrete Pavement (JCP) roads. The IRI results from Interstate Highways (IH), United States (US) highways, State Highways and Farm to Market (FM) roads with and without surface treatments are reported in this paper. The analyses indicate that the mean IRI data as measured in the left and right wheel paths of roads using automated inertial profilers are log-normally distributed. IRI as measured in the right wheel path is consistently higher than that measured in the left, in particular for FM roads with surface treatments. ACP generally have the lowest IRI values (highest ride quality) followed by CRCP and the JCP. Findings suggest that significant improvements in the overall ride quality of the Texas roadway network can be achieved by focusing research efforts on FM roads with surface treatments. The use of fine thin hot mix asphalt (HMA) to overlay old surface treatments as well as paving of shoulders on FM roads is recommended.
